The article discusses research on gay men's sexual behavior, AIDS and HIV infection. The research on gay man's sexual behavior has posed complex ethnographic and methodological problems which have acquired unorthodox and innovative strategies for their solution. The study included many problems. The issue of whether to take the test was highly contentious within the gay community, and many of the most vocal and articulate gay men were strongly opposed to it. A strategy had therefore to be adopted which would maximize the number of subjects consenting to be tested and make it possible to take the blood samples in a natural setting. It was decided therefore to recommend to subjects that they be tested anonymously, that is without being told the result of the test, but to arrange for the result to be given if he wished it. Confidentiality of data is of course always a problem, especially when dealing with sensitive areas like sex and income. It becomes doubly sensitive when the behavior may be proscribed and illegal, where the sexual orientation of the subjects is often a closely-guarded secret. The most sensitive characteristics concern identity and HIV sero-status. Subjects were allowed therefore to use a pseudonym, with a warning that they should remember what name they had chosen, as they would be contacted in the next wave by that pseudonym. Estimates in these areas of sensitivity lead to prevalence and incidence figures which are systematically downwardly biased.

Elevated urinary uracil concentrations are characteristic of genetic deficiencies of enzymes catalysing the first two steps of the pyrimidine degradative pathway (Van Gennip et al 1993), dihydropyrimidine dehydrogenase (DPD) and dihydropyrimidinase (DHP). DPD is rate limiting for the whole pathway. Homozygotes are detected by the raised levels of uracil and thymine in urine. The pyrimidine synthetic route shares a common intermediate. carbamoyl phosphate, with the urea cycle. In urea-cycle disorders such as ornithine carbamoyltransferase (OCT) deficiency, orotic aciduria resulting from an increased flux through the pathway has been used to detect hemizygotes or carriers following a protein, or more recently, an allopurinol load (Sebesta et al 1994). This paper demonstrates that uracil may be an even more useful guide to OCT carrier status and compares urinary uracil with concentrations found in genetic pyrimidine defects associated with neurological deficits.
