The celebrated minimax principle of Yao says that for any Boolean-valued function f with finite domain, there is a distribution µ over the domain of f such that computing f to error against inputs from µ is just as hard as computing f to error on worst-case inputs. Notably, however, the distribution µ depends on the target error level1: the hard distribution which is tight for bounded error might be trivial to solve to small bias, and the hard distribution which is tight for a small bias level might be far from tight for bounded error levels. In this work, we introduce a new type of minimax theorem which can provide a hard distribution µ that works for all bias levels at once. We show that this works for randomized query complexity, randomized communication complexity, some randomized circuit models, quantum query and communication complexities, approximate polynomial degree, and approximate logrank. We also prove an improved version of Impagliazzo's hardcore lemma. Our proofs rely on two innovations over the classical approach of using Von Neumann's minimax theorem or linear programming duality. First, we use Sion's minimax theorem to prove a minimax theorem for ratios of bilinear functions representing the cost and score of algorithms. Second, we introduce a new way to analyze low-bias randomized algorithms by viewing them as "forecasting algorithms" evaluated by a certain proper scoring rule. The expected score of the forecasting version of a randomized algorithm appears to be a more fine-grained way of analyzing the bias of the algorithm. We show that such expected scores have many elegant mathematical properties--for example, they can be amplified linearly instead of quadratically. The intestinal epithelium is a polarized monolayer of cells, with an apical side facing the lumen and a basal side facing the blood stream. In mice, both proteins and mRNAs have been shown to exhibit global basal-apical polarization; however, polarization in the human intestine has not been systematically explored. Here, we employed laser-capture microdissection to isolate apical and basal epithelial segments from intestinal tissues of 8 individuals and performed RNA sequencing and mass-spectrometry proteomics. We find a substantial polarization of mRNA molecules that largely overlaps polarization patterns observed in mice. This mRNA polarization remains consistent across different zones of the intestinal villi and is generally correlated with the polarization of proteins. Our protein analysis exposes streamlined intracellular nutrient transport and processing and reveals that mitochondria and ribosomes are less polarized in humans compared to mice. Our study provides a resource for understanding human intestinal epithelial biology. Background: Ovarian aging, often leads to increased use of a donor oocyte, which is associated with greater risk for age-related diseases. Objective: To evaluate the association between women conceiving through oocyte donation (OD) and future cancer-related morbidity, as compared with women conceiving through IVF (in vitro fertilization) with autologous oocytes (AO), spontaneous conceptions (SC), and nulliparas. Methods: This retrospective, cohort study was based on the electronic health records of a very large health maintenance organization. The cohort included mothers who delivered before age 45, during 2000–2019. The index date for surveillance was the delivery date of the relevant pregnancy. Each woman from the OD group was matched to a woman the same age at delivery and with the same number of children. Cancer diagnosis was the main outcome. Results: Matching: 664 OD cases to 664 AO, 700 OD cases to 700 SC, and 700 OD cases to 700 nulliparas. Mean follow-up times were 8.9 ± 3.8 OD, 10 ± 4.1 AO, and 6.4 ± 4.1 years SC. Cancer-related morbidity rates were comparable between OD and the other groups, but compared with nulliparas, a trend was noted (1.6% and 3.1%, respectively, p = 0.07). Survival analysis curves were not significantly different, although a trend was shown in the curve comparing to nulliparity (p = 0.07). In a Cox regression model corrected for BMI, smoking and hormone replacement therapy exposure, cancer in the OD group did not differ compared to the other groups. Older adults struggle with tasks requiring selective attention amidst distractions. Experimental observations about age-related decline have relied on visual search designs using static displays. However, natural environments often embed dynamic structures that afford proactive anticipation of task-relevant information. We investigate the capacity to benefit from spatiotemporal predictions across the adult lifespan. Participants (N = 300, aged 20–80) searched for multiple targets that faded in and out of displays among distractors. Half of the targets appeared at a fixed time and approximate location, whereas others appeared unpredictably. Overall search performance was reduced with age. Nevertheless, prediction-led behaviour, reflected in a higher detection of predictable targets, remained resistant to aging. Predictions were most pronounced when targets appeared in quick succession. When evaluating response speed, predictions were also significant but reduced with progressing age. While our findings confirm an age-related decline, we identified clear indications for proactive attentional guidance throughout adulthood. [ABSTRACT FROM AUTHOR]

Bitter taste receptors (TAS2Rs), a subfamily of G-protein coupled receptors (GPCRs) expressed orally and extraorally, elicit signaling in response to a large set of tastants. Among 25 functional TAS2Rs encoded in the human genome, TAS2R14 is the most promiscuous, and responds to hundreds of chemically diverse ligands. Here we present the cryo–electron microscopy (cryo-EM) structure of the human TAS2R14 in complex with its signaling partner gustducin, and bound to flufenamic acid (FFA), a clinically approved nonsteroidal anti-inflammatory drug. The structure reveals an unusual binding mode, where two copies of FFA are bound at distinct pockets: one at the canonical receptor site within the trans-membrane bundle, and the other in the intracellular facet, bridging the receptor with gustducin. Together with a pocket-specific BRET-based ligand binding assay, these results illuminate bitter taste signaling and provide tools for a site-targeted compound design. Background/Objectives: This study aimed to explore a relationship between the fetal subarachnoid space (SAS) width and various fetal pathologies, employing fetal brain MRI scans. Methods: A retrospective collection of fetal brain MRI scans of 78 fetuses was performed with sonographic indications of microcephaly, macrocephaly, or fetal growth restriction (FGR), during a 7-year period at a single tertiary center. The SAS width (named the SAS index) was manually measured in millimeters in ten specific anatomical locations (four in the axial plane and six in the coronal plane), and then converted to centiles by comparing it to (previously collected) data of apparently healthy fetuses. We evaluated the median SAS