Shunt resistance R sh is a critical parameter for photovoltaic cells designed for low light indoor applications because it negatively affects the open circuit voltage, fill factor, and conversion efficiency. Standard CIGS cells are known to have low R sh and are, therefore, unpromising candidates for indoor energy sources. In this paper, we extend the original work of Virtuani et al. by determining the electrical specifications of many CIGS cells with copper contents [Cu]/([Ga]+[In]) as low as 0.33 and gallium contents between 0.28 and 0.81. First, IV data are fitted by a standard single-diode electrical circuit model for each illumination, resulting in light-dependent parameters. Then, we use a procedure based on a single dataset of electrical variables, i.e., independent of light, corrected by the experimental collection function, which captures light-dependent physical mechanisms. In this way, we are able to correctly reproduce the illuminance dependence of the electrical response of the PV cell over three orders of magnitude, in particular with a fixed value of the shunt resistance. Background: Household food insecurity (HFI) is a serious public health concern in Lebanon. Adverse mental health issues have been reported among food insecure households in addition to physical and nutritional complications. Caregivers in food insecure families tend to adopt different coping mechanisms to mitigate the effects of food insecurity (FI) on their children. Objective: This cross-sectional observational study aimed to explore the relationship between FI, maternal depression, child malnutrition, and differential coping mechanisms adopted by mothers. Methods: A total of 219 women were enrolled in this study; FI was assessed using the household food insecurity assessment (HFIAS), maternal depression using the patient health questionnaire (PHQ-9), and their children's nutritional status through recall of anthropometric measurements. Pearson's correlations and logistic regressions were performed to evaluate the associations between HFI, maternal depression, and children's nutritional status. Results: A strong positive correlation between HFI and maternal depression (p = 0.001) and children's nutritional status (p = 0.008) was shown. Logistic regressions revealed that being food secure decreased the risk of maternal depression (OR = 0.328, 95% CI 0.125–0.863, p = 0.024), while it did not predict children's nutritional status. Eight main themes related to coping mechanisms were identified. Conclusions: This study highlights the understudied relationship between food insecurity and maternal depression, showing an increased prevalence of HFI among residents in Lebanon with a positive correlation with increased maternal depression. Further investigation is warranted to better explore how to mitigate the negative impact of food insecurity on mental health, maternal nutritional needs, and Infant and Young Child Feeding (IYCF) practices in Lebanon. This review synthesizes 48 academic studies on Initial Coin Offerings (ICOs) from 2018 to 2023, employing bibliometric and content analyses to illuminate their complexities and innovation in entrepreneurial finance. Background/Objectives: Research has shown that athletes often have poor nutritional knowledge, particularly regarding dietary supplements. The purpose of this study was to investigate the effectiveness of an online nutrition education program in improving nutritional and dietary supplement knowledge among varsity athletes at the University of Guelph in Ontario, Canada. Methods: A total of 30 varsity athletes at the University of Guelph were randomized into experimental [n = 18] and control [n = 12] groups. A randomized wait-list controlled intervention was used, where participants in the experimental group were granted access to an online, 4-week nutrition education program, while no program was provided to the control group within the duration of the study. The program covered nutrition topics and focused on sports nutrition and dietary supplementation. Both groups were administered a validated online nutrition and dietary supplement questionnaire at baseline and post-intervention. Results: Mean baseline knowledge scores for the experimental and control groups were 13.78 ± 2.76 and 13.92 ± 2.39, respectively, and were not significantly different [p = 0.888]. Post-intervention scores were 16.28 ± 1.49 and 14.5 ± 1.88 for the experimental and control groups, respectively, [p < 0.05]. There was a significant interaction between the intervention and time on knowledge. Conclusions: These results indicate that this nutrition education program was successful in significantly improving nutritional and dietary supplement knowledge in varsity athletes. Early therapeutic intervention in high-risk smoldering multiple myeloma (HR-SMM) has shown benefits, however, no studies have assessed whether biochemical progression or response depth predicts long-term outcomes. The single-arm I-PRISM phase II trial (NCT02916771) evaluated ixazomib, lenalidomide, and dexamethasone in 55 patients with HR-SMM. The primary endpoint, median progression-free survival (PFS), was not reached (NR) (95% CI: 57.7–NR, median follow-up 50 months). The secondary endpoint, biochemical PFS, was 48.6 months (95% CI: 39.9–NR) and coincided with or preceded SLiM-CRAB in eight patients. For additional secondary objectives, the overall response rate was 93% with 31% achieving complete response (CR) and 45% very good partial response (VGPR) or better. CR correlated strongly with the absence of SLiM-CRAB and biochemical progression. MRD-negativity (10-5 sensitivity) predicted a 5-year biochemical PFS of 100% versus 40% in MRD-positive patients (p = 0.051), demonstrating that deep responses significantly improve time to progression. Exploratory single-cell RNA sequencing linked tumor MHC class I expression to proteasome inhibitor response, and a lower proportion of GZMB+ T cells within clonally expanded CD8+ T cells associated with suboptimal outcomes. Depth of response (CR/VGPR, MRD-) links to better outcomes, while biochemical progression predicts end-organ damage. Purpose: Understanding behavior toward sustainable consumption remains among the most challenging contemporary topics and requires continual investigation. The aim of this paper is to explore young Bahraini women's level of awareness of sustainable consumption while also considering their attitudes, motivations and behavior as key elements in the study. Design/methodology/approach: A qualitative approach was used to collect the data. More specifically, 46 semi-structured interviews with young Bahraini women took place and the results were produced using thematic analysis. Findings: Results reveal that consumers' awareness levels toward sustainable consumption have been growing but awareness remains variable among consumers. In turn, motivation toward supporting sustainability is more associated with environmental reasons than cultural or economic ones. The attitude toward sustainable consumption is generally positive among young consumers. Moreover, sustainable consumption has been found to be present through various means in the purchase decisions of young Bahraini