Search

Social class researchers in social psychology have pushed the field to become more focused on and attentive to the critical role of sociocultural contexts. In this article, we label and articulate the key ingredients of the approach that many social psychological researchers have come to use: what we refer to as a social‐class‐in‐context perspective. This perspective means attending to the contextual differences in resources that create social class differences in psychology and behavior. We also suggest some additional steps that researchers can take to become even more attentive to and responsive to the important role of contexts in creating social class. As a first step, we suggest the importance of adopting a definition of social class that directly explicates its relationship to similar constructs, such as power and status, and also links it to the contexts that produce it. Second, building on this definition of social class, we then describe the importance of taking a multi‐level approach to understanding how different social class contexts shape psychology and behavior. AbstractTheoretical understandings of micropolitics and ablism provided a framework to analyze power relations and their impacts in a participatory design process. Micropolitics allowed a focus on small scale interactions and relational processes that impact outcomes of participation, and revealed the importance of problem framing and coalition building. We also found resistance to ablism through micropolitical strategies including: the sharing of experiential narratives; the use of expressive communication styles; and the development of alliances. These helped challenge norms of ablism in the deliberative process. Suggested practices for planners and facilitators who want to intervene in status quo ablest practices are identified. In sub-Saharan Africa, young people are at high risk of HIV infection, representing nearly 4 out of 5 new infections. HIV self-testing (HIVST), a new and proactive testing scheme that involves self-collection of a specimen and interpretation of results, is deemed potentially helpful for increasing testing amongst population groups like young people who do not frequently use routine testing services. This study assessed young people's intention to use HIVST in urban areas of southern Ethiopia drawing on the Theory of Planned Behaviour. A community-based cross-sectional study was conducted with 634 participants aged 15–24 years from six town administrations of two zones in January to February 2023. The participation rate was 634/636 yielding 99.7%. The OraQuick HIVST kit was demonstrated to young people recruited in a door-to-door survey with a face-to-face interview using an electronic questionnaire in a mobile phone-based application. Intention to use HIVST was measured from a 6-point Likert scale with scores of agreements ranging from 1 to 6. Descriptive statistics and ordinal logistic regression analysis were done using STATA version 18. Most of the participants agreed that they would use HIVST if it was available (86.3% agreeing or strongly agreeing). Interestingly, young people who perceived themselves at some to high risk were 0.51 times less likely to be in the higher order of intention to use when HIVST is available to them than those who perceived themselves at no to low risk. Intention to use HIVST increased by a factor of 1.29, 1.84 and 2.35 for every one-unit increase on the mean favourable attitude, perceived behavioural control, and acceptability scores, respectively. The majority of young people intended to use HIVST. Young people's perceived behavioural control, and acceptability of HIVST affected their intention to use. Intention and subsequent use of HIVST can be enhanced through an understanding of the role of risk perception and positive attitude, confidence to perform and acceptance of the test. The application of force is a key aspect of performance during athletic activities. In jumping, the timing and magnitude of force application are important performance factors. The relative timing of forces has only been investigated in the countermovement jump. This study aimed to explore if the synchronization of peak concentric force with the instance of zero velocity during a drop jump impacted performance and examine the relationship of force–time curve shape to performance. Sixty‐six state‐level athletes (24 males and 42 females) completed drop jumps from a 30 cm box onto dual force plates. The jump with the highest reactive strength index (RSI) score was taken for analysis and classified as synchronous or asynchronous based on the relative timing of peak concentric force and zero velocity. RSI and other force–time variables were compared between groups, and functional principal component analysis (fPCA) was performed on the force–time curves, which were used to perform functional principal component regression (fPCR). Synchronous jumpers exhibited greater RSI scores and shorter contact times compared to asynchronous jumpers. Performance differences were largely driven by improved concentric kinetics, which the fPCR model revealed to have the greatest influence on RSI. Sex was also found to be a significant factor for RSI in the fPCR model. fPCA revealed that greater force application preceding and throughout the amortization phase was positively associated with RSI. The timing of peak concentric force and the shape of the force–time curve are key