Post-metachronous ampulla of Vater carcinoma after gastric cancer is an extremely rare between multiple primary cancers. The published literature contains only a few cases. Among the cases males are found more frequently. The majority of the cases reported after early gastric cancer. The main problem is to distinguish between new cancer and metastasis in gastric cancer patients. Early onset of jaundice, which is the main clinical finding, is important in diagnosis. The main type of surgery is pancreaticoduodenectomy. In this study, the clinic, diagnosis, and treatment of the post-metachronal ampulla of Vater cancer and the highlights of management options reviewed. [ABSTRACT FROM AUTHOR]

This study evaluated the shrinkage of surgical margins in colorectal specimens prior to resection and at final examination and established a correction factor to ensure a sufficient resection margin. The proximal and distal surgical margins in normal anatomic positions were measured in patients with colorectal cancer prior to resection during surgery. Fresh specimens were sent to the pathology laboratory, and the distance of the tumor to the proximal and distal margins were measured before formalin fixation and after 24 h formalin fixation. The mean proximal surgical margins prior to resection, after resection, at 24 h after fixation, and after a histopathological examination were 19.8 (SD7.1) cm, 17 (SD7.5) cm, 14 (SD5.6) cm, and 13. 6 (SD5.3) cm, respectively. The values for the distal surgical margin were 12 (SD10.9) cm, 10.3 (SD9) cm, 7. 9 (SD7.7) cm and 7.7 (SD6.4) cm in respective order. The correction factor calculated for the proximal and distal resection margins prior to resection and after the final pathological examination were 1.45 and 1.55, respectively. It would be beneficial to establish and make use of correction factors when predicting shrinkages in surgical margins in curative colorectal cancer surgery. [ABSTRACT FROM AUTHOR]

Acinar cystic transformation (ACT) or acinar cell cystadenoma is a rare benign tumor of the pancreas. Their clinical presentation is nonspecific, thus hampering their detection and frequently leading to misdiagnosis. In this report, we present a case of ACT of the pancreas in a 54-year-old man that was vague abdominal symptoms. A magnetic resonance imaging was shown a well-defined cystic mass located in the head of the pancreas, measuring 57×47×23 mm. Our patient was treated by pancreaticoduodenectomy. In the macroscopic examination of the mass, a multicystic lesion was found in cream-colored pancreatic tissue. Histological and immunohistochemical studies examination revealed a cystic mass containing multiple cysts at varying sizes, lined by epithelial cells without atypia and positive staining of CK7, CK8/18, and CK19. A high index of clinical suspicion is required to diagnose this tumor. The surgical approach is to ensure accurate diagnosis and to avoid complications. [ABSTRACT FROM AUTHOR]

Purpose: Warm-ischemia-induced injuries might be encountered during renal transplants from cadavers and healthy donors. Toll-like receptors (TLR) in ischemia-reperfusion (I/R) injury are one of the indicators of intracellular injury pathways. The intensity of ischemic injury is directly proportionate to high TLR levels. To minimize the I/R injury, we investigated TLR2 and TLR4 levels on rats, which were pretreated with tacrolimus (FK506) before I/R. Methods: Eight Wistar albino rats in the study group were administered .01 mg/kg intramuscular tacrolimus. Administration to the study group was performed 24 and 1 h before warm ischemia. Eight rats in the control group were injected with 0.1 c.c. of distilled water. Blood samples were collected from the tail veins of all the rats on the first, second and third days. Expression levels of TLR2 and TLR4 genes were analyzed using the polymerase chain reaction method, to determine any significant difference between the control and study groups on the days when blood was taken. Results: TLR2 ( p = 0.045) and TLR4 ( p = 0.022) levels in the study group were found to be statistically, and significantly, lower than those in the control group, on the second day following warm-ischemia- and reperfusion-induced injury. Conclusions: Administration of immunosuppressive drugs to healthy donor rats led to a statistically significant reduction in the expression levels of TLR2 and TLR4 in the early period. In light of the data obtained by this study, we hypothesize that a preoperative therapy on donors might have a role in preventing I/R injury. [ABSTRACT FROM AUTHOR]
