This study presents the first data on a 2018–2021 campaign to monitor polycyclic aromatic hydrocarbon levels, PAHs, in the final stretch of the Hooghly River in West Bengal, India. The range of sedimentary PAHs was 0–47,366 ng/g, higher than the ranges given by the literature for comparable study areas. The assay reveals an outstanding level of PAHs contamination in the fine sediments of the Hooghly River and Sundarban wetland, where the dominance of 4–6 ring PAHs was 83% of the total. The diagnostic ratios based on molecular ratios of PAHs show that the pollution comes mainly from the combustion of petroleum. The ratio of anthracene relative to anthracene plus phenanthrene, ANT/(ANT + PHE), was >0.30, which is higher than the reference discriminant ratio of >0.10, suggesting that PAHs were from the combustion source. In the meantime, fluoranthene over the sum of fluoranthene plus pyrene, FLT/(FLT + PYR), was >0.5 and indicated coal combustion, in agreement with the literature. The mean level of carcinogenic hydrocarbons was at 18% of the total measured PAH, with a peak of 91%, revealing significant potential risk for humans and ecosystems. The toxicity equivalence factors, TEF, of the individual PAHs and the total BaP equivalent toxicity, TEQ, were adopted as a comparison reference of sediment quality. At most sites, toxic effect ranges were classified as high and very high. The results of this research call for public authorities to remedy a situation of severe ecological risk. Microplastics, MPs, in aquatic environments pose serious threats when associated with other pollutants, such as pharmaceuticals, PHs. This review is a continuation of an earlier paper on the role of MPs as containers and carriers of heavy metals, HMs, persistent organic pollutants, POPs, pharmaceuticals, PHs, and personal care products, PCPs, in marine environments and published in the Journal of Marine Science and Engineering. The current effort aims to elucidate the most recent data on the interaction and association of MPs with PHs and the ecotoxicological implications on food webs. This review focuses on the nature of the interaction from different perspectives, such as the hydrophobicity and hydrophilicity of the polymer and drug, the polymer surface, and the rate of weathering. The effects of environmental conditions, such as mechanical stress, photodegradation, pH, salinity, dissolved organic matter, and gastrointestinal features of marine biota, were reported. This review reports on experimental laboratory, mathematical, and field data on MPs' carrier and accumulation role in PHs and their release and ecotoxicological effects on water bodies. From the survey of the data, it emerges that the nature of the interaction and the effects on biota are very complex and variable, and perhaps only a systematic mechanic approach of data collection with a statistical approach using big data and deep learning will contribute in the future to clarify. The review examines the most recent advances on the effect of persistent pollutants on sea turtles. Research on this topic is still limited, especially that related to toxicity, since they are protected species and in vivo toxicity studies are difficult, with most studies carried out on deceased individuals. Besides toxicology, the review considers direct links to pollution sources, reproduction impact, health effects and biomarkers of pollution exposure. Little attempts have been made so far to gather data that would provide insight into the causes of the observed health trends. Considering this, studies correlating PPs accumulation with health parameters were also discussed. Coastal erosion affects several beaches worldwide in association with natural and human pressures. In recent decades, the coastland located on the Paraíba do Sul River mouth has been recognized as one of the main areas that suffer from this geomorphological dynamic. Furthermore, the scientific literature, even if scarce, has pointed out that these dynamics are causing negative socio-environmental responses and making the area less resilient. In the last few decades, hundreds of homes and other urban structures have become debris due to erosion caused by sea ingression, triggered by the coupling of environmental processes and dynamics occurring at different spatial and temporal scales. Thus, the present research aimed to analyze how the anthropic changes carried out in the Paraíba do Sul hydrographic basin are related to the intense coastal erosion at the mouth. Variations in fluvial–marine hydrosedimentary dynamics triggered this due to the construction of dams to supply large cities and for agricultural purposes. Water and sediment samples were collected and analyzed in different sectors of the main channel and on the adjacent coast, mapping land use in the basin area and surveying socioeconomic data. The results suggest that human pressures on the hydrographic basin during the last century are directly associated with the imbalance of the environmental system and, consequently, the progression of coastal erosion on the Paraíba do Sul delta shoreline. A peri-implant fracture near the volar plate of the distal radius represents a rarity and can be associated with a mechanical failure of the devices. A literature review was conducted including all fractures that occurred around a volar wrist plate, which could be associated with an ulna fracture. All articles published until December 2021 were considered according to the guidelines presented in the PRISMA Statement. The search was conducted with the PubMed electronic database, Cochrane Database of Systematic Reviews, Medline, Embase, and Google Scholar. Only nine cases of these fractures were reported in the literature. The causes could be due to delayed union/non-union of the old fracture after low energy traumas, high energy trauma in patients with poor bone quality, or hardware mechanical failure. Furthermore, the literature review of peri-implant radius fracture shows different level of radius fracture and types of implant failure. In the last decade, metal engineered nanomaterials (ENMs) have seen an exponential use in many critical technologies and products, as well an increasing release into the environment. Coastal ecosystems worldwide may receive ENM-polluted waters and wastes, with a consequent alteration of