Objective: Depression and anxiety are common and well defined psychiatric comorbidities in multiple sclerosis (MS), which decrease the quality of life. Misophonia is an underrecognized condition that consists of abnormal emotional responses to specific environmental sounds or rarely to images. Limbic system impairments can be the leading cause of both psychiatric disorders in MS and misophonia. The prevalence of misophonia and its relationship with depression and anxiety have not been studied in the MS population. This study aimed to investigate the frequency of misophonia in MS and its effects on depression and anxiety severity. A secondary aim was to evaluate the effect of the factors associated with MS on the misophonia severity. Materials and Methods: Fifty-nine patients with MS were recruited for the study. MS type, disease duration, attack frequency per year and smoking habbits were evaluated. The presence and severity of misophonia, and depression and anxiety scores in MS group were compared with 63 healthy controls. Results: The frequency of misophonia was significantly higher in MS group (37.3%) compared to controls (19.4%) (p=0.046). Severe depression (23.7%) and anxiety (28.8%) were more frequent in the MS group compared to controls (1.6% and 11.1% p<0.001). Patients with MS and misophonia (MSwM) had a significantly higher frequency of severe depression (p=0.004) and anxiety (p=0.019) than patients with MS without misophonia (MSwoM). MS type, duration, attack frequency and smoking habbits were comparable between MSwM and MSwoM. There was a significant positive correlation between the misophonia score and the Beck depression score (p=0.029) in patients with MS. Tacrolimus and cyclosporine A are immunosuppressant drugs with narrow therapeutic windows. The aim of this study was to investigate the stability of tacrolimus and cyclosporin A levels in whole blood samples under different storage conditions. Whole blood samples were obtained from 15 patients receiving tacrolimus and 15 patients receiving cyclosporine A. Samples were immediately analyzed and then stored at different conditions (room temperature (24°C−26°C) for 24 hours, +4°C for 24 and 48 hours, and −20°C for one month) and then analyzed again. For tacrolimus, there was a significant difference between samples analyzed immediately and those kept 24 hours at room temperature (P=0.005) (percent change 32.89%). However, there were no significant differences between the other groups. For cyclosporine A, there was a significant difference between samples analyzed immediately and those kept 24 hours (P=0.003) (percent change 19.47%) and 48 hours (P=0.002) (percent change 15.38%) at +4°C and those kept 24 hours at room temperature (P=0.011) (percent change 9.71%). Objective: Urinalysis and urine culture are most common tests for diagnosis of urinary tract infections. The aim of our study is to examine the diagnostic performance of urine analysis and the role of urine analysis to determine the requirements for urine culture. Methods: Urine culture and urine analysis results of 362 patients were retrospectively analyzed. Culture results were taken as a reference for chemical and microscopic examination of urine and diagnostic accuracy of the test parameters, that may be a marker for urinary tract infection, and the performance of urine analysis were calculated for predicting the urine culture requirements. Results: A total of 362 urine culture results of patients were evaluated and 67% of them were negative. The results of leukocyte esterase and nitrite in chemical analysis and leukocytes and bacteria in microscopic analysis were normal in 50.4% of culture negative urines. In diagnostic accuracy calculations, leukocyte esterase (86.1%) and microscopy leukocytes (88.0%) were found with high sensitivity, nitrite (95.4%) and bacteria (86.6%) were found with high specificity. The area under the curve was calculated as 0.852 in ROC analysis for microscopic examination for leukocytes. Conclusion: Full-automatic urine devices can provide sufficient diagnostic accuracy for urine analysis. The evaluation of urine analysis results in an effective way can predict the necessity for urine culture requests and especially may contribute to a reduction in the work load and cost. Objectives To assess serum prolidase enzyme activity and oxidative stress in patients with Behçet's disease (BD). Methods The study population consisted of BD patients (n = 42) and healthy participants (n = 29). BD patients were classified as active (n = 18) or inactive (n = 24) according to disease activity. Serum prolidase enzyme activity, total antioxidant status (TAS), total oxidative status (TOS), oxidative stress index (OSI), and malondialdehyde (MDA) levels were measured. Results In BD patients with active disease, serum prolidase activity was significantly higher compared with the inactive and control participants. Serum prolidase activity was also significantly higher in all BD patients in comparison with controls. Serum prolidase activity was also positively correlated with OSI, C-reactive protein, and active BD. MDA, TOS, and OSI levels were all significantly higher in the BD group when compared with the healthy control participants. Serum TAS levels were significantly lower in BD patients in comparison with healthy controls. Conclusion High prolidase activity may indicate critical biological activities relevant to pathological events in BD, and this activity may be a biological indicator of disease. This study was undertaken to evaluate the effect of simvastatin, a cholesterol-lowering agent, on vascular endothelial growth factors (VEGFs), nitric oxide (NO) levels and neuroprotection, in rats with experimentally induced traumatic brain injury (TBI). Forty Wistar albino rats were categorized into four groups: sham operated (S), trauma (T), trauma + vehicle (T + V) and trauma + simvastatin (T + S). The T, T + V and T + S groups were subjected to TBI. The T + V group was administered vehicle [ethanol:saline (1/2)] and the T + S group was administered 1 mg/kg of simvastatin 3 h after the injury insult. Blood and brain tissue specimens were obtained 24 h after the trauma to measure VEGFs and NO levels and perform histopathological examinations. The histopathological injury scores of brain tissues were significantly higher in the T group, and simvastatin significantly prevented brain injury in the T + S group. In the T group, significant increases of VEGF levels in serum and brain tissues were noted, which were prevented with simvastatin treatment in the T + S group. The markedly high levels of NO in brain tissues of the T group were decreased by simvastatin treatment in the T + S group. It can be concluded that, as evidenced by histopathological findings, simvastatin treatment improves neuropathology in acute stages of TBI. [ABSTRACT FROM AUTHOR]

Objective: Previous studies have shown the role of oxidative stress in methanol neurotoxicity. CAPE is known to have an antioxidant property that is shown in many experimental studies. In this study, we aimed to investigate whether CAPE has a protective effect against oxidative stress observed in the cerebellar tissue in methanol intoxication. Material and Method: In this study, a total of 40 rats were split into 5 groups: control group (n=8), MTX-alone group (n=8), MTX+methanol group (n=8), MTX+methanol+ethanol group (ethanol group) (n=8), and MTX+metanol+CAPE group (CAPE group) (n=8). All the rats except the control group were delivered methotrexate (MTX) therapy (0.3 mg/kg/day, via i.p. route) for 7 days in order to induce methanol toxicity. The control group received no drug therapy. Seven days later, 3 g/kg (i.p.) methanol was delivered in the ethanol and CAPE groups. Four hours after the delivery of methanol, ethanol group received 0.5 g/kg ethanol (i.p.) and CAPE group received 10 µmol/kg CAPE (i.p.), while the other groups were delivered only saline (i.p.). The rats were decapitated after 8 hours and the cerebellar tissues were removed. PON-1, TAS, and MDA levels were measured in the tissues. Results: MTX-alone group demonstrated decreased TAS and PON-1 levels (p=0.001 and p=0.004, respectively) and increased MDA level (p=0.001), as compared to the control group. When MTX+methanol group was compared with the MTX-alone group, MTX+methanol group was found to have decreased TAS and PON-1 activities (p=0.037 and p=0.046, respectively) and increased MDA level (p=0.022). The ethanol group was found to show a significant decrease in MDA level (p=0.001), as compared with the MTX+methanol group. The CAPE group exhibited increased TAS and PON-1 levels (p=0.001 and p=0.001, respectively) and decreased MDA levels, as compared with the MTX+methanol group.Discussion: Cerebellum demonstrates oxidative stress secondary to methanol intoxication. Aim: Although the urine culture is used as the reference standard to determine presence or absence of urinary tract infection, the