BACKGROUND: Both single and bilateral lung transplantation are recognised options for patients who have end-stage chronic obstructive pulmonary disease (COPD); however, which procedure leads to longer survival remains unclear. We aimed to compare survival after each procedure by analysing data from the registry of the International Society for Heart and Lung Transplantation. METHODS: We analysed data for 9883 patients with COPD, 3525 (35.7%) of whom underwent bilateral lung transplantation, and 6358 (64.3%) single lung transplantation, between 1987 and 2006. We accounted for possible selection bias with analysis of covariance, propensity-score risk adjustment, and propensity-based matching. FINDINGS: Median survival after either type of lung transplantation for patients with COPD was 5.0 years (95% CI 4.8-5.2). Survival for patients who had lung transplantation before 1998 was 4.5 years (4.3-4.8), compared with 5.3 years (5.0-5.5) for those who had it after 1998 (p<0.0001). The proportion of patients who had bilateral lung transplantation increased from 101/467 (21.6%) in 1993 to 345/614 (56.2%) in 2006. Median survival time after bilateral lung transplantation was longer than that after single lung transplantation: 6.41 years (6.02-6.88) versus 4.59 years (4.41-4.76) (p<0.0001). Pretransplant characteristics of patients who had single and bilateral lung transplantation differed, but whichever method was used to adjust for baseline differences, bilateral lung transplantation was associated with longer survival than was single lung transplantation; the hazard ratio ranged from 0.83 (0.78-0.92) for analysis of covariance to 0.89 (0.80-0.97) for propensity-based matching. However, bilateral lung transplantation had little benefit compared with single lung transplantation for patients who were 60 years and older (HR 0.95; 0.81-1.13). Background: Although lung retransplantation is the only definitive therapeutic option in lung recipients with bronchiolitis obliterans syndrome (BOS), its value remains a considerable source of controversy. We report our experience of retransplantation for BOS performed in our center since 1988.Methods: Between 1988 and 2002, 15 lung retransplantations for BOS were performed. Patient survival, causes of death, long-term functional status, and BOS recurrence rate were reviewed.Results: The retransplantation procedure was single-lung transplantation (SLT) in all cases (ipsilateral SLT, n = 4; contralateral SLT, n = 9; SLT after previous double-lung transplantation, n = 2). The median time between primary lung transplantation and retransplantation was 31 months (range, 12 to 39 months). The median follow-up duration of the 10 patients surviving beyond 6 months was 49.5 months (range, 16.5 to 105 months), and 5 patients were followed up for > 5 years. Actuarial survival rates at 1 year, 2 years, and 5 years after retransplantation were 60%, 53%, and 45%, respectively. Ten patients died during long-term follow-up, 6 of them from infection (60%). The retained graft was the initial site of the fatal infection in four of these six patients (66%). Two other patients with a retained graft experienced disabling chronic purulent expectoration arising from the old graft. In the 10 patients surviving beyond 6 months, mean best FEV(1) was 58 +/- 13% of predicted (+/- SD), and actuarial freedom from BOS (stage 1, 2, or 3) at 1 year, 3 years, and 5 years was 90%, 72%, and 66%, respectively.Conclusion: Lung retransplantation offered a viable therapeutic option for selected lung recipients with BOS. Objectives: We sought to assess the efficacy and safety of thrombolytic therapy in patients with an acute pulmonary embolism (PE).Background: Thrombolytic therapy is approved for the treatment of acute PE; however, the safety and efficacy of this therapy remain debated.Methods: A meta-analysis of randomized, controlled trials comparing thrombolytic agents with intravenous heparin in patients with acute PE was performed. Trials were identified through a combined search