habitats and contamination of aquatic biota. There is a scarcity of data regarding the fate of these emerging contaminants in such environments. Open issues include the determination of the sources, the quantification of the interactions with marine sediments, the bioaccumulation pathways, the ecotoxicology on marine fauna and the identification of the principal biotic and abiotic factors that may alter metal ENMs toxicity. Little is known about their potential transference into the food web, as well toxicity features and co-stressors of single or multiple ENMs under laboratory and real environmental conditions for various taxonomic phyla. This review reports current knowledge on the ecological impact of ENMs under the complex environmental conditions of estuary systems, identifies gaps in current knowledge and provides directions for future research. The spatial distribution of trace elements in surface sediments of the Hooghly estuary was studied over the monsoons in 2014–2017. As, Cd, Ni, Pb and U were two- to sixteen-fold the crust means with increasing levels toward the estuary, with Ni peak during the post-monsoon. Pearson's correlation matrix, cluster analysis, enrichment factors and pollution index revealed the anthropic source and association of trace elements with Fe, Mn and Al and of Pb with U. Geoaccumulation index revealed for Ni an extremely contaminated situation at the estuary water during monsoon and for Cd a heavily contaminated situation at freshwater location. The potential contamination index was >6; thus, sediments were very severely contaminated by As, Cd and Ni with worst situation for As and Cd at fresh and brackish water and during post-monsoon. The shallow water benthic foraminiferal and ostracod assemblages of the Gulf of Pozzuoli, located in the central Tyrrhenian Sea, were studied to investigate the relationship between calcareous meiofaunas and contaminant concentrations in bottom sediments exposed to prolonged industrial pollution. Both benthic foraminifers and ostracods displayed high-diversity and low-dominance, unusual features in highly contaminated environments. High-diversity values were possibly linked to the oligotrophic, well-oxygenated, and CaCO3-supersaturated coastal Mediterranean waters. The comparison with historical data suggested that assemblage composition changed in the last decades, with an increase in the relative abundance of benthic foraminiferal (Quinqueloculina seminulum, Bulimina elongata) and ostracod (Xestoleberis, Loxoconcha, Semicytherura rarecostata) taxa. They probably represent organisms tolerant to the environmental variations in the last decades. The relationships between granulometry and diversity indices, high correlation values between Quinqueloculina lata and heavy metal pollution, and the preference of the ostracod genera Urocythereis and Paracytheridea for very shallow marine waters were highlighted. Self-inflicted fractures simulating traffic accident represent a new social fraud opportunity for criminality. Recognising scams through an increase of awareness of existence of self-inflicted arm fractures for insurance fraud could help community health workers to report these injuries to the competent authorities. In this article, authors have recognised an unusual but consistent pattern of upper and lower limb fractures whose incidence does not coincide in numerical terms with what is reported in literature. The aim of the present study is to describe fracture patterns observed over the past 2 years. Introduction: We aimed to evaluate the efficacy of the use of the electromagnetic distal targeting system in treating humeral shaft fracture. Methods: Patients were divided in: Group 1) patients that received a distal locking screw placement following the free-hand technique; Group 2) patients in which the distal locking screw was performed using the SURESHOT device. Results: No differences were noted comparing Group 1 (freehand) [71,9 range 40-135 min] to Group 2 (SURESHOT)[ 70, range 25-125 min]. Background. Healing of tibia fractures occurs over a wide time range of months, with a number of risk factors contributing to prolonged healing. In this prospective, multicentre, observational study, we investigated the capability of FRACTING (tibia FRACTure prediction healING days) score, calculated soon after tibia fracture treatment, to predict healing time. Methods. The study included 363 patients. Information on patient health, fracture morphology, and surgical treatment adopted were combined to calculate the FRACTING score. Fractures were considered healed when the patient was able to fully weight-bear without pain. Results. 319 fractures (88%) healed within 12 months from treatment. Forty-four fractures healed after 12 months or underwent a second surgery. FRACTING score positively correlated with days to healing: r=0.63 (p<0.0001). Average score value was 7.3 ± 2.5; ROC analysis showed strong reliability of the score in separating patients healing before versus after 6 months: AUC = 0.823. Conclusions. This study shows that the FRACTING score can be employed both to predict months needed for fracture healing and to identify immediately after treatment patients at risk of prolonged healing. In patients with high score values, new pharmacological and nonpharmacological treatments to enhance osteogenesis could be tested selectively, which may finally result in reduced disability time and health cost savings. Introduction: We conduct a systematic and qualitative review of the current literature to evaluate studies that described bilateral ruptures of the extensor mechanism of the knee. Methods: A comprehensive literature search was performed to evaluate all studies included in the literature until September 2016. Results: Fourteen studies with a total of 44 patients met the inclusion criteria. There were 14 patients with CRF (61%), 6 patients were affected by diabetes mellitus (14%) while other 6 patients were obese patients (14%). Purpose: To investigate the effect of applying an additional suture to enhance the biomechanical behavior of the suture-meniscus construct used during the transtibial pull-out repair technique.Methods: A total of 20 fresh-frozen porcine tibiae with intact medial menisci were used. In one half of all specimens (N = 10), two non-absorbable sutures were passed directly