In a workforce characterized by constant disruption across social, economic, and environmental domains, as well as widespread employee suffering in the form of stress, burnout, and disengagement, both scholars and leaders have called for business to undergo transformative change in an effort to bring about collective flourishing. Drawing inspiration from Buddhist philosophy and research on the communicative constitution of organizations, we present a new theory that explains how organizations can foster a positive form of organizing—what we term humanistic organizing—through a tripartite framework of mindful organizational communication. Our theory demonstrates that transforming organizations to become more humanistic is a matter of rethinking an organization's underlying ethos, grounding that new ethos in humanistic principles, and then: (a) embedding the wisdom of this new ethos in organizational communication, (b) intentionally drawing on this wisdom in everyday talk and text, and (c) ethically enacting this wisdom in the form of ongoing organizing practices, which may include revising the preexisting organizing practices. We illustrate our theory through two case illustrations: one that reflects an organization that has demonstrated humanistic organizing from its inception, and another that reflects an organization that underwent a transformative process to become more humanistic. Purpose: To quantify the change in session rating of perceived exertion training impulse (RPE-TRIMP) that may occur in response to increased running distance at 3 running velocity ranges in elite sprinters. Methods: We monitored training load in elite sprinters (women: n = 7; men: n = 11) using wearable Global Positioning System technology and RPE-TRIMP for a total of 681 individual training sessions during a 22-week competition-preparation period. Internal training load was operationalized by RPE-TRIMP, and external training load was operationalized by distance covered in 3 velocity ranges. A linear mixed-effects model with athlete as a random effect was fit to RPE-TRIMP with total distance covered at ≤69.99% (low-velocity running [LVR]), 70% to 84.99% (high-velocity running [HVR]), and 85% to 100% (very-high-velocity running [VHVR]) of individual maximum velocity. Results: Increased running distance in all 3 velocity ranges (LVR, HVR, and VHVR) resulted in a significant (P <.001) increase in RPE-TRIMP. Coefficients (95% CIs) were.10 (.08–.11) for LVR,.23 (.18–.28) for HVR, and.44 (.35–.53) for VHVR. A 50-m increase in running distance covered in the LVR, HVR, and VHVR velocity ranges was associated with increases in RPE-TRIMP of 5, 11.5, and 22 arbitrary units, respectively. Conclusions: Internal training load, calculated as RPE-TRIMP, increased with increases in total distance covered in the LVR, HVR, and VHVR velocity ranges (P <.001). We investigated the structure of ice under nanoporous confinement in periodic mesoporous organosilicas (PMOs) with different organic functionalities and pore diameters between 3.4 and 4.9 nm. X-ray scattering measurements of the system were performed at temperatures between 290 and 150 K. We report the emergence of ice I with both hexagonal and cubic characteristics in different porous materials, as well as an alteration of the lattice parameters when compared to bulk ice. This effect is dependent on the pore diameter and the surface chemistry of the respective PMO. Investigations regarding the orientation of hexagonal ice crystals relative to the pore wall using x-ray cross correlation analysis reveal one or more discrete preferred orientation in most of the samples. For a pore diameter of around 3.8 nm, stronger correlation peaks are present in more hydrophilically functionalized pores and seem to be connected to stronger shifts in the lattice parameters. MiMiC is a framework for performing multiscale simulations in which loosely coupled external programs describe individual subsystems at different resolutions and levels of theory. To make it highly efficient and flexible, we adopt an interoperable approach based on a multiple-program multiple-data (MPMD) paradigm, serving as an intermediary responsible for fast data exchange and interactions between the subsystems. The main goal of MiMiC is to avoid interfering with the underlying parallelization of the external programs, including the operability on hybrid architectures (e.g., CPU/GPU), and keep their setup and execution as close as possible to the original. At the moment, MiMiC offers an efficient implementation of electrostatic embedding quantum mechanics/molecular mechanics (QM/MM) that has demonstrated unprecedented parallel scaling in simulations of large biomolecules using CPMD and GROMACS as QM and MM engines, respectively. However, as it is designed for high flexibility with general multiscale models in mind, it can be straightforwardly extended beyond QM/MM. In this article, we illustrate the software design and the features of the framework, which make it a compelling choice for multiscale simulations in the upcoming era of exascale high-performance computing. The software for chemical interaction networks (SCINE) project aims at pushing the frontier of quantum chemical calculations on molecular structures to a new level. While calculations on individual structures as well as on simple relations between them have become routine in chemistry, new developments have pushed the frontier in the field to high-throughput calculations. Chemical relations may be created by a search for specific molecular properties in a molecular design attempt, or they can be defined by a set of elementary reaction steps that form a chemical reaction network. The software modules of SCINE have been designed to facilitate such studies. The features of the modules are (i) general applicability of the applied methodologies ranging from electronic structure (no restriction to specific elements of the periodic table) to microkinetic modeling (with little restrictions on molecularity), full modularity so that SCINE modules can also be applied as stand-alone programs or be exchanged for external software packages that fulfill a similar purpose (to increase options for computational campaigns and to provide alternatives in case of tasks that are hard or impossible to accomplish with certain programs), (ii) high stability and autonomous operations so that control and steering by an operator are as easy as possible, and (iii) easy embedding into complex heterogeneous environments for molecular structures taken individually or in the context of a reaction network. As investigations in the biomedical applications of plasma advance, a demand for describing safe and efficacious delivery of plasma is emerging. It is quite clear that not all plasmas are "equal" for all applications. This Perspective discusses limitations of the existing parameters used to define plasma in context of the need for the "right plasma" at the "right dose" for each "disease system." The validity of results extrapolated from in vitro studies to preclinical and clinical applications is discussed. We make a case for studying the whole system as a single unit, in situ. Furthermore, we argue that while plasma-generated chemical species are the proposed key effectors in biological systems, the contribution of physical effectors (electric fields, surface charging, dielectric properties of target, changes in gap electric fields, etc.) must not be ignored. While important for athletic development and well-being in youth sport, knowledge remains limited around the processes underpinning triadic relationships between parents, athletes, and coaches. This study aimed to examine the relational processes that drive the functioning of parent–athlete–coach triads across three developmental stages of youth tennis. Using a collective-case-study design, 10 players, 10 coaches, and 9 mothers completed preinterview tasks and semistructured interviews and provided conversational history. Reflexive thematic analysis led to the generation of two higher order themes: foundations of relationship quality and factors enabling team effectiveness. Findings highlighted how specific relationship qualities (i.e., commitment, trust, respect, and parent–coach proximity) and team effectiveness constructs (i.e., shared goals, collaborative and adjusted roles, support, and role-specific communication) served to facilitate the tennis experience for triads. There is increasing interest in studying molecular motions in ionic liquids to gain better insights into their transport properties and to expand their applications. In this study, we have employed the fast field cycling relaxometry and pulsed field gradient nuclear magnetic resonance techniques to investigate the rotational and translational dynamics of fluorinated imide-based ionic liquids (ILs) at different temperatures. We have studied a total of six ILs composed of the 1-butyl-3-methylimidazolium cation ([BMIM]+) combined with chemically modified analogs of the bis((trifluoromethyl)sulfonyl)imide anion ([NTf2]− or [TFSI]−). The primary objective of this paper is to broaden the understanding of how the anion's conformational flexibility, fluorination, and mass affect the molecular dynamics of cations and anions. Our results indicate that flexibility has the most significant impact on the rotational and translational motions of ions. Meanwhile, the effect of fluorination and mass is only relevant when conformational flexibility does not change significantly between the ILs being compared. [ABSTRACT FROM AUTHOR]

Purpose: To retrospectively evaluate the outcome of implant-supported or combined tooth-implant-supported prostheses retained by electroplated double-crowns after 1–12 years. Methods: Twenty-five patients were retrospectively examined in a private dental practice in Hamburg, Germany. All had been rehabilitated with a removable prosthesis retained by electroplated double-crowns, for at least one year. Fifteen patients had implant-supported prostheses and 10 had combined tooth-implant-supported prostheses in the maxilla or the mandible. Biological and technical complications were recorded at the clinical examination and extracted from the patient records. Kaplan–Meier implant and tooth survival rates were calculated. Potential risk factors for severe complications were identified. Oral health-related quality of life (OHRQoL) was measured by a short version of the Oral Health Impact Profile (OHIP) questionnaire. Patients reported subjective chewing function using a visual analogue scale. Results: Kaplan–Meier survival rates were 100% for natural abutments and 90.9% for implants after 11.8 years (p = 0.54). Two implants in two patients were lost at 8 and 9 years due to peri-implantitis in the "solely implant" group. The most common complications were decementation of primary crowns and wear of the prosthetic teeth. The mean OHIP score for the group "tooth-implant-supported" was 5.2 ± 5.0, whereas the mean score for the "solely implant" group was 1.7 ± 2.9 (p = 0.039). Patients rated their subjective masticatory function very high with an average score of 9.4 ± 0.8 out of a possible 10. Conclusions: Implant-supported or combined tooth-implant-supported prostheses retained by electroplated double-crowns are a viable method of treatment with a satisfactory outcome. Generative artificial intelligence in general and ChatGPT in particular have risen in importance. ChatGPT is widely known and used increasingly as an information source for different topics, including science. It is therefore relevant to examine how ChatGPT portrays science and science-related issues. Research on this question is lacking, however. Hence, we simulate "interviews" with ChatGPT and reconstruct how it presents science, science communication, scientific misbehavior, and controversial scientific issues. Combining qualitative and quantitative content analysis, we find that, generally, ChatGPT portrays science largely as the STEM disciplines, in a positivist-empiricist way and a positive light. When comparing ChatGPT's responses to different simulated user profiles and responses from the GPT-3.5 and GPT-4 versions, we find similarities in that the scientific consensus on questions such as climate change, COVID-19 vaccinations, or astrology is consistently conveyed across them. Beyond these similarities in substance, however, pronounced differences are found in the personalization of responses to different user profiles and between GPT-3.5 and GPT-4. [ABSTRACT FROM AUTHOR]

Isoenzymes are variant forms of the same enzyme activity that exhibits different tissue distribution. When cells are damaged, intracellular content leaks into the blood stream, making them potential biochemical markers. Undergraduate health sciences students are expected to interpret laboratory findings of serum levels of biochemical markers and understand their relevance for diagnosis, prognosis and treatment. This study aimed to design an electrophoresis laboratory class that challenges health sciences students to discuss the experimental approach and interpret the results obtained from electrophoresis of Creatine Kinase (CK) isoenzymes separation. Mixtures of dyes were used to simulate the serum composition of CK isoenzymes in different clinical conditions and the resulting bands were quantified by densitometry. Two online questionnaires assessed students' learning progress and acceptance of the activity. Undergraduate students from biomedicine (n=10) and medicine (n=42) programs participated in a 120-minute pilot