In jawed vertebrates, genes of the major histocompatibility complex (MHC) play a key role in immunity by encoding cell-surface proteins that recognize and bind non-self antigens. High variability at MHC suggests that these loci may also function in social signalling such as mate choice and kin recognition. This requires that MHC genotype covaries with some perceptible phenotypic trait. In mammals and fish, MHC is signalled chemically through volatile and non-volatile peptide odour cues, facilitating MHC-dependent mate choice and other behaviours. In birds, despite evidence for MHC-dependent mating, candidate mechanisms for MHC signalling remain largely unexplored. However, feather preen wax has recently been implicated as a potential source of odour cues. We examined whether the chemical composition of preen wax correlates with MHC class IIb genotypes of wild song sparrows (Melospiza melodia). Pairwise chemical distance reflected amino acid distance at MHC for male-female dyads, although not for same-sex dyads. Chemical diversity did not reflect MHC diversity. We used gas chromatography-mass spectrometry (GC-MS) to characterize preen wax compounds, and identified four wax esters that best reflect MHC similarity. Multiphase OH and O3 oxidation reactions with atmospheric organic aerosol (OA) can influence particle physicochemical properties including composition, morphology, and lifetime. Chemical aging of initially insoluble or low-soluble single-component OA by OH and O3 can increase their water solubility and hygroscopicity, making them more active as cloud condensation nuclei (CCN) and susceptible to wet deposition. However, an outstanding problem is whether the effects of chemical aging on their CCN activity are preserved when mixed with other organic or inorganic compounds exhibiting greater water solubility. In this work, the CCN activity of laboratory-generated biomass burning aerosol (BBA) surrogate particles exposed to OH and O3 is evaluated by determining the hygroscopicity parameter, κ, as a function of particle type, mixing state, and OH and O3 exposure applying a CCN counter (CCNc) coupled to an aerosol flow reactor (AFR). Levoglucosan (LEV), 4-methyl-5-nitrocatechol (MNC), and potassium sulfate (KS) serve as representative BBA compounds that exhibit different hygroscopicity, water solubility, chemical functionalities, and reactivity with OH radicals, and thus exemplify the complexity of mixed inorganic/organic aerosol in the atmosphere. The CCN activities of all of the particles were unaffected by O3 exposure. Following exposure to OH, κ of MNC was enhanced by an order of magnitude, from 0.009 to ~ 0.1, indicating that chemically aged MNC particles are better CCN and more prone to wet deposition than pure MNC particles. No significant enhancement in κ was observed for pure LEV particles following OH exposure. κ of the internally mixed particles was not affected by OH oxidation. Objective: To introduce a series of papers discussing previously undocumented tobacco industry activities regarding strategies to avoid products liability litigation, understand nicotine addiction, and manipulate both internal and external scientific research on the effects of both active and passive smoking.Data Sources: Documents from Brown and Williamson Tobacco Corporation (B&W), the British American Tobacco Company (BAT), and other tobacco interests provided by an anonymous source, obtained from Congress, and received from the private papers of a former BAT officer.Study Selection: All available materials, including confidential reports regarding research and internal memoranda exchanged between tobacco industry lawyers.Conclusions: These documents provide our first look at the inner workings of the tobacco industry during the crucial period in which the scientific case that smoking is addictive and kills smokers solidified. The documents show a sophisticated legal and public relations strategy to avoid liability for the diseases induced by tobacco use. The documents show that lawyers steered scientists away from particular research avenues, which is inconsistent with the company's purported disbelief in the causation and addiction claims; if the company had been genuinely unconvinced by the causation and addiction hypotheses, then it should have had no concern that new research would provide ammunition for the enemy. Quite the contrary, the documents show that B&W and BAT recognized more than 30 years ago that nicotine is addictive and that tobacco smoke is "biologically active" (eg, carcinogenic). Background Optimal vascular function is vital for prevention of dementia. We hypothesized that elderly post-stroke survivors who preserve cognitive function show unperturbed cerebral microvasculature compared with those who develop dementia. Methods Using stereological spherical probe software, we compared the length density ( Lv, cumulative vessel length per unit tissue volume) of hippocampal microvessels in post mortem brain tissue from post-stroke survivors, Alzheimer's disease ( AD), vascular dementia ( VaD) and normal ageing control subjects. We also assessed microvessel diameters in the same subjects. Microvessels were identified by markers of endothelial cells (glucose transporter 1; GLUT1), basement