over the meniscal root from the tibia side of the meniscus to the femoral side (2SS). In other ten specimens, three sutures were passed over the meniscal root (3SS). All specimens were subjected to cyclic loading followed by load-to-failure testing. Displacement of the construct was recorded at 100, 500, and 1000 cycles. Further, stiffness (500-1000 cycles) and ultimate load and modes of failure of the suture-meniscus construct were also recorded.Results: There was no statistically significant difference between the Group 2SS and Group 3SS at the 1st (1.6 ± 0.7 vs 1.4 ± 0.4 mm) and the 100th cycle (2 ± 0.7 vs 1.8 ± 0.4 mm). At 500 and 1000 cycles, the 2SS fixation technique resulted in significantly more displacement than the 3SS fixation technique (2.8 ± 0.6 vs 2.3 ± 0.5 mm; 3.1 ± 0.7 vs 2.5 ± 0.5 mm) (p < 0.05). No differences between two groups were noted concerning ultimate load to failure and stiffness (500-1000 cycles).Conclusion: Three single sutures technique provided superior biomechanical properties compared with the two single sutures technique during the conducted fatigue tests.Clinical Relevance: Applying three simple stitches during meniscal root repair might be beneficial for healing of the posterior meniscal root, potentially reducing the post-operative immobilization time. Objective: To report our experience with the hybrid external fixator in emergency. Methods: We assessed 80 cases of pilon fracture treated with the external fixator during the period of January 2009 and December 2016:55 men (69%) and 30 women (33%) with a mean age of 40 years (range between 16 and 70). About 45 occurred as isolated trauma, 35 instead were politrauma. Each patient underwent standard radiographic examination and a CT examination. There were 28 open fractures (35%), (Gustilo type 1, 2 and 3) while closed fractures showed soft tissue involvement of various grade (2-3 Tscherne classification). In all cases, the external fixation, sometimes associated with other reduction and synthesis techniques, was used. The timing of surgery was dictated by the condition of the soft tissues. For clinical evaluation, the Mazur score with mean follow-ups at 12 months was utilized. Results: The final range of ankle motion was 15° dorsal and 10 plantar flexion. In about 80 cases there was an average Mazur score of83. The mean score was 90; in open fractures 85 to 72. Radiographic healing of fractures in 60 patients occurred in 120 days (mean 105 days), at the time when the external fixator was removed. Conclusion: Pilon fractures are complex and often present complications; the definitive treatment, in emergency or delayed, with hybrid external fixator permits a stable synthesis with minimal soft tissue damage. Purpose: The purpose of this study was to characterize the biomechanical effect of two grasping suture techniques used during ligament reconstruction: the modified rolling-hitch (MRH) and the modified finger-trap (MFT).Methods: Flexor profundus tendons were harvested from fresh pig hind-leg trotters. Each specimen was mounted on an electro-mechanic universal testing machine (Instron 3367). In half of all tendons (15 specimens), the suture was passed around the tendon following the MRH knot (Group 1). In the remaining half of all tendons (15 specimens), the suture was passed over a distance of 30 mm according to the MFT suture technique (Group 2). As per standard intra-operative technique, a 1 cm residual tendon stub was left free from suture in all samples. All specimens were preconditioned to a load of 50 N for 10 min, followed by three cycles loading between 50 and 120 N. At this point, each sample was cyclically tensioned between 35 and 240 N, at 1 Hz for 200 cycles. Load-to-failure test was then carried out at a rate of 200 mm/min.Results: Rupture of the suture material at the knot was the mode of failure in all specimens during the loaded to failure test. Significant difference was found between Group 1 vs Group 2 for the elongation between the 0th cycle and 10th cycle, the elongation between the 10th cycle and 200th cycle, the mean stiffness at the 10th cycle, and the mean stiffness at the 190th cycle. No significant differences were noted between Group 1 and Group 2 concerning the ultimate load-to-failure.Conclusion: This study showed that both suture methods appear to be biomechanically effective in a porcine tendon model. Purpose: The purpose of the present study was to assess the risk of femoral tunnel collisions between the medial collateral ligament (MCL) and the posterior cruciate ligament (PCL) tunnels during a simultaneous PCL and MCL reconstruction.Methods: Fourth generation medium and large synthetic femur bones were used. On each femur, a MCL tunnel and a PCL tunnel were reamed. The MCL tunnel was drilled at 0°, 20° and 40° of axial and coronal angulations. The PCL femoral tunnel was reamed to simulate two different tunnel directions that could be obtained through an inside-out and outside-in technique. Tunnels were filled with epoxy resin augmented with BaSO4, and a multidetector CT examination of each specimen was performed.Results: High rate of tunnel collision (62.5 %) was found when the MCL femoral tunnel was reamed with a coronal angulation of 0° and 20°. The rate of tunnel collision significantly decreased (0 %) when the MCL tunnel was reamed proximally with a coronal angulation of 40°. No differences were found between the two PCL tunnel directions in terms of tunnel collision.Conclusion: The results of this study can help surgeons to better direct the femoral MCL tunnel in order to avoid a collision between femoral tunnels during a combined MCL and PCL reconstruction. In order to minimize such potential complications, the MCL tunnel should be created limiting the axial angulation and it should be drilled with a proximal angulation from 20° to 40°, depending on the medial condyle width. Background: Symptomatic hardware represents the most frequent complication reported following surgical treatment of patellar fracture. For this reason, some authors suggested using nonabsorbable sutures to fix the fracture with various techniques. The aim of this study was to evaluate clinical and radiological results of patients treated following a modified Pyriford technique using a FiberWire suture (Arthrex, Naples, FL, USA). Materials and Methods: We