The sungazer (Smaug giganteus) is a strict grassland specialist lizard endemic to South Africa's highveld grasslands. It is currently listed as Vulnerable (IUCN) and is primarily threatened by anthropogenic activities. Because sungazers are habitat specialists, climate change may be detrimental to the species, considering their life‐history traits, and the area of available suitable habitat. We assessed how climate change may impact the sungazers' geographic range by first producing an ecological niche model (ENM) for the species within a buffered region of its extent of occurrence (buffered EOO). The ENM was then projected to 2040, 2060, 2080 and 2100 under two climate change scenarios using Shared Socioeconomic Pathways (SSP); SSP245 (moderate‐case) and SSP585 (worst‐case). A mean ensemble of three global circulation models for each time period and scenario was used to create habitat suitability maps which were refined using a natural grassland variable overlay. Resulting maps were clipped to the sungazers' EOO and interpreted distribution. Within the interpreted distribution, models predicted an area of 10 198 km2 of current suitable habitat. At this scale, future habitat suitability is predicted to remain relatively stable (area: 9910 km2; 3% decline) under SSP245 by 2100. However, a 24% decline (area: 7705 km2) in habitat suitability was predicted under SSP585. Within the buffered EOO, habitat suitability increased in south‐western regions, which was more prominent under SSP585. Although this finding suggests that sungazers could track favourable conditions, their life history and low dispersal ability makes climate tracking unlikely. Because sungazers only occur in primary grasslands, regions dominated by agricultural activities, further land use developments are likely to affect the species survival. Thus, careful conservation management is essential, and we recommend the establishment of protected areas with cognizance of our predictions for current and future suitable habitat within the sungazers' interpreted distribution. Elucidating factors that drive microhabitat selection in a species is important for informing the conservation management of the species. For species that use microhabitats as long-term refuge sites, selection pressures are likely to be strong because the microhabitat must fulfil their eco-physiological and life history requirements. Smaug giganteus (Sungazer) is a threatened (Vulnerable) cordylid lizard species in the highveld grasslands of South Africa – a key agricultural area in the country. Individuals dig burrows as refuges, which increases their sensitivity to habitat transformation. In addition, previous attempts to translocate populations have failed due to a lack of knowledge about their specific microhabitat requirements. We assessed microhabitat characteristics that may influence burrow site selection by measuring vegetation characteristics surrounding Sungazer burrows and comparing these to vegetation at randomly selected sites. We also recorded orientation, aspect, soil composition, and the presence of other animal dung. Sungazers selected sites with sparse vegetation cover, and where grass height was short, but had no preference for soil composition. There was no relationship between the presence of ungulate dung and the placement of Sungazer burrows. Most Sungazer burrows were orientated in a northerly direction on north-facing slopes, suggesting that burrow placement has thermoregulatory advantages. We recommend that to identify suitable sites for Sungazer translocations, landscapes are thoroughly surveyed with cognisance to the findings in this study, and that a soft-release translocation protocol follows to maximise translocation success. Purpose: Both posture and loading rate are key factors in the herniation process and can determine the mechanism of disc failure. The aim of this study was to test the hypothesis that disruption visible with HR-MRI post-testing corresponds with microstructural features and further elucidate the mechanism by which this disruption weakens the disc. This will enable us to gain new insights into the herniation process. Methods: Thirty ovine lumbar spinal segments were subjected to combinations of four loading conditions (0–12° flexion, 0–9° lateral bending, 0–4° axial rotation, 0–1500 N axial compression) for 1000 loading cycles at 2 Hz in a dynamic disc loading simulator. The discs were scanned in an ultra-high field MRI (11.7 T) then examined using brightfield microscopy to examine their microstructure. Results: Four discs herniated and seven discs suffered nucleus displacement. These discs contained pre-existing defects in the central posterior annulus. Generally, following testing discs contained more posterior annulus disruption, Microstructural investigation revealed there was clear correspondence between HR-MRI and microstructural observations, and that the mid-outer annular-endplate junction had failed in all discs examined in this study. Conclusions: While all discs suffered outer annulus damage, only the discs containing pre-existing defects herniated. These pre-existing defects weakened the inner and mid annulus, allowing herniation to occur once the mid and outer annular wall was compromised. The objective of this study was to describe existing regulations of non-medical cannabis legalization in North America to inform recommendations for future health policy. These regulations are among the first in the world and will set a precedent for other jurisdictions globally who legalize cannabis. This was a review of online grey literature on regulatory approaches to non-medical cannabis legalization in North American jurisdictions. We conducted an internet search in June 2019 to identify government and public health resources published after January 1, 2012. We were able to achieve data saturation using a limited number of resources. Data extraction was conducted by two independent reviewers, with disagreements resolved by