women. Originality/value: This study exclusively explores the awareness levels of young Bahraini women regarding sustainable consumption with a focus on the elements of the behavioral process, namely, "awareness, attitude, motivation, and behavior", as central research pillars. Purpose: Limited research exists on the role of information access as a key success factor for rural entrepreneurs. The purpose of this study is to examine the importance of information access, among other inequality forms, in shaping entrepreneurial outcomes. Design/methodology/approach: The authors used a qualitative, inductive research methodology. They conducted interviews with rural entrepreneurs and gained insights into the role of information access and basic tangible resources in shaping their ventures. Findings: The authors identified two critical factors that impact rural entrepreneurs: inequalities in access to information and basic tangible resources. They found that inequalities in rural entrepreneurship are often interrelated, rather than isolated. The authors demonstrated that the relationship between entrepreneurship and inequality is not solely characterized by a positive or negative correlation but is a dynamic interplay where certain inequality forms may create barriers to opportunities for some individuals while creating opportunities for others. Practical implications: The authors uncovered key barriers in rural entrepreneurship and constructed a roadmap to effectively address these challenges, providing valuable insights to policymakers and support initiatives and enabling the target of high-impact resources. This research supports efforts to provide rural entrepreneurs with equal opportunities to grow and succeed. Purpose: This study examines the risk spillovers between Indonesian sectorial stocks (Energy, Basic Materials, Industrials, Consumer Cyclicals, Consumer Non-cyclical and Financials), the aggregate index (IDX) and two commodities (gold and West Texas Intermediate Crude Oil [WTI] futures). Design/methodology/approach: The study uses two methodologies: the TVP-VAR model of Antonakakis and Gabauer (2017) and the quantile connectedness approach of Ando et al. (2022). The data cover the period from October 04, 2010, to April 5, 2022. Findings: The results show that the IDX, industrials and materials are net transmitters, while the financials, consumer noncyclical and energy sectors are the dominant shock receivers. Using the quantile connectedness approach, the role of each sector is heterogeneous and asymmetric, and the return spillover is stronger at lower and higher quantiles. Furthermore, the portfolio hedging results show that oil offers more diversification gains than gold, and hedging oil is more effective during the pandemic. Practical implications: This study provides valuable insights for investors to diversify their portfolios and for policymakers to develop policies, regulations and risk management tools to promote stability in the Indonesian stock market. The results can inform the design of market regulations and the development of risk management tools to ensure the stability and resilience of the market. Originality/value: This study is the first to examine the spillovers between commodities and Indonesian sectors, recognizing the presence of heterogeneity in the relationship under different market conditions. Externally restraining volume changes of concrete, that is, thermal effects and shrinkage, may result in tensile stresses and eventually cracking. Such cracking risk is controlled/mitigated by the provision of steel reinforcement, which presumes a correct understanding of the cracking patterns under different types of restraint conditions. Reinforced concrete (RC) members may be restrained at their edges or end, or in many cases a combination of the two. Existing guidance on the subject is mostly based on end restrained members, however, it is applied to predict the behavior under edge restraint too. Researchers have identified that the mechanisms of cracking associated with edge and end restraints are quite different. To this purpose, findings from an experimental investigation aiming to understand the behavior of edge restrained RC walls were utilized to validate a finite element (FE) model. Subsequently, this FE model was used to parametrically study walls having different aspect ratios and subjected to different forms of restraint. Cracking patterns, widths, and extent appeared to greatly depend on the type of restraint and wall aspect ratio. The influence of combined restraint, for instance, was found to be more significant in walls with aspect ratio less than 4. Reversible addition fragmentation chain transfer (RAFT) solution polymerisation of the bio-based lactone monomer γ-methyl-α-methylene-γ-butyrolactone (γMeMBL) has been demonstrated in DMSO and Cyrene. RAFT control was evidenced by control over molecular weights, low disperisites, and kinetic evaluation. Purified P(γMeMBL) homopolymers exhibited high glass transition temperatures (206–221 °C) and excellent thermal stabilities. Background: Burnout is a pervasively increasing threat to personal and professional wellbeing and performance. It is yet understudied in relation to basic psychological needs (BPN), especially in at-risk population such as medical residents. This study intends to explore the differential relationship between various aspects of burnout including depersonalization (DP), emotional exhaustion (EE) and lack of personal achievement (PA) and subsets of BPN satisfaction or frustration namely autonomy, relatedness, and competence, with the framework of the Self-Determination Theory (SDT) in healthcare. Materials: A total of 110 medical residents in various Lebanese hospitals were included. Demographics and standardized scales were used to measure basic psychological need satisfaction and frustration (BPNSFS), burnout (MBI), depression and anxiety (PHQ-4). Residents were also asked about subjective evaluation of academic training and level of impact by ongoing crises (COVID-19 pandemic, Beirut port explosion and financial breakdown). Results: Result point to alarming prevalence of burnout and mental distress in our sample. It also indicates a differential correlation between gender, financial security and various subsets of burnout. It lastly points to association of DP with overall satisfaction scale (Beta = 0.342, p = 0.001) and PHQ-4 scores (Beta = -0.234, p = 0.017), while feeling burdened to attend lectures and having been physically affected by the Beirut blast correlated with a sense of PA (Beta = 0.332, p = 0.010, Beta = 0.187, p = 0.041 respectively) and PHQ-4 (Beta = 0.341, p = 0.000), interacting with COVID-19 patients (Beta = 0.168, p = 0.020) and feeling protected in the working environment (Beta = -.231, p = 0.002) showed a significant association with EE. Discussion: Within the SDT framework, this study highlights the complex interplay between collective crises, subjective evaluations or work conditions and other demographics with aspects of burnout in medical residents. Pharmacogenomics and precision medicine are promising new tools in the fight against antimicrobial resistance by providing personalized treatment approaches