class at Universidade Federal Fluminense, Brazil. The post-class test showed a 21.9 % increase in correct answers compared to the pre-class test. In the survey, students positively rated the laboratory class, expressing improved understanding of theoretical concepts, recognizing the importance of basic biochemistry for their future careers and comprehending the electrophoretic evaluation of CK isoenzymes. In the United States, it is estimated that 15% of Latinos will experience a depressive or anxiety disorder during their lifetime. Education, prevention programming, and health interventions around topics such as stress, nutrition, mental health, and health maintenance for Latino immigrants are lacking, inadequate, or nonexistent. This type of programming may be protective against depression and anxiety. A total of 19 Latina women completed a five-week, group-based intervention to learn about stress, stress management, nutrition, mental health, and healthy behaviors in a culturally sensitive environment taught by native Spanish speakers. Program evaluation occurred through in-depth interviews and changes in anxiety and depression scores via the Generalized Anxiety Scale and the Patient Health Questionnaire, respectively. The team saw statistically significant decreases in the GAD-7 scores pre- and post-intervention (8.00 versus 5.08, p<0.05), but no differences in the PSS or the PHQ-2 scores. Group-based intervention and education taught by native Spanish speakers may be an acceptable and feasible approach to addressing anxiety in Latina immigrants. [ABSTRACT FROM AUTHOR]

Rationale: Opioid use during pregnancy can lead to negative infant health outcomes, including neonatal opioid withdrawal syndrome (NOWS). NOWS comprises gastrointestinal, autonomic nervous system, and neurological dysfunction that manifest during spontaneous withdrawal. Variability in NOWS severity necessitates a more individualized treatment approach. Ultrasonic vocalizations (USVs) in neonatal mice are emitted in isolation as a stress response and are increased during opioid withdrawal, thus modeling a negative affective state that can be utilized to test new treatments. Objectives: We sought to identify the behavioral and USV profile, brainstem transcriptomic adaptations, and role of kappa opioid receptors in USVs during neonatal opioid withdrawal. Methods: We employed a third trimester-approximate opioid exposure model, where neonatal inbred FVB/NJ pups were injected twice-daily with morphine (10mg/kg, s.c.) or saline (0.9%, 20 ul/g, s.c.) from postnatal day(P) 1 to P14. This protocol induces reduced weight gain, hypothermia, thermal hyperalgesia, and increased USVs during spontaneous morphine withdrawal. Results: On P14, there were increased USV emissions and altered USV syllables during withdrawal, including an increase in Complex 3 syllables in FVB/NJ females (but not males). Brainstem bulk mRNA sequencing revealed an upregulation of the kappa opioid receptor (Oprk1), which contributes to withdrawal-induced dysphoria. The kappa opioid receptor (KOR) antagonist, nor-BNI (30 mg/kg, s.c.), significantly reduced USVs in FVB/NJ females, but not males during spontaneous morphine withdrawal. Furthermore, the KOR agonist, U50,488h (0.625 mg/kg, s.c.), was sufficient to increase USVs on P10 (both sexes) and P14 (females only) in FVB/NJ mice. Conclusions: We identified an elevated USV syllable, Complex 3, and a female-specific recruitment of the dynorphin/KOR system in increased USVs associated with neonatal opioid withdrawal severity. Digital tools are becoming more commonly used by older adults in daily life as well as in residential care to facilitate group activities. There is, however, limited research exploring how the participants orient to the tools and whether the choice of material affects the interaction between the participants. This study investigates the use of analogue and digital communication tools in group activities with older adults aged 70–90 with and without dementia in residential care. Interactions during group activities, in which analogue and digital pictures were used to stimulate conversations, were analyzed to identify differences and similarities in how the two different tools are used, as well as any potential differences between how the two groups (persons with and without dementia) orient to the materials. The analysis shows that the use of a communication supporting tool, be it analogue or digital, promotes conversations. However, the digital condition seems to afford shared focus around the device. The results also reveal that the two participating groups orient toward the material objects in a similar fashion. Thus, the choice of material objects used in group activities has consequences for the interaction and should be taken into account when planning for group activities involving persons with and without dementia. Objectives We evaluate the effectiveness of large language models (LLMs), specifically GPT-based (GPT-3.5 and GPT-4) and Llama-2 models (13B and 7B architectures), in autonomously assessing clinical records (CRs) to enhance medical education and diagnostic skills. Materials and Methods Various techniques, including prompt engineering, fine-tuning (FT), and low-rank adaptation (LoRA), were implemented and compared on Llama-2 7B. These methods were assessed using prompts in both English and Spanish to determine their adaptability to different languages. Performance was benchmarked against GPT-3.5, GPT-4, and Llama-2 13B. Results GPT-based models, particularly GPT-4, demonstrated promising performance closely aligned with specialist evaluations. Application of FT on Llama-2 7B improved text comprehension in Spanish, equating its performance to that of Llama-2 13B with English prompts. Low-rank adaptation significantly enhanced performance, surpassing GPT-3.5 results when combined with FT. This indicates LoRA's effectiveness in adapting open-source models for specific tasks. Discussion While GPT-4 showed superior performance, FT and LoRA on Llama-2 7B proved crucial in improving language comprehension and task-specific accuracy. Identified limitations highlight the need for further research. Conclusion This study underscores the potential of LLMs in medical education, providing an innovative, effective approach to CR correction. Low-rank adaptation emerged as the most effective technique, enabling open-source models to perform on par with proprietary models. OBJECTIVE: To compare the vascular effects of pursuing more versus less glucose lowering in patients with younger or older age at diabetes diagnosis, and with shorter or longer diabetes duration. RESEARCH DESIGN AND METHODS: We studied 11,138 participants from the Action in Diabetes and Vascular Disease: Preterax and Diamicron MR