factors for drop jump performance, and these concepts should be investigated further in other athletic activities. For Black people of the African diaspora, who have survived generational oppression including enslavement, and exist in persistently hostile environments in which anti-Black racism is structural and interpersonal, an expansive view of posttraumatic growth (PTG) is required to promote personal and collective healing. Using the intergenerational healing and well-being framework, the authors examine historical and contemporary examples of personal and collective healing among Black people to reimagine pathways to PTG. Implications for helping professions when rethinking PTG in the context of systemic anti-Black racism are presented. Introduction: Dynamic interaction of cam and pincer deformities can result in pathological contact forces in femoroacetabular impingement (FAI) syndrome. Similar deformities were noted during acetabular fracture reconstruction. We hypothesise that the prevalence of FAI deformity is higher in patients sustaining acetabular fractures than in a control group. Methods: This retrospective radiographic study compared the parameters that define FAI deformity in 100 patients consecutively admitted to a trauma unit with an acetabular fracture and a control group of 100 patients with lateral compression pelvic fracture without acetabular injury. Both underwent identical clinical imaging. Centre-edge angles (CEA) and alpha angles were used to define patients with pincer, cam, or mixed FAI deformity. Results: There were significantly more patients with FAI deformity in the acetabular fracture group than in the control group (71% vs. 18%, p < 0.0001). There were considerably more fractures involving both anterior and posterior acetabular columns in patients with 'pincer' sockets (p = 0.02, OR = 2.95, 95% CI, 1.12–7.78). Complex integral membrane proteins, which are embedded in the cell surface lipid bilayer by multiple transmembrane spanning polypeptides, encompass families of proteins that are important target classes for drug discovery. These protein families include G protein-coupled receptors, ion channels, transporters, enzymes, and adhesion molecules. The high specificity of monoclonal antibodies and the ability to engineer their properties offers a significant opportunity to selectively bind these target proteins, allowing direct modulation of pharmacology or enabling other mechanisms of action such as cell killing. Isolation of antibodies that bind these types of membrane proteins and exhibit the desired pharmacological function has, however, remained challenging due to technical issues in preparing membrane protein antigens suitable for enabling and driving antibody drug discovery strategies. In this article, we review progress and emerging themes in defining discovery strategies for a generation of antibodies that target these complex membrane protein antigens. Brian Friel's Translations and Aminatta Forna's Ancestor Stones both use a character with a speech impairment as a metaphorical representation of silencing and its limits under colonialism. When read purely as metaphors, as in the extensive body of criticism on Friel's play, these characters offer competing visions of what is lost and what is not when precolonial cultures and languages are suppressed. However, a purely metaphorical reading reinforces both the broad tendency to use disability to represent other forms of oppression and the specific tendency to use communication disabilities to represent the supposed silencing of marginalized people, both of which reaffirm ableist conceptions of disability as deficiency and pull our attention away from the individuality of the disabled characters. A closer reading of each text shows the flaws in a strictly metaphorical reading, as both characters complicate the idea that their inability to vocalize is a form of silence, as they demonstrate that people with communication disabilities can have much to tell people and many ways of doing so. Caregivers of children with attention deficit-hyperactivity disorder (ADHD) and autism spectrum disorder (ASD) experience more stress than caregivers of typically developing children but there is limited research evaluating caregivers' quality of life (QoL). This study aimed to describe the association of caregiver QoL in children with ASD and/or ADHD. This study included patients with ADHD and/or ASD seen in one pediatric specialty clinic between September 2018-August 2020. Caregivers were classified as those caring for children with ASD-only, ADHD-only, or youth with both conditions (ADHD + ASD). An adapted version of the PedsQL Family Impact Module was used to measure caregiver QoL. The sample included caregivers of 931 children. The majority of these children were male (74.7%), non-Hispanic white (63.3%), and aged 6 to 12 years (57.8%). Across the groups, significant differences were observed in patient age (p < 0.0001), preferred language (p = 0.005), and insurance (p = 0.001). Caregivers of non-Hispanic Black children had 4-times the odds of reporting feeling isolated from others (OR 4.36, 95% CI 1.19-16.00 p = 0.03). Those caring for children with ADHD-only had significantly lower odds of reporting helplessness or hopelessness (OR 0.45, 95% CI 0.26-0.80, p = 0.004), and difficulty talking about their child's health with others (OR 0.30, 95% CI 0.17-0.54, p < 0.0001). Similarly, caregivers of children