of the MEDLINE, EMBASE, and Current Contents databases. Three outcome measures were assessed: 1) mortality, 2) recurrence of PE, and 3) major hemorrhage.Results: Nine trials including 461 patients were identified. Compared with intravenous heparin, thrombolytic therapy had no significant effect on mortality (relative risk [RR] 0.63, 95% confidence interval [CI] 0.32 to 1.23) or the recurrence of PE (RR 0.59, 95% CI 0.30 to 1.18), but was associated with an increased risk of major hemorrhage (RR 1.76, 95% CI 1.04 to 2.98). These results were homogeneous and largely unaffected by the formulation of thrombolytic agent, the clinical severity of PE, the extent of vascular obstruction determined radiologically, or the methodologic quality of the included trials.Conclusions: Compared with intravenous heparin, thrombolytic therapy does not appear to have therapeutic benefit in unselected patients with acute PE, but it is associated with an increased risk of major hemorrhage. Given the small number of patients included in the randomized trials thus far, the negative results in terms of the efficacy outcomes should be interpreted with caution. Study Objectives: To assess incidence, outcome, and early predictors of mortality for patients with primary graft failure (PGF) following lung transplantation (LTx), and to develop an injury severity score able to accurately predict ICU mortality for these patients.Design: Retrospective cohort analysis.Setting: Two LTx centers in Paris.Patients: Two hundred fifty-nine patients who underwent LTx over a 12-year period.Measurements and Results: One hundred thirty-one patients (50.6%) met PGF criteria: radiographic graft infiltrate within the first 3 days following LTx associated with gas exchange impairment (PaO(2)/fraction of inspired oxygen ratio < 300 mm Hg). This syndrome was associated with an increased duration of mechanical ventilation (9.1 +/- 1 days vs 3.1 +/- 0.6 days, mean +/- SD; p < 0.001) and ICU mortality (29% vs 10.9%; p < 0.01). The patients with PGF were randomly assigned to developmental (n = 85) and validation (n = 46) samples. Using logistic regression analysis, four variables were found associated with ICU mortality in these patients: age, degree of gas exchange impairment, graft ischemic time, and severe early hemodynamic failure. An ischemia/reperfusion injury severity score was derived using these four variables. Model calibration was good in the developmental and validation samples, as was model discrimination (area under receiver operating characteristic curves, 0.93 and 0.85, respectively).Conclusion: PGF following LTx is a frequent event, with significant ICU morbidity and mortality. BACKGROUND: The incidence of asymptomatic pericardial effusion is high after cardiac surgery. Nonsteroidal anti-inflammatory drugs (NSAIDs) are widely prescribed in this setting, but no study has assessed their efficacy. OBJECTIVE: To assess whether the NSAID diclofenac is effective in reducing postoperative pericardial effusion volume. DESIGN: Multicenter randomized, double-blind, placebo-controlled study. (Clinical trials.gov registration number: NCT00247052) SETTING: 5 postoperative cardiac rehabilitation centers. PATIENTS: 196 patients at high risk for tamponade because of moderate to large persistent pericardial effusion (grade 2, 3, or 4 on a scale of 0 to 4, as measured by echocardiography) more than 7 days after cardiac surgery. INTERVENTION: Random assignment at each site in blocks of 4 to diclofenac, 50 mg, or placebo twice daily for 14 days. MEASUREMENTS: The main end point was change in effusion grade after 14 days of treatment. Secondary end points included frequency of late cardiac tamponade. RESULTS: The initial mean pericardial effusion grade was 2.58 (SD, 0.73) for the placebo group and 2.75 (SD, 0.81) for the diclofenac group. The 2 groups showed similar mean decreases from baseline after treatment (-1.08 grades [SD, 1.20] for the placebo group vs. -1.36 (SD, 1.25) for the diclofenac group). The mean difference