Porcelain laminate veneers (PLV) offer a conservative aesthetic solution for interdental space closure. However, space is typically not distributed symmetrically, and the mesiodistal width of the restored teeth may increase unless multiple teeth are prepared for conventional PLV to maintain appropriate individual tooth proportion. Ceramic fragments can be suggested to close the space without modifying tooth size and dental proportion. This article presents two cases where interdental gaps were closed using lingually placed porcelain laminate (LPPL). All stages of the restorative procedures, including treatment planning, temporization, tooth preparation, and cementation of these delicate restorations, are detailed. Based on the results of these clinical reports, we regard the use of LPPL as a successful treatment option in selected cases, as evidenced by 2- and 6-year follow-ups. Introduction: Orthodontic discrepancies are a common finding in patients with supernumerary teeth (ST). The presence of a ST can cause a number of orthodontic discrepancies, including delayed eruption or retention of adjacent teeth, crowding, spacing, and abnormal root formation. The aim of the present study was to assess the effect of extraction of an anterior supernumerary tooth on the underlying orthodontic discrepancies without additional treatment for a 6-month period. Methods: This was a prospective, longitudinal, observational, study. It included 40 participants with orthodontic malocclusions due to maxillary anterior supernumeraries. We examined the changes in the crowding and excessive space in the anterior and posterior segments on cast models. Results: In the group that presented with crowding, a statistically significant decrease of 0.95 ± 0.17 mm (P < 0.001) was found between T0 and T1. Of the participants, three exhibited full self-correction. The excessive space at T0 (3.06 mm) decreased by 1.78 ± 0.19 mm to T1 (1.28 mm) in the anterior segment. Seven participants showed full self-correction of the diastemas after the 6-month observation period. Conclusion: The results imply that orthodontic treatment can be postponed for at least 6 months after the extraction of the supernumerary tooth as potential self-correction can be expected. This natural alleviation of the malocclusions may make the orthodontic treatment simpler, shorten the treatment time and decrease overall appliance wear time. [ABSTRACT FROM AUTHOR]

Bone grafts are commonly used to treat periodontal bone abnormalities. Studies indicate that using graft materials in directed tissue regeneration is effective. However, treating deep and extensive bony defects can result in a large area that lacks blood flow from the periodontal ligament or alveolar bone. This poses a risk of wound failure when an incision of the defect-associated papilla is performed. (17) Bianchi and Basetti developed a new surgical technique called the "Whale's Tail" to address this issue. This approach can effectively regenerate broad intrabony defects in the esthetic zone while preserving interdental tissues over grafting material. The proposed procedure entails the elevation of a sizeable flap from the buccal to the palatal side, thereby providing an unobstructed view and access to the intrabony defect in question. (1) Background: Since ancient times, people have been concerned with the aesthetics and attractiveness of their own person. The present study aims to analyze the beauty of the smile according to the level of training in the field of dental aesthetics of the respondents.; (2) Methods: To test this hypothesis, a questionnaire with 22 questions was applied to which 306 participants voluntarily answered; (3) Results Several variables related to dental aesthetics were analyzed based on digitally modified facial photographs; (4) Conclusions: The study showed that the degree of training of the patients is a factor that influences the perception of the smile. The article discusses the treatment of mesiodens in different stages of dentition. Topics include the clinical presentation and radiographic features of mesiodens; treatment approaches, including extraction methods using specialized forceps to prevent damage to adjacent permanent tooth germs; and post-extraction management, including follow-up care and the use of removable orthodontic appliances like Hawley's plates for diastema closure and alignment in older children.

Purpose: The purpose of this multicenter study was to evaluate the survival and quality outcome of direct composite buildups in the anterior dentition based on representative sample sizes. Materials and Methods: At three university clinics in Germany, the survival and quality outcome of n = 667 direct composite buildups performed between 2001 and 2012 was evaluated in n = 198 participants. Survival outcomes were categorized as failure (F), survival (SR), or successful (S). Restorations still in place and without failure prior to follow-up (n = 567) were rated using modified USPHS/FDI criteria to obtain the quality outcome. Detailed failure analysis was done by means of Cox regression models. Results: The restricted mean for overall survival was 15.5 years. N = 576 restorations were classified as successful (S), n = 81 survived with repair (SR) and n = 8 failed (F). Two restorations were removed due to iatrogenic interventions. Overall survival rates after 2, 10, and 15 years were 98.8% (CI: 97.6 and 99.4), 91.7% (CI: 89.0 and 93.8), and 77.6% (CI: 72.2 and 82.2), respectively. Functional survival rates were 100.0%, 98.9% (CI: 97.5 and 99.5), and 98.5% (CI: 96.7 and 99.3), respectively. Clinical quality was rated as excellent or good for most restorations. The dominant failure mode was chipping; however, regression analysis did not detect any influence of the evaluated parameters "enlargement range," "position in the jaw," or "tooth type" on failure. Conclusion: This multicenter study represents the first of its type including clinical survival and quality data on 576 direct anterior composite buildups over a restricted mean follow-up of 15.5 years in a relatively large group of participants. In particular, functional survival was outstanding due to the clinical reliability of this treatment option, while simultaneously providing flexibility, reparability, and minimal invasiveness. Patients with missing anterior teeth along with diastema or generalised spacing present as a peculiar clinical situation. For patients seeking fixed modality of treatment and who wish to retain their existing diastema or spacing, rehabilitation with a conventional fixed dental prosthesis, resin bonded fixed dental prosthesis would lead to unesthetic results leaving them with limited treatment options. Background: Mixed dentition determines an important period of occlusion development in childhood. Objective: This study aimed to evaluate the aesthetic impact of the midline diastema cut‐off point in mixed dentition. Design: A cross‐sectional study was performed with 257 children aged 8‐10 years in late mixed dentition, with midline diastema of 1‐5 mm and normal maxillary overjet. Orthodontic Aesthetic Subjective Impact Score determined the aesthetic impact of the midline diastema. Four quartiles of diastema measurements and impact scores were considered as cut‐off points. Receiver Operating Characteristic curves were constructed, and the distances representing the distances to the ideal point were calculated. The Yuden index determined the efficiency of measuring the diastema to assess the aesthetic impact. The Kruskal‐Wallis test compared the groups of participants classified according to the diastema cut‐off by the sample quartile regarding the impact score. The analyses were performed with a significance level of 5%. Results: There was no significant difference between the four groups of measurement quartiles of the diastema regarding the aesthetic impact (P >.05). Midline diastema has been a popular aesthetic problem for children. Several treatment options are available, however, the use of elastic bands without other appliances for closing the diastema still remains a pressing issue in Vietnam, which could be done by the patient’s parents either themselves or under the dental practitioners’ suggestion. Objective: To report a specific case about the extensive extrusion regarding upper central incisors in a pediatric patient due to misusing the elastic band, in which management was complex due to an unfavorable situation for splinting. Case Report: Single rubber band usage for orthodontic movement in closing diastema has been an unresolved disaster in Vietnam, leading to the need for early diagnosis and immediate intervention when facing the complications. Introduction: Malocclusion profoundly impacts a child's emotional well-being and affects an individual's quality of life. Early identification improves the chances of organising preventive and interceptive treatment to limit the intensity of developmental aberrations. The data for malocclusion among children of Visakhapatnam is lacking. Aim: To study and evaluate the prevalence of malocclusion and ascertain gender dimorphism among six- and nine-year-old school-going children of Visakhapatnam Materials and Methodols: A cross-sectional, double-stage sample study was conducted among 616 school-going children of Visakhapatnam, Andhra Pradesh, India, by a single calibrated examiner using a modified Index for Preventive and Interceptive Orthodontic Needs (IPION). IBM Statistical Package for the Social Sciences (SPSS), version 25.0, was used to analyse data and Pearson's chi-square test to elicit gender differences in the prevalence of malocclusion traits. Results: Prevalence among six-year-old children: interproximal caries-38.4%, premature tooth loss-10.1%, supernumerary teeth-0.3%, upper molar rotation-0.3%, lower molar tipping-2%, anterior crossbite-2.7%, posterior crossbite-0.7%, overjet more than 3 mm-5.1%, overbite more than 2/3rd-8.4%, open bite-2%, incompetent lips-13.8%. Prevalence among nine-year-old children: interproximal caries-49.5%, premature tooth loss-8.8%, active frenum-2.8%, supernumerary teeth-0.6%, diastema-3.4%, upper molar rotation-2.2%, lower molar tipping-5%, impended eruption of first permanent molar-1.3%, overjet more than 3 mm-11%, overbite more than 2/3rd-12.5%, open bite-2.2%, anterior crossbite-8.2%, posterior crossbite tendency-3.1%, Class I-75.6%, Class II-18.8%, Class III (functional shift)-0.6%, Class III (no functional shift)-3.76%, incompetent lips-16.3%. Conclusion: Children from both cohorts demonstrated malocclusion traits. The nine-year-old group had greater prevalence for all components examined except premature tooth loss. Objectives: The aim of this retrospective evaluation was to assess the short‐term outcome of two monoshade universal resin composites with high chameleon effect in anterior direct veneer and diastema closure restorations, and to investigate the possible reasons for failure. Material and Methods: Patients subjected to veneer and diastema closure restorations with two monoshade universal resin composites (Essentia Universal Shade; EU, GC Corp., and Omnichroma; OC, Tokuyama) operated between January 2018 and March 2019 were selected for the present retrospective evaluation. A total of 159 composite restorations (78 veneers and 81 diastema closure restorations) performed by a single operator in 44 patients (mean age: 33.6) were included in the study. Two blinded and calibrated examiners performed 1‐ and 2‐year assessments of the restorations with respect to FDI criteria, using medical/clinical history and dental photography records. Data were analyzed using Pearson Chi‐square with Continuity Correction, Fisher's Exact tests, and Cox regression (a < 0.05). Results: The cumulative overall survival rates of EU and OC restorations were 94.6% (97.3% for the first year) and 88.6% (95.3% for the first year), respectively, with no significant difference from each other (p = 0.316). The cumulative overall survival rates of direct veneer and diastema closure restoration types were 90.2% (95.1% for the first year) and 92.4% (97.4% for the first year), respectively, with no significant difference (p = 0.559). The reasons for failure were evaluated as fracture of the restoration, failure in esthetic anatomical form, and color mismatch. All the failed direct veneer restorations were due to fractures (FDI score of 5.4), whereas 5 of 6 failed diastema closure restorations were due to color mismatch (FDI score of 3.4). Regarding the composite materials, there were no significant differences between the success rates of the restoration types (p = 0.442 for EU, p = 1.000 for OC). With respect to the restoration types, there were also no significant differences between the success rates of the resin‐based composites (p = 1.000 for direct veneer restorations and p = 0.228 for the diastema closure restorations). In addition, no significant difference was observed between male and female patients regarding the acceptable and unacceptable scores (p = 1.000). Conclusions: The 2‐year clinical performance of the two monoshade universal composites in anterior veneer and diastema closure restorations were both considered successful and similar. Despite the lack of shade selection, both monoshade universal composites presented a successful color match. However, the diastema closure restorations might be more prone to color mismatch compared to the veneers over time, while veneer restorations presented more fractures than the diastema closure restorations. Clinical Significance: Monoshade universal composites presented successful short‐term clinical outcomes regarding both function and esthetics in anterior direct veneer and diastema closure restorations. Background: Progressive familial intrahepatic cholestasis is a heterogeneous group of disorders, leading to intrahepatic cholestasis, with the possibility of chronic liver failure and biliary cirrhosis. Oligodontia is either the manifestation of a specific syndrome or is non-syndromic. To the best of our knowledge, this is the first case report of type 3 progressive familial intrahepatic cholestasis and concurrent oligodontia, craniosynostosis, dens in dente, taurodontism, and delayed permanent dentition in the medical and dental literature. Case presentation: We present the dental and medical histories and comprehensive dental management of a girl with type 3 progressive familial intrahepatic cholestasis and several dental anomalies, who was referred to a dental clinic due to severe dental caries and pain. Conclusion: Our findings suggest that PFIC with manifestations as oligodontia, craniosynostosis, dens in dente, taurodontism, and delayed permanent dentition, might indicate an unknown syndrome; otherwise, the craniofacial anomalies are the manifestations of an independent disease coinciding with PFIC. Moreover, our case is a good example of the importance of timely medical and dental care in confining further health-related complications. Maxillary midline diastema is a frequently occurring malocclusion found to be a common aesthetic complaint of patients. It occurs due to of multifactorial etiology such as labial frenum, microdontia, and oral habits such as thumb sucking, Tongue thrusting, or Dental malformations. Proper technique and materials for successful treatment are grounded on time and the patient's physical, psychological, and economical conditions. The use of Direct composite resin restoration in diastema closure permits the dentist and patients to gain complete control of the above-mentioned and recreation of an aesthetic natural smile. The case report describes a case of midline diastema closure using direct composite resin restoration. The patient was recalled for follow-up for 1 year. No discoloration, sensitivity, line of demarcation, or fracture lines were seen. Therefore, midline diastema closure using direct composite resin restoration is considered highly aesthetic, and durable and could fulfill the aesthetic need of the patient in the present case. Introduction: The midline diastema is space or gap between the maxillary central incisors. it is a common esthetic complaint of patients. Aim: The aim of this article is to know the knowledge and awareness of dental students in the management of Midline diastema. Materials and Methods: A questionnaire of 16 questions was created and entered in the online survey creator 'Google Forms' and shared among each student of about 100 individually and privately and data were collected subject to statistical analysis using SPSS software. Statistical tests used were descriptive statistics and Chi square tests. P value less than 0.05 was considered statistically significant. p value was found to be 0.261 which statistically insignificant Conclusion: Results of this study suggest that Preclinical dental students were lacking some knowledge in management of Midline diastema, so they need an effective education and awareness campaign to increase their knowledge and awareness on management of Midline diastema. Objective: To evaluate the effect magnitude of different parameters on smile attractiveness. Materials and methods: A reference and 13 images were produced by manipulating 13 parameters. Image rating was performed with a 4‐point Likert scale from least attractive (1) to most attractive (4). Image raters included laypeople, dental students, dentists, and dental specialists. Friedman and Wilcoxon image were used for estimate of effect size. Parameters were classified into small (0.10‐ < 0.30), medium (0.30‐ < 0.5), or large (≥0.50). Results: A total of 1040 people participated with good consistency (α = 0.861), and moderate reliability (0.64–0.7). The reference image had the highest rank (laypeople:11.79, dental background: 12.55). For effect size; gingival margin level (−0.11, −0.01), teeth width proportion (−0.09, −0.10), inverted smile arch (−0.09, −0.21), commissure line cant (−0.15, −0.17) and low smile (−0.24, −0.23) had small effect size; occlusal plane cant (−0.36, −0.49), midline cant (−0.36, −0.48), and midline shift (−0.37, −0.49) had medium effect size; diastema (−0.55, −0.54) and color (−0.56, −0.56) had large effect size for the laypeople and dental groups. High smile (−0.42, −0.51), incisor edge symmetry (−0.46, −0.54) had medium effect size in laypeople group and large effect size in the dental group. Width to length tooth proportion (−0.26, −0.39) had small effect size in the laypeople group and medium effect size in the dental group. Conclusions: Smile parameters had different effect magnitude on smile attractiveness and were classified into small, medium, or large parameters. Neither laypeople nor professionals have a collective judgment on what constitutes a beautiful smile. Clinical significance: This study investigated the effect magnitudes of 13 smile parameters and presented a small, medium, and large smile parameters classification. Ceramics have greatly evolved since they came into the eye of the dental community. Currently, there is a demand for a highly aesthetic as well as strong restoration and high translucent zirconia fits that criteria pretty well. This has significantly increased the applicability of zirconia as a restorative material in aesthetically demanding cases. Veneers have an innate disadvantage of lack of strength, decreased fracture resistance and chipping. Using these highly translucent zirconia veneers will help overcome these disadvantages. Therefore, the objective of the case series was to test the effectiveness of adhesively bonded highly translucent zirconia veneers using resin cement. This type of veneers provides satisfactory aesthetic outcomes and decreases the risk of fracturing during the trial and clinical use. However, further studies with long-term follow-ups are needed. This case report emphasizes on esthetic rehabilitation of maxillary anterior teeth with porcelain laminate veneers. A male patient aged 24 years reported with a chief complaint of spacing between teeth in the upper front teeth region. The patient was unwilling for orthodontic correction since time was the major constraint. A complete intraoral and extraoral examination was performed, preoperative photographs were taken, and study models were prepared. Tooth component, i. e., dental midline, incisal lengths, tooth dimensions, zenith points, axial inclinations, interdental contact area and point, incisal embrasure, symmetry, and balance, was observed. The soft-tissue examination included gingival health, interdental embrasure, and smile line. Clinical examination revealed diastema between 11, 12, 21, and 22. Discoloration was noted in 21 teeth, and the patient reported a history of root canal treatment for the same. Considering the esthetic need of the patient and the duration of treatment, porcelain veneers were planned for the correction of diastema. Tooth preparation for veneers was made with incisal overlap design with respect to 12, 11, and 22, and e-max crown was planned for 21. Background: The aim of the study was to analyze the content and quality of clinical diastema closure videos related to the direct resin composite restoration technique available on YouTubeTM. Materials and Methods: The videos were determined using the words "diastema closure," "teeth gap fill," and "teeth gap close" in the YouTube™ search section. 300 videos were reviewed, and 55 of them were included in the study. Demographics of videos, interaction index, and viewing rates were evaluated. The videos were analyzed in three parameters in terms of content score (CS), technical features (TF), and global quality scale (GQS). Results: The dentist accounts took the first place (51%) in the distribution of those who uploaded the videos. The mean values of the videos from the evaluation criteria were 8.3/15p for CS, 3.1/5p for TF, and 2.7/5p for GQS. There was a statistically significant positive correlation between the viewing rates, TF, and GQS values (P < 0.05). Conclusion: Operational videos of diastema closure with direct composite resin restoration treatments should be uploaded to YouTube™ after approval by experts. In addition, the TF of operational videos needs to be better. Students should be warned about videos which contain insufficient information. [ABSTRACT FROM AUTHOR]

Correction of Class III malocclusions with maxillary deficiency and posterior cross bites in adults can be challenging. Depending upon the severity of the malocclusion treatment options include comprehensive orthodontic treatment with or without surgical aid. With advent of clear aligners systems as Invisalign many patients are seeking orthodontic treatment as an alternative to comprehensive orthodontic treatment with traditional fixed braces. Two cases with posterior cross-bites are presented here. The first case of a 17-year-old female illustrates the difficulty in correcting posterior cross-bites and the second case of a 35-year-old female demonstrates the treatment of posterior cross-bite and maxillary midline diastema using clear aligners (Invisalign®). The exploration of using Invisalign® combined with palatal expanders in conjunction with removable elastics to gain desired results was shown to be a useful device for orthodontic correction. A common consequence of moderate to extreme periodontitis is pathologic migration. This denotes tooth movement when the periodontal disease interjects the equilibrium among the elements that preserve physiological tooth position. The balancing factors can migrate the teeth in any direction. The etiology of pathological migration tends to be multifactorial, thus achieving early diagnosis is imperative, which will ultimately lead to the prompt removal of the etiological factors while avoiding severe bone destruction. In this case maxillary central incisors had diastema due to pathological migration with mobility grade I in maxillary left central incisor. Many cases of moderate to severe pathological migration need a suitable, interdisciplinary approach. Nevertheless, since it is possible to detect mild cases of Pathological tooth migration (PTM) at an early stage, spontaneous correction of migrated teeth can be accomplished by periodontal therapy alone. Background: Crown lengthening is a surgical procedure performed to create a symmetrical gingival line and is carried out to expose necessary additional tooth structure. A short clinical crown occurs as a result of dental malformations, eruption disharmony, and genetic variation. Purpose: The purpose of this case report is to explain the treatment management of crown lengthening over the case of diastema closure by indirect veneer restoration. Case: 32 years old female presented with a chief complaint over a space between her anterior maxillary teeth. She wants to rectify her smile. Case Management: The management in this case are preparation of Digital Smile Design (DSD), crown lengthening on 11 and 21, and indirect veneer on 11, 21, 22. Introduction: A modified frenal treatment for aberrant frenum is presented in this report to reduce scar tissue formation and maintain the attached gingiva. Description: The case report describes two cases in which a V-shaped incision removed the aberrant frenum and then the flaps of the frenum were sutured at the mid line. Results: The results showed reduced scar tissue in the mid line and the tissue healed with adequate attached gingiva. Take-Away Lessons: The modified frenotomy technique presented here is ideal for a large frenum that could expose the underlying connective tissue that could reduce the scar tissue formation. Purpose: This study was performed to identify the prevalence and etiological factors of maxillary midline diastema among a sample of adult patients undergoing orthodontic treatment in orthodontic private clinics in Qaloubya, Fayoum, and Giza governorates. Patients and methods: Three hundred seventy-five patients (199 females and 176 males) were included in this study. The full records (intraoral photographies and radiographs) of all patients were examined and surveyed for the presence or absence of maxillary midline diastema and the possible etiological factors. Results: 17.3% of the studied patients showed maxillary midline diastema, while 82.7% of them did not show diastema. There was no significant difference between the prevalence of midline diastema in different sexes (P = 0.079). The most common etiological factors were 38.5% congenitally missed laterals, 33.8% highly attached frenum, 20.0% generalized spaces, and 26.2% had flared incisors. Conclusion: Prevalence of midline diastema in the studied patients was 17.3%, while 82.7% of patients did not have diastema. [ABSTRACT FROM AUTHOR]

Due to greater awareness about the importance of retaining tooth structure, patients today increasingly tend to select the least invasive treatment. Continuous improvement in the physicochemical properties of restorative dental materials, as well as the refinement of techniques, have led to more conservative therapeutic alternatives for esthetic and functional restorations. Painstaking analysis and diagnosis also are essential to understand the underlying etiology in each case. Purpose The present study aimed to evaluate the periodontal status and the distance between the teeth one year after frenectomy in patients with abnormal frenums in the maxillary and mandibular midline. Materials and Methods This study included 50 patients (24 men and 26 women) between the ages of 13 and 53 who have frenum-induced diastemas between the incisors. The abnormal frenums were removed via conventional frenectomy. The distances between the teeth before and one year after the surgery were measured with a caliper. To determine the periodontal status, the pocket depth, plaque index, and bleeding on probing were measured from four surfaces. In addition, the amount of attached gingiva and degree of gingival recession were recorded and were statistically analysed. Results A significant decrease in the distance between teeth before and after frenectomy was observed (p<0.05). There was a statistically significant difference in the amount of gingival attachment, pocket depth, degree of gingival recession, plaque index, and bleeding on probing (p<0.05). Conclusion The removal of abnormal frenums with frenectomy can contribute to the reduction in the distance between the teeth. Diastema between the teeth negatively affects the patients' smile, psychology and daily activities by creating a disharmony in the patients' face. The development of diastema has been attributed to several factors such as labial frenulum, microdontia, mesiodens, peg-shaped lateral incisors, agenesis, cysts, habits such as finger sucking, tongue thrusting, or lip sucking, dental malformations, genetics, proclinations, dental-skeletal discrepancies, and imperfect coalescence of interdental septum. Patients often present with complex problems that require a multidisciplinary treatment approach which includes determination of the aetiological factors, soft tissue morphology, occlusion, patient demands and aesthetic consideration to achieve satisfactory outcomes. Lack of current literature on classification of diastemas and multi-disciplinary approach of management led to the proposal of a new classification the ATAC (Anatomic and Therapeutic Classification) for management of the diastema. This case report highlights the use of the proposed classification for management of diastemas, requiring a perio-restorative intervention using a Chu's proportion gauge to achieve ideal aesthetics. The evolution of adhesive procedures and ceramic systems has allowed for the placement of minimum thickness laminate veneers on unprepared teeth. A 20-year-old post-orthodontics male patient with multiple diastemas required restorative procedures. A multidisciplinary approach was performed, involving gingivectomy, in-office bleaching, and application of minimum thickness veneers. The immediate and 1-year follow-up results exceeded expectations; however, long-term clinical trials are necessary to evaluate the performance of this new type of indirect restoration.