consensus. Eleven US states, the District of Columbia, and Canada have enacted legal recreational cannabis regulations. The legal age of cannabis possession matches the legal drinking age in all jurisdictions except one. Most consumption is in private residences only, with some provinces/territories permitting public consumption where tobacco is permitted. Most jurisdictions allow for home growing of up to 6 (US) or 4 (Canada) plants and a maximum possession of 1 oz. (US) or 1.06 oz. (Canada). Cannabis is available for purchase only in private retail stores in US states, while Canada has also legalized online sales. Impaired driving assessment is not cannabis-specific in most US states, while Canada has federal driving limits. Although North American approaches to regulating recreational cannabis use are consistent in many aspects, some exceptions exist (e.g., home growing, personal possession). More research is needed to assess the impact of variations in regulatory policies on potential harms from legalization to inform future policy decisions in North America and abroad. Background: Individuals with tuberous sclerosis complex (TSC) are at increased risk of developing self‐injurious behaviour. The persistence of this deleterious behaviour over years is reported in aetiologically heterogeneous samples to be between 60% and 80% but is unknown for TSC. Method: In this study, we determined the 3‐year persistence of self‐injury in a sample (n = 52) of children (with and without ID) and adults (with ID) with TSC and examined characteristics associated with persistence. Results: Findings for self‐injury were contrasted to those for aggression and property destruction to examine the specificity of results to this behaviour. Self‐injury was persistent in 84.6% of those with TSC who showed this behaviour, in contrast to 66.7% both for aggression and destruction. Persistent self‐injury was associated with poor self‐help skills, greater overactivity/impulsivity and more behavioural indicators of pain. These latter two characteristics were also associated with persistent aggression. No characteristics were associated with persistence of property destruction. Conclusion: These findings suggest that self‐injurious behaviours in individuals with TSC, together with aggressive and destructive behaviours, are highly persistent and would benefit from targeted intervention. Objectives: Clinical trials for progressive neurodegenerative disorders such as Alzheimer's Disease and Amyotrophic Lateral Sclerosis have been hindered due to the absence of effective pharmacodynamics markers to assay target engagement. We tested whether measurements of new protein production would be a viable pharmacodynamics tool for RNA‐targeted therapies. Methods: Transgenic animal models expressing human proteins implicated in neurodegenerative disorders – microtubule‐associated protein tau (hTau) or superoxide dismutase‐1 (hSOD1) – were treated with antisense oligonucleotides (ASOs) delivered to the central nervous system to target these human mRNA transcripts. Simultaneously, animals were administered 13C6‐leucine via drinking water to measure new protein synthesis after ASO treatment. Measures of new protein synthesis and protein concentration were assayed at designated time points after ASO treatment using targeted proteomics. Results: ASO treatment lowered hTau mRNA and protein production (measured by 13C6‐leucine‐labeled hTau protein) earlier than total hTau protein concentration in transgenic mouse cortex. In the CSF of hSOD1 transgenic rats, ASO treatment lowered newly generated hSOD1 protein driven by decreases in newly synthesized hSOD1 protein, not overall protein concentration, 30 days after treatment. At later time points, decreases in newly generated protein were still observed after mRNA lowering reached a steady state after ASO treatment. Interpretation: Measures of newly generated protein show earlier pharmacodynamics changes for RNA‐lowering therapeutics compared with total protein concentration. Early in ASO treatment, decreases in newly generated protein are driven by changes in newly synthesized protein. Body temperature (T b ) is one of the most influential factors affecting physiological processes in ectothermic animals. Reptiles use behaviours such as shuttling, and postural and orientation adjustments to ensure that T b remains close to a target. It is thought that target body temperature (T target ) is aligned to the physiological and behavioural thermal optima of the animal. The Sungazer ( Smaug giganteus ), a Threatened lizard species, is unique amongst the Cordylidae in that lizards inhabit self-excavated burrows in open grasslands, a habitat in which there are few exposed rock basking sites. In this situation, T b s are likely to be greatly influenced by postural and orientation adjustments during basking. We measured T b s using modified iButtons, and calculated T target of free-ranging Sungazers in order to assess the impact of body posture and orientation in the thermoregulation of the species. We used camera traps to record lizard behaviour at burrow entrances and aligned these measures to measures of T b . T target was 30 ± 1.4 °C, and T b s remained within this range for 371 ± 162 min per day. The anterior body-up postures resulted in faster heating and higher T b s. Lizards heated faster when facing away from the sun and spent proportionally more time in this orientation in the morning when T b s were lower than T target . Our findings suggest that their wide behavioural repertoire allows Sungazers to thermoregulate effectively over a broad range of environmental conditions and may thus be able to effectively respond to changing climatic conditions. Objective: Accumulation of misfolded superoxide dismutase‐1 (SOD1) is a pathological hallmark of SOD1‐related amyotrophic lateral sclerosis (ALS) and is observed in sporadic ALS where its role in pathogenesis is controversial. Understanding in vivo protein kinetics may