based on an individual genetic makeup. Pharmacists, equipped with appropriate education and tools, can play a significant role in integrating pharmacogenomics and precision medicine into clinical practice and lead the rational use of pharmacogenomics in daily patient care. Improving patient outcomes in primary healthcare settings is the ultimate output. To date, there has been a lack of assessment and comparison of the knowledge and perceptions of pharmacists across different countries. A self-administered questionnaire was sent to 223 participants from Greece and Lebanon. Results showed that 96% and 68% of the pharmacists in both countries were familiar with the terminology of antimicrobial resistance and precision medicine. A deeper investigation showed gaps concerning the profound understanding of antimicrobial resistance from the one health perspective and the preventive measures. Knowledge of pharmacogenomics, its tests, and the value of precision medicine was low in both groups, with more pronounced deficiencies in the Lebanese group. Additionally, pharmacists didn't perceive their current role in implementing pharmacogenomics and precision medicine in the combat against antimicrobial resistance, but they expressed openness to future involvement in this area. This research can serve as a foundational tool to establish or adjust policies to allocate resources and support both systems and people to enhance precision medicine implementation in low-to-middle-income countries. Furthermore, it highlights the importance of leveraging the experiences and knowledge of high-income countries to effectively address the challenge of antimicrobial resistance. As greener strategies become the top priority of all nations, their contributions toward decarbonized economies arise as an important topic. The present study attempts to weigh the effect of such strategies. We investigate this relationship using panel regressions and applying many tests based on data gathered from 2010 to 2019. Our findings underscore the positive impact of adopting green initiatives on mitigating environmental pollution. Specifically, we demonstrate that renewable energy consumption serves as a vital substitute for traditional energy sources in G7 nations, offering significant opportunities for private sector involvement in sustainable energy ventures. However, green finance initiatives exhibit positive outcomes while certain aspects of economic development exert detrimental consequences on environmental conditions. The current study is useful for environmental protection agencies, organizations, analysts, policymakers, and the private sector, as the role of green finance and energy efficiency is highly manifested. By channeling and reallocating appropriate funds to green projects, including those led by private enterprises, G7 countries can proactively address environmental degradation and align with the objectives outlined in the Paris Agreement. Background/Objectives: Women's health represents an integral component of public health. With breast cancer being the first worldwide and cervical cancer the fourth most common cancer among women, and while habits of intimate hygiene remain mediocre, it is crucial to address these issues. We aimed to evaluate the efficacy of a community-based intervention on the knowledge and preventive measures (K&P) of women toward breast cancer, cervical cancer, and intimate hygiene. Methods: This longitudinal multicentric prospective interventional study was conducted on women residing in Lebanon aged 18–83 years old. The awareness sessions took place either in person or online to address these three topics, covering the eight governorates of Lebanon. Our sample included women both from the general population and from Order of Malta Lebanon (OML)'s centers and mobile medical units. A stratified randomized sampling method was implemented using age and governorates. Women were interviewed before (pre-test) and after (post-test) the awareness session. The K&P score and the improvement post-intervention were represented in a function of all the study variables. A p ˂ 0.05 was found statistically significant. Results: A total of 657 women, with the majority being nonsmokers and having a bachelor's degree as their highest level of education, completed surveys before and after the intervention, showcasing a significant overall K&P mean score improvement of 50.48% in the pre- vs. post-test (an average score of 22.01 ± 5.95 over 38 (57.93% of correct answers) vs. 33.12 ± 3.41 over 38 (89.58% of correct answers), respectively, p < 0.001). A significant difference was also noted between pre- and post-test (p < 0.001) in each of the three topics individually, with improvements of 52.39% for breast cancer, 60.00% for cervical cancer, and 22.27% for intimate hygiene. Conclusions: National awareness campaigns are key to shedding light on breast and cervical cancer matters and improving women's reproductive health and intimate hygiene in Lebanon. Addressing the knowledge gaps and promoting early screening and healthy habits through national public health policies can empower women to protect their health and well-being. Background: Pregnancy is a crucial period for maternal and fetal health, and in Lebanon, where cultural and economic factors influence dietary practices, there is an urgent need to evaluate the food consumption patterns and diet quality of pregnant women. Aim: To evaluate the food consumption patterns, energy intake, as well as macro- and micro-nutrient intake among a nationally representative sample of Lebanese pregnant women aged 18–49 years old. Methods: A cross-sectional study was carried out from March to October 2023, involving 500 pregnant women from all eight Lebanese governorates. Sociodemographic and medical information was gathered, food consumption was evaluated using a validated Food Frequency Questionnaire (FFQ) and three 24-h recall, and anthropometric measurements were recorded. Results: The current population did not meet the USDA healthy pattern recommendations for whole grain, seafood, dairy, nuts, seeds and soy products consumption but exceeded the guidelines for vegetables, meats, poultry, eggs, oils, and refined grains. According to Mediterranean diet guidelines, the sample fell short in recommended intakes for fruits, olives/nuts/seeds, eggs, and olive oil, while surpassing the recommended levels for potatoes, legumes, pulses, sweets, red meat, processed meat, and fish and seafood. None of the participants met the energy requirements for their trimester and age group. In terms of macronutrient intake, the requirements for protein, unsaturated fats, and fiber were not met, while intakes of fats and sugars were exceeded. Regarding micronutrients, the recommended levels were not fully achieved, with particularly low intakes of vitamin D and iodine, as well as inadequate adherence to recommendations for iron, calcium, vitamin A, vitamin E, zinc, and choline. Additionally, a third of the participants did not meet the recommended intakes for folate and vitamin B12. Conclusions: The findings reveal significant dietary inadequacies among the current population, with participants failing to meet essential recommendations