Background: Metabolic bariatric surgery (MBS) is standardized and safe. Nevertheless, complications such as anastomotic leakage (AL) or staple-line leakage (SLL) can occur. In upper GI or colorectal surgery, endoluminal vacuum therapy (EVT) offers a therapeutic alternative to revisional surgery. Data on EVT in patients with leakage after MBS remain scarce. The aim of this study is to evaluate the efficacy of EVT and its potential as endoscopic alternative to revisional surgery. Material and Methods: All patients treated for AL or SLL with EVT after MBS between 01/2016 and 08/2023 at the Department for General Surgery, Medical University Vienna, were included in this retrospective, single-center study. Therapeutic value of EVT as management option for acute postoperative leakage after MBS in daily practice was evaluated. Statistical analyses were performed descriptively. Results: Twenty-one patients were treated with EVT within the observational period of 7 years. In 11 cases (52.4%), the index surgery was a primary bariatric intervention; in 10 cases (47.6%), a secondary surgery after initial MBS was performed. Favored approach was a combination of revisional surgery and EVT (n = 18; 85.7%), intermediate self-expanding metal stent (SEMS) in 16 (76.2%) cases. EVT was changed six times (0–33) every 3–4 days. Mean EVT time was 25.1 days (3–97). No severe associated complications were detected and EVT showed an efficacy of 95.2%. Conclusion: This small case series supports the trend to establish EVT in daily clinical practice when revisional surgery after MBS is needed, thus preventing further reoperation and reducing associated morbidity and mortality in critically ill patients. We present the validation of a radiochemical procedure for the determination of 55Fe and 63Ni in water and steel samples from nuclear decommissioning activities. The in-house validation was an essential requirement of the accreditation to ISO/IEC 17025. A combination of co-precipitation, anion chromatography and extraction chromatography steps was used for separating and purifying both radionuclides. The activities were measured by liquid scintillation counting. Due to the lack of reference materials, the method was validated by analysing simulated samples and by the standard addition technique. The parameters precision/accuracy, repeatability/linearity, selectivity/specificity, decision threshold, detection limit and uncertainty of the method were evaluated. [ABSTRACT FROM AUTHOR]

Mercury (Hg) compounds were used in the past in primers for rifle and handgun ammunition. Despite its toxicity, little is known about the contamination of shooting-range soils with this metal. We present new data about the Hg contamination of surface soils from numerous shooting ranges of Switzerland. Our study demonstrates that Hg is measurable at high levels in surface soils from the shooting ranges. In three of the investigated ranges, concentrations above the maximum Swiss guidance value of Hg in soil of 500 µg kg were measured. Since the use of mercury-containing ammunition was stopped in the 1960s, our results demonstrate the high persistence of Hg in soils and their slow recovery by natural mechanisms. Aims: Despite many studies on the impact of arable land conversion to Short Rotation Coppice (SRC), few studies have been carried out on soil biota. This study aims at assessing biological and physico-chemical soil properties that are affected by SRC compared to forestry, grassland and an agrosystem. Methods: All samples were collected in the Aisne valley (France), from the same type of soil, with four land uses, i.e. willow SRC, agrosystem, grassland and alluvial forest, 3 years after SRC was planted. We studied fertility, the biological community (earthworm diversity, density and biomass, bacterial and fungal density and community structures) and biochemical parameters (enzyme activities, basal respiration and nitrification). Results: After 3 years' growth, soil biological parameters (fungal abundance, laccase activity, anecic earthworm proportion and earthworm diversity) and CEC were higher in the SRC than in the agrosystem soil. In parallel, fungal abundance was higher in SRC than in forest and grassland soils. Conclusion: Compared to annual arable crops, SRC promoted biological properties. However, in the short term, the parameters we measured were lower than in the forest and grassland soils. The use of certain parameters as indicators of soil functioning/quality assessment to discriminate the four land uses is discussed. [ABSTRACT FROM AUTHOR]

Splash-form tektites are glassy rocks ranging in size from roughly 1 to 100 mm that are believed to have formed from the splash of silicate liquid after a large terrestrial impact from which they are strewn over thousands of kilometres. They are found in an array of shapes including spheres, oblate ellipsoids, dumbbells, rods and possibly fragments of tori. It has recently become appreciated that surface tension and centrifugal forces associated with the rotation of fluid droplets are the main factors determining the shapes of these tektites. In this contribution, we compare the shape distribution of 1163 measured splash-form tektites with the results of the time evolution of a 3D numerical model of a rotating fluid drop with surface tension. We demonstrate that many aspects of the measured shape distribution can be explained by the results of the dynamical model. [ABSTRACT FROM AUTHOR]