who had ADHD + ASD reported higher odds of difficulty making decisions together as a family (OR 14.18, 95% CI 1.15-17.91, p=0.04) and difficulty solving family problems together (OR 45.12, 95% CI 2.70-752.87), p = 0.008). Despite decades of research on HPV vaccination, the U.S. has still not achieved optimal vaccination rates. This study investigated two different message strategies (agency assignment and deviance regulation marking) for encouraging HPV vaccination in two different populations: parents considering vaccinating their children and young adults (18–26) considering vaccinating themselves. Using a 2 × 3 × 3 online experiment, this study examined the differences between the two message target groups and the effects of the different message strategies on psychological precursors to HPV vaccination and vaccination intention. The findings indicate that using both agency and deviance regulation marking message strategies provided a significant increase in HPV vaccination intentions. However, there were many differences in terms of the variables influencing HPV vaccination decision making in young adults (18–26) and parents making decisions for their children. Specifically, using human agency and a deviance message increased HPV vaccine confidence in young adults, but did not in parents. Interprofessional education (IPE) initiatives, where students from multiple professions learn from, with and about each other, have been implemented in various care environments. However, no reviews have examined the impact they have in older adult’s care home settings, despite their potential to enhance care quality. This review scopes out the available and comparable evidence reporting on the impact and structure of IPE initiatives based in the care home sector. The review aims to report countries and settings where IPE research is conducted in care homes, characterise these IPE programs (length, type, and activities), determine the professional groups involved, and assess the impact of IPE on students, residents, and care home staff. The search was limited to articles published in English from January 2010 to July 2023. Key databases and gray literature were searched, resulting in the inclusion of 10 studies. A draft charting table was developed to record key characteristics of the included studies. Thematic analysis resulted in the construction of four themes: knowledge, skills, personal development, and models for future delivery. Literature demonstrates the benefits of IPE initiatives in older adult’s care home settings, though it mainly focuses on student learning and development. Few studies examine the impacts of IPE on staff or residents, and there is limited emphasis on IPE in UK care homes. Simple Summary: Medullary thyroid carcinoma (MTC) is a rare but aggressive form of thyroid cancer accounting for over 10% of deaths related to thyroid malignancies. MTC can be either hereditary or sporadic. Although it can be cured if it is detected early and completely removed, most patients with MTC are diagnosed when the cancer has already spread beyond the thyroid gland, making it difficult to treat. Managing MTC is complex, despite the availability of newer treatment options like targeted therapy, which works in less than 30% of cases and can lead to severe side effects in some patients. MTC behaves differently in each patient, making the selection of appropriate treatments challenging, even for multidisciplinary teams of experts. This article aims to describe the challenges faced during the diagnostic workup and management of MTC patients. It highlights how holomics, which is an integrated approach combining various biological data types, and artificial intelligence (AI) can assist in improving patient outcomes. By simultaneously analyzing and integrating findings from biochemical, radiological, and histological investigations, genetic studies, and other sources, along with the personal information of a patient, AI can enhance decision-making processes. This innovative approach has the potential to personalize and optimize treatment strategies, leading to better management and improved outcomes for patients with MTC. Background/Objective: Medullary thyroid carcinoma (MTC) is a rare yet aggressive form of thyroid cancer comprising a disproportionate share of thyroid cancer-related mortalities, despite its low prevalence. MTC differs from other differentiated thyroid malignancies due to its heterogeneous nature, presenting complexities in both hereditary and sporadic cases. Traditional management guidelines, which are designed primarily for papillary thyroid carcinoma (PTC), fall short in providing the individualized care required for patients with MTC. In recent years, the sheer volume of data generated from clinical evaluations, radiological imaging, pathological assessments, genetic mutations, and immunological profiles has made it humanly impossible for clinicians to simultaneously analyze and integrate these diverse data streams effectively. This data deluge necessitates the adoption of advanced technologies to assist in decision-making processes. Holomics, which is an integrated approach that combines various omics technologies, along with artificial intelligence (AI), emerges as a powerful solution to address these challenges. Methods: This article reviews how AI-driven precision oncology can enhance the diagnostic workup, staging, risk stratification, management, and follow-up