This study developed a VR-based mobile learning system to simulate the natural environment of a geopark for university students to learn geological knowledge. This learning method used a Google cardboard equipped with smartphones, where students can watch stereoscopic scenes and experience virtual environments as if visiting the geopark. In order to evaluate the learning effectiveness of the proposed system, we collected the learning outcomes, learning motivation, and cognitive load of students before and after the learning activity. The results indicated that students had significantly better learning achievement in their comprehension ability and learning motivation after using the proposed system. Therefore, it can be inferred that the VR-based mobile learning system not only helps students restructure knowledge and improves their comprehension skills but also enhances their learning motivation tendencies. In addition, the results showed that the students did not have high intrinsic and extraneous cognitive loads but had a high germane cognitive load. This retrospective study investigated the incidence, medication use, and outcomes in pediatric autosomal-dominant polycystic kidney disease (ADPKD) using Taiwan's National Health Insurance Research Database (NHIRD). A 1:4 matched control group of individuals included in the NHIRD during the same period was used for comparative analyses. A total of 621 pediatric patients were identified from 2009 to 2019 (mean age, 9.51 ± 6.43 years), and ADPKD incidence ranged from 2.32 to 4.45 per 100,000 individuals (cumulative incidence, 1.26–1.57%). The incidence of newly developed hypertension, anti-hypertensive agent use, nephrolithiasis, and proteinuria were significantly higher in the ADPKD group than the non-ADPKD group (0.7 vs. 0.04, 2.26 vs. 0.30, 0.4 vs. 0.02, and 0.73 vs. 0.05 per 100 person-years, respectively). The adjusted hazard ratios for developing hypertension, proteinuria, nephrolithiasis and anti-hypertensive agent use in cases of newly-diagnosed pediatric ADPKD were 12.36 (95% CI 4.92–31.0), 13.49 (95% CI 5.23–34.79), 13.17 (95% CI 2.48–69.98), and 6.38 (95% CI 4.12–9.89), respectively. The incidence of congenital cardiac defects, hematuria, urinary tract infections, gastrointestinal diverticulosis, dyslipidemia, and hyperuricemia were also higher in the ADPKD group. Using digital learning content to realize learning in games is a rapidly-developing direction of interest for teachers and researchers. This study has developed a digital role-playing gaming system to review Social Studies course content to a fifth Grade class at an elementary school. It allows students to experience the historical storyline of Tamsui, located in the north of Taiwan, by playing the role of Dr. Mackay. An experiment was conducted to evaluate the potential effects of the proposed system when applied to the reviewing activity of Humanities courses. The results of the post-test scores showed that the proposed system is not superior to the traditional teaching method which uses PowerPoint-based slides with the teacher's oral instruction, but it still enhances their overall learning performance after the reviewing activity. Moreover, the results of the questionnaire survey showed that the students who reviewed by using the proposed system showed a higher learning motivation than those reviewing with the traditional teaching method. It is therefore concluded that our role-playing digital gaming system can be used to assist the teachers' teaching activities, because it allows the students to complete their reviewing activity by themselves, and it effectively enhances their learning performances and motivation. The article suggests that businesses have overlooked older adults, widening digital inequality, but there is an opportunity for companies to develop accessible applications and products tailored to their needs. Most of the MIP technology are used as sensors, especially for acetaminophen detection. However, there is a very limited study using MIP as acetaminophen separation. Thus, the current study elucidates the use of MIP for acetaminophen uptake in aqueous media. Application of MIP can reduce fouling. Acetaminophen, methacrylic acid (MAA), ethylene glycol dimethacrylate (EDGMA), 1,1'-Azobis(cyclohexanecarbonitrile) (ABCN) were used as template, functional monomer, cross-linker and initiator respectively. The molarity of cross-linker and functional monomer were varied for the study of imprinting effect of MIPs. High concentration of cross-linker exhibits poor binding ability of MIPs while high molarity of monomer demonstrated better performance in binding capacity. The optimum MIP was observed from template:monomer:cross-linker molar ratio at 1:58:15 with binding capacity of 5.2 mg/g polymer. Next, pristine PES and molecularly imprinted membrane (MIM) were fabricated using phase inversion method. MIM was prepared by adding optimum MIPs in casting solution for the study of antifouling properties as compared to pure membrane. The relative flux of MIM has showed a poor antifouling behaviour in real wastewater sample while demonstrating a good performance in synthetic solution. However, MIM has revealed a better rejection of acetaminophen in synthetic solution and aquaculture wastewater as compared to pristine