clarify how SOD1 influences neurodegeneration and inform optimal dosing for therapies that lower SOD1 transcripts. Methods: We employed stable isotope labeling paired with mass spectrometry to evaluate in vivo protein kinetics and concentration of soluble SOD1 in cerebrospinal fluid (CSF) of SOD1 mutation carriers, sporadic ALS participants and controls. A deaminated SOD1 peptide, SDGPVKV, that correlates with protein stability was also measured. Results: In participants with heterozygous SOD1A5V mutations, known to cause rapidly progressive ALS, mutant SOD1 protein exhibited ~twofold faster turnover and ~ 16‐fold lower concentration compared to wild‐type SOD1 protein. SDGPVKV levels were increased in SOD1A5V carriers relative to controls. Thus, SOD1 mutations impact protein kinetics and stability. We applied this approach to sporadic ALS participants and found that SOD1 turnover, concentration, and SDGPVKV levels are not significantly different compared to controls. Interpretation: These results highlight the ability of stable isotope labeling approaches and peptide deamidation to discern the influence of disease mutations on protein kinetics and stability and support implementation of this method to optimize clinical trial design of gene and molecular therapies for neurological disorders. Pulp suspensions possess a yield stress that varies with concentration and fibre type, and understanding of this property is important to the pulp and paper industry. Unfortunately, pulp suspensions are highly susceptible to wall slip, which greatly complicates many rheological measurements including determination of the apparent yield stress. Using the vane method as a starting point, a slip-free method for measuring the apparent yield stress of pulp suspensions is developed. The findings from this method development are also used to assess the likelihood of slip having interfered with previous pulp rheometry studies and to offer some guidelines for the design of future experiments. Finally, the apparent yield stresses of suspensions of two National Institute of Standards and Technology reference pulps are determined for concentrations between 1% and 5% w/w, and the results contrasted with previous studies. The tensile strength behavior and recyclability of the paper prepared with the addition of polyamideamine-epichlorohydrin (PAE) were investigated. The dry and wet tensile strengths obtained with different PAE dosage were measured. The highest wet-to-dry strength ratio of 35% was obtained at 10 mg/g; above this addition level wet strength dropped slightly and then remained constant. The repulpability of strengthened paper was correlated directly with wet strength. The effect of electrolyte on tensile strength was also quantified by varying sodium chloride and calcium chloride concentration in the furnish stock. Without PAE, high salt concentrations (100 mM) reduced the tensile strength by 15-20%. At constant PAE addition level of 10 mg/g, low levels of salt addition (of either 10 mM NaCl or 10 mM CaCl2) slightly improved the strength; paper strength decreased at high salts concentrations. The cation valency and concentration in the process water were important variables which affected the efficiency of PAE. These results present the significance of developing sustainable wet strength agents that can be applied in demanding salty conditions while maintaining the product recyclability. Introduction: Organisms that produce extended-spectrum beta-lactamase (ESBL) are significant causes of infection among deployed service members. These specific organisms have increased resistance to several antibiotics, limiting the choice of therapy for the provider. Currently, the deployed microbiology lab uses, by default, the Siemens NBPC30 panel to identify and measure antibiotic susceptibility of gram-negative organisms. However, when an ESBL is suspected, additional confirmatory testing is performed, during which time the health care provider is forced to use broad spectrum antibiotics to protect the patient from infection. In this study, we evaluated the NBPC30 and NBC41 panels for their ability to rapidly and accurately detect ESBL-producing organisms. Methods: Identification and antimicrobial susceptibility testing of 79 strains of Enterobacteriaceae isolated from patients treated at Ibn Sina tertiary hospital (Baghdad) were performed using the NBPC30 and NBC41 panels. These results were confirmed using a Kirby-Bauer disk diffusion reference method described by the Clinical Laboratory Standards Institute. Sensitivities and specificities of the panels were determined in relation to this reference method. Results: Sensitivity and specificity of the NBC41 were 96.7% and 89%, while they were 86.7% and 72% for the NBPC30 panel. False positive and false negative rates were higher for the NBPC30 panel. Conclusion: Our data shows that the NBC41 panel is superior to the NBPC30 panel in rapidly identifying ESBLproducing organisms. Use of the NBPC41 panel decreases the turnaround time by 24 hours, allowing the provider to more accurately apply appropriate antibiotic therapy. Additionally, the NBPC41 panel provides more useful antibiotic susceptibility results compared to the NBPC30. The increased incidence of sexually transmitted infections has historically been associated with military personnel at war. The incidence of gonorrhea and Chlamydia in personnel deployed in the current wars in Iraq and Afghanistan has not been reported. An electronic records' review of testing done from January 2004 to September 2009 revealed higher rates of Chlamydia than gonorrhea, especially among females who deploy to Iraq. Additionally, increasing Chlamydia rates were noted over the study. Overall, the rates of gonorrhea and Chlamydia were the same or lower than age- and year-matched U.S. rates reported by the Center for Disease Control and Prevention. Ongoing education with emphasis on prevention and treatment are needed, as are development of specific