care of patients with MTC by processing vast amounts of complex data quickly and accurately. Articles published in English language and indexed in Pubmed were searched. Results: AI algorithms can identify patterns and correlations that may not be apparent to human clinicians, thereby improving the precision of personalized treatment plans. Moreover, the implementation of AI in the management of MTC enables the collation and synthesis of clinical experiences from across the globe, facilitating a more comprehensive understanding of the disease and its treatment outcomes. Conclusions: The integration of holomics and AI in the management of patients with MTC represents a significant advancement in precision oncology. This innovative approach not only addresses the complexities of a rare and aggressive disease but also paves the way for global collaboration and equitable healthcare solutions, ultimately transforming the landscape of treatment and care of patients with MTC. By leveraging AI and holomics, we can strive toward making personalized healthcare accessible to every individual, regardless of their economic status, thereby improving overall survival rates and quality of life for MTC patients worldwide. Background: Young people (YP) are disproportionately affected by the HIV pandemic in sub-Saharan Africa (SSA), but testing rates remain low despite global targets of testing 95% of people with HIV infection by 2030. HIV self-testing (HIVST) has been recently introduced to reach high-risk population groups such as these. Thus, synthesis of emerging evidence on the acceptability and use of HIVST among YP in SSA is needed so that comprehensive information can be generated to inform policy and practice. Methods: We employed a mixed methods systematic review of quantitative and qualitative literature reporting on HIVST among YP involving any design and published in English by 31st of October 2023. The review synthesized quantitative evidence on acceptability and use of HIVST, and qualitative evidence on perspectives of YP about HIVST. We searched databases of published articles (e.g. MEDLINE, CINAHL) and Gray literature sources (e.g. Google, Google Scholar). The concepts for the search included self-testing, HIV/AIDS, and countries in SSA. Two authors independently screened, retrieved full-text, and assessed quality of the studies. Results: A total of 4150 studies were retrieved and 32 studies were finally included in the review. Acceptability of HIVST computed from a single item asking YP on their preference or willingness or demand for HIVST was moderate (34–67%) to high (≥ 67%) among YP in SSA. Nine of the fourteen studies that reported on acceptability found high acceptability of HIVST. Use of HIVST ranged from 0.8 to 100% while in most studies the use rate was below 50%. Key barriers to HIVST use were coping with a positive test in the absence of counselling and support, physical discomfort, and cost of kits. Perceived enablers included perceptions of HIVST as promoting personal empowerment and autonomy; privacy and confidentiality; and convenience in location, time, and skill. Conclusions: HIVST was highly accepted but not well utilized among YP in SSA. YP showed diversified needs with mixed preferences for location, and modalities of service provision. Overall, the review identified heterogeneous evidence in terms of methods, population, outcome measures, and results. This study sought to reach an international expert consensus regarding the diagnosis and treatment of adults with a sexual interest in children in cases where treatment is non-mandated. An expert panel of 27 participants (19 professionals and eight individuals with sexual interest in children) completed at least one of three rounds of an online Delphi survey. In the first round, participants responded to open-ended questions about assessment approaches, treatment goals, treatment approaches, and adapting assessment and treatment for sexual exclusivity. In rounds two and three, participants rated their level of agreement with each of the recommendations derived from Round 1. Consensus was reached for 48 practice guidelines, particularly for recommendations that relate to general clinical practice, such as assessing for other mental health conditions. There was more contention for the inclusion of forensic practices. The present recommendations can serve as a set of tentative guidelines that may guide assessment and treatment of sexual interest in children in non-mandated settings. [ABSTRACT FROM AUTHOR]

BACKGROUND: Operating room (OR) handoffs are not universally standardized, although standardized sign outs have been proven to provide effective communication in other aspects of healthcare. We hypothesize that creating a standardized handoff will improve communication between OR staff. STUDY DESIGN: A frontline stakeholder approached our quality improvement team with concern regarding inadequate quality surgical technician handoffs during staff changes. An audit tool was created for a pilot cohort of 23 cases to evaluate surgical technician handoffs from May 2022 to November 2022. Handoffs occurred in 82.6% of cases. Elements of handoff varied significantly, with an average of 34.4% completion of critical handoff elements. Audits were reviewed with stakeholders to develop a standardized communication checklist, including domains regarding sponges, sharps, hidden items, replaced items, instruments, implants, medications, procedure