Adopting both a self-determination theory perspective and a proactive, asset-oriented approach to coping with stressors, we propose a hypothesized model to explain physical education students' year-long development of course-specific grit-perseverance (Study 1) and mental toughness (Study 2). In both studies, we used a randomized controlled trial research design with longitudinally assessed dependent measures (four waves) to test a hypothesized model in which teacher participation in an autonomy-supportive teaching workshop (experimental condition) would increase students' T2 perceived autonomy-supportive teaching and T2 perceived autonomy-supportive classmates, both of which would increase T3 need satisfaction, which would then explain longitudinal gains in students' T4 grit-perseverance (Study 1) and mental toughness (Study 2). In both Study 1 (57 teachers, 3,147 students) and Study 2 (38 teachers, 2,057 students), a multilevel structural equation modeling analysis showed that the hypothesized model fit the data very well. We conclude that the developmental roots of grit-perseverance and mental toughness can emerge proactively out of the asset-oriented experiences of interpersonal support and psychological need satisfaction that are central to self-determination theory. Recently, there has been a call for the application of behavior analytic technology to more diverse domains. Although general‐case procedures are most often applied in autism service delivery, they can be applied to more diverse domains ranging from organizational behavior management to public health initiatives. This literature review, therefore, provides an overview of technological descriptions of general‐case procedures and their applications in areas other than autism intervention. Evaluated variables include the population, independent and dependent variables, generalization measures, and social validity assessments. The results suggest that most researchers are not using a technological description of general‐case procedures nor are they assessing for generalization. Recommendations for future researchers and clinicians include better descriptions of general‐case procedures, evaluation with different populations, assessment of acceptability and more comprehensive evaluations of generalization. The present study evaluated the use of computerized equivalence-based instruction (EBI) to teach classes representing schedules of reinforcement to staff members. Four, 5-member classes representing fixed ratio, fixed interval, variable ratio, and variable interval schedules of reinforcement were taught. The participants were 16 staff members between the ages of 22 and 30 years of age that were employed at a private school for individuals with autism. Participants were randomly assigned to either an experimental or control group. A pretest-training-posttest-maintenance design was used to evaluate the effects of EBI on participants' performance during both a computer-based test and a written multiple-choice test. Participants in the control group were only exposed to pretest and posttests. All participants in the experimental group acquired the baseline trained relations during match-to-sample instruction. Test scores improved from pretest to posttest and derived (untrained) relations emerged across all participants in the experimental group following training but did not change for the participants in the control group. In addition, participants maintained the learned relations one week after EBI was completed. The present study demonstrated that EBI is an effective teaching procedure to teach schedules of reinforcement to staff members in the field of applied behavior analysis. [ABSTRACT FROM AUTHOR]

In a recent study, Gallant et al. (2021) found that both match-to-sample (MTS) and the stimulus-pairing yes/no procedure (SPYN) resulted in a high likelihood of equivalence class formation. Due to this ceiling effect, however, any potential differential effects of the SPYN and MTS trial formats could not be identified. Thus, the purpose of the present study was to compare the effectiveness and efficiency of MTS and SPYN using procedures that were less likely to result in class formation. A pretest-train-posttest sequence was used. During the pretest and posttest, both MTS and SPYN trials were used to assess all trained and derived relations. Class members consisted of abstract visual stimuli. We also assessed the degree to which class-consistent responding would generalize to a card sorting task. During training of baseline relations, half of the participants were exposed only to MTS or only SPYN trials. Posttest results showed that the trial format used to train the baseline relations had no differential effects on class formation; however, participants exposed to MTS training mastered the baseline relations in fewer trials and required less time than those in the SPYN group. Introduction: Patients undergoing thoracic surgery commonly receive perioperative physio-therapy, typically consisting of education, mobilization, breathing exercises, and mobility exercises. To date, no study has described physiotherapy practice for patients undergoing thoracic surgery in Sweden. Purpose: To investigate physiotherapy interventions for patients undergoing thoracic surgery in Sweden. Methods: All physiotherapists currently working in thoracic surgery units in Sweden (n = 8) were eligible to participate. A survey was e-mailed to the physiotherapists, to determine physiotherapy interventions offered to patients undergoing thoracoscopy or open thoracic surgery. Of 21 physiotherapists, 13 (62%) responded, representing seven hospitals. Results: Physiotherapists reported routinely providing preoperative education and postoperative treatment for all patients undergoing thoracic surgery. Breathing exercises and mobilization were usually initiated on the day of surgery or the first postoperative day. Common treatments were deep breathing exercises, with or without positive expiratory pressure, and airway clearance techniques. Upper limb and shoulder exercises were typically initiated on the first or second day after surgery. The most important factor reported to influence treatment choice was personal experience of the attending physiotherapist. No routine post-discharge rehabilitation was provided. Conclusion: Physiotherapists in Sweden reported routinely treating patients scheduled for thoracic surgery, both pre and postoperatively. Courts rely heavily on the recommendations of mental health evaluators when making decisions about competency (e.g., Zapf, Hubbard, Cooper, Wheeles, & Ronan, 2004), including civil competency (e.g., Quickel, Demakis, & Reeve, 2017); however, very little is known about how evaluators arrive at their opinions regarding an individual's capacity. The current study assessed clinical decision-making using a policy capturing methodology. An ecologically valid sample (N = 47) of predominantly white (87.23%) and female (55.30%) clinicians who were eligible to conduct civil competency evaluations in North Carolina were recruited to examine the degree to which five key factors (functional test data, cognitive test data, personality test data, hospitalization history, and/or medication compliance) influenced clinician competency determinations. Personal insight into one's decision-making process was also measured. Consistent with previous research, functional test data was weighted most heavily in evaluator decisions, though all five factors were considered. Further analysis suggested variable levels of self-awareness of one's decision-making policies by participants depending on the factor. In addition, there was high agreement among clinicians when cases were straightforward, but perfect disagreement (half recommending competency, half recommending incompetency) among clinicians when cases