Observations of high supersaturations with respect to ice inside cirrus clouds with high ice water content (> 0.01 g kg-1) and high crystal number densities (> 1 cm-3) are challenging our understanding of cloud microphysics and of climate feedback processes in the upper troposphere. However, single measurements of a cloudy air mass provide only a snapshot from which the persistence of ice supersaturation cannot be judged. We introduce here the "cirrus match technique" to obtain information about the evolution of clouds and their saturation ratio. The aim of these coordinated balloon soundings is to analyze the same air mass twice. To this end the standard radiosonde equipment is complemented by a frost point hygrometer, "SnowWhite", and a particle backscatter detector, "COBALD" (Compact Optical Backscatter AerosoL Detector). Extensive trajectory calculations based on regional weather model COSMO (Consortium for Small-Scale Modeling) forecasts are performed for flight planning, and COSMO analyses are used as a basis for comprehensive microphysical box modeling (with grid scale of 2 and 7 km, respectively). Here we present the results of matching a cirrus cloud to within 2-15 km, realized on 8 June 2010 over Payerne, Switzerland, and a location 120 km downstream close to Zurich. A thick cirrus cloud was detected over both measurement sites. We show that in order to quantitatively reproduce the measured particle backscatter ratios, the small-scale temperature fluctuations not resolved by COSMO must be superimposed on the trajectories. The stochastic nature of the fluctuations is captured by ensemble calculations. Possibilities for further improvements in the agreement with the measured backscatter data are investigated by assuming a very slow mass accommodation of water on ice, the presence of heterogeneous ice nuclei, or a wide span of (spheroidal) particle shapes. However, the resulting improvements from these microphysical refinements are moderate and comparable in magnitude with changes caused by assuming different regimes of temperature fluctuations for clear-sky or cloudy-sky conditions, highlighting the importance of proper treatment of subscale fluctuations. The model yields good agreement with the measured backscatter over both sites and reproduces the measured saturation ratios with respect to ice over Payerne. Conversely, the 30% in-cloud supersaturation measured in a massive 4 km thick cloud layer over Zurich cannot be reproduced, irrespective of the choice of meteorological or microphysical model parameters. The measured supersaturation can only be explained by either resorting to an unknown physical process, which prevents the ice particles from consuming the excess humidity, or -- much more likely -- by a measurement error, such as a contamination of the sensor housing of the SnowWhite hygrometer by a precipitation drop from a mixed-phase cloud just below the cirrus layer or from some very slight rain in the boundary layer. Atmospheric temperature and humidity profiles are important for weather prediction, but climate change has increased the interest in upper-air observations asking for very high-quality reference measurements. This paper discusses an experimental approach to determine the radiation-induced error on radiosonde air temperature measurements. On the one hand, solar shortwave and thermal longwave radiation profiles were accurately measured during radiosonde ascents from the surface to 35-km altitude. On the other hand, air temperature was measured with several thermocouples on the same flight, simultaneously under sun-shaded and unshaded conditions. The radiation experiments reveal that thermal radiation errors on the very thin thermocouple of the Meteolabor SRS-C34 radiosonde are similar during night- and daytime. They produce a radiative cooling in the lower troposphere and the upper stratosphere, but a radiative heating in the upper troposphere and lower stratosphere. Air temperature experiments with several thermocouples, however, show that solar radiation produces a radiative heating of about +0.2°C near the surface, which linearly increases to about +1°C at 32 km (~10 hPa). The new solar radiation error profile was then applied to SRS-C34 measurements made during the Eighth WMO Intercomparison of High Quality Radiosonde Systems, held in Yangjiang, China, in July 2010. The effects of thermal and solar radiation errors are finally shown in contrast to the 10 other internationally used radiosonde systems, which were flown during this international campaign. To better understand variability in weather and climate, it is vital to address past atmospheric circulation. This need requires meteorological information not just from the surface but also at upper levels. Current global upper-level datasets only reach back to the 1940s or 1950s and do not cover some important periods in the first half of the twentieth century. Extending the observational record is therefore considered important in order to analyze climate variability in the past and verify global climate models used to predict future climate change. Although earlier upper-air data from platforms such as