between groups was -0.28 grade (95% CI, -0.63 to 0.06 grade; P = 0.105). Eleven cases of late cardiac tamponade occurred in the placebo group and 9 in the diclofenac group (P = 0.64). These differences persisted after adjustment for grade of pericardial effusion at baseline, treatment site, and type of surgery. LIMITATION: The sample was not large enough to find small beneficial effects of diclofenac or assess the cardiovascular tolerance of diclofenac. CONCLUSION: In patients with pericardial effusion after cardiac surgery, diclofenac neither reduced the size of the effusions nor prevented late cardiac tamponade. Introduction: Among risk factors for the progression of bronchiolitis obliterans syndrome (BOS) after lung transplantation (LT), the influence of time to BOS onset is not known. The aim of the study was to assess if BOS occurring earlier after LT is associated with worse functional prognosis and worse graft survival.Method: We retrospectively compared functional outcome and survival of all single-LT (SLT) recipients who had BOS develop during follow-up in our center according to time to onset of BOS (< 3 years or > or = 3 years after transplantation).Results: Among the 29 SLT recipients with BOS identified during the study period, 20 patients had early-onset BOS and 9 patients had late-onset BOS. The mean decline of FEV(1) over time during the first 9 months in patients with early-onset BOS was significantly greater than in patients with of late-onset BOS (p = 0.04). At last follow-up, patients with early-onset BOS had a lower mean FEV(1) value (25% vs 39% of predicted, p = 0.004), a lower mean PaO(2) value (54 mm Hg vs 73 mm Hg, p = 0.0005), a lower 6-min walk test distance (241 m vs 414 m, p = 0.001), a higher Medical Research Council index value (3.6 vs 1.6, p = 0.0001), and a higher percentage of oxygen dependency (90% vs 11%, p = 0.001) compared with patients with late-onset BOS. In addition, graft survival of patients with early-onset BOS was significantly lower than that of patients with late-onset BOS (log-rank test, p = 0.04). There were 18 of 20 graft failures (90%) in the early-onset BOS group, directly attributable to BOS in all cases (deaths [n = 10] or retransplantation [n = 8]). In the late-onset BOS group, graft failure occurred in four of nine patients due to death from extrapulmonary causes in three of four cases. The median duration of follow-up after occurrence of BOS was not statistically different between patients with early-onset BOS and patients with late-onset BOS (31 +/- 28 months and 37 +/- 26 months, respectively; p = not significant).Conclusion: The subgroup of patients who had BOS develop > or = 3 years after SLT are less likely to have worrisome functional impairment develop in long-term follow-up. Considering the balance between the advantages and risks, enhancement of immunosuppression should be regarded with more caution in this subgroup than in patients with early-onset BOS. Les données de vraie vie sur l'utilisation des CO dans des populations nationales font actuellement défaut. L'objectif de cette étude était d'analyser l'utilisation en vie réelle des CO chez les patients français atteints d'un LES, à partir des données médico-administratives du Système national des données de santé (SNDS). Cette étude a utilisé le SNDS, qui contient les données pseudonymisées de plus de 66 millions de personnes. Les patients inclus dans l'étude étaient atteints d'un LES, identifiés par le code CIM-10 M32 (LES) documenté dans le cadre d'une affection de longue durée (ALD) et/ou au cours d'une hospitalisation, vivants au 1er janvier 2020. Les comorbidités du LES et les complications des CO ont été identifiées à l'aide d'algorithmes validés. Les traitements ont été identifiés par les délivrances de médicaments en pharmacie et les doses journalières de CO (exprimées en équivalent-prednisone) ont été calculées pour l'année 2019. Ces données ont été comparées à celles d'un groupe de témoins sans SLE issus de la population générale, appariés