membrane at 50.2 and 73.06% respectively. In this study, a head-mounted camera was used to track eye behaviors and estimate the gaze point on the user's visual plane. The integration of the elastic mechanism design makes the headset adaptable for various users. The wearable cases were prototyped with low-cost cameras to produce an efficient eye tracking solution. This proposed system can effectively extract and estimate pupil ellipse from a few camera images of an eye and compute the corresponding three-dimensional eye model. The system can match later images of the same pupil ellipse from a head-mounted camera to give the possible visual angles. To estimate the gaze point, the system uses multiple-point calibration to solve the related polynomial formula for future angle-to-gaze mapping. The proposed eye-tracking algorithms can provide a low-complexity solution with high accuracy, precision, and speed. This tracking system is a low-cost and promising system that can be used in headsets for virtual reality, auxiliary equipment, interactive machine, and human–machine interface applications. The proposed eye-tracking algorithm can achieve satisfactory performance without using a high-end high-speed camera and can be detected under different lighting sources, and the average errors of the detection results are stably within 9 pixels and at a distance of 50 cm from the screen; while the average error of the fixation mapping results is within 3°. The Singapore Health Services cluster (SingHealth) radiology film archives are a valuable repository of local radiological cases dating back to the 1950s. Some of the cases in the archives are of historical medical interest, i.e. cerebral angiography in the workup of patients with hemiplegia. Other cases are of historical social interest, being conditions seen during earlier stages of Singapore's development, i.e. bound feet. The archives form a unique portal into the development of local radiology as well as the national development of Singapore. A selection from the archives is published in 2020 in commemoration of the 20th anniversary of the formation of SingHealth, the 55th National Day of Singapore, and the 125th anniversary of the International Day of Radiology. In most cultural heritage courses, students physically visit several renowned heritage sites for educational purposes. However, because of time and manpower limitations, many teachers use traditional outdoor instruction methods to transmit vital information regarding these sites and buildings. This approach could result in students merely memorizing knowledge for test-taking purposes, rather than engaging in active thinking. Therefore, this study proposed using a ubiquitous guide-learning system to enhance students’ performance and active participation in cultural heritage courses. To evaluate the effectiveness of the proposed system, a trial experiment was conducted to visit eight heritage sites featured in the cultural heritage course offered by a local university. In total, 62 students were randomly assigned to either an experimental group testing the proposed system or a control group using traditional outdoor instruction methods. The experimental results indicated that our ubiquitous guide-learning system significantly improved students’ learning achievements. The outcome of the Instructional Materials Motivation Survey questionnaire indicated that the experimental group participants were motivated to learn and interested in using the proposed system. Moreover, the two groups had statistically significant results for the attention, confidence, and satisfaction factors. Therefore, implementing our proposed system in conjunction with cultural heritage education courses may yield a significant learning advantage for students by improving overall learning performance and motivation. Although Augmented Reality (AR) technology has already been adopted into mobile learning environments, additional effort must be put towards providing strong evidence that AR-based mobile systems are excellent educational tools that make a positive impact in or outside of the classroom. Our study utilized a similar AR-based mobile learning system developed to authentically teach a liberal arts course at a Taiwanese university. Under controlled study environments, we were able to design a system that could impart relevant concepts from the course to students outside of the traditional classroom setting. Experimental results demonstrated that students with the opportunity to learn through the proposed system demonstrated higher learning motivation, had better learning performance and comprehension abilities than those studying via the conventional outdoor instruction approach. Moreover, the questionnaire survey conducted after our study revealed that "confidence" was the most highly rated motivational factor among students who used the proposed system, suggesting that students were quite motivated to learn using the novel system supplemented with technological advances because they felt more sure of their knowledge and performance. Thus, we believe that using the proposed system can effectively improve the learning outcomes of students enrolled in the liberal arts course, with the particular benefit of boosting confidence in gaining new knowledge, which