membrane (collagen IV; COL4) and smooth muscle cell α-actin ( SMA). Results We found increased Lv of both GLUT1 and COL4 immunostained microvessels ( P < 0.05) in the hippocampal CA1 region of post-stroke demented ( PSD) and AD cases compared with post-stroke nondemented ( PSND), control and VaD subjects. However, no changes were apparent in the CA2 region. We also noted significant increase in Lv in the entorhinal cortex of AD compared with PSND and PSD subjects. The mean diameter of microvessels was decreased in PSD, compared with PSND, as well as in AD and VaD compared with controls. Cumulative frequency analysis showed PSND subjects to have significantly greater proportion of microvessels with diameters, ranging from 7 to 12 μm. Conclusions An increase in microvascular Lv in AD and PSD suggests either an increase in angiogenesis or the formation of newer microvessel loops in response to cerebral hypoperfusion. A promising, new, in vivo prostate dosimetry system has been developed for clinical radiation therapy. This work outlines the preliminary end-to-end testing of the accuracy and precision of the new OARtrac scintillation dosimetry system. We tested 94 calibrated plastic scintillation detector (PSD) probes before their final integration into endorectal balloon assemblies. These probes had been calibrated at The University of Texas MD Anderson Cancer Center Dosimetry Laboratory. We used a complete clinical OARtrac system including the PSD probes, charge coupled device camera monitoring system, and the manufacturer's integrated software package. The PSD probes were irradiated at 6 MV in a Solid Water��phantom. Irradiations were performed with a 6 MV linear accelerator using anterior–posterior/posterior–anterior matched fields to a maximum dose of 200 cGy in a 100�cm source-axis distance geometry. As a whole, the OARtrac system has good accuracy with a mean error of 0.01% and an error spread of �5.4% at the 95% confidence interval. These results reflect the PSD probes’ accuracy before their final insertion into endorectal balloons. Future work will test the dosimetric effects of mounting the PSD probes within the endorectal balloon assemblies. [ABSTRACT FROM AUTHOR]

Objectives: To explore the patient experience, and the role of ophthalmologists and other health and social care professionals in the certification and registration processes and examine the main barriers to the timely certification of patients. Design: Qualitative study. Setting: Telephone interviews with health and social care professionals and patients in three areas in England. Participants: 43 health and social care professionals who are part of the certification or registration process. 46 patients certified as severely sight impaired (blind) or sight impaired (partially sighted) within the previous 12 months. Results: Certification and registration are life changing for patients and the help they receive can substantially improve their lives. Despite this, ophthalmologists often found it difficult to ascertain when it is appropriate to certify patients, particularly for people with long-term conditions. Ophthalmologists varied in their comprehension of the certification process and many regarded certification as the 'final stage' in treatment. Administrative procedures meant the process of certification and registration could vary from a few weeks to many months. The avoidable delays in completing certification can be helped by Eye Clinic Liaison Officers (ECLO). Conclusions: A better understanding of the certification and registration processes can help improve standards of support and service provision for people who are severely sighted impaired or sight impaired. Better education and support are required for ophthalmologists in recognising the importance of timely referral for rehabilitative support through certification and registration. A decontamination trial on the effectiveness of hot water or acidified sodium chlorite (SANOVA) treatment on Salmonella spp., Escherichia coli and Total Viable Count (TVC) was undertaken on pork carcases prior to primary chilling in two large pork abattoirs in Australia using belly-strip excision sampling. A total of 123 samples from Abattoir A and 400 samples from Abattoir B were cultured and analysed. Test pigs were selected from herds with a known high level of on-farm Salmonella infection. At Abattoir A, Salmonella spp. were not isolated from carcases. The prevalence of E. coli on control carcases was 92.9% compared with 9.8% for hot water and 12.5% for SANOVA treated carcases. The mean log E. coli concentration for control carcases was 0.89 cfu/gram, compared with −0.83 cfu/gram from hot water and −0.75 cfu/gram from SANOVA treated carcases. The mean log TVC for control carcases was 4.06 compared with 1.81 cfu/gram for hot water and 2.76 cfu/gram for SANOVA treated carcases. At Abattoir B, the prevalence of Salmonella on control carcases was 16% compared with 2.7% for hot water and 7.0% for SANOVA treated carcases. The prevalence of E. coli on control carcases was 69.3% compared with 22% for hot water and 30% for SANOVA treated carcases. The mean log E. coli concentration for control carcases was 0.45 cfu/gram, compared with −0.65 cfu/gram from hot water and −0.60 cfu/gram from