In August 2020, then 17-year-old Kyle Rittenhouse shot three White men in Kenosha, Wisconsin, during protests that followed the police murder of Jacob Blake. After the shooting, myriad forms of care unfolded in support of Kyle Rittenhouse. In recent years, scholarship on care and care ethics has sought to "trouble" care, pushing beyond the traditional boundaries of care, considering forms of care that lie outside of traditional caring relationships, and bringing into view care that does not result in a universal good. In this article I add to this recent engagement by calling for greater attention to the ways in which care (re)produces systems of violence. Namely, using the events surrounding the trial of Kyle Rittenhouse, I document the networks of support and resource flows that cared for Kyle Rittenhouse before, during, and following his trial. Objectives. To evaluate the association between distance from closest abortion facility and number of fulfilled requests through no-test telehealth medication abortion (NTMA) asynchronous service. Methods. Using deidentified 2020–2022 electronic medical record data from Aid Access users in US states where NTMA is prescribed by US-based clinicians, we describe individual user demographics and their resident county characteristics. We conducted a county-level geospatial analysis of distance to abortion facility (Myers Abortion Facility Database) on fulfilled requests using Poisson regression. Results. US-based clinicians fulfilled NTMA requests to 8411 individuals in 21 states and the District of Columbia. Each 100-mile increase in distance to an abortion facility increased per-capita NTMA by 61% (95% confidence interval [CI] = 26%, 86%). Most individuals were aged 20 to 29 years (54%), had no living children (57%), were less than 6 weeks' gestation (62%), and lived in urban areas (65%). Almost half (49%) lived in higher socially vulnerable counties compared with 17% in less socially vulnerable counties. Conclusions. In the United States, telehealth medication abortion is a critically important service for individuals who are young, socially vulnerable, and living in counties far from abortion care facilities. This study explores how faculty across disciplines access and share scholarly serial content and what expectations they have for immediacy. The authors conducted twenty-five in-depth, semi-structured interviews with faculty of various ranks representing all Illinois State University (ISU) colleges. The findings, presented in the words of participants and triangulated with data from local sources, suggest that faculty use a variety of context-specific mechanisms to access and share serial literature. Participants discuss how they use library services such as databases, subscriptions, interlibrary loan, and document delivery, coupled with academic social networks, disciplinary repositories, author websites, and other publicly available sources to obtain the full text of articles, in addition to their manifold considerations for sharing and requesting content. The urgency with which faculty need to gain access to scholarly literature is dependent on intersecting elements of discipline, current projects, how the resource will be used, the perceived competitiveness of the field, career stage, and personal practices. The findings reiterate that scholarly literature remains integral to the research and teaching of faculty even as needs and practices for accessing and sharing it grow more individualized and distributed. Purpose of Review: Extremes of blood pressure (BP) are common among patients that visit emergency departments. In this review article, we discuss the specific indications for invasive blood pressure monitoring in the ED, particularly in the context of undifferentiated shock and hypertensive emergencies. Recent Findings: In most cases, non-invasive techniques suffice for blood pressure monitoring, however, in certain patient presentations intermittent automated oscillometry bears significant drawbacks. The most evident drawback is the extended intervals between measurements. Invasive BP (IBP) monitoring offers a pivotal tool for patients with critical illness who require accurate, timely, blood pressure monitoring and indirectly monitors for complications involving vital organ systems. Summary: In the management of patients with critical illness or at risk for end organ injury, invasive methods that directly measure BP via arterial cannulation continues to be an established standard. Overall, evaluating patients on an individual basis, with the understanding that patients who present with extreme blood pressure values need closer monitoring, should prompt consideration of invasive methods of blood pressure monitoring. With the growing interest in designing circadian-effective lighting, it is becoming increasingly important to determine how different lighting designs aimed at supporting circadian entrainment might also affect visual performance, preference, discomfort glare and lighting power density (LPD). These outcome measures were simultaneously examined in a controlled setting at night. Four ceiling lighting configurations, using combinations of direct and indirect lighting, were implemented along with one design that utilised local lighting. Every design delivered the same high level of circadian-effective lighting to participants. Saliva samples were obtained to measure nocturnal melatonin suppression. Two visual performance computer tasks together with subjective assessments of sleepiness, discomfort glare and preference were administered to participants. LPDs were determined. None of the lighting configurations created unacceptable levels of discomfort glare, and only one was above the required maximum allowable LPD. Lighting configuration had no differential effect on nocturnal melatonin suppression, visual performance and sleepiness. While the results show that a wide range of lighting approaches can meet visual, non-visual and energy objectives, the majority of participants preferred the local lighting over the ceiling-mounted lighting. Background In 2023, Tennessee replaced $6.2 M in US Centers for Disease Control and Prevention (CDC) human immunodeficiency virus (HIV) prevention funding with state funds to redirect support away from men who have sex with men (MSM), transgender women (TGW), and heterosexual Black women (HSBW) and to prioritize instead first responders (FR), pregnant people (PP), and survivors of sex trafficking (SST). Methods We used a simulation model of HIV disease to compare the clinical impact of Current, the present allocation of condoms, preexposure prophylaxis (PrEP), and HIV testing to CDC priority risk groups (MSM/TGW/HSBW); with Reallocation , funding instead increased HIV testing and linkage of Tennessee-determined priority populations (FR/PP/SST). Key model inputs included baseline condom use (45%–49%), PrEP provision (0.1%–8%), HIV testing frequency (every 2.5–4.8 years), and 30-day HIV care linkage (57%–65%). We assumed Reallocation would reduce condom use (−4%), PrEP provision (−26%), and HIV testing (−47%) in MSM/TGW/HSBW, whereas it would increase HIV testing among FR (+47%) and HIV care linkage (to 100%/90%) among PP/SST. Results Reallocation would lead to 166 additional HIV transmissions, 190 additional deaths, and 843 life-years lost over 10 years. HIV testing reductions were most influential in sensitivity analysis; even a 24% reduction would result in 287 more deaths compared to Current. With pessimistic assumptions, we projected 1359 additional HIV transmissions, 712 additional deaths, and 2778 life-years lost over 10 years. Conclusions Redirecting HIV prevention funding in Tennessee would greatly harm CDC priority populations while conferring minimal benefits to new priority populations. [ABSTRACT FROM AUTHOR]

