In this paper, we propose a novel contour search algorithm (CSA) for image processing. Its efficacy is evaluated through a comparative analysis with established techniques such as Canny Edge and Snakes: Active contour models, utilizing infrared thermography (IRT) images. Based on the new CSA, we investigate the influence of different pulse shapes on the IRT post-processing methodology, particularly focusing on the thermal shock response spectrum (TSRS), using two different heat sources: xenon flash lamps and a laser. Moreover, this allows for a more precise quantitative comparison of the TSRS with existing IRT post-processing techniques, including pulse phase thermography and thermal signal reconstruction, concerning the detection of defects in composite materials, particularly in carbon fiber-reinforced polymer. A quantitative comparison was performed using the Tanimoto criterion and signal-to-noise ratio. A more detailed analysis is conducted to identify inherent limitations and potential benefits of the new TSRS methodology. We further investigate and experimentally confirm our previous finding on the qualitative correlation between the one-dimensional thermal N-layer model and test data from the TSRS optimization process for defect determination. This correlation can eliminate the time-consuming optimization step, making TSRS a more attractive alternative to common IRT methods and enhancing the quantitative description of defects. Social media users are frequently targeted by commercial and political advertisements, which many perceive as a privacy threat. Although commercial and political targeting intend to influence users in different ways, comparative studies about user perceptions and responses to different kinds of targeting are scarce. Moreover, notwithstanding the legal obligation to make people aware of targeting in the European Union, current transparency cues do not immediately inform about the use of personal data. The present preregistered between-subjects online experiment (N = 1,248) investigates social media users' privacy perceptions and reactions to commercial and political targeting using an expanded protection motivation theory approach. Results revealed that more salient transparency cues can increase users' perception of ad manipulativeness, which in turn positively relates to privacy protection motivation. Perceiving privacy risks from targeting and protecting against it as effective positively relates to both protection motivation and desired legal regulation of targeting. This article examines how collective bargaining through unions and workplace codetermination through works councils relate to labor market imperfections and how labor market imperfections relate to employer wage premia. Based on representative German plant data for the years 1999–2016, the authors document that 70% of employers pay wages below the marginal revenue product of labor and 30% pay wages above that level. Findings further show that the prevalence of wage markdowns is significantly smaller when organized labor is present, and that the ratio of wages to the marginal revenue product of labor is significantly larger. Finally, the authors document a close link between labor market imperfections and mean employer wage premia, that is, wage differences between employers corrected for worker sorting. SUMMARY: Plant height can be an indicator of plant health across environments and used to identify superior genotypes. Typically plant height is measured at a single timepoint when plants reach terminal height. Evaluating plant height using unoccupied aerial vehicles allows for measurements throughout the growing season, facilitating a better understanding of plant‐environment interactions and the genetic basis of this complex trait. To assess variation throughout development, plant height data was collected from planting until terminal height at anthesis (14 flights 2018, 27 in 2019, 12 in 2020, and 11 in 2021) for a panel of ~500 diverse maize inbred lines. The percent variance explained in plant height throughout the season was significantly explained by genotype (9–48%), year (4–52%), and genotype‐by‐year interactions (14–36%) to varying extents throughout development. Genome‐wide association studies revealed 717 significant single nucleotide polymorphisms associated with plant height and growth rate at different parts of the growing season specific to certain phases of vegetative growth. When plant height growth curves were compared to growth curves estimated from canopy cover, greater Fréchet distance stability was observed in plant height growth curves than for canopy cover. This indicated canopy cover may be more useful for understanding environmental modulation of overall plant growth and plant height better for understanding genotypic modulation of overall plant growth. This study demonstrated that substantial information can be gained from high temporal resolution data to understand how plants differentially interact with the environment and can enhance our understanding of the genetic basis of complex polygenic traits. Significance Statement: Unmanned aerial vehicles were used to collect high temporal resolution morphological data of maize plants and model growth curves of genetically diverse inbred lines. These growth curves provide a more complete understanding of how plants differentially interact with the environment. Although public–private collaboration abounds in addressing societal grand challenges, little is known about its performance relative to private-sector collaboration. Drawing from the innovation collaboration literature, and qualitative insights from interviews and contracts, we argue that collaboration risk impairs task performance of public–private versus private–private collaboration. We attribute this to two collaboration risk mechanisms: cooperation problems arising from misaligned social versus economic incentives, and coordination problems due to differences in organizational governance and processes. We test our hypotheses using the development history of 2,496 antimicrobial drugs from 1995–2019. After accounting for endogenous selection into collaboration, we find that public–private collaboration is less effective than private–private collaboration. However, this performance gap reduces for innovation tasks under high technological uncertainty (i.e., in the discovery stage and for developing new drug action mechanisms). To probe the distinctive cooperation and coordination mechanisms, we also examine innovation