retrospectively evaluated a case series of seventeen patients with displaced patellar fractures treated by open reduction and internal fixation with a modified tension band using FiberWire sutures. Clinical and radiological outcome were evaluated. Union time, complications, and reoperation rate were observed and recorded. Results: All fractures healed (time to union 9.2 ± 2 weeks), and no fixation failure was observed. Slight losses of reduction (<4 mm) were noted in two patients at 4 weeks postoperatively. The average Lysholm and Bostman scores at the final follow-up were 91 ± 5.7 (range 83-100) and 28.3 ± 1.6 (range 26-30), respectively. Conclusion: Modified tension band using FiberWire sutures showed satisfactory clinical results, with a low incidence of complications and reoperations. Background: Reduction of tibial plateau fractures and maintain a level of well aligned congruent joint is key to a satisfactory clinical outcome and is important for the return to pre-trauma level of activity. Stable internal fixation support early mobility and weight bearing. The augmentation with bone graft substitute is often required to support the fixation to mantain reduction. For these reasons there has been development of novel bone graft substitutes for trauma applications and in particular synthetic materials based on calcium phosphates and/or apatite combined with calcium sulfates. Injectable bone substitutes can optimize the filling of irregular bone defects. The purpose of this study was to assess the potential of a novel injectable bone substitute CERAMENT™/BONE VOID FILLER in supporting the initial reduction and preserving alignment of the joint surface until fracture healing. Methods: From June 2010 through May 2011 adult patients presenting with acute, closed and unstable tibial plateau fractures which required both grafting and internal fixation, were included in a prospective study with percutaneous or open reduction and internal fixation (ORIF) augmented with an injectable ceramic biphasic bone substitute CERAMENT™/BONE VOID FILLER (BONESUPPORT™, Lund, Sweden) to fill residual voids. Clinical follow up was performed at 1, 3, 9 and 12 months and any subsequent year; including radiographic analysis and Rasmussen system for knee functional grading. Results: Twenty four patients, balanced male-to-female, with a mean age of 47 years, were included and followed with an average of 44 months (range 41-52 months). Both Schatzker and Müller classifications were used and was type II or 41-B3 in 7 patients, type III or 41-B2 in 12 patients, type IV or 41-C1 in 2 patients and type VI or 41-C3 in 3 patients, respectively. The joint alignement was satisfactory and manteined within a range of 2 mm, with an average of 1.18 mm. The mean Rasmussen knee function score was 26.5, with 14 patients having an excellent result and the remaining 10 with a good result. Conclusion: It can be concluded that radiological and clinical outcome was satisfactory and obtained in all cases without complications. This injectable novel biphasic hydroxyapatite and calcium sulfate ceramic material is a valuable armamentarium in the treatment of trauma where bone graft is required. [ABSTRACT FROM AUTHOR]

We studied the contamination level of the soils of the Sarno River basin in southwestern Italy by combined acute toxicity test with Dapnia magna and chemical extraction of polycyclic aromatic hydrocarbons (PAHs) and organochlorine pesticides (OCPs). For the ecotoxicological assessment, 188 samples were taken and coincided with those of a previous study (2013) where heavy metals were surveyed. For the organics assessment, 21 samples were selected nearby representative areas of elevated anthropic pressure. About 10.1 % of the samples showed noticeable sign of D. magna mortality, 61-100 %, and fall along the potentially floatable areas of Sarno and Solofrana basins with high degree of contamination by Cr, As, Zn, and Hg. High levels of ecotoxicity, 61-100 %, were determined in the lower Sarno River basin in areas of moderate or low degree of contamination by Cd, Cu Hg, Pb, Sb, Sn, and Zn. Benzo(a)pyrene, indenopyrene, and benzo(g,h,i)perylene were present at concentrations of 0.32, 0.23, and 0.18 mg kg, respectively, 2- to 3-fold the law limits with most of the samples falling nearby the points where the ecotoxicity output was close to 100 %. Among OCPs, pp′-DDT had a mean of 0.225 mg kg and hence about more than 200- and 2-fold the residential, 0.01 mg kg, and commercial/industrial limits, 0.1 mg kg and determined mainly in the central Sarno valley in an area where elevated concentrations of benzopyrene and D. magna mortality were also observed. Purpose: A suture passed along the part of the graft that will be inserted into the femoral tunnel is widely used by surgeons, because it could prevent the graft sliding on the femoral fixation device during pulling from the tibial side. The aim of this study was to evaluate the biomechanical effects of suturing the intratunnel femoral part of the graft during an anterior cruciate ligament (ACL) reconstruction. Methods: Bovine digital extensor tendons and tibias were harvested from 20 fresh-frozen mature bovine knees ranging in age from 18 to 24 months. Quadruple-strand bovine tendons were passed through the tibial tunnel and secured distally with a bioabsorbable interference screw. In one half of all grafts ( N = 10), the looped-over part of the graft was sutured in a whipstitch technique over a distance of 30 mm (Group 1). In one half of all grafts ( N = 10), the looped-over part was left free from any suture (Group 2). The grafts were preconditioned at 50 N for 10 min, followed by cyclic loading at 1 Hz between 50 N and 250 N for 1,000 cycles. Load-to-failure test was then carried out at a rate of 1 mm/s. Results: There was no statistically significant difference between mean stiffness at pullout and yield load between the two groups. In all specimens on Group 1, failure occurred following to partial breaking and then slipping of the tendons between the screw and the tunnel. Concerning Group 2, in six cases failure occurred as described for Group 1 specimens. In the remaining four cases, failure occurred entirely through the ligament mid-substance. Conclusions: Suturing in a whipstitch fashion the femoral portion of the graft doesn't affect the mechanical