The circulation of seasonal influenza A viruses (IAVs) in humans relies on effective evasion and subversion of the host immune response. While the evolution of seasonal H1N1 and H3N2 viruses to avoid humoral immunity is well characterized, relatively little is known about the evolution of innate immune antagonism phenotypes in these viruses. Numerous studies have established that only a small subset of infected cells is responsible for initiating the type I and type III interferon (IFN) response during IAV infection, emphasizing the importance of single cell studies to accurately characterize the IFN response during infection. We developed a flow cytometry-based method to examine transcriptional changes in IFN and interferon stimulated gene (ISG) expression at the single cell level. We observed that NS segments derived from seasonal H3N2 viruses are more efficient at antagonizing IFN signaling but less effective at suppressing IFN induction, compared to the pdm2009 H1N1 lineage. We compared a collection of NS segments spanning the natural history of the current seasonal IAV lineages and demonstrate long periods of stability in IFN antagonism potential, punctuated by occasional phenotypic shifts. Altogether, our data reveal significant differences in how seasonal and pandemic H1N1 and H3N2 viruses antagonize the human IFN response at the single cell level. Author summary: Successful replication and transmission of influenza A viruses (IAVs) requires effective subversion of the innate immune response. We developed a new single cell method to examine the interferon (IFN) response to IAV infection at the single cell level. We found that the seasonal H3N2 and H1N1 lineages differ significantly in their ability to antagonize IFN signaling and suppress IFN induction, revealing unexpected nuances in how these lineages interact with the innate immune system. We also directly examined how the IFN antagonism potential of the viral NS segment has evolved over compared decades of circulation in humans. Transient receptor potential melastatin‐4 (TRPM4) forms a complex with N‐methyl‐D‐aspartate receptors (NMDARs) that facilitates NMDAR‐mediated neurotoxicity. Here we used pharmacological tools to determine how TRPM4 regulates NMDAR signaling. Brophenexin, a compound that binds to TRPM4 at the NMDAR binding interface, protected hippocampal neurons in culture from NMDA‐induced death, consistent with published work. Brophenexin (10 μM) reduced NMDA‐evoked whole‐cell currents recorded at 22°C by 87% ± 14% with intracellular Ca2+ chelated to prevent TRPM4 activation. Brophenexin inhibited NMDA‐evoked currents recorded in Na+‐free solution by 87% ± 13%, suggesting that brophenexin and TRPM4 modulate NMDAR function. Incubating cultures in Mg2+‐free buffer containing tetrodotoxin, 6‐cyano‐7‐nitroquinoxaline‐2,3‐dione, and bicuculline for 30 min inhibited NMDA‐evoked increases in intracellular Ca2+ concentration ([Ca2+]i) recorded at 22°C by 50% ± 18% and prevented inhibition by brophenexin. In the absence of these inhibitors, brophenexin inhibited the NMDA‐evoked response by 51% ± 16%. Treatment with the TRPM4 inhibitor 4‐chloro‐2‐(1‐naphthyloxyacetamido)benzoic acid (NBA; 10 μM) increased NMDA‐evoked Ca2+ influx by 90% ± 15%. Increasing extracellular NaCl to 237 mM, a treatment that activates TRPM4, inhibited the NMDA‐evoked increase in [Ca2+]i by a process that occluded the inhibition produced by brophenexin and was prevented by NBA. In recordings performed at 32°C–34°C, brophenexin inhibited the NMDA‐evoked [Ca2+]i response by 42% ± 10% but NBA was without effect. These results are consistent with a model in which TRPM4 interacts with NMDARs to potentiate Ca2+ flux through the NMDAR ion channel and thus provides a potential mechanism for the neuroprotection afforded by NMDAR/TRPM4 interface inhibitors such as brophenexin. Background/Objectives: Methicillin-resistant Staphylococcus aureus (MRSA) can cause cases of community-acquired pneumonia, hospital-acquired pneumonia, and ventilator-associated pneumonia, and nasal colonization with this pathogen increases the risk of infection. Due to its high negative predictive value, multiple studies support using the MRSA nasal polymerase chain reaction (PCR) test to discontinue antimicrobials that target MRSA in the setting of a negative test result. The purpose of this project was to assess the utility of a protocol to allow pharmacists the ability to order MRSA nasal PCR screenings in hospitalized patients with pneumonia. Results: The pre-protocol group included a random sample of 100 patients, and the post-protocol group included 625 patients. Vancomycin DOTs when pharmacists ordered PCRs were significantly lower compared to the pre-protocol group (p < 0.5; 95% CI, 0.46–2.24). The average length of stay and readmission rates at 30 days were significantly lower in the post-protocol group compared to the pre-protocol group (p < 0.05 and p = 0.02, respectively), but there was no significant difference in mortality (p = 0.33). Methods: A protocol was implemented at our institution that allowed pharmacists to order an MRSA nasal PCR test in patients with pneumonia. This retrospective chart review compared a cohort of patients who received vancomycin from before implementation of the protocol to patients who received vancomycin after the protocol's implementation. The primary endpoint was vancomycin days of therapy (DOTs) between the pre-protocol group and the post-protocol group. Other endpoints assessed included the length of hospitalization, readmission rates, and mortality. Conclusions: Pharmacists ordering MRSA nasal PCR tests significantly reduced vancomycin DOTs, average length of stay, and 30-day readmission rates, contributing to positive outcomes in patients with pneumonia. The boundary between the thermosphere and exosphere is often given the simplified description of being a separation between highly collisional continuum mechanics and a collisionless domain. The realistic smooth transition through this space has