SANOVA treated carcases. The mean log TVC for control carcases was 3.00 cfu/gram compared with 2.10 cfu/gram for hot water and 2.53 cfu/gram for SANOVA treated carcases. The reductions in prevalence and mean log concentrations in the present trial were all found to be statistically significant and indicate that carcases decontamination with either hot water or SANOVA are effective risk management options immediately available to the pork industry. This exploratory, multicenter, open-label study evaluated the efficacy and safety of FTY720, as a part of an immunosuppressive regimen, in combination with everolimus and steroids in de novo renal transplant recipients at increased risk of delayed graft function (DGF). Patients received FTY720 (5 mg) and everolimus (4 mg) 2–12 h pre-transplantation, followed by 2.5 mg/d FTY720 and concentration-controlled everolimus (4–8 ng/mL) post-transplant for 12 months. Induction therapy was prohibited. After enrollment of 56 of the planned 200 patients between 2000 and 2002, the recruitment was terminated. The primary endpoint, rate of graft loss, or death at three months was 15.4% and the biopsy-confirmed acute rejection was 42.3%. Death or graft loss at 12 months in the DGF and non-DGF arms was 36.0% and 25.9%, respectively. The mean estimated creatinine clearance at three months was 63 and 55 mL/min in the non-DGF and DGF groups, respectively, while at 12 months it was 56 mL/min in both the groups. Although there was no comparator arm, the results from this exploratory study (compared with data from other phases II and III trials) indicated no apparent benefits of FTY720-based regimens for prevention of acute rejection and preservation of renal function in renal transplant recipients at high risk of DGF. In the context of UK policy to promote employment for people with disability as a means to greater social inclusion, this study investigated how people with severe mental health problems fare in existing supported employment agencies. The aim of the study was to identify factors associated with successful placement in work and to test the impact of working on psychological well-being in this group. One hundred and fifty-five users of six English agencies were followed up for 1 year (2005-2006). Information was collected about their employment status, job-seeking behaviour, perceived obstacles to work, self-esteem and hope, and the employment support received. Eighty-two per cent of those working at baseline were still in work a year later. The support agencies helped 25% of unemployed clients into work, a statistically significant increase in the proportion of clients in employment. Gaining employment was associated with improvements in financial satisfaction and self-esteem. There was a trend towards working half time. People who had been out of work longer were less likely to secure employment. No significant associations were found between getting a job and personal characteristics, the quantity of employment support given, nor the recipient's rating of the support offered. The odds of moving into work were nearly four times higher for those people who visited a job centre prior to the start of the study. Clients of specialist agencies rated their provision more highly than clients of pan-disability agencies. These results demonstrate the benefits of working for this group and support the development of employment services with an individualised, rapid placement approach, linked to job centre advice and expert mental health service input. Aims: Carotid sinus hypersensitivity (CSH) is an ageing-related autonomic disorder, rarely occurring before the age of 50 years but increasing in incidence thereafter. Clinical symptoms of CSH include falls and dizziness, thought to be precipitated by dysfunctional baroreflex responses. CSH is highly prevalent in Alzheimer's disease (AD), Parkinson's disease (PD) and dementia with Lewy bodies (DLB); diseases that are associated with variable degeneration of medullary autonomic nuclei which regulate baroreflex responses. Currently, there are no descriptions of the integrity of medullary autonomic nuclei in CSH. We hypothesized medullary autonomic degeneration is found in elderly patients with CSH. Methods: Using in vitro digital imaging, we quantified the burden of tau, amyloid β and α-synuclein in autonomic nuclei of 12 patients prospectively assessed with CSH (age 83 years) compared with 14 (80 years) control subjects. Results: We found increased tau ( P < 0.000) accumulation in baroreflex associated nuclei, but not the hypoglossal or raphe in the CSH patients. Medullary tau accumulation was not related to the development of AD in the CSH patients. Tau was colocalized to catecholaminergic neurones and occurred in the absence of neuronal loss. We found no difference in α-synuclein, amyloid β or microglial numbers between the CSH cases and controls. Conclusions: We suggest that hyperphosphorylated tau accumulation particularly in tyrosine hydroxylase containing neurones may impair central regulation of baroreflex responses in patients with CSH. Many scaphoid fractures can be treated with percutaneous screw insertion, but fracture displacement usually necessitates open reduction. Two surgeons treated 20 consecutive patients with displaced fractures of the scaphoid using arthroscopic-assisted percutaneous screw fixation. Thirteen patients had dorsal (antegrade) and seven had palmar (retrograde) percutaneous screw insertion. At an average follow-up of 18 (range 6–48) months, all of the fractures were healed and there were no implant problems. The early results of arthroscopic-assisted percutaneous screw fixation of displaced fractures of the scaphoid suggest that union can be obtained and good to excellent function achieved predictably without the need for open exposure. Avoidance of an open exposure limits wrist ligament injury and may preserve blood supply. Further evaluation of this procedure is merited. [ABSTRACT FROM PUBLISHER]

