This paper presents the analysis of the effect of differential signal coupler positioning accuracy on its high frequency performance parameters for contact-less high speed chip-to-chip data transmission on PCB application. Our considerations are continuation of the previous works on differential signal coupler concept, design methodology and analysis for high speed data transmission monitoring. The theoretical analysis of possible coupler positioning innaccuracies is extended for representative design cases by simulations carried out using EM simulator. The results reported here confirm that the concept of contactless monitoring of high speed chip-to-chip data transmission in a pair of coupled microstrip lines is of practically usefulness without applying the expensive and precise positioning system. This paper presents a technique of decoupling differential signals transmitted in a pair of microstrip lines on a printed circuit board (PCB), using dedicated coupler for high speed data transmission monitoring in chip-to-chip interconnections. The coupler used for signal probing is overlayed on the pair of microstrip lines under test, and provides a signal to the next blocks of the measurement system without disturbing transmission in the lines. Starting from the basic configuration obtained in the previous work, we describe a procedure of re-designing the coupler for enhanced directivity so that it is suitable for high-speed data transmission monitoring. The new coupler structure has improved topology based on stepped-impedance resonators with geometry parameters adjusted using numerical optimization techniques. The correctness of the design has been confirmed by the time-domain measurements performed for real chip-to-chip communication signals, using a dedicated measurement system based on Xilinx Virtex-6 FPGA Development Kit. The presented technology can be used for reducing the cost of the product diagnostics by eliminating dedicated measurement connectors, run-time system debugging or reverse engineering. This study describes a concept of a novel microstrip directional coupler for differential signal decoupling, which can be used to digital signal overhearing on printed-circuit-boards. The complete design method is proposed with rules given analytically. Considered methodology is suitable for synthesis of couplers with low coupling factors, which have negligible influence on the transmission in main line. Theoretical considerations are confirmed by numerical tests performed using full-wave simulator for the practical case of the coupler project and measurement results. The authors demonstrate that the accuracy of the design method is sufficient for practical system design both in terms of required coupling factor and influence on the main transmission line. To describe a case series of patients investigated in internal medicine for orbital inflammation (OI) since the individualization of the clinical entity of the IgG4-related orbital disease (IgG4 ROD). Thirty four patients were consecutively referred by a specialized center where orbital biopsy was performed when the lesion was surgically accessible. Fourteen patients were excluded in case of missing data or lymphoma, periocular xanthogranuloma or Graves' orbitopathy. Patients with systemic or auto-immune disease according to the international criteria, or presenting with idiopathic orbital inflammation syndrome (IOIS), were included. Knowing the histological similarities between IOIS and IgG4 ROD, immunohistochemical assessment of plasma cells for IgG4 positivity was performed for every patient with available biopsy. Clinical and biological characteristics, treatment and response to treatment of included patients are reported. Among 22 included patients, 10 presented with orbital manifestation of a systemic or autoimmune disease including 2 sarcoidosis (9%) and 8 (36%) cases of non specific OI which were reclassified in IgG4 ROD. Finally, IOIS of various clinicopathological presentation was diagnosed for 12 patients including 10 with histological documentation. Whereas relapse and resistance were not found to be related to IgG4 positivity (50% in both IOIS and IgG4 ROD groups), another treatment in addition to corticosteroids was more often necessary in IgG4 ROD patients (50%) than in IOIS patients (25%). After ruling out auto-immune orbital diseases, especially IgG4 ROD, IOIS should be discussed. Factors conditioning the corticosteroid response are yet to be determined. [ABSTRACT FROM AUTHOR]