centiles using the Kruskal–Wallis and Mann–Whitney U tests for statistical comparison. Results: Seventy-eight subjects (mean gestational age of MRI scan 34.2 ± 2.2 weeks) were evaluated. The median SAS centiles were consistently higher in the macrocephaly group compared to the microcephaly group in all ten anatomical locations (statistically significant except coronal left inferior temporal gyri). Most pronounced difference was displayed in the insula gyri (axial and coronal). The median SAS centiles were higher in the microcephaly group when compared with FGR across all ten anatomical locations (all were statistically significant except for coronal frontal and insula gyri), and the maximal difference was found in the frontal gyri of both planes. The median SAS indexes (IQR) of the three groups in millimeters: macrocephaly 91.55 (86.35–101.05), microcephaly 59.46 (50.00–66.91), and FGR 53.21 (49.71–59.10), p < 0.001. Large-sample datasets are essential in hydrological science to support modelling studies and advance process understanding. However, these datasets often lack standardization, which impedes their combination. Caravan is a community initiative to create a large-sample hydrology dataset of meteorological forcing data, catchment attributes, and discharge data for catchments around the world. Compared to existing large-sample hydrology datasets, the focus of Caravan is to use globally consistent forcing and attribute data to facilitate global studies. Caravan is a community project designed to be expanded by members of the hydrological community using a common cloud-based framework. This dataset is currently the 6th extension to Caravan, based on a subset of hydrological discharge data and station-based watersheds from the Global Runoff Data Centre (GRDC), which are covered by an open data policy. The GRDC is an international data centre operating under the auspices of the World Meteorological Organization (WMO), which collects quality-controlled river discharge data and associated metadata from the National Meteorological and Hydrological Services (NMHS) of WMO Member States. The dataset covers stations from 5,356 catchments and 25 countries, spans the years 1950–2023. This takes the total number of Caravan catchments (core dataset plus extensions) to 22,372 (1589 catchments accounting for duplicates). Addressing the psychiatric aspects of serious illness in palliative care (PC) is crucial to both care delivery and outcomes. Psychiatric comorbidities are common among patients with PC needs and can significantly impact their total burden of symptomatic distress, overall quality of life, functional independence, and healthcare utilization. Yet, these aspects of care are often deferred to mental health consultant teams in the context of busy PC services and often limited human resources. To provide comprehensive and person-centered care, PC clinicians must understand the interplay between medical conditions and psychiatric presentations within a biopsychosocial framework to respond empathically, efficiently, and effectively. This article is the first of a two-part series developed in collaboration with a group of psychiatric-palliative care specialists. This article explores ten common physical manifestations of psychiatric illness and treatment among patients facing serious illnesses. The second article will provide pragmatic tips PC clinicians should know about the psychiatric manifestations of nonpsychiatric serious illness and treatment. Combined, these two articles support a holistic approach that PC clinicians can use to prioritize and integrate both mental and emotional well-being throughout the continuum of serious illness. Older parental age at birth is associated with increased risk of autism spectrum disorders (ASD) in offspring. Independently, shorter telomere length (TL) has also been shown to be associated with ASD in children. However, older paternal age at birth, with or without controlling for maternal age, has been associated with longer TL, a seemingly contradictory finding. Here, we conducted a retrospective cohort study among participants in the UK Biobank to disentangle associations between leukocyte TL and ASD status in adults, and the potential moderation by parental age on adult offspring's TL. Participants with ASD diagnosis (N = 87) with a mean age of 46.0 (SD 4.4) years were matched to participants without ASD diagnosis (N = 870) based on age, sex, ethnicity, education, household income, and assessment center. No statistically significant differences were seen in TL between participants with and without ASD when parental age at birth was not considered. However, there was a significant interaction between ASD diagnostic status and parental age on participants' TL, such that older paternal or maternal age at birth was more strongly associated with longer TL in participants with ASD. This study suggests that the shortened TL observed in children with ASD in previous research may partially depend on parental age at birth. Future studies tracking TL attrition before ASD diagnosis are warranted to depict temporal associations and the interacting effects of parental age at birth and ASD status on TL across the lifespan. Lay Summary: Older biological age, as measured by shortened telomere length, has been observed in children with autism spectrum disorders (ASD). In contrast, older parental age at birth has been related to both ASD risk and longer telomere length. In the present study, telomere length did not differ significantly between adults with and without ASD. However, we found that longer telomere length among adults with ASD versus adults without ASD could be more apparent in individuals with older parents. These findings suggest that parental age at birth may explain telomere length differences between individuals with and without ASD, and that the shorter telomere length previously found in children with ASD may depend on parental age at birth. The practical realization of Nickel‐rich layered oxide cathode materials such as LiNi0.8Mn0.1Co0.1O2 (NMC811) is hampered by several structural and interfacial instabilities over prolonged cycling. Several reports have proposed surface passivation via an artificial cathode electrolyte interphase (ACEI) as a promising method for mitigating the parasitic reactions affecting NMC811 while simultaneously improving its electrochemical performance over prolonged cycling. Herein, we report an in‐house designed (tBuMe2Si)2Zn single source precursor for developing SixZnyOz ternary CEI thin films on NMC811 via molecular layer deposition (MLD) in combination with O3 or H2O as oxidizing agent. We demonstrate that the single precursor (tBuMe2Si)2Zn avoids the need for two different precursors (Si & Zn). In‐depth spectroscopic studies reveal the mechanism of the formation of organosiloxane/zinc‐oxide composite thin film, via intermediates of unprecedented organo‐silicon‐zinc compounds. Understanding the reaction mechanism paved the path for a successful deposition of ACEI on NMC811. Rate capability studies shows the ACEI protected cathodes exhibit higher discharge capacity at 4 C than pristine NMC811. Furthermore, studies on full cells with graphite anode were conducted to evaluate the practical viability of SixZnyOz ACEI thin films on NMC811. After prolonged cycling the ACEI‐coated NMC811 full cells significantly improved the electrochemical performance than pristine NMC811 by ~12%. [ABSTRACT FROM AUTHOR]