task efficiency. Findings reveal a wider gap in task effectiveness but not efficiency between public–private and private–private collaboration, suggesting more significant cooperation than coordination problems. Our theoretical and practical insights pertain to whether, when, and how public–private and private–private collaboration offer viable pathways for socially beneficial innovation tasks. Targeted political advertising (TPA) on social media builds on tailoring messages to (groups of) individuals' characteristics based on user data. Questions have been raised about the impact of TPA on recipients and society. In this study, we focus on the fit of TPA, that is, the congruence between TPA and recipients' preferences, and draw on congruity theories, social identity theory (SIT), and persuasion knowledge. In a two-wave panel study (N = 428) during a Viennese state election, we investigated the relationships between individuals' perceived fit and misfit of TPA on perceptions about the manipulative intent as well as the benefits and harms for democracy. The findings showed that perceived fit of TPA at Time 1 decreased perceived manipulative intent and increased perceived benefits of TPA at Time 2. The perceived misfit of TPA at Time 1 did not influence individuals' perceptions at Time 2, and perceptions about the harms of TPA to democracy stayed stable. Findings imply that political campaigners might benefit from targeting but raise questions about individuals' defense mechanisms against the persuasive technique. [ABSTRACT FROM AUTHOR]

People with social anxiety disorder tend to interpret ambiguous social information in a negative rather than positive manner. Such interpretation biases may cause and maintain anxiety symptoms. However, there is considerable variability in the observed effects across studies, with some not finding a relationship between interpretation biases and social anxiety. Poor psychometric properties of interpretation bias measures may explain such inconsistent findings. We evaluated the internal consistency, test–retest reliability, convergent validity, and concurrent validity of four interpretation bias measures, ranging from more implicit and automatic to more explicit and reflective: the probe scenario task, the recognition task, the scrambled sentences task, and the interpretation and judgmental bias questionnaire. Young adults (N = 94) completed interpretation bias measures in two sessions separated by one week. Psychometric properties were poor for the probe scenario and not acceptable for the recognition task. The reliability of the scrambled sentences task and the interpretation and judgmental bias questionnaire was good, and they correlated highly with social anxiety and each other, supporting their concurrent and convergent validity. However, there are methodological challenges that should be considered when measuring interpretation biases, even if psychometric indices suggest high measurement validity. We also discuss likely reasons for poor psychometric properties of some tasks and suggest potential solutions to improve the assessment of implicit and automatic biases in social anxiety in future research. Summary: The purpose of this study was to determine the real-world incidence and predictors of additional vertebroplasty or balloon kyphoplasty after initial vertebral augmentation, as a proxy for subsequent symptomatic vertebral fracture. Of patients, 15.5% underwent subsequent vertebral augmentation. The patient's comorbidities are strongly associated with risk of subsequent treatment. Purpose: To determine the real-world incidence and predictors of additional vertebroplasty or balloon kyphoplasty after initial vertebral augmentation, as a proxy for subsequent symptomatic and disabling vertebral fracture. Methods: We conducted a retrospective cohort study using commercial insurance claims data (Optum's de-identified Clinformatics® Data Mart Database). Adult patients who underwent subsequent treatment for vertebral fracture within 24 months of initial balloon kyphoplasty (BKP) or vertebroplasty (VP) were classified into "subsequent treatment" or "no subsequent treatment" cohorts. Survival analysis was applied to investigate the effect of risk factors on subsequent treatment. Results: Between 1 January 2008 and 30 June 2020, a total of 32,513 adult patients underwent a BKP/VP procedure following a diagnosis of vertebral compression fracture in the preceding 12 months. Five thousand thirty-five patients (15.5%) underwent a subsequent BKP/VP treatment within 2 years; 90% had a single fracture level treated. An increased hazard of subsequent treatment was associated with a number of fractures treated at initial BKP/VP (≥ 4 levels, adjusted hazard ratio (AHR) 1.68 (95% CI 1.24–2.26); steroid use, AHR 1.9 (95% CI 1.31–1.48); Elixhauser Comorbidity Index ≥ 4, AHR 1.44 (95% CI 1.17–1.77); and multiple myeloma, AHR 1.31 (95% CI 1.13–1.53)). Age < 70 years was associated with reduced hazard of subsequent treatment (AHR 0.81, 95% CI 0.74–0.89). Conclusions: One in seven patients underwent subsequent treatment for vertebral fracture after initial vertebral augmentation. Baseline patient characteristics were associated with increased risk of subsequent fracture within 2 years, suggesting that a patient's natural history is strongly associated with risk of subsequent treatment rather than the initial surgical procedure itself. Background and Objective: Cenobamate is a novel anti-seizure medication (ASM) with unusually high responder rates even in patients with refractory epilepsy. Due to its enzyme-inducing properties, cenobamate could negatively affect bone metabolism, similar to other ASMs; however, effects of long-term cenobamate treatment on bone health have not yet been investigated. The aim of this longitudinal observational study was to assess the effects of 1 year of continuous, adjunctive cenobamate treatment on bone health in patients with drug-resistant, focal epilepsy. Methods: Adult patients from a tertiary epilepsy centre received cenobamate add-on to their concomitant anti-seizure medication. Bone mineral density at femoral neck and lumbar spine, as well as bone formation biomarkers, electrolytes and liver enzymes in serum were assessed at baseline and after 12 months of continuous cenobamate therapy. Results: Forty-seven patients (29 male, median age 40 years) were included in the study. Median daily dose of cenobamate at 12 months was 250 mg. Moderate, yet statistically significant reduction of the T-score at