Geophysical data provide the chance to investigate a volcano's dynamics; considerable information can especially be gleaned on the stress and strain patterns accompanying the internal processes and the effect of magma ascent on the main structures triggering earthquakes. Here, we analysed in detail the seismicity recorded over the last two decades on Etna volcano (southern Italy), focusing on earthquakes distribution and focal mechanism clustering; the ground deformation pattern affecting the volcanic edifice with the inflation and deflation phases was also examined. Analysed data were compared in order to shed light on possible relationships with the volcanic activity and to better understand the internal dynamics of the volcano over time. Seismic tomography is a very powerful and effective approach to look at depths beneath volcanic systems thus helping to better understand their behaviour. The P-wave and S-wave velocity ratio, in particular, is a key parameter useful to discriminate the presence of gas, fluids and melts. We computed the first 3-D overall model of Vp, Vs and Vp/Vs for the Lipari–Vulcano complex, central sector of the Aeolian volcanic archipelago (southern Italy). The investigated area has been characterized in recent times by fumaroles, hydrothermal activity and active degassing. In particular, in the Vulcano Island, several episodes of anomalous increases of fumarole temperature and strong degassing have been recorded in the past decades and the last "crisis", started in September 2021, is still ongoing. For tomographic inversion we collected ~ 4400 crustal earthquakes that occurred in the last thirty years and we used the LOcal TOmography Software LOTOS. The results clearly depicted two low Vp and Vp/Vs anomalies located up to ~ 8 km depths below Vulcano and the western offshore of Lipari, respectively. These anomalies can be associated to the large presence of gas and they furnish a first picture of the gas-filled volumes feeding the main degassing activity of the area. [ABSTRACT FROM AUTHOR]

We analyze Global Navigation Satellite System (GNSS) and tilt data from the permanent monitoring networks of Etna volcano starting just after the 24 December 2018 eruption to an unusual two‐month period of deflation in February–March, 2021, which coincided with the occurrence of 17 lava fountain episodes. Based on changes in slope in the GNSS displacement time series, we divide the period starting 7 months after the eruption into five phases, spanning the continued inflation of the edifice punctuated by short periods of effusive and strombolian activity (four phases) and a 2‐month phase of intensive deflation. Our model indicates a progressive deepening of the internal pressure sources followed by a fast ascending source starting two‐months before the first 2021 paroxysms. We explain these results in light of a recent volcanological model on the nature and behavior of magma ascending through the Etnean feeding system. Plain Language Summary: On 16 February 2021, an eruptive phase characterized by 17 paroxysms from the Southeast Crater began at Etna volcano, producing km‐high volcanic plumes and large tephra fallouts, causing hazard to air traffic and impacting inhabited areas. The paroxysms caused the most intense deflation of the volcanic edifice in the last two decades. We model both the inflation phases preceding the paroxysms and the period of deflation to investigate the internal pressure sources. Data suggest that the volcanic pressure source deepens progressively, during the recharging phases, and ascends only 2 months before the start of the paroxysms. A complex interaction between the high amounts of volatile elements dissolved in the molten rock and the arrival of new melt intrusions from depth allowed us to explain this singular behavior. Introduction: Persons with disorders of consciousness (DoC) may perceive pain without being able to communicate their discomfort. Nociception Coma Scale (NCS) and its revised form (NCS-R) have been proposed to assess nociception in coma survivors with DoC.Objective: Aim of the present study was to compare, in non-communicative patients with DoC, NCS-R scores obtained with the standard pressure on fingernail bed (standard stimulus, SS) versus other personalized painful stimuli (PS), to verify possible correlations between NCS-R and Coma Recovery Scale-Revised (CRS-R).Materials and Methods: Twenty-one patients with DoC were included in the study. Responsiveness and pain perception were assessed by CRS-R and NCS-R with standard stimulus (NCS-R-SS) and personalized stimulation (NCS-R-PS). Statistical analysis was performed with the nonparametric Wilcoxon test for comparison of both total NCS-R-SS and NCS-R-PS scores.Results: NCS-R at admission showed that 9 of 21 patients (42.8%) had higher scores in response to personalized stimulus compared to standard stimulus. Significant correlation with CRS-R were found for both NCS-R-SS (R = 0.701, p = .008) and NCS-R-PS (R = 0.564, p = .045). Discussion: The preliminary results obtained in the present study suggest that NCS-R-PS may disclose pain perception in a larger number of non-communicative patients with DoC, compared to NCS-R-SS. New data and interpretations of the geodynamics of eastern Sicily point to deep crustal shortening taking place in the area. Reconstructions of the lithospheric system, seismicity distribution, and stress state in the crust indicate that deformation is expressed by a large thrust‐ramp cutting through the entire lower plate. The tectonic structure is propagating directly beneath the Mount Etna volcano, one of the few active volcanoes in Europe. Geostructural interpretation of tomographic sections allows for interpretations of the compressional structure as originating in response to trench‐parallel breakoff of the Ionian slab. Following the simple assumption that if a slab retreats, it must either be compensated or alternatively pushed by the fore‐arc mantle, we argue that the opening of a gateway in the slab has encouraged the fore‐arc mantle to flow toward the Mount Etna region. Mantle mobilization has had a twofold influence on both magmatic source mixing and the inception of underplating processes beneath the Mount Etna. A shortening prevailing over extension in the crust below the volcano seems to have a significant impact on the dynamics of the Mount Etna volcanic system, which manifested through anomalous signals over the last thousands of years. Since a tectonic inversion of previous dilatational magma pathways is expected in such a converging setting, the documented variations are believed to be consistent with a volcano experiencing a declining phase. Comparison with other extinct volcanic systems in the southern Tyrrhenian margin, lying atop a detached slab and involved in contraction, provides insights into the evolution of Mount Etna. We have modeled the fast dike intrusion that started on 24 December 2018 at Mount Etna. The intrusion was accompanied by an intense seismicity swarm that also continued the following day. Since previous studies did not detail the overall chain of events in time during the magma ascent, here we propose a combined analytical and FEM modeling of all available continuous deformation data, focusing on the signals over 2 days (24–25 December) when the continuous deformation networks recorded clear variations directly related to the dike ascent. High‐rate GPS enabled obtaining an early and reliable source model. Borehole instruments (strainmeters and tiltmeters) highlighted clear variations, starting about 1 hr before those recorded by GPS, and moreover, made it possible to improve the dike ascent modeling. In particular, our continuous deformation data clearly revealed not one but two dikes and showed how they evolved in time. We inferred one dike that began propagating in the first kilometer above sea level, continued to rise with a maximum opening of 1.9 m and increased its horizontal dimension until reaching the ground surface. Soon after, continuous deformation networks revealed a new elongated intrusion in the southern flank, matching the south shifted position of the seismic swarm. This second dike, with a thicker opening of 4.9 m, started from a depth of about −3 km (below sea level) but did not reach the ground surface. This proposed multiparametric modeling of continuous deformation data has therefore enabled disentangling the complexity of the real volcanic processes. Primary Objective: The primary aim of this study was to determine the frequency of severe impaired self-awareness (ISA) in patients with severe traumatic brain injury (TBI) and the correlates of selected clinical, neuropsychiatric and cognitive variables. The secondary aim of the study was to assess depression and apathy on the basis of their level of self-awareness. Methods: Thirty patients with severe TBI and 30 demographically matched healthy control subjects (HCs) were compared on measures of ISA, depression, anxiety, alexithymia, neuropsychiatric symptoms and cognitive flexibility. Results: Twenty percent of the patients demonstrated severe ISA. Severe post-acute ISA was associated with more severe cognitive inflexibility, despite the absence of differences in TBI severity, as evidenced by a Glasgow Coma Scale (GCS) score lower than 9 in all cases in the acute phase. Patients with severe ISA showed lower levels of depression and anxiety but tended to show more apathy and to have greater difficulty describing their emotional state than patients with severe TBI who showed minimal or no disturbance in self-awareness. Conclusion: These findings support the general hypothesis that severe ISA following severe TBI is typically not associated with depression and anxiety, but rather with apathy and cognitive inflexibility. Introduction. Severe acquired brain injury (sABI) is considered the most common cause of death and disability worldwide. sABI patients are supported by their caregivers who often exhibit high rates of psychological distress, mood disorders, and changes in relationship dynamics and family roles. Objectives. To explore lifestyle changes of caregivers of sABI patients during the postacute rehabilitation, by investigating possible differences between primary and secondary caregivers. Primary caregivers spend most of the time with the patient, providing daily care and taking most responsibility for the day-to-day decisions, while secondary caregivers are those who provide additional support. Methods. Three hundred forty-seven caregivers of sABI patients were asked to fill in an unpublished self-report questionnaire to explore their possible lifestyles changes. Results. A statistically significant difference was found between primary and secondary caregivers in time spent in informal caregiving (p<0.001). The primary caregivers reduced all leisure activities compared to secondary carers (p<0.05). Conclusions. By comparing the percentage of leisure activities performed by caregivers before and after the patient’s sABI onset, all caregivers showed high percentages of changes in lifestyle and habits, even though primary caregivers reported more negative lifestyle changes than secondary caregivers. Further studies are needed to investigate needs and burden experienced by caregivers of sABI patients during the postacute rehabilitation phase, also in relation to the patients’ outcome, to address support interventions for them and improve their quality of life. We developed a functional semi-structured scale to observe Hemineglect symptoms in Activities of Daily Living (H-ADL). The scale could assist clinicians in assessing rehabilitation priorities aimed at correcting any persisting errors or omissions. In addition, the scale could also be used by caregivers to observe patients’ progress and improve their participation. Two groups of right brain-damaged patients (25 with hemineglect; 27 without hemineglect) were tested twice: at admission and before discharge from hospital. A control group of healthy individuals matched to patients for age and education and patients’ caregivers also participated. Two raters (A; B), experts in neuropsychology, observed patients and healthy individuals using the H-ADL. We found that the H-ADL final scores correlated with the standard hemineglect tests. The three groups differed in performance and differences also emerged between the first and the second assessment, suggesting an improvement due to the remission of hemineglect as a consequence of the treatment. Raters A and B did not differ in their observations, but there were some discrepancies with caregivers’ observations. Therefore, although caregivers could help clinicians in detecting persistent hemineglect behaviour, the assessment should be performed by experts in neuropsychology. [ABSTRACT FROM PUBLISHER]

We reply to the comments of De Natale and Pino (2013) on the paper ‘Are the source models of the M 7.1 1908 Messina Straits earthquake reliable? Insights from a novel inversion and sensitivity analysis of levelling data’ by Aloisi et al. (2012, Geophys. J. Int., 192, 1025–1041). We entirely reject their speculative comments and confirm our viewpoint about the impossibility of discriminating between the two oppositely dipping fault models on the basis of the levelling data alone; we state again that their role as a keystone for modellers is untenable. The comments of De Natale and Pino (2013) are welcomed insofar they give us the possibility to improve our previous analysis, and to criticize the mainstream hypothesis favouring to a low-angle East dipping fault in the Sicilian side of the Messina Straits as responsible of the 1908 destructive earthquake. For decades, many authors have attempted to define the location, geometry and kinematics of the causative fault for the 1908 December 28, M 7.1 earthquake that struck the Messina Straits between Sicily and Calabria (southern Italy). The coseismic displacement caused a predominant downwarping of the Straits and small land uplift away from it, which were documented by levelling surveys performed 1 yr before and immediately after the earthquake. Most of the source models based on inversion of levelling data suggested that the earthquake was caused by a low angle, east-dipping blind normal fault, whose upper projection intersects the Earth surface on the Sicilian (west) side of the Messina Straits. An alternative interpretation holds that the causative fault is one of the high-angle, west-dipping faults located in southern Calabria, on the eastern side of the Straits, and may in large part coincide with the mapped Armo Fault. Here, we critically review the levelling data with the aim of defining both their usefulness and limits in modelling the seismogenic fault. We demonstrate that the levelling data alone are not capable of discriminating between the two oppositely dipping fault models, and thus their role as a keystone for modellers is untenable. However, new morphotectonic and geodetic data indicate that the Armo Fault has very recent activity and is accumulating strain. The surface observations, together with appraisal of macroseismic intensity distribution, available seismic tomography and marine geophysical evidence, lends credit to the hypothesis that the Armo and possibly the S. After a recharge phase that began in 2007, on 13 May 2008, a new eruption started on Mt. Etna volcano. The final intrusion was very fast, accompanied by a violent seismic swarm and marked by ground deformation recorded at permanent tilt and GPS stations. The violence of the eruptive event generated concern that the eruptive fissures might propagate downslope towards populated areas. The ground deformation modelling explains both the mechanism of the intrusion as well as the attempt of the dyke to propagate in the shallower part of the northern sector of the volcano. We show that the 2008 intrusion was characterized by a mechanism, which is new and different to the ones modelled in previous eruptions, following the path of the central conduit in the first part of the intrusion (below 1.6 km) and then breaking off towards the east in the last shallow part. Objective: To assess the natural story of HIV-associated affective and cognitive disorders and the relationship with clinical, pharmacological, immunological and behavioural factors. Method: A total of 395 HIV-positive patients, naive to Highly Active Antirectroviral therapy (HAART), with no severe psychiatric disorders have been enrolled in the Neuro-ICONA Study. All participants were administered a comprehensive data collection instrument including an addiction behaviour survey, a medical problem list, a psychiatric assessment, a validated neuropsychological test battery. Results: The global prevalence of cognitive impairment and of prominent depressive symptomatology were 17.9 and 15.5%, respectively. A significant difference in the prevalence of prominent depressive symptomatology was observed between patients in HAART and those not taking HAART(14.1 vs. 23.8%; P =0.05). Conclusion: Depressive and cognitive disorders affect a substantial proportion of HIV-seropositive subjects. The prevalence of prominent depressive symptomatology appears to significantly vary in relationship to the therapeutic protocol. [ABSTRACT FROM AUTHOR]

Background: Specific information about determinants of sexual behaviour of HIV infected heterosexuals, like injecting drug use (IDU), are essential to design interventions aimed at promoting safer sex practices.Methods: We analysed data on sexual behaviour collected, between March 1997 and March 1999, through a self administered questionnaire among 1050 IDUs and 642 non-IDU heterosexuals enrolled in a prospective multicentre cohort study on the natural history of HIV infection.Results: Among non-IDU heterosexuals, more women (48.5%) than men (25.1%) (p<0.001) reported that they were infected by HIV positive regular partners whose HIV status they were not aware of. Among the 1119 heterosexual males, one fifth reported having had more than 25 sexual partners during their lifetime. Condom use in the last sexual intercourse was more common among heterosexual IDUs (64.9%) than among non-IDU heterosexual males (58.3%) (p=0.05). Heterosexual IDU males were more likely (66.7%) than non-IDU heterosexuals (50.6%) to have an HIV negative partner (p<0.001). Of the 573 heterosexual females studied, 10.2% reported having had more than 25 lifetime sex partners. This proportion was higher among heterosexual IDUs (18.8%) than among non-IDU heterosexuals (4.3%) (p<0.001). Nearly 50% of the women in both groups reported having used a condom in the last intercourse. Almost 57% of heterosexual IDUs had a current HIV negative partner, compared with 34.9% non-IDU heterosexuals (p<0.001). In both sexes, the findings from univariate analysis were confirmed by multiple logistic regression analysis.Conclusions: This study identified some important differences, in both males and females, in sexual lifestyles according to injecting drug use (for example, in terms of HIV negative partners). This study describes the sexual behaviours of women living with HIV, and assesses differences by history of drug use. Its general aim is to contribute in the design of programmes to help people with HIV/AIDS (PWH/A) adopt and maintain safe sexual behaviours. A self-administered questionnaire on sexual and drug use behaviours was distributed to study participants. Between 1997 and 1999, 573 women with HIV infection naive to antiretroviral therapies completed the questionnaire (of whom 234 reported a history of injection drug use (IDU)) and were enrolled in the study. Non-IDU women reported fewer sexual partners, both in their lifetime and in the preceding month, than IDU women: 19% of IDU and 4% of non-IDU women reported more than 25 lifetime sexual partners (p < 0.001). Interestingly, 83% of non-IDU women were infected by their regular partners: these women reported the lowest number of sexual partners. No difference emerged between IDU and non-IDU women in terms of number of sexual intercourse in the two weeks preceding the interview or in terms of condom use in the last intercourse (reported, overall, by 54% of these 573 women). Among women who had sex partners at the time of interview, more non-IDU (65%) than IDU (43%) women reported HIV-positive partners (p < 0.001). Overall, these findings stress a marked heterogeneity in the levels of past and recent sexual promiscuity according to history of drug use. Compensatory hypertrophy was induced in the rat extensor digitorum longus muscle by extirpation of the synergistic tibialis anterior. Injections ofH-thymidine (2 μCi/g body weight) were given at 24, 48, and 72 h after the operation. In animals killed immediately after the last injection both satellite cell nuclei and true muscle nuclei were labeled, whereas in animals killed 4 weeks later only true muscle nuclei were labeled. Context: It has been suggested that early announcements of research works to be published in peer-reviewed journals may diminish newsworthiness of scientific articles, but this issue has not been widely studied.Objective: To analyze the impact on the news media, in terms of volume and prominence of coverage, of a scientific article published in peer-reviewed journals about issues with relevance to public health compared with the impact of preliminary release of information on the same issue.Design: Analysis of press coverage of Creutzfeldt-Jakob disease (CJD) and bovine spongiform encephalopathy (mad cow disease) in the 7 newspapers with the widest circulation in Italy, between March 20, 1996, when the British secretary of state for health announced the identification of 10 cases of a new-variant CJD, described April 6, 1996, in The Lancet, and May 10, 1996. Related newspaper articles were identified by hand search.Main Outcome Measures: Numbers of newspaper articles published before and after publication of the Lancet article.Results: We collected 535 articles, of which 62 (11.6%) appeared on the front page. The number of articles published daily peaked on March 26 with 48 items and 1 article on the front page of all the newspapers. A total of 386 (72%) of the 535 articles and 56 (88.7%) of the 62 published on the front page were published in the first 2 weeks of the study period, before the Lancet publication.Conclusions: Our analysis suggests that, in the case of issues of public health importance, when peer-reviewed research is published after a health risk is disclosed to the public, its impact in the media is small. Extractions of muscle proteins of dystrophic, vitamin-E deficient rabbits show that in advanced dystrophy the actomyosin content of the extract (ionic strength = 0.11) is considerably reduced. The myosin content is even more reduced and may completely disappear from the extract. Actin, on the other hand, does not seem to be quantitatively effected by the dystrophy.