were more complex. Implications for civil competency assessment and decision-making are discussed. IMPACT STATEMENT: Functional assessment data drives clinician decisions regarding civil capacity, over and above the impact of cognitive and personality test data, hospitalization history, and medication compliance. The presence of disorder substantially influences the behaviour of physical systems. It can give rise to slow or glassy dynamics, or to a complete suppression of transport as in Anderson insulators1, where normally extended wavefunctions such as light fields or electronic Bloch waves become exponentially localized. The combined effect of disorder and interactions is central to the richness of condensed-matter physics2. In bosonic systems, it can also lead to additional quantum states such as the Bose glass3,4—an insulating but compressible state without long-range phase coherence that emerges in disordered bosonic systems and is distinct from the well-known superfluid and Mott insulating ground states of interacting bosons. Here we report the experimental realization of the two-dimensional Bose glass using ultracold atoms in an eight-fold symmetric quasicrystalline optical lattice5. By probing the coherence properties of the system, we observe a Bose-glass-to-superfluid transition and map out the phase diagram in the weakly interacting regime. We furthermore demonstrate that it is not possible to adiabatically traverse the Bose glass on typical experimental timescales by examining the capability to restore coherence and discuss the connection to the expected non-ergodicity of the Bose glass. Our observations are in good agreement with recent quantum Monte Carlo predictions6 and pave the way for experimentally testing the connection between the Bose glass, many-body localization and glassy dynamics more generally7,8.The two-dimensional Bose glass state of matter is realized experimentally using ultracold atoms in an eight-fold symmetric quasicrystalline optical lattice, and the phase transition between Bose glass and superfluid is directly observed. Social Stories™ are a story-based intervention commonly used to teach students with disabilities how to behave in new routines or changing environments, academic skills, and social interactions including tacting others' emotions. According to the National Standards Project, story-based interventions are considered an established trea™ent. However, the efficacy of Social Stories™ is confounded by limitations in chosen experimental designs, threats to internal validity, and additional interventions within trea™ent packages. The current study used Social Stories™ to teach classroom social skills to children diagnosed with an intellectual disability. We used a multiple baseline across participants design to evaluate changes in classroom social skills when using Social Stories™. We taught a total of four response classes, including: saying, "please," saying, "thank you," appropriate hand raising, and giving assistance. The results indicate that Social Stories™ may be an effective intervention to increase classroom social skills. This is one of the first studies to evaluate and demonstrate the effectiveness of Social Stories™ as a standalone intervention. The present study also extends the current social studies literature in several areas, such as developing ways to promote generalization and assessing the social validity of the goals, procedures, and outcomes of the study. Background: Generalist approaches can help address several challenges facing today's primary care. However, GPs report insufficient support to deliver advanced generalist medicine (AGM) in daily practice, struggling within a healthcare system that imposes strict adherence to single-disease focused guidelines. Aim: To examine the professional and educational experiences of newly qualified GPs attending a course on AGM to understand how to redesign primary care systems to support their generalist work. Design and setting: This was a qualitative study focusing on AGM in UK general practice (England), conducted in the context of the research evaluation of an online career development programme on AGM. Method: We conducted 36 interviews and six focus groups with newly qualified GPs attending an online career development programme on AGM, and analysed data using framework analysis. Results: Three tensions experienced by the participants were identified: tension between realistic and idealistic practice; tension between different decision-making paradigms; and tension in the formation of the GPs' professional identities. These were owing to grey areas of practice deeply rooted in primary care systems — namely areas of work not adequately addressed by current education and service design. Conclusion: Our findings have implications for tackling the general practice workforce crisis, highlighting that solutions targeting individual problems will not suffice by themselves. By making visible the grey areas of everyday general practice, we describe the changes needed to target tensions as described by the GPs in this study to ultimately enable, enhance and make visible the complex work of generalist medicine. Cleaner synthesis of amines remains a key challenge in organic chemistry because of their prevalence in pharmaceuticals, agrochemicals and synthetic building blocks. Here, we report a different paradigm for chemoselective hydrogenation of nitro compounds to amines, under mild, aqueous conditions. The hydrogenase enzyme releases electrons from H2 to a carbon black support which facilitates nitro-group reduction. For 30 nitroarenes we demonstrate full conversion (isolated yields 78 – 96%), with products including pharmaceuticals benzocaine, procainamide and mesalazine, and 4-aminophenol – precursor to paracetamol (acetaminophen). We also showcase gram-scale synthesis of procainamide with 90% isolated yield. We demonstrate potential for extension to aliphatic substrates. The catalyst is highly selective for reduction of the nitro group over other unsaturated bonds, tolerant to a wide range of functional groups, and exhibits excellent stability in reactions lasting up to 72 hours and full reusability over 5 cycles with a total turnover number over 1 million, indicating scope for direct translation to fine chemical manufacturing. The reduction of nitro-groups is a common synthetic route to amines, but biocatalytic strategies for such reactions are still being developed. In this study, the authors repurposed the hydrogenase enzyme by immobilisation on carbon black to yield a heterogeneous chemobiocatalyst for selective production of amines. [ABSTRACT FROM AUTHOR]

The enzyme CbA5H, a [FeFe]‐hydrogenase from Clostridium beijerinckii, has previously been shown to survive exposure to oxygen, making it a promising candidate for biotechnological applications. Thus far [NiFe]‐hydrogenases are typically considered for such applications, due to the superior O2‐tolerance and therefore simplified enzyme handling. However, methods for production of [FeFe]‐hydrogenases are generally more successful than for other classes of hydrogenases, therefore in this work we focus on demonstrating scalable CbA5H production, and report results with active enzyme prepared in bioreactors (up to 10 L) with >20‐fold improvement in purified enzyme yield. We then go on to confirm excellent H2/H+‐cycling activity of the air‐purified protein, highlighting that CbA5H can be prepared and isolated without the need for complex and expensive infrastructure. Next, we demonstrate good stability of the air‐purified CbA5H both in solution assays, and as a heterogenous catalyst system when immobilized on a carbon support. Finally, we successfully implement this enzyme within previously demonstrated biotechnologies for flavin and NADH recycling, highlighting its relevance in chemical