femoral neck but not lumbar spine was found after 1 year of cenobamate treatment, also in a subgroup of patients (n = 37) without enzyme inducers in the comedication. Additionally, we observed statistically significant changes in bone formation biomarkers: decreased serum level of osteocalcin and increased bone-specific alkaline phosphatase. Bone minerals (calcium and phosphorus) as well as vitamin D3 remained unchanged. Parathormone was statistically significantly reduced. There was a highly statistically significant increase in serum gamma-glutamyl transferase (GGT) levels after 12 months of treatment, reflecting an underlying hepatic enzyme induction by cenobamate. Conclusion: A statistically significant decrease of the T-score at femoral neck, as well as prominent alterations in the bone formation biomarkers, suggest an increase in bone turnover after 1 year of cenobamate treatment. The underlying mechanism is most likely attributed to the hepatic enzyme activation, indicated by a prominent elevation of serum GGT. Evidence for folate's protective effects on neural tube defects led the USA and Chile to start mandatory folic acid (FA) fortification programs, decreasing up to 50%. However, 30% of the population consuming fortified foods reach supraphysiologic serum levels. Although controversial, several epidemiological and clinical observations suggest that folate increases cancer risk, giving concern about the risks of FA supplementation. The Cancer stem cells (CSCs) model has been used to explain survival to anticancer therapies. The Notch-1 pathway plays a role in several cancers and is associated with the stemness process. Different studies show that modulation of metabolic pathways regulates stemness capacity in cancer. Supraphysiologic concentrations of FA increase the proliferation of HT-29 cells by Notch-1 activation. However, whether folate can induce a stemness-like phenotype in cancer is not known. We hypothesized that FA protects from glucose deprivation-induced cell death through Notch-1 activation. HT-29 cells were challenged with glucose deprivation at basal (20 nM) and supraphysiological (400 nM) FA and 5–MTHF concentrations. We analyzed changes in stemness-like gene expression, cell death and different energetic metabolic functions. Supraphysiological concentrations of FA increased stemness-like genes, and improved survival and oxygen consumption, inducing AMPK phosphorylation and HSP-70 protein expression. We evaluated the Notch-1 pathway using the DAPT and siRNA as inhibitors, decreasing the stemness-like gene expression and preventing the FA protection against glucose deprivation-induced cell death. Moreover, they decreased oxygen consumption and AMPK phosphorylation. These results suggest that FA protects against glucose deprivation. These effects were associated with AMPK activation, a critical metabolic mediator in nutrient consumption and availability that activates the Notch-1 pathway. [ABSTRACT FROM AUTHOR]

Transitional care management (TCM) visits have been shown to reduce 30-day readmissions, but it is unclear whether the decrease arises from the TCM visit itself or from clinic-level changes to meet the requirements of the TCM visits. We conducted a cross-sectional analysis using data from Northwell Health to examine the association between the type of post-discharge follow-up visits (TCM visits versus non-TCM visits based on billing) and 30-day readmission. Furthermore, we assessed whether being seen by a provider who frequently utilizes TCM visits or the TCM visit itself was associated with 30-day readmission. We included adult patients hospitalized to Medicine service and subsequent follow-up visits within two weeks of discharge between February 24, 2018, and February 24, 2020. We examined 1) post-discharge follow-up visit type (TCM visit versus non-TCM visit) and 2) provider characteristics (frequent TCM visit utilization or not). The primary outcome was unplanned hospital readmission within 30 days following hospital discharge. After propensity matching, TCM follow-up visits were associated with decreased 30-day readmissions (hazard ratio = 0.74 [0.63–0.88]) compared to non-TCM visits. Among patients with non-TCM follow-up visits, those seen by a provider who frequently used TCM visits had decreased odds (OR = 0.84 [0.71–0.99]) of 30-day readmission compared to those seen by providers who did not use TCM visits regularly. Among patients who followed up with providers who frequently use TCM visits, TCM visits were associated with decreased 30-day readmission compared to patients with non-TCM visits (OR = 0.78 [0.62–0.98]). The study has limitations, including the health system database not capturing all out-of-network follow-up visits. The reduction in 30-day readmission associated with TCM visits likely arises from both the visit itself and being seen by a provider who frequently uses TCM visits. [ABSTRACT FROM AUTHOR]

Background: Papillary renal neoplasm with reverse polarity is a recently recognized low‐grade neoplasm with a favorable prognosis. To date, its cytologic features have not been well documented. Methods: Two patients with papillary renal neoplasm with reverse polarity sampled by fine needle aspiration and core needle biopsy are described, one of whom is under active surveillance without clinical progression and the other is alive and well 16 years after partial nephrectomy. Results: The cytologic features included a mix of papillae and dispersed cells with abundant oncocytic cytoplasm and round, bland nuclei apically displaced away from the papillary core. Immunohistochemistry showed positive staining for GATA3 in both cases. Molecular studies on one of the cases showed a KRAS p.G12V mutation. Conclusions: The cytologic features of this distinctive, indolent neoplasm are important to recognize because patients with papillary renal neoplasm with reverse polarity may be excellent candidates for partial nephrectomy or even active surveillance. This study describes the cytologic features of papillary renal neoplasm with reverse polarity, a recently recognized low‐grade neoplasm with a favorable prognosis. The cytologic features of this indolent neoplasm are important to recognize because a patient with papillary renal neoplasm with reverse polarity may be an excellent candidate for active surveillance. Purpose: The purpose of this qualitative study was to examine the perceptions of communication sciences and disorders (CSD) assistant professors in the United States related to barriers and facilitators