radio-sondes, aircraft, pilot balloons, registering balloons, and kites are available from various sources, no systematic compilation and quality assessment of upper-level data prior to the International Geophysical Year (1957/58) has ever been performed. Here we present the Comprehensive Historical Upper-Air Network (CHUAN). It is a consistent global historical upper-air dataset that has been derived from heterogeneous data available from various sources as well as from newly digitized data. This paper describes the CHUAN dataset, the metadata, the quality control procedures, and the relationship to existing datasets. Some examples are given of its usefulness for analyzing weather and climate during the first half of the twentieth century. The CHUAN dataset comprises 3987 station records worldwide or about 16.4 million profiles (of which 12.6 million are before 1958 and 5.3 million, primarily from pilot balloons, are before 1948). We recently described the neuroimaging and clinical findings in 6 children with cerebellar clefts and proposed that they result from disruptive changes following prenatal cerebellar hemorrhage. We now report an additional series of 9 patients analyzing the clinical and neuroimaging findings. The clefts were located in the left cerebellar hemisphere in 5 cases, in the right in 3, and bilaterally in one child who had bilateral cerebellar hemorrhages as a preterm infant at 30 weeks gestation. In one patient born at 24 weeks of gestation a unilateral cerebellar hemorrhage has been found at the age of 4 months. Other findings included disordered alignment of the folia and fissures, an irregular gray/white matter junction, and abnormal arborization of the white matter in all cases. Supratentorial abnormalities were found in 4 cases. All but 2 patients were born at term. We confirm the distinct neuroimaging pattern of cerebellar clefts. Considering the documented fetal cerebellar hemorrhage in our first series, we postulate that cerebellar clefts usually represent residual disruptive changes after a prenatal cerebellar hemorrhage. Exceptionally, as now documented in 2 patients, cerebellar clefts can be found after neonatal cerebellar hemorrhages in preterm infants. The short-term outcome in these children was variable. Integrated Water vapour (IWV) has been measured since 1994 by the TROWARA microwave radiometer in Bern, Switzerland. Homogenization techniques were used to identify and correct step changes in IWV related to instrument problems. IWV from radiosonde, GPS and sun photometer (SPM) was used in the homogenisation process as well as partial IWV columns between valley and mountain weather stations. The average IWV of the homogenised TROWARA time series was 14.4mm over the 1996-2007 period, with maximum and minimum monthly average values of 22.4mm and 8mm occurring in August and January, respectively. A weak diurnal cycle in TROWARA IWV was detected with an amplitude of 0.32 mm, a maximum at 21:00UT and a minimum at 11:00 UT. For 1996- 2007, TROWARA trends were compared with those calculated from the Payerne radiosonde and the closest ECMWF grid point to Bern. Using least squares analysis, the IWV time series of radiosondes at Payerne, ECMWF, and TROWARA showed consistent positive trends from 1996 to 2007. The radiosondes measured an IWV trend of 0.45±0.29%/y, the TROWARA radiometer observed a trend of 0.39±0.44%/y, and ECMWF operational analysis gave a trend of 0.25±0.34%/y. Since IWV has a strong and variable annual cycle, a seasonal trend analysis (Mann-Kendall analysis) was also performed. The seasonal trends are stronger by a factor 10 or so compared to the full year trends above. The positive IWV trends of the summer months are partly compensated by the negative trends of the winter months. The strong seasonal trends of IWV on regional scale underline the necessity of long-term monitoring of IWV for detection, understanding, and forecast of climate change effects in the Alpine region. Seasonal Mann Kendall analysis revealed a significant positive trend in July of +0.19 (0.14) to +0.34 (0.25)mm/y for all three datasets. In December, the Payerne radiosonde showed a non-significant negative trend of -0.12 (0.13)mm/y, whilst ECMWF and TROWARA indicated a significant negative trend of -0.20 (0.14) and -0.36 (0.24)mm/y, respectively. The STARTWAVE (STudies in Atmospheric Radiative Transfer and Water Vapour Effects) project aims to investigate the role which water vapour plays in the climate system, and in particular its interaction with radiation. Within this framework, an ongoing water vapour database project was set up which comprises integrated water vapour (IWV) measurements made over the last ten years by ground-based microwave radiometers, Global Positioning System (GPS) receivers and sun photometers located throughout Switzerland at altitudes between 330 and 3584 m. At Bern (46.95° N, 7.44° E) tropospheric and stratospheric water vapour profiles are obtained on a regular basis and integrated liquid water, which is important for cloud characterisation, is also measured. Additional stratospheric water vapour profiles are obtained by an airborne microwave radiometer which observes