Controlled Evaluation (ADVANCE) trial, classified into subgroups defined by age at diabetes diagnosis (≤50, >50-60, and >60 years) and diabetes duration (≤5, >5-10, and >10 years). RESULTS: Intensive glucose lowering significantly lowered the risk of the primary composite outcome of major macrovascular and microvascular events (hazard ratio 0.90, 95% CI 0.82-0.98) with no evidence of heterogeneity in the proportional effects across subgroups defined by age at diagnosis or diabetes duration (P for heterogeneity = 0.86 and 0.47, respectively). Similar consistent treatment effects were also observed for all-cause death, cardiovascular death, and the components of major vascular events. Cystic fibrosis (CF) transmembrane conductance regulator (CFTR) modulators are available to the majority of people with CF in the United States; little is known about pregnancy outcomes with modulator use. The aim of this retrospective study was to determine the impact of CFTR modulators on maternal outcomes. Does pregnancy differentially affect outcomes in female individuals with CF with and without CFTR modulator exposure? Data on pregnancies from 2010 to 2021 were collected from 11 US adult CF centers. Multivariable longitudinal regression analysis was performed to assess whether changes in percent predicted FEV 1 (ppFEV 1), BMI, pulmonary exacerbations (PEx), and Pseudomonas aeruginosa prevalence differed from prior to, during, and following pregnancy according to CFTR modulator use while adjusting for confounders. Infant outcomes are also described based on maternal modulator use. Among 307 pregnancies, mean age at conception was 28.5 years (range, 17-42 years), before pregnancy ppFEV 1 was 74.2, and BMI was 22.3 kg/m2. A total of 114 pregnancies (37.1%) had CFTR modulator exposure during pregnancy (77 with highly effective modulator therapy [HEMT] and 37 with other modulators). The adjusted mean change in ppFEV 1 from before pregnancy to during pregnancy was –2.36 (95% CI, –3.56 to –1.16) in the unexposed group and 2.60 (95% CI, 0.23 to 4.97) in the HEMT group, with no significant change from during pregnancy to 1 year after pregnancy. There was an overall decline in ppFEV 1 from before pregnancy to after pregnancy in the no modulator group (–2.56; 95% CI, –3.62 to –1.49) that was not observed in the HEMT group (1.10; 95% CI, –1.13 to 3.34). PEx decreased from before pregnancy to after pregnancy in the HEMT group, and BMI increased from before pregnancy to during pregnancy in all groups but with no significant change after pregnancy. Missing infant outcomes data precluded firm conclusions. We observed superior pregnancy and after pregnancy pulmonary outcomes in individuals who used HEMT, including a preservation of ppFEV 1 , compared with those unexposed to HEMT. Research question: How does Russian and German reading comprehension develop during the first 3 years of primary school in Russian heritage speakers and which factors predict performance in each language? Methodology: 40 Russian-German children were tested annually from first to third grade (age in grade 1 M = 7; 1 years). Reading was assessed with comparable Russian and German word and sentence reading comprehension tests. The Russian test was available in Cyrillic and Latin script (the latter for children who could not read Cyrillic letters). Potential predictors included lexicon size in Russian and German and rapid automatized naming (RAN) of numbers (both assessed in primary school). Other factors were assessed in kindergarten, that is, nonverbal reasoning and, derived from a parents' questionnaire, their socioeconomic status and language input in the family. Data and analysis: Linear regressions were used to compare the development of Russian and German. Stepwise regression analyses identified predictors for reading comprehension in each language. Findings: German reading ability improved faster than that of Russian, likely due to the increasing amount of German input outside the family, literacy instruction in German at school, and the large variance in Russian literacy abilities. Children who could read Cyrillic script, who likely received some form of Russian literacy instruction, and who had a larger Russian lexicon size exhibited better reading performance. These results are discussed regarding the divergent attainment trajectories. Decisive factors for German were German lexicon size and RAN. Originality: This longitudinal study tracked Russian and German reading development over 3 years in primary school, assessed Russian reading in Cyrillic and Latin scripts, and identified predictors for bilingual reading development in both languages. We present an investigation into the effects of high-energy proton damage on charge trapping in germanium cross-strip detectors with the goal of accomplishing three important measurements. First, we calibrated and characterized the spectral resolution of a spare COSI-balloon detector in order to determine the effects of intrinsic trapping, finding that electron trapping due to impurities dominates over hole trapping in the undamaged detector. Second, we performed two rounds of proton irradiation of the detector in order to quantify, for the first time, the rate at which charge traps are produced by proton irradiation. We find that the product of the hole trap density and cross-sectional area, [ n σ ] h , follows a linear relationship with the proton fluence, F p , with a slope of (5.4 ± 0.4) × 10 - 11 cm / p + . Third, by utilizing our measurements of physical trapping parameters, we performed calibrations which corrected for the effects of trapping and mitigated degradation to the spectral resolution of the detector. This essay is written as an urgent call for collaboration. It is a provocation for academics in non-crisis environments to proactively reach out to our academic colleagues in Ukraine and other severely disrupted crisis environments around the world to work together to create and extend knowledge and understanding about interpersonal and organizational phenomena during extreme circumstances. We – those of us who are higher education practitioners living within non-crisis environments – have colleagues around the world who are navigating through fractured and uncertain contexts. They are calling out to be seen, to be heard, to be engaged and partnered with in generative and holistic ways. In this essay, we begin a conversation about potential topics for collaborative exploration in our learning and teaching spaces with the hope that they will inspire action and connection between academics living within relative peace and privilege and those living within severe disruption and crisis. Background: Many factors, including some related to the patient, implant selection, and the surgeon's skill and