for whole grains and key food groups, alongside insufficient energy intake for their trimesters and age groups. Critical micronutrient deficiencies, particularly in vitamin D, iodine, and B vitamins, highlight the urgent need for targeted nutritional interventions and public health initiatives to improve dietary practices among pregnant women in Lebanon. Monocytes are crucial players in innate immunity. The human cytomegalovirus (CMV) infection has significant impacts on monocyte effector functions and gene expression. CMV, a β-herpesvirus, disrupts key monocyte roles, including phagocytosis, antigen presentation, cytokine production, and migration, impairing their ability to combat pathogens and activate adaptive immune responses. CMV modulates monocyte gene expression, decreasing their capacity for antigen presentation and phagocytosis while increasing pro-inflammatory cytokine production, which can contribute to tissue damage and chronic inflammation. CMV also alters monocyte migration to sites of infection while promoting trans-endothelial migration, thus aiding viral dissemination. Additionally, the virus affects reactive oxygen species (ROS) production, thereby contributing to end-organ disease associated with CMV infection. Overall, these changes enhance viral persistence during acute infection and facilitate immune evasion during latency. We highlight the clinical significance of these disruptions, particularly in immunocompromised patients such as transplant recipients, where the modulation of monocyte function by CMV exacerbates risks for infection, inflammation, and graft rejection. Background: Unlike eating disorders (ED) that are officially recognized and focusing on the quantity of food, Orthorexia Nervosa (ON) revolves around an unhealthy fixation on the quality of food eaten. Existing ON scales differ in how these conceptualize and define ON, ensuing inconsistency in assessments that not only affects the validity and reliability of ON related research, but also impacts the ability of healthcare professionals to identify and provide support for individuals struggling with ON. Objective: Create the first Eastern locally validated tool that considers the cultural nuances and specificity of the Lebanese general population's eating attitudes and their social context, addresses the limitations of existing scales and thereby provides a valid instrument that can be used in Lebanon and culturally-similar countries. Methods: Data were collected between September 2023 and February 2024, enrolling 320 participants for the exploratory factor analysis and 658 for the confirmatory analysis. Results: Starting from an initial pool of 25 items, the exploratory-confirmatory (EFA-CFA) factor analyses retained 13 items. The LONI showed a unidimensional factor structure, and satisfactory convergent and concurrent validity with a composite reliability (ω and α) of 0.90 providing clear evidence of its high reliability, supporting the stability and consistency of LONI scores across different subsamples. The structural characteristics, factor loadings, and item intercepts of the LONI remained consistent regardless of gender, affirming the tool's stability and reliability in measuring ON traits across diverse populations. Conclusion: The LONI is a valid tool for assessing ON, particularly in accounting for Lebanese and culturally similar populations. The scale's unidimensional structure allows for straightforward calculation of the score, making it practical for both clinical and research applications, while offering a unique advantage in considering regional dietary patterns compared with existing ON screening tools due to its cultural specificity and psychometric strengths. The ON prevalence was high in the present sample, underscoring the significance of sociocultural and behavioral factors in its manifestation. The LONI demonstrated a unidimensional factor structure, indicating that all items measured the same underlying construct. It showed high reliability, with composite reliability scores (ω and α) of 0.90, confirming its stability and consistency across different groups, including gender. The LONI is a valid tool for assessing ON. Its unidimensional structure allows for straightforward scoring, making it practical for clinical and research use. Background: Intravascular papillary endothelial hyperplasia (Masson tumor) is a rare, benign vascular tumor characterized by the proliferation of endothelial cells within blood vessels. While it typically affects the skin and subcutaneous tissues, it can occasionally appear in unusual locations. Case presentation: We report a rare case of Masson tumor located in the right atrial appendage of a healthy individual. This case was complicated by atrial wall rupture and subsequent cardiac tamponade. The tumor was successfully resected under cardiopulmonary bypass. Conclusion: This case highlights a rare occurrence of Masson tumor in the heart, emphasizing the importance of prompt surgical intervention to prevent life-threatening complications such as cardiac tamponade. Recreational use of nitrous oxide (N2O) has become a major health issue worldwide, with a high number of clinical events, especially in neurology and cardiology. It is essential to be able to detect and monitor N2O abuse to provide effective care and follow-up to these patients. Current recommendations for detecting N2O in cases of recreational misuse and consumption markers are lacking. We aimed to update current knowledge through a review of the literature on N2O measurement and kinetics. We reviewed the outcomes of experiments, whether in preclinical models (in vitro or in vivo), or in humans, with the aim to identify biomarkers of intoxication as well as biomarkers of clinical severity, for laboratory use. Because N2O is eliminated 5 min after inhalation, measuring it in exhaled air is of no value. Many studies have found that urine and blood matrices concentrations are connected to ambient concentrations, but there is no similar data for direct exposure. There have been no studies on N2O measurement in direct consumers. Currently, patients actively abusing N2O are monitored using effect biomarkers (biomarkers related to the effects of N2O on metabolism), such as vitamin B12, homocysteine and methylmalonic acid. Background and Aim: Atopic dermatitis (AD) impacts the quality of life of patients and their families. This survey‐based study aimed to understand the perspectives of physicians in Egypt and Lebanon regarding available educational initiatives and support programs for patients with AD, which was complemented by the social analytics study, offering a more comprehensive understanding of the perceptions of AD among social media users. Methods: The survey included 200 physicians, comprising primary care physicians, family medicine physicians, pediatricians, and dermatologists from Egypt and Lebanon. The social analytics study leveraged artificial intelligence to analyze 100 million websites across the region, identifying mentions of AD‐related terminologies. Results: The physician survey uncovered gaps in AD awareness and education in Egypt and Lebanon, with limited educational initiatives and digital applications