Tongue injuries in children vary from slight lacerations to complete amputation. It could be due to falls, self-mutilation, epilepsy, child abuse, or sports injuries. The anterior dorsum is the most common site, followed by the mid dorsum and anterior ventral part. These injuries may cause dysfunction of the tongue, hemorrhage, and disfigurement. Management of injuries of the tongue in young children is challenging. There are no established guidelines for managing tongue lacerations. This is a case report of a 3-year-old girl with a laceration on the anterolateral part of the tongue with nonapproximating wound edges. The laceration extended from the dorsal to the ventral part. The parents were apprehensive for the surgical management of the injury due to the age of their daughter. Hence, healing of the injury with conservative management without suturing was achieved. The management of traumatic injuries of the tongue in children is difficult due to their age and cooperation. The pediatrician has to decide whether to heal the tongue by primary or secondary intention. Our decision to allow the healing by secondary intention was successful. The tongue healed without any complications, barring a small scar, with satisfactory esthetic and functional outcome. Both the parents and the child were satisfied with the management of the injury. Children with tongue injuries often visit the pediatric emergency department. The vast majority of cases can be conservatively treated, while some injuries require operative repair. The aim of this article was to demonstrate a "back-to-basics" approach to a refractory bifid tongue injury in a toddler. A 1-year-old toddler with a tongue injury was unsuccessfully treated three times within a week by a surgeon in another specialty; all reconstructions were mucosal only. The case was then referred to our maxillofacial unit for proper management. On examination, the recurrent injury seemed to occur because of the patient's self-biting habit. We performed the fourth reconstruction of the tongue muscles and mucosa, and because of no dental prosthetic laboratory available in our hospital, we used transparent adhesive drapes fixed by resorbable sutures to cover the patient's anterior teeth instead of bite guards. The toddler was fed via a nasogastric tube for 1 week under 2-day antibiotic prophylaxis and routine oral care. The patient was discharged without any complications 1 week later. The causes of repeated orofacial injuries should be identified and require particular attention to establish a proper treatment. For intraoral injuries in pediatric patients, self-biting habits should not be overlooked. Abstract Objectives Chronic injury in tongue causes the variety of reactions in the oral cavity, frequently leading to its functional and structural disintegrity including inflammation and sensory dysfunction, but its detailed profiles were not elucidated yet. One of the chronically injured tongue such as tongue piercing, as a pathological aspect, is currently popular among younger people but may be associated with severe side effects, leading to pathophysiological complications. However, the pathophysiological aspects and related cellular and molecular mechanisms underlying tongue injury are not clearly understood. Design In this study, we designed an experimental model system using C57BL/6 male mice that mimics a chronically injured situation by penetrating the middle part of tongue with silk suture. After 5 and 10 days mice were sacrificed and tongues were collected and processed for histological evaluation and immunohistochemistry. Results We found that the anterior tongue showed localization of neuro-inflammatory signaling molecules such as myeloperoxidase (MPO), matrix metalloproteinase 2 (MMP2), tumor necrosis factor-α (TNF-α), nerve growth factor, and transient receptor potential cation channel subfamily V member 1 (TRPV1) without any apparent inflammation in temporal manner. In addition, the signal for AM1-43, an activity-dependent nerve terminal probe, decreased within the fungiform papillae on the anterior tongue after injury. Conclusions These results implied that the distinct localizations of inflammatory cytokines and neurotrophin would contribute altered sensory function in anterior tongue following the chronic injury. Our study indicates the possible pathophysiologic mechanism underlying neuro-inflammation following chronically injury of tongue. A buffalo with history of fighting among fellow animals was presented with dysphagia, profuse salivation and bleeding from mouth. Clinical examination revealed extensive tongue lacerations. The oral cavity was cleaned thoroughly and lacerations were sutured. Another case of severe tongue laceration in a newborn Jersey calf due to faulty snare application to relieve dystocia was treated successfully by suturing. Both animals recovered uneventfully.