expertise, likely contribute to the risk of THA revision. However, surgeon factors have not been extensively analyzed in national joint replacement registries, and there is limited insight into their potential as a confounding variable for revision outcomes; for example, if surgeons with higher revision rates choose more successful prostheses, would this alone reduce their revision rate? Questions/purposes: This study used Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) data for patients receiving primary THA for a diagnosis of osteoarthritis to answer the following questions: (1) Will the difference in revision rates among surgeons change or disappear when only procedures performed with the best prostheses or prostheses that have been identified as having higher revision rates are considered? (2) Is the benefit associated with using the best-performing prostheses different among surgeons with different revision rates? (3) Do the reasons for revision differ between surgeons with higher rates of revision compared with surgeons with lower rates of revision? Methods: All primary THA procedures performed and recorded in the AOANJRR for osteoarthritis from September 1, 1999, to December 31, 2022, were considered for inclusion. Each THA prosthesis used was categorized per the AOANJRR as superior-performing, middle-performing, or identified as having a higher rate of revision by the AOANJRR benchmarking process. Surgeons who had performed at least 50 procedures and had a recorded 2-year cumulative percent revision (CPR) were included. After applying these restrictions, the study consisted of 302,066 procedures performed by 476 known surgeons. For the primary outcome measure of all-cause revision, we examined the variation in all-cause revision rates across individual surgeons when different classes of devices were used to assess whether differences between surgeons persisted when accounting for prosthesis selection. For the purposes of descriptively comparing reasons for revision between surgeons with higher-than-average or lower-than-average risk of revision, surgeons were classified into quartiles and outcomes compared when these surgeons used the same class of prosthesis. Results: The difference in rates of revision among surgeons remained even after accounting for the effects of the prosthesis used. For any given surgeon, identified prostheses were associated with higher revision rates compared with both superior-performing prostheses (HR 1.73 [95% CI 1.57 to 1.92]; p < 0.01) and medium-performing prostheses (HR 1.31 [95% CI 1.20 to 1.43]; p < 0.01). All surgeons demonstrated a lower revision rate when using a superior-performing prosthesis, but the difference was greatest for surgeons with the highest rates of revision. Surgeons with the lowest rates of revision had a 19-year CPR of 3.9% (95% CI 3.0% to 5.0%) when using a superior-performing prosthesis compared with 5.4% (95% CI 4.0% to 7.3%) for procedures in which an identified prosthesis was used. Surgeons with the highest rates of revision had a 19-year CPR of 10.9% (95% CI 8.6% to 13.8%) when using a superior-performing prosthesis, and this increased to 20.4% (95% CI 18.0% to 23.1%) for procedures in which an identified prosthesis was used. The reasons for revision differ between surgeons, with causes of revision likely preventable and not related to the prosthesis choice being apparent for surgeons with high revision rates. Conclusion: The choice of implant and the surgeon performing the index procedure both affected the risk of revision as well as the reasons for revision. Surgeons could improve the survivorship of the arthroplasties they perform by choosing implants identified by registries as having lower revision rates. Acceptance of the fact that surgeons have different revision rates is needed, and detailed analysis is required to explain why surgeons with high revision rates have increased rates of likely preventable revisions, and outside of prosthesis choice, how revision rates can be lowered. The influence of training, fellowship completion, ongoing education, patient selection, indications for surgery, and factors underlying prosthesis decision-making should be assessed. This study focuses on two central elements of news journalism: editorial convergence and the diversity of news content. The goal was to examine a potentially negative impact of a more centrally organized newsroom on structural and content diversity in news journalism. Therefore, we examined three German newspapers with different degrees of editorial convergence to assess their organizational development as well as their reporting's development regarding content diversity. Our results do not support a direct impact of editorial convergence on diversity. Background: Studies link multidrug-resistant organism (MDRO) rectal colonization to increased infection risk, data from Greece, a country with high rates of MDRO, are limited. Methods: We assessed bloodstream infection (BSI) risk following rectal colonization by MDROs across three Greek hospitals (2019–2022). Results: Of 4,370 inpatients, 31.1% were colonized by carbapenem-resistant Enterobacterales (CRE), 30.1% carbapenem-resistant Acinetobacter baumannii (CRAB), 5.8% carbapenem-resistant Pseudomonas aeruginosa (CRPA), 28.4% vancomycin-resistant enterococci (VRE). Subsequent BSI from the same MDRO species was developed in 15.6% of CRE, 19.7% of CRAB, 9.2% of CRPA and 3.5% of VRE carriers. Previous rectal colonization increases significantly MDRO BSI risk [RR (95%CI): CRE 5.2 (3.9–6.8), CRAB 2.7 (2.2–3.3), CRPA 9.6 (5.8–16.0), VRE 2.5 (1.5–4.2)]. This article focuses on the interconnections between online and offline activism in Palestine, with a detailed analysis of the case of Sheikh Jarrah (east Jerusalem). We examine current strategies of Palestinian digital resistance that have highlighted the relevance of interlinking struggles both at the grassroots level and on the internet. Within the settler colonial framework, the hashtag #SaveSheikhJarrah stands as one of the most emblematic contemporary struggles against the Israeli military occupation, particularly against the ongoing evictions taking place in areas such as Sheikh Jarrah in east Jerusalem and the West Bank. In recent years, this hashtag has drawn unprecedented global attention to the Palestinian cause. This article is based on extensive digital ethnography and semi-structured interviews with young Palestinian activists conducted from 2021 to 2023. Through an exploration of activists' narratives surrounding the #SaveSheikhJarrah movement, we critically examine the interplay of diverse forms of resistance in the age of social media. Our lens of analysis links grassroots and online struggles, offering a fresh perspective on digital activism in the current