historically presented a challenge to model as the assumptions used to simplify the Boltzmann equation in fluid models are invalidated at higher altitudes. A lack of rigorous modeling of the region limits the ability to understand the dynamics of light atmospheric species. This manuscript describes the dynamics present in a two‐way coupled fluid‐particle atmospheric model extending from the mesosphere through the exosphere with a smooth transition between fluid and particle domains. This model is used to examine the coupled nature of the thermosphere and exosphere using the fluid simulation TIME‐GCM and the direct simulation Monte Carlo simulation Monaco. The coupled model allows for examination of the thermosphere circulation and exosphere transport mechanisms, as well as their impacts on the distribution of hydrogen. In this analysis, upper transport regions in the exosphere are revealed and distinguished from lower transport regions during June solstice. Furthermore, coupling allows TIME‐GCM to account for effects of lateral exospheric transport of hydrogen, altering its upper boundary condition and consequentially the spatial distribution of hydrogen throughout the thermosphere. Finally, it is asserted that a self‐consistent hydrogen exobase distribution is necessary to constrain other analytical extrapolation techniques used to predict the vertical hydrogen profile in the exosphere. Plasma interactions are excluded from this study to isolate neutral dynamics. Plain Language Summary: With increasing altitude, the density of the atmosphere decreases. High above the ground, the gas can no longer be described as a continuous fluid. Instead, the motion of individual atoms is important. This transition occurs near 350–500 km (220–310 miles) in altitude, between the thermosphere and exosphere, making this region difficult to simulate. A common solution is to use a direct simulation Monte Carlo model to describe the motion of atoms in the exosphere and a fluid model to describe gas circulation in the thermosphere. However, previous studies have imposed unrealistic conditions on such combined models and not allowed the thermosphere to react to exospheric gas motion. This study describes the results of allowing a simulation of each region to be informed by the other. A focus is placed on neutral hydrogen, which comprises an increasing fraction of the gas at high altitudes, influences Earth's reaction to solar events, and undergoes atmospheric escape. With the new model, hydrogen motion in the exosphere can be fully realized. This motion affects the upper boundary of the thermospheric model and its hydrogen distribution. Finally, simplified methods for finding hydrogen density in the exosphere are compared to examine what type of errors may result. We report a large cryogenic magnetocaloric effect stemming from an unconventional borderline first-order magnetic phase transition with negligibly small thermomagnetic hysteresis in a rare-earth-based intermetallic compound PrNdIn. The sample exhibits maximum magnetic field-induced entropy change as large as −10 J/Kg K (for 20 kOe magnetic field change) near the boiling point of oxygen. Magnetocaloric properties of PrNdIn are comparable to those of other known potential magnetocaloric materials with operating temperatures ranging between 50 and 125 K. The magnetic properties of the present sample are qualitatively reminiscent of those of the binary Pr2In and Nd2In, including the emergence of a second low-temperature anomaly in the temperature dependence of magnetization. We investigated associations between prenatal genocidal trauma, including maternal rape, and postnatal adverse childhood experiences (ACEs) on DNA methylation of genes associated with the stress response. In a comparative cross-sectional study of 91 Rwandan young adults, categorized by prenatal exposure to genocide and maternal rape, genocide without rape, and unexposed controls, we analyzed DNA methylation from dried blood spots and assessed ACEs and depression and anxiety symptoms at age 24. Prenatal exposure to maternal rape was associated with DNA methylation changes in BDNF and SLC6A4, with the association in BDNF attenuated after including ACE exposure in the model. Genocide exposure without rape was associated with methylation changes in PRDM8 after adjusting for early adversity. Methylation in BDNF and SLC6A4 correlated with depression and anxiety symptoms. These findings underscore the impact of prenatal and postnatal trauma on DNA methylation and mental wellbeing, emphasizing the need for continued support for survivors in the decades after conflict. Head-up displays (HUDs) have the potential to change work in operation environments by providing hands-free information to wearers. However, these benefits may be accompanied by trade-offs, primarily by increasing cognitive load due to dividing attention. Previous studies have attempted to understand the trade-offs of HUD usage; however, all of which were focused on land-based tasks. A gap in understanding exists when examining HUD use in aquatic environments as immersion introduces unique environmental and physiological factors that could affect multitasking. In this study, we investigated multitasking performance associated with swimming with a HUD. Eighteen participants completed three tasks: swimming only, a HUD-administered word recall task, and a dual-task combining both tasks. Results revealed significant dual-task interference in both tasks, though possibly less pronounced than in land-based tasks. These findings enhance not only help characterise dual-task performance, but also offer valuable insights for HUD design for aquatic settings. However, past research examining HUD task performance has been restricted to land-based contexts. This paper documents the methodology and preliminary results from a perturbed parameter ensemble (PPE) technique, where multiple parameters are varied simultaneously and the parameter values are determined with Latin hypercube sampling. This is done with the Community Atmosphere Model version 6 (CAM6), the atmospheric component of the Community Earth System Model version 2 (CESM2). We apply the PPE method to CESM2–CAM6 to understand climate sensitivity to atmospheric physics parameters. The initial simulations vary 45 parameters in the microphysics, convection, turbulence and aerosol schemes with 263 ensemble members. These atmospheric parameters are typically the most uncertain in many climate models. Control simulations and targeted simulations to understand climate forcing due to aerosols and fast climate feedbacks are analyzed. The use of various emulators is explored in the multi-dimensional space mapping input parameters to output metrics. Parameter impacts on various model outputs, such as radiation, cloud and aerosol properties, are evaluated. Machine learning is also used to probe optimal parameter values against observations. Our