sur l'âge et le sexe. Au total, 31 852 patients avec un LES (âge moyen 49,7 ans, 86,3 % de femmes, ancienneté de la maladie 7,1 ans) ont été inclus. Parmi ces patients, 48,2 % étaient traités par CO. La dose quotidienne moyenne de CO était inférieure à 5 mg/j chez 35,8 %, comprise entre 5 et 7,5 mg/j chez 6,4 % et supérieure à 7,5 mg/j chez 6,0 %. L'utilisation de CO était significativement plus fréquente chez les femmes (p < 0,0001) et diminuait avec l'âge (p < 0,0001). Par ailleurs, 14,2 % des patients recevant 5 à 7,5 mg/j de CO et 13,2 % de ceux recevant plus de 7,5 mg/j n'étaient pas traités par antipaludéens, immunosuppresseurs ou traitement biologique pour leur LES. Les manifestations spécifiques du LES, notamment les atteintes rénales, l'atteinte cutanée, la polyarthrite, la pleurésie, étaient significativement plus fréquentes chez les patients ayant reçu des doses plus importantes de CO (p < 0,0001 pour tous). Les complications des CO, notamment les maladies cardiovasculaires, les infections, l'ostéoporose et l'obésité, étaient significativement plus fréquentes chez les patients atteints de LES recevant ≥ 5 mg/j de CO (p < 0,0001 pour tous). D'un point de vue sociétal, le coût annuel moyen global des consommations de soins en 2019 était de 5894 € pour les patients atteints de LES et de 2601 € pour les témoins appariés (p < 0,0001). Parmi les patients atteints de LES, le coût augmentait de manière significative en fonction de la dose quotidienne de CO : 4477 € pour les patients sans CO, 6247 € pour une dose de CO < 5 mg/j, 9571 € pour CO de 5–7,5 mg/j et 13 399 € pour CO ≥ 7,5 mg/j. À notre connaissance, il s'agit de la première étude nationale portant sur l'utilisation en vie réelle des CO chez les patients atteints de lupus érythémateux disséminé. La proportion de patients traités avec des doses élevées de CO ≥ 7,5 mg/j reste inacceptable et associée à une fréquence significativement accrue des principales complications liées aux CO et à une augmentation significative des coûts des soins de santé. À ce jour, peu d'études ont évalué l'épidémiologie du LES à l'échelle nationale. L'objectif de cette étude était d'actualiser les données de prévalence et d'incidence du LES en France, à partir des données médico-administratives du système national des données de santé (SNDS). Cette étude a utilisé le SNDS, qui contient les données pseudonymisées de plus de 66 millions de personnes. Les patients inclus dans l'étude étaient atteints d'un LES, identifiés par le code CIM-10 M32 (LES) documenté dans le cadre d'une affection de longue durée (ALD) et/ou au cours d'une hospitalisation. Deux sous-populations ont été considérées : la population prévalente constituée des patients vivants au 01/01/2020 et une population incidente constituée des patients nouvellement diagnostiqués en 2017 et 2018. Au 01/01/2020, un total de 31 852 cas prévalents de LES a été identifié, soit un taux de prévalence brut de 47,4 cas pour 100 000. La majorité des patients étaient des femmes (86,3 %). L'âge moyen était de 49,2 (± 15,8) ans chez les femmes et de 52,5 (± 15,9) ans chez les hommes. L'ancienneté moyenne de la maladie était de 7,2 (± 6,3) ans chez les femmes et de 6,3 (± 5,6) ans chez les hommes. La prévalence maximale était observée chez les femmes âgées de 40 à 49 ans (142,3/100 000) et chez les hommes âgés de 60 à 69 ans (23,7/100 000) (tableau). Les taux de prévalence bruts par région en France métropolitaine étaient compris entre 32,4/100 000 en Pays de la Loire et 75,8/100 000 en Corse. Ces taux étaient plus élevés en Martinique (108,4/100 000) et en Guadeloupe (104,9/100 000). Parmi l'ensemble des cas prévalents, 10,2 % et 0,3 % bénéficiaient de la CMUc et de l'AME, respectivement. Un total de 2 971 et 2 695 cas incidents de SLE ont été identifiés respectivement en 2017 et 2018, soit des taux d'incidence bruts de 4,4/100 000 en 2017 et 4,0/100 000 en 2018. Parmi les patients incidents en 2018, la majorité était des femmes (83,9 %). Le pic d'incidence en 2018 a été observé chez les