to engaging in open science practices and identify opportunities for improving open science training and support in the field. Method: Thirty-five assistant professors (16 from very high research activity [R1] institutions, 19 from institutions with other Carnegie classifications) participated in one 1-hr virtual focus group conducted via Zoom recording technology. The researchers used a conventional content analysis approach to analyze the focus group data and develop categories from the discussions. Results: Five categories were developed from the focus groups: (a) a desire to learn about open science through opportunities for independent learning and learning with peers; (b) perceived benefits of engaging in open science on assistant professors' careers, the broader scientific community, and the quality of research in the field of CSD; (c) personal factors that act as barriers and/or facilitators to engaging in open science practices; (d) systemic factors that act as barriers and/or facilitators to engaging in open science practices; and (e) differences in perceptions of R1 and non-R1 assistant professors. Conclusions: Assistant professors in CSD perceive benefits of open science for their careers, the scientific community, and the field. However, they face many barriers (e.g., time, lack of knowledge and training), which impede their engagement in open science practices. Preliminary recommendations for CSD assistant professors, academic institutions, publishers, and funding agencies are provided to reduce barriers to engagement in open science practices. Background: In this study, we expand the supportive evidence for total disc arthroplasty (TDA) with results up to 21 years in a large patient cohort who received a semiconstrained ball-and-socket lumbar prosthesis. The objectives of the study were to compare the results for 1 versus 2-level surgeries and to evaluate whether prior surgery at the index level(s) impacts clinical outcomes. Methods: From 1999 to 2013, 1,187 patients with chronic lumbar degenerative disc disease (DDD) underwent lumbar TDA, of whom 772 underwent a 1-level procedure and 415 underwent a 2-level procedure. A total of 373 (31.4%) of the 1,187 patients had prior index-level surgery. Patients were evaluated preoperatively; at 3, 6, 12, 18, and 24 months postoperatively; and yearly thereafter. The follow-up duration ranged from 7 to 21 years (mean, 11 years and 8 months). Collected data included radiographic, neurological, and physical assessments, as well as self-evaluations using the Oswestry Disability Index (ODI) and visual analog scale (VAS) for back and leg pain. Perioperative data points, complication rates, and reoperation or revision rates were also assessed. Patients were divided into 4 groups: 1-level TDA with no prior surgery at the index level, 1-level TDA with prior surgery, 2-level TDA with no prior surgery, and 2-level TDA with prior surgery. Results: All groups showed dramatic reduction in the ODI at 3 months postoperatively and maintained these scores over time. Although VAS pain did not diminish to its final level as rapidly for patients with prior surgery, there was no significant difference between the groups in terms of pain reduction at 24 months postoperatively. Of 1,187 patients, 49 (4.13%) required either a new surgery at another level or revision or reoperation at the index level. Rates were too low in all groups to compare them statistically. Total TDA revision and adjacent-level surgery rates over 7 to 21 years were very low (0.67% and 1.85%, respectively). Conclusions: This study demonstrates the robust long-term clinical success of 1 and 2-level lumbar TDA as assessed at 7 to 21 years postoperatively in one of the largest evaluated cohorts of patients with TDA. Patients had dramatic and maintained reductions in disability and pain scores over time and low rates of index-level revision or reoperation and adjacent-level surgery relative to published long-term fusion data. Additionally, patients who underwent 1-level lumbar TDA and those who underwent 2-level TDA demonstrated equivalent improvement, as did patients with prior surgery at the index level and those with no prior surgery. The aim of this study was to analyze the demographic, clinical, and hematological aspects of the population in a dog shelter located in the municipality of Lavras, Brazil. All animals were microchipped and evaluated by veterinarians. Whole blood samples were obtained from 329 dogs in the months of July–August 2019 and from 310 dogs in the months of January–February 2020. Most of the dogs were of mixed breed, received anti-rabies and polyvalent vaccines (100%), were dewormed (100%), and were spayed/neutered (98.59%), with a predominance of adult (86.51%), short-hair (67.51%), normal body condition (65.57%), medium-size (62.57%), and female (62.36%). The main clinical alterations detected were enlarged lymph nodes (38.69%), skin lesions (31.50%), overweight (23.32%), obesity (6.07%), elevated temperature (17.05%), and ear secretion (15.72%). Regarding hematological alterations, thrombocytopenia (36.31%), leukopenia (15.92%), anemia with decreased hemoglobin values (10.60%), hematocrit (9.70%), and red blood cells (5.14%) were observed. Most of the shelter dogs were apparently healthy, but specific measures for nutritional, dermatological, otological and disease management should be implemented once the health changes are verified, as they impact the general state of the population and adoptions. The transition to virtual learning resulting from the COVID-19 pandemic drastically altered the work of many teachers. Despite the expansion of remote instruction, more research is needed to demonstrate what works for classroom management during virtual learning. We report the findings from an online survey related to online classroom management. Nearly 300 educators from 30 states completed the survey. We examined special and general education teachers' perceived knowledge, use, and effectiveness of evidence-based classroom management practices translated for remote instruction and report findings related to self-efficacy when managing classrooms online. Results provide empirical support for the application of established classroom management practices in virtual learning contexts. Community-acquired pneumonia (CAP) is often considered for children presenting to the emergency department (ED) with respiratory symptoms. It is unclear how often children are diagnosed with CAP following an ED visit for respiratory illness. We performed a retrospective case-control study to evaluate 7-day CAP diagnosis among children 3 months to 18 years discharged from the ED with respiratory illness from 2011 to 2021 and who receive care at 4 hospital-affiliated primary care clinics. Logistic regression was performed to assess for predictors of 7-day CAP diagnosis. Seventy-four (0.7%, 95% confidence interval [CI] = 0.6%, 0.9%) of 10 329 children were diagnosed with CAP within 7 days, and fever at the index visit was associated with increased odds of diagnosis (odds ratio [OR] = 3.32, 95% CI = 1.75-6.28). Community-acquired pneumonia diagnosis after discharge from the ED with respiratory illness is rare, even among children who are febrile at time of initial evaluation. [ABSTRACT FROM AUTHOR]