findings show that PPE is a valuable tool for climate uncertainty analysis. Furthermore, by varying many parameters simultaneously, we find that many different combinations of parameter values can produce results consistent with observations, and thus careful analysis of tuning is important. The CESM2–CAM6 PPE is publicly available and extensible to other configurations to address questions of other model processes in the atmosphere and other model components (e.g., coupling to the land surface). Sexual reproduction requires the choreographed interaction of female cells and molecules with sperm and seminal fluid. In internally fertilizing animals, these interactions are managed by specialized tissues within the female reproductive tract (FRT), such as a uterus, glands, and sperm storage organs. However, female somatic reproductive tissues remain understudied, hindering insight into the molecular interactions that support fertility. Here, we report the identification, molecular characterization, and analysis of cell types throughout the somatic FRT in the premier Drosophila melanogaster model system. We find that the uterine epithelia is composed of 11 distinct cell types with well-delineated spatial domains, likely corresponding to functionally specialized surfaces that interact with gametes and reproductive fluids. Polyploidy is pervasive: More than half of lower reproductive tract cells are =4C. While seminal fluid proteins (SFPs) are typically thought of as male products that are transferred to females, we find that specialized cell types in the sperm storage organs heavily invest in expressing SFP genes. Rates of amino acid divergence between closely related species indicate heterogeneous evolutionary processes acting on male-limited versus female-expressed seminal fluid genes. Together, our results emphasize that more than 40% of annotated seminal fluid genes are better described as shared components of reproductive transcriptomes, which may function cooperatively to support spermatozoa. The study explores whether racial identity and appearance-based trustworthiness judgments can affect recognition of pain in medical students differing in levels of resting heart rate variability (HRV), a measure of parasympathetic control of the heart. After undergoing HRV assessment, 68 medical students (37 females) participated in a dynamic pain recognition task, viewing video clips of White and Black faces, which differed in perceived trustworthiness based on facial appearance, transitioning from neutral to intense pain expressions. Response time, pain intensity attribution and treatment recommendations were analyzed. Pain was recognized slower and estimated as less intense in Black compared to White faces, leading to a lower likelihood of recommending therapy. Pain recognition was faster for untrustworthy-looking White faces compared to trustworthy ones, while perceived trustworthiness had a minimal impact on the speed of pain recognition in Black faces. However, untrustworthy-looking faces were estimated to express more pain, particularly for Black faces. Notably, these biases were more pronounced in individuals with low, rather than high, resting HRV. Considering that therapeutic decisions mirrored pain intensity attribution, it would be important to increase awareness of these biases during medical training in order to promote equity in future pain assessment and treatment. The evidence that a healthy and safe workforce provides a competitive business advantage is increasingly clear. However, how to obtain this may be unclear to many. This article presents a case study showcasing how one large employer worked toward improving its culture of health and well-being. Measuring progress using an established corporate health assessment tool, results improved 75% over a 5-year period. In addition, site scan culture checks showed annual improvement, exceeding best-in-class scores by the fifth year. Building a culture of health and well-being often requires a few years to implement fully and involves a commitment to plan, deploy, improve, and manage over time. Communiqués were central to the coordination of the mass popular uprising that challenged Israeli rule over Palestinians from 1987 until the early 1990s. These short political texts were called manasheer or bayanat al-Intifada, in Arabic. The article explores how election deepfakes fit into broader frameworks of defamation and impersonation law and suggests that the U.S. Congress and the judiciary may address the threat by borrowing from both areas of law. Topics include brief overview on deepfake technology and its use in election campaigns, information on defamation and impersonation laws, analysis of deepfakes within the defamation and impersonation frameworks and mechanical crafting of the proposed remedy.

Objectives: Cortisol is an important metabolic hormone that regulates multiple physiologic systems. Cortisol metabolism is sensitive to early life environments, including that experienced prenatally. Limited research has evaluated factors that predict variation in maternal and offspring toddler hair cortisol, which is important since hair cortisol represents different dynamics of hypothalamic pituitary adrenal (HPA)‐axis function than more common salivary or serum measures. Methods: To address this gap, we longitudinally evaluated whether maternal depression measured in pregnancy and 1 month postnatal was associated with maternal and offspring hair cortisol levels approximately 15 months after birth (n = 46 mothers, 40 toddlers; mean 15.6 months postnatal, SD = 2.9 months). Results: Mean depression symptoms were highest during the prenatal period. Prenatal, but not postnatal, maternal depression was associated with offspring hair cortisol levels (B = 0.095, p =.01). Maternal hair cortisol was not associated with depression measured at either time point. Conclusions: These findings indicate that offspring hair cortisol more than a year after birth is associated with maternal prenatal depression, consistent with previous research in salivary cortisol, suggesting that long‐term offspring stress physiology may be influenced by conditions experienced in utero. Antiracist, anticapitalist feminist protests are at the forefront of contemporary mass protest in the Americas and beyond. As an interdisciplinary, intergenerational, international collective of feminist activist-intellectuals, we have been engaging with and analyzing contemporary protest since the global uprisings of the 2010s. Few things are bigger in feminism today than mass protest, yet there has been scant theorizing about it. Our manifesto accounts for the ubiquity of feminist epistemologies and practices marking an epochal shift in activism around the world. It proceeds in three parts. First, we analyze the current moment as a feminist conjuncture. Second, we conceptualize contemporary feminist protest, which we maintain