discourse on culture and communication within Palestinian studies. Much guidance on how to identify and contact a doctoral supervisor can be found on YouTube. There is a wealth of advice videos presented by 'insiders' including students, academics, consultants and institutional representatives. This article explores such 'find a supervisor' videos, characterising them as texts in the broader genre of doctoral writing advice. The article examines a sample of these videos thematically and then discursively, offering insight into the advice they give, as well as their positionality and rhetorical constructions of authority. Although potentially helpful to applicants, particularly those without strong networks, these videos nonetheless contribute to a complex advice market which requires critical scrutiny in terms of motivation and message. The article argues that, although supervisor advice videos may provide accessible support, they also capitalise on doctoral anxiety and perpetuate a culture of compliance with higher education norms, rather than encourage institutional and cultural transformation towards inclusivity. Objective: To assess if off‐label oral solution of misoprostol compared with licensed oral tablet of misoprostol approved for induction of labour (IOL) is as efficient in resulting in vaginal delivery within 24 h, using a non‐inferiority design. Design: Prospective, randomised, non‐inferiority, open‐label, blinded endpoint trial. Setting: Two tertiary level hospitals, Stockholm, Sweden, January 2022 to May 2023. Population: In all, 874 women, without previous caesarean section, with an unripe cervix and a singleton, cephalic foetus at 37 + 0 to 42 + 0 gestational weeks, with a normal cardiotocography, planned for IOL were included. Methods: Women were randomised 1:1 to intervention (25 μg oral solution of misoprostol) or control (25 μg oral tablet of misoprostol) two‐hourly for a maximum of eight doses. Subsequent methods of induction followed clinical practice. Main Outcome Measures: The primary outcome was vaginal delivery within 24 h tested using non‐inferiority testing procedures at a non‐inferiority margin of 5 percentage points. Secondary efficacy outcomes were tested for superiority of either treatment. Analyses were by intention‐to‐treat. Results: There were 207 (47.4%) vaginal deliveries within 24 h for women receiving oral solution and 192 (43.9%) vaginal deliveries within 24 h for women receiving oral tablet, establishing non‐inferiority with an absolute risk difference of 3.4% (95% CI −3.2% to 10.0%). Women receiving oral solution required fewer doses to reach active labour than women receiving oral tablet (5.7 vs. 6.1, p = 0.007). There were no significant differences for other secondary or safety outcomes. Conclusions: Off‐label oral solution of misoprostol was non‐inferior to the licensed oral tablet regarding efficacy of IOL defined as vaginal delivery within 24 h. Objectives. To evaluate the association between distance from closest abortion facility and number of fulfilled requests through no-test telehealth medication abortion (NTMA) asynchronous service. Methods. Using deidentified 2020–2022 electronic medical record data from Aid Access users in US states where NTMA is prescribed by US-based clinicians, we describe individual user demographics and their resident county characteristics. We conducted a county-level geospatial analysis of distance to abortion facility (Myers Abortion Facility Database) on fulfilled requests using Poisson regression. Results. US-based clinicians fulfilled NTMA requests to 8411 individuals in 21 states and the District of Columbia. Each 100-mile increase in distance to an abortion facility increased per-capita NTMA by 61% (95% confidence interval [CI] = 26%, 86%). Most individuals were aged 20 to 29 years (54%), had no living children (57%), were less than 6 weeks' gestation (62%), and lived in urban areas (65%). Almost half (49%) lived in higher socially vulnerable counties compared with 17% in less socially vulnerable counties. Conclusions. In the United States, telehealth medication abortion is a critically important service for individuals who are young, socially vulnerable, and living in counties far from abortion care facilities. Cytomegalovirus (CMV) infection or reactivation in immune-compromised individuals can lead to a wide range of severe complications including hepatitis. However, its relation with immune checkpoint inhibitors (ICIs) induced hepatitis (ICI-hepatitis) and tumor responses in advanced melanoma patients remains unclear. Hundred and ninety metastatic cutaneous melanoma patients (mCM) who received ICI treatment, with CMV IgG or IgM information available at baseline, were included in the study (Cohort 1). Clinical characteristics and immune cell count in the blood were retrieved from medical records. In addition, anti-CMV IgG and IgM were measured in pre and on-treatment serum samples from 49 advanced skin cancer patients using ELISA (Cohort 2). In the event of a positive anti-CMV IgM, further analysis with PCR was performed. Univariate and multivariate analysis was used to assess the relationship between CMV IgG or IgM and ICI-hepatitis tumor outcomes. Twenty-one patients (11%) developed hepatitis during ICI treatment (Cohort 1). ICI-hepatitis was significantly associated with disease control rate (DCR; p = 0.017) and longer progression-free survival (PFS; p = 0.008) in mCM patients. Detection of CMV IgG or IgM antibodies were not associated with ICI-hepatitis (p > 0.05). However, increased CMV IgG values at baseline correlated with disease progression (p = 0.047) and shorter PFS (p = 0.081). In addition, increased CMV IgG values were associated with reduced levels of monocytes (p = 0.005), eosinophils (p = 0.062), and neutrophils (p = 0.065) in the blood. In summary, anti-CMV IgG or IgM in the blood may not be associated with ICI-hepatitis, but high anti-CMV IgG at baseline indicates poor outcomes in ICI-treated mCM patients. [ABSTRACT FROM AUTHOR]