The educational staff that meets the children at school struggles to cope with the death of a child, and often avoids talking with them about the death and their sense of loss. This way of coping is complicated in the case of grief among children with special needs, called "disenfranchised grief ". The aim of this qualitative study was to examine the experience and the perceptions of the educational staff in schools for special education and to assess their way of coping, support and care they employ following the death of a student. 15 staff members from different schools in Israel, interviewed. The findings reveal that schools are a significant source of support for students, staff and parents, alike, in the processing of the loss. The special education frameworks generally maintained a special and close connection with students and families. procedures and guidelines are required following death of a student. Diabetes treatment options have improved dramatically over the last 100 years, however, close to 2 million individuals in the U.S. alone live with type 1 diabetes (T1D) and are still dependent on multiple daily insulin injections and/or continuous insulin infusion with a pump to stay alive and no oral medications are available. After decades of focusing on immunosuppressive/immunomodulatory approaches for T1D, it has now become apparent that at least after disease onset, this by itself may not be sufficient, and in order to be effective, therapies need to also address beta cell health. This Perspective article discusses the emergence of such a beta cell-targeting, novel class of oral T1D drugs targeting thioredoxin-interacting protein (TXNIP) and some very recent advances in this field that start to address this unmet medical need. It thereby focuses on repurposing of the antihypertensive drug, verapamil found to nonspecifically inhibit TXNIP and on TIX100, a new chemical entity specifically developed as an oral anti-diabetic drug to inhibit TXNIP. Both have shown striking anti-diabetic effects in preclinical studies. Verapamil has also proven to be beneficial in adults and children with recent onset T1D, while TIX100 has just been cleared by the U.S. Food and Drug Administration (FDA) to proceed to clinical trials. Taken together, we propose that such non-immunosuppressive, adjunctive therapies to insulin, alone or in combination with immune modulatory approaches, are critical in order to achieve effective and durable diseasemodifying treatments for T1D. Summary: Forests face many threats. While traditional breeding may be too slow to deliver well‐adapted trees, genomic selection (GS) can accelerate the process. We describe a comprehensive study of GS from proof of concept to operational application in western redcedar (WRC, Thuja plicata).Using genomic data, we developed models on a training population (TrP) of trees to predict breeding values (BVs) in a target seedling population (TaP) for growth, heartwood chemistry, and foliar chemistry traits. We used cross‐validation to assess prediction accuracy (PACC) in the TrP; we also validated models for early‐expressed foliar traits in the TaP.Prediction accuracy was high across generations, environments, and ages. PACC was not reduced to zero among unrelated individuals in TrP and was only slightly reduced in the TaP, confirming strong linkage disequilibrium and the ability of the model to generate accurate predictions across breeding generations. Genomic BV predictions were correlated with those from pedigree but displayed a wider range of within‐family variation due to the ability of GS to capture the Mendelian sampling term.Using predicted TaP BVs in multi‐trait selection, we functionally implemented and integrated GS into an operational tree‐breeding program. At the 39th meeting of the International Society of Traumatic Stress Studies, four leading scientists and clinicians were invited to reflect on their careers, focusing on the biological mechanisms and markers of traumatic stress. Dr. Raul Andero has contributed to understanding how stress alters memory networks in the brain, influencing the development of novel treatments. Dr. Tanja Jovanovic has pioneered the measurement and mechanistic understanding of fear learning, bridging basic and clinical research. Dr. Murray B. Stein has scaled up clinical and lab observations to large populations, refining the field's understanding of traumatic stress. Dr. Arieh Y Shalev has shaped the definition of traumatic stress, pioneering the longitudinal investigation of stress and integrating advanced computational methods to identify individuals at risk. These panelists were asked to reflect on their initial problems, ambitions, concerns, and unexpected challenges, as well as the influence of their work, on new research trajectories. Their insights provide valuable lessons about the process and content of their work, and their pioneering efforts have significantly advanced our understanding of the biological mechanisms and markers of traumatic stress. Social media influencers from advantaged groups often assume allyship roles and use their stage to support disadvantaged groups fighting against discrimination. However, influencer allyship messages on social media are not always effective and it is unclear what determines reactions of disadvantaged group members toward them. This research examines how the composition of influencer allyship messages—specifically whether the message centers the influencer or the disadvantaged group—affects reactions to it. Across seven studies, we find that influencer allyship messages that center the disadvantaged group are more empowering and, consequently, elicit greater engagement from members of the disadvantaged group. This centering effect manifests only among disadvantaged group members and depends on their empowerment needs and on the empowerment potential of the influencer. Accordingly, the effect is more pronounced for influencers with a large followership, as well as among disadvantaged recipients who believe that their group is bestowed with lower status. Toddlers with smaller vocabulary than expected for their age are considered late talkers (LT). This study explored the effects of characteristics of words on vocabulary acquisition of 12- to 24-month-old LT children compared with an age matched (AM) and a vocabulary matched (VM) group of typically developing peers. Using the Hebrew-MacArthur-Bates-Communicative Development Inventory questionnaire, words were rated by adults for babiness, concreteness and iconicity and scored for phonological complexity. The effects of the characteristics of the words on their acquisition were examined. Results showed that the vocabulary