projects to define the risk factors and timing of acquisition of sexually transmitted infections in combat zones. We synthesize peptide-functionalized nanoparticles by growing the peptide directly from the nanoparticles in a grafting-from process. We demonstrate the procedure by grafting a short, pH and oxidation responsive peptide sequence from 300 nm silica nanoparticles. The peptide allows destabilization of the particles in response to pH by neutralization of electrostatic charge, while manipulation of oxidizing conditions in the system offers the ability to select for irreversible, covalent bonding between the particles. In one system, we show the assembly of an asymmetrically functionalized set of particles, which may have applications in the formation of binary particle networks. The method of preparing peptide-coated particles should greatly simplify existing processes used to create peptide-functionalized nanoparticles. [ABSTRACT FROM AUTHOR]

Abstract: AVS1-P2 is a recently completed video compression standard developed by the Audio and Video Coding Standard (AVS) Workgroup of China. The standard promises comparable compression efficiency to the H.264/MPEG-4 AVC video codec with lower implementation complexity and royalty fees. AVS1-P2 is the Chinese next generation national video coding standard with an increasing amount of industrial importance as it is being required for different applications and services. It is expected that AVS1-P2 will be prominent in emerging Chinese digital video application markets, especially for personal video recorder (PVR) and video on demand (VOD). This paper discusses how trick modes for PVR and VOD can be performed with AVS1-P2 content. It begins with generating an index table to facilitate AVS1-P2 trick modes followed by a discussion of basic and advanced trick modes. Next, the minimum decoder speed and display frame buffer requirements are analyzed for smooth trick play, especially for 1× rewind. VOD stream delivery strategies for various trick modes and transitions are then discussed. Finally, an overview of other topics that may affect implementation such as content protection and encoding is provided. We have used solid-phase peptide synthesis to graft a peptide monolayer from a solid in order to modify the interfacial properties. We grafted a 15-residue peptide, EKEKEKEKEKEKEGG, containing a zwitterionic sequence of alternating lysine and glutamic acid residues from the surface of an aminosilanized silicon wafer by placing the silicon wafer within a commercial microwave peptide synthesizer. Such synthesizers are routinely used to make peptides on porous beads, but the peptides are subsequently cleaved and used independently of the solid support; our aim is to utilize the covalently bound peptide to control the surface properties without the need for cleavage and reattachment. We confirmed the presence of this peptide layer on the surface by X-ray photoelectron spectroscopy and ellipsometry. Atomic force microscopy was then used to study the forces between the peptide-modified surface and a borosilicate glass sphere as a function of the solution pH. The adsorbed peptide makes the silicon wafer pH responsive: at high pH the glass particle is repelled from the wafer, and at low pH it is attracted. Previous studies with synthetic polymers have shown that the “grafting from” method allows a much higher film density than “grafting to”. We propose that the application of grafting from strategies to peptide layers may offer three additional benefits: (1) the film density can be controlled independently of the primary sequence of the peptide, (2) the sequence constraints for spontaneous adsorption are removed, and (3) the procedure is fast and efficient, which may lead to lower costs and the ability for high-throughput surface biofunctionalization. Objective:To determine the frequency and risk factors for clinic and pharmacy use in preterm infants during the first year after neonatal intensive care unit (NICU) discharge.Study Design:We analyzed clinic visits and prescriptions in a cohort of 23 to 32 weeks infants. We constructed multivariable regression models to determine risk factors for high use.Result:The 892 preterm infants experienced 18 346 pediatric visits (mean 20 visits per infant per year) and filled 2100 prescriptions (mean 5.5 prescriptions per year among infants taking medications). Most visits were non-well child care visits to pediatric primary care providers. Prematurity and bronchopulmonary dysplasia (BPD) are important risk factors: infants at 23 to 26 weeks gestation or infants with BPD had an average 29 visits per year and 9 prescriptions per year among infants taking medication. However, half of the highest using infants were relatively healthy infants at 27 to 32 weeks gestation who escaped BPD, NEC or grade 3 to 4 intraventricular hemorrhage.Conclusion:Premature infants had frequent pediatric visits and prescription medications. We have measured the viscosity of suspensions of colloidal silica particles (d300 nm) and the properties of silica surfaces in solutions of a polymer consisting of zwitterionic monomer groups, poly(sulfobetaine methacrylate), polySBMA. This polymer has potential use in modifying surface properties because the polymer is net uncharged and therefore does not generate double-layer forces. The solubility of the polymer can be controlled and varies from poor to good by the addition of sodium chloride salt. Ellipsometry was used to demonstrate that polySBMA adsorbs to silica and exhibits an increase in surface excess at lower salt concentration, which is consistent with a smaller area per molecule at low salt concentration. Neutron reflectivity measurements show that the adsorbed polymer has a thickness of about 3.7 nm and is highly hydrated. The polymer can be used to exercise considerable control over suspension rheology. When silica particles are not completely covered in polymer, the suspension produces a highly viscous gel. Atomic force microscopy was used to show this is caused by bridging of polymer between the particles. At higher surface coverage, the polymer can produce either a high or very low viscosity slurry depending on the sodium chloride concentration. At high salt concentration, the suspension is stable, and the viscosity is lower. This is probably because the entrainment of many small ions renders the polymer film highly hydrophilic, producing repulsive surface forces and lubricating the flow of particles. At low salt concentrations, the polymer is barely soluble and more densely adsorbed. This produces less stable and more viscous solutions, which we attribute to attractive interactions between the adsorbed polymer layers. [ABSTRACT FROM AUTHOR]