Background: Despite a lack of data regarding their efficacy, both caffeine and doxapram have been recommended for treatment of hypercapnia in equine neonates with central nervous system damage. Hypothesis: Caffeine and doxapram alleviate hypercapnia in foals with hypoxic-ischemic encephalopathy. Animals: Sixteen foals treated with either caffeine (n = 8) or doxapram (n = 8). Methods: Information on age, body temperature, heart rate, respiratory rate, arterial blood gas parameters, duration of therapy, and outcome was abstracted from each medical record. Results: Therapy with doxapram resulted in a significant decrease in partial pressure of carbon dioxide (PaCO2 [ P= .004]), bicarbonate concentration ( P= .002), and base excess ( P= .005) compared with baseline values but failed to correct acidemia. In contrast, administration of caffeine did not result in significant changes from baseline values. The percentage decrease in PaCO2 and bicarbonate concentration was significantly greater in foals treated with doxapram than in foals treated with caffeine ( P= .004). The proportions of foals that achieved the targeted PaCO2 (≤ 50 mmHg) were significantly higher in foals treated with doxapram than in foals treated with caffeine ( P= .029). The proportion of survivors in the 2 treatment groups was not significantly different. The article offers guidelines for the prevention of lower extremity arterial ulcers in patients with lower extremity peripheral arterial occlusive disease (PAOD). It is stated that an early diagnosis is paramount to preventing ulcers in patients with undiagnosed PAOD. Signs of PAOD develop at the leg, ankle, or heel level. Furthermore it is stated that the use of lipid-lowering therapies decreases long-term ischemic arterial ulcer development.

Research into mental health and employment has focused largely on people who are unemployed. This paper reports the experiences of 20 clients of employment support agencies who had succeeded in returning to work. A number of barriers to getting back to work were identified, but receiving employment support could enable people to overcome them. There was consistency with previous studies of factors associated with high and low levels of job satisfaction. Even those participants who were less satisfied with their jobs identified benefits and none described any negative effects. The quality of the employment support provided was important, including advice and counselling during the job search, enabling informed choice about disclosure and support in work. Job retention targets are required for funding programmes in addition to placement targets. Further research into the timing and processes of disclosure and into occupational health screening processes would be helpful. [ABSTRACT FROM AUTHOR]

Aims: This study evaluates real world employment support for people with severe mental health problems in the UK. Given a policy context which promotes social inclusion and welfare to work, we wanted to find out about typical employment services and their effects on people with mental health problems. Method: A case study design was adopted, incorporating qualitative and quantitative interviews with staff and users of each service. After screening for severity, 182 individuals met our inclusion criteria for current use of the services. They were interviewed about their current engagement with support services, their employment and job satisfaction if employed. Measures of self esteem, hope/optimism and job satisfaction were taken as outcomes. Results: We report here associations between the outcome measures and: current working status; support from the agency; and demographic features at baseline. People who were working had higher outcome scores than those who were in work preparation or training. People working with support had higher job satisfaction. Conclusions: The study highlights the methodological issues raised by doing real world research. These findings were generated by heterogeneous interventions in non-experimental settings. Despite this, they tend to support previous evidence in favour of early placement and ongoing support in work. The article presents the guidelines in treating patients with arterial insufficiency ulcers of the lower extremities. The developers of the guidelines include academicians, private practice physicians, nurse clinicians and research nurses. The guidelines have been developed in seven categories including diagnosis, surgery, wound bed preparation, dressings, adjuvant therapy and long-term maintenance.