culture is an expensive and time-consuming method. The objectives of the study were to compare dipstick urinalysis with microscopic urinalysis and to compare dipstick and microscopic urinalysis results with urine culture results, by calculating performance characteristics of these tests. Method: The 250 morning urine specimens were performed by using dipstick and microscopic urinalysis and cultured. Laboratory urinalysis included semi-automated dipstick reading by a Miditron-M Reflectance photometer and microscopic examination. Result: Thirty-five point six percent (89/250) of patients had urine cultures with 105 colonies/mL or greater. Sensitivity and specificity of microscopic urinalysis were 91% and 68%, whereas in dipstick uri­nalysis they were 80% and 60%, respectively. Negative predictive val­ues were 84% for urine dipsticks and 93% for microscopic urinalysis. Positive predictive values were 52% and 61% for dipstick and micro­scopic urinalysis, respectively. Conclusion: The study has suggested that both urinalysis methods can be used for rapid diagnosis. Urine culture is an expensive test for routine use and should not be applied unless the result of the nitrite, leukocyte or bloods are positive in dipstick or leukocyte, erythrocyte or bacteria are positive in microscopic examination. Aim: Metabolic syndrome (MetS) is an important health problem of the modern age. It is a group of risk factors for diabetes mellitus and cardiovascular disease with unclear etiopathogenesis. Our aim in this study is to find the frequency and awareness level of metabolic syndrome in primary health care professionals in order to achieve success in early diagnosis and management. Methods: This cross-sectional descriptive study was carried out in primary health care professionals in eight city centers of Southeastern Anatolia. All participants gave written consent. They were examined for MetS using 2006 criteria of International Diabetes Federation (IDF) and they completed a questionnaire for awareness. SPSS (Statistical Package for Social Sciences) for Windows 21.0 software (SPSS, Chicago, IL, USA, version 21.0) was used for statistical analysis. Results: Of 493 participants, 304 (%61.7) were women and 189 (%38.3) were men and the mean age was 34.60±8.57 years. Of participants, 390 gave serum samples, 173 (%35.2) stated that they did not know their body mass indexes (BMI), 115 (%23.3) declared that they have never measured their waist circumference, 92 (%46.9) of 196 physicians stated that they have no post-graduate training about MetS, 168 (%85.7) stated that they do not know the formula of HOMAIR (Homeostatic Model Assessment - Insulin Resistance). MetS was detected in 76 (%19.5) out of 390 participants. The prevalence of metabolic syndrome was %39,4 in men while it was %8.7 in women. The mean age of participants with MetS was 40.28±8.70 years (35.60±1.67 in women, 42.20±1.15 in men). MetS was found to be four-times more common in men than in women (OR=3.86 and p<0.001). Depending on BMI value of > 30, MetS was found to be three-times more common in those with BMI value of >30 (OR=3.00 and p=0.007). Conclusion: The level of awareness for MetS among primary health care professionals should be improved. Every indivi-dual should be informed, examined and managed for MetS and obesity in primary care. Health Ministry, increasing the training program in our region and across Turkey or increasing the participation in training program organized by the Ministry will raise awareness about the metabolic syndrome. Amaç: Pemfigus vulgaris, otoimmün kökenli, deride bül oluşumuyla seyreden bir deri hastalığıdır. Bu çalışmada pemfigus vulgarisli hastalarda T helper 1 (Th1) ve T helper 2 (Th2) ve T helper 17 (Th17) tiplerindeki sitokinlerin düzeylerini araştırmayı ve sağlıklı kontrollerle karşılaştırmayı hedefledik. Gereç ve Yöntem: Çalışmaya 37 pemfigus vulgaris hastası ile 35 sağlıklı birey dahil edildi. Hastalar deri ve mukoza tutulum derecesine göre hafif, orta ve şiddetli hastalık olarak üç alt grupta sınıflandırıldı. Serum interlökin (IL) -2, IL-4, IL-6, IL-8, IL-10, IL-12, IL-17A ve interferon (IFN)-γ düzeyleri enzyme-linked immunosorbent assay yöntemiyle ölçüldü. Serum sitokin düzeyleri, hasta alt grupları arasında ve hastalar ile kontroller arasında SPSS 15.0 programı kullanılarak karşılaştırıldı. Bulgular: Serum IL-2, IL-4, IL-6 düzeyleri pemfigus vulgarisli hastalarda kontrollere göre anlamlı derecede yüksekti (sırasıyla