We demonstrate that the absolute separation between an atomic force microscope (AFM) tip and a solid substrate can be measured in the presence of an irreversibly adsorbed polymer film. The separation is obtained from the analysis of a scattered evanescent wave that is generated at the surface of the solid. By comparing our scattering measurements to conventional AFM measurements, we also show an example where a conventional AFM measurement gives the incorrect force-distance profile. We validate the measurement of separation from scattering by examining the force-separation profile in the presence of surfactant solution. This validation is possible because the tip can be used to desorb the surfactant film that forms in surfactant solution, so we are able to measure both the scattering and the cantilever deflection when the tip is in contact with the solid substrate. The main limitation of our technique now is the lack of a rigorous method for predicting the intensity of scattering from the tip in contact with a solid that is coated with a film. Abstract: The performance of a real-time polymerase chain reaction (PCR) assay using the Smart Cycler instrument and a minor groove binding MGB Eclipse™ probe (Epoch Biosciences, Bothell, WA) for identification of Mycobacterium tuberculosis complex in acid-fast bacillus smear-positive and smear-negative clinical specimens was assessed by comparing results to the Amplified M. tuberculosis Direct Test (MTD) and mycobacterial culture plus clinical diagnosis. After initial testing, the overall sensitivity, specificity, and positive and negative predictive values of PCR for the 172 specimens submitted for mycobacterial culture were 86.3%, 100%, 100%, and 94.5%, respectively. These same values for MTD were 98.0%, 99.2%, 98.0%, and 99.2%. For 83 additional specimens, only MTD and PCR were performed; 5 specimens were positive and 78 were negative by both tests. The sensitivity of the PCR assay was improved by using different primers and probes. The time to a result for real-time PCR, starting with a decontaminated sample, was less than 3 h compared with 5–6 h for the MTD. [Copyright &y& Elsevier]

STUDY DESIGN.: Explant analysis for bone ingrowth of retrieved cervical disc prosthesis in chimpanzees and humans.OBJECTIVES.: To assess the bone ingrowth into retrieved Bryan Cervical Discs.SUMMARY OF BACKGROUND DATA.: Bone ingrowth in cervical disc prothesis has not been documented in the literature.METHODS.: Chimpanzee: Two chimpanzees underwent placement of the Bryan disc at C3–C4 and 3 months later had explantation and interbody fusion. Human: Two patients had removal of their Bryan disc and interbody fusion for failure to resolve symptoms at 8 and 10 months. The explants were analyzed for bone ingrowth.RESULTS.: Chimpanzee: Histologic analysis showed bony ingrowth through the interstices of the porous coating and apposition ranging from 10% to 50% of toluidine blue-stained sections. New ingrowth, rather than bony impaction, was confirmed with fluorochrome-labeled sections Human: Bone ingrowth was a mean of 30.1% (12% SD). No difference was observed between peripheral, intermediate, or central locations.CONCLUSIONS.: Adequate bony apposition was found in all primate device-to-vertebral body interfaces. Human retrievals also demonstrated significant ingrowth in all four surfaces. This compares with hip and knee arthroplasty percent ingrowth rates of 10% to 30%. Abstract: Bone sialoprotein (BSP) is a phosphorylated glycoprotein that is expressed almost exclusively in mineralizing connective tissues. In bone, expression of BSP correlates with the differentiation of osteoblasts and the onset of mineralization. To determine how the tissue- and differentiation-specific transcription of BSP is regulated, various lengths of promoter sequence were ligated to a luciferase reporter and stably transfected into a rat stromal bone marrow cell line, RBMC-D8 and undifferentiated C3H10T1/2 cells. Luciferase transcription of reporter constructs including 5.4 kb (mBSP5.4Luc) and 9.0 kb (mBSP9.0Luc) of the BSP promoter was strongly up-regulated in parallel with endogenous BSP mRNA in differentiating SBMCs, but not in C3H10T1/2 cells. In contrast, 0.1 kb and 1.4 kb BSP promoter constructs did not show selective expression. To determine tissue-specific expression in vivo, transgenic mice expressing reporter constructs for the 9.0 kb promoter and a 4.8 kb promoter lacking two upstream Cbfa1/Runx2 elements (mBSP9.0Luc and mBSP4.8Luc, respectively) were generated. Analysis of various tissues collected from 1-, 4-, 7-, 14-, and 42-day-old mice revealed extremely high levels of luciferase activity in calvaria, mandible, and tibia of the mBSP9.0Luc mice. In contrast, soft tissues showed negligible luciferase expression. Mice harboring the 4.8 kb transgene also showed selective luciferase expression but displayed a significantly lower activity in mineralized tissues. Northern hybridization of endogenous BSP mRNA and immunostaining of BSP in mBSP9.0Luc mice showed a temporo-spatial expression pattern consistent with the luciferase activity. These results indicate that regulatory elements within the 9.0 kb region of the promoter are required for strong, tissue- and differentiation-specific expression of BSP. [Copyright &y& Elsevier]