Background: Companion animals, like household dogs, are an overlooked transmission point for zoonotic pathogens such as nontyphoidal Salmonella (NTS). Given the proximity of dogs to humans and the use of critically important antibiotics in companion animal medicine, household dogs represent a risk for the spread of antimicrobial‐resistant (AMR) Salmonella. Methods and Results: To this end, we aimed to leverage existing biosurveillance infrastructure to investigate AMR and the zoonotic potential of NTS isolated from dogs and humans. We identified all NTS strains isolated from domestic dogs via the Veterinary Laboratory Investigation and Response Network between May 2017 and March 2023 (N = 87), and spatiotemporally matched strains isolated from humans in the NCBI Pathogen Isolate Browser (N = 77). These 164 strains, collected from 17 states in the United States, formed the basis of our analysis. Strains isolated from dogs comprised diverse serovars, with most being clinically relevant to human health. All strains possessed AMR determinants for drug classes deemed critically or highly important by the World Health Organization. We identified sixteen NTS isolates from humans closely related to ≥1 of six dog‐associated strains. Conclusions: Collectively, our data emphasize the importance of antimicrobial stewardship and sustained biosurveillance beyond human‐ and agriculture‐associated veterinary medicine, using a One‐Health framework that accounts for all transmission points including companion animals. Background: COVID-19 caused a profound global impact, resulting in significant cases and deaths. The progression of COVID-19 clinical manifestations is influenced by a dysregulated inflammatory response. Early identification of the subclinical progression is crucial for timely intervention and improved patient outcomes. While there are various biomarkers to predict disease severity and outcomes, their accessibility and affordability pose challenges in resource-limited settings. We explored the potentiality of the neutrophil-to-lymphocyte ratio (NLR) as a cost-effective inflammatory marker to predict disease severity, clinical deterioration, and mortality in affected patients. Methodology: A hospital-based retrospective cohort study was conducted at KCMC Hospital among COVID-19 patients followed from admission to discharge between 1st March 2020 and 31st March 2022. NLR was calculated as the absolute neutrophil count in μL divided by the absolute lymphocyte count in μL. The NLR cut-off value was determined using Receiver Operating Characteristic (ROC) analysis and assessed its predictive ability at admission for in-hospital mortality. The Chi-square test compared the proportion of NLR by patient characteristics. The association of NLR with disease severity and mortality was analyzed using the modified Poisson and Cox regression models, respectively. Results: The study included 504 patients, with a median age of 64 years, 57.1% were males, and 68.3% had severe COVID-19. The in-hospital COVID-19 mortality rate was 37.7%. An NLR cutoff value of 6.1 or higher had a sensitivity of 92.1% (95% CI 89.2%–94.0%) and a specificity of 92.0% (95% CI 89.7%–94.4%). Additionally, 39.5% of patients with an NLR value of 6.1 or higher had increased risk of severe disease, subsequent clinical deterioration, and mortality. Conclusion and recommendation: An NLR value of 6.1 or higher at the time of hospital admission associated with severe disease, clinical deterioration, and mortality in patients with COVID-19. Integration of NLR as a prognostic parameter in COVID-19 prognosis scales could improve risk assessment and guide appropriate management strategies for COVID-19 patients, as well as for potential future viral-related pneumonias. Human cytomegalovirus (HCMV) encodes four viral Fc-gamma receptors (vFcγRs) that counteract antibody-mediated activation in vitro, but their role in infection and pathogenesis is unknown. To examine their in vivo function in an animal model evolutionarily closely related to humans, we identified and characterized Rh05, Rh152/151 and Rh173 as the complete set of vFcγRs encoded by rhesus CMV (RhCMV). Each one of these proteins displays functional similarities to their prospective HCMV orthologs with respect to antagonizing host FcγR activation in vitro. When RhCMV-naïve male rhesus macaques were infected with vFcγR-deleted RhCMV, peak plasma DNAemia levels and anti-RhCMV antibody responses were comparable to wildtype infections of both male and female animals. However, the duration of plasma DNAemia was significantly shortened in immunocompetent, but not in CD4 + T cell-depleted animals. Since vFcγRs were not required for superinfection of rhesus macaques, we conclude that these proteins can prolong lytic replication during primary infection by evading virus-specific adaptive immune responses, particularly antibodies. The role of viral Fc-gamma receptors in rhesus cytomegalovirus (RhCMV) infection is unclear. Platelets are enriched in miRNAs and harbor Ago2 as the principal RNA silencing Argonaute. However, roles in thrombopoiesis and platelet function remain poorly understood. We generated megakaryocyte/platelet-specific Ago2-deleted (Ago2 KO) mice and assessed proteomic and functional effects. We predicted platelet hyperreactivity with Ago2 deletion due to large-scale upregulated protein expression. Platelet counts were normal. Mean volumes were increased, associated with larger, though fewer megakaryocytes. Ago2-deleted platelets from male mice showed hyperreactivity to thromboxane but not to other agonists compared to controls, whereas Ago2-deleted platelets from female mice showed normal reactivity. Ago2 KO mice displayed normal hemostasis and clot dynamics. Proteomes of Ago2-deleted and wild type platelets were mostly similar. However, Ago1 - undetectable in wild type platelets - was upregulated in Ago2-deleted platelets in both males and females, confirmed by immunoblotting. Female Ago2-deleted platelets selectively showed downregulation of a protein cohort established in breast cancer cells to be transcriptionally regulated by estrogen receptor-beta coupled to Ago2, whereas male Ago2-deleted platelets did not. Thus, Ago2 is important for platelet development and function, putatively partially rescued by upregulation of Ago1. Introduction: The regulation of Cd4 expression during T-cell development and immune responses is essential for proper lineage commitment and function in the periphery. However, the mechanisms of genetic and epigenetic regulation are complex, and their interplay not entirely understood. Previously, we demonstrated the need for CD4 upregulation during positive selection to ensure faithful commitment of MHC-II-restricted T cells to the CD4 lineage. In this study, we investigate whether a conserved region, here called NCE, that is proximal to the Cd4 silencer and contains E4m has the required developmental-stage-specific canonical enhancer function and TCR responsiveness to mediate the CD4 upregulation required to prevent lineage errors. Methods: To investigate the role of NCE, transient transfection of reporter plasmids was performed in thymoma cell lines arrested at the double-positive (DP, CD4+CD8+) and intermediate (INT, CD4+CD8lo) stages of development. CRISPR/Cas9-mediated deletion of the coreNCE/E4m region was carried out in these cell lines to assess its impact on CD4 surface expression, re-expression rates, and TCR signaling responsiveness. To avoid developmental alterations from direct manipulation of the endogenous Cd4 locus