Background: Adding functional information by CT-derived fractional flow reserve (FFRct) to coronary CT angiography (CCTA) and assessing its temporal change may provide insight into the natural history and physiopathology of cardiac allograft vasculopathy (CAV) in heart transplantation (HTx) patients. We assessed FFRct changes as well as CAV progression over a 2-year period in HTx patients undergoing serial CT imaging. Methods: HTx patients from Erasmus MC and Mount Sinai Hospital, who had consecutive CCTAs 2 years apart were evaluated. FFRct analysis was performed for both scans. FFRct values at the most distal point in the left anterior descending (LAD), left circumflex (LCX), and right coronary artery (RCA) were measured after precisely matching the anatomical locations in both analyses. Also, the number of anatomical coronary stenoses of > 30% was scored. Results: In total, 106 patients (median age 57 [interquartile range 47–67] years, 67% male) at 9 [6–13] years after HTx at the time of the baseline CCTA were included. Median distal FFRct values significantly decreased from baseline to follow-up for the LAD from 0.85 [0.79–0.90] to 0.84 [0.76–0.90] (p = 0.001), LCX from 0.92 [0.88–0.96] to 0.91 [0.85–0.95] (p = 0.009), and RCA from 0.92 [0.86–0.95] to 0.90 [0.86–0.94] (p = 0.004). The number of focal anatomical stenoses of > 30% increased from a median of 1 [0–2] at baseline to 2 [0–3] at follow-up (p = 0.009). Conclusions: The distal coronary FFRct values in post-HTX patients in each of the three major coronary arteries decreased, and the number of focal coronary stenoses increased over a 2-year period. Temporal FFRct change rate may become an additional parameter in the follow-up of HTx patients, but more research is needed to elucidate its role. Background and Objectives: Noradrenergic dysregulation is important in the pathophysiology of posttraumatic stress disorder (PTSD) and alcohol use disorder (AUD); pharmacotherapies targeting adrenergic function have potential as treatment for comorbidity. Dexmedetomidine (sublingual film formulation—BXCL501; IGALMI) is a highly potent, selective ⍺2‐adrenergic receptor agonist and may be superior to other pharmacotherapeutic approaches. A within subjects, phase 1b safety laboratory study was conducted to evaluate adverse effects of BXCL501 when combined with alcohol; BXCL501's potential efficacy was also explored. Methods: Heavy drinker participants with a diagnosis of or who were at risk for PTSD participated in three separate test days which included pretreatment with BXCL501 (40 µg, 80 µg or placebo) administered in a randomized, double‐blind fashion, followed by three testing conditions: alcohol cue reactivity, trauma‐induced reactivity, and IV ethanol administration. Safety outcomes included blood pressure (BP) and sedation. Exploratory outcomes included alcohol craving, trauma‐induced anxiety and craving and subjective effects of alcohol. Results: Ten of twelve randomized participants competed the entire study. BXCL501 (80 µg) was associated with expected mild changes in BP and sedation; administration with alcohol did not affect those parameters. There were no clinically significant adverse effects. BXCL501 attenuated trauma‐induced anxiety and attenuated subjective effects of alcohol. Discussions and Conclusions: BXCL501 is safe for use in humans who may drink alcohol while undergoing treatment. BXCL501 may be explored as a potential treatment for PTSD and AUD. OBJECTIVE: To evaluate whether continuous glucose monitoring (CGM) could assist providers in intensifying glycemic management in hospitalized patients with type 2 diabetes. RESEARCH DESIGN AND METHODS: At six academic hospitals, adults with type 2 diabetes hospitalized in a non–intensive care setting were randomly assigned to either standard therapy with glucose target 140–180 mg/dL (standard group) or intensive therapy with glucose target 90–130 mg/dL guided by CGM (intensive group). The primary outcome was mean glucose measured with CGM (blinded in standard group), and the key secondary outcome was CGM glucose <54 mg/dL. RESULTS: For the 110 participants included in the primary analysis, mean ± SD age was 61 ± 12 years and mean HbA1c was 8.9 ± 2.3% (73.8 ± 1.6 mmol/mol). During the study, CGM-measured mean glucose was 170 mg/dL for the intensive group (n = 60) vs. 175 mg/dL for the standard group (n = 50; risk-adjusted difference −7 mg/dL, 95% CI −19 to 5; P = 0.25). Only 7% of the intensive group achieved the mean glucose target range of 90–130 mg/dL. CGM readings <54 mg/dL were infrequent (0.2% for intensive and 0.4% for standard; adjusted treatment group difference −0.1%, 95% CI −0.6 to 0.3). One severe hypoglycemia event occurred in the standard group. CONCLUSIONS: The study's glucose management approach using CGM did not improve glucose levels compared with standard glucose management in the non–intensive care unit hospital setting. Background: Training gaps regarding the diagnosis and management of eating disorders in diverse populations, including racial, ethnic, sexual, and gender minoritized groups, have not been thoroughly examined. Objective: This study aimed to examine resident physicians' knowledge and attitudes regarding eating disorders in diverse populations, with a focus on areas for improved training and intervention. Methods: Ninety-two resident physicians in internal medicine, emergency medicine, obstetrics/gynecology, psychiatry, and surgery at an academic center completed an online survey from 12/1/2020–3/1/2021, which comprised multiple choice and vignette-style open-ended questions to assess knowledge and attitudes toward the management and clinical presentations of eating disorders. Overall, the survey response rate was 25.7%. Descriptive statistics were