leads to an improvement of overall learning performance and motivation. Aim: We hypothesise that clinically well late-preterm infants (LPI) (34+0 -36+6 weeks) are neurologically more immature than their term counterparts, and this immaturity persists even when these infants reach term-corrected age (TCA). The primary aim of our study was to characterise and contrast the neurodevelopmental profile of well LPI with full-term infants (FTI) (39+0 -41+6 weeks) using the Hammersmith Neonatal Neurological Examination (HNNE). Our secondary aim was to obtain local reference ranges for the 34 items in the HNNE in an Asian-dominant population.Methods: LPI were assessed at two time points: 12-72 h of life and at TCA of 39+0 -41+6 weeks, while FTI were assessed at 12-72 h of life using the HNNE. Each of the 34 items on the HNNE was assigned an optimality score (OS) of 0, 0.5 or 1, totalling up to 34. A quantitative comparison of the neurobehavioral patterns was made using two-sample t-tests.Results: A total of 212 infants (79 LPI and 133 FTI) were recruited. Mean OSs for LPI and FTI at birth were (25.11 ± 3.36)/34 and (31.19 ± 1.50)/34, respectively, with a mean difference of 6.08 (P value <0.0001). The mean OS for LPI on reaching TCA was (28.91 ± 2.30)/34, with a mean difference of 2.28 (P value <0.0001). Reference OSs for the 34 items on the HNNE were also obtained.Conclusion: LPI are more immature than their term counterparts even on reaching TCA, with discrepancies most apparent in 'tone' and 'movement'. This paper proposes a novel interactive ubiquitous learning system (IULS) for authentically teaching a cultural heritage course and imparting relevant concepts to students. Experimental results demonstrated that learning performance was significantly improved after students used the IULS. This study also demonstrated that students using the IULS achieved superior results to those achieved using conventional teaching models. No gender bias affecting the performance of students in the experimental group was observed because each student consistently performed to a higher standard on the learning tasks compared with students in the control group, particularly in learning comprehension-related content. The IULS was more effective in enhancing the overall learning performance of students exhibiting different cognitive styles in liberal arts education courses. Furthermore, the results of a survey on Instructional Materials Motivation Survey revealed that Relevance was the most highly rated motivational factor among students who used the IULS, suggesting that students were motivated to use the IULS. Thus, we believe that a liberal arts education supplemented with the IULS yields a significant learning advantage for students by improving overall learning performance and motivation. Statins mediate vascular protection and reduce the prevalence of cardiovascular diseases. Recent work indicates that statins have anticonvulsive effects in the brain; however, little is known about the precise mechanism for its protective effect in kainic acid (KA)-induced seizures. Here, we investigated the protective effects of atorvastatin pretreatment on KA-induced neuroinflammation and hippocampal cell death. Mice were treated via intragastric administration of atorvastatin for 7 days, injected with KA, and then sacrificed after 24 h. We observed that atorvastatin pretreatment reduced KA-induced seizure activity, hippocampal cell death, and neuroinflammation. Atorvastatin pretreatment also inhibited KA-induced lipocalin-2 expression in the hippocampus and attenuated KA-induced hippocampal cyclooxygenase-2 expression and glial activation. Moreover, AKT phosphorylation in KA-treated hippocampus was inhibited by atorvastatin pretreatment. These findings suggest that atorvastatin pretreatment may protect hippocampal neurons during seizures by controlling lipocalin-2-associated neuroinflammation. [ABSTRACT FROM AUTHOR]

Objectives: Febrile infants younger than 3 months old present a diagnostic dilemma to the emergency physician. We aim to describe a large population of febrile infants less than 3 months old presenting to a pediatric emergency department (ED) and to assess the performance of current heart rate guidelines in the prediction of serious infections (SI). Materials and methods: We performed a retrospective review of febrile infants younger than 3 months old, between March 2015 and Feb 2016, in a large tertiary pediatric ED. We documented the primary outcome of SI for each infant, as well as the clinical findings, vital signs, and Severity Index Score (SIS). We assessed the performance of the Paediatric Canadian Triage and Acuity Scale (PaedCTAS), Advanced Pediatric Life Support (APLS) guidelines and Fleming normal reference values, using sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and area under receiver operating characteristics curve (AUC). Results: 1057 infants were analyzed, with 326 (30.6%) infants diagnosed with SI. High temperature, tachycardia, and low SIS score were significantly associated with SI. Item analysis showed that the SIS performance was driven by the presence of mottling (p = 0.003) and high temperature (p<0.001). The APLS guideline had the highest sensitivity (66.0%, 95% CI 60.5–71.1%), NPV (73.3%, 95% CI 69.7–76.5%) and AUC (0.538), while the PaedCTAS (2 standard deviation from normal) had the highest specificity (98.5%, 95% CI 97.3–99.3%) and PPV (55.2%, 95% CI 32.7–71.0%). Conclusions: Current guidelines on infantile heart rates have a variable performance. In our study, the APLS heart rate guidelines performed with the highest sensitivity, but no individual guideline predicted for SIs satisfactorily. [ABSTRACT FROM AUTHOR]