OBJECTIVE: A previously developed pretest score was validated to stratify patients presenting for exercise testing with suspected coronary disease according to the presence of angiographic coronary disease. Our goal was to determine how well this pretest score risk stratified patients undergoing pharmacological and exercise stress tests concerning prognostic endpoints. DESIGN: Retrospective cohort analysis. SETTING: University hospital stress laboratory. PATIENTS: 7452 unselected ambulatory patients with symptoms of suspected coronary disease undergoing stress testing between 1995 and 2004. MAIN OUTCOMES MEASURES: All-cause death, cardiac death and non-fatal myocardial infarction. RESULTS: The rate of all-cause death was 5.5% (CI 5.0 to 6.1) with 4.3 (SD 2.4) years of follow-up (Exercise 2.8% (CI 2.3 to 3.2) v Pharmacological group 11.9% (CI 10.5 to 13.3); p<0.001). The rate of cardiac death/myocardial infarction was 2.6% (CI 2.2 to 3.0) (Exercise 1.4% (CI 1.1 to 1.8) v Pharmacological group 5.3% (CI 4.3 to 6.2); p<0.001). In both groups, stratification by pretest score was significant for all-cause death and the combined endpoint. However, stratification was more effective in the pharmacological group using the combined endpoint rather than all-cause death. Pharmacological stress patients in intermediate and high risk groups were at higher risk than their respective exercise test cohorts. Referral for pharmacological stress testing was found to be an independent predictor of time to death (2.7 (CI 2.0 to 3.6); p<0.001). CONCLUSION: A pretest score previously validated to stratify according to angiographic outcomes, effectively risk stratified pharmacological and exercise stress patients according to the combined endpoint of cardiac death/myocardial infarction. Postcollisional (1.8-1.7 Ga) intracontinental deformation in the Trans-Hudson Orogen (Canada) produced a series of orogen-parallel high-angle faults and folds. In seismic reflection profiles, the faults are imaged by subvertical zones of diffractions and truncated reflections that extend to 4-8 s (12-24 km). The folded and faulted upper part of the crust is underlain by laterally coherent shallow-dipping reflections that are locally bounded by discrete, highly reflective zones. These zones are interpreted as detachments (shear zones) and can be traced from the upper to lower crust, where some of them appear to pass into laterally continuous reflections that define the Moho. Two distinct regimes of postcollisional crustal deformation are inferred from the seismic images: high-angle faulting and lateral block extrusion in the upper crust and low-angle ductile shearing in the mid/lower crust. The surface geology indicates that the faults resulted in southwest (orogen-parallel) extrusion of the orogen's internal zone relative to the bounding Archean Hearne and Superior cratons. Faulting was concurrent with the development of upright folds with trends that are subparallel to the extrusion direction. The seismic images suggest that the high-angle fold/fault structures are kinematically linked to low-angle detachments represented by laterally coherent, highly reflective zones. The detachment shear zones are inferred to have a top-to-the-southwest sense of shear associated with a subhorizontal, northeast-southwest extension direction, parallel to those observed for 1.83-1.80 Ga collisional shear zones exposed in major postcollisional fold culminations. Long-lived orogen-parallel extension is interpreted as a consequence of the boundary conditions imposed by the northeast trend of both the Superior and Hearne margins. [ABSTRACT FROM AUTHOR]

Shear zones in the Flin Flon area record five episodes of development. These have been studied using structural analysis and 40Ar/39Ar thermochronology. Deformation occurred as a result of collision between the Archean Rae-Hearne and Archean Superior plates during the Trans-Hudson Orogeny (1900 to 1700 Ma). The geodynamic evolution encompasses a transition from island arc supracrustal development (∼1900 Ma) to tectonic mountain building under brittleductile conditions (>1860 Ma), two episodes of ductile lineation-schistosity (L-S) shear zones on the prograde metamorphic path (1860-1810 Ma), and two episodes of retrograde displacement between 1790 and 1690 Ma, during final suturing of the Archean cratons with Paleoproterozoic assemblages. Metamorphism evolved in three stages. Contact aureoles to granitoid bodies formed at 1860-1840 Ma. Hornblende homfels facies 40Ar/39Ar ages indicate cooling through 500°C at ∼1840 Ma. This was followed by regional heating (burial), which produced regional peak thermal recrystallization at ∼1820-1790 Ma, under lower greenschist to lower amphibolite conditions. Lastly, retrograde metamorphism took place during unroofing between 1790 and 1690 Ma, with a regional cooling rate of 2 °C/m.y. [ABSTRACT FROM AUTHOR]