must be theorized in itself rather than subsumed within the study of feminist movements. Finally, we advance an agenda for a feminist theory of protest, concluding that as activist assemblages, protest processes rearrange the ever-more diverse components of feminist fields, creating informal, fluid, if sometimes ephemeral, rhizomatic, and recombinant collectivities, articulations, and agglomerations. Feminist and other popular protests produce new organizational forms and generate realignments in activist fields. We propose that today's generative forms of protest presage the future shape of what we have conventionally called "social movements." We thereby address what we believe are big and largely unexplored feminist questions about protest and movements that should inspire new directions in feminist and critical social theory for years to come. Chronic loneliness and low perceived social support have been recognized as risk factors for both mental and cardiovascular disorders. It has been proposed that their link to psychophysiological problems may involve changes in parasympathetic activity. However, the exact underlying psychopathological mechanisms and the moderating effects of gender are still not thoroughly examined. Thus, the present study investigated associations between perceived social functioning and resting vagal tone in the context of potential cognitive and subclinical mediators and gender differences. Three hundred twenty‐five young adults (aged 18–35, 180 women) underwent an electrocardiogram measurement of 6‐minute resting heart rate variability (HRV). They also completed questionnaires assessing loneliness, perceived social support, social cognitive biases, depressive and social anxiety symptoms, and general mental health. In men, HRV was significantly and negatively associated with poorer perceived social functioning, depressive symptoms, and self‐reported social cognitive biases, while in women, there was a quadratic link between HRV and depressive symptoms and HRV and general mental health. Moderated mediation analysis revealed that depressive symptoms fully mediated the relationship between perceived social functioning and HRV in men. The results suggest that decreased resting vagal tone in lonely individuals is linked to depressive symptomatology rather than to specific social cognitive biases and that this association is significant only in men. While previous work on the relationship between loneliness and parasympathetic activity has yielded mixed results, the present study provides evidence for an association between reduced resting vagal tone, loneliness, and perceived social support. Importantly, we showed that this association was mostly related to subclinical depressive symptoms and was only observed in men, which may help better understand individual differences related to maladaptive physiological processes in lonely individuals. Objective Conduct a scoping review of research studies that describe rule-based clinical decision support (CDS) malfunctions. Materials and Methods In April 2022, we searched three bibliographic databases (MEDLINE, CINAHL, and Embase) for literature referencing CDS malfunctions. We coded the identified malfunctions according to an existing CDS malfunction taxonomy and added new categories for factors not already captured. We also extracted and summarized information related to the CDS system, such as architecture, data source, and data format. Results Twenty-eight articles met inclusion criteria, capturing 130 malfunctions. Architectures used included stand-alone systems (eg, web-based calculator), integrated systems (eg, best practices alerts), and service-oriented architectures (eg, distributed systems like SMART or CDS Hooks). No standards-based CDS malfunctions were identified. The "Cause" category of the original taxonomy includes three new types (organizational policy, hardware error, and data source) and two existing causes were expanded to include additional layers. Only 29 malfunctions (22%) described the potential impact of the malfunction on patient care. Discussion While a substantial amount of research on CDS exists, our review indicates there is a limited focus on CDS malfunctions, with even less attention on malfunctions associated with modern delivery architectures such as SMART and CDS Hooks. Conclusion CDS malfunctions can and do occur across several different care delivery architectures. To account for advances in health information technology, existing taxonomies of CDS malfunctions must be continually updated. Alzheimer's disease (AD) is a progressive neurodegenerative disease characterized by amyloid plaques and cognitive decline, the latter of which is thought to be driven by soluble oligomeric amyloid-β (oAβ). The dysregulation of G protein–gated inwardly rectifying K+ (GIRK; also known as Kir3) channels has been implicated in rodent models of AD. Here, seeking mechanistic insights, we uncovered a sex-dependent facet of GIRK-dependent signaling in AD-related amyloid pathophysiology. Synthetic oAβ1–42 suppressed GIRK-dependent signaling in hippocampal neurons from male mice, but not from female mice. This effect required cellular prion protein, the receptor mGluR5, and production of arachidonic acid by the phospholipase PLA2. Although oAβ suppressed GIRK channel activity only in male hippocampal neurons, intrahippocampal infusion of oAβ or genetic suppression of GIRK channel activity in hippocampal pyramidal neurons impaired performance on a memory test in both male and female mice. Moreover, genetic enhancement of GIRK channel activity in hippocampal pyramidal neurons blocked oAβ-induced cognitive impairment in both male and female mice. In APP/PS1 AD model mice, GIRK-dependent signaling was diminished in hippocampal CA1 pyramidal neurons from only male mice before cognitive deficit was detected. However, enhancing GIRK channel activity rescued cognitive deficits in older APP/PS1 mice of both sexes. Thus, whereas diminished GIRK channel activity contributes to cognitive deficits in male mice with increased oAβ burden, enhancing its activity may have therapeutic potential for both sexes. Spatial-temporal shifts in submersed aquatic vegetation community structure resulting from a selective herbicide treatment in Lake Sampson, Florida, USA. Lake Reserv Manage. 