of the toddlers from all three groups consisted of words that are more relevant to a child's world and experience (babiness), more concrete, more phonologically simple, and more iconic. When LT children were compared to AM children, the characteristics of their vocabulary were different in terms of structure (e.g., iconicity) and meaning (e.g., concreteness), but similar in phonological complexity and babiness. When they were compared to VM peers, the vocabulary characteristics of LT children were similar, though they used words that were less related to babies' world. The parallel patterns found for LT and VM children versus AM children suggest that LT children's vocabularies differ from those of their age-matched peers due to their smaller size and not due to structural differences. Background: Previous studies demonstrated the influential role of sustained attention in the reading comprehension of alphabetic writing systems. However, there is limited understanding of how these cognitive functions contribute to reading comprehension in non-alphabetic systems, such as Japanese. This study seeks to explore this gap, focusing on how sustained attention and response inhibition function in a writing system where some of the characters represent meanings rather than sounds, introducing another layer of difficulty in the complex process of reading; Methods: Seventy-five Japanese 9th grade students performed a task to assess sustained attention and response inhibition. The cognitive test was carried out using tablets to enable feasible parallel group administration while maintaining high comparability with ecological classroom settings. Reading comprehension was measured using an exam that the participants took as part of their educational routine; Results: Our results indicate that both sustained attention and response inhibition significantly contributed to the reading comprehension of Japanese 9th grade students; Conclusions: These results replicate and expand previous studies documenting the contribution of sustained attention on the reading comprehension of alphabetic writing systems to a non-alphabetic system. Moreover, our findings unravel another important cognitive factor, namely response inhibition in reading comprehension. Haṭhayoga texts, influential in the development of modern posture-based yoga, offer a conceptual framework intertwining physical cultivation, health, and soteriological aims. This article explores two interconnected inquiries regarding early Haṭhayoga texts: How do they conceptualize health, and how do they perceive the relationship between health and soteriology? These investigations illuminate the place of health in early yoga texts within Hindu traditions prior to its global recognition. This article reveals that early haṭha texts present a holistic approach to health, highlighting the interconnectedness of curing the mind and healing the body. This process entails not only eradicating physical diseases but also the transcendence of all forms of suffering. The transformative process of eliminating the misleading self-identity (ahaṁkāra) is pivotal, leading directly to health. This relationship extends beyond metaphorical comparisons of healing ignorance and suffering. This ontological framework proposes a harmonious integration between the material and soteriological realms, viewing the healthy body as a physical manifestation of the divine essence. Thus, the emergence of the biomedical discourse surrounding yoga in modern times did not occur autonomously. Haṭhayoga, with its roots dating back to the beginning of the second millennium CE, played a crucial role in shaping this historical transformation. [ABSTRACT FROM AUTHOR]

We find that foreign‐born CEOs are more inclined than domestic‐born ones to acquire across borders, and that this inclination is explained by their preference for targets in their birth country. This preference is motivated by foreign‐born CEOs' information advantage in their birth country and by these CEOs' desire to give back to the birth country. CEOs' desire to help their birth country also influences target location, increasing the likelihood of a target in countries that colonized the CEO birth country. The motives that drive acquisitions in acquirer CEOs' birth countries measurably affect the returns of acquirers' shareholders, target premium, and acquisition synergy. Both acquirers' returns and synergy but not target premium are abnormally positive when the motive for an acquisition is a CEO's information advantage in the birth country. Introduction: We assessed the efficacy of taurolidine lock (TL) in preventing catheter-related bloodstream infections (CRBSIs) and related hospitalizations in children with parenteral nutrition (PN) in the home setting. Methods: This study is a retrospective case series study. All children with intestinal failure in a single center in southern Israel who were administered PN and treated with TL between 2017 and 2024 were included. The rates of CRBSI episodes, related hospitalizations and pathogen distribution in the pre-TL and post-TL periods were compared. Results: Overall, 14 patients were included. The median pre-TL and post-TL periods were 990 and 1260 days, respectively. The rate of CRBSI episodes due to bacterial infection per 1000 days declined by 45%, from 6.2 to 3.7, with p = 0.0008, while fungal CRBSI rates were low (<10% of all positive cultures) and did not decline significantly. Similarly, the hospitalization episode rate per 1000 days declined by 41%, from 7.6 to 4.5, with p = 0.001. Conclusions: Taurolidine lock treatment for children with central-line PN resulted in a substantial decrease in CRBSI episodes and related hospitalizations. [ABSTRACT FROM AUTHOR]

This article seeks to explore the way in which key, specifically situated actors within the UK water rescue community understand and perceive challenges relating to navigating policy and practical challenges inherent to their role. Utilizing Lipsky's notion of the 'street level bureaucrat', focus groups and interviews were undertaken with water rescue practitioners who bridge the gap between high‐level strategy implementers and ground‐level operational personnel. Utilizing a grounded theory approach, the research uncovered how these operators conceptualize the boundaries in relation to their work, the role of knowledge and knowledge exchange, the nature of the obligation, especially with regard to differences between statutory and voluntary agencies, and in relation to ever‐changing public attitudes towards risk and expectations of a right to be rescued. This was underscored by consistent reference to challenges posed by funding, resourcing and the contingent nature of both political will and public interest in their work. The findings, drawing on these uniquely situated actors, emphasized the importance of practitioners interacting with and knowing one another, in addition to having a shared understanding of the water environment on the coast and inland, as being a key means to navigate the 'bureaucracy' of water rescue. The ways