synthesis, and we demonstrate production of an important API precursor, 3‐quinuclidinol at >0.4 g scale in standard benchtop hydrogenation infrastructure, with >100,000 CbA5H turnovers over 18 operational hours. High dimensional learning is data-hungry in general; however, many natural data sources and real-world learning problems posses some hidden low-complexity structure that permit effective learning from relatively small sample sizes. We are interested in the general question of how to discover and exploit such hidden benign traits when problem-specific prior knowledge is insufficient. In this work, we address this question through random projection's ability to expose structure. We study both compressive learning and high dimensional learning from this angle by introducing the notions of compressive distortion and compressive complexity. We give user-friendly PAC bounds in the agnostic setting that are formulated in terms of these quantities, and we show that our bounds can be tight when these quantities are small. We then instantiate these quantities in several examples of particular learning problems, demonstrating their ability to discover interpretable structural characteristics that make high dimensional instances of these problems solvable to good approximation in a random linear subspace. In the examples considered, these turn out to resemble some familiar benign traits such as the margin, the margin distribution, the intrinsic dimension, the spectral decay of the data covariance, or the norms of parameters—while our general notions of compressive distortion and compressive complexity serve to unify these, and may be used to discover benign structural traits for other PAC-learnable problems. Purpose: Emotional and functional well-being (EWB and FWB) are important components of mental health and quality of life. This study aims to evaluate long-term EWB and FWB in breast cancer (BC) survivors. Methods: The Carolina Breast Cancer Study Phase 3 oversampled Black and younger (< 50 years in age) women so that they each represent approximately 50% of the study population and assessed participants' EWB and FWB with the Functional Assessment of Cancer Therapy—Breast (FACT-B) at 5- (baseline), 25-, and 84-months post diagnosis. Multinomial logit models were used to estimate adjusted odds ratios (ORs) and 95% confidence intervals (CIs) for associations between demographic and clinical characteristics and well-being change relative to baseline. Results: Among 2,781 participants with BC, average EWB and FWB improved with time since diagnosis. Persistent FWB decrements were associated with Black race [OR 1.4 (95% CI 1.2–1.7) and 1.3 (95% CI 1.1–1.6), at 25-months and 84-months respectively], older age [OR 1.4 (95% CI 1.1–1.7) and 1.5 (95% CI 1.2–1.8), respectively], no chemotherapy, and recurrence [OR 2.9 (95% CI 1.8–4.8) and 3.1 (95% CI 2.1–4.6), respectively]. EWB decrements were associated with advanced stage and recurrence. Decrements in combined (FWB+EWB) well-being were associated with recurrence at both follow-up survey timepoints [ORs 4.7 (95% CI 2.7–8.0) and 4.3 (95% CI 2.8–6.6), respectively]. Conclusions: Long-term well-being varies by demographics and clinical features, with Black women and women with aggressive disease at greatest risk of long-term decrements. [ABSTRACT FROM AUTHOR]

Repeats can mediate rearrangements and recombination in plant mitochondrial genomes and plastid genomes. While repeat accumulations are linked to heightened evolutionary rates and complex structures in specific lineages, debates persist regarding the extent of their influence on sequence and structural evolution. In this study, 75 Plantago plastomes were analyzed to investigate the relationships between repeats, nucleotide substitution rates, and structural variations. Extensive repeat accumulations were associated with significant rearrangements and inversions in the large inverted repeats (IRs), suggesting that repeats contribute to rearrangement hotspots. Repeats caused infrequent recombination that potentially led to substoichiometric shifting, supported by long-read sequencing. Repeats were implicated in elevating evolutionary rates by facilitating localized hypermutation, likely through DNA damage and repair processes. This study also observed a decrease in nucleotide substitution rates for loci translocating into IRs, supporting the role of biased gene conversion in maintaining lower substitution rates. Combined with known parallel changes in mitogenomes, it is proposed that potential dysfunction in nuclear-encoded genes associated with DNA replication, recombination, and repair may drive the evolution of Plantago organellar genomes. Autonomy-supportive teaching increases prosocial and decreases antisocial behavior. Previous research showed that these effects occur because autonomy-supportive teaching improves students' need states (a student-level process). However, the present study investigated whether these effects also occur because autonomy-supportive teaching improves the classroom climate (a classroom-level process). Teachers from 80 physical education classrooms were randomly assigned to participate (or not) in an autonomy-supportive teaching intervention, while their 2,227 secondary-grade students reported their need satisfaction and frustration, supportive and hierarchical classroom climates, and prosocial and antisocial behaviors at the beginning, middle, and end of an academic year. A doubly latent, multilevel structural equation model showed that teacher participation in the intervention (experimental condition) increased class-wide need satisfaction, a supportive climate, and prosocial behavior and decreased class-wide need frustration, a hierarchical climate, and antisocial behavior. Together, greater collective need satisfaction and a more supportive climate combined to explain increased prosocial behavior, while lesser need frustration and a less hierarchical climate combined to explain decreased antisocial behavior. Microplastic pollution (MP) in marine environments around the globe is severe and insufficient precautions have yet to be taken for its prevention. The focus of this study was on quantifying MPs from beach sediment and seawater samples and identifying their distributions and types along the western coast of Sri Lanka from the Kelani River estuary to the Mahaoya estuary. Nine sites along this 42 km stretch were selected, and random sampling was employed to collect a minimum of eight sediment samples from each site between October and December 2021. Water samples were also collected, parallel to the sediments, from the ocean surface. FTIR analysis revealed that most of the MPs found were polyethylene (PE), polypropylene (PP), polystyrene (PS), polyethylene terephthalate (PET), and phenol formaldehyde resin. The mean abundance of MPs varied from 2.0 ± 0.6 items/L to 161.0 ± 15.7 items/L in water samples and from 3.0 ± 0.3 items/m2 to 656.0 ± 34.5 items/m2 in sediment samples. The MPs found were identified in different shapes as fragments (80.2%), pellets (14.9%), fibers (2.7%), and foams (2.5%). Analysis revealed that the beach sediments were contaminated with PS, phenol formaldehyde resin, PET, PP, and PE, while the surface seawater was dominated by phenol formaldehyde resin, PS, PP, and PE. PURPOSE: Our purpose was to describe the prevalence and predictors of symptom and function clusters related to physical, emotional, and social components of general health-related quality of life (HRQOL) in a population-based sample of prostate cancer (PCa) survivors. METHODS: Participants (N = 1,162) completed a baseline survey at a median of 9 months after diagnosis to ascertain the co-occurrence of eight symptom and functional domains that are common across all cancers and not treatment-specific. We used latent profile analysis (LPA) to identify subgroup profiles of survivors with low, moderate, or high HRQOL levels. Multinomial logistic regression models were used to identify clinical and sociodemographic factors associated with survivors' membership in the low versus moderate or high HRQOL profile. RESULTS: The LPA identified 16% of survivors who were categorized in the low HRQOL profile at baseline, indicative of the highest symptom burden and lowest functioning. Factors related to survivors' membership in the low versus higher HRQOL profile groups included less than age 65 years at diagnosis, identifying as non-Hispanic Black race, not working, being a former versus never smoker, systemic therapy, less companionship, more comorbidities, lower health care financial well-being, or less spirituality. Several factors remained associated with remaining in the low versus higher HRQOL profiles on the follow-up survey (n = 699), including younger age, Black race, comorbidity, and lower financial and spiritual well-being. CONCLUSION: About one of six PCa survivors experienced elevated physical and psychosocial symptoms that were independent of local curative therapy, but with younger age, race, comorbidity, and lower financial and spiritual well-being as stable risk factors for poor HRQOL over time. We evaluated the effects of a discrimination training procedure for establishing praise as a reinforcer for three children with autism spectrum disorder. After establishing two praise words as discriminative stimuli and two nonsense words as S‐deltas, we evaluated whether the stimuli then functioned as reinforcers by presenting each stimulus as a consequence for a new response. The results demonstrated that previously neutral praise words functioned as reinforcers and nonsense words did not. As in previous studies on establishing reinforcers, the effects were transitory, and praise words did not continue to function as reinforcers after repeated exposure without discrimination training. Purpose: Successful patient-focused drug development involves selecting and measuring outcomes in clinical trials that are important to patients. The U.S. Food & Drug Administration's definition of clinical benefit includes how patients feel, function, or survive. Patients are considered the experts in describing how they feel and function. In cancer trials, patient-reported measures of physical function provide insight into how patients function at baseline, benefit from the interventions being studied, and the impact of treatment side effects. We conducted a qualitative study with adults diagnosed with cancer to describe facets of physical function from their perspective and to identify which facets are most important to this patient population. Methods: Using concept elicitation and cognitive interviewing techniques, we conducted semi-structured interviews with 72 adults ≥ 22 years of age with cancer who received treatment with an anticancer drug or biologic within six months of the interview. We selected participants using purposive sampling with the aim to elicit diverse experiences regarding how they may interpret and respond to questions related to physical function. Participants were presented with patient-reported outcome (PRO) items representative of PRO measures used in cancer and general populations. Results: Five facets of how physical function relates to activities were defined from the patient perspective: ability, difficulty, limitation, satisfaction, and completion. More than half of the participants indicated that ability was the most important facet of physical function. The next most important were satisfaction (18.3%), limitation (14.1%), difficulty (5.6%), and completion (2.8%). Conclusion: This study demonstrates that we must be more specific about the facets of physical function that we set out to assess when we use PRO measures to describe the patient experience. Purpose: The U.S. Food & Drug Administration has identified physical functioning (PF) as a core patient-reported outcome (PRO) in cancer clinical trials. The purpose of this study was to identify PF PRO measures (PROMs) in adult cancer populations and classify the PROMs by content covered (facets of PF) in each measure. Methods: As part of the Patient Reports of Physical Functioning Study (PROPS) research program, we conducted a targeted literature review to identify PROMs that could be used in clinical trials to evaluate PF from the patient perspective. Next, we convened an advisory panel to conduct a modified, reactive, Delphi study to reach consensus on which PF facets are assessed by PROMs identified in the review. The panel engaged in a "card sort" activity to classify PROM items by PF facets. Consensus was reached when 80% of panel members agreed that at least one facet was being measured by each PROM item. Results: The literature review identified 13 PROMs that met inclusion criteria. Eight facets of PF were identified for classification in the Delphi study: ability, completion, difficulty, limitation, quality, frequency, bother, and satisfaction. Through two rounds, the panel documented and classified conceptual approaches for each PRO item presented. The most prevalent PF facets were ability, difficulty, and limitation. Conclusion: Classifying PF PROMs by PF facets will promote more consistent communication regarding the aspects of PF represented in each PROM, helping researchers prioritize measures for inclusion in cancer clinical trials. In Australia, community gardens (CGs) commonly take the form of communally-operated growing spaces situated on public land administered by local governments (LGs). LGs and their policies are gatekeepers for CGs, and their support can determine garden viability; however, evidence regarding the nature of "support" in this context is limited. This research identified CG policies through the websites of all LGs (n = 207) in New South Wales (NSW) and Victoria, Australia and analysed these policies to describe which LGs have a CG policy, the aims of LGs' support, the departments responsible for CGs, and what practical supports LGs provide. Thirty-nine CG policies were analysed. Most commonly, policies existed to provide a standardised framework for how LG would facilitate community groups to establish and manage CGs, and to define the roles and responsibilities of LG and community groups. Departments responsible for CG policy implementation were categorised as Community, Environment and Infrastructure, Planning, and/or Strategic and Corporate. Overall, LGs took a "hands-off" facilitator role, rather than one of an implementation partner. Common supports during application and establishment included LG arranging a licence agreement for leasing the land, assisting with site identification, and helping groups with the application process. During the garden maintenance phase, the most common supports were providing platforms for networking between CGs, providing education and information, assisting groups to apply for grants, and providing in-kind support. Recommendations for LGs, community members, and other levels of government are provided. Key Policy Highlights Currently, the nature of support provided by Australian local governments to community gardens is variable and largely limited to a "facilitator" role. Local government support could be strengthened by producing clear and comprehensive policies, reducing bureaucratic red tape, and institutionalising processes that preserve public land for food production. Professor Irene Manton (1904–1988) was in the vanguard of technical advances in botany through the use of electron microscopy. As well as new techniques and discoveries communicated through a considerable body of scientific publications, she left behind a collection of antiquities spanning a broad range of cultures and dates. Through these objects, Manton investigated intellectual problems in the history of science and communicated her findings with a highly original and idiosyncratic approach. At first sight a disparate group of unprovenanced antiquities, through the lens of Manton's archives this collection can be understood as a rich repository of material evidence. This critical examination of Manton's collecting practices and the uses she made of her ancient objects provides new evidence for the variety of approaches to collecting and interpreting antiquities, spanning disciplinary boundaries, over the course of the twentieth century. Purpose: To synthesise qualitative research on individuals diagnosed with (or reportedly showing traits of) borderline pattern personality disorder who underwent dialectical behaviour therapy, aiming to comprehend their perceptions of change processes and the therapy's effects. Method: A comprehensive literature search was conducted across multiple online databases and grey literature sources. Papers were quality appraised using an adapted version of the Critical Appraisal Skills Programme tool. A metaethnographic approach was employed during the synthesis. Results: Eleven studies met criteria for inclusion in the review. The main themes identified through the synthesis process were the impact of DBT, the supportive structure and the 1:1 therapy component. Conclusions: The synthesis uncovered the importance of various processes within DBT that patients perceived as active ingredients for their change. Social prescribing link workers are recently introduced roles in English primary care. One of their intended functions is to support patients with conditions influenced by the wider, social determinants of health. Their main purpose is to connect people to community resources to meet their nonmedical needs. However, our research reveals that link workers provide not only connections but also what we have described as "holding" for individuals with complex needs, who lack informal networks of support or who are waiting to access services. We explore the concept of holding, its meaning and significance in this context, and consider its consequences. As part of a realist evaluation, we observed seven link workers in GP practices in England during focussed ethnographies over a 3‐week period. We took field notes and interviewed 61 patients and 93 healthcare and voluntary sector professionals. Nine to twelve months later, we carried out follow‐up interviews with forty‐one patients, seven link workers, and a link worker manager. We identified four functions of holding: supporting patients waiting for services, sustaining patients as they prepare for change, reducing the emotional burden of primary healthcare professionals, and bearing witness to patients' distress. Holding appears to be a vital but often overlooked aspect of social prescribing. Patients benefit from having a reliable and consistent person to support their emotional needs. However, similar to the impact of holding on other primary care professionals, there are unintended consequences: some link workers exceed their capacity, become overburdened, experience burnout, and leave their job. Recognizing the importance of holding and understanding its role in link workers' primary care responsibilities are critical. If holding work is accepted as a role for link workers, providing training and support to them should be prioritised to ensure successful implementation and positive outcomes for patients, link workers, and primary healthcare staff. Objective: The growing deprescribing field is challenged by a lack of consensus around evidence and knowledge gaps. The objective of this overview of systematic reviews was to summarize the review evidence for deprescribing interventions in older adults. Methods: 11 databases were searched from 1st January 2005 to 16th March 2023 to identify systematic reviews. We summarized and synthesized the results in two steps. Step 1 summarized results reported by the included reviews (including meta-analyses). Step 2 involved a narrative synthesis of review results by outcome. Outcomes included medication-related outcomes (e.g., medication reduction, medication appropriateness) or twelve other outcomes (e.g., mortality, adverse events). We summarized outcomes according to subgroups (patient characteristics, intervention type and setting) when direct comparisons were available within the reviews. The quality of included reviews was assessed using A MeaSurement Tool to Assess systematic Reviews 2 (AMSTAR 2). Results: We retrieved 3,228 unique citations and assessed 135 full-text articles for eligibility. Forty-eight reviews (encompassing 17 meta-analyses) were included. Thirty-one of the 48 reviews had a general deprescribing focus, 16 focused on specific medication classes or therapeutic categories and one included both. Twelve of 17 reviews meta-analyzed medication-related outcomes (33 outcomes: 25 favored the intervention, 7 found no difference, 1 favored the comparison). The narrative synthesis indicated that most interventions resulted in some evidence of medication reduction while for other outcomes we found primarily no evidence of an effect. Results were mixed for adverse events and few reviews reported adverse drug withdrawal events. Limited information was available for people with dementia, frailty and multimorbidity. All but one review scored low or critically low on quality assessment. Conclusion: Deprescribing interventions likely resulted in medication reduction but evidence on other outcomes, in particular relating to adverse events, or in vulnerable subgroups or settings was limited. Animals, and mammals in particular, vary widely in their "pace of life," with some species living long lives and reproducing infrequently (slow life histories) and others living short lives and reproducing often (fast life histories). These species also vary in the importance of maternal care in offspring fitness: In some species, offspring are fully independent of their mothers following a brief period of nutritional input, while others display a long period of continued dependence on mothers well after nutritional dependence. Here, we hypothesize that these two axes of variation are causally related to each other, such that extended dependence of offspring on maternal presence leads to the evolution of longer lives at the expense of reproduction. We use a combination of deterministic modeling and stochastic agent-based modeling to explore how empirically observed links between maternal survival and offspring fitness are likely to shape the evolution of mortality and fertility. Each of our modeling approaches leads to the same conclusion: When maternal survival has a strong impact on the survival of offspring and grandoffspring, populations evolve longer lives with less frequent reproduction. Our results suggest that the slow life histories of humans and other primates as well as other long-lived, highly social animals such as hyenas, whales, and elephants are partially the result of the strong maternal care that these animals display. We have designed our models to be readily parameterized with demographic data that are routinely collected by long-term researchers, which will facilitate more thorough testing of our hypothesis. How do people identify new and better lifegoals for themselves? The goal breakthrough model (GBM) purports to answer this question. The GBM draws from creative process theories of preparation, incubation, illumination, and elaboration to explain how people "cross the Rubicon" to new purposes, in response to felt dissatisfaction. Neuroscience research supporting the GBM is reviewed, highlighting brain‐sequences linking Default Mode Network activity, Cognitive Control Network activity, and Salience Network activity. This understanding of the neural basis of creative goal‐functioning informs an elaborated version of the GBM, one that is less linear and more dynamic than its predecessor model. Overall, the GBM proposes a novel explanation for how people can actively prompt their nonconscious minds to provide new and better behavioral alternatives to consider. [ABSTRACT FROM AUTHOR]

Many guidelines have been published to help diagnose food allergies, which have included feeding difficulties as a presenting symptom (particularly for non-IgE-mediated gastrointestinal allergies). This study aimed to investigate the prevalence of feeding difficulties in children with non-IgE-mediated gastrointestinal allergies and the association of such difficulties with symptoms and food elimination. An observational study was performed at Great Ormond Street Hospital for Children NHS Foundation Trust, London, UK. Children aged 4 weeks to 16 years without non-allergic co-morbidities who improved on an elimination diet using a previously published Likert scale symptom score were included. This study recruited 131 children, and 114 (87%) parents completed the questionnaire on feeding difficulties. Feeding difficulties were present in 61 (53.5%) of the 114 children. The most common feeding difficulties were regular meal refusals (26.9%), extended mealtimes (26.7%), and problems with gagging on textured foods (26.5%). Most children (40/61) had ≥2 reported feeding difficulties, and eight had ≥4. Children with feeding difficulties had higher rates of constipation and vomiting: 60.7% (37/61) vs. 35.8% (19/53), p = 0.008 and 63.9% (39/61) vs. 41.5% (22/53), p = 0.017, respectively. Logistic regression analysis demonstrated an association between having feeding difficulties, the age of the child, and the initial symptom score. Gender and the number of foods excluded in the elimination diet were not significantly associated with feeding difficulties. This study found that feeding difficulties are common in children with non-IgE-mediated gastrointestinal allergies, but there is a paucity of food allergy specific tools for establishing feeding difficulties, which requires further research in the long-term and consensus in the short term amongst healthcare professions as to which tool is the best for food allergic children. The current study examined the efficacy and efficiency of incorporating instructive feedback within matrix training to teach children with autism spectrum disorder (ASD) to label common characters and cities. Experimenters taught one set of responses using a non-overlapping matrix, a second set of responses using an overlapping matrix, and a third set of responses using a non-overlapping matrix along with secondary targets to three individuals with ASD. The results demonstrated that all teaching methods were effective, and all trained and untrained responses were acquired. Matrix training with instructive feedback was equally as efficient as non-overlapping matrix training and overlapping matrix training, requiring about the same number of sessions for each participant to acquire the responses. The findings demonstrated that establishing recombinative generalization through matrix training and instructive feedback is equally and maybe even more effective and efficient than matrix training in isolation in some circumstances. Objective: We systematically examined comparative gout flare risk after initiation or escalation of different urate‐lowering therapies (ULTs), comparative flare risk with and without concomitant flare prophylaxis, adverse event rates associated with flare prophylaxis, and optimal duration of flare prophylaxis. Methods: We searched the Medline, Embase, Web of Science, and Cochrane databases and clinical trial registries from inception to November 2021 for trials investigating adults with gout initiating or escalating ULT. We performed random effects network meta‐analyses and calculated risk ratios (RRs) between treatments. Bias was assessed using the revised Cochrane risk‐of‐bias tool. Results: We identified 3,775 records, of which 29 publications (27 trials) were included. When compared to placebo plus prophylaxis, the RR of flares ranged from 1.08 (95% confidence interval [CI] 0.87–1.33) for febuxostat 40 mg plus prophylaxis to RR 2.65 [95% CI 1.58–4.45] for febuxostat 80 mg plus lesinurad 400 mg plus prophylaxis. Compared to ULT alone, the RR of flares was lower for ULT plus rilonacept 160 mg (RR 0.35 [95% CI 0.25–0.50]), ULT plus rilonacept 80 mg (RR 0.43 [95% CI 0.31–0.60]) and ULT plus colchicine (RR 0.50 [95% CI 0.35–0.72]). There was limited evidence for other flare prophylaxis and on prophylaxis harms and optimal duration. Primarily because of missing outcome data and bias in the selection of reported results, 71.4% and 63.4% of studies were assessed as high risk of bias for flares and adverse events, respectively. Conclusion: The RR of flares when introducing ULT varies depending on ULT drug and dosing strategies. There were limited data on ULT escalation. Flare prophylaxis with colchicine and rilonacept reduces flare incidence. More research is required on the harms and optimal duration of prophylaxis. [ABSTRACT FROM AUTHOR]

Groundwater-influenced ecosystems (GIEs) are increasingly vulnerable due to groundwater extraction, land-use practices, and climate change. These ecosystems receive groundwater inflow as a portion of their baseflow or water budget, which can maintain water levels, water temperature, and chemistry necessary to sustain the biodiversity that they support. In some systems (e.g., springs, seeps, fens), this connection with groundwater is central to the system's integrity and persistence. Groundwater management decisions for human use often do not consider the ecological effects of those actions on GIEs. This disparity can be attributed, in part, to a lack of information regarding the physical relationships these systems have with the surrounding landscape and climate, which may influence the environmental conditions and associated biodiversity. We estimate the vulnerability of areas predicted to be highly suitable for the presence of GIEs based on watershed (U.S. Geological Survey Hydrologic Unit Code 12 watersheds: 24–100 km2) and pixel (30 m × 30 m pixels) resolution in the Atlantic Highlands and Mixed Wood Plains EPA Level II Ecoregions in the northeastern United States. We represent vulnerability with variables describing adaptive capacity (topographic wetness index, hydric soil, physiographic diversity), exposure (climatic niche), and sensitivity (aquatic barriers, proportion urbanized or agriculture). Vulnerability scores indicate that ~26% of GIEs were within 30 m of areas with moderate vulnerability. Within these GIEs, climate exposure is an important contributor to vulnerability of 40% of the areas, followed by land use (19%, agriculture or urbanized). There are few areas predicted to be suitable for GIEs that are also predicted to be highly vulnerable, and of those, climate exposure is the most important contributor to their vulnerability. Persistence of GIEs in the northeastern United States may be challenged as changes in the amount and timing of precipitation and increasing air temperatures attributed to climate change affect the groundwater that sustains these systems. Aims: Caregivers rate improved communication ability as one of the most desired outcomes for successful interventions for individuals with Angelman syndrome (AS). When measuring communication ability in clinical trials, the reliability of such measures is critical for detecting significant changes over time. This study examined the reliability of the Observed-Reported Communication Ability (ORCA) measure completed by caregivers of individuals with AS. Methods: The ORCA measure was completed by 249 caregivers with 170 caregivers completing the ORCA measure again after 5–12 days. Generalizability theory was used to examine the following sources of measurement error in ORCA scores: concepts, subdomains, assessment points, and the interactions among those facets and the object of measurement: communication ability. Three generalizability studies were conducted to understand the reliability of the ORCA measure for different measurement designs. Decision studies were carried out to demonstrate the optimization of measurement procedures of the ORCA measure. Results: G and Phi coefficients of the original measurement design exceeded the 0.80 threshold considered sufficiently reliable to make relative and absolute decisions about the communication ability of individuals with AS based on their caregivers' observed scores. The optimization procedures indicated that increasing the number of communication concepts and/or assessment points leads to more reliable estimates of communication. Conclusion: The ORCA measure was able to reliably distinguish different levels of communication ability among individuals with AS. Seven passive acoustic surveys for marine mammal sounds were conducted by deploying sonobuoys along ship tracks during Antarctic voyages spanning years 2006-2021. These surveys included nearly 330° of longitude throughout Antarctic (south of 60°S) and sub-Antarctic (between 50-60°S) latitudes. Here, we summarise the presence of calls from critically endangered Antarctic blue whales (Balaenoptera musculus intermedia) detected on all seven of these surveys. We describe and compare the spatial distribution of detections of three different types of Antarctic blue whale calls: unit-A, Z-calls, and D-calls. Three sets of voyages partially overlapped spatially but in different years, providing three regions (Indian Sector, Dumont d'Urville Sea, Ross Sea) to investigate differences over time for these three different call types. The proportion of sonobuoys with calls present was significantly higher in the more recent years for seven of the 15 combinations of years, regions, and call type. The proportion of sonobuoys with calls present was significantly lower only for one of the 15 combinations (unit A in the Ross Sea between 2015 vs 2017), and not significantly different for the remaining seven pairwise comparisons. We discuss possible explanations for these observations including: differences in probability of detection, whale behaviour, whale distribution, and abundance. These explanations are not mutually exclusive and cannot yet be resolved without application of complex analytical methods and collection of additional data. Lastly, we discuss future work that could help clarify the contributions of each of these potential drivers of acoustic detection. We propose continued acoustic data collection, application of new analytical methods, and collection of other synergistic data from Antarctic blue whales on their feeding grounds as a basis for future work on this species. This could provide a cost effective and holistic means of monitoring their status after the effects of 20th century industrial whaling, as well as their responses to natural and anthropogenic changes to their main prey, Antarctic krill, and a changing climate. Background Predictive models show promise in healthcare, but their successful deployment is challenging due to limited generalizability. Current external validation often focuses on model performance with restricted feature use from the original training data, lacking insights into their suitability at external sites. Our study introduces an innovative methodology for evaluating features during both the development phase and the validation, focusing on creating and validating predictive models for post-surgery patient outcomes with improved generalizability. Methods Electronic health records (EHRs) from 4 countries (United States, United Kingdom, Finland, and Korea) were mapped to the OMOP Common Data Model (CDM), 2008-2019. Machine learning (ML) models were developed to predict post-surgery prolonged opioid use (POU) risks using data collected 6 months before surgery. Both local and cross-site feature selection methods were applied in the development and external validation datasets. Models were developed using Observational Health Data Sciences and Informatics (OHDSI) tools and validated on separate patient cohorts. Results Model development included 41 929 patients, 14.6% with POU. The external validation included 31 932 (UK), 23 100 (US), 7295 (Korea), and 3934 (Finland) patients with POU of 44.2%, 22.0%, 15.8%, and 21.8%, respectively. The top-performing model, Lasso logistic regression, achieved an area under the receiver operating characteristic curve (AUROC) of 0.75 during local validation and 0.69 (SD = 0.02) (averaged) in external validation. Models trained with cross-site feature selection significantly outperformed those using only features from the development site through external validation (P  < .05). Conclusions Using EHRs across four countries mapped to the OMOP CDM, we developed generalizable predictive models for POU. Our approach demonstrates the significant impact of cross-site feature selection in improving model performance, underscoring the importance of incorporating diverse feature sets from various clinical settings to enhance the generalizability and utility of predictive healthcare models. Purpose: Incidence of bleeding amongst warfarin and direct oral anticoagulant (DOAC) users is greater following a respiratory tract infection (RTI). It is unclear whether immediate antibiotics modify this association. We estimated the risk of bleeding amongst warfarin and DOAC users with RTI by antibiotic treatment. Methods: This retrospective cohort study used data from the Clinical Practice Research Datalink (CPRD) GOLD for adults in England prescribed warfarin or a DOAC, who sought primary care for an RTI between 1st January 2011 and 31st December 2019. Outcomes were major bleeding (hospital admission for intracranial or gastrointestinal bleeding), and non‐major bleeding (hospital admission or General Practice consult for epistaxis, haemoptysis, or haematuria). Cox models derived hazard ratios (HRs) and 95% confidence intervals (CIs) for each outcome, adjusting for confounders using inverse probability of treatment weighting. Results: Of 14 817 warfarin and DOAC users consulting for an RTI, 8768 (59%) were prescribed immediate antibiotics and 6049 (41%) were not. Approximately 49% were female, and median age was 76 years. Antibiotics were associated with reduced risk of major bleeding (adjusted HR 0.38, 95% CI 0.25 to 0.58). This was consistent across several sensitivity analyses. Antibiotics were also associated with a reduced risk of non‐major bleeding (adjusted HR 0.78, 95% CI 0.61 to 0.99). Conclusions: Immediate antibiotics were associated with reduced risk of bleeding amongst warfarin and DOAC users with an RTI. Further work is needed to understand mechanisms and confirm whether a lower threshold for antibiotic use for RTI in this population may be beneficial. [ABSTRACT FROM AUTHOR]