Une dacryoadénite est une inflammation des glandes lacrymales dont le diagnostic est principalement clinique mais peut être confirmé par des examens d'imagerie spécialisés (échographie, scanner ou IRM). On distingue les dacryoadénites infectieuses (bactériennes ou virales) des autres causes : néoplasiques (en particulier lymphome du MALT) ou plus souvent inflammatoires, parfois dans le cadre d'une pathologie systémique sous-jacente. L'objectif de cette étude était de décrire les caractéristiques démographiques et cliniques, les étiologies ainsi que le pronostic de patients avec une dacryoadénite non-infectieuse. Nous avons mené une étude rétrospective entre janvier 2012 et janvier 2022 sur l'ensemble des patients adultes ayant consulté pour une dacryoadénite dans un centre français d'ophtalmologie spécialisé. Le diagnostic reposait sur les données de l'examen clinique et était confirmé par un examen morphologique (échographie ou IRM orbitaire). Les patients avec une dacryoadénite infectieuse et ceux n'ayant pas eu de consultation de contrôle étaient exclus. Les données démographiques et cliniques ont été récupérées par étude des dossiers médicaux. Un total de 117 patients remplissait les critères d'inclusion, comprenant 92 femmes (79 %) avec un âge moyen de 44 ans (± 14 ans). La durée médiane entre les premiers symptômes et la première consultation était de 48 jours (IQR : 16–154). La dacryoadénite était habituellement unilatérale (64 %) avec un début progressif (51 %), sans symptomatologie extra-ophtalmologique (89 %) mais avec des douleurs oculaires (52 %) et un œdème palpébral (80 %), rarement associés à des symptômes inflammatoires : érythème palpébral (25 %), chemosis (14 %) et hyperhémie conjonctivale (21 %). Environ 15 % des patients présentaient une limitation oculomotrice avec diplopie (16 %) et 13 patients (11 %) présentaient une inflammation intra-oculaire associée. Le diagnostic était confirmé par une échographie (n = 42) et/ou par une IRM orbitaire (n = 102). À l'IRM, l'inflammation s'étendait au-delà de la glande lacrymale dans 44 % des cas : muscles orbitaires (34 %), graisse extraconique (42 %), intraconique (36 %) et paupière (34 %). À l'issue du bilan étiologique, un diagnostic était finalement posé chez 50 patients (43 %) : sarcoïdose (n = 16), maladie associée aux IgG4 (n = 12), lymphome du MALT (n = 7), syndrome de Gougerot-Sjögren (n = 4), vascularite à ANCA (n = 4) ou autre maladie auto-immune (n = 6). Soixante-sept patients (57 %) avaient une dacryoadénite idiopathique. Une biopsie chirurgicale de glande lacrymale a été réalisée chez 82 patients (70 %) et contribuait au diagnostic chez 26 patients (32 %). Un scanner TAP a été réalisé chez 60 patients et retrouvait des anomalies dans 50 % des cas, principalement des adénopathies médiastinales (n = 14) et des micronodules/nodules pulmonaires (n = 10). La durée médiane de suivi était de 18 mois (IQR : 8–35) avec un suivi plus court (10 mois vs 25 mois, p = 0,005) et davantage de perdus de vue (42 % vs 19 %, p = 0,02) chez les patients avec une dacryoadénite idiopathique comparativement aux patients avec une pathologie systémique sous-jacente. Un traitement par voie systémique a été introduit chez 42 % (n = 25/60) des patients avec une dacryoadénite idiopathique et 82 % des patients (n = 32/39) avec une maladie systémique sous-jacente (p < 0,001) avec une dose médiane de prednisone de 30 vs 40 mg/jour (p = 0,4) et un recours aux immunosuppresseurs dans 7 et 38 % des cas, respectivement (p < 0,001). Le taux de guérison complète était de 60 % chez les patients avec