available for patients. The perceptions of physicians varied regarding the use of telemedicine in dermatological disease management. According to the social analytics study, online discussions about AD predominantly originated from Egypt, featuring educational content on causes, diagnosis, management, and AD patient journey. Discussions included news about training programs, AD‐related healthcare initiatives, and drug approvals. Some authors, beauty clinics, and manufacturers actively promoted their services and products. Patients actively engaged in online discussions on self‐care and natural remedies, sharing their experiences of living with AD. Notably, there were substantial volumes of incorrect and inaccurate information being shared and promoted by some authors. Conclusion: Education about AD is crucial for patients and healthcare professionals. While social media offers opportunities for increased patient engagement, the prevalence of misinformation poses a significant challenge. Purpose: This study aims to investigate the dynamics of return connectedness of the Standard & Poor's (S&P) Gulf Cooperation Council (GCC) composite index with five regional equity indices, three global equity indices and other different asset classes during the COVID-19 pandemic period. Design/methodology/approach: This study uses daily data spanning from January 2, 2018, to December 23, 2021. A subsample analysis is conducted to determine the role of uncertainty in modifying the connectedness structure during the ongoing pandemic period. Findings: The results of this study show that the nature of connectedness is time-frequent, with clear evidence for a higher level of connectedness during stress periods, especially after the onset of the pandemic. The GCC index is found to be a net receiver of shocks to other assets, with an increase in magnitude during the COVID period. Research limitations/implications: This study is limited by the use of only daily data, and future research could consider using higher frequency data. Practical implications: The results of this study confirm the disturbing effects of the pandemic on the GCC index and its connectedness with other assets, which matters for policymakers and investors. Background: Evidence on the performance of Generative Pre-trained Transformer 4 (GPT-4), a large language model (LLM), in the ophthalmology question-answering domain is needed. Methods: We tested GPT-4 on two 260-question multiple choice question sets from the Basic and Clinical Science Course (BCSC) Self-Assessment Program and the OphthoQuestions question banks. We compared the accuracy of GPT-4 models with varying temperatures (creativity setting) and evaluated their responses in a subset of questions. We also compared the best-performing GPT-4 model to GPT-3.5 and to historical human performance. Results: GPT-4-0.3 (GPT-4 with a temperature of 0.3) achieved the highest accuracy among GPT-4 models, with 75.8% on the BCSC set and 70.0% on the OphthoQuestions set. The combined accuracy was 72.9%, which represents an 18.3% raw improvement in accuracy compared with GPT-3.5 (p<0.001). Human graders preferred responses from models with a temperature higher than 0 (more creative). Exam section, question difficulty and cognitive level were all predictive of GPT-4-0.3 answer accuracy. GPT-4-0.3's performance was numerically superior to human performance on the BCSC (75.8% vs 73.3%) and OphthoQuestions (70.0% vs 63.0%), but the difference was not statistically significant (p=0.55 and p=0.09). Conclusion: GPT-4, an LLM trained on non-ophthalmology-specific data, performs significantly better than its predecessor on simulated ophthalmology board-style exams. Background: Diabetes Mellitus is a prevalent disease with a growing impact on individuals worldwide. Evaluating the prevalence of subclinical left ventricular dysfunction and understanding its associations with microvascular complications, uncontrolled glycemia, diabetes duration, and patient age is crucial. Our aim is to determine the utility of screening for this condition. Methods: We conducted a retrospective cohort study involving 159 asymptomatic individuals with type 2 diabetes. Bivariate analysis was employed to assess potential factors and their associations with subclinical left ventricular dysfunction. Patients with a history of cardiac disease or interventions were excluded. Results: The average age of our sample was 61.5 years. Almost half of the patients exhibited an HbA1c exceeding 7% (50.3%), and approximately half had an ejection fraction (EF) of less than 55% (50.9%). In the bivariate analysis, a notable difference in microvascular diabetic complications was observed among different EF groups. Specifically, nephropathy (62%), neuropathy (57.5%), and retinopathy (74.4%) were significantly more prevalent among patients with an EF < 55%. We also identified a significant age difference between groups, with a higher mean diabetes duration (14.1 ± 7.7 years) in the lower EF group. Notably, 63.7% of patients with an HbA1c exceeding 7% exhibited an EF < 55%. Older patients were associated with a lower EF, with an adjusted odds ratio (aOR) of 0.94. An HbA1c of 7% or less was linked to a higher likelihood of an EF > 55%. Conclusion: We established a correlation between subclinical left ventricular systolic dysfunction and microvascular complications. Background: Obsessive-compulsive disorder (OCD) and eating disorders (ED) share common features, including the presence of obsessions and compulsions, and they often co-occur. Additionally, there is a significant comorbidity between ED and childhood traumatic experiences (CTE), as well as between CTE and OCD. Various biological and environmental factors have been proposed to explain the connection between ED, OCD, and CTE. This study explores the link between CTE and the comorbidity of ED and OCD, with the hypothesis that specific types of CTE may increase the risk of developing OCD in individuals with ED. Methods: Participants (N = 562) were enrolled at an eating disorder unit in Montpellier, France, between March 2013 and January 2020. The Childhood Trauma Questionnaire (CTQ), Eating Disorder Examination Questionnaire (EDE-Q), and Mini International Neuropsychiatric Interview (MINI) were used to evaluate childhood maltreatment, assess clinical characteristics associated with ED, and categorize participants into two groups: patients with and without OCD. Results: Bivariate analysis revealed that patients with comorbid ED and OCD had higher EDE-Q scores (p < 0.001), more anxiety disorders (p < 0.001), depressive disorders (p = 0.02), post-traumatic stress disorder (PTSD) (p < 0.001), and a higher incidence of sexual abuse (p < 0.001) and physical neglect (p = 0.04) compared to those without OCD. Multivariate analysis showed that the association between CTE and OCD was influenced by the presence of an anxiety disorder (p = 0.01) and a higher EDE-Q total score (p = 0.03), with a significant association with a history of sexual abuse (p = 0.04). Conclusions: This demonstrates that CTE increases the risk of comorbid OCD in ED patients, correlating with more clinically severe ED and a higher likelihood of anxiety disorders. Plain English