large parts of the northern hemisphere during yearly flight campaigns. The database allows us to validate the various water vapour measurement techniques. Comparisons between IWV measured by the Payerne radiosonde with that measured at Bern by two microwave radiometers, GPS and sun photometer showed instrument biases within ±0.5 mm. The bias in GPS relative to sun photometer over the 2001 to 2004 period was -0.8mm at Payerne (46.81° N, 6.94° E, 490 m), which lies in the Swiss plains north of the Alps, and +0.6mm at Davos (46.81° N, 9.84° E, 1598 m), which is located within the Alps in the eastern part of Switzerland. At Locarno (46.18° N, 8.78° E, 366 m), which is located on the south side of the Alps, the bias is +1.9 mm. The sun photometer at Locarno was found to have a bias of -2.2mm (13% of the mean annual IWV) relative to the data from the closest radiosonde station at Milano. This result led to a yearly rotation of the sun photometer instruments between low and high altitude stations to improve the calibrations. In order to demonstrate the capabilites of the database for studying water vapour variations, we investigated a front which crossed Switzerland between 18 November 2004 and 19 November 2004. During the frontal passage, the GPS and microwave radiometers at Bern and Payerne showed an increase in IWV of between 7 and 9 mm. The GPS IWV measurements were corrected to a standard height of 500m, using an empirically derived exponential relationship between IWV and altitude. A qualitative comparison was made between plots of the IWV distribution measured by the GPS and the 6.2µm water vapour channel on the Meteosat Second Generation (MSG) satellite. Both showed that the moist air moved in from a northerly direction, although the MSG showed an increase in water vapour several hours before increases in IWV were detected by GPS or microwave radiometer. This is probably due to the fact that the satellite instrument is sensitive to an atmospheric layer at around 320 hPa, which makes a contribution of one percent or less to the IWV. We report on seven children (five males, two females) who presented with marked, often asymmetrical, toe-walking from onset of independent walking, associated with abnormal foot postures and increased tone at the ankles with characteristics of dystonia. Most of the children had presented with unusual pre-walking locomotion and a mild delay in independent walking. They did not fit into the usual categories of 'habitual' toe-walking or congenital short tendo calcaneus but nor did they have the clinical signs of spastic diplegia or of a peripheral neuromuscular disease. Normalization occurred progressively in the second to fourth years of life. The children were re-examined several years later (1 to 11y) and were normal. We believe that their persistent toe-walking corresponded to a variant of 'transient focal dystonia of infancy'. Knowledge of its existence may justify a period of observation without special investigations, surgery, or casting. [ABSTRACT FROM AUTHOR]

Respiratory syncytial virus (RSV) lower respiratory tract infections are considered to be a serious disease in centres such as the Sophia Children's Hospital (Rotterdam, the Netherlands), but as more benign infections in others such as the Geneva Children's Hospital (Switzerland). To assess the clinical severity of RSV infections at the two sites, 151 infants primarily admitted with a virologically confirmed RSV infection were studied prospectively (1994-5) and retrospectively (1993-4) (55 infants in Geneva and 96 in Rotterdam). Parameters of RSV morbidity which were more severe in Rotterdam during the two winter seasons were apnoea (1.8 v 23.9%), the rate of admission to the intensive care unit (3.6 v 28.1%), mechanical ventilation (0 v 7.3%), and length of stay in hospital (6.8 v 9.1 days). In Geneva higher respiratory rates (59.2 v 51.2), more wheezing (65.5 v 28.8%), and more retractions (81.8 v 63.3%) were recorded. Fewer infants younger than 4 months (54.9 v 68.7%), but more breast fed infants (94.1 v 38.5%), were admitted in Geneva, although the morbidity parameters remained different after correction for these two variables in multivariate analyses. Thus unidentified local factors influence the pattern and severity of RSV infection and may affect the results of multicentre prophylactic and therapeutic studies. [ABSTRACT FROM AUTHOR]

A newborn female born at term was admitted at 28 hours for seizures and generalized hypotonia. Cerebral ultrasound showed a right temporal echogenic lesion confirmed on MRI and thought to be secondary to thrombosis of the vein of Labbé. The EEG showed epileptic discharges over the right temporal region. Extensive thrombotic studies revealed a transiently decreased PTT consistent with a prothrombotic state. The hypotonia did not resolve after the acute phase as expected, raising the possibility of another underlying cause. Because of a peculiar phenotype with almond-shaped eyes and bitemporal depression, Prader-Willi syndrome (PWS) was suspected. Methylation analysis confirmed PWS, FISH analysis excluded a deletion in 15q11-q13, maternal uniparental disomy (UPD) was confirmed.