une dacryoadénite idiopathique contre 56 % chez les patients avec une maladie systémique sous-jacente. Le taux de rechute de la dacryoadénite était respectivement de 11 et 30 % (p = 0,03) au cours du suivi. Le diagnostic de dacryoadénite non-infectieuse doit être évoqué devant une tuméfaction palpébrale d'évolution prolongée et/ou ne répondant pas à une antibiothérapie bien conduite. Dans notre étude, un diagnostic étiologique a pu être posé dans 43 % des cas grâce à un bilan biologique, morphologique et une biopsie de la glande lacrymale, informative dans 32 % des cas. Nous n'avons pas trouvé de différence notable entre les patients avec une dacryoadénite idiopathique isolée et les patients présentant une pathologie systémique sous-jacente en dehors d'un taux de rechute de la dacryoadénite plus important, justifiant d'une surveillance plus prolongée. Objectives: To measure the frequency of infraorbital nerve enlargement (IONE) on magnetic resonance imaging (MRI) in European patients suffering from an IgG4-related ophthalmic disease (IgG4-ROD) as compared to patients suffering from non-IgG4-related ophthalmic disease (non-IgG4-ROD).Methods: From January 2006 through April 2015, 132 patients were admitted for non-lymphoma, non-thyroid-related orbital inflammation. Thirty-eight had both pre-therapeutic orbital MRI and histopathological IgG4 immunostaining. Fifteen patients were classified as cases of IgG4-ROD and 23 patients as cases of non-IgG4-ROD. Two readers performed blinded analyses of MRI images. The main criterion was the presence of an IONE, defined as the infraorbital nerve diameter being greater than the optic nerve diameter in the coronal section.Results: IONE was present in 53% (8/15) of IgG4-ROD cases whereas it was never present (0/23) in cases of non-IgG4-ROD (P < 0.0001). IONE was only present in cases where, on MRI, the inflammation of the inferior quadrant was present and in direct contact with the ION canal.Conclusions: In European patients suffering from orbital inflammation, the presence of IONE on an MRI is a specific sign of IgG4-ROD. To evaluate the relative contribution of different Magnetic Resonance Imaging (MRI) sequences for the extraction of radiomics features in a cohort of patients with lacrimal gland tumors. This prospective study was approved by the Institutional Review Board and signed informed consent was obtained from all participants. From December 2015 to April 2017, 37 patients with lacrimal gland lesions underwent MRI before surgery, including axial T1-WI, axial Diffusion-WI, coronal DIXON-T2-WI and coronal post-contrast DIXON-T1-WI. Two readers manually delineated both lacrimal glands to assess inter-observer reproducibility, and one reader performed two successive delineations to assess intra-observer reproducibility. Radiomics features were extracted using an in-house software to calculate 85 features per region-of-interest (510 features/patient). Reproducible features were defined as features presenting both an intra-class correlation coefficient ≥0.8 and a concordance correlation coefficient ≥0.9 across combinations of the three delineations. Among these features, the ones yielding redundant information were identified as clusters using hierarchical clustering based on the Spearman correlation coefficient. All the MR sequences provided reproducible radiomics features (range 14(16%)−37(44%)) and non-redundant clusters (range 5-14). The highest numbers of features and clusters were provided by the water and in-phase DIXON T2-WI and water and in-phase post-contrast DIXON T1-WI (37, 26, 26 and 26 features and 14,12, 9 and 11 clusters, respectively). A total of 145 reproducible features grouped into 51 independent clusters was provided by pooling all the MR sequences. Introduction Le syndrome d’inflammation orbitaire idiopathique (SIOI), anciennement appelé pseudotumeur inflammatoire de l’orbite, correspond à une inflammation non spécifique des structures orbitaires sans cause locale ou systémique retrouvée, ce qui fait de cette pathologie rare un