proprieties of the ACL graft. When suspension fixation device is used, suturing the looped-over part of the graft could be helpful in order to provide equal tension in all of the strands of the graft at time of tibial fixation. Two olive mill wastewaters (OMW) samples, OMWa and OMWb, containing different polyphenolic loads were used for decontaminating an unauthorized dump site in the Campania region, south Italy. In a bench-scale experiment, OMWa at pH 6.0 (OMWa pH6.0) and 4.7 (OMWa pH4.7), OMWb at pH 4.7 (OMWb pH4.7) and OMWa free of the polyphenolic moiety polyphenol-free OMWa (PF-OMWa) were added to the soil for a 96 h contact time. At 96 h, OMWa pH4.7 was more effective than OMWa pH6.0, with Cd, Cu, Pb and Zn removal percentages of 30.7–68.1. Cd and Pb levels were 6.0 and 915 mg kg −1, respectively, decreasing below the regulatory limits for industrial and commercial areas (15.0 and 1 × 10 3 mg kg −1, respectively). A threefold decrease in Zn levels was also observed from 13.5 × 10 3 to 4.3 × 10 3 mg kg −1. The metal removal efficiency of PF-OMWa dropped from 30.7 % to 15.6 % for Cd and from 37.9 % to 1.3 % for Pb. OMWb pH4.7 at 96 h was more efficient than OMWa pH4.7, with mean removal percentages of 32.5 versus 7.8, respectively. [ABSTRACT FROM AUTHOR]

The long-term variability of total Cu content from fungicides applied in a certified wine region of Spain (La Rioja) and of other metals (Cd, Cr, Ni, Pb, and Zn) was evaluated in three young vineyard soils and subsoils unamended and amended with spent mushroom substrates (SMS) over a 3-year period (2006-2008). SMS is a promising agricultural residue as an amendment to increase the soil organic matter content but may modify the behaviour of metals from pesticide utilisation in vineyards. Fresh and composted SMS was applied each year at a rate of 25 t ha (dry-weight). Copper concentrations in the three unamended soils were 21.2-88.5, 25.5-77.1, and 29.4-78.4 mg kg. They exceeded natural Cu concentrations of the region and reference sub-lethal hazardous concentration for soil organism. The concentrations of Cd, Ni, Pb, and Zn were largely below the sub-lethal limits. Thus, although Cu levels were lower than those of established vineyards, vine performance, and productivity might be affected. The variation in behaviour between different amendments for each soil was high, so a generic conclusion could not be drawn. The amendment practice seemed to have caused temporarily Cu mobilization respect to untreated soils. Total zinc concentrations fall within the range of the natural soil of La Rioja and were significantly affected ( p < 0.05) especially by fresh state SMS addition, with increasing up to 75% respect to untreated specimen. The results indicated a build-up of fresh sites for metal retention at both surface and subsurface level, although no accumulation of metals was observed in the short-term period. Enormous progress has been made in the field of hip replacement surgery over the past decades, and challenges once thought insurmountable have been overcome. Alongside a natural evolution of prostheses and surgical techniques, there has been a progressive increase in the number of younger patients undergoing this type of surgery. Given the longer life expectancy of such patients and the high risk of revision associated with hip replacement surgeries, orthopedic surgeons and researchers are increasingly pursuing techniques and devices that can be used with utmost respect for the natural anatomy and biomechanics of the hip. Consequently, short-stem prostheses are being widely recommended. This type of prosthesis allows the preservation of the femoral neck, there-fore offering the possibility to use conventional prostheses also in eventual revision surgeries instead of specific revision prostheses, offering multiple benefits for the patient. Fractures of the distal radius are the most common type of fractures in the upper extremities, accounting for 17% of all fractures treated in the Emergency Department. Treatment techniques are constantly evolving: while the classic treatment of choice involved a plaster cast and immobilization, today, thanks to studies on the biomechanics of fractures and the ongoing innovation in the field of fixation devices, the preferred treatment methods are increasingly geared towards a faster and more physiological functional recovery. In this article, the authors provide a detailed overview of the treatment methods in use today, such as the latest generation of plates and screws used for the reduction and stabilization of fractures, which allow an early mobilization and respect the complex anatomy of the wrist. Femoroacetabular impingement (FAI) is a recently defined condition that affects mainly young adults, often athletes, and represents one of the most common causes of osteoarthritis of the hip. An early diagnosis is especially important to successfully treat this condition; in fact, if the hip joint is not yet compromised on initial diagnosis, it is possible to improve the prognosis of the patient thanks to conservative surgical treatment. Purpose: The purpose of this study was to evaluate the risk of tunnel collisions of the fibular collateral ligament (FCL) and posterolateral bundle anterior cruciate ligament (PLB-ACL) tunnels during a combined FCL and double-dundle (DB) ACL reconstruction. Methods: Thirty-six 4th-generation synthetic femurs (Sawbones, Pacific Research Laboratories, Vashon, WA) were utilized, and two different femur sizes were used. A FCL tunnel and a PLB-ACL tunnel were reamed on each femur. The tunnels of synthetic specimens that did not have a collision were filled with an epoxy resin augmented with BaSO and radiographic evaluation, and Multidetector CT exams of the specimens were performed. Results: The rate of tunnel collision when the FCL tunnel was reamed to a depth of 30 mm was 75 and 69.4% for the 25 mm depth. There was a significantly increased risk of tunnel collision when the FCL tunnel was reamed proximally with coronal angulations of 20° and 40°. No collisions were noted when the FCL tunnel was reamed parallel to the distal condylar line and with axial angulations of 20° and 40°. Conclusion: This study provides new insight into tunnel positioning during a combined FCL and DB-ACL reconstruction. The results show that a concomitant FCL injury