Cilostazol is a selective inhibitor of type 3 phosphodiesterase (PDE3) and has been widely used as an antiplatelet agent. Cilostazol mediates this activity through effects on the cyclic adenosine monophosphate (cAMP) signaling cascade. Recently, it has attracted attention as a neuroprotective agent. However, little is known about cilostazol's effect on excitotoxicity induced neuronal cell death. Therefore, this study evaluated the neuroprotective effect of cilostazol treatment against hippocampal neuronal damage in a mouse model of kainic acid (KA)-induced neuronal loss. Cilostazol pretreatment reduced KA-induced seizure scores and hippocampal neuron death. In addition, cilostazol pretreatment increased cAMP response element-binding protein (CREB) phosphorylation and decreased neuroinflammation. OBJECTIVE This paper aims to explain societal differences in the event of leaving the parental home as part of the transition to adulthood, in the United States, Germany, China, and Taiwan. It proposes bridge hypotheses between societal characteristics such as kinship system and welfare regime and home-leaving behavior, and tests them with nationally representative panel studies. METHODS Four panel studies (NLSY97 for the USA; PAIRFAM for Germany; CFPS for China; TYP for Taiwan) were harmonized for similar cohorts, with an age span of 15 to 30 years. Testing was based on age-specific tabulations of household composition and separate discrete-time event history models. The inherent compliance of soft fluidic actuators makes them attractive for use in wearable devices and soft robotics. Their flexible nature permits them to be used without traditional rotational or prismatic joints. Without these joints, however, measuring the motion of the actuators is challenging. Actuator-level sensors could improve the performance of continuum robots and robots with compliant or multi-degree-of-freedom joints. We make the reinforcing braid of a pneumatic artificial muscle (PAM or McKibben muscle) “smart” by weaving it from conductive insulated wires. These wires form a solenoid-like circuit with an inductance that more than doubles over the PAM contraction. The reinforcing and sensing fibers can be used to measure the contraction of a PAM actuator with a simple linear function of the measured inductance, whereas other proposed self-sensing techniques rely on the addition of special elastomers or transducers, the technique presented in this paper can be implemented without modifications of this kind. We present and experimentally validate two models for Smart Braid sensors based on the long solenoid approximation and the Neumann formula, respectively. We test a McKibben muscle made from a Smart Braid in quasi-static conditions with various end loads and in dynamic conditions. We also test the performance of the Smart Braid sensor alongside steel. The addition of animated pedagogical agents (APAs) in computer-assisted learning (CAL) systems could successfully enhance students' learning motivation and engagement in learning activities. Conventionally, the APA incorporated multimedia materials are constructed through the cooperation of teachers and software programmers. However, the thinking and technical gap between two roles, a concept/content provider (teacher) and an implementer (software programmer), may lead to the concept distortion and loss in the final teaching material implementations. This often results in preventing a teacher from preparing his or her own APA-based multimedia learning systems. This study aims to develop a courseware to facilitate the CAL through the combination of the two roles into a single one and then teachers can use it freely by themselves. The courseware can help teachers to develop appropriate multimedia instructional materials incorporating APAs, which attracts students' attention and enhances their interest in learning. Furthermore, in this study two separate trial experiments were conducted to measure the teachers' satisfaction with the courseware and students' learning motivation after using APA-based multimedia learning systems. The results show that the participating teachers were satisfied with the courseware and would like to apply it in their instruction. Moreover, the participating students were strongly motivated by the customized APA-based multimedia learning systems. Therefore, the proposed courseware could give teachers a good opportunity to add pedagogical value to the curriculum, so that students' learning interest and motivation could be enhanced. Background: Obesity has deleterious effects on the brain, and metabolic dysfunction may exacerbate the outcomes of seizures and brain injuries. However, it is unclear whether obesity affects excitotoxicity-induced neuronal cell death. The purpose of this study was to investigate the effects of a high-fat diet (HFD) on neuroinflammation and oxidative stress in the hippocampus of