in which Lipsky's model can complement contemporary discussions regarding social identity within the emergency response to promote both vertical and horizontal cohesion and clarity are examined. Clustered regularly interspaced short palindromic repeats (CRISPR)-Cas (CRISPR-associated proteins) systems provide acquired heritable protection to bacteria and archaea against selfish DNA elements, such as viruses. These systems must be tightly regulated because they can capture DNA fragments from foreign selfish elements, and also occasionally from self-chromosomes, resulting in autoimmunity. Most known species from the halophilic archaeal genus Haloferax contain type I-B CRISPR-Cas systems, and the strongest hotspot for self-spacer acquisition by H. mediterranei was a locus that contained a putative transposable element, as well as the gene HFX_2341, which was a very frequent target for self-targeting spacers. To test whether this gene is CRISPR-associated, we investigated it using bioinformatics, deletion, over-expression, and comparative transcriptomics. We show that HFX_2341 is a global transcriptional regulator that can repress diverse genes, since its deletion results in significantly higher expression of multiple genes, especially those involved in nutrient transport. When over-expressed, HFX_2341 strongly repressed the transcript production of all cas genes tested, both those involved in spacer acquisition (cas1, 2 and 4) and those required for destroying selfish genetic elements (cas3 and 5–8). Considering that HFX_2341 is highly conserved in haloarchaea, with homologs that are present in species that do not encode the CRISPR-Cas system, we conclude that it is a global regulator that is also involved in cas gene regulation, either directly or indirectly. The interactions of microorganisms among themselves and with their multicellular host take place at the microscale, forming complex networks and spatial patterns. Existing technology does not allow the simultaneous investigation of spatial interactions between a host and the multitude of its colonizing microorganisms, which limits our understanding of host–microorganism interactions within a plant or animal tissue. Here we present spatial metatranscriptomics (SmT), a sequencing-based approach that leverages 16S/18S/ITS/poly-d(T) multimodal arrays for simultaneous host transcriptome- and microbiome-wide characterization of tissues at 55-µm resolution. We showcase SmT in outdoor-grown Arabidopsis thaliana leaves as a model system, and find tissue-scale bacterial and fungal hotspots. By network analysis, we study inter- and intrakingdom spatial interactions among microorganisms, as well as the host response to microbial hotspots. SmT provides an approach for answering fundamental questions on host–microbiome interplay. Interactions among plant hosts, bacteria and fungi are measured at high spatial resolution. Background: Recruitment to residency programs in hospitals located in other than major hubs ("remotely located") is a challenge in many countries. In 2011, the Israeli Ministry of Health launched a 10-year financial incentive to encourage physicians to enroll in residency programs in such hospitals. Nearly 1 billion New Israeli Shekels (260 million US$) were invested in that program which had only limited success. As a new physician association's collective agreement is impending, we aimed to measure the effectiveness of selected incentives in attracting medical school graduates to residencies in remotely located hospitals. Methods: This study included Israeli medical students in their final year of medical school. We used an online questionnaire with multiple-choice demographic questions and a 5-point Likert scale to gauge the effect of various incentives on their preference for residency location. Results: Between July and November 2022, 522 students responded (405 studied in Israeli medical schools [out of 705 students] and 117 in foreign medical schools [out of 1936 students]). Forty-two percent had at least one clerkship in a remotely located hospital, and 24% had included at least one remotely located hospital among their top five choices for internship. Only 13% reported that they prefer a residency program in those institutions. The incentive selected by students as most persuasive was government assistance in acceptance to and financial support for a fellowship abroad, followed by a financial grant and fewer on-call hours. Only 7% of the students indicated that no incentive would influence them to choose a remotely located hospital for their residency training. Medical education in a remotely located university and the choice of at least one remotely located hospital among the top five choices for internship were significantly associated with positive incentive receptivity, whereas male sex and older age were associated with negative receptivity. Conclusion: This study on the attitudes of Israeli medical school graduates toward incentives aimed at attracting them to residencies in remotely located institutions revealed that career development opportunities and assistance in obtaining fellowships might influence their choice. Background: Surrogate Decision-Makers (surrogates) are frequently employed in decision-making for critically ill adults. There are insufficient data considering the surrogate experience, stress, and potential for mitigation. Methods: An anonymous online survey queried (1) medical situation (2) total stress (3) demographics (4) potential factors, including sources of information about patient wishes, external sources of support or competing stressors, and their interactions with the medical team through the experience. Results: 108 respondents were included; 91 completed all items. Most respondents ranked their experience as a surrogate as one of the most stressful experiences of their lives; this was associated with whether it was an end-of-life decision (P =.003), Respondent Religion (P =.015), or religious or spiritual beliefs (P =.024), and having their own health problems (P =.008). On individual Likert responses, surrogates reported significant stress mitigation when they felt they had been helpful (P <.001), knew the patient's wishes (P =.0011), specifically discussed patient wishes (P <.001), or patient's wishes were documented (P <.001). Items about surrogate-team interaction also met significance, including the physician being communicative and available (P <.001), respectful (P =.007), honest (P <.001), and validating (P =.001). Conclusions: Surrogate stress is an evolving area for research. Social emotional learning (SEL) of teachers is crucial both to teachers and their students. Thus, teachers' professional development should focus on cognitive, emotional, and social skills in order to promote teachers' identity formation. Teacher professional development is a life-long process that begins in teachers' education. The Biographical Narrative, the story about our lives, was chosen as a method with the potential to promote SEL. Eighteen preservice teachers participated in a workshop focusing on