OBJECTIVE: To investigate the excess mortality and life-years lost associated with diabetes and prediabetes in China. RESEARCH DESIGN AND METHODS: This national cohort study enrolled 135,405 participants aged 18 years or older from the general population in China. Cox proportional hazards regression models were used to estimate adjusted mortality rate ratio (RR). The life table method was used to estimate life expectancy. RESULTS: Among the 135,405 participants, 10.5% had diabetes and 36.2% had prediabetes in 2013. During a median follow-up of 6 years, 5517 deaths were recorded, including 1428 and 2300 deaths among people with diabetes and prediabetes, respectively. Diabetes and prediabetes were significantly associated with increased risk of all-cause (diabetes: RR, 1.61 [95% CI 1.49, 1.73]; prediabetes: RR, 1.08 [95% CI 1.01, 1.15]), and cardiovascular disease (diabetes: RR, 1.59 [95% CI 1.41, 1.78]; prediabetes: RR, 1.10 [95% CI 1.00, 1.21]) mortality. Additionally, diabetes was significantly associated with increased risks of death resulting from cancer, respiratory disease, liver disease, and diabetic ketoacidosis or coma. Compared with participants with normoglycemia, life expectancy of those with diabetes and prediabetes was shorter, on average, by 4.2 and 0.7 years at age 40 years, respectively. The magnitude of the associations of diabetes and prediabetes with all-cause and cardiovascular disease mortality varied by age and residence. CONCLUSIONS: In this national study, diabetes and prediabetes were significantly associated with reduced life expectancy and increased all-cause and cause-specific mortality risks. The disparities in excess mortality associated with diabetes and prediabetes between different ages and residences have implications for diabetes and prediabetes prevention and treatment programs. Enhanced motility in malignant cell is one hallmark of tumor metastasis. The clinical reality is that each of us responds differently to treatment, driving a significant interest in the development of therapies that are 'right' to the individual. A biomedical analytical tool with low-cost, sensitive and short assaytime is critical for personalized medicine. Herein, a cost-effective and user-friendly microfluidic device was developed for studying of cell migration. A two-step photolithography procedure was conducted to reveal microchannels and microchambers with different depth. The PDMS/glass slide hydride device was assembled between a polymethyl methacrylate (PMMA) clamp which can adjust the pressure imposed on the device to control the fluid communication between mainchambers, thus identical wound (cell-free space) with clear edge can be easily formed within channel without extra chemical, mechanical force, fluidic manipulation and sophisticated microstructure. Using this device, we evaluated the combinatory of BRAFV600E inhibitor vemurafenib and epidermal growth factor receptor (EGFR) inhibitor gefitinib in inhibiting of melanoma cell migration with only 20 μL cell consumption, highlighting its potential in assaying rare clinical biopsy for personalized medicine. In addition, the on-chip migration model followed strictly follow the principle of conventional in vitro scratch/wound healing assay, facilitating it is translation to biologist. Cell migration is one of the key cell functions in physiological and pathological processes, especially in tumor metastasis. However, it is not feasible to monitor the important biochemical molecules produced during cell migrations in situ by conventional cell migration assays. Herein, for the first time a device containing both electrochemical sensing and trans-well cell migration modules was fabricated to sensitively quantify biochemical molecules released from the cell migration process in situ. The fully assembled device with a multi-wall carbon nanotube/graphene/MnO2 nanocomposite functionalized electrode was able to successfully characterize hydrogen peroxide (H2O2) production from melanoma A375 cells, larynx carcinoma HEp-2 cells and liver cancer Hep G2 under serum established chemotaxis. The maximum concentration of H2O2 produced from A375, HEp-2 and Hep G2 in chemotaxis was 130±1.3 nM, 70±0.7 nM and 63±0.7 nM, respectively. While the time required reaching the summit of H2O2 production was 3.0, 4.0 and 1.5 h for A375, HEp-2 and Hep G2, respectively. By staining the polycarbonate micropore membrane disassembled from the device, we found that the average migration rate of the A375, HEp-2 and Hep G2 cells were 98±6%, 38±4% and 32 ±3%, respectively. The novel bi-module cell migration platform enables in situ investigation of cell secretion and cell function simultaneously, highlighting its potential for characterizing cell motility through monitoring H2O2 production on rare samples and for identifying underlying mechanisms of cell migration.