Abstract: Background:: Rapid diagnostic methods for respiratory syncytial virus are useful tools available for the clinician. Objectives:: The Thermo Electron™ RSV OIA® (optical immunoassay kit) was prospectively compared with direct immunofluorescent assay and viral culture at Primary Children''s Medical Center, Salt Lake City, Utah. Study design:: Specimens from three hundred and thirty patients exhibiting respiratory symptoms were collected for testing by the three methods above. Several specimens were positive by both OIA and DFA with a negative culture result. These culture results were verified by RT-PCR analysis. Results:: Overall, 107 specimens were positive for RSV by the reference tests (culture or RT-PCR). DFA analysis identified an additional 40 patient specimens positive for other respiratory viruses. Compared to the reference tests the sensitivity, specificity, positive, and negative predictive values of the OIA for detection of RSV were 87.9%, 99.6%, 98.9% and 94.5%, respectively. Conclusions:: The rapid OIA assay format proved to be cost effective, and simple to use in comparison to DFA and viral culture. The Binax™ NOW® immunochromatographic assay for respiratory syncytial virus was prospectively compared with direct fluorescent assay and viral culture at Primary Children''s Medical Center, Salt Lake City, Utah during February 2003. Three hundred ten patient specimens were collected for testing, of which 102 specimens were positive for respiratory syncytial virus by the reference tests, direct immunofluorescence assay (DFA), and culture or molecular analysis. DFA analysis identified an additional 40 patient specimens positive for other respiratory viruses. Compared to the reference tests, the sensitivity, specificity, and positive and negative predictive values of the rapid immunochromatographic assay for detection of respiratory syncytial virus were 89.2%, 100.0%, 100.0%, and 94.9%, respectively. This rapid assay format proved to be cost-effective and simple to use in comparison to DFA and viral culture. Negative rapid test results should still be confirmed with a secondary test. A common problem with many video transmission applications is the wide range of available bandwidths between the server and different clients. These environments require efficient multicast video service, the capability to transmit and receive the same video sequence at different resolutions. Two approaches to achieve multicast service are scalable coding (dependent bitstream coding) and simulcast coding (independent bitstream coding). One would expect scalable coding to have higher coding efficiency because a scalable coded bitstream can exploit similar information in another bitstream. This reasoning would suggest that multicast implementations should only use scalable coding for maximum coding efficiency. However, this article shows results where simulcast coding has been found to outperform spatial scalability (one type of scalable coding). In this article, methods are described to select between simulcast coding and spatial scalability for multicast video transmission. These techniques can be used to determine the proper multicast coding approach for providing service to clients with different communication links. The methodology described can also be used to construct decision regions to guide more general scenarios or adaptively switch between the two coding approaches. (1) Populations of flowering male and female shoots of Mercurialis perennis were grown in three experiments under different light screens: (i) the sexes grown together in a replacement series under a screen transmitting 11% incident light; (ii) the density of the plants varied under an 11% light screen; and (iii) the sexes grown separately with Holcus mollis under light screens transmitting 31% and 2% incident light. (2) In the replacement series with mixed sexes, the males grew more than the females at (male : female) 10 : 10 while the females grew more at 17 : 3 an 3 : 17. The sexes grew similarly in monoculture. The results for total yield of the plants suggest that competition between the sexes was less than that within them. (3) In the density experiment, the males grew more than the females as the density increased from 20 to 30 plants/box and both grew similarly at 42 plants/box. (4) It is suggested that observed uneven sex ratios in natural populations of shoots may be a result of the sexual differences in growth as the density of the plants increases. (5) In the replacement series with H. mollis under the 31% light screens the males grew more than the females at (M. perennis : H. mollis) 3 : 17 while the females grew more than the suggest that males may persist longer in association with other species. (6) No consistent results were obtained from the H. mollis replacement series under the 2% light screens. (1) Transplants of Mercurialis pernnis were grown under three canopy types; beech, mixed woodland and no overhead canopy (open), and in three different combinations under each canopy type; flowering males and females, flowering and non-flowering males and females, and flowering males and females with Holcus mollis. (2) The males grew more than the females under the open and beech canopies, while the females grew more under the mixed canopy. The growth of the sexes did not vary consistently between the three treatments. (3) The results may explain the observed predominance of male flowering shoots in canopy openings of a woodland, but do not explain the predominance of female shoots at the treebases. (4) Both sexes grew most with H. mollis under the canopy type where they grew most in monoculture. However, the results suggest that the differences between the growth of the sexes were increased by growing with H. mollis. (5) An estimate of the flowering shoot ratio was obtained, which showed that the ratio did not vary significantly with canopy type, suggesting