40:248–263. Submersed aquatic vegetation (SAV) is a major ecological component of Florida's shallow lake systems. Hydrilla (H; Hydrilla verticillata [L.F.] Royle) is a nonnative, invasive SAV dominating many of these lakes and a high management priority in conservation. Lake Sampson is a shallow 776-ha lake in north Florida dominated by hydrilla since the 1980s, with native Illinois pondweed (P; Potamogeton illinoensis Morong) and American eelgrass (V; Vallisneria americana Michx.) historically represented. In March 2021, an in-water treatment of the herbicide florpyrauxifen-benzyl was administered to the littoral zone covering 46% of the lake surface. Five months after treatment (MAT), H infestations were reduced by 99%, recruitment by 69%, and invasions to cohabitating native populations by 90%. Conversely, total combined P populations increased occupancy from 15% to 23%, while V populations exhibited only a slight decrease in occupancy, going from 19% to 18%. By 17 MAT, hydrilla infestations, recruits, and invasions had all increased, while native populations started to retract from earlier gains. Moreover, surveys revealed a significant shift in SAV communities to shallower depths that were immediate 1 month after treatment and sustained 17 months later with the posttreatment reduction of hydrilla exclusively occupying deeper water. Posttreatment changes in water quality were apparent and possibly influential to SAV community dynamics. Although this in-water herbicide treatment exhibited good selectivity and reduced hydrilla competition, unintended side effects in water quality may have impeded the long-term succession of resilient native SAV communities that could compete with the recovery of hydrilla. With the proliferation of low Earth orbit (LEO) satellites carrying GNSS receivers on‐board commercial operators such as Spire, Starlink, OneWeb, and Amazon, an abundance of high‐cadence tracking data could become available to the scientific community. While GNSS measurements from geodetic‐grade receivers on satellites like SWARM, CHAMP, GRACE, and GOCE have been extensively used for atmospheric density retrieval, limited research has explored the potential of less accurate data from commercial operators. This study focuses on two methods to estimate atmospheric densities from precision orbit determination (POD) products—precise positions and velocities—utilizing synthetic data sets. The first method, termed "POD accelerometry" treats the POD products as measurements to a reduced‐dynamic POD scheme with the goal of estimating densities using stochastic parameters. The second method known as the energy dissipation rate (EDR) approach derives densities from changes in orbital energy. The relative contributions of various error sources—dynamics model uncertainties, and POD noise—to the estimated densities are studied for a limited set of orbital regimes and space weather activity, and possible error mitigation strategies are suggested. The performance of the two methods and their sensitivities to these various error sources are compared for circular orbits in the altitude regime 300–800 km during solar minimum F10.7=72.5 $\left({F}_{10.7}=72.5\right)$. EDR and POD accelerometry have comparable performances for high drag, low POD noise environments, whereas the latter performs considerably better in low drag <10−6m/s2 $\left(< 1{0}^{-6}\ \mathrm{m}/{\mathrm{s}}^{\mathrm{2}}\right)$, high POD noise (>25 ${ >} 25$ cm) environments, with densities retrieved at higher cadences for the orbital regimes considered in this work during solar minimum. We consider the two-sided many-to-one matching problem and introduce a class of preferences reflecting natural forms of complementarities. For example, academic departments hire seniors and then supporting juniors, teams recruit different roles and specialties, starting with the critical ones, and firms hire workers at various levels, starting with the executives. The key feature is that a firm can partition workers into types and prioritize certain types before others. Despite this partitionability requirement of choice functions being weaker than substitutes—an essential condition concerning the existence of a stable assignment—we show that it still guarantees the existence of a stable assignment and is further a maximal domain for such. While a growing body of interprofessional education (IPE) literature demonstrates a positive impact on learner knowledge, there is limited data on its long-term impact on collaborative practice (CP). With the growth of the aging population globally, understanding both the long-term impact on CP and sustainability of community-based geriatric experiential IPE programs are imperative. This study explores the impact of the Interprofessional Geriatric Curriculum (IPGC), a community-based geriatric IPE program, on post-graduate clinical practice among seven health professions. This study utilized a cross-sectional descriptive design, where both qualitative and quantitative data were collected in the same online survey of health professionals' to measure their perceptions of the impact IPGC has had on their respective clinical practice 1–3 years post-graduation. Forty-six per cent of health profession graduates provided clinical care for people 65 years of age or older; 81% worked in interprofessional teams; 80% reported the IPGC experience significantly impacted their practice (N = 137), and all used validated assessment tools taught in the IPGC program in their practice. Eight themes emerged from the list of what health professionals learned from IPGC that they use regularly in their clinical practice: four themes were interprofessional in nature (i.e. teamwork and team-based care, interprofessional communication, roles/responsibilities, and personal/professional) and four themes related to geriatrics (i.e. aging, screening and assessment, medications, and didactic content). This study is one of the first to describe the sustained influence of a community-based IPE program across multiple health professional disciplines on clinical geriatric practice. The concept of general arousal has a long history in emotion research. However, the concept is more complex and nuanced than is generally appreciated. In this comment, we note some of the early conceptualizations of arousal and how they might comport with more modern representations of the construct. Importantly, we show how modern conceptualizations which incorporate the physiological complexity of arousal measurement and peripheral-central nervous system interactions might help to provide a more solid framework for the construct moving forward. Antisense oligonucleotides (ASOs) are promising therapeutics for treating various neurological disorders. However, ASOs are unable to readily cross the mammalian blood-brain barrier (BBB) and therefore need to be delivered intrathecally to the central nervous system (CNS). Here, we engineered a human transferrin receptor 1 (TfR1) binding molecule, the oligonucleotide transport vehicle (OTV), to transport a tool ASO across the BBB in human TfR knockin (TfRmu/hu KI) mice and nonhuman primates. Intravenous injection and systemic delivery of OTV to TfRmu/hu KI mice resulted in sustained knockdown of the ASO target RNA, Malat1, across multiple mouse CNS regions and cell types, including endothelial cells, neurons, astrocytes, microglia, and oligodendrocytes. In addition, systemic delivery of OTV enabled Malat1 RNA knockdown in mouse quadriceps and cardiac muscles, which