Fracture repair has previously been shown to be sensitive to mechanical environment, yet the specific relationship between strain character, magnitude and frequency, as well as other mechanical parameters, and tissue formation is not well understood. This study aimed to correlate strain distribution within the healing fracture gap with patterns of tissue formation using a rat model of a healing osteotomy subject to mechanical stimulation in bending. Finite element models based on realistic tissue distributions were used to estimate both the magnitude and spatial distribution of strains within the fracture gap. The spatial distribution of regenerating tissue was determined by microcomputed tomography and histology, and was confirmed using reverse transcription-polymerase chain reaction (RT-PCR). Results suggest that tensile strains suppress chondrogenesis during the mechanical stimulation period. After stimulation ends, however, tensile strains increased chondrogenesis followed by rapid bone formation. In contrast, in compressive environments, bone is formed primarily via intramembranous ossification. Taken together, these results suggest that intermittent tensile strains during fracture repair stimulate endochondral ossification and promote eventual bone healing compared to intermittent compressive strains and unstimulated fractures. Further understanding of these relationships may allow proposal of optimal therapeutic strategies for improvement of the fracture repair process. [Copyright &y& Elsevier]

They are unrelated to specific disorders, and their impact on cognitive and other brain functions is poorly characterized. Pathological studies often omit systematic survey of WML because of the need to study multiple full coronal tissue blocks, and uncertainty over the significance of lesions identified in periventricular and deep subcortical regions. Post-mortem MRI provides a means of mapping WML but the sensitivity and specificity of the method are unresolved. In this study post-mortem MRI of WML in fixed brain slices was compared with pathology in 33 brains donated to the Medical Research Council Cognitive Function and Ageing Study (MRC CFAS). This study shows that MRI detection of WML was less sensitive than pathology: periventricaular lesions (PVL) sensitivity = 95% (87–99%), specificity = 71% (44–90%); deep subcortical lesions (DSCL) sensitivity = 86% (79–93%), specificity = 80% (72–88%). False negative MRI was associated with milder pathology, but lesions detected by myelin attenuation alone showed both microglial and endothelial activation. Therefore post-mortem MRI of formalin-fixed brain slices is a reliable method to obtain systematic data on the severity and distribution of cerebral white matter disease, and appears to detect those WML most likely to have clinical impact. Following an initial bout of damaging exercise, a successive bout of similar exercise typically results in less injury, known as the "protective effect". Unloading due to spinal cord injury (SCI) increases the susceptibility to contraction-induced muscle injury. We tested the hypothesis that two bouts of isometric actions would evoke the same damage in the quadriceps femoris (QF) of patients with SCI. Six male subjects [32 (5) years old, 182 (9) cm, 81 (21) kg, injury level C6-T7, 6 (2) years post-injury, mean (SD)] were tested at two time points (Time1, Time2), separated by 8 weeks. Magnetic resonance images were taken of the QF prior to, immediately after, and 3 days after electromyostimulation (EMS) that evoked isometric knee extension. EMS (50 Hz) consisted of five sets of ten contractions (2 s on/6 s off, 1 min b/t sets) followed by three sets of ten contractions (1 s on/1 s off, 30 s b/t sets). Relative cross-sectional area of stimulated and injured skeletal muscle was obtained by quantifying pixels with an elevated T2. Relative area of stimulated QF was the same for both time points [92 (6)% and 89 (7)%] as was torque loss (approximately 55%). Three days post-EMS, the relative area of stimulated QF injured was not different between time points [30 (14)% vs 29 (17)%, P>0.05]. Recent reports showing that circadian light cues may induce gene-specific patterns of expression in periventricular hypothalamic neurons has extended the functional correlates of light-pulse stimuli, which are conventionally restricted to circadian phase-setting actions within the suprachiasmatic nucleus (ScN). The aims of the present study were, first, to broaden these observations to the protein level using an Egr-1/NGFI-A antibody, and second to investigate the cell-type specificity of induction with respect to associated hypophysiotrophic neurons. In order to co-localize Egr-1 with neuroendocrine peptides, a same-species, double immunofluorescence protocol has been used. The Egr-1 antibody used for immunohistochemistry was first characterized by Western analysis, and we have shown that the C19 antisera detects a single 75kDa protein species, which has a primarily nuclear subcellular localization. Immunohistochemical analysis has shown that Egr-1 protein is markedly induced by a 1h nocturnal light pulse both in the body of the suprachiasmatic nucleus and in a dorsal ScN zone, which extends up into the