do not represent a contraindication to perform a DB-ACL reconstruction as long as the FCL tunnel is reamed with no proximal angulation and is directed anteriorly with an axial angulation between 20° and 40°. Isolated posterolateral corner (PLC) tears are relatively rare events. Various surgical techniques to treat posterolateral knee instability have been described; because surgical results are linked to cruciate reconstructions it has been difficult to date to define whether one surgical procedure has better prognosis than another. The goal of this study is to determine the clinical outcome of PLC reconstruction following fibular-based technique. We retrospectively evaluated a case series of patients who received isolated PLC reconstruction between March 2005 and January 2007. Ten patients were surgically treated for isolated injuries and were available for follow-up; average patient age was 27.4 years (range 16–47 years). All patients were treated following the fibular-based technique: double femoral tunnel was performed in six patients, while in the remaining four patients, the reconstruction of the PLC was performed with a single femoral tunnel. Six patients had semitendinosus allograft and four had semitendinosus autograft. All patients had the same evaluation and the same rehabilitation protocol. Mean follow-up was 27.5 months (range 18–40 months). Mean range of motion (ROM) was 143.5° for flexion (range 135–150°) and 0.5° for extension (range 0–3°). Three patients showed 1+ on varus stress test, while on Dial test another three patients showed 10° reduction of external rotation compared with contralateral knee. The average Lysholm score was 94 points (range 83–100), and the mean International Knee Documentation Committee (IKDC) subjective result was 88.48 (range 74–96.5). Based on Lysholm score, the results were excellent in eight knees and good in two knees. On IKDC evaluation, two patients were grade A and eight were grade B. No significant difference in clinical results was observed between single and double femoral tunnel. The 5-day biological oxygen demand (BOD5) remains a key indicator for proof of compliance with environmental regulators in the monitoring and management of winery effluent. Inter-conversion factors from alternative tests that are more rapid, accurate and simpler to perform have been determined that allow prediction of BOD5 in winery wastewaters, generally, and at different stages of production and treatment. Mean values obtained from this dataset offer rule of thumb interconversion factors: BOD5 = 0.7 Chemical Oxygen Demand (COD), BOD5 = 2.3 Total Organic Carbon (TOC) and BOD5 = 2.7 Dissolved Organic Carbon (DOC). Specific predictive linear relationships are also provided. Out of the relationships between BOD5 vs COD, TOC and DOC, in winery wastewater, irrespective of vintage or non-vintage production periods and stage of treatment, TOC offered the most reliable prediction of BOD5. Ethanol, glucose and fructose were evaluated in untreated wastewater as predictors of BOD5 due to their high specificity in winery effluent. A significant relationship was determined between BOD5 and (ethanol + glucose + fructose; R² = 0.64, n = 19; p , 0.05), but relationships between BOD5 and ethanol and BOD5 vs (glucose + fructose) were weak (R ² = 0.45 and 0.34; n = 19; p , 0.05 respectively,). There was a very strong linear correlation (y = 0.9767x+52.8; R ² = 0.97; n = 23; p , 0.05) in COD data in winery effluents when using a commercially available mercury free test kit compared with using a traditional COD test kit that contained mercury. This suggests that mercury free COD test kits could be used by the wine industry for organic pollution assessment with associated reductions to user and environmental risk, as well as reducing the costs of kit waste disposal. A pilot-scale winely wastewater treatment system was developed to treat wastewater produced by a small winery (approximately 1200 metric tons of grapes crushed). The pilot system consisted of a sedimentation/aerobic process combined with a bioremediation wastewater cell planted with Juncus ingens. The main design specifications, detailed descriptions of the plant, and analysis of the influent and effluent character- istics (pH, electrical conductivity, total suspended solids, chemical oxygen demand [COD], etc.) are reported for each segment of the system. Over the study period, the mean winery wastewater flowrate was 3.5 m3/d at organic loads of 5000 to 14 000 mg-COD/L. The study measured average removal rates of 72% for COD and 65% for total organic carbon and dissolved carbon. The application of wastewater to the soil increased the soil salinity in the top 30 cm, but remained stable below this. The system also seemed to be effective at neutralizing the pH of the acidic winery wastewater and at removing the phosphorus pool (65%) in the wastewater, whereas the levels of nitrogen and most of the cations increased in the treated effluent. The absorbing/adsorbing and degradation capacity of the soil of the wastewater bioremediation cell did not appear to be exhausted after one vintage. Juncus ingens appeared to grow moderately well, until the end of the vintage, when dieback began to occur. An infilling with organic matter of the surface soil layer under the root zone was observed, which reduced water infiltration and hence system treatment capacity. A study was conducted to determine the contaminant level of the Sarno River basin, a small creek located in southwestern Italy. Six sampling site locations were selected to coincide with two previous studies conducted in 1975 and 1985 to provide a basis for comparison. For each location, twelve samples were collected over the period October 1997–September 1998. Results indicated a degradation in river water quality, especially near the mouth. This was reflected by a dissolved oxygen concentration of about 0.8 mg L-1 near the urban areas of Scafati, Pompei and Castellamare of Stabia. At the same locations, BOD5 and COD values were two to five times higher than the allowable regulatory maximum levels, respectively. About 4 km from the headwaters, the average oxygen concentration was 4.7 mg L-1, BOD5 values were close to the standard limit (37 vs 40 mg L-1), and COD values were above the standard limits (252 vs 160 mg L-1). Highest concentrations