diagnostic d’exclusion. Les maladies systémiques (MS) ou les lymphomes ayant une présentation similaire, la vérification histologique est souhaitable dès que possible. Bon nombre d’études ont montré que les lymphoplasmocytes présents au sein de cette inflammation non spécifique prenaient parfois l’immunomarquage IgG4 aussi intensément qu’en cas de maladie associée aux IgG4. Le SIOI pathologie engageant le pronostic visuel rechuterait dans 30 à 50 % des cas sous traitement, les corticoïdes étant classiquement prescrits en première intention. L’objectif initial de cette étude multicentrique nationale était de décrire les caractéristiques anatomocliniques, morphologiques et immunologiques des patients atteints de SIOI à l’inclusion dans la cohorte. Patients et méthodes Quatre-vingt-sept patients âgés de plus de 18 ans ont été inclus entre mars 2012 et juillet 2015 dans la mesure où ils étaient pris en charge pour un SIOI – diagnostiqué dans des centres spécialisés français pour la prise en charge des tumeurs de l’orbite – après exclusion d’une MS définie – dont le traitement par la cortisone était parfaitement traçable. Ont été collectées les données cliniques, les données anatomopathologiques, morphologiques et immunologiques au diagnostic, ainsi que les traitements éventuels du début de la prise en charge jusqu’à l’inclusion. Les prélèvements histologiques ainsi que les IRM orbitaires ont été relus en aveugle de façon centralisée par un expert. L’inflammation orbitaire a été décrite selon la classification de Mombaerts et al. [1] distinguant la forme cellulaire pure, celle où prédomine la fibrose, et les formes purement granulomateuse et vascularitique. Les patients ont été déclarés IgG4 positifs lorsqu’ils satisfaisaient aux critères histologiques d’Umehara et al. [2] . Les patients dont le diagnostic de SIOI avait été réalisé moins de 3 moins avant l’inclusion étaient considérés comme « de Novo », au-delà, « en cours de suivi ». Dans ce dernier cas, trois profils évolutifs ont été décrits – la rémission correspondant à l’arrêt de la corticothérapie ou l’obtention d’une dose minimale efficace ≤ 10 mg/j – la rechute, en cas de reprise ou re ascension de la corticothérapie – et la résistance, en cas de posologie minimale efficace des corticoïdes ≥ 20 mg/j. Résultats Selon l’analyse des 60 premiers dossiers traités, les patients (32 femmes et 28 hommes), avaient un âge médian de 51 ans (19–80). Trente-huit (63 %) patients atteints de SIOI satisfaisaient au moins un critère de MS sans jamais valider tous les critères définissant la MS. Parmi eux, 17 (45 %) patients avaient possiblement une granulomatose avec polyangéite et éosinophilie, 13 (34 %) un lupus érythémateux systémique, 10 (26 %) une granulomatose avec polyangéite, 7 (18 %) un syndrome de Gougerot-Sjögren. De plus, on notait la présence d’ANCA chez 4 (11 %) patients. La biopsie orbitaire a été pratiquée chez 39 (65 %) patients. Sur 30 prélèvements analysés, on retrouvait parmi 21 cas (70 %) interprétables, 9 (43 %) patients ayant une forme classique d’inflammation orbitaire et 13 (62 %) une forme sclérosante. Parmi les 12 (57 %) patients validant les critères histologiques de la maladie associée aux IgG4, les formes cellulaires et sclérosantes étaient rencontrées dans 5 (42 %) et 7 (58 %) cas, respectivement. Dix-sept (28 %) patients avaient un SIOI « de novo » et 43 (72 %) « en cours de suivi ». Parmi ces derniers, le SIOI évoluait depuis une médiane de 27 mois (5–314). À l’inclusion, 16 (37 %), 20 (47 %) et 6 (14 %) des patients étaient en rémission, rechute ou corticorésistant, respectivement. Conclusion La majorité des patients SIOI inclus dans cette cohorte nationale avaient potentiellement une MS sous-jacente et semblaient avoir une évolution contrastée sous corticoïdes. C’est leur suivi prospectif qui permettra de déterminer dans quelle mesure le SIOI évoluera vers une véritable MS mais aussi de définir ses profils évolutifs sous traitement et les facteurs qui conditionnent son évolution (ClinicalTrials.gov number, NCT01443000 ).