kainic acid (KA)-treated mice.Results: Mice were fed with a HFD or normal diet for 8 weeks and then received a systemic injection of KA. HFD-fed mice showed hypercholesterolemia, insulin resistance, and hepatic steatosis. HFD-fed mice showed greater susceptibility to KA-induced seizures, an increased number of terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL)-positive cells, neuroinflammation, and oxidative stress. Furthermore, we found that KA treatment increased HFD-induced calpain1, nuclear factor E2-related factor 2, and heme oxygenase-1 expression in the hippocampus.Conclusions: These findings imply that complex mechanisms affected by obesity-induced systemic inflammation, neuroinflammation, ER stress, calcium overload, and oxidative stress may contribute to neuronal death after brain injury. Sirtuin 1 (SIRT1) is a mammalian NAD+-dependent protein deacetylase that regulates cellular metabolism and inflammatory response. The organ-specific deletion of SIRT1 induces local inflammation and insulin resistance in dietary and genetic obesity. Macrophage-mediated inflammation contributes to insulin resistance and metabolic syndrome, however, the macrophage-specific SIRT1 function in the context of obesity is largely unknown. C57/BL6 wild type (WT) or myeloid-specific SIRT1 knockout (KO) mice were fed a high-fat diet (HFD) or normal diet (ND) for 12 weeks. Metabolic parameters and markers of hepatic steatosis and inflammation in liver were compared in WT and KO mice. SIRT1 deletion enhanced HFD-induced changes on body and liver weight gain, and increased glucose and insulin resistance. In liver, SIRT1 deletion increased the acetylation, and enhanced HFD-induced nuclear translocation of nuclear factor kappa B (NF-κB), hepatic inflammation and macrophage infiltration. HFD-fed KO mice showed severe hepatic steatosis by activating lipogenic pathway through sterol regulatory element-binding protein 1 (SREBP-1), and hepatic fibrogenesis, as indicated by induction of connective tissue growth factor (CTGF), alpha-smooth muscle actin (α-SMA), and collagen secretion. Myeloid-specific deletion of SIRT1 stimulates obesity-induced inflammation and increases the risk of hepatic fibrosis. Educational robotics has been regarded as an effective instructional tool over the past decade. Many studies have tested the role of robots in supporting educational classroom activities. However, reliable empirical evidence confirming the effectiveness of educational robots remains limited. Therefore, this study developed an educational robot-based learning system that provides an attractive teaching application that combines multimedia objects with an educational robot. This study examined the impact of the proposed learning system on student learning performance and motivation. The four-stage experimental procedure involved a pre-test, post-test, learning activities, and questionnaire survey. The participants comprised one homeroom teacher and 52 second-grade students from two classes at an elementary school in Taiwan. All of the students were assigned to use either the proposed learning system or a PowerPoint-based learning system. The pre-test and post-test results show that the proposed learning system improved student performance more than the PowerPoint-based learning system did. A questionnaire based on the Instructional Materials Motivation Survey (IMMS) was employed to measure four motivational factors (attention, relevance, confidence, and satisfaction). The IMMS survey results show that satisfaction and relevance were the highest rated motivational factors among students who used the proposed learning system. The experimental results indicate that the students were motivated to use the educational robot-based learning system. Exendin-4 (Ex-4), a glucagon-like peptide-1 receptor (GLP-1R) agonist, has been known to reverse hepatic steatosis in ob/ob mice. Although many studies have evaluated molecular targets of Ex-4, its mechanism of action on hepatic steatosis and fibrosis has not fully been determined. In the liver, glucose transporter 4 (GLUT4) is mainly expressed in hepatocytes, endothelial cells and hepatic stellate cells (HSCs). In the present study, the effects of Ex-4 on GLUT4 expression were determined in the liver of ob/ob mice. Ob/ob mice were treated with Ex-4 for 10 weeks. Serum metabolic parameters, hepatic triglyceride levels, and liver tissues were evaluated for hepatic steatosis. The weights of the whole body and liver in ob/ob mice were reduced by long-term Ex-4 treatment. Serum metabolic parameters, hepatic steatosis, and hepatic fibrosis in ob/ob mice were reduced by Ex-4. Particularly, Ex-4 improved hepatic steatosis by enhancing GLUT4 via GLP-1R activation in ob/ob mice. Ex-4 treatment also inhibited hepatic fibrosis by decreasing expression of connective tissue growth factor in HSCs of ob/ob mice. The article discusses a study which compared the morphologic features of persistent pure glass opacity nodules (GGN) of ≥10 millimeters (mm) in diameter at thin-section compute tomography (TSCT) scan with histopathology and patient prognosis. It concluded that in persistent pure GGN with this diameter, the size and mass of the nodule are determinants of invasive adenocarcinoma, for which surgical resection leads to excellent prognosis.