that the differences in the numbers of flowering shoots of each sex at treebases and canopy openings in a wood land are not attributable to differences in the flowering to non-flowering soot ratio. The light climate was similar in both habitats in April but greater in the openings in August. (4) Removal of canopies of Pteridium aquilinum and Rubus fruticosus, above populations of M. perennis, resulted in a greater increase in the numbers of male shoots than of female shoots. (5) These results suggest that male and female plants differ in numbers, or growth, in different woodland microhabitats and that these differences are correlated with the above-ground environment (e.g. light). The development of cost effective models of hospital care and discharge planning for people with HIV is a vital policy issue. However, almost no data exist evaluating cost and quality differences in alternate hospital models of care. This empirical study retrospectively evaluates social work discharge planning for patients with HIV disease in two hospital care models: a cluster AIDS unit and general inpatient site beds. The independent effect of each hospital model of care on length of stay is assessed in a multivariate analysis, controlling for level of care needs and other social and clinical factors. Results reveal that the cluster AIDS unit, where a specialized AIDS social work staff works in collaboration with the interdisciplinary AIDS team, is associated with a significant reduction in hospital length of stay for persons with HIV disease and complex discharge planning needs. These results support the hypothesis that discharge planning services, performed by specialized social workers, are a cost effective investment for hospitals treating patients with complex chronic conditions, such as AIDS. Further research should be developed to systematically evaluate the cost effectiveness of hospital-based social workers, using prospective experimental designs, in order to establish the net impact of social work discharge planning services on patient and family outcomes and institutional and social costs. The article focuses on the need to establish guidelines on quality assurance (QA) to be used during deployment of U.S. military service members. It stresses the need for non-laboratory technicians to be fully trained and competence assessed before performing tests and reporting results. All medical treatment facilities are encouraged to re-evaluate their monthly standing orders with the U.S. Army Medical Material Center-Southwest Asia (USAMMC-SWA). The benefits of standardizing QA guidelines are also mentioned.

Neuroligins, postsynaptic cell adhesion molecules that are linked to neuropsychiatric disorders, are extensively studied, but fundamental questions about their functions remain. Using in vivo replacement strategies in quadruple conditional knockout mice of all neuroligins to avoid heterodimerization artifacts, we show, in hippocampal CA1 pyramidal neurons, that neuroligin-1 performs two key functions in excitatory synapses by distinct molecular mechanisms. N-methyl-D-aspartate (NMDA) receptor-dependent LTP requires trans -synaptic binding of postsynaptic neuroligin-1 to presynaptic β-neurexins but not the cytoplasmic sequences of neuroligins. In contrast, postsynaptic NMDA receptor (NMDAR)-mediated responses involve a neurexin-independent mechanism that requires the neuroligin-1 cytoplasmic sequences. Strikingly, deletion of neuroligins blocked the spine expansion associated with LTP, as monitored by two-photon imaging; this block involved a mechanism identical to that of the role of neuroligin-1 in NMDAR-dependent LTP. Summary: Microbial communities have an important role in natural ecosystems and have an impact on animal and human health. Intuitive graphic and analytical tools that can facilitate the study of these communities are in short supply. This article introduces Microbial Community Analysis GUI, a graphical user interface (GUI) for the R-programming language (R Development Core Team, 2010). With this application, researchers can input aligned and clustered sequence data to create custom abundance tables and perform analyses specific to their needs. Introduction: Although engagement is generally predictive of positive outcomes in technology-based behavioral change interventions, engagement measures remain largely atheoretical and lack treatment-specificity. This study examines the extent to which adherence measures based on the underlying behavioral change theory of an Acceptance and Commitment Therapy (ACT) app for smoking cessation predict smoking outcomes, and user characteristics associated with adherence.Methods: Study sample was adult daily smokers in a single arm pilot study (n=84). Using the app's log file data, we examined measures of adherence to four key components of the ACT behavior change model as predictors of smoking cessation and reduction. We also examined baseline user characteristics associated with adherence measures that predict smoking cessation.Results: Fully adherent users (24%) were over four times more likely to quit smoking (OR=4.45; 95% CI=1.13, 17.45; p=0.032). Both an increase in tracking the number of urges passed (OR=1.02; 95% CI=1.00, 1.03; p=0.043) and ACT modules completed (OR=1.27; 95% CI=1.01, 1.60; p=0.042) predicted cessation. Lower baseline acceptance of cravings was associated with over four times higher odds of full adherence (OR=4.59; 95% CI=1.35, 15.54; p=0.014).Conclusions: Full adherence and use of specific ACT theory-based components of the app predicted quitting. Abstract: The current study seeks to elucidate a biological mechanism which may mediate neuroinflammation, and decreases in both blood–brain barrier stability and neuron viability at the intracortical microelectrode-tissue interface. Here, we have focused on the role of pro-inflammatory reactive oxygen species. Specifically, adult rats implanted within intracortical microelectrodes were systemically administered