in vivo , BAC-transgenic reporter mice were generated with the locus modified to express EGFP in the presence or absence of NCE. EGFP mRNA levels were measured via RT-qPCR, and EGFP fluorescence was analyzed in post-selection thymocytes. Results: Our in vitro experiments demonstrate that NCE by itself can function as an enhancer at the INT, but not the DP stage of development. Furthermore, CRISPR/Cas9-mediated deletion of coreNCE/E4m resulted in reduced CD4 surface levels, slower re-expression rates, and reduced TCR signaling responsiveness in INT cells, but not in DP cells. In vivo , NCE-sufficient transgenic mice exhibited upregulation of Cd4 reporter EGFP mRNA levels at the INT stage and a corresponding upregulation of EGFP fluorescence, whereas NCE-deficient mice showed a significant loss of Cd4 reporter EGFP mRNA and no detectable EGFP production in any post-selection thymocytes. Discussion: This study demonstrates that the canonical enhancer function of coreNCE/E4m is essential for CD4 upregulation following positive selection. Early investigation revealed a reduced risk of SARS-CoV-2 infection among social contacts of COVID-19 vaccinated individuals, referred to as indirect protection. However, indirect protection from SARS-CoV-2 infection-acquired immunity and its comparative strength and durability to vaccine-derived indirect protection in the current epidemiologic context of high levels of vaccination, prior infection, and novel variants are not well characterized. Here, we show that both vaccine-derived and infection-acquired immunity independently yield indirect protection to close social contacts with key differences in their strength and waning. Analyzing anonymized SARS-CoV-2 surveillance data from 9,625 residents in California state prisons from December 2021 to December 2022, we find that vaccine-derived indirect protection against Omicron SARS-CoV-2 infection is strongest within three months of COVID-19 vaccination [30% (95% confidence interval: 20–38%)] with subsequent modest protection. Infection-acquired immunity provides 38% (24–50%) indirect protection for 6 months after SARS-CoV-2 infection, with moderate indirect protection persisting for over one year. Variant-targeted vaccines (bivalent formulation including Omicron subvariants BA.4/BA.5) confer strong indirect protection for at least three months [40% (3–63%)]. These results demonstrate that both vaccine-derived and infection-acquired immunity can reduce SARS-CoV-2 transmission which is important for understanding long-term transmission dynamics and can guide public health intervention, especially in high-risk environments such as prisons. Inflammation-associated perturbations of the gut microbiome are well characterized, but poorly understood. Here, we demonstrate that disparate taxa recapitulate the metabolism of the oxidized sugars glucarate and galactarate, utilizing enzymatically divergent, yet functionally equivalent, gud/gar pathways. The divergent pathway in commensals includes a putative 5-KDG aldolase (GudL) and an uncharacterized ABC transporter (GarABC) that recapitulate the function of their non-homologous counterparts in pathogens. A systematic bioinformatic search for the gud/gar pathway in gut microbes identified 887 species putatively capable of metabolizing oxidized sugars. Previous studies showed that inflammation-derived nitrate, formed by nitric oxide reacting with superoxide, promotes pathogen growth. Our findings reveal a parallel phenomenon: oxidized sugars, also produced from reactions with nitric oxide, serve as alternative carbon sources for commensal microbes. Previously considered a pathogen virulence factor, oxidized sugar metabolism is also present in specific commensals and may contribute to their increased relative abundance in gastrointestinal inflammation. Metabolism of oxidized sugars during inflammation has been implicated as a pathogen colonization factor. To undertake a mixed-methodology implementation study to improve the well-being of men with gastrointestinal late effects following radical radiotherapy for prostate cancer. All men completed a validated screening tool for late bowel effects (ALERT-B) and the Gastrointestinal Symptom Rating Score (GSRS); men with a positive score on ALERT-B were offered management following a peer reviewed algorithm for pelvic radiation disease (PRD). Health-related quality of life (HRQoL) at baseline, 6 and 12 months; and healthcare resource usage (HRU) and patient, support-giver, staff experience and acceptability of staff training (qualitative analysis) were assessed. Two nurse- and one doctor-led gastroenterology services were set up in three UK cancer centres. Men (n = 339) who had had radical radiotherapy for prostate cancer at least 6 months previously, were recruited; of which 91/339 were eligible to participate; 58/91 men (63.7%) accepted the referral. Diagnoses included: radiation proctopathy (n = 18); bile acid malabsorption (n = 15); fructose or lactose intolerance and/or small intestinal bacterial overgrowth (n = 20); vitamin B12/D deficiency (n = 20). Increases in quality of life, sexual activity and/or sexual function, and decrease in specific symptoms (e.g. bowel-related or urinary) between 6 and 12 months were observed. Limited HRU modelling suggested staff costs were £117-£185, depending on the service model; total costs averaged £2,243 per patient. Both staff and patients welcomed the new service although there was concern about long-term funding and sustainability beyond the timeframe of the study (qualitative). PRD is increasingly recognised worldwide as an ongoing consequence of curative pelvic radiotherapy, despite widespread implementation of advanced radiotherapy techniques. Instead of turning to emergency phone systems, social media platforms, such as Twitter, have emerged as alternative and sometimes preferred venues for members of the public in the US to communicate during hurricanes and other natural disasters. However, relevant posts are likely to be missed by responders given the volume of content on platforms. Previous work successfully identified relevant posts through machine-learned methods, but depended on human annotators. Our study indicates that a GPU-accelerated version of BERTopic, a transformer-based topic model, can be used without human training to successfully discern topics during multiple hurricanes. We use 1.7 million tweets from four US hurricanes over seven years and categorize identified topics as temporal constructs. Some of the more prominent topics related to disaster relief, user concerns, and weather conditions. Disaster managers can use our model, data, and constructs to be aware of the types of themes social media users are producing and consuming during hurricanes. [ABSTRACT FROM AUTHOR]