overview, and specimens. An acronym of these domains, SHRIMPS, was affixed to each OR wall. RESULTS: In the initial Plan-Do-Study-Act cycle, piloted in urology, general surgery, and neurosurgery, 100% of the 15 observed cases included handoff, averaging 76 seconds per handoff. Additionally, 100% of cases announced a handoff to the surgeon, and all elements were addressed 99.6% of the time. Plan-Do-Study-Act cycle 2 involved implementation to all service lines. Of the 68 cases observed, 100% included handoff, averaging 69.4 seconds per handoff, with 98.2% of elements addressed, though only 97.1% of handoffs were announced. CONCLUSIONS: Little communication standardization exists within the OR, especially regarding intraoperative staff changes. For the first four decades of the twentieth century premature babies in the United States were primarily treated in infant incubators not in hospitals but in a public setting—at the Coney Island amusement park. Although incubators are now an indispensable medical device their origins lie in public exhibitions rather than a professional medical environment. This article uncovers the longer history behind this unusual episode in neonatal care and technology. Offering the first comprehensive account of the early history of the infant incubator it traces how these devices were first developed and showcased at exhibition sites across Europe in the 1890s. A comparative study of these exhibitions in Europe and the United States from 1890 to 1943 highlights significant differences in how industry science education and spectacle interacted in each country. Moreover it examines the changing relationship between public displays and professional medicine during this period illustrating how technology developed in public spaces before transitioning into the professional medical domain. The relationship between art and technology in the late nineteenth century was contested but increasingly characterized by a harmonious integration that reflected a progressive and optimistic view of technological innovation. This cover essay examines an advertising poster designed by the German-Italian commercial artist Adolfo Hohenstein for a public exhibition of infants in incubators which opened in Paris in 1896. Hohenstein's poster for the Maternité Lion with its distinctive and innovative use of an art nouveau style captures the widespread enthusiasm for the new technologies and industries that characterized the art nouveau movement. The aesthetics of art nouveau expressed a broader cultural optimism about modernity and progress around the turn of the twentieth century. In many countries, such as Canada, the USA, England, and Australia, to graduate from a regulated profession such as nursing, students must complete a set of work‐integrated learning (WIL) hours and demonstrate their ability to safely perform physical skills and apply knowledge in relation to professional standards. For a disabled nursing student (DNS) undergoing training in higher education institutions (HEI), securing proper accommodations to participate effectively in WIL experiences has been difficult due to concerns related to risks to self and patient safety. This study used critical discourse analysis to investigate the framing of risk and safety in association with providing DNS with accommodation in WIL. Our data were collected from an intensive codesign (group‐based discussion) session with participants (n = 16), including clinicians and DNS, from four institutions (two WIL‐sites and two HEI organizations). Using an iterative thematic approach based on Foucauldian framework, our analysis revealed three ways in which health professionals discursively framed risk and safety: (a) beliefs that a lack of disclosure by disabled students decreases patient safety and indicates poor self‐reflection, (b) concerns that accommodating students poses a risk to institutions offering WIL experiences and to the relationships between HEI and institutions offering WIL‐sites, and (c) a framing which challenges the dominant discourse by thinking about safety and risk more expansively. Our findings suggest the first two of these framings are ableist and function to enact barriers to access for DNSs in WIL, whereas the third framing serves the goal of inclusion. Recommendations to address the current ableist discourse within the Canadian nursing context are provided, which may also be applicable across other regulated health professions. Background: Normothermic regional perfusion (NRP) and normothermic machine perfusion (NMP) are organ procurement and transport techniques that can improve organ quality, facilitate longer transport, and reduce postoperative complications, increasing organ availability and improving outcomes. NRP and NMP often require allogeneic red blood cells (RBCs). Our academic transfusion service began providing RBCs to support NRP and NMP for adult heart transplant (HT), orthotopic liver transplant (OLT), and multiorgan transplant (MOT) in August 2020. Methods: This single‐center, retrospective study describes the implementation process and analyzes the characteristics of RBC support during the first 3 years of the perfusion programs. Timing and quantity of units issued and used, organ recipient demographics, and transplant outcomes were obtained from transfusion service and electronic medical records. Results: From 2020 to 2023, the transfusion service received 233 