were detected during peak tomato production. During this time, settleable solids were also elevated, two to six times the regulatory standard. Coliform bacteria densities were found in excess of the standard limits at all sites. Near the head of the river, mean fecal coliform (22,571 MPN mL-1) and streptococci densities (14,214 MPN mL-1) surpassed the regulatory level of 120 and 20 MPN mL-1, respectively, reflecting the input from the urban settings of Sarno and agricultural fields. The heavy metal dissolved concentrations were low at all sampling sites. Most of the analyzed contaminants increased when comparend with 1985 data. BOD5 increased 5 to 10 times at sites near the urban settings of Scafati, Pompei and Castellamare di Stabia, whereas COD increases were twofold at all sites. The production of rare earth elements, REE, has significantly increased over the past years, in parallel with the latest advances in nanotechnologies and representing a new group of emerging contaminants. They find application in construction, transport, agriculture, electronics, catalysis, and biomedicine. Their extraordinary intrinsic characteristics are fundamental for overcoming current technological challenges. The accumulation of REE is consistent in near-shore waters being affected by runoff, wastewater discharge, and proximity to built-up areas. Bioavailability in water, sediments, and accumulation in marine biota as well their endocrine disruptor effect is mostly unknown. There is a significant gap of knowledge on the ecotoxicological behaviour of REE in marine areas. The existing investigations have been performed inside well-mixed estuarine systems, due to complex hydrodynamics and multiple sediment transport situations. This hampers the definition of regulatory thresholds for REE concentrations and emissions. The review summarizes the existing information on REE geochemistry and physicochemical conditions influencing dissolution, surface complexation reactions, and distribution at the continent–ocean interface, as well as their speciation, bioavailability, and detrimental effects on living organisms. This review is a follow-up to a previous review published in Journal of Marine Science and Engineeringon the issues of accumulation, transport, and the effects of microplastics (MPs) in the oceans. The review brings together experimental laboratory, mathematical, and field data on the dual role of MPs as accumulators of hydrophobic persistent organic compounds (POPs), and their release-effect in the marine ecosystem. It also examines the carrier role, besides POPs, of new emerging categories of pollutants, such as pharmaceuticals and personal care products (PPCPs). This role becomes increasingly important and significant as polymers age and surfaces become hydrophilic, increasing toxicity and effects of the new polymer-pollutant associations on marine food webs. It was not the intention to provide too many detailed examples of carriers and co-contaminants, exposed marine species, and effects. Instead, the views of two different schools of thought are reported and summarized: one that emphasizes the risks of transport, exposure, and risk beyond critical thresholds, and another that downplays this view. One of the major concerns regarding the presence of plastics in ocean environments are the effects on marine biota. Plastics can be distinguished in macro- (≥25 mm), meso- (<25 mm–5 mm) micro- (<5 mm–1 µm), and nano-plastic (<1 µm) and are practically omnipresent in aquatic habitats and subject to long-range transport. The purpose of this review is to report the last findings on the release, transfer, accumulation, and effects of micro-plastics, MPs, in the oceans. MPs have the chance to adsorb different kind of organisms and compounds on their outer surface, including bacteria, viruses, algae, and abiotic substances. In this way, they can cause sever hazard once they enter the food chain. Their harm to higher organisms is discussed as well as main routes of MPs–organism interactions, i.e., ventilation, and ingestion. Potential effects on populations, communities, and ecosystems and uptake routes and transition into tissues are discussed. In consideration of the potential threats of plastic particles to ecological functions and human health risks, we recommend specific directions of future research approaches. [ABSTRACT FROM AUTHOR]

A subtalar dislocation of the foot is an uncommon injury, and cases of posterior subtalar dislocation are even rarer. This injury is characterized by a simultaneous dislocation of talocalcaneal and talonavicular joints while tibiotalar and calcaneocuboid articulations remain intact. Subtalar dislocation is commonly accompanied by fractures of the malleoli, talus, or fifth metatarsal and by a rotational component of the subtalar joint. Subtalar dislocation can occur in any direction and produces significant deformity. This article presents a case of pure posterior subtalar dislocation in an 80-year-old man who presented with pain and deformity in his left ankle following a motorcycle accident. Radiographs demonstrated dislocation of both the talonavicular and talocalcaneal joints. The calcaneus was displaced posteriorly without medial or lateral displacement. No rotational component of the calcaneus was noted, suggesting the trauma was in pure hyperplantar flexion. We hypothesize that pure hyperplantar flexion could lead to a progressive subtalar ligament weakening that may result in a complete ligament rupture if the plantar flexion force is prolonged. This could be observed in the presence of good bone quality and if the force is applied distally at the navicular bone. [ABSTRACT FROM AUTHOR]