reported. Vignette-style questions were analyzed using inductive coding and the frequency of responses was reported. Results: A minority of resident physicians self-reported confidence in their knowledge of the medical complications (n = 42, 45%), risk factors (n = 38, 41%), and clinical presentations (n = 32, 35%) associated with eating disorders. Responses to vignette-style questions correctly identified relevant management methods (such as electrolyte monitoring and referral to specialty care), but demonstrated limited knowledge of the clinical presentation of eating disorders. Furthermore, most respondents reported a lack of knowledge regarding eating disorders in sexual and gender minoritized patients (n = 68, 73.9%) as well as racial and ethnic minoritized patients (n = 64, 69.6%). Conclusions: Our findings suggest concerning gaps in knowledge and confidence among resident physicians with regard to the diagnosis and treatment of eating disorders, particularly in racial, ethnic, sexual, and gender minoritized patients. Moreover, responses to vignette-like questions indicate significant homogeneity in respondents' perceptions of the clinical presentation of eating disorders, reflecting cultural biases which associate eating disorders with underweight, young, female patients. The majority did not feel competent in treating eating disorders in diverse populations and expressed desire for additional training in this area. More research is needed to better understand and address these gaps in eating disorder training, with the goal of increasing equity in patient outcomes. This study examines how population change is associated with changes in sociodemographics and economic outcomes across diverse geographic contexts in the United States from 2000 to 2020. Using Census Tract-level data and generalized additive models (GAMs), we found that communities experiencing population growth showed significant improvements in socioeconomic indicators: for example, a 50% population increase in Northeast metropolitan non-coastal areas was associated with a $10,062 rise [95% confidence interval (CI) = $9,181, $10,944] in median household income. Conversely, areas with population decline faced increasing challenges to community composition: communities experiencing a 50% population decline in West coastal metropolitan areas saw their median age increase by 2.556 years (95% CI = 2.23, 2.89 years), indicating an accelerated aging population. We observed a positive relationship between population growth and local economic growth, with areas experiencing population decline or slow growth showing below-average economic growth. While population change alone explained 10.1% of the variance in county-level GDP growth, incorporating sociodemographic shifts alongside population change using a partial least squares regression (PLSR) more than doubled the explanatory power to 21.4%. Overall, we often found the strength of relationships and sometimes the direction varied by geographic context: coastal areas showed distinct patterns from inland regions, and metropolitan areas responded differently than rural ones. For instance, the percentage of owner-occupied housing was negatively associated with population growth in metropolitan areas, but positively associated in non-metropolitan areas. Our research provides valuable insights for policymakers and planners working to address community changes, particularly in the context of anticipated climate-induced migration. The results suggest that strategies for maintaining economic vitality need to consider not just population retention, but also demographic profiles and socioeconomic opportunities across different geographic contexts. The application of upconversion nanoparticles (UCNPs) for cell and tissue analysis requires a comprehensive understanding of their interactions with biological entities to prevent toxicity or harmful effects. Whereas most studies focus on cancer cells, this work addresses non-cancerous cells with their regular in vitro physiology. Since it is generally accepted that surface chemistry largely determines biocompatibility in general and uptake of nanomaterials in particular, two bilayer surface coatings with different surface shielding properties have been studied: (i) a phospholipid bilayer membrane (PLM) and (ii) an amphiphilic polymer (AP). Both surface modifications are applied to (12–33) nm core-shell UCNPs NaYF4(Yb, Er)@NaYF4, ensuring colloidal stability in biological media. The impact of UCNPs@AP and UCNPs@PLM on non-cancerous epithelial-like kidney cells in vitro was found to differ significantly. UCNPs@PLM did not exhibit any measurable effect on cell physiology, even with prolonged exposure. In contrast, UCNPs@AP caused changes in cell morphology and induced cell-death after approximately 30 h. These variations in toxicity are attributed to the distinct chemical stability of these particles, which likely influences their intracellular disintegration. [ABSTRACT FROM AUTHOR]

Alzheimer's Disease (AD) is a debilitating neurodegenerative disease that affects 47.5 million people worldwide. AD is characterised by the formation of plaques containing extracellular amyloid-β (Aβ) and neurofibrillary tangles composed of hyper-phosphorylated tau proteins (pTau). Aβ gradually accumulates in the brain up to 20 years before the clinical onset of dementia, making it a compelling candidate for early detection of AD. It has been shown that there is increased deposition of Aβs in AD patients' retinas. However, little is known about microglia's ability to function and clear Aβ within the retina of AD and control eyes. We labelled microglia with ionised calcium-binding adaptor molecule 1 (IBA-1) in AD and age-matched control donor retinas. We then used interactive machine learning to segment individual microglia in 3D. In the temporal mid-peripheral region, we found that the number of microglia was significantly lower in AD retinas compared to controls. Unexpectedly, the size of the microglia was significantly larger in the AD retinas compared to controls. We also labelled retinal microglia for Cluster of Differentiation 68 (CD68), a transmembrane glycoprotein expressed by cells in the monocyte lineage and a marker of phagocytic activity and activated microglia. The size of CD68 + cells was statistically different between AD and control microglial, with CD68 + cells being larger in AD. In contrast, there was no difference in either size or shape for CD68- microglia between the two groups, suggesting an important