femmes âgées de 30 à 39 ans (11,6/100 000) et chez les hommes âgés de 50 à 59 ans (2,5/100 000). Les taux d'incidence bruts par région en France métropolitaine en 2018 étaient compris entre 2,6/100 000 en Pays de la Loire et 5,6/100 000 en Corse. Ces taux étaient plus élevés en Guadeloupe (8,5/100 000) et en Martinique (8,4/100 000). Parmi l'ensemble des cas incidents en 2018, un total de 12,7 % et 0,5 % bénéficiaient de la CMUc et de l'AME, respectivement. Anifrolumab est un anticorps monoclonal humain de type immunoglobuline G1 kappa qui se lie à la sous-unité 1 du récepteur à l'interféron de type I (IFNAR1). Il est indiqué en traitement additionnel chez les patients adultes atteints d'un lupus érythémateux systémique (LES) modéré à sévère, actif avec présence d'auto-anticorps malgré un traitement standard. L'objectif de l'analyse présentée est de décrire les caractéristiques et l'évolution des patients ayant bénéficié d'anifrolumab 300 mg, (solution à diluer pour perfusion ; une injection toutes les 4 semaines) dans le cadre d'une autorisation temporaire d'utilisation de cohorte (ATUc). Anifrolumab a été utilisé dans le cadre d'une ATUc, octroyée par l'ANSM en juin 2021 dans l'indication suivante : « en traitement de fond additionnel chez les patients adultes atteints d'un LES insuffisamment contrôlé (ou intolérant) malgré un traitement optimal avec les biothérapies actuellement disponibles ». Cette ATUc a pris fin en mai 2022. Les données présentées sont issues des fiches de collecte de données dans le cadre du protocole d'utilisation thérapeutique et de recueil d'information. Au total, 26 patients ont été inclus dans cette ATUc. Selon les définitions de l'ANSM, 23 d'entre-deux sont considérés comme « traités » (traitement fourni), et 7 comme « exposés » (réception d'au moins une fiche de suivi contenant une date d'administration). Pour les autres patients « traités », aucune fiche de suivi n'a été complétée. L'âge moyen des 23 patients traités est de 40,2 ans (écart-type [ET] 9,9 ans). Il s'agit de 21 femmes et 2 hommes, avec un poids moyen de 66,3 kg (ET 18,0 kg). L'ancienneté du diagnostic de LES est en moyenne de 13,9 ans (ET 8,2 ans). Il s'agit pour les 23 patients d'un LES modéré à sévère actif, avec un score SELENA–SLEDAI moyen de 9,7 (ET 5,3 ; n = 21) et un score PGA moyen de 2,3 (ET 0,7 ; n = 15). En moyenne, à l'inclusion, les patients présentent 6,1 articulations douloureuses (ET 5,9 ; n = 16) et 1,9 articulations gonflées (ET 2,9 ; n = 15). Le score CLASI-A moyen est de 11,9 (ET 8,2 ; n = 14). La durée préalable moyenne de traitement par bélimumab était de 13,4 mois (ET 13,8). Les caractéristiques des 7 patients exposés sont similaires à celles des patients traités. La durée moyenne d'exposition au traitement est de 5,7 mois (ET 3,3). Après un mois de traitement, une amélioration clinique des 7 patients exposés a été observée. Le nombre d'articulations douloureuses chez les patients exposés est passé de 6,0 (ET 7,7) en moyenne à 1,2 (ET 1,3) ; de même le nombre d'articulations gonflées est passé de 1,8 (ET 3,5) en moyenne à 0,3 (ET 0,8). Le score d'activité de la maladie (SELENA-SLEDAI) s'est amélioré en passant de 10,3 (ET 5,9) à 5,4 (ET 3,6). L'activité globale de la maladie évaluée par le clinicien à l'aide du PGA a diminué de moitié en passant de 2,0 (ET 0,4) à 1,0 (ET 0,7). Concernant les atteintes cutanées (CLASI-A), une amélioration a été observée ; le score est passé de 20,3 (ET 4,6) à 12,0 (ET 7,4). Ces bénéfices cliniques se maintiennent au cours du temps. Sur l'ensemble des patients traités (n = 23), 6 cas comprenant 16 suspicions d'effets/évènements indésirables, n'ayant pas entraîné l'arrêt du traitement, ont été déclarés pour 6 patients. Sur l'ensemble des cas rapportés, 2 ont été considérés comme graves et non reliés à anifrolumab : coxalgie droite aiguë et suspicion d'herpès. Aucun cas ne comprenait des évènements d'évolution fatale ou avec mise en jeu du pronostic vital. Du fait du faible de nombre de patients, des conclusions limitées