A beneficial radioprotective agent has been used to treat the radiation-induced lung injury. This study was performed to investigate whether curcumin, which is known to have anti-inflammatory and antioxidant properties, could ameliorate radiation-induced pulmonary inflammation and fibrosis in irradiated lungs. Rats were given daily doses of intragastric curcumin (200 mg/kg) prior to a single irradiation and for 8 weeks after radiation. Histopathologic findings demonstrated that macrophage accumulation, interstitial edema, alveolar septal thickness, perivascular fibrosis, and collapse in radiation-treated lungs were inhibited by curcumin administration. Radiation-induced transforming growth factor-β1 (TGF-β1), connective tissue growth factor (CTGF) expression, and collagen accumulation were also inhibited by curcumin. Moreover, western blot analysis revealed that curcumin lowered radiation-induced increases of tumor necrosis factor-α (TNF-α ), TNF receptor 1 (TNFR1), and cyclooxygenase-2 (COX-2). Curcumin also inhibited the nuclear translocation of nuclear factor-κ B (NF-κB) p65 in radiation-treated lungs. Background: Hyperglycemia leads to cardiac oxidative stress and an imbalance in glucose homeostasis. Diabetic cardiomyopathy is characterised by cardiac hypertrophy and fibrosis. However, the underlying mechanisms of diabetic cardiomyopathy are not fully understood. This study aimed to investigate the effects of alpha-lipoic acid (ALA) on cardiac energy metabolism, antioxidant effect, and fibrosis in the hearts of Otsuka Long-Evans Tokushima fatty (OLETF) rats. Methods: Animals were separated into non-diabetic Long-Evans Tokushima Otsuka (LETO) rats and diabetes-prone OLETF rats with or without ALA (200 mg/kg/day) administration for 16 weeks. Diabetic cardiomyopathy was assessed by staining with Sirius Red. The effect of ALA on AMPK signalling, antioxidant enzymes, and fibrosis-related genes in the heart of OLETF rats were performed by Western blot analysis or immunohistochemistry. Results: Western blot analysis showed that cardiac adenosine monophosphate-activated kinase (AMPK) signalling was lower in OLETF rats than in LETO rats, and that ALA treatment increased the signalling in OLETF rats. Furthermore, the low antioxidant activity in OLETF rats was increased by ALA treatment. In addition to increased Sirius red staining of collagen deposits, transforming growth factor-β1 (TGF-β1) and connective tissue growth factor (CTGF) were expressed at higher levels in OLETF rat hearts than in LETO rat hearts, and the levels of these factors were decreased by ALA. Conclusions: ALA enhances AMPK signalling, antioxidant, and antifibrogenic effect. Theses findings suggest that ALA may have beneficial effects in the treatment of diabetic cardiomyopathy. Background: Cigarette/alcohol use and premarital sex, and their subsequent consequences on the well-being of college students, are international health promotion issues. However, little is known about the temporal relationship of these risk behaviors among Taiwanese college students. Methods: This study utilizes data from the Taiwan Youth Project, a cohort sample of 20-year-olds (N = 2,119) with a 2-year follow-up, to explore the relationship between adolescent cigarette/alcohol use, and subsequent premarital sex. To incorporate the Taiwanese context where the normative value of abstinence until marriage remains strong, multivariate logistic regression models included data on premarital sex attitudes, stressful life events, peer influence, as well as family and individual factors which might influence this relationship. Results: The sample consists of 49% male and 51% female college students. About 16% of the sample report having had premarital sex by age 20. After excluding sexually active youth, 20% of males and 13% of females report engaging in premarital sex in the 2-year follow-up interview. Multivariate logistic regression analyses reveal adolescent alcohol use is significantly associated with a higher likelihood of engaging in premarital sex for both genders; adolescent smoking is significantly associated with premarital sexual activity among males, but not females. Our results indicate liberal premarital sexual attitudes and stressful personal events are also significantly associated with premarital sexual activity. Conclusions: These findings suggest health promotion programs for college students need to take developmental and gender perspectives into account. The article discusses a study of the effect of protective ventilatory settings on lung impairment during one-lung ventilation (OLV) compared with conventional ventilation in patients undergoing lung resection surgery in the U.S. in 2009. The protective ventilation strategy was found to be beneficial in terms of fewer pulmonary complications. Results show that lung injury is associated with high ventilating pressures. Also noted is the link between exposure to 100% oxygen and absorption atelectasis.