periventricular nucleus (PeN). In contrast, induction of Egr-1 was not observed within the arcuate region of the hypothalamus. Double immunofluorescence histochemistry has shown that Egr-1 is extensively, although not exclusively, co-localized with somatostatin in PeN neurons. The selective induction of the 75kDa protein product of the egr-1 gene in PeN somatostatinergic neurons, as contrasted with hypophysiotrophic (somatostatin and growth hormone-releasing hormone [GH-RH]) neurons of the arcuate nucleus is indicative of a cell-and stimulus-specific neuroendocrine paradigm, which may be used to address temporal characteristics of somatostatin function that determine pulsatile growth-hormone secretion. Acute pulmonary edema has been associated with cold-water immersion in swimmers and divers. We report on eight divers using a self-contained underwater breathing apparatus (scuba) who developed acute pulmonary edema manifested by dyspnea, hypoxemia, and characteristic chest radiographic findings. All cases occurred in cold water. All scuba divers were treated with complete resolution, and three have returned to diving without further episodes. Mechanisms that would contribute to a raised capillary transmural pressure or to a reduced blood-gas barrier function or integrity are discussed. Pulmonary edema in scuba divers is multifactorial, and constitutional factors may play a role. Physicians should be aware of this potential, likely underreported, problem in scuba divers. The Resin Transfer Molding (RTM) manufacturing process is frequently used to fabricate parts consisting of doubly curved shells. Woven and stitched bi-directional fabrics deform when draped over tooling surfaces with double curvatures. This deformation results in a corresponding change in the preform's permeability tensor, which will change the fluid impregnation pattern during mold filling stage. This paper presents the results of a study in which flow through deformed preforms was investigated. Experiments were conducted in which two different preforms were radially injected after being sheared to a specified angle. The resulting elliptical flowfronts were used to determine the change in the orientation and degree of anisotropy of the permeability tensor caused by the preform's deformation. The experimental results were then compared with a numerical model developed to describe flow through typical RTM preforms using a deformable unit cell. The anisotropy ratio of the principal permeability tensor was found to increase with the shearing angle as observed experimentally and was verified by the numerical model. The success of the RTM mold design and the accuracy of mold filling simulations are dependent on the use of the correct spatial permeability tensor inside the mold. A predictive model that can provide the permeability changes as a function of local shearing deformation within the draped and compacted preforms should move us closer to a realistic simulation of mold filling in net-shaped composite structures. Objective: To learn how nicotine has been regarded by a major tobacco company.Data Sources: Documents from Brown and Williamson Tobacco Corporation (B&W), the British American Tobacco Company (BAT), and other tobacco interests provided by an anonymous source, obtained from Congress, and received from the private papers of a former BAT officer.Study Selection: All available materials, including confidential reports regarding research and internal memoranda exchanged between tobacco industry lawyers.Conclusions: During a period of 22 years (1962 to 1984), employees of B&W and BAT conducted research and commented on the pharmacology of nicotine. They consistently regarded nicotine as the pharmacological agent that explained tobacco use. In the early part of the period under study, officials of the companies wrote about nicotine addiction explicitly. Inhalation of cigarette smoke by the consumer was recognized throughout the period as necessary for the normal function of a cigarette. The documents contain little indication that research was conducted on either the taste or the flavor of nicotine. BAT researchers appear to have determined that sidestream smoke produces irritation, that it contains toxic substances including N-nitrosamines, and that it is "biologically active" (eg, carcinogenic) in laboratory tests. During the 1980s, the primary purpose of BAT's research related to ETS was to develop a new cigarette that emitted less irritating and less biologically active sidestream smoke. Publicly, the tobacco industry has denied that exposure to ETS has been proven dangerous to health. It has criticized the methodology of published research on ETS, even when some of its own consultants have privately acknowledged that the research was valid. In addition, the industry has funded scientific research with the stated purpose of anticipating and refuting the evidence against ETS. The tobacco industry's strategy regarding passive smoking has been remarkably similar to its strategy regarding active smoking. It has privately conducted internal research, at least some of which has supported the conclusion that passive smoking is dangerous to health, while it has publicly denied that the hazards have been