Marine litter has been considered one of the most serious global challenges, requiring urgent action by governmental bodies, especially in African Small Island Developing States (SIDS), where resources and research are limited. In addition to this, waste management and environmental education and ocean literacy programs in schools are scarce, with islands suffering more seriously from these problems. Despite the amount of literature regarding causes and impacts of marine litter, there is still not enough research conducted concerning the public perceptions on both the problem and the potential solutions. This is even more noticeable in African developing countries, where resources and research are scarce. Perception plays a key role for ecosystem management and conservation policies. This study intends to explore the perceptions of local island fishing communities in Cape Verde regarding marine litter, in order to contribute for an improvement of marine ecosystem management and development of conservation policies. To achieve that, two participatory sessions were conducted in two communities in the island of Santiago—Porto Mosquito and Porto Gouveia—where brainstorming and active listening were used to create shared and authentic spaces for dialogue between the members of the community. Results show that the population of both communities were very aware of the marine litter problem. They were able to identify the lack of a proper waste management system in the island and the inappropriate behaviours of the population as the main causes of this problem. Equipment damages and the presence of plastic inside the fish were the most relevant impacts identified by the participants. These findings reinforce previous research on the importance of public engagement and environmental education to contribute to the conservation of marine ecosystems and to build a strong collaborative ocean governance. Microplastic (MP) debris is recognized to be one of the most serious threats to marine environments. They are found in all seas and oceanic basins worldwide, even in the most remote areas. This is further proof that the transport of MPs is very efficient. In the present study, we focus our attention on MPs' transport owing to the Stokes drift generated by sea waves. Recent studies have shown that the interaction between heavy particles and Stokes drift leads to unexpected phenomena mostly related to inertial effects. We perform a series of laboratory experiments with the aim to directly measure MPs' trajectories under different wave conditions. The main objective is to quantify the inertial effect and, ultimately, suggest a new analytical formulation for the net settling velocity. Archeological sites close to coastal volcanic-sedimentary aquifers are threatened by groundwater contaminated by natural and anthropogenic processes. The paper reports on a hydrogeological, chemical (major, minor and trace elements) and isotopic (δD-H2O, δ18O-H2O, δ15N-NO3, δ18O-NO3, δ11B, 222Rn) survey of groundwater at the Cumae archaeological site, which is located in the coastal north-western sector of the volcanic district of Phlegraean Fields (southern Italy), where groundwater flooding phenomena occur. Results show the presence of a complex coastal volcanic-sedimentary aquifer system where groundwater quality is influenced mainly by: (i) aquifer lithology and localized ascent of magmatic fluids along buried volcano-tectonic discontinuities, (ii) mixing of groundwater, deep mineralized fluids and seawater during groundwater pumping, and (iii) nitrate contamination >50 mg/L from non-point agricultural sources. Moreover, δD and δ18O point toward fast recharge from seasonal precipitations, while the isotopic ratios of N and O in nitrate reveal the contribution of mineral and organic fertilizers as well as leakage from septic tanks. Results can assist the local archaeological authority for the safeguarding and management of the archaeological heritage of the Cumae site. [ABSTRACT FROM AUTHOR]

A multidisciplinary survey was carried out on the quality of water and sediments of the estuary of the Sele river, an important tributary of the Tyrrhenian Sea, to assess anthropogenic pressures and natural variability. Nine sediment sites were monitored and analyzed for granulometry, morphoscopy, benthic foraminifera and ostracod assemblages, heavy metals, and polycyclic aromatic hydrocarbons. Surface water was assayed for ionic composition and phytoplankton biomass. Total organic carbon (TOC) and total nitrogen (TN) in sediments were higher in the inner part of the estuary (IE), up to 12.7 and 0.7% because of anthropic influence. In waters, N-NH4, N-NO3, and Ptot. were high, with loads of Ptot in IE exceeding ~fourfold the limit. Here, it was also observed that the highest primary production was Chl-a, 95.70 µg/L, with cryptophytes, 37.6%, and diatoms, 33.8%, being the main phytoplanktonic groups. The hierarchical analysis split the estuary into two areas, with marked differences in anthropic pollution. Waters were classified as poor–bad level with respect to the content of nutrients. Sedimentological assay reveals littoral erosion and poor supply of river sandy sediments. The erosion environment is confirmed by the presence of meiobenthic recent marine forms intrusion inside the river. The capacity for ascidians to inhabit coastal sea floor worldwide relies on their peculiar tolerance to environmental variables and pollution, which is considered, together with high levels of genetic diversity, among the main drivers of their invasive potential. In spite of the continued interest in the genetics of invasive species, little attention has been paid toward the microevolutionary processes that drive structure and fate of ascidian populations over time under chemically polluted conditions. Understanding the interplay between environmental and population dynamics is critical to predict the biodiversity of marine coastal ecosystems. In the present study, a local population of the ascidian Ciona robusta living in the Fusaro Lagoon has been monitored over a 13‐month period of sampling. Physico‐chemical parameters (temperature, salinity, turbidity, dissolved oxygen, heavy metals), genetic composition (microsatellites, ITS‐2), abundance and biomass (wet and dry weight) were assessed with the aim to infer fine‐scale temporal variation of population structure with respect to rapid environmental change. Analysis of biomass showed that C. robusta is highly sensitive to salinity and oxygen concentrations. Further, genetic analysis suggested a highly dynamic population structure, likely due to the strong clustering of temporal samples and distinct responses to environmental conditions, including bioaccumulation of heavy metals. Here, we hypothesize that rapid variation in allele frequencies of neutral markers in C. robusta populations may increase the ability of the species to colonize habitats that are subject to strong variation and are under heavy human pressure.