requests to support NRP and NMP perfusion cases. Of these, 105 cases resulted in RBC use, and units were returned or discarded in 112 cases. A total of 131 patients received perfusion‐facilitated transplants (92 HT, 27 OLT, and 12 MOT). The majority of perfusion‐facilitated HTs utilized NRP (81/92, 88%), whereas most perfusion‐facilitated OLTs utilized NMP (21/27, 78%). Across all 233 requests, a total of 381 RBC units were used to facilitate 131 transplants, averaging 2.91 units/transplant. Recent concerns that commercial bottom trawling can contribute to a significant release of sequestered marine carbon have highlighted a need for research in this area. Here, a Before-After Control-Impact (BACI) experimental design was utilized in a previously untrawled area of the eastern Bering Sea. Six pairs of experimental and control corridors were sampled before, after, and 1 year after a trawl disturbance. Each experimental corridor was fished four consecutive times over ∼12 h with a commercial otter trawl. Results were contextualized with minimum detectable effects (MDE), and showed no evidence of a trawl effect on total organic carbon (P  = .999, MDE ± 0.05% TOC), total nitrogen (P  = .999, MDE ± 0.02% TN), δ13C, and δ15N isotope ratios and sediment size classes. Interannual changes observed in δ15N, sand, silt, and clay are attributed to natural variation. The study suggests that the characteristics of the study site, such as storm disturbances, high sand content, and low carbon content, limited the bottom-trawl effect on sediment composition following this initial trawl disturbance. Suicide is a leading cause of death among Black emerging adults. The concurrent effect of the COVID-19 pandemic and racial discrimination were projected to exacerbate suicide vulnerability for Black Americans. The purpose of the present study was to utilize a risk-resilience model to examine the effects of racial discrimination and COVID-related stress on suicide risk for Black emerging adults, as well as the moderating effect of three central components of radical healing: critical consciousness, resilience, and cultural authenticity. Study participants included 521 Black emerging adults between the ages of 18 and 29 (51.6% male; Mage = 24.6, SD = 2.6) who completed measures evaluating symptoms of racial discrimination, COVID-related stress, suicide risk, and psychological well-being. After controlling for age, gender, socioeconomic status, and general stress, structural equation modeling analyses revealed unique and interactive effects of racial discrimination, COVID-related stress, and culturally relevant protective factors on suicide risk for Black emerging adults. These findings provide preliminary insight into novel risk and protective factors that influence suicide risk for Black emerging adults. Public Significance Statement: Overall, findings suggest that the synergistic effects of COVID-related stress and racial discrimination play a role in suicide vulnerability for Black emerging adults. The present study's contributions are important in the context of providing empirical evidence for the role of social marginalization and culturally relevant buffers in suicide risk for Black Americans, highlighting the need for culturally informed models of suicide risk. This paper builds on decolonizing educational research discourse. Rich, generative, and diverse forms of knowledge production, includes that of the Caribbean. Specifically, the paper uses the Black Caribbean method of Liming, which is an indigenous methodology. The paper illustrates how educational research practices can be enriched by Black and Caribbean ways of thinking, being and knowing. This diversity would support a pivot from Western methods. The author employs reflections from her dissertation writing and research experience, while highlighting the dire need to incorporate and institutionalize methods from Black scholars, Black communities, and the Global South. Via this paper, I illustrate how Liming has allowed for greater discourse, and learning with the diverse communities served. Liming's contributions are beneficial in educational research as well its utility for other areas of research. Background: The number of Hospital-at-Home (HaH) programs rapidly increased during the COVID-19 pandemic and after issuance of Centers for Medicare and Medicaid Services' (CMS) Acute Hospital Care at Home (AHCaH) waiver. However, there remains little evidence on effective strategies to equitably expand HaH utilization. Objective: Evaluate the effects of a multifaceted implementation strategy on HaH utilization over time. Design: Before and after implementation evaluation using electronic health record (EHR) data and interrupted time series analysis, complemented by qualitative interviews with key stakeholders. Participants: Between December 2021 and December 2022, we identified adults hospitalized at six hospitals in North Carolina approved by CMS to participate in the AHCaH waiver program. Eligible adults met criteria for HaH transfer (HaH-eligible clinical condition, qualifying home environment). We conducted semi-structured interviews with 12 HaH patients and 10 referring clinicians. Interventions: Two strategies were studied. The discrete implementation strategy (weeks 1–12) included clinician-directed educational outreach. The multifaceted implementation strategy (weeks 13–54) included ongoing clinician-directed educational outreach, local