Summary: Background: Primary cutaneous lymphomas (PCL) are rare skin tumors of lymphoproliferative neoplasms and belong to the heterogeneous group of non‐Hodgkin's lymphomas. PCL encompass a broad spectrum of clinical and histologic manifestations, with cutaneous T‐cell lymphoma (CTCL) being the most common (73%). Due to the rarity of the diseases, population‐based studies of care and epidemiology are limited. Patients and methods: Based on anonymized, age‐ and sex‐adjusted SHI (statutory health insurance) claims data of approximately five million SHI‐insured patients, a retrospective analysis was conducted over a six‐year period (2012–2017) to determine the prevalence, incidence, and lethality in patients with mature‐cell T/NK‐cell lymphoma in Germany. Results: A total of 1,336 patients with T‐cell lymphoma were identified during the observation period. The six‐year prevalence ranged from 27.35 to 43.58 per 100,000. Patients were 65% male with a mean age of 66 years (SD 15). There were 246 patients (approx. 20%) who died within the 6 years, up to 7% per year. The calculated incidence in 153 identified patients in 2017 is 3.65 to 3.92 per 100,000. Conclusions: For the first time, valid epidemiologic findings of patients with mature T‐cell and NK‐cell lymphomas were obtained using SHI claims data in Germany. This article analyses the economic impact of inbound student mobility in Hungary. The research is based on an innovative approach not only showing the geoeconomic impact based on expenditures of the different groups of incoming students (Erasmus+, degree mobility of self-funded or scholarship holder students), but also links the data to specific sectors of the national and local economies. Thus, the paper reveals that international student mobility affects not only the internationalization of the higher education institutions or even the entire higher education sector, but also the wider macro environment, carrying geopolitical considerations. Based on the findings, a new aspect or "side effect" of the global and/or governmental "soft power" initiatives (e.g. Erasmus+, Belt and Road Initiative, Stipendium Hungaricum) can be identified in economic development. With the Hungarian case, the article intends to contribute to a better understanding of the changing geopolitics of higher education in the twenty-first century. [ABSTRACT FROM AUTHOR]

The article discusses the challenges and complexities of diagnosing and treating cutaneous T-cell lymphoma (CTCL), a rare group of non-Hodgkin lymphomas that primarily affect the skin. The lack of standardized approaches and specialized training programs for skin lymphoma specialists in Europe has led to divergent approaches in patient care. The HORIZON program, which involved clinical and research rotations across five European centers, aimed to address this gap and foster a collaborative global network. The rotations provided valuable insights into diagnostics, therapeutics, and research methodologies, including the use of advanced techniques and technologies. Consistency was observed in adopting both traditional and newer treatments, aligning with the latest guidelines. The involvement of clinical nurse specialists, particularly in the UK, was noted as an opportunity for holistic and comprehensive patient-centered care. The program has laid the groundwork for a collaborative network and ongoing research projects. A second program is planned for 2025. Simple Summary: The surrounding cells of the tumor tissue are called microenvironment, and they may contribute to disease progression and/or antitumor immune response. Much has been learned about the microenvironment of mycosis fungoides and Sézary syndrome over the past decades and how it is affected by treatments. This review summarizes the recent advances in the field. Mycosis fungoides and Sézary syndrome are epidermotropic cutaneous lymphomas, and both of them are rare diseases. Mycosis fungoides is the most frequent primary cutaneous lymphoma. In about 25% of patients with mycosis fungoides, the disease may progress to higher stages. The pathogenesis and risk factors of progression in mycosis fungoides and Sézary syndrome are not yet fully understood. Previous works have investigated inter- and intrapatient tumor cell heterogeneity. Here, we overview the role of the tumor microenvironment of mycosis fungoides and Sézary syndrome by describing its key components and functions. Emphasis is put on the role of the microenvironment in promoting tumor growth or antitumor immune response, as well as possible therapeutic targets. We focus on recent advances in the field and point out treatment-related alterations of the microenvironment. At best, stable disease (SD) was achieved after 13 of the treatment lines (43%), partial response (PR) after 12 (40%), CR after two (7%) and progressive disease (PD) after three (10%). GSS treatment is challenging and mostly allows patients to achieve SD or PR. At the last visit, one patient (14%) was in ongoing CR 5 years after stopping methotrexate and three patients (43%) had controlled disease, including one without treatment. Summary: Background: Cutaneous T‐cell lymphomas (CTCLs) are rare forms of non‐Hodgkin's lymphoma of T‐cell origin that occur mainly in the skin. The most common form is mycosis fungoides (MF), but Sézary syndrome (SS), a more aggressive form of CTCL, is another relevant subgroup. Due to the rare nature of the disease, population‐based studies of the epidemiology and disease burden and insights into care delivery are limited. Patients and methods: Based on an anonymized, age and sex‐adjusted routine dataset comprising approximately five million people with statutory health insurance, a retrospective, longitudinal healthcare research study was conducted over a six‐year period (2012–2017). Results: In 55 % of patients with MF and SS, the initial diagnosis was documented in an outpatient setting; in 59 % of cases by a dermatologist. Immunophenotyping by flow cytometry is considered an important investigative tool for the detection and follow‐up surveillance of blood involvement of cutaneous lymphomas, as the disease stage is the most important prognostic factor in MF and SS; this was performed in only 10 % of patients. The first‐line treatment was topical (76 %), in particular with corticosteroids (66 %). Conclusions: The findings from this healthcare research point to the need for increased guideline‐based care. Objectives The aim of this study was to measure the prevalence of skin diseases in aged nursing home residents and to explore possible associations with demographic and medical characteristics. Design Descriptive multicentre prevalence study. Setting and participants The study was conducted in a random sample of ten institutional long-term care facilities in the federal state of Berlin, Germany. In total, n=223 residents were included. Results In total, 60 dermatological diseases were diagnosed. The most frequently diagnosed skin disease was xerosis cutis (99.1%, 95% CI 97.7% to 100.0%) followed by tinea ungium (62.3%, 95% CI 56.0% to 69.1%) and seborrheic keratosis (56.5%, 95% CI 50.2% to 63.0%). Only few bivariate associations have been detected between skin diseases and demographic and medical characteristics. Conclusion Study results indicate that almost every resident living in residential care has at least one dermatological diagnosis. Dermatological findings range from highly prevalent xerosis and cutaneous infection up to skin cancer. Not all conditions require immediate dermatological treatment and can be managed by targeted skin care interventions. Caregivers need knowledge and diagnostic skills to make appropriate clinical decisions. It is unlikely that specialised dermatological care will be delivered widely in the growing long-term care sector. Photoaging is associated with increasing pigmentary heterogeneity and darkening of skin color. However, little is known about age-related changes in skin pigmentation on sun-protected areas. The aim of this explorative study was to measure skin color and dyspigmentation using image processing and to evaluate the reliability of these parameters. Twenty-four volunteers of three age-groups were included in this explorative study. Measurements were conducted at sun-exposed and sun-protected areas. Overall skin-color estimates were similar among age groups. The hyper- and hypopigmentation indices differed significantly by age groups and their correlations with age ranged between 0.61 and 0.74. Dorsal forearm skin differed from the other investigational areas (p < 0.001). We observed an increase in dyspigmentation at all skin areas, including sunprotected skin areas, already in young adulthood. Associations between age and dyspigmentation estimates were higher compared to color parameters. All color and dyspigmentation estimates showed high reliability. Dyspigmentation parameters seem to be better biomarkers for UV damage than the overall color measurements. [ABSTRACT FROM AUTHOR]

Background/Aims: Antibiotic-induced drug resistance requires new approaches in topical acne treatment. Tyrothricin is known to produce no resistance. In this study, it was tested for the first time in topical acne treatment. The efficacy and tolerability of topical tyrothricin 0.1% was evaluated. Methods: Arandomized, active comparator-controlled, exploratory, observer-blind clinical study was conducted in 24 patients with acne papulopustulosa. Randomization on a split-face was either tyrothricin versus clindamycin + benzoyl peroxide (BPO) (n = 12) or tyrothricin versus BPO 5% (n = 12). The main outcome was change in inflammatory and noninflammatory lesion counts. Results: The mean differences in inflammatory lesion counts from baseline were -12.3 (95% CI: -20.5 to -4.1) in clindamycin + BPO, -10.2 (95% CI: -15.3 to -5.0) in BPO 5%, and -7.7 (95% CI: -11.7 to -3.7) in tyrothricin. Tyrothricin reduced noninflammatory lesions (mean difference: -6.5 (95% CI: -11.6 to -1.4) and caused less product-related adverse events (n = 31) compared to BPO (n = 37) and clindamycin + BPO (n = 20). Conclusion: The results indicate that tyrothricin might be a candidate for treating acne and it seems to be more tolerable than both comparator treatments. © 2015 The Author(s) Published by S. Dermal and epidermal structures in human skin change during intrinsic and extrinsic aging. Epidermal thickness is one of the most often reported parameters for the assessment of skin aging in cross-sectional images captured by optical coherence tomography (OCT). We aimed to identify further parameters for the non-invasive measurement of skin aging of sun-exposed and sun-protected areas utilizing OCT. Based on a literature review, seven parameters were inductively developed. Three independent raters assessed these parameters using four-point scales on images of female subjects of two age groups. All items could be detected and quantified in our sample. Interrater agreement ranged between 25.0% and 83.3%. The item scores "stratum corneum reflectivity," "upper dermal reflectivity," and "dermoepidermal contrast" showed significant differences between age groups on the volar and dorsal forearm indicating that they were best able to measure changes during skin aging. "Surface unevenness" was associated with the skin roughness parameters, Rz and Rmax, on the inner upper arm and volar forearm supporting the criterion validity of this parameter on sun-protected skin areas. Based on the interrater agreement and the ability to differentiate between age groups, these four parameters are being considered as the best candidates for measuring skin aging in OCT images. Facial skin ageing is caused by intrinsic and extrinsic mechanisms. Intrinsic ageing is highly related to chronological age. Age related skin changes can be measured using clinical and biophysical methods. The aim of this study was to evaluate whether and how clinical characteristics and biophysical parameters are associated with each other with and without adjustment for chronological age. Twenty-four female subjects of three age groups were enrolled. Clinical assessments (global facial skin ageing, wrinkling, and sagging), and biophysical measurements (roughness, colour, skin elasticity, and barrier function) were conducted at both upper cheeks. Pearson’s correlations and linear regression models adjusted for age were calculated. Most of the measured parameters were correlated with chronological age (e.g., association with wrinkle score, r=0.901) and with each other (e.g., residual skin deformation and wrinkle score, r=0.606). After statistical adjustment for age, only few associations remained (e.g., mean roughness (Rz) and luminance (L*),  β=-0.507, R2=0.377). Chronological age as surrogate marker for intrinsic ageing has the most important influence on most facial skin ageing signs. Changes in skin elasticity, wrinkling, sagging, and yellowness seem to be caused by additional extrinsic ageing. [ABSTRACT FROM AUTHOR]

ABSTRACT Pressure ulcers (PUs) are injuries to the skin and underlying tissues, caused by sustained deformations and occur frequently in aged patients. Skin microtopography and stiffness affect the interaction of skin with contact surfaces contributing to PU development. We simulated immobility in 20 healthy females (mean age 69.9 years). Skin microtopography and stiffness were measured at the PU predilection sites before and after loading. Skin roughness decreased at the heels by 18.1% after 90 minutes ( p = 0.022), but remained unchanged at the sacrum and the upper back. Structural elasticity and elastic deformations increased at all skin areas; changes over time were significant at the sacrum ( p = 0.005) and the heel, ( p = 0.002). The residual skin deformation increased at all skin areas after loading significantly at the sacrum (32.0%, p = 0.013) and upper back (20.6%, p = 0.007). The structural 'biological' elasticity of the skin decreased significantly at the upper back after loading, but remained unchanged at the heels. All skin changes recovered after unloading. Results indicate that prolonged loading causes structural skin changes in humans in vivo in PU predilection sites. The pathogenesis of PUs is different at the heels, the sacral and upper back skin. The article presents a case study of a 19-year-old female was referred to us in 2010 for evaluation of papulonodules on the right arm since 2006, concerning for marginal zone B-cell lymphoma (MZL). Topics include inflammatory skin condition present with patches, plaques, and nodules that mimic other skin diseases, including lymphomas; and MZL has a proliferation of cells ranging from plasma cells to marginal zone B cells.

The article presents case study of 60-year-old woman was managed at Hôpital Saint-Louis for plaque-stage MF (T2N0M0B0). It mentions that Mogamulizumab was initiated in January 2019 due to insufficient response to 5 first-line therapies and the severe involvement of the palms significantly impacting her quality of life. It also mentions that three months after development of the oral lichenoid lesion, genital lichen planus developed.

Simple Summary: Cutaneous T-cell lymphomas (CTCL) are rare malignant diseases. In this study we have compared the cutaneous lymphoma registries of different countries, which included information on at least 100 patients. The frequencies of each CTCL subtype were compared within and between continents. We found that the registries differed importantly in terms of size and quality. Some rare CTCL subtypes, such as NK/T-cell lymphoma or subcutaneous panniculitis-like lymphomas, were more frequent in Asian countries, while others were evenly distributed. We discuss possible reasons for this and provide suggestions on how to build future CTCL registries. Cutaneous T-cell lymphomas (CTCL) are a heterogenous group of rare diseases. Many studies have reported on local epidemiology or geographic clustering, however we lack information from a global perspective. A systematic review and meta-analysis was conducted in Medline and the Cochrane Library based on a previously registered protocol and according to the preferred reporting of items for systematic reviews and meta-analyses (PRISMA). We selected publications that enrolled at least 100 patients with primary cutaneous lymphomas according to the current classifications. The relative frequencies (proportions) of subtypes were compared between studies and geographic regions in a meta-analysis. In total, 26 studies met our inclusion criteria, reporting on altogether 16,953 patients. Within primary cutaneous lymphomas, CTCL appeared to be 15% more frequent in Asian populations. Mycosis fungoides (MF) accounted for 62% of CTCL, with an important heterogeneity in frequencies between studies and continents. The proportion of Sézary syndrome (SS) was 3%, stable worldwide. Rare CTCL, such as NK/T-cell lymphoma or subcutaneous panniculitis-like lymphoma, were more frequent in Asian studies. This global meta-analysis of CTCL confirmed the predominance of CTCL among primary cutaneous lymphomas (83% on average) in the three analyzed continents, most of which were MF cases. It revealed the same proportions of SS across continents, and the heterogeneity of MF frequencies, suggesting the possible role of environmental factors in the pathophysiology of the latter.