are difficult to target with oligonucleotides alone. Systemically delivered OTV enabled a more uniform ASO biodistribution profile in the CNS of TfRmu/hu KI mice and greater knockdown of Malat1 RNA compared with a bivalent, high-affinity TfR antibody. In cynomolgus macaques, an OTV directed against MALAT1 displayed robust ASO delivery to the primate CNS and enabled more uniform biodistribution and RNA target knockdown compared with intrathecal dosing of the same unconjugated ASO. Our data support systemically delivered OTV as a potential platform for delivering therapeutic ASOs across the BBB. Background: A recent meta-analysis revealed that vagally mediated heart rate variability (vmHRV; a biomarker of emotion regulation capacity) significantly decreases in the luteal phase of the menstrual cycle. As two follow-up studies suggest, these vmHRV decreases are driven primarily by increased luteal progesterone (P4). However, analyses also revealed significant interindividual differences in vmHRV reactivity to the cycle, which is in line with longstanding evidence for interindividual differences in mood sensitivity to the cycle. The present study begins to investigate whether these interindividual differences in vmHRV cyclicity can explain who is at higher risk of showing premenstrual emotional changes. We expected a greater degree of midluteal vmHRV decrease to be predictive of a greater premenstrual increase in negative affect. Methods: We conducted an observational study with a naturally cycling community sample (N = 31, M = 26.03 years). Over a span of six weeks, participants completed (a) daily ratings of negative affect and (b) counterbalanced lab visits in their ovulatory, midluteal, and perimenstrual phases. Lab visits were scheduled based on positive ovulation tests and included assessments of baseline vmHRV and salivary ovarian steroid levels. Results: In line with previous research, multilevel models suggest that most of the sample shows ovulatory-to-midluteal vmHRV decreases which, however, were not associated with premenstrual emotional changes. Interestingly, it was only the subgroup with luteal increases in vmHRV whose negative affect markedly worsened premenstrually and improved postmenstrually. Conclusion: The present study begins to investigate cyclical changes in vmHRV as a potential biomarker of mood sensitivity to the menstrual cycle. The results demonstrate a higher level of complexity in these associations than initially expected, given that only atypical midluteal increases in vmHRV are associated with greater premenstrual negative affect. Potential underlying mechanisms are discussed, among those the possibility that luteal vmHRV increases index compensatory efforts to regulate emotion in those with greater premenstrual negative affect. However, future studies with larger and clinical samples and more granular vmHRV assessments should build on these findings and further explore associations between vmHRV cyclicity and menstrually related mood changes. As technology continues to advance, interest in how humans perceive and interact with autonomous agents has increased, spurring ample research within human-agent teams (HATs). However, the word team is a bit of a misnomer, in that much of this research has examined one human in relation to one agent. We extend the HAT literature by applying a multilevel lens to develop future research questions regarding trust in heterogeneous HATs. First, we assert that trust is an attitude that is multireferent, suggesting research should explore not just humans' trust in agents, but many other perspectives such as agents' trust in humans. Second, we assert that trust is multilevel, suggesting research should explore higher-level emergent forms of trust. Third and fourth, we assert trust is dynamic and event-based, suggesting research should explore discontinuous changes in trust in response to events such as agent- and human-enacted trust violation and repair. The article focuses on a study which assesses the feasibility of implementing a culturally appropriate Alzheimer's disease and related dementias (ADRD) workflow in primary care setting in the U.S. To determine the impact of the new workflow, ADRD workflow outcomes and demographic data pre- and post-implementation were extracted from the electronic medical record (EMR). Results revealed that all ADRD workflow outcomes were higher post-implementation compared to pre-implementation.

Objective: Poor ergonomic posture during interventional procedures might lead to increased physical discomfort and work-related musculoskeletal disorders. Adjunctive equipment such as lead aprons (LAs) has been shown to increase ergonomic posture risk (EPR). The objective of this study was to evaluate the effectiveness of StemRad MD (StemRad Ltd., Tel Aviv, Israel), a weightless exoskeleton-based radiation protective ensemble, in reducing EPR on the operator using wearable inertial measurement unit (IMU) sensors. Methods: A prospective, observational study was conducted at an academic hospital. Inertial measurement unit sensors were affixed to the upper back of 9 interventionalists to assess ergonomic risk posture during endovascular procedures while wearing a traditional LA or the StemRad MD radiation protection system. Total fluoroscopy time, procedure type, and ergonomic risk postures were recorded and analyzed. Results: Twenty-one cases were performed with StemRad MD and 30 with LAs. Mean procedure time for the StemRad MD procedures was 48.4±23.3 minutes (range: 24–106 min), and for LA procedures, it was 34.66±25.83 minutes (range: 6–100 min) (p=.060). The operators assumed low-risk ergonomic positions in 96.1% of StemRad MD cases and in 62.9% of LA cases (p=.001), and high-risk ergonomic positions in 0% and 6.2%, respectively (p=.80). Mean EPR score for StemRad MD was 1.16, and for the LA, it was 1.49 (p=.001). Conclusions: StemRad MD significantly reduces the EPR to the torso compared with a LA-based radiation protection system. Clinical Impact: Poor ergonomic posture during interventional procedures might leas to work-related musculoskeletal disorders for healthcare workers. StemRad MD, a weightless, exoskeleton-based radiation protection system was shown to significantly reduce ergonomic posture risk to the torso compared to conventional lead aprons. The tumor suppressor protein p53, a transcription factor playing a key role in cancer prevention, interacts with DNA as its primary means of determining cell fate in the event of DNA damage. When it becomes mutated, it opens damaged cells to the possibility of reproducing unchecked, which can lead to formation of cancerous tumors. Despite its critical role, therapies at the molecular level to restore p53 native function remain elusive, due to its complex nature. Nevertheless, considerable information has been amassed, and new means of investigating the problem have become available. Objectives: We consider structural, biophysical, and bioinformatic insights and their implications for the role of direct and indirect readout and how they contribute to binding site recognition, particularly those of low consensus. We then pivot to consider advances in computational approaches to drug discovery. Materials and methods: We have conducted a review of recent literature pertinent to the p53 protein. Results: Considerable literature corroborates the idea that p53 is a complex allosteric protein that discriminates its binding sites not only via consensus sequence through direct H-bond contacts, but also a complex combination of factors involving the flexibility of the binding site. New computational methods have emerged capable of capturing such information, which can then be utilized as input to machine learning algorithms towards the goal of more intelligent and efficient de novo allosteric drug design. Conclusions: Recent improvements in machine learning coupled with graph theory and sector analysis hold promise for advances to more intelligently design allosteric effectors that may be able to restore native p53-DNA binding activity to mutant proteins. Clinical relevance: The ideas brought to light by this review constitute a significant advance that can be applied to ongoing biophysical studies of drugs for p53, paving the way for the continued development of new methodologies for allosteric drugs. Higher self‐reported rumination, a common form of trait perseverative cognition, is linked with lower resting heart rate variability (HRV), which indicates poorer cardiac function and greater disease risk. A meta‐analysis and systematic review indicated that in samples with fewer European Americans, the association of rumination with both heart rate and blood pressure was stronger. Thus, trait rumination may be more strongly associated with resting HRV among ethnically minoritized populations. The current study investigated whether differences in the association of self‐reported rumination with resting HRV varied by ethnicity in a sample (N = 513; Mage = 19.41; 226 Women) of self‐identified African Americans (n = 110), Asian Americans (n = 84), and European Americans (n = 319). Participants completed a five‐minute baseline period to assess resting HRV, followed by the Ruminative Responses Scale, which contains three facets of rumination including brooding, depressive, and reflective rumination. On average, Asian Americans reported higher levels of rumination relative to European Americans. African Americans had higher resting HRV than Asian Americans. Adjusting for covariates, higher self‐reported rumination was significantly associated with lower resting HRV in both African and Asian Americans, but not significantly so in European Americans. This finding was consistent for brooding and reflective, but not depressive rumination. Overall, this study lends insight into a psychological mechanism—rumination—that may impact health disparities among ethnically minoritized individuals, contributing to an understanding of how stress gets under the skin among such minoritized populations. Background: While assisted reproductive technologies (ART) have helped many people experiencing infertility become pregnant, the ART process can take a psychological toll. This study examined whether and how perceived stress- and depression-related symptoms vary among individuals at different stages of the infertility and ART process, and whether ART-specific stressors and emotional support are associated with mental health symptomatology. Methods: Data were collected using an online REDCap survey administered between July 2021 and March 2022. The survey was administered to 240 participants who had experienced infertility, including those who had not yet accessed ART, those undergoing ART but who were not yet pregnant, those currently pregnant through ART, and those who had given birth in the last year through ART. Each participant completed the Cohen Perceived Stress Scale (range 0–40) and the Edinburgh Depression Scale (range 0–30). Participants who had undergone ART were asked about their experience of ART-specific stressors and how helpful partner and provider support had been during the ART process. Survey data were analyzed using ANOVA and multivariate linear regressions. Results: 88% of participants reported medium or high levels of perceived stress, and 43.8% of respondents showed probable indications of depression. Perceived stress and depression symptoms were significantly higher for individuals currently undergoing, but not yet pregnant from, ART treatments. These effect sizes were substantial; for example, depression scores in this group were five points higher than among currently pregnant individuals and nine points higher than among postpartum individuals. For the subset of participants who had used or were currently undergoing ART (N = 221), perceived social stigma and the physical and time demands of ART were significantly associated with higher stress and depression symptoms, while partner emotional support was associated with lower perceived stress. Conclusions: The ART process exacerbates perceived stress and depression symptoms among individuals experiencing infertility. Given the potential long-term impacts on both parent and child wellbeing, clinicians and policymaking groups, including the American Society for Reproductive Medicine (ASRM), should consider making access to mental health services a standard of care during infertility treatment. Magnetic resonance imaging (MRI) is widely used in clinical care and medical research. The signal‐to‐noise ratio (SNR) in the measurement affects parameters that determine the diagnostic value of the image, such as the spatial resolution, contrast, and scan time. Surgically implanted radiofrequency coils can increase SNR of subsequent MRI studies of adjacent tissues. The resulting benefits in SNR are, however, balanced by significant risks associated with surgically removing these coils or with leaving them in place permanently. As an alternative, here the authors report classes of implantable inductor–capacitor circuits made entirely of bioresorbable organic and inorganic materials. Engineering choices for the designs of an inductor and a capacitor provide the ability to select the resonant frequency of the devices to meet MRI specifications (e.g., 200 MHz at 4.7 T MRI). Such devices enhance the SNR and improve the associated imaging capabilities. These simple, small bioelectronic systems function over clinically relevant time frames (up to 1 month) at physiological conditions and then disappear completely by natural mechanisms of bioresorption, thereby eliminating the need for surgical extraction. Imaging demonstrations in a nerve phantom and a human cadaver suggest that this technology has broad potential for post‐surgical monitoring/evaluation of recovery processes. [ABSTRACT FROM AUTHOR]