summary: This study looked at the link between childhood trauma and the occurrence of both obsessive-compulsive disorder (OCD) and eating disorders (ED). We studied 562 participants who were being treated for eating disorders in Montpellier, France, over a seven-year period. Despite progress in standardization, there are still significant variations in preanalytical requirements, methods, nomenclature, and reporting work flows. It provides recommendations for the components of the lipid panel, fasting requirements, reporting of results, and specific recommendations for non-high-density lipoprotein cholesterol (non-HDL-C), low-density lipoprotein cholesterol (LDL-C), lipoprotein(a) [Lp(a)], apolipoprotein B (apo B), point-of-care lipid testing, and LDL subfraction testing. Rectal cancer surgery is complex and more technically challenging than colonic surgery. Over the last 30 years internationally, there has been a growing impetus for centralizing care to improve outcomes for rectal cancer. Centralizing care may potentially reduce variations of care, increase standardization and compliance with clinical practice guidelines. However, there are barriers to implementation at a professional, political, governance and resource allocation level. Centralization may increase inequalities to accessing healthcare, particularly impacting socioeconomically disadvantaged and rural populations with difficulties to commuting longer distances to "centres of excellence". Furthermore, it is unclear if centralization actually improves outcomes. Recent studies demonstrate that individual surgeon volume rather than hospital volume may be more important in achieving optimal outcomes. There is a lack of research regarding dietary supplement (DS) use among Gay, Bisexual, Transgender, Two-Spirit, Queer (GBT2Q) men and non-binary individuals, despite the higher prevalence of body image issues and societal pressure within this community. This study aimed to investigate patterns and predictors of DS use in this population, including types of DS used, sources of information, and reasons for DS use. A validated and anonymous online questionnaire was completed by 204 participants (52.5% men, 43.1% gay, mean age 29.34 + 6.77 years) across Canada, who were consuming DS at the time of the study. Analyses included descriptive statistics to characterize the sample based on gender, sexuality, age, ethnicity, and other demographic attributes, using Pearson's chi-square tests and multi-way cross-tabulation analyses. Additionally, regression analyses, including binary and logistic regressions, were employed to identify predictors of DS use. Data analysis concluded that vitamins/minerals (92.2%), proteins (84.3%) and carbohydrates (75.5%) were the most consumed types of supplements. Identifying as certain genders and sexualities was significantly associated with supplement preferences, such that men reported higher use of amino acids (p = 0.033) and non-vitamins/mineral antioxidants compared to individuals identifying as other genders (p = 0.006). Moreover, bisexual participants consumed amino acids (p = 0.043) and carbohydrates (p = 0.026) more frequently when compared to non-bisexual participants. The most listed reason for DS use was to improve immunity (60.3%), with health care professionals being listed as the source of information by most participants (51.0%). Findings from this study can serve as a foundation for further research in this area and can guide the formulation and implementation of adequate policies targeting this underserved population. Simple Summary: Prostate cancer (PCa) has significant disparities based on geography, affecting healthcare access and outcomes. This study reviews how Geographic Information Systems (GIS) are used to research these disparities. GIS helps visualize PCa incidence, survival, and mortality trends, but its application varies, leading to inconsistent results. The review followed Cochrane and PRISMA guidelines, analyzing 25 relevant studies. Most studies used GIS for mapping PCa data, geocoding, and spatial analysis to identify areas with poor PCa outcomes. However, inconsistencies in GIS methods and geographical scales used reduced the robustness of findings. The review suggests that better GIS techniques and interdisciplinary collaboration could improve the accuracy of PCa disparity research and support public health interventions. Introduction: PCa is one of the cancers that exhibits the widest disparity gaps. Geographical place of residence has been shown to be associated with healthcare access/utilization and PCa outcomes. Geographical Information Systems (GIS) are widely being utilized for PCa disparities research, however, inconsistencies in their application exist. This systematic review will summarize GIS application within PCa disparities research, highlight gaps in the literature, and propose alternative approaches. Methods: This paper followed the methods of the Cochrane Collaboration and the criteria set of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Articles published in peer-reviewed journals were searched through the PubMed, Embase, and Web of Science databases until December 2022. The main inclusion criteria were employing a GIS approach and examining a relationship between geographical components and PCa disparities. The main exclusion criteria were studies conducted outside the US and those that were not published in English. Results: A total of 25 articles were included; 23 focused on PCa measures as outcomes: incidence, survival, and mortality, while only 2 examined PCa management. GIS application in PCa disparities research was grouped into three main categories: mapping, processing, and analysis. GIS mapping allowed for the visualization of quantitative, qualitative, and temporal trends of PCa factors. GIS processing was mainly used for geocoding and smoothing of PCa rates. GIS analysis mainly served to evaluate global spatial autocorrelation and distribution of PCa cases, while local cluster identification techniques were mainly employed to identify locations with poorer PCa outcomes, soliciting public health interventions. Discussion: Varied GIS applications and methodologies have been used in researching PCa disparities. Multiple geographical scales were adopted, leading to variations in associations and outcomes. Geocoding quality varied considerably, leading to less robust findings. Limitations in cluster-detection approaches were identified, especially when variations were captured using the Spatial Scan Statistic. GIS approaches utilized in other diseases might be applied within PCa disparities research for more accurate inferences. A novel approach for GIS research in PCa disparities could be focusing more on geospatial disparities in procedure utilization especially when it comes to PCa screening techniques. Conclusions: This systematic review summarized and described the current state and trend of GIS application in PCa disparities research. Although GIS is of crucial importance when it comes to PCa disparities research, future studies should rely on more robust GIS techniques, carefully select the geographical scale studied, and partner with GIS scientists for more accurate inferences. Deep learning (DL) has shown potential to provide powerful representations of bulk RNA-seq data in cancer research. However, there is no consensus regarding the impact of design choices of DL approaches on the performance of the learned representation, including the model architecture, the training methodology and the various hyperparameters. To address this problem, we evaluate the performance of various design choices of DL representation learning methods using TCGA and DepMap pan-cancer datasets and assess their predictive power for survival and gene essentiality predictions. We demonstrate that baseline methods achieve comparable or superior performance compared to more complex models on survival predictions tasks. DL representation methods, however, are the most efficient to predict the gene essentiality of cell lines. We show that auto-encoders (AE) are consistently improved by techniques such as masking and multi-head training. Our results suggest that the impact of DL representations and of pretraining are highly task- and architecture-dependent, highlighting the need for adopting rigorous evaluation guidelines. Promoting sustainable communities aims at creating both environmentally and socially responsible living environments. This paper explores the role of affordable housing in promoting the long-term sustainability of a community within healthy living conditions by closely examining the relationship between affordable housing, urban development policies, and sustainability, with the capital of Lebanon, Beirut, serving as a case study. The first part of the paper focuses on the current building laws issued in the official newspaper in 2004 using a content analysis methodology to demonstrate the impact of the changes in the laws on the new morphology and social fabric of the city through the creation of a favorable environment for big developers, wealthy property owners, and real estate agencies and, at the expense of old city residents and low-income families. The second part of the paper uses the qualitative analysis methodology to justify the presence of large unused stock of residential units in Beirut, referring to information from multiple data sources selected based on their applicability to sustainable development, affordable housing, and urban planning in areas related to the case study, Beirut. This part then investigates the potential presented by this stock of residential units in Beirut to increase the supply of affordable housing and foster a sustainable community. This paper argues that the promoted vertical expansion of the city weighs heavily on the environment and fails to provide a diverse mix of housing units, excluding a significant portion of the community from the city. Alternative development models aligning with principles of sustainable development and challenging the current building laws can promote social inclusivity, reduce urban sprawl, and minimize environmental impacts associated with new constructions, thus preserving the city's physical and social fabric. Simple Summary: There is an increased interest for the investigation of immunotherapy and immune-related biomarkers in rectal cancer. We retrospectively analysed the expression of the programmed death-ligand 1 (PD-L1) in diagnostic biopsies and resection samples from a cohort of 83 rectal cancer patients. Using three different methods for the analysis of PD-L1, we found that the expression of this biomarker was lower in resection samples than in diagnostic biopsies. Also, we observed that higher levels of PD-L1 in resection specimens were associated with better survival outcomes. The results of our study contribute to the knowledge of PD-L1 expression in rectal cancer, having the potential to inform the design of future immunotherapy trials in this setting. Immune checkpoint inhibitors and immune-related biomarkers are increasingly investigated in rectal cancer (RC). We retrospectively analysed PD-L1 expression in diagnostic biopsy and resection samples from RC patients treated at our centre between 2000 and 2020. PD-L1 immunostaining (22C3 clone) was evaluated according to tumour proportion (TPS), immune cell (ICS), and the combined positive score (CPS). Eighty-three patients were included. At diagnosis, PD-L1 expression ≥1%/≥5% was observed in 15.4%/0%, 80.7%/37.4%, and 69.2%/25.6% of patients based on TPS, ICS, and CPS, respectively. At surgery, the respective figures were 4.6%/1.5%, 60.2%/32.5%, and 50.7%/26.2%. Using the 1% cut-off and regardless of the scoring system, PD-L1 was less expressed in surgery than biopsy samples (p ≤ 0.04). In paired specimens, PD-L1-ICS reduction was especially observed following neoadjuvant long-course (chemo)radiotherapy (p = 0.03). PD-L1-ICS of ≥5% in surgical samples (HR: 0.17; p = 0.02), and a biopsy-to-surgery increase in PD-L1-ICS (HR: 0.19; p = 0.04) was predictive for longer disease-free survival, while the PD-L1-ICS of either ≥1% (HR 0.28; p = 0.04) or ≥5% (HR 0.19; p = 0.03) in surgical samples and the biopsy-to-surgery increase in PD-L1-ICS (HR: 0.20; p = 0.04) were associated with better overall survival. In this paper an investigation in the feasibility of federated Product Lifecycle Management is presented, starting from a presentation of the guiding architecture pattern and an evaluation of implementation alternatives. Interoperability standards from Open Services for Lifecycle Collaboration (OSLC) are introduced along with presentation of tool infrastructure for automatic generation of OSLC interfaces. Purpose: To report our experience with laparoscopic repair of anterior congenital diaphragmatic hernia (CDH) using extracorporeal subcutaneous knot tying and to define recurrence risk factors. Methods: This retrospective unicentric study included children who underwent laparoscopic repair of anterior CDH without patch, using extracorporeal knot tying of sutures passed through the full thickness of the abdominal wall (2013–2020). A systematic review of the literature with meta-analysis was performed using the MEDLINE database since 2000. Results: Eight children were included (12 months [1–183]; 10.6 kg [3.6–65]). Among the two patients with Down syndrome, one with previous cardiac surgery had a recurrence at 17 months postoperatively. In our systematic review (26 articles), among the 156 patients included, 10 had a recurrence (none with patch). Recurrence was statistically more frequent in patients with Down syndrome (19.4%) than without (2.5%) (p < 0.0001), and when absorbable sutures were used (50%) instead of non-absorbable sutures (5.3%) (p < 0.0001). Conclusion: Laparoscopic repair of anterior CDH without patch was a safe and efficient surgical approach in our patients. The use of a non-absorbable prosthetic patch should be specifically discussed in anterior CDH associated with Down syndrome and/or in case of previous cardiac surgery to perform a diaphragmatic tension-free closure. Background: Physical comparison may be a factor in body dissatisfaction and related issues, like eating disorders and depression. The Physical Appearance Comparison Scale-Revised (PACS-R) is a scale developed to assess the frequency of physical comparison. Because there is no validated scale for body comparison in Arabic, this study aims to address this gap by validating the PACS-R in the Arabic language. Methods: The PACS-R was translated to Arabic following a conventional forward-backward translation procedure, and was administered to a sample of 359 Lebanese adults along with The Depression Anxiety Stress Scale, and the Rosenberg self-esteem scale (RSES) for convergent validity. The factor structure was studied by confirmatory factor analysis (CFA), and composite reliability was assessed using McDonald's omega and Cronbach's alpha. Results: Results suggested a one-factor structure of the Arabic PACS-R, with good internal consistency (McDonald's ω = 0.97 / Cronbach α = 0.97). Measurement invariance was established across sex groups, with no significant difference being reported between males and females in terms of PACS-R scores (15.42 ± 10.64 vs. 13.16 ± 11.88; t(357) = 1.84; p =.066). Finally, adequate convergent validity was tested and found to be adequate, with PACS-R scores found to be correlated negatively with self-esteem and positively with psychological distress. Conclusion: The present findings preliminarily establish the Arabic PACS-R as an effective instrument for researchers and practitioners aiming to explore the physical comparison among Arabic-speaking populations, thus contributing to research and clinical work in the Arabic community. Purpose: To assess the quality of hydroxychloroquine (HCQ)-induced retinopathy screening at a Canadian tertiary center, we concentrate on risk factor documentation within the electronic health record, in accordance with the 2016 AAO guidelines. Methods: We performed a retrospective quality assessment study based on chart review of patients who underwent screening for HCQ-induced retinopathy at the Centre Hospitalier de l'Université de Montréal (CHUM) from 2016 to 2019. We evaluated four key risk factors for HCQ-induced retinopathy: daily dose, duration of use, renal disease, and tamoxifen use, using a three-tier grading system (ideal, adequate, inadequate) for documentation assessment. Pareto and root cause analyses were conducted to identify potential improvement solutions. Results: Documentation quality varied in our study: daily dosage was 33% ideal, 31% appropriate, and 36% inappropriate. Duration of use documentation was 75% ideal, 2% adequate, and 24% inadequate. Renal disease documentation was only 6% ideal, with 62% adequate and 32% of charts lacking any past medical history. Among women's charts, tamoxifen use wasn't documented at all, with 65% adequately documenting medication lists. Pareto analysis indicated that improving renal disease and tamoxifen documentation could reduce 64% of non-ideal records, and enhancing daily dose documentation could decrease this by up to 90%. While national Lebanese studies done on breast cancer attest to the importance of awareness campaigns none, however, examine the type of content that most effectively reaches women. The purpose of this study, therefore, was to examine, through an experiment, the effectiveness of direct versus indirect messaging, in altering knowledge, risk perception, attitudes, behaviors and self-efficacy among two groups of women, those aged 18–40 and 41–65 years. Findings suggest that the designed breast cancer communication messages were perceived as informative, trustworthy and believable regardless of message type. An interaction effect indicated that the older age group (41+) who were exposed to the indirect message felt more confident in taking proactive measures than the 18- to 40-year-old participants. In addition, those exposed to the indirect message were more likely to behave as communicated than those in the direct message condition. At the very least, the messages, both direct and indirect, helped minimize misconception or correct people's knowledge about the subject. This shows that participants and by extension people, in general, pay attention to breast cancer awareness messaging. Making these messages available to all age groups and motivating them to take proactive/preventative measures becomes even more significant, especially with the higher incidence of cancer among women aged 18–40 years in Lebanon. This paper explores spatial systems of modularity on the shoreline of Ein el Mrayseh located in Beirut. The modular systems are studied from a contextual lens while observing different modular interventions as design strategies on this shoreline. The Ein el Mrayseh corniche is one of the few remaining public spaces of Beirut, and is considered one of the most important outlet for the city residents. It is used by different types of groups, from different classes and backgrounds. Based on a series of observations, the paper analyses modular interventions by different stakeholders on this stretch. Two terms were coined: rigid and soft modularity, as linguistic and analytical modes to understand the spatial practices of Ein el Mrayseh. The paper argues that the users of this public space have developed soft modular strategies - dynamic and in constant flux, as a mean to tame the rigid modular strategies enforced on the site and to appropriate the space. Background: Giant cell arteritis (GCA) is the most prevalent systemic vasculitis in people older than 50 years. Any delay in diagnosis impairs patients' quality of life and can lead to permanent damage, particularly vision loss. Objective: To evaluate a diagnostic strategy for GCA using color Doppler ultrasound of the temporal artery as a first-line diagnostic test, temporal artery biopsy (TAB) as a secondary test, and physician expertise as the reference method. Design: Prospective multicenter study with a 2-year follow-up. (ClinicalTrials.gov: NCT02703922) Setting: Patients were referred by their general practitioner or ophthalmologist to a physician with extensive experience in GCA diagnosis and management in one of the participating centers: 4 general and 2 university hospitals. Patients: 165 patients with high clinical suspicion of GCA, aged 79 years (IQR, 73 to 85 years). Intervention: The diagnostic procedure was ultrasound, performed less than 7 days after initiation of corticosteroid therapy. Only ultrasound-negative patients underwent TAB. Measurements: Bilateral temporal halo signs seen on ultrasound were considered positive. Ultrasound and TAB results were compared with physician-diagnosed GCA based on clinical findings and other imaging. Results: Diagnosis of GCA was confirmed in 44%, 17%, and 21% of patients by ultrasound, TAB, and clinical expertise and/or other imaging tests, respectively. Their diagnosis remained unchanged at 1 month, and 2 years for those with available follow-up data. An alternative diagnosis was made in 18% of patients. The proportion of ultrasound-positive patients among patients with a clinical GCA diagnosis was 54% (95% CI, 45% to 62%). Limitation: Small sample size, no blinding of ultrasound and TAB results, lack of an objective gold-standard comparator, and single diagnostic strategy. Conclusion: By using ultrasound of the temporal arteries as a first-line diagnostic tool in patients with high clinical suspicion of GCA, further diagnostic tests for patients with positive ultrasound were avoided.