peuvent être tirées de ces données de première utilisation d'anifrolumab 300 mg, solution à diluer pour perfusion. Background Multidrug-resistant ( MDR) bacteria are a growing concern worldwide. The aim of this study was to describe the epidemiology and risk factors of MDR bacteria detected in respiratory invasive samples during hospitalization in the intensive care unit ( ICU) after lung transplantation ( LT). Methods This study was based on a retrospective analysis of 176 patients hospitalized in the ICU after LT in 2006-2012. Respiratory invasive samples were performed according to a routine protocol. MDR pathogens were defined according to in vitro susceptibility tests. Results A total of 1176 bacteria were cultured. Susceptibility testing was performed on 1046 strains and 404 (39%) MDR were detected in 90 (51%) patients. Pseudomonas aeruginosa, coagulase-negative staphylococci, and Enterobacteriaceae (mainly Enterobacter species) were the most common MDR pathogens. On multivariate analysis, an ICU stay >14 days, presence of a tracheostomy, and previous exposure to broad-spectrum antibiotics were associated with MDR acquisition (odds ratio [ OR] 3.7; 95% confidence interval [1.69-8.12]; OR 3.28 [1.05-10.28]; and OR 2.25 [1.17-4.34], respectively). We consistently observed an increasing emergence of resistance to several antibiotics, from week 1 to week 4 of ICU hospitalization: for ticarcillin, piperacillin-tazobactam, ceftazidime, imipenem/cilastatin, amikacin, and ciprofloxacin in P. aeruginosa; and for piperacillin-tazobactam, cefepime, and amikacin in Enterobacteriaceae. Conclusion A large proportion of MDR bacteria are detected on respiratory invasive samples in LT patients, and the risk of their emergence is mainly determined by the previous exposure to broad-spectrum antibiotics and the length of ICU stay. Three weeks after single-lung transplantation for pulmonary fibrosis, a patient with high serum levels of de novo donor-specific antibodies received high-dose intravenous immunoglobulin (IVIG) infusion (scheduled dose: 2 g/kg on 2 days) to prevent antibody-mediated rejection. Within the first hours after completion of infusions, he experienced acute lung injury involving the transplanted lung. Given the clinical evolution and the absence of an alternative diagnosis, transfusion-related acute lung injury (TRALI) was diagnosed. The IVIG administered on each day was from the same batch. At day 110, because of an increase in the serum titers of donor-specific antibodies, IVIG therapy was reintroduced but from a different batch, with excellent clinical tolerance. The lung injury was explored biologically, but no mechanism was revealed. Given the increasing use of IVIG in solid-organ recipients, clinicians should be aware of possible TRALI after IVIG infusion. Background: Neurologic complications of endocarditis can influence diagnosis, therapeutic plans, and prognosis.Objective: To describe how early cerebral magnetic resonance imaging (MRI) affects the diagnosis and management of endocarditis in hospitalized adults.Design: Single-center prospective study between June 2005 and October 2008. (ClinicalTrials.gov registration number: NCT00144885)Setting: Tertiary care university hospital in France.Patients: 130 patients with endocarditis.Intervention: Cerebral MRI with angiography performed up to 7 days after admission and before any surgical intervention.Measurements: 2 experts jointly established the endocarditis diagnostic classification (according to Duke-modified criteria) and therapeutic plans just before and after MRI and then compared them.Results: Endocarditis was initially classified as definite in 77 patients and possible in 50 and was excluded in 3. Sixteen patients (12%) had acute neurologic symptoms. Cerebral lesions were detected by MRI in 106 patients (82% [95% CI, 75% to 89%]), including ischemic lesions in 68, microhemorrhages in 74, and silent aneurysms in 10. Solely on the basis of MRI results and excluding microhemorrhages, diagnostic