HaH assistance via nurse navigators, involvement of clinical service line executives, and individualized audit and feedback. Measures: We assessed weekly averaged HaH capacity utilization, weekly counts of unique referring providers, and patient characteristics. We analyzed themes from qualitative data to determine barriers and facilitators to HaH use. Results: Our evaluation showed week-to-week increases in HaH capacity utilization during the multifaceted implementation strategy period, compared to discrete-period trends (slope-change odds ratio—1.02, 1.01–1.04). Counts of referring providers also increased week to week, compared to discrete-period trends (slope-change means ratio—1.05, 1.03–1.07). The increase in HaH utilization was largest among rural residents (11 to 34%). Barriers included HaH-related information gaps and referral challenges; facilitators included patient-centeredness of HaH care. Conclusions: A multifaceted implementation strategy was associated with increased HaH capacity utilization, provider adoption, and patient diversity. In this paper we examine the changing demographics of overdose deaths in West Virginia based upon death certificate and economic data in West Virginia for the years 2001–2016. During the period under study, opioid overdose deaths accounted for 7% of deaths overall, and a significant number of premature deaths. Those affected were an average of 41 years old and were more likely to be unmarried and less than college educated. Eighty percent were employed at the time of death and individual employment in specific industries was associated with an increased risk of opioid overdose death. Previous state and national level policies attempting to address the opioid crisis may have lagged behind the data trends for this time period and thus been less effectual. Future policies intended to ameliorate the harms associated with the opioid crisis should reflect current data trends to help the communities and individuals at high risk for overdose death. We use |$1-4$|  arcsec (⁠|$300-1200$| au) resolution |$^{12}$| CO (2 − 1) data from the MASSES (Mass Assembly of Stellar Systems and their Evolution with the Submillimeter Array) project to measure the projected opening angles of 46 protostellar outflows in the Perseus Molecular Cloud, 37 of which are measured with sufficiently high confidence to use in further analysis. We find that there is a statistically significant difference in the distributions of outflow opening angles for Classes 0 and I outflows, with a distinct lack of both wide-angle Class 0 outflows and highly collimated Class I outflows. Synthesizing our results with several previous studies, we find that outflows widen with age through the Class 0 stage but do not continue to widen in the Class I stage. The maximum projected opening angle reached is approximately 90 |$^{\circ }$| |$\pm$| 20 |$^{\circ }$|⁠ , with the transition between widening and remaining constant occurring near the boundary between the Classes 0 and I phases of evolution. While the volume fractions occupied by these outflows are no more than a few tens of per cent of the total core volume, at most, recent theoretical work suggests outflows may still be capable of playing a central role in setting the low star formation efficiencies of 25  per cent–50  per cent observed on core scales. The sexual exploitation of children in travel and tourism (SECTT), also referred to as child sex tourism, involves engaging in sexual activity with someone under the age of 18 in the context of travel and/or tourism. Research on the perpetration of SECTT is vastly underdeveloped. The present review considers the operationalization of SECTT, its prevalence, and existing theoretical models. Existing theory on SECTT provides a good overview of environmental factors that contribute to SECTT, but the model is insufficient in fully accounting for individual-level risk factors related to sexual offending against children. The present paper expands an existing theoretical model of SECTT, the ecosystem model, to account for these individual risk factors and considers their interaction with environmental factors. This Article provides a model for examining the impact of the Convention on the Elimination of All Forms of Discrimination Against Women ("CEDAW") on implementing gender equality reforms using Canada and India, two CEDAW State Parties, as case studies. It also explores the influence of heteropatriarchy, deeplyrooted cultural norms perpetuating gender inequality, on hindering CEDAW's ratification in the United States, as well as CEDAW's effectiveness in implementing reforms in Canada and India. The analysis showcases how non-governmental organizations ("NGOs") in these countries have nevertheless achieved limited successes through their mobilization of CEDAW to address specific gender injustices, such as gender violence against Indigenous women and workplace sexual harassment. However, even if CEDAW facilitates a state's enactment of reforms, the enactment may not result in the reforms' implementation because the treaty alone cannot overcome state-sanctioned misogynistic beliefs. The research contributes to the scholarly dialogue regarding CEDAW's effectiveness in three ways. First, this Article provides a model for understanding CEDAW's impacts on implementing gender equality measures through a comparative analysis of such measures' implementation in Canada and India post-treaