In 1952, while working on my Ph.D. at the University of Michigan, two men from the Department of State came to our apartment and picked up my passport and that of my husband, Chandler Davis. In 1962, Chandler was finally allowed to immigrate to Canada with his family and take up a professorship at the University of Toronto. The ten years in between were packed with politics: Chandler's refusal to answer questions before the House Un-American Activities Committee (HUAC); his firing from the University of Michigan; his court case challenging HUAC; his six months' prison term for contempt of Congress; and throughout, his inability to get a tenure-track appointment at any American university. I myself was not called to confess my political views or memberships but investigations against Chandler were based on the publication of Operation mind – a pamphlet which I had co-authored together with Elizabeth Douvan and which is presented here. I was without a passport and not part of a university community for years. This article reflects on the impact of this experience of persecution on my work as a historian, and the relationship between politics, activism, and what Marc Bloch and Lucien Febvre called 'the historian's craft' and 'consciousness'. Arctic Indigenous food systems are typified by diverse social interactions that maintain food security. Given the numerous challenges Arctic Indigenous communities face, it is important to understand diversity in these systems and how they are impacted by change. This study adopts a mixed methods approach that combines participatory methods with network analysis to assess the diversity of food-related interactions between different actor types in the mixed food system on St. Paul Island, Alaska. We conducted 36 interviews with local food system actors to identify current food security challenges and food-related interactions in the local food system. We used network analysis to investigate the diversity of interactions in the food system quantitatively. We simulated the effects of two key food security challenges (out-migration and knowledge loss) on food system diversity and explored interventions to increase diversity. Qualitative data from the same interviews were used to embed our results in the lived realities of the St. Paul Island community. Our approach reveals diverse interactions in the local food system, reflecting both subsistence and market-based elements of the St. Paul Island mixed economy. While both challenges threaten the system's diversity, out-migration presents a more immediate risk to food system diversity than knowledge loss. We advocate for targeting a combination of food system interventions at the community level to enhance food system diversity effectively. Our research contributes to understanding the importance of diversity for Arctic Indigenous food security and emphasizes the need to recognize the complex interactions between food system interventions and their implications for Arctic Indigenous communities. Background: Intern preparation courses are often broad in scope; there are few published specialty‐specific programs outside of General Surgery and Obstetrics. We designed an internal medicine (IM) residency preparatory course at the University of Maryland School of Medicine, which aimed to prepare graduating medical students for the rigours of IM residency training, mapped to Entrustable Professional Activities (EPAs). Methods: Fourteen fourth‐year medical students who were matriculating into IM residency programs enrolled in a 4‐week long residency preparation course. The course was designed to teach skills using case‐based learning modules, specialty topic seminars, simulation laboratories, procedure laboratories and clinical practice. Participants were surveyed before and after the course on their perceived knowledge and ability with the skills tested. Results: With the exception of 'giving signout to a colleague', there was a significant difference in the participant's perceived ability for each skill taught within the course (P < 0.03 for each), with mean pre‐course scores of 1.4–3.7 (SD = 0.5–1.2) and mean post‐course scores of 3.2–4.2 (SD = 0.5–1.3). A second survey on course evaluation and perceived impact, completed 3 months after starting intern year, resulted in all respondents reporting that the information learned during the course had directly affected their care of patients on a daily or weekly basis. The modified Ottawa scale was the primary assessment means for the EPAs, with participants approaching entrustment at the conclusion of the course. Half of this piece appeared under the title "Postscript on Cultivation: Editorial Note" in Common Knowledge 8, no. 2 (spring 2002), and half was written in 2023 by one of the coauthors as a posthumous tribute to the other. The historian Natalie Zemon Davis died on the fourteenth day of the latest war between Hamas and Israel in Gaza. The relevance of "Postscript," which was written following the attacks by al‐Qaeda in the United States on September 11, 2001, is that it redirects the attention of scholars from the battlefield and mass media to the ivory towers in which intellectuals are sometimes said to hide. Cultivated civilians ought to know that cultures strive to erase one another in ways vile but unsurprising. Those who learn history expect atrocity and should busy themselves less with assignment of blame than with archaeology, the preservation of objects, the collection of reports and stories, and the reinforcement — by retiring from the streets into libraries and other spaces of patient cooperation — of the partition between barbarism and civilization. With reference to the religious wars of the sixteenth century, during which — Davis showed in a famous essay — Catholics and Protestants fought each other by day but met by night to collect and distribute alms, Perl commends the urge to withdraw from both combat and protest into institutions doing good works quietly. This qualitative study sought to identify important physical activity program components for breast cancer. Methods: Women with previous breast cancer (n = 11) and allied health professionals (n = 7) participated in one-on-one semi-structured interviews (n = 15) or focus groups (n = 1). Qualitative data were analyzed using reflexive thematic analysis methods. Results: Four main themes were generated including (1) the need for physical activity programs; (2) person-centered programs; (3) flexible physical activity programs; and (4) systems factors. These reflected the health and non-health benefits of physical activity, the need to facilitate agency, the diversity in individual characteristics, preferences, abilities, and commitments of people with lived experience of cancer, as well as the need for physical activity programs to be integrated within the broader health system. Conclusion: Strategies to support physical activity engagement for breast cancer should embrace the diversity of those who are diagnosed with cancer as well as the diversity in which physical activity can be achieved. Due to the shortcomings of traditional schools, innovative supplementary (e.g. out-of-school, summer) programs have been identified as important sites for the positive development and learning of Black youth. This study foregrounds Black youth perspectives to offer additional insight into the role of supplementary programming. Drawing from 15 semi-structured, pre-post interviews with Black youth participating in a six-week summer CDF Freedom Schools program, we analyzed core distinctions drawn between youths' experiences in the program and in their traditional schools. Black youth attested to how the program expanded opportunities for them to express themselves, build community, engage in critical structural analyses, and imagine sociopolitical possibilities beyond the constraints of the present. We conclude with a discussion of implications, with an eye toward further specifying the types of educational contexts required to counteract detrimental aspects of traditional schooling and cultivate dispositions toward more just futures. To determine the effects of a multi-dimensional intervention (consisting of education, recommendations for medical management, and short-term case management) provided by pulmonology nurse practitioners(NP) on inpatient and post-discharge outcomes for patients admitted with asthma exacerbations to a Pediatric Intensive Care Unit(PICU). A retrospective cohort study was completed on subjects with an asthma exacerbation admitted to the PICU from 1 January 2015 to 31 December 2018. Records were reviewed for 12-months post-discharge. We compared inpatient and post-discharge outcomes for those who did vs. did not receive NP consultation. The primary outcome evaluated was optimization of discharge medications. Rates of follow up, repeat ED visits and hospitalizations were also reviewed. Two hundred and twenty two subjects met inclusion and exclusion criteria; of those, 101 (45.5%) patients received NP consultation and 121 (54.5%) had PICU management only. Patients with NP consultation were more likely to have controllers initiated (34.6% vs. 15%) or adjusted (55.5% vs. 33.3%) per asthma guidelines (p < 0.001). The consult group were more likely to have an asthma follow-up appointment made prior to discharge (99% vs. 45%, p < 0.001), and were more likely to attend the appointment (51% vs. 21%, p < 0.001). There were no significant differences between groups for ED visits or readmission for asthma 12-months post-discharge. Patients with NP consultation were more likely to have controllers started or adjusted per guidelines and were more likely to attend specialty follow-up appointments post-discharge. No impact was seen on ED visits or readmissions. Implementation of such a program may aid in optimizing asthma management and continuity of care post hospitalization. [ABSTRACT FROM AUTHOR]

Background: In response to the onset of the COVID-19 pandemic, telehealth was rapidly rolled out in health services across Australia including those delivering cancer care. This study aimed to understand people with cancer and carers' experiences with telehealth for cancer care during the COVID-19 pandemic and associated restrictions. Method: Semi-structured interviews conducted with people with cancer and carers via telephone or online video link between December 2020 and May 2021. Participants were recruited through cancer networks and social media. Interviews were transcribed and thematic analysis undertaken. Results: Twenty-three patients and 5 carers were interviewed. Telephone-based appointments were most common. Responses to telehealth were influenced by existing relationships with doctors, treatment/cancer stage and type of appointment. Four themes were derived: (i) benefits, (ii) quality of care concerns, (iii) involving carers, and (iv) optimising use of telehealth. Benefits included efficiency and reduced travel. Quality of care concerns identified subthemes: transactional feel to appointments; difficulties for rapport; suitability for appointment type and adequacy for monitoring. Both patients and carers noted a lack of opportunity for carers to participate in telephone-based appointments. Aligning appointment mode (i.e. telehealth or in person) with appointment purpose and ensuring telehealth was the patient's choice were seen as essential for its ongoing use. Discussion and conclusions: While telehealth has benefits, its potential to reduce the quality of interactions with clinicians made it less attractive for cancer patients. Patient-centred guidelines that ensure patient choice, quality communication, and alignment with appointment purpose may help to increase telehealth's utility for people affected by cancer. In this review of literature, we attend to some of the ways that well-intentioned hopes for fostering creativity and encouraging greater inclusion may also rely on problematic premises that work to reify exclusionary logics and practices. More specifically, we historicize and critically examine how creativity studies—often despite explicit efforts to broaden notions of creativity, include marginalized populations, and democratize education—have differently reanimated racializing and ableizing discourses over seven decades of efforts to scientifically study, define, and/or cultivate creativity. In our emphasis on the relationship between past and present designs, we attend to more pernicious tensions and concerns that continue to hold implications for scholarship, research, policy, and practice. Background The risk of coccidioidomycosis (CM) as a life-threatening respiratory illness or disseminated CM (DCM) increases as much as 150-fold in immunosuppressed patients. The safety of biologic response modifiers (BRMs) as treatment for patients with autoimmune disease (AI) in CM-endemic regions is not well defined. We sought to determine that risk in the Tucson and Phoenix areas. Methods We conducted a retrospective study reviewing demographics, Arizona residency length, clinical presentations, specific AI diagnoses, CM test results, and BRM treatments in electronic medical records of patients ≥18 years old with International Classification of Diseases (ICD-10) codes for CM and AI from 1 October 2017 to 31 December 2019. Results We reviewed 944 charts with overlapping ICD-10 codes for CM and AI, of which 138 were confirmed to have both diagnoses. Male sex was associated with more CM (P  = .003), and patients with African ancestry were 3 times more likely than those with European ancestry to develop DCM (P  < .001). Comparing CM+/AI+ (n = 138) with CM+/AI– (n = 449) patients, there were no significant differences in CM clinical presentations. Patients receiving BRMs had 2.4 times more DCM compared to pulmonary CM (PCM). Conclusions AI does not increase the risk of any specific CM clinical presentation, and BRM treatment of most AI patients does not lead to severe CM. However, BRMs significantly increase the risk of DCM, and prospective studies are needed to identify the immunogenetic subset that permits BRM-associated DCM. [ABSTRACT FROM AUTHOR]

This paper argues that the terms through which we interpret and work to develop expansive pedagogical practices are overly constrained by the binary of adult-centered versus child-centered education. Analyzing ethnographic data developed over three years in a making/tinkering afterschool program serving Black, Latinx, and Asian American students (K-5), we explicate and imagine beyond this binary by (1) analyzing key forms of pedagogical talk, listening, and embodied assistance that supported generative forms of learning and relationality and defied categorization as either adult- or child-centered; and (2) theorizing joint activity as a pedagogical practice by historicizing and unmooring the work of critical education from the perpetual negation of Western, adult-centered models, thereby creating distinct grounds for specifying the role of direct assistance and its salience for questions of educational dignity and justice. Taken together, we argue for a more complex view of when and how direct teaching can support meaningful learning, and further delineate the relationships between such teaching and a broader ethos of joint, intergenerational activity. Objective  Very low birth weight (VLBW) infants are exposed to medications with insufficient evidence describing pharmacokinetics and safety. Objective was to quantify and identify risk factors associated with the highest quartile of medication exposure. Study Design  Retrospective record review of VLBW infants admitted to a level-IV neonatal intensive care unit (NICU). We obtained baseline clinical and demographic characteristics, as well as data on all medications received during admission. Characteristics of patients within the upper quartile of medication use were compared with remaining patients. Results  Identified 106 infants, mean birth weight (BW) = 961 g, gestational age = 27.3 weeks. Infants received a median = 20 medications (range, 4–72). Those in the top quartile of medication use received ≥30 medications while in the NICU and had higher odds of being male sex, lower BW, longer length of hospital stay (LOHS), and bronchopulmonary dysplasia. Sepsis did not affect medication exposure. Antibiotics, opiates, and reflux medications were among the top prescribed. Conclusion  Infants are exposed to a large number of medications during NICU hospitalization, including potentially unnecessary antibiotics and reflux medications. Male sex, the presence of certain comorbidities such as necrotizing enterocolitis, and LOHS, are associated with higher exposure. A relationship between bitter and fat taste sensitivity, CD36 rs1761667 and TAS2R38 has been demonstrated. However, research is scarce and does not take diet into account. This study aimed to explore associations between genetics, fat and bitter taste sensitivity and dietary fat intake in healthy UK adults. A cross-sectional study was carried out on 88 Caucasian participants (49 females and 39 males aged 35 ± 1 years; body mass index 24.9 ± 0.5 kg/m2). Bitter taste sensitivity was assessed using phenylthiocarbamide (PTC) impregnated strips and the general Labeled Magnitude Scale. Fat taste sensitivity was assessed by the Ascending Forced Choice Triangle Procedure and dietary intake with a semi-quantitative food frequency questionnaire. Genotyping for rs713598, rs1726866, rs10246939, and rs1761667 was performed. Participants with TAS2R38 PAV/PAV diplotype perceived PTC strips as more bitter than groups carrying AVI haplotypes (AVI/AVI, P = 1 × 10−6; AVI/AAV, P = 0.029). CD36 rs1761667 was associated with fat taste sensitivity (P = 0.008). A negative correlation between bitter taste sensitivity and saturated fat intake was observed (r s = −0.256, P = 0.016). When combining the CD36 genotypes and TAS2R38 diplotypes into one variable, participants carrying both TAS2R38 AVI haplotype and CD36 A allele had a higher intake of saturated fat compared to carriers of CD36 GG genotype or TAS2R38 PAV/PAV and PAV/AAV diplotypes (13.8 ± 0.3 vs. 12.6 ± 0.5%TEI, P = 0.047) warranting further exploration in a larger cohort. Objective: To test whether azithromycin eradicates Ureaplasma from the respiratory tract in preterm infants.Design: Prospective, phase IIb randomised, double-blind, placebo-controlled trial.Setting: Seven level III-IV US, academic, neonatal intensive care units (NICUs).Patients: Infants 240-286 weeks' gestation (stratified 240-266; 270-286 weeks) randomly assigned within 4 days following birth from July 2013 to August 2016.Interventions: Intravenous azithromycin 20 mg/kg or an equal volume of D5W (placebo) every 24 hours for 3 days.Main Outcome Measures: The primary efficacy outcome was Ureaplasma-free survival. Secondary outcomes were all-cause mortality, Ureaplasma clearance, physiological bronchopulmonary dysplasia (BPD) at 36 weeks' postmenstrual age, comorbidities of prematurity and duration of respiratory support.Results: One hundred and twenty-one randomised participants (azithromycin: n=60; placebo: n=61) were included in the intent-to-treat analysis (mean gestational age 26.2±1.4 weeks). Forty-four of 121 participants (36%) were Ureaplasma positive (azithromycin: n=19; placebo: n=25). Ureaplasma-free survival was 55/60 (92% (95% CI 82% to 97%)) for azithromycin compared with 37/61 (61% (95% CI 48% to 73%)) for placebo. Mortality was similar comparing the two treatment groups (5/60 (8%) vs 6/61 (10%)). Azithromycin effectively eradicated Ureaplasma in all azithromycin-assigned colonised infants, but 21/25 (84%) Ureaplasma-colonised participants receiving placebo were culture positive at one or more follow-up timepoints. Most of the neonatal mortality and morbidity was concentrated in 21 infants with lower respiratory tract Ureaplasma colonisation. In a subgroup analysis, physiological BPD-free survival was 5/10 (50%) (95% CI 19% to 81%) among azithromycin-assigned infants with lower respiratory tract Ureaplasma colonisation versus 2/11 (18%) (95% CI 2% to 52%) in placebo-treated infants.Conclusion: A 3-day azithromycin regimen effectively eradicated respiratory tract Ureaplasma colonisation in this study.Trial Registration Number: NCT01778634. Background: As determination of brain death is infrequent in neonates, the AAP endorses donation after circulatory determination of death as an acceptable alternative. Despite this recommendation, neonatal organ donation is infrequent. Timely referral to OPOs is a vital first step in the organ donation process. The aim of this study was to identify patient and provider factors impacting timely referral for neonatal organ donation. Methods: Medical records were reviewed for deaths occurring in a Level IV NICU from 2007 to 2017. Clinical and demographic factors, provider type, timing of OPO referral (before or after death), and outcome were assessed. Bivariate and multivariable logistic regression models were utilized to identify predictors of OPO referral characteristics. Results: Between 2007 and 2017, 329 deaths occurred in the NICU or delivery room. Of the 265 infants meeting inclusion criteria, 96% had late referrals (after death) and were declined for organ donation. Frequency of timely referrals (before death) improved when OPO contact was by an attending neonatologist, when withdrawal of life support was planned, and with increasing birthweight, gestational age, and PMA. Factors associated with decreased OPO referral included male sex, lower weight at death, earlier PMA, and deaths occurring while receiving maximal intensive care support. No organs or tissues were donated. Conclusions: This study is the first to report NICU referral patterns for organ donation. We found that timely provider referral of neonates to the OPO was rare. Background: Exposure to ethanol during pregnancy is the cause of fetal alcohol spectrum disorder. The function of L1 cell adhesion molecule (L1), critical for proper brain development, is dependent on detergent‐resistant membrane microdomains (DRM). Ethanol at low concentrations disrupts L1 function measured by inhibition of downstream signaling and alterations in L1‐DRM distribution in cerebellum in vivo and in cerebellar granule neurons (CGN) in vitro. We have previously shown that choline pretreatment of CGN partially prevents ethanol toxicity through improving L1 function in vitro. Here we show that choline supplementation reduces the impact of ethanol on L1 in cerebellum in vivo. Methods: Pregnant rat dams were placed on choline free diet on gestational Day 5 (G5). Pups were treated with saline or choline from postnatal day (P) 1–5. On P5, pups were intubated twice 2 hr apart with ethanol or Intralipid® for a total dose of 6 g/kg/d and sacrificed 1 hr after the last intubation. The cerebella were harvested and L1 phosphorylation/dephosphorylation status and distribution in DRM were analyzed. Results: Ethanol reduced L1 tyrosine phosphorylation and L1‐Y1176 dephosphorylation in cerebella, and caused an increase in the percent of L1 in DRM. Choline supplementation of pups reduced the ethanol‐induced changes in L1 phosphorylation status and ameliorated ethanol‐induced redistribution of L1 into DRM. Resource distribution networks are the infrastructure facilitating the flow of resources in both biotic and abiotic systems. Both theoretical and empirical arguments have proposed that physical systems self-organise to maximise power production, but how this trajectory is related to network development, especially regarding the heterogeneity of resource distribution in explicitly spatial networks, is less understood. Quantifying the heterogeneity of resource distribution is necessary for understanding how phenomena such as economic inequality or energetic niches emerge across socio-ecological and environmental systems. Although qualitative discussions have been put forward on this topic, to date there has not been a quantitative analysis of the relationship between network development, maximum power, and inequality. This paper introduces a theoretical framework and applies it to simulate the power consumption and inequality in generalised, spatially explicit resource distribution networks. The networks illustrate how increasing resource flows amplify inequality in power consumption at network end points, due to the spatial heterogeneity of the distribution architecture. As increasing resource flows and the development of hierarchical branching can both be strategies for increasing power consumption, this raises important questions about the different outcomes of heterogeneous distribution in natural versus human-engineered networks, and how to prioritise equity of distribution in the latter. PURPOSE: Electrophysiological event-related potentials (ERP's) have been reported to change after concussion. The objective of this study is to use a simple 2-tone auditory P300 ERP in routine clinical settings to measure changes from baseline after concussion and to determine if these changes persist at return to play when other standard measures have normalized. METHODS: Three-hundred sixty-four (364) student athletes, aged 17–23 years, participating in contact sports were tracked over consecutive years. In this blinded study P300, plus physical reaction times and Trail Making tests, were collected alongside standard clinical evaluations. Changes in these measures after concussion were compared to clinical outcomes over various stages of post-injury recovery. RESULTS: Concussed players experienced significant reaction time and/or P300 amplitude changes compared to pre-concussion baseline measurements (p < 0.005). P300 changes persisted in 38% of the players after standard measures, including reaction times, had cleared. Many of those players slow to normalize were part of the sub-concussive symptom group and/or appeared more prone to repeat concussions. CONCLUSION: These data suggest significant P300 amplitude changes after concussion that are quantifiable and consistent. These changes often normalized slower than other standard assessments. The study of water as a K–12 science idea often divorces its properties from its deeply politicized history as a resource that has been limited, compromised, and intentionally withheld from nondominant communities. Although a robust body of scholarship has aptly critiqued decontextualized and depoliticized pedagogies and called for critical science-learning environments designed through the lens of equity, historicity, and power, more insight is needed into how children develop in relation to these design imperatives and within sociopolitical contexts where environmental issues pose a direct threat. We report select findings from a 2-year ethnographic project that investigated Black student agency in a school with a place-based design. Specifically, we hone in on the themes of water and water justice, which inspired the development of a socio-scientific unit enacted in two 4th-/5th-grade classrooms. This unit coincided with the initial spike in public awareness around the still unresolved water crisis in Flint, MI. For this article, we situate the "Flint" module as an illustrative case of justice-centered science pedagogy and analyze Black students' disciplinary, affective, and sociopolitical understandings. We found that children's meaning-making shifted from individualized accounts to critical, systemic explanations of environmental justice issues. The saliency of children's affective understandings throughout the unit was also captured. We expound on these findings and conclude with a discussion of implications, particularly as it relates to the ethics and politics of developing critical scientific capacity in young children to confront lived environmental human rights issues. This study compared the characteristics of 136 (M = 46 years, range 21-79) online child sexual abuse offenders in Australia based on theories of characteristics of child sexual abuse contact offenders. Using data gleaned from Australian Federal Police investigations, characteristics that may influence offenders with a preference for increasingly graphic and violent child sex abuse images (based on the Combating Paedophile Information Networks in Europe Project (COPINE) typology of image preferences) were analysed. Important findings were the tendency for escalation in image preference to be associated with callousness in offenders; and a preference for images, video and written stories being associated with a preference for COPINE Levels 8, 9 and 10. Post hoc analysis investigating victim age, gender, format of material and level of deviance (as shown by the highest COPINE ratings) revealed that offenders who preferred all age groups also preferred all genders and formats of material. Objective The objective of this study was to determine whether anthropometric measurements such as head circumference (HC), body mass index (BMI), and body surface area (BSA) can predict car seat tolerance screen (CSTS) failure. We hypothesized that subjects with larger HC and lower BMI/BSA would have increased risk of failure. Study Design We performed retrospective medical record review of infants born in 2013 who qualified for CSTS due to being born preterm (< 37 weeks) or low birth weight (LBW). We performed bivariate analyses of clinical and demographic risk factors including anthropometrics to identify predictors of CSTS failure. Results In this study, 19 of 366 subjects failed their CSTS (5.3%). There were no significant differences in HC, length, BSA, BMI, or ratio of HC to length, BMI, or BSA between those who passed versus failed. On subgroup analysis of preterm infants, the significant predictor of failure was multiple gestation (p = 0.024). In the term LBW cohort, requiring nasal canula (NC) (p = 0.03) and having limited or no prenatal care (PNC) (p = 0.0006) were both significant predictors of failure. Conclusion Neither absolute measurements nor the ratios of HC to body measurements predicted CSTS outcome. Multiple gestation was a significant predictor of failure in preterm infants, while NC requirement and poor PNC were predictors in LBW term infants. [ABSTRACT FROM AUTHOR]

Despite the abundance of literature that attests to a myriad of complex and entrenched problems within urban education, the authors of this forum maintain that in addition to shedding light on oppressive structures and ideologies, critical pedagogues must remain steadfastly engaged in solution-oriented endeavors. Using Cheryl McLaughlin's analysis as a worthy starting point, we consider both old paradigms and new ideas regarding the transformation of public science education. Topics for discussion include theoretical framing, urban teacher preparation, science education reform approaches, and the role of scholarship. While it is evident that securing rigorous and empowering science education for diverse learners will require arduous, collective effort on many fronts, this text upholds a sense of optimism in the potential of activist-teacher-researchers to overcome barriers to liberatory science teaching and learning.