the anti-oxidant, resveratrol, both the day before and the day of surgery. Animals were sacrificed at two or four weeks post-implantation for histological analysis of the neuroinflammatory and neurodegenerative responses to the microelectrode. At two weeks post-implantation, we found animals treated with resveratrol demonstrated suppression of reactive oxygen species accumulation and blood–brain barrier instability, accompanied with increased density of neurons at the intracortical microelectrode-tissue interface. Four weeks post-implantation, animals treated with resveratrol exhibited indistinguishable levels of markers for reactive oxygen species and neuronal nuclei density in comparison to untreated control animals. However, of the neurons that remained, resveratrol treated animals were seen to display reductions in the density of degenerative neurons compared to control animals at both two and four weeks post-implantation. Initial mechanistic evaluation suggested the roles of both anti-oxidative enzymes and toll-like receptor 4 expression in facilitating microglia activation and the propagation of neurodegenerative inflammatory pathways. Collectively, our data suggests that short-term attenuation of reactive oxygen species accumulation and blood–brain barrier instability can result in prolonged improvements in neuronal viability around implanted intracortical microelectrodes, while also identifying potential therapeutic targets to reduce chronic intracortical microelectrode-mediated neurodegeneration. Methicillin-resistant Staphylococcus aureus (MRSA) in health-care settings results in life-threatening infections. We examined the incidence of MRSA at the combat support hospital located at the Ibn Sina Hospital in Baghdad, Iraq. We compiled isolate data from 2005 to 2009 characterizing antibiotic susceptibilities, annual trends, patient populations, infection sites, and hospital locations. Approximately 46.1 % of S. aureus were MRSA, with increase in numbers of yearly isolates. MRSA was isolated in higher numbers from U.S. military personnel. Non-U.S. patient isolates displayed higher antibiotic susceptibility compared to U.S. military personnel isolates. Outpatient clinic, forward operating bases, and intermediate care ward 1 isolated the most MRSA. Common isolation sites were wound and skin cultures. Community-acquired MRSA was likely present in 291 out of 303 isolates based on antibiotic susceptibility. Our data suggests that most MRSA were community-acquired with limited nosocomial spread. Background In the UK, the closure of 'long-stay' hospitals was accompanied by the development of community teams (CTs) to support people with intellectual disabilities (IDs) to live in community settings. The self-reported experiences of staff working in such teams have been neglected. Methods Focusing on a single county-wide service, comprising five multi-disciplinary and inter-agency CTs, we measured perceptions among the health care and care management Team members of (1) their personal well-being; (2) the functioning of their team; and (3) the organisation's commitment to quality, and culture. Results Almost three-quarters of the questionnaires were returned (73/101; 72%). The scores of health care practitioners and care managers were very similar: (1) the MBI scores of more than half the respondents were 'of concern'; (2) similarly, almost four in ten respondents' scores on the Vision scale of the TCI were 'of concern'; (3) the perceived commitment to quality (QIIS-II Part 2) was uncertain; and (4) the organisational culture (QIIS-II, Part 1) was viewed as primarily hierarchical. Discussion The perceived absence of a vision for the service, combined with a dominant culture viewed by its members as strongly focussed on bureaucracy and process, potentially compromises the ability of these CTs to respond proactively to the needs of people with IDs. Background: Colorectal cancer (CRC) remains the most commonly diagnosed cancer among Korean Americans (KAs) in part due to low screening rates. Recent studies suggest that some KA patients engage in medical tourism and receive medical care in their home country. The impact of medical tourism on CRC screening is unknown. The purpose of this paper was to 1) investigate the frequency of medical tourism, 2) examine the association between medical tourism and CRC screening, and 3) characterize KA patients who engage in medical tourism. Methods: This is a community-based, cross-sectional study involving self-administered questionnaires conducted from August 2013 to October 2013. Data was collected on 193 KA patients, ages 50-75, residing in the Seattle metropolitan area. The outcome variable is up-to-date with CRC screening, defined as having had a stool test (Fecal Occult Blood Test or Fecal Immunochemical Test) within the past year or a colonoscopy within 10 years. Predictor variables are socio-demographics, health factors, acculturation, knowledge, financial concerns for medical care costs, and medical tourism. Results: In multi-variate modeling, medical tourism was significantly related to being up-to-date with CRC screening. Participants who engaged in medical tourism had 8.91 (95% CI: 3.89-23.89) greater odds of being up-todate with CRC screening compared to those who did not travel for healthcare. Factors associated with engaging in medical tourism were lack of insurance coverage (P = 0.008), higher levels of education (P = 0.003), not having a usual place of care (P = 0.002), older age at immigration (P = 0.009), shorter years-of-stay in the US (P = 0.003), and being less likely to speak English well (P = 0.03). Conclusions: This study identifies the impact of medical tourism on CRC screening and characteristics of KA patients who report engaging in medical tourism. Healthcare providers in the US should be aware of the customary nature of medical tourism among KAs and consider assessing medical tests done abroad when providing cancer care. [ABSTRACT FROM AUTHOR]