emotional and social skills as part of professional development and shared their Biographical Narrative with the group. Data from the reflections of 12 participants were collected and analyzed qualitatively. The purpose of the study was to understand preservice teachers' perceptions about their professional development, and the contribution of the Biographical Narrative to the social emotional learning of the tellers, the listeners, and the group. The findings revealed that the participants related to cognitive, emotional, and social aspects of development. The developmental process occurs throughout all stages of the method—planning, performance, and reflection. In addition, the Biographical Narrative influenced not only the tellers, but also the listeners and the group. A parent's death shakes the child's world and unbalances the family structure. School can be a source of support, yet there is scarce research on the experience of bereaved children in school a few years after the loss and from their own perspective. In this qualitative-phenomenological study we set out to identify the subjective experience of parentally bereaved children and adolescents in school, at least one year after the death. Twenty parentally bereaved children and adolescents were interviewed. Findings indicated that most felt they were regarded differently by staff at school. For some, this felt right, while others were uncomfortable. Most needed consideration and flexibility and wished the staff would discuss their needs with them. We conclude that it is important to directly inquire about and address each student's needs. Purpose: This randomized controlled trial aimed to ascertain the effect of a pre-procedure informational video on anxiety, pain perception, and satisfaction levels in patients undergoing amniocentesis. Methods: Patients were randomized into two groups: a video group who watched an informational video prior to the procedure, and a control group who received standard care. Anxiety was gauged both pre- and post-procedure via the State-Trait Anxiety Inventory (STAI) score. Post-procedure, patients' perceived pain, anxiety, and satisfaction levels were evaluated using the Visual Analog Scale questionnaire (VAS). Results: Of 110 randomized patients, 100 completed the study and were included in the final analysis. No significant difference was noted in overall anxiety levels between the study and control groups. However, in-procedure anxiety was significantly lower in the video group compared to the control group (p = 0.04). Among patients undergoing amniocentesis for the first time, the subgroup analysis revealed reduced levels of anxiety during the procedure and diminished pain 10 min after the procedure in the video group compared to the control group. (p = 0.041 and p = 0.025, respectively). Conclusion: A pre-procedural informational video could help in alleviating anxiety and mitigating pain during amniocentesis. Background/Objectives: Although high live birth rates are associated with oocyte donation (OD), these pregnancies are associated with increased obstetric and perinatal risks. This study evaluated maternal and neonatal risks after OD compared to in vitro fertilization (IVF) with autologous oocytes, and to spontaneous pregnancies (SPs), among singletons, twins and triplets. Methods: A retrospective, large, population-based cohort study was conducted based on electronic data from Maccabi Healthcare Services. A total of 469,134 pregnancies were grouped according to the mode of conception. The main outcome measures were preterm birth (PTB), small for gestational age (SGA) and pregnancy-induced hypertension (PIH). The data were analyzed separately for singletons, twins and triplets. Results: The mean maternal age was older in the OD group compared with the IVF and SP groups (singletons: 39.7 ± 4.1 vs. 34.5 ± 4.8 and 31.7 ± 5.3 years; twins: 39 ± 4.6 vs. 32.6 ± 4.4 and 31.2 ± 5.1 years; and triplets: 35.6 ± 2.5 vs. 32 ± 3.9 and 29.7 ± 5 years). The mean gestational age was younger among the OD group compared to the SP group (singletons: 37.5 ± 3 vs. 39 ± 2 p = 0.001, and twins: 35 ± 3 vs. 36 ± 2.5 p = 0.001). Higher rates of PTB < 37, PTB < 34 and PTB < 28 weeks were found among OD singletons. Multivariable logistic regressions for PTB < 37 weeks and SGA in singletons demonstrated that OD and IVF are significant risk factors (OR = 4.1, 95%CI = 3.3–5.2; OR = 4.3, 95%CI = 4.1–4.6; OR = 1.9, 95%CI = 1.3–2.6; OR = 2.2, 95%CI = 2–2.4, respectively). Significantly higher rates of PIH were demonstrated among the OD vs. IVF and SP groups in singleton (4.3% vs. 1.7% and 0.7%) and in twin pregnancies (7.5% vs. 4.3% and 3.4%). Background: To compare retention times of various ophthalmic viscosurgical devices (OVDs) and soft‐shell combinations. Methods: Experimental study. Eighteen rabbit eyes were divided into six groups of three eyes, based on OVDs tested. A: Endocoat, B: HealonPro, C: Viscoat, D: Provisc, E: Endocoat and HealonPro and F: Viscoat and Provisc. OVDs were stained with 10% fluorescein dye before being injected into the anterior chamber. Phacoemulsification, using fixed parameters, was performed. If OVD persisted after 60 s, a standardised irrigation and aspiration technique replaced phacoemulsification. The time until central and complete clearance of the OVD were video‐recorded and measured. Results: Mean central retention times (CRT) were found to be: HealonPro—5.33 ± 2.56 s, Provisc—3.33 ± 1.11 s, Endocoat—75.0 ± 3.26 s, Viscoat—62.33 ± 5.19 s, combining HealonPro with Endocoat—22.67 ± 4.75 s and Provisc with Viscoat—11.0 ± 0.82 s. The mean total retention time (TRT) was: Endocoat—80.0 ± 8.17 s, Viscoat—81.67 ± 2.09 s, HealonPro with Endocoat—81.33 ± 3.35 s, and Provisc with Viscoat—71.0 ± 2.94 s. For HealonPro and Provisc, CRT and TRT remained identical across all trials. Conclusions: Retention times varied, with cohesive OVDs exhibiting shorter durations than dispersive OVDs. Among dispersive OVDs, TRTs were comparable; however, Endocoat displayed an extended CRT. In soft shell trials, the combination of HealonPro and Endocoat exhibited prolonged CRT and TRT, suggesting enhanced corneal protection. [ABSTRACT FROM AUTHOR]