proven. A previously unreported association between a familial predisposition to colonic neoplasia and familial adult onset lower motor neuron (LMN) degeneration is reported. Two brothers presented at the ages of 53 and 44 years with multiple colonic adenomata and invasive colonic carcinoma respectively. Subsequently both developed a virtually identical pattern of motor neuron disease of progressive muscular atrophy type. At presentation both had LMN weakness affecting predominantly the upper limb and neck muscles. The disease progressed rapidly to involve the lower limb and bulbar musculature and both brothers died after a 15 month course. Necropsy was performed on one brother and showed pathological changes confined to the LMNs with no evidence of involvement of the pyramidal tracts or motor cortex. The combination of these diseases in two brothers may be of importance in the search for genes responsible for familial motor neuron disorders. It is suggested that a genomic search should be directed initially to the vicinity of known colon neoplasia genes, particularly 5q, 17q and 18q. [ABSTRACT FROM AUTHOR]

Background/aims: Atypical melanocytic nevi (AMN) share some or all of the clinical features of malignant melanoma (MM). The clinical sensitivity for diagnosing MM by physicians experienced in evaluating pigmented lesions is reported to range from 73% to 89%. This study attempted to determine whether clinical sensitivity can be increased by dermoscopy (epilumnescence microscopy, dermatoscopy). Methods: A total of 72 melanocytic neoplasms were histologically diagnosed as either AMN or early MM (<1 mm Breslow thickness). Prior to excisional biopsy, each lesion was photographed without oil being applied to its surface (clinical photo); subsequently it was photographed with oil applied (dermoscopic photo). Each 35 mm color transparency, visualized on a rear-view projector, was analyzed by two experienced and two less experienced observers. Each of them recorded one clinical, one 'overall' dermoscopic, and one 'scored' dermoscopic diagnosis: either AMN or MM for each. Results: Histologically, 21 lesions were diagnosed as MM and 51 as AMN. The observers, except one of the less experienced, had an increase in sensitivity ranging from 5 to 15% for diagnosing MM with dermoscopy. The increase was higher for the scored dermoscopy than for overall dermoscopy. Diagnostic Accuracy (DA) changed from −20% to +11%. Specificity (SP) changed from −23% to +24% with scored dermoscopy. Three of the 21 MMs were missed by one or more observers using all three in vivo diagnostic methods. Conclusions: In this study dermoscopy, when used by experienced dermatologists, increased diagnostic sensitivity and index of suspicion but decreased specificity and diagnostic accuracy for diagnosing MM. The results of a three year study of the viruses isolated from sewage by means of five tissue culture types are reported. The enteroviruses identified demonstrated a seasonal pattern which was similar to that of clinical isolates. Echoviruses and coxsackieviruses displayed a changing pattern of dominant serotypes in both sewage and clinical isolates; echovirus 6, 19, 3 and coxsackievirus B4, B5, A9 successively became the most common serotypes. The poliovirus in sewage was all vaccine-like in character. Reovirus, type 2 was abundant; adenovirus, of several serotypes, were the least often identified. [ABSTRACT FROM PUBLISHER]

A method has been developed for the detection of waterborne viruses which combines the speed of PCR techniques with the assurance of viability given by tissue culture. Environmental water samples are concentrated by adsorption/elution and protein precipitation, subjected to a brief period of culture and the tissue culture fluid assayed for the presence of viruses. RT-PCR was carried out with the EZ rTth DNA polymerase system. We investigated the effects of thorns and spines on the feeding of 5 herbivore species in arid Australia. The herbivores were the rabbit ( Oryctolagus cuniculus), euro kangaroo ( Macropus robustus), red kangaroo ( Macropus rufus), sheep ( Ovis aries), and cattle ( Bos taurus). Five woody plants without spines or thorns and 6 woody plants with thorns were included in the study. The spines and thorns were not found to affect the herbivores' rates of feeding (items ingested/min), but they did reduce the herbivores' rates of biomass ingestion (g-dry/item). The reduction in biomass ingested occurred in two ways: at a given diameter, twigs with spines and thorns had less mass than undefended plants, and the herbivores consumed twigs with smaller diameters on plants with spines and thorns. The relative importance of the two ways that twigs with spines and thorns provided less biomass varied with herbivore body mass. Reduced twig mass was more important for small herbivores, while large herbivores selected smaller diameters. The effectiveness of spines and thorns as anti-herbivore defenses did not vary with the evolutionary history of the herbivores (i.e. native vs. introduced). Spines and thorns mainly affected the herbivores' selection of maximum twig sizes (reducing diameter and mass), but the minimum twig sizes selected were also reduced. [ABSTRACT FROM AUTHOR]