Purpose: The purpose of this study was to describe provider characteristics African American pregnant women identified as important when interacting with their prenatal care providers in an outpatient office setting. Study Design and Method: A descriptive qualitative design was used to explore provider characteristics desired by African American women receiving prenatal care at two inner-city hospital—based obstetric clinics. A total of 22 African American women between the ages of 19 and 28 years participated in the study. Findings: Four major provider characteristic themes emerged from the data: (a) demonstrating quality patient—provider communication, (b) providing continuity of care, (c) treating the women with respect, and (d) delivering compassionate care. Discussion and Conclusion: An overarching theme revealed by the data analysis was the desire by African American women in this study to have their prenatal providers know and remember them. They wanted their providers to understand the context of their lives from their prenatal interactions. Incorporating findings from this study to improve patient—provider interactions during prenatal care could provide an increased understanding of the many complex variables affecting African American women’s lives. Implications for Practice and Research: Prenatal care provides an opportunity for African American women to develop a trusting relationship with a provider. Developing models of prenatal care congruent with the realities of African American women’s lives has the potential to improve patient— provider interactions and potentially affect birth outcomes. Chronic airway inflammation is a characteristic feature of destructive cigarette smoking (CS)-induced lung disease, particularly in patients with emphysema. Celecoxib, a specific cyclooxygenase-2 (COX-2) inhibitor, is widely used to treat inflammation. However, the exact mechanisms underlying this drug's anti-inflammatory effects have not yet been determined in pulmonary emphysema. Here, we explore whether celecoxib attenuates CS-induced inflammation in rat lungs. Rats were exposed to smoke and received celecoxib via intragastric feeding daily for 20 wk. We found that celecoxib inhibited interalveolar wall distance and pulmonary inflammation in the lungs of CS-treated rats. Celecoxib inhibited serum NO production, iNOS, COX-2 expression, and PGE2 production in CS-treated lung tissues. Our immunohistochemical data showed that CS-induced CD68 and COX-2 expression were inhibited by celecoxib. Furthermore, celecoxib attenuated the activation of phospho-IκBα and NF-κB in CS-treated rat lung. In addition, there was an inhibitory effect of celecoxib on the COX-2 expression and NF-κB activation in LPS-stimulated RAW 264.7 macrophages. Celecoxib also attenuated NF-κB activation in COX-2 siRNA-transfected RAW 264.7 macrophages. Thus, our findings suggest that the antiinflammatory effects of celecoxib are mediated by its effects on NF-κB-regulated gene expression, which ultimately reduces the progression of CS-induced pulmonary emphysema. Integrated 18fluorine fluorodeoxyglucose (18F‐FDG) positron emission tomography/computed tomography (PET/CT) has shown somewhat variable sensitivity and specificity for mediastinal nodal staging in granulomatous disease endemic areas. The purpose of the study was to prospectively evaluate the efficacy of PET/CT for mediastinal nodal staging in nonsmall cell lung cancer (NSCLC) patients in a tuberculosis‐endemic country.Prospective assessment of the diagnostic efficacy of integrated PET/CT for detecting mediastinal nodal metastasis was performed in 674 patients (M:F ratio = 502:172; mean age, 61 years) with NSCLC. Patients underwent an integrated PET/CT examination and subsequent surgical nodal staging (by mediastinoscopy only in 121 patients and by thoracotomy in 553). Nodes showing greater 18F‐FDG uptake than mediastinum at PET without benign calcification or high attenuation >70 household unit (HU) at unenhanced CT were regarded as being positive for malignancy. The histologic nodal assessment results were used as reference standards.Of 2477 mediastinal nodal stations evaluated in 674 patients, 275 (11%) stations in 180 (27%) patients proved to be malignant. On a per‐person basis, the overall sensitivity, specificity, and accuracy of PET/CT for mediastinal nodal staging were 61% (110 of 180), 96% (473 of 494), and 86% (583 of 674), respectively. On a per‐nodal station basis, they were 46% (126 of 275), 98% (2154 of 2202), and 92% (2280 of 2477).Integrated PET/CT provides high specificity and reasonably high accuracy, but somewhat low sensitivity for mediastinal nodal staging of NSCLCs. The high specificity is achieved at the expense of sensitivity by interpreting calcified nodes or nodes with high attenuation at CT, even with high FDG uptake at PET, as benign in a tuberculosis‐endemic region. This study uses Burt's theory of structural holes and Lin, Fu and Hsung's theory of position-generated networks to examine a hypothesis about the overlap of couples' social networks and the degree of joint behaviour in