Suicide is the second leading cause of death among youth aged 15–24 years. Identifying modifiable risk factors relevant to adolescents is crucial for suicide prevention. Sleep patterns have been linked to suicidality in adults, but lack sufficient study in youth. This ecological momentary assessment (EMA) study aimed to explore the relationship between objectively and subjectively measured sleep characteristics and next-day suicidal ideation in high-risk youth. We included 29 adolescents (12–18 years old) admitted to the inpatient psychiatric ward post-suicide attempt or due to suicidal intent within the previous month. We conducted objective (actigraphy) and subjective (sleep diary) sleep pattern assessments over ten consecutive days. Daily suicidal ideation was evaluated using a questionnaire based on the validated C-SSRS interview. A significant positive association was observed between sleep onset latency (SOL) and expressing a "death wish" the following day (OR = 1.06, 95% CI [1–1.11], p =.04), with each minute of longer SOL increased the risk for a death wish the following day by 6%. In addition, a marginally significant negative association was observed between total sleep time (TST) and expressing a "death wish" the following day (OR = 0.57, 95% CI [0.3–1.11], p = 0.1), with each one-hour decrease in objectively measured TST increasing the odds of a death wish by 43%. Our study highlights the interplay between sleep patterns and suicidal ideation, with SOL and TST playing a significant role that may function as proximal risk factors for suicidality and as a target for intervention while treating suicidal youth. The recent worldwide surge of warfare and hostilities exposes increasingly large numbers of individuals to traumatic events, placing them at risk of developing posttraumatic stress disorder (PTSD) and challenging both clinicians and service delivery systems. This overview summarizes and updates the core knowledge of the genetic, molecular, and neural circuit features of the neurobiology of PTSD and advances in evidence-based psychotherapy, pharmacotherapy, neuromodulation, and digital treatments. While the complexity of the neurobiology and the biological and clinical heterogeneity of PTSD have challenged clinicians and researchers, there is an emerging consensus concerning the underlying mechanisms and approaches to diagnosis, treatment, and prevention of PTSD. This update addresses PTSD diagnosis, prevalence, course, risk factors, neurobiological mechanisms, current standard of care, and innovations in next-generation treatment and prevention strategies. It provides a comprehensive summary and concludes with areas of research for integrating advances in the neurobiology of the disorder with novel treatment and prevention targets. Summary: Fungal infections, a leading cause of mortality among eukaryotic pathogens, pose a growing global health threat due to the rise of drug-resistant strains. New therapeutic strategies are urgently needed to combat this challenge. The PCA pathway for biosynthesis of Co-enzyme A (CoA) and Acetyl-CoA (AcCoA) from vitamin B5 (pantothenic acid) has been validated as an excellent target for the development of new antimicrobials against fungi and protozoa. The pathway regulates key cellular processes including metabolism of fatty acids, amino acids, sterols, and heme. In this study, we provide genetic evidence that disruption of the PCA pathway in Saccharomyces cerevisiae results in a significant alteration in the susceptibility of fungi to a wide range of xenobiotics, including clinically approved antifungal drugs through alteration of vacuolar morphology and drug detoxification. The drug potentiation mediated by genetic regulation of genes in the PCA pathway could be recapitulated using the pantazine analog PZ-2891 as well as the celecoxib derivative, AR-12 through inhibition of fungal AcCoA synthase activity. Collectively, the data validate the PCA pathway as a suitable target for enhancing the efficacy and safety of current antifungal therapies. Studies investigating coherence in teacher preparation often attempt to achieve comprehensive coherence across all components of training to optimize the educational process. Nevertheless, the pursuit of intense coherence presents notable challenges. To address these challenges, we propose the concept of portion coherence, exemplified by the Trio model, which guides pre-service teachers in integrating theories into their practices. Through a mixed-method study, we compared pre-service teachers' perceptions of the importance and relevance of introductory courses at the semester's outset and conclusion. While both groups reported a reduction in perceived importance, the intervention group noted a significantly smaller decline in perceived relevance compared to the control group. This suggests that models of portion coherence may enhance the perceived relevance of educational courses, while the perceived relevance of introductory education courses may serve as an indicator of crosscourse coherence. The study aimed to evaluate the Couples' Closeness–Distance Scale (CCDS), an observational assessment reflecting couples' relational dynamics; based on joint‐drawing, it offers measurable dimensions. Forty cohabiting Israeli couples participated in the joint‐drawing task and answered self‐report questionnaires for attachment, differentiation of self, and relationship satisfaction. The paintings were assessed by trained observers on the CCDS scales (too distant, too close, autonomy, engagement) for both the women's and men's experiences. A two‐level‐dyadic model showed that differentiation and relationship satisfaction predicted the CCDS and revealed a "pursue–withdraw cycle": Women sensed the men as too distant, while men sensed the women as too close. Participants' sense of discomfort from their partners' distance related to less autonomy and engagement during the painting, with a greater effect on the women. Simple Summary: This study examines the application of magnetic nanoparticle hyperthermia (MNH), a cancer treatment technique that utilizes magnetic particles at the scale of nanometers and a controlled magnetic field to selectively heat and destroy cancer cells. The study focuses on a specific system, the Sarah Nanotechnology System, which combines these magnetic particles and a device that generates the magnetic field. The main goal is to ensure this treatment is safe for patients. We used a combination of real-world experiments and computer simulations to test how the system affects the body's temperature, particularly aiming to avoid overheating healthy tissues. We used a virtual human model to predict temperature changes during treatment. The findings are promising for safely using this advanced technology in cancer treatment, potentially offering a new, targeted approach for patients with advanced-stage tumors. This could be a significant step forward in cancer therapy, highlighting the importance of combining experimental and computational methods in medical research. The present study focuses on the development of a methodology for evaluating the safety of MNH systems, through the numerical prediction of the induced temperature rise in superficial skin layers due to eddy currents heating under an alternating magnetic field (AMF). The methodology is supported and validated through experimental measurements of the AMF's distribution, as well as temperature data from the torsos of six patients who participated in a clinical trial study. The simulations involved a computational model of the actual coil, a computational model of the cooling system used for the cooling of the patients during treatment, and a detailed human anatomical model from the Virtual Population family. The numerical predictions exhibit strong agreement with the experimental measurements, and the deviations are below the estimated combined uncertainties, confirming the accuracy of computational