p<0,001, p<0,001, p<0,001). IL-6 düzeyleri hastalık şiddeti ile koreleydi (r=0,490, p=0,003). Serum sitokin düzeyleri hastalık alt grupları arasına göre anlamlı değildi. Sonuç: Çalışmamızda pemfigus vulgariste Th2 sitokinlerdeki artışın ön planda olduğu saptanmıştır. Objective: Oxidative stress is characterized byan imbalance between production and destruction of reactive oxygen species. In our study, we aimed to determine the relationship between the oxidant-antioxidant balance in patients with showing some biochemical factors reflecting the severity of the disease and sleep parameters in patients with Obstructive Sleep Apnea Syndrome (OSAS). Methods: 93 patients were included to the study at the sleep centre, Department of Chest Diseases, Dicle University Hospital. All patients were subjected to all night polysomnography. Serum TOS and TAS levels were measured. Results: The mean age of patients was 47.4±7.2 years. There was not significant between patient and control groups in terms of age, gender, body mass index and frequency of symptoms (p> 0.05).The mean apnea-hypopnea index (AHI) was 25.1units/hour. We found these parameters significant compared with the control group; apnea-hypopnea index, arousal index, oxygen desaturation index, serum total oxidant and serum total antioxidant level (p<0.05). The mean serum levels TOS and TAS in patients with OSAS were found 34.2 mmol/L and 1.18 mmol/L, respectively. The mean serum levels TOS and TAS in control group were found 21.7 mmol/L and 1.54 mmol/L, respectively. Differences between two groups were significant (p<0.05). Background: The objective of this work is to evaluate plasma total antioxidant capacity (TAC), total oxidant status (TOS), and dehydroepiandrosterone sulphate (DHEA-S) levels in patients diagnosed with acute central serous chorioretinopathy (CSCR) and control samples. Methods: The TAC, TOS, and DHEA-S levels were assessed in the plasma of 46 CSCR patients and compared with 40 control samples. Results: The TAC level was 1.16 ± 0.08 and 1.20 ± 0.09 mmol Trolox eq./l; TOS level was 28.77 ± 33.33 and 19.95 ± 10.42 μmol H0/l; DHEA-S level was 203.79 ± 84.75 μg/dl and 249.36 ± 122.93 μg/dl in the CSCR group and in the control group, respectively. The plasma TAC and DHEA-S values were significantly lower in the CSCR group than in the control group ( p = 0.027 and p = 0.046, respectively). There was no significant difference between the CSCR and the control groups in terms of age, gender, and TOS levels ( p > 0.05). Conclusions: We demonstrated that the levels of plasma DHEA-S and antioxidative parameters were reduced in CSCR. Our results suggest that the antioxidant defense system may be inadequate or corrupted in CSCR. Reduced DHEA-S level is one of the factors that trigger this insufficiency. [ABSTRACT FROM AUTHOR]

Aim: The present study aimed to determine the levels of total cholesterol, triglycerides (TG), low-density lipoproteins (LDL), high-density lipoproteins (HDL), and vitamins B1 and B2 in intrahepatic cholestasis of pregnancy (ICP) patients, and to evaluate if these were involved in the pathophysiology of the disease. Methods: The prospective randomized study included 35 pregnant patients who were admitted to the Gynecology and Obstetrics Polyclinic, Faculty of Medicine at Dicle University and who were diagnosed with ICP (Group 1), and 40 healthy pregnant women who were admitted in the same period and who had no systemic diseases that might complicate the pregnancy during the pregnancy follow-up (Group 2). Serum lipid levels and vitamins B1 and B2 were determined and compared, and statistical comparisons of the groups were made. Results: There was no difference between the TG levels of the two groups (P = 0.631). Total cholesterol, LDL, HDL, and vitamin B1 and B2 levels were higher in Group 1 than in Group 2 (P = 0.001, P = 0.001, P = 0.001, P = 0.001, and P = 0.032, respectively). Conclusions: Increased levels of vitamins B1 and B2 may indicate a need for increased energy metabolism at the fetus. So we believe that new studies are required, which will be supported by the placental analyses of the pyruvate and lactate levels in maternal blood at delivery and fetal cord blood in order to develop a better understanding on the fetal effects of energy metabolism. Vascular endothelial growth factor (VEGF) is a molecule implicated in the pathogenesis of several eye diseases. In this experimental study, we planned to evaluate the effects of surgical menopause and hormone replacement therapy on VEGF levels. Thus, we studied the effects of treatments involving estrogen, estrogen and progesterone (E/P) in combination, and genistein, which is a selective estrogen modulator, on serum VEGF levels and the expression of VEGF in the retinas of rats with surgical menopause. The rats were randomly divided into five groups. Bilateral ovariectomy (OVX) was performed in all groups except for the shamoperated group. estrogen, E/P, genistein or water (sham and control groups) treatments were given for 8 weeks through the orogastric catheter. Serum VEGF level and immunohistochemical staining of VEGF in retinal tissue were analyzed in each group. Serum VEGF levels were significantly higher in the OVX + estrogen and OVX + genistein groups than in the control and sham groups. It was also higher in the OVX + E/P group than in the sham and control groups; however, in this case, the difference was not statistically significant. The OVX + estrogen, OVX + E/P, and OVX + genistein groups exhibited increased VEGF staining in comparison with the control and sham groups. However, the difference was not statistically significant. The current study demonstrated that estrogen replacement treatment (ERT) leads to an increase in serum VEGF levels and progesterone plays a protective role in this increase. Purpose: The objective of this study was to evaluate serum omentin, resistin and tumour necrosis factor-α (TNF-α) levels in Behcet disease (BD) patients with and without ocular involvement, as well as control subjects. Methods: Omentin, resistin and TNF-α levels were assessed in the plasma of 51 BD patients and compared with those of 24 control subjects. Results: The plasma resistin and TNF-α levels were significantly higher in the BD patients than in the control group ( p = 0.028 and p = 0.011, respectively), whereas the plasma omentin level was significantly lower in BD patients than in the control group ( p = 0.035). In the ocular BD, non-ocular BD and control groups, the omentin levels were 8.9 ± 4.65, 8.6 ± 3.61, and 12.4 ± 6.24 ng/mL; resistin levels were 0.29 ± 0.21, 0.24 ± 0.2 and 0.15 ± 0.45 ng/mL; and TNF-α levels were 25.45 ± 3.65, 24.03 ± 2.49 and 21.93 ± 4.86 ng/mL, respectively. Omentin/resistin and TNF-α/omentin ratios were more significant parameters in the demonstration of the differences in the groups; the former was lower and the latter was higher in the patient groups ( p = 0.001 and p = 0.002, respectively). Conclusions: We demonstrated that the plasma omentin level and omentin/resistin ratio were decreased, whereas the resistin and TNF-α levels and TNF-α/omentin ratio were increased in BD patients. Peroxisomes are involved in various metabolic reactions. Rhizomelic chondrodysplasia punctata (RCDP) type 1 is one of the peroxisomal biogenesis disorders caused by mutations in the PEX7 gene and is inherited in an autosomal recessive manner. We present a nine-year-old boy with skeletal abnormalities and dysmorphic facial appearance. The patient was born to parents who were first cousins. Very-long-chain fatty acids and pristanic acid levels were in the normal range, but an elevated phytanic acid level was detected by gas chromatography/mass spectrometry. The PEX7 gene was sequenced in the patient and his parents. A novel homozygous mutation, c.192delT (p.F64Lfs*10), was identified in the patient and was present in heterozygosity in both parents. In conclusion, the clinical presentation and peroxisome profile of the patient suggest that this novel mutation leads to RCDP type 1. [ABSTRACT FROM AUTHOR]

Objective This study was performed to investigate serum prolidase enzyme activity and oxidative stress in patients diagnosed with fibromyalgia (FM). Methods The study population consisted of 40 patients with a previous diagnosis of FM and 30 healthy subjects. We measured serum prolidase enzyme activity, total antioxidant status (TAS), total oxidative status (TOS), oxidative stress index (OSI), and paraoxonase-1 (PON-1) levels. Results On average, FM patients were diagnosed within 3.2 years of symptom onset, and patients had a mean of 14 tender points. There were no significant differences between patients and controls in age, body mass index, serum TAS, or PON-1 levels. However, patients with FM demonstrated higher serum prolidase activity, TOS, and OSI than the control group. Serum prolidase activity was positively correlated with serum TOS, OSI, and visual analog scale pain and fatigue scores. No correlation was found between serum prolidase activity and FM duration or the average number of tender points. Discussion Our results demonstrate a previously unreported association between serum prolidase enzyme activity and FM. Increased prolidase activity may contribute to the pathogenesis of FM, and measuring serum prolidase enzyme activity may be a useful FM biomarker. [ABSTRACT FROM AUTHOR]