conjugal role relationships. The authors devised five overlap types of couples' networks based on two-dimensional network characteristics: the diversity of ego's kin ties and the degree of cross-linkages through spouse's contacts. They measured the conjugal role by how couples manage their day-to-day expenditure and classified them into three categories: mainly managed by wife, mainly managed by husband and joint management. This study used the 2001 Taiwan Social Change Survey to test the hypothesis on the couples' networks and conjugal roles. After controlling for all other variables, the overlap types of the couples' social networks still had significant effects on their management of family expenditure. Specifically, the couples with high diversity of ego's kin ties and high cross-linkages through spouse practised the joint conjugal role pattern of family expenditure. The reason is that the high diversity of ego's kin ties provides plural patterns of conjugal roles to be identified and the high cross-linkages through the spouse provide the bargaining power from the spouse. Using corresponding couple data from Taiwan, Shanghai and Hong Kong, the main goal of this article is (1) to explore the conjugal consistency or disparity in marital values and in perceptions of marital relations among three Chinese societies; (2) to compare the relative importance of conjugal disparity vs personal factors in the explanation of marital relations reported; (3) to delineate the effect of demographic pairing in contrast with the effect of value discrepancy of couples in the perceptions of marital relations. Findings indicate that despite the high consistency percentage of perceptions of marital relations on the aggregate level, there exists a substantial amount of inconsistency among Chinese conjugal pairs. Taiwan couples are relatively more consistent than their counterparts, and Shanghai couples have the lowest estimated consistency value. Marital value is examined by conjugal priority and gender-role values. The cross-society comparison shows that Shanghai couples have more westernized values in both indices, Taiwan couples reveal the most traditional pattern of conjugal priority, while Hong Kong couples have more traditional gender-role values, particularly relating to the mother's role. Taiwan couples also enjoy higher consistency measures. To answer whether conjugal disparity affects perceptions of marital relations - especially whether disparity in the background or in values makes a significant difference, the results generally support the expected negative relation, but vary depending on the context examined. Both personal and pairing factors are important in the account of subjective perceptions of marital relations. With regard to conjugal disparity, it is found that discrepancy in demographic background seems to be more significant than value disparity. In fact, the expected relation between value disparity and unsatisfying marital relations only receives support among Taiwanese wives. The article concludes the importance of the conjugal unit in the study of marital relations. Future studies are suggested to include both objective and subjective indicators of conjugal disparity. An investigation into the factors affecting the adaptation of spouses of Taiwan expatriates allowed for their modes of adaptation to be classified into ‘adjustment’, ‘reaction’ and ‘withdrawal’. Albeit a sample of 15 spouses were interviewed using a semi-structured questionnaire, the research findings indicate that if an expatriate's spouse is characterized as having high cultural flexibility, high social orientation, a high degree of willingness to communicate, a high conflict resolution orientation, low ethnocentricity and a high orientation towards knowledge, the overseas adaptation tends to be of the ‘adjustment’ mode. Cross-society comparisons have become increasingly important to the study of the perceived quality of life. A major critique of these studies, however, is the lack of attention to intra-culture variation. This paper examines how subjective quality of life differs, at both societal and individual levels, between Taiwan and Hong Kong, two Chinese societies that share their cultural heritage but vary in their social and political systems. We draw upon data from synchronic surveys conducted in November 2000 in Taiwan (n=1222) and Hong Kong (n=1044). Ordinal regression analyses confirm that, for both societies, reported satisfaction at the societal level contributes to the variance in individuals’ quality of life. Moreover, the perceived fairness with regard to personal effort within the opportunity structure provided by the society also significantly affects an individual’s quality of life. Different factors accounting for quality of life variation in Taiwan and in Hong Kong are discussed. This paper suggests that for the intra-cultural, cross-society comparisons on quality of life, inclusion of various societal level indicators is not only important, but the subjective perception of the fairness that society also contributes a significant effect.