difference in the active states of CD68 + microglia in AD retina. There was also significantly increased CD68 immunoreactivity in individual microglia within the AD group. Overall, this study reveals unique differences in the size and activity of the retinal microglia, which may relate to their potential chronic activation due to increased levels of Aβs in the AD retina. Summary: MYSM1 deficiency causes inherited bone marrow failure syndrome (IBMFS). We have previously identified an IBMFS patient with a homozygous pathogenic variant in MYSM1 who recovered from cytopenia due to spontaneous correction of one MYSM1 variant in the haematopoietic compartment, an event called somatic genetic rescue (SGR). The study of the genetic and biological aspects of the patient's haematopoietic/lymphopoietic system over a decade after SGR shows that one genetically corrected haematopoietic stem cell (HSC) can restore a healthy and stable haematopoietic system. Introduction: The effects of viral infections might be apolipoprotein E (apoE) isoform-dependent. In humans, there are three major apoE isoforms, E2, E3, and E4. E4 is associated with the enhanced entry of several viruses into the brain and their disease progression. A concern of infection by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is the development of post-acute COVID-19 syndrome, also known as long COVID. Genetic risk factors for developing long COVID were reported. Methods: In this study, we used virus-like particles (VLPs) that include expression of the SARS-CoV-2 nucleocapsid (N), membrane (M), and envelope (E) structural proteins together with S. In the current study, we used human E2, E3, and E4 targeted replacement mice to assess whether these VLPs affect body weight, behavioral and cognitive performance, and circadian body temperatures. Using VLPs allow working outside an ABSL-3 facility. Results: The effects of VLPs on some behavioral measures were apoE isoform-dependent, with the E2 mice being more affected than E3 or E4 mice. The overall decreased activity in the open field containing objects in week 2 indicate that VLPs can also reduce activity levels in an apoE isoform-independent fashion. Discussion: The results of the current study indicate that even in the absence of viral replication, detrimental effects of VLPs on behavioral measures and circadian body temperatures are seen. The emerging laser writing represents an efficient and promising strategy for covalent two dimensional (2D)‐patterning of graphene yet remains a challenging task due to the lack of applicable reagents. Here, we report a versatile approach for covalent laser patterning of graphene using a family of trivalent organic iodine compounds as effective reagents, allowing for the engraving of a library of functionalities onto the graphene surface. The relatively weak iodine‐centered bonds within these compounds can readily undergo laser‐induced cleavage to in situ generate radicals localized to the irradiated regions for graphene binding, thus completing the covalent 2D‐structuring of this 2D‐film. The tailor‐made attachment of distinct functional moieties with varying electrical properties as well as their thermally reversible binding manner enables programming the surface properties of graphene. With this delicate strategy the bottleneck of a limited scope of functional groups patterned onto the graphene surface upon laser writing is tackled. [ABSTRACT FROM AUTHOR]

The non‐covalent functionalization of black phosphorus (BP) was studied with a scope of ten tailor‐made perylene diimides (PDIs). A combination of UV/Vis‐, fluorescence‐, as well as Raman spectroscopy and atomic force microscopy was used to investigate the structural factors, which contribute to a pronounced PDI‐BP interaction and thus support the protection of BP nanosheets against oxidative degradation. We were able to show, that water‐soluble, amphiphilic PDIs with highly charged head groups can be used for the non‐covalent functionalization of BP in aqueous media. Here, based on the hydrophobic effect, an efficient adsorption of the respective PDI molecules takes place and leads to the formation of a passivating film, yielding a considerable stabilization of the BP flakes under ambient conditions exceeding 30 days. Reverse osmosis (RO) is the most common method for treating salt and brackish water. As a membrane‐driven process, a key challenge for RO systems is their susceptibility to scaling and biofouling. To address these issues, functional coatings utilizing metal nanoparticles (MNPs) are developed. In this study, silver, gold, and copper nanoparticles are applied onto thin‐film composite (TFC) membranes using plasma‐enhanced magnetron sputtering. The elemental composition, surface morphology, and hydrophilicity of the coatings are analyzed using X‐ray photoelectron spectroscopy (XPS), scanning electron microscopy (SEM), and contact angle measurements. The antimicrobial properties and the filtration efficiency of the coated membranes are assessed through application‐specific experimental setups. Silver and copper nanoparticles exhibit superior antimicrobial properties, reducing microorganism adhesion by a factor of 103 compared to uncoated membranes. Under appropriate coating conditions, no deterioration in filtration performance is observed. Enhancing the adhesion of MNPs is necessary for achieving sustained release of metal ions. Glucagon‐like peptide‐1 receptor agonist (GLP‐1RA) medications have been shown to be effective in achieving optimal glucose control and reducing all‐cause death, cardiovascular death, nonfatal myocardial infarction, hospitalization for heart failure, and end‐stage kidney disease in individuals with type 1 (T1D) and type 2 diabetes (T2D). However, use of these medications has been associated with increased hypoglycaemia risk in patients treated with concomitant antihyperglycaemic medications. The risk is particularly high in patients with T1D due to their loss of glucagon counter‐regulatory response. This article reviews the effect of GLP‐1RA formulations on the development of hypoglycaemia in individuals with T1D and T2D treated with insulin therapy, discusses the benefits of continuous glucose monitoring with GLP‐1RA treatment, and presents strategies for safely initiating GLP‐1RA therapy in these individuals. Background: Helicobacter pylori is a persistent pathogen in the human stomach. However, the proposed transmission route via the oral cavity is not understood and under intense debate. While dozens of studies have shown by PCR that H. pylori DNA is frequently present in the oral cavity, data on the growth and characterization of viable H. pylori from this compartment are very scarce, and it is unclear whether the bacteria can survive in the oral cavity for longer time periods or even colonize it. Methods: Selective growth methods, scanning electron microscopy, urease assay, Western blotting, PCR, and gene sequencing were applied to identify and examine viable H. pylori in decayed milk teeth. Results: Here, we studied viable H. pylori in the plaque and root canals of 170 endodontically infected deciduous teeth that were extracted from 54 children. While H. pylori DNA was detected in several plaque and many root canal samples by PCR, live bacteria could only be cultivated from 28 root canals, but not from plaque. These 28 isolates have been identified as H. pylori by PCR and sequencing of vacA, cagA and htrA genes, phylogenetic analyses, protein expression of major H. pylori virulence factors, and by signal transduction events during infection of human cell lines. Conclusions: Thus, the microaerobic environment in the root canals of endodontically infected teeth may represent a protected and transient reservoir for live H. pylori, especially in individuals with poor dental hygiene, which could serve as a potential source for re-infection of the stomach after antibiotic therapy or for transmission to other individuals. Background: Strong relationships between trainees and physician supervisors can positively influence how trainees navigate workplace learning. How trainees act and learn in clinical workplaces characterized by rapidly developing and dissolving supervisory pairings is less well understood. This study uses the emergency department (ED) to examine the impact of transient supervisory relationships on how residents approach clinical learning opportunities. Methods: We retrospectively analyzed pediatric and emergency medicine resident rotations in an urban, tertiary, academic pediatric ED between July 2018 and June 2022. Using social network analysis (SNA), we identified resident–attending dyads and patients seen by each dyad. This informed semistructured interviews to understand how transience in supervisory relationships influences how residents approach and interpret clinical experiences. With self‐determination theory as an organizing framework, the investigators performed line‐by‐line coding with constant comparative analysis which supported subsequent theoretical coding. Results: During the study, 526 residents completed 1013 rotations with 87 attendings. A mean (±SD) of 25 (±7) attendings supervised a resident per rotation, with dyads caring for a mean (±SD) of 4 (±4) patients. Twelve residents were interviewed and described different paths to learning depending on the transience of their relationships with clinical supervisors. More sustained contact presented an opportunity to build competence by fostering autonomy and feedback, while briefer contact advanced residents' competence by exposing them to variable practice patterns. Conclusions: Combining SNA with qualitative analysis revealed that residents in the ED experience a spectrum of contact with attendings and perceive different paths to learning depending on the transience of this relationship. Objective: To compare the effectiveness of endometriosis excision alone to excision plus hysterectomy, with and without bilateral oophorectomy, for endometriosis‐related symptoms. Design: Multicentre prospective cohort. Setting: Eighty‐six specialist endometriosis centres. Population: Women undergoing rectovaginal endometriosis surgery between 2009 and 2021. Methods: We performed multivariable regression with random effects for patient and centre, controlling for age, BMI, smoking, laparoscopic versus open approach and type of bowel surgery performed, with sensitivity analysis for loss to follow‐up. Main Outcome Measures: Pain scores, bowel symptoms and quality‐of‐life measures. Results: Compared to endometriosis excision alone, women undergoing hysterectomy with conservation of ovaries had greater improvement in non‐cyclical pain (MD: 1.41/10, 95% CI: 1.03–1.78, p < 0.001), dyspareunia (MD: 1.12/10, 95% CI: 0.71–1.53, p < 0.001), back pain (MD: 1.29/10, 95% CI: 0.92–1.67, p < 0.001) and quality‐of‐life scores (MD: 8.77/100, 95% CI: 5.79–11.75, p < 0.001) at 24 months post‐operatively. Women undergoing hysterectomy with bilateral oophorectomy also had greater improvement in non‐cyclical pelvic pain (MD: 2.22/10, 95% CI: 1.80–2.63, p < 0.001), dyspareunia (MD: 1.05/10, 95% CI: 0.59–1.52, p < 0.001), back pain (MD: 1.18/10, 95% CI: 0.77–1.59, p < 0.001) and quality of life (MD: 12.41/100, 95% CI: 9.07–15.74, p < 0.001) at 24 months compared to endometriosis excision alone. Compared to hysterectomy with ovarian conservation, hysterectomy with bilateral oophorectomy was associated with greater improvement in non‐cyclical pelvic pain (MD: 0.81/10, 95% CI: 0.32–1.30, p = 0.001) at 24 months and quality of life (MD: 3.74/100, 95% CI: 0.56–6.92, p = 0.021) at 12 months, although this result was sensitive to loss to follow‐up. Conclusions: Patients who undergo endometriosis excision plus hysterectomy experience greater improvement in pain and quality of life compared to those who have endometriosis excision alone. The docking of an acyl carrier protein (ACP) domain with a downstream ketosynthase (KS) domain in each module of a polyketide synthase (PKS) helps ensure accurate biosynthesis. If the polyketide chain bound to the ACP has been properly modified by upstream processing enzymes and is compatible with gatekeeping residues in the KS tunnel, a transacylation reaction can transfer it from the 18.1‐Å phosphopantetheinyl arm of the ACP to the reactive cysteine of the KS. AlphaFold‐Multimer predicts a general interface for these transacylation checkpoints. Half of the solutions obtained for 50 ACP/KS pairs show the KS motif TxLGDP forming the first turn of an α‐helix, as in reported structures, while half show it forming a type I β‐turn not previously observed. Solutions with the latter conformation may represent how these domains are relatively positioned during the transacylation reaction, as the entrance to the KS active site is relatively open and the phosphopantetheinylated ACP serine and the reactive KS cysteine are relatively closer—17.2 versus 20.9 Å, on average. To probe the predicted interface, 20 mutations were made to KS surface residues within the model triketide lactone synthase P1‐P6‐P7.