modeling. This article examines the use of annual earnings guidance as a mechanism used by managers to reduce the volatility of analyst earnings forecasts and allow them to report smooth earnings without missing quarterly analyst forecasts. Facing the pressure to meet or beat analyst forecasts and driven by the perceived capital market benefits of reporting a smooth earnings path, managers attempting to influence investors' earnings expectations over a longer horizon can issue annual guidance to smooth the time-series path of analyst forecasts, a strategy we term as "expectation smoothing." Our empirical results suggest that annual guidance reduces the volatility of analysts' multi-period forecasts, which in turn contributes to a smoother actual earnings and higher likelihood of meeting analysts' quarterly forecasts. We also find that issuing quarterly guidance does not affect the smoothness of analysts' earnings expectations and that managers with longer horizons are more likely to issue annual guidance, consistent with the unique long-term effects of annual earnings guidance. Nickel‐rich cathode materials such as LiNi0.9Co0.05Mn0.05O2 (NMC90) have gained attention due to their ability to deliver high energy densities while being cost‐effective for Lithium‐ion battery manufacturing. However, NMC90 cathodes suffer irreversible parasitic reactions such as electrolyte decomposition, severe capacity fading and impedance build‐up upon prolonged cycling. Herein, we synthesize a conformal ultrathin, surface protection layer on NMC90 powder using ZnxOyvia atomic layer deposition technique (ZnxOy@NMC90). Prolonged electrochemical investigation of full cells at high discharge rates of 2 C shows that ZnxOy@NMC90 cells yielded ~31 % improvement in discharge capacity compared to pristine NMC90. Furthermore, operando electrochemical mass spectroscopy studies show that the ZnxOy@NMC90 cells have significantly suppressed electrolyte decomposition as compared to pristine NMC90 cells. Post‐cycling electrochemical impedance spectroscopy studies show that the ZnxOy@NMC90 full cells have significantly reduced impedance compared with pristine NMC90 cells. Additionally, post cycling manganese dissolution studies show that ZnxOy@NMC90 cells have greatly enhanced chemo‐mechanical integrity thereby contributing to improved electrochemical performances. Our results underscore the potential of tailored ZnxOy surface coatings on nickel‐rich cathode materials to address critical challenges in advanced energy storage systems, offering promising prospects for the development of high‐energy‐density lithium‐ion batteries. As Pb does not form a solid solution with pure Cu or with the Sn–Cu alloy phases, it is normally observed in the metal matrix as globular droplets embedded within or in interstitial positions among the crystals of Sn‐bronze (normally the α phase) as the last crystallizing phase during the cooling process of the Cu–Sn–Pb ternary melt. The disequilibrium Sn content of the Pb droplets has recently been suggested as a viable parameter to detect modern materials [Shilstein, Berner, Feldman, Shalev & Rosenberg (2019). STAR Sci. Tech. Archaeol. Res.5, 29–35]. The application assumes a time‐dependent process, with a timescale of hundreds of years, estimated on the basis of the diffusion coefficient of Sn in Pb over a length of a few micrometres [Oberschmidt, Kim & Gupta (1982). J. Appl. Phys.53, 5672–5677]. Therefore, Pb inclusions in recent Sn‐bronze artifacts are actually a metastable solid solution of Pb–Sn containing ∼3% atomic Sn. In contrast, in ancient artifacts, unmixing processes and diffusion of Sn from the micro‐ and nano‐inclusions of Pb to the matrix occur, resulting in the Pb inclusions containing a substantially lower or negligible amount of Sn. The Sn content in the Pb inclusions relies on accurate measurement of the lattice parameter of the phase in the Pb–Sn solid solution, since for low Sn values it closely follows Vegard's law. Here, several new measurements on modern and ancient samples are presented and discussed in order to verify the applicability of the method to the detection of modern artwork pretending to be ancient. This study aimed to examine attachment and adult separation anxiety (ASA) among women who lost a parent in their youth. We hypothesized that insecure attachment and increased ASA from a romantic partner would be found among women who have lost a parent in youth, compared to women whose parents were both alive. Sixty women who lost one or both parents in their youth and 60 who had living parents participated in the study (mean age: 32.3, range: 18–62 years). Participants filled out the ASA and Short Attachment questionnaires. Women who lost a parent reported higher levels of anxious attachment and ASA from partner; the two groups did not differ, however, in terms of avoidant attachment. Additionally, similar effects on ASA and attachment were found among adult women who lost a father or a mother in their youth. In conclusion, the loss of a parent early in life may be associated with an insecure attachment style and increased ASA. [ABSTRACT FROM AUTHOR]

Genetic counseling (GC) following abnormal Down syndrome (DS) screening tests aims to ensure learning of complex medical concepts and discussion of counselees' personal desires. Pre‐GC use of electronic learning tools (e‐learning tools) can facilitate GC sessions by allowing more time for dialogue rather than learning medical and genetic concepts, enabling greater focus on the counselee's decisional, psychological, and personal needs. Few studies have investigated such tools for DS screening tests and those who have focused on screening uptake rather than abnormal results and implications. This study evaluated prenatal GC outcomes following implementation of an e‐learning tool utilizing an educational animated movie for couples of varied ethnic backgrounds in northern Israel, with abnormal DS screening tests. E‐learning tool impact was assessed as knowledge level, informed choices, satisfaction with the intervention and GC process, the state of anxiety and duration of the GC meeting. The 321 study participants were randomized to three groups: animation movie, booklet, and control. All participants had been asked to complete pre‐ and post‐counseling questionnaires. Outcome scores were compared between the research groups. Results showed increased knowledge level in general among participants in the animation group; among minority participants, the highest knowledge level was in the animation group. Anxiety levels and informed choices were not statistically different among the groups. However, watching the animation, Jewish ethnicity, good level of genetic literacy, and academic degree were significant predictors of informed choice, and those who watched the animation were three times more likely to make an informed choice than the control group. Our findings suggest that this e‐learning tool is efficient and acceptable for the general population. Special attention is needed for minorities with lower genetic literacy and education.