classification of 17 of 53 (32%) cases of nondefinite endocarditis was upgraded to either definite (14 patients) or possible (3 patients). Endocarditis therapeutic plans were modified for 24 (18%) of the 130 patients, including surgical plan modifications for 18 (14%). Overall, early MRI led to modifications of diagnosis or therapeutic plan in 36 patients (28% [CI, 20% to 36%]).Limitation: Investigators did not assess whether the MRI-related changes in diagnosis and therapeutic plans improved patient outcomes or led to unnecessary procedures and increased costs.Conclusion: Cerebral lesions were identified by MRI in many patients with endocarditis but no neurologic symptoms. Guedj N, Couvelard A, Arcangeli G, Dubois S, Thabut G, Lesèche G, Fournier M, Degott C & Groussard O (2004) Histopathology 44, 251–256 Angiogenesis and extracellular matrix remodelling in bronchioloalveolar carcinomas: distinctive patterns in mucinous and non-mucinous tumours Bronchioloalveolar carcinomas (BACs) are rare primitive lung adenocarcinomas growing along the alveolar septum without stromal, vascular or pleural invasion. We report an immunohistochemical study of their vascular microenvironment. In three mucinous BACs (M-BAC) and three non-mucinous BACs (NM-BAC) we examined the following parameters in comparison with the normal lung: (i) constituents of the alveolar extracellular matrix; (ii) qualitative and quantitative changes of alveolar capillaries; and (iii) expression of vascular endothelial growth factor (VEGF) by tumour cells. In M-BAC, the alveolar matrix was unchanged compared with the normal parenchyma. Capillaries expressed normal alveolar endothelial markers and their average surface was calculated, as in normal lung, as 8%. VEGF was negative in tumour cells. In NM-BAC, the alveolar wall was thickened by deposits of fibronectin and type III collagen containing myofibroblasts and the basement membrane was disrupted. Capillaries did not retain alveolar endothelial markers and their surface was calculated as 19%. Tumour cells expressed high levels of VEGF. In contrast to NM-BAC, M-BAC do not modify the alveolar structure and seem to exploit the normal alveolar vascular bed to grow, without inducing neoangiogenesis. Objectives: We compared the accuracy of B-type natriuretic peptide (BNP) assay with Doppler echocardiography for the diagnosis of decompensated congestive left-heart failure (CHF) in patients with acute dyspnea.Background: Both BNP and Doppler echocardiography have been described as relevant diagnostic tests for heart failure.Methods: One hundred sixty-three consecutive patients with severe dyspnea underwent BNP assay and Doppler echocardiogram on admission. The accuracy of the two methods for etiologic diagnosis was compared on the basis of the final diagnoses established by physicians who were blinded to the BNP and Doppler findings.Results: The final etiologic diagnosis was CHF in 115 patients. Twenty-four patients (15%) were misdiagnosed at admission. The BNP concentration was 1,022 +/- 742 pg/ml in the CHF subgroup and 187 +/- 158 pg/ml in the other patients (p < 0.01). A BNP cutoff of 300 pg/ml correctly classified 88% of the patients (odds ratio [OR] 85 [19 to 376], p < 0.0001), but a high negative predictive value (90%) was only obtained when the cutoff was lowered to 80 pg/ml. The etiologic value of BNP was low in patients with values between 80 and 300 pg/ml (OR 1.85 [0.4 to 7.8], p = 0.4) and also in patients who were studied very soon after onset of acute dyspnea. Among the 138 patients with assessable Doppler findings, a "restrictive" mitral inflow pattern had a diagnostic accuracy for CHF of 91% (OR 482 [77 to 3,011], p < 0.0001), regardless of the BNP level.Conclusions: Bedside BNP measurement and Doppler echocardiography are both useful for establishing the cause of acute dyspnea. However, Doppler analysis of the mitral inflow pattern was more accurate, particularly in patients with intermediate BNP levels or "flash" pulmonary edema. [ABSTRACT FROM AUTHOR]