Objectives: We aimed to evaluate antioxidant paraoxonase- 1 (PON-1) activity together with malondialdehyde (MDA) (an oxidative stress parameter) levels in patients with epilepsy. Materials and methods: Forty-five epilepsy patients were included in the study and compared with healthy controls (n = 45). The levels of serum MDA and PON-1 activities were measured by the Ohkawa method and the Eckerson method, respectively. Results: Serum MDA level was significantly higher (P = 0.015), whereas PON-1 activity was sigificantly lower (P = 0.011) in the patient group than the controls. Conclusions: Increased reactive oxygen species levels in epilepsy may result in a oxidative stress, which in turn could result in decreased antioxidant PON-1 activity and increased MDA levels. There is considerable evidence suggesting that cytokines play important roles in pain and in mediating neurovascular inflammation associated with migraine headaches. Although consensus exists to recommend topiramate (TPM) for migraine prevention, the mechanism of action in this regard is unknown. We measured serum interleukin-6 (IL-6) levels in 66 migraine patients. Of these patients, 23 (34.9%) were taking TPM for migraine, and 43 (65.1%) were not. The IL-6 levels were compared with those of healthy controls without migraine, from the population living in the same region. The mean IL-6 levels in migraine patients taking TPM and patients who did not were 67.06 ± 92.09 pg/mL and 44.09 ± 59.19 pg/mL, respectively ( P > 0.05). The IL-6 levels were higher in the patients taking TPM. The IL-6 level in the controls was 8.60 ± 7.36 pg/mL, which was significantly lower than the patient group using TPM ( P = 0.001). Our results show that, although IL-6 may be involved in pain induction or inflammatory mechanisms of migraine attacks, the serum IL-6 level was not reduced in migraine patients receiving TPM therapy. In conclusion, we found high IL-6 levels in migraine patients both with and without TPM therapy, suggesting that high IL-6 levels during pain-free periods could be a conditioning factor, making patients more vulnerable to pain attacks in chronic migraine. Objective: The purpose of this study was to investigate the effectiveness of honokiol, a natural molecule that was shown to have antioxidant effects, in prevention of intra-abdominal adhesion formation in a rat model. Methods: This study was conducted on a total of 40 non-pregnant Sprague-Dawley rats which were divided into 4 groups as follows: sham, control, saline and honokiol groups. Both uterine horns of the rats in control, saline and honokiol groups were exposed and a 2 cm segment of the antimesenteric surface of both uterine horns was traumatized by a scalpel. The saline group was administered 2 ml of saline/day intraperitoneally for 5 days following the operation. The honokiol group, on the other hand, was administered honokiol intraperitoneally at a dose of 1 mg/kg/day for 5 days following the operation. On postoperative day 14, 3 mL of intracardiac blood sample was taken from the rats for biochemical analyses, and the rats were sacrificed this way. From the blood samples Total antioxidant status (TAC) and Total oxidant status (TOS) levels were studied. The adhesions were evaluated according to the microscopic adhesions classification of Zuhlke et al. Skin tissues were also evaluated for the presence of inflammation, granulation tissue and fibrosis. Results: Adhesion and inflammation scores were all significantly lower in the honokiol group compared to the saline and control groups (p<0.008). Similarly, fibrosis score was significantly lower in the honokiol group compared to the saline group (p<0.008). However, in comparison of TAS and TOS levels; there was no significant difference between groups. Conclusion: Honokiol was found to be effective in prevention of intra-abdominal adhesion formation in a rat model. However, larger studies are needed to shed light on the exact role of honokiol in intra-abdominal adhesion formation and determine the molecular aspects of the promising results found in this study.
