In 2015, the US Bureau of Labor Statistics reported that 15 million people were self-employed in the United States. This represented about 10% of total employment. Under the Affordable Care Act (ACA), the self-employed without employees are treated as individuals and are subject to the individual mandates of the policy. Less than half of the American states created health exchanges to act as a marketplace for health insurance under the new policy while the remaining states defaulted to the federal exchange. Using Current Population Survey data from 2013 and 2015, this study finds that the ACA increased the likelihood that the self-employed would privately purchase health insurance. Also, states with health exchanges saw an increase in the likelihood that the self-employed would privately purchase insurance as compared with the states that defaulted. This is important for entrepreneurship since benefits, including insurance, is often cited as a reason for individuals to remain employed as wage earners instead of moving into self-employment. Résumé: Depuis le début de la pandémie à Covid-19, Santé Publique France propose des indicateurs à des niveaux d'intérêt administratif. Entre le niveau départemental et la commune, il en existe un autre d'intérêt scientifique : les aires d'attraction des villes. Ce niveau représente en effet la réalité des déplacements, notamment entre le domicile et le travail. Nous montrons ici la faisabilité d'utilisation de cet échelon pour les cartes de Santé Publique France, ses intérêts et ses limites. Pour mieux répondre à la pandémie, il convient d'employer l'échelle la plus proche de la réalité quotidienne et non seulement l'échelle la plus pratique administrativement. Since the start of the Covid-19 pandemic, Santé Publique France has been proposing indicators at levels of administrative interest. Between the departmental level and the commune, there is another one of scientific interest: the urban catchment areas. This level represents the reality of travel, particularly between home and work. We show here the feasibility of using this level for the Santé Publique France maps, its interests and its limits. Objective: To report an in-depth analysis of policy change for integrated community case management of childhood illness (iCCM) in six sub-Saharan African countries. We analysed how iCCM policies developed and the barriers and facilitators to policy change.Methods: Qualitative retrospective case studies drawing from document reviews, semi-structured interviews and in-country validation workshops were conducted in Burkina Faso, Kenya, Malawi, Mali, Mozambique and Niger. These countries were selected to maximise variation in iCCM policy status, community health worker (CHW) models and different African regions.Results: Country iCCM policies evolved in an ad hoc fashion, but were substantially influenced by the history of primary health care and the nature of CHW programmes. Technical officers within Ministries of Health led iCCM policy change with support from international donors, but neither communities nor political leadership was mobilised. Concerns about achieving the Millennium Development Goals, together with recognition of the shortcomings of existing child health programmes, led to the adoption of iCCM policies. Availability of external financing played a critical role in facilitating policy change.Conclusions: iCCM policy change has been promoted by international agencies, but national governments have struggled to align iCCM with country health systems. Greater investment is needed in tailoring global policy initiatives to match country needs. High-level, political ownership of iCCM policies could facilitate policy change, as could clearer strategies for ensuring the long-term sustainability of such policies. [ABSTRACT FROM AUTHOR]

Over the last 20 years or so the comparative study of health policy and services has become a well-established field of research. However, over this same period, health policy and services themselves have undergone significant transformations, and this has changed the business of comparison itself. Comparisons have become more diverse in terms of geographical and substantive scope, in the inclusion of practice in addition to policy, and at the level of analysis with greater interest for sub-national levels. This transformation results in a range of challenges related to ensuring comparability, to comparing beyond the nation-state and to finding appropriate data for comparison. Importantly, although the challenges are not necessarily greater now, they are different ones. Across the globe the concept of gender mainstreaming is indicative of substantive transformations, and healthcare is a particularly important policy arena. Yet existing research reveals only modest success in the implementation of gender policies in national healthcare systems, despite the availability of complex tools and guidelines. This article introduces an approach that links gender mainstreaming with approaches into policy transfer as dynamic processes of translation involving active players. In a scoping exercise the authors select England and Germany as case studies and draw on document analysis, other secondary sources and additional expert information. The analysis reveals varieties of translation of gender mainstreaming into national healthcare systems – even within the legal framework of the European Union – and the crucial relevance of feminist actors. The Japanese government has promoted the development of a home care system that integrates the provision of health and social care services and links formal and informal care. Yet the coordination of different forms of care faces a number of challenges. This article introduces a micro-sociological approach focusing on the relevance of ‘mutual aid’ in local communities as a guide to practical action. The article explores interactions, communication and coordination in multidisciplinary care delivery using a qualitative approach focusing on a region of the country with advanced integrated home care as a case study. The findings highlight the relevance of ‘mutual aid’ for integrated home care. Across countries healthcare systems face new pressures for change towards more cost-effective and sustainable health services for all citizens. In recent years health policy-makers have responded to these challenges mainly by introducing or reinforcing market mechanisms and by expanding managerial controls. This is leading to major transformations in health policy and, subsequently, to the organization and delivery of healthcare services. This collection brings an international perspective to the research on contemporary transformations in health policy and services, by focusing on countries as wide-ranging as the US and Western and Eastern European countries, to Japan, China and Australia and by addressing different healthcare sectors from hospital to home care. Together these international experiences help us to better understand both the risks of social inequality embedded in new health policies and the opportunities of mobilizing new resources towards better healthcare for all citizens. Background Despite a broadening consensus about the effectiveness of intermittent preventive treatment (IPTp) in preventing the adverse outcomes of malaria during pregnancy, policy change to IPTp was initially limited to East Africa. In West Africa, where the policy change process for the prevention of malaria during pregnancy started much later, IPTp has been taken up swiftly. Objective To describe the factors that contributed to the rapid adoption of policies to prevent malaria during pregnancy in West Africa. Results and Conclusion Several factors appear to have accelerated the process: (1) recognition of the extent of the problem of malaria during pregnancy and its adverse consequences; (2) a clear, evidence-based program strategy strongly articulated by an important multilateral organization (World Health Organization); (3) subregionally generated evidence to support the proposed strategy; (4) a subregional forum for dissemination of data and discussion regarding the proposed policy changes; (5) widespread availability of the proposed intervention drug (sulfadoxine–pyrimethamine); (6) technical support from reputable and respected institutions in drafting new policies and planning for implementation; (7) donor support for pilot experiences in integrating proposed policy change into a package of preventive services; and (8) financial support for scaling up the proposed interventions.