Over the past decade, polypharmacy has increased dramatically. Measurable harms include falls, fractures, cognitive impairment, and death. The associated costs are massive and contribute substantially to low-value health care. Deprescribing is a promising solution, but there are barriers. Establishing a network to address polypharmacy can help overcome barriers by connecting individuals with an interest and expertise in deprescribing and can act as an important source of motivation and resources. Over the past decade, several deprescribing networks were launched to help tackle polypharmacy, with evidence of individual and collective impact. A network approach has several advantages; it can spark interest, ideas and enthusiasm through information sharing, meetings and conversations with the public, providers, and other key stakeholders. In this special report, the details of how four deprescribing networks were established across the globe are detailed. Networks create links between people who lead existing and/or budding deprescribing practices and policy initiatives, can influence people with a shared passion for deprescribing, and facilitate sharing of intellectual capital and tools to take initiatives further and strengthen impact. Survivors of campus sexual assault are often tasked with healing while sharing an environment with their perpetrators. However, little is known about the effects of a shared environment on survivor well-being. A qualitative thematic analysis design was employed to address this gap. We conducted semi-structured interviews with nine female survivors who identified their perpetrators as fellow students at a large state university in the southeastern U.S. The five themes that emerged are situated within an ecological framework, which is also used to guide our recommendations for supporting survivors' well-being within higher education. In many European cities, railway stations have become urban hubs in which a variety of functions and uses are focused. These include retail, cafés, offices and housing. They are often also complemented by adjacent amenity open space, in both hard and soft landscaping, alongside a broader provision for other modes, such as public transport, walking and cycling. In this sense, large-scale transport hubs resemble small-scale city centres and, like other urban centres, the public space in stations should meet the needs of a wide range of users. The design and management of railway stations is highly complex and the risk of such nodes failing as quality public spaces is significant. In addition, railway stations play a pivotal role in encouraging public transport use and they have the potential to contribute to a modal shift from more polluting transport modes. We argue that the design and management of such spaces is a key variable in the modal choices many travellers make. Based on empirical research across Europe, this paper presents a set of indicators for assessing the quality of railway stations and their adjacent areas and summarizes our research findings to increase the attractiveness of rail journeys through enhanced public spaces. As a chronic inflammatory disease of the central nervous system, multiple sclerosis (MS) is of great individual health and socio-economic significance. To date, there is no prognostic model that is used in routine clinical care to predict the very heterogeneous course of the disease. Despite several research groups working on different prognostic models using traditional statistics, machine learning and/or artificial intelligence approaches, the use of published models in clinical decision making is limited because of poor model performance, lack of transferability and/or lack of validated models. To provide a systematic overview, we conducted a "Cochrane review" that assessed 75 published prediction models using relevant checklists (CHARMS, PROBAST, TRIPOD). The article discusses the New Voices program established by the US National Academies of Sciences, Engineering, and Medicine (NASEM) in 2018. The program aims to bring diverse and innovative perspectives from early- and mid-career leaders to important discussions on how science, engineering, and medicine shape the global future. The program selects members through a competitive application process and supports them in enhancing scientific and policy solutions to national and global challenges. The article emphasizes the interdisciplinary nature of the program and the importance of inclusivity and diversity in scientific research and policymaking. The program has initiated various projects, including the development of an AI chatbot for organizing and sharing public health information, examining the impact of climate change on engineering design, and promoting science diplomacy and diversity in STEM fields. The program has also provided opportunities for members to engage in NASEM activities and global events, contributing to their career advancement and the advancement of science. Co-design across the Exascale Computing Project has been critical for both enabling science applications and bringing disparate communities together. Developing and porting applications to the various high-performance computing architectures on pre-exascale and exascale computers has been quite challenging due to the diversity of hardware features and software stacks. The Co-design Center for Particle Applications (CoPA) has developed and enhanced the Cabana and Parallel, Rapid O(N), and Graph-Based Recursive Electronic Structure Solver (PROGRESS)/Basic Matrix Library (BML) libraries to facilitate the creation of new particle applications, make existing particle applications exascale capable, and allow teams to explore new capabilities. Particle methods from the atomistic, mesoscale, and continuum through cosmological scales have been built with Cabana, along with new possibilities for application coupling. Similarly, the PROGRESS/BML library has enabled quantum particle applications with linear algebra solvers to use advanced hardware. We present a general framework for dealing with set heterogeneity in data and learning problems, which is able to exploit low complexity components. The main ingredients are (i) A definition of complexity for elements of a convex union that takes into account the complexities of their individual composition – this is used to cover the heterogeneous convex union; and (ii) Upper bounds on the complexities of restricted subsets. We demonstrate this approach in two different application areas, highlighting their conceptual connection. (1) In random projection based dimensionality reduction, we obtain improved bounds on the uniform preservation of Euclidean norms and distances when low complexity components are present in the union. (2) In statistical learning, our generalisation bounds justify heterogeneous ensemble learning methods that were incompletely understood before. Background: Sleep disturbance is a prevalent condition among people living with dementia (PLwD) or mild cognitive impairment (MCI). Its assessment and management within primary care is complex because of the comorbidities, older age, and cognitive impairment typical of this patient group. Aim: To explore how primary care clinicians assess, understand, and manage sleep disturbance for PLwD or MCI; if and why such initiatives work; and how people and their carers experience sleep disturbance and its treatment. Design and setting: A realist review of existing literature conducted in 2022. Method: Six bibliographic databases were searched. Context–mechanism–outcome configurations (CMOCs) were developed and refined. Results: In total, 60 records were included from 1869 retrieved hits and 19 CMOCs were developed. Low awareness of and confidence in the treatment of sleep disturbance among primary care clinicians and patients, combined with time and resource constraints, meant that identifying sleep disturbance was difficult and not prioritised. Medication was perceived by clinicians and patients as the primary management tool, resulting in inappropriate or long-term prescription. Rigid nursing routines in care homes were reportedly not conducive to good-quality sleep. Conclusion: In primary care, sleep disturbance among PLwD or MCI is not adequately addressed. Over-reliance on medication, underutilisation of non-pharmacological strategies, and inflexible care home routines were reported as a result of low confidence in sleep management and resource constraints. This does not constitute effective and person-centred care. Future work should consider ways to tailor the assessment and management of sleep disturbance to the needs of individuals and their informal carers without overstretching services.