A method is presented for the determination of a triaxial ellipsoid ( such as a strain ellipsoid) from three nonparallel plane sections of the ellipsoid. The sections need be neither orthogonal nor central sections of the ellipsoid. Measurement errors are used to adjust the observed plane ellipses so that they are exact sections of the nearest true ellipsoid, whose dimensions and orientation are then found by solution of a system of six linear equations. A solution of the inverse problem is also presented: given a triaxial ellipsoid with known orientation, to determine the shape and orientation of the ellipse on a plane section. The problem is solved by expanding the equation of an ellipsoid with rotated coordinates, then setting one dimension to zero. Also, a method is presented for the projection of a triaxial ellipsoid onto a plane surface. This is solved by taking the derivative of the ellipsoid equation in the direction of the normal to the plane surface. [ABSTRACT FROM AUTHOR]

In a 4-year controlled, prospective trial, histomorphometric analysis was used to compare the tissue-level skeletal effects of fluoride therapy in 43 postmenopausal women (75 mg NaF/day) with those of 35 matching placebo subjects; all subjects received 1500 mg/day elemental calcium supplement. In addition to an initial, baseline biopsy, a second biopsy was obtained after 6, 18, 30 or 48 months. Measurements were made on a third biopsy obtained from 8 subjects following at least 72 months of fluoride therapy. The change in cancellous bone volume or trabecular thickness in fluoride-treated subjects was not different from a change in placebo-treated subjects. However, paired analysis in the fluoride-treated subjects indicated that bone volume was increased between the first and second biopsies ( p<0.005). Both osteoid length and width were significantly increased in fluoride compared with placebo subjects; however, only the osteoid surface increased linearly ( r=0.63, p<0.001). The mineral apposition rate and relative tetracycline-covered bone surface were not different between fluoride and placebo treatment, although they were decreased in both groups in the second biopsy. The tetracycline-covered bone surface returned to normal in the third biopsy. Definitive evidence for osteomalacia is a prolonged mineralization lag time, which following fluoride treatment was found to be increased 9-fold in the second biopsy and 4-fold in the third biopsy. Further evidence for osteomalacia was increased osteoid thickness by 6 months, evidence of focal areas of interstitial mineralization defects, and broad tetracycline labels of low fluorescence intensity. In the third biopsies, osteoclastic resorption was observed beneath osteoid seams. Fluoride therapy increased the cortical width compared with placebo treatment ( p<0.02), and increased the osteoid surface in Haversian canals, but did not change the osteoid width, resorption surface or cortical porosity. After an initial rise, serum fluoride levels remained constant, and the urine values fell slightly. The bone fluoride concentration rose throughout the treatment period, and was correlated with the change in osteoid-covered bone surface ( r=0.56, p<0.001). Although we found definitive evidence for osteomalacia, the cause of the osteomalacia was not determined in this study. On the other hand, the presence of bone resorption beneath unmineralized osteoid and of osteocyte halos is suggestive of hyperparathyroidism. Thus, it is possible that the strong stimulus for bone formation brought about by fluoride therapy resulted in relative calcium deficiency. [ABSTRACT FROM AUTHOR]

Presents a study which discussed the clinical data, calcium metabolic studies and histologic examination of bone in a patient with severe progressive bone demineralization with respect to the etiology of the bone disease. Method of the study; Results and discussion; Conclusion.

Primary hyperoxaluria is a rare congenital disorder characterized by large quantities of urinary oxalate with resultant nephrocalcinosis and nephrolithiasis and by deposits of calcium oxalate in other organs. Renal failure occurs early in life. Reports of unsuccessful renal transplantation attempts in this disorder underscore the need for antemortem diagnosis. Percutaneous bone biopsy is a relatively new procedure that is easily done at bedside, safe, and of potentially high yield in the demonstration of tissue oxalate. Three cases presented here show the characteristic histologic picture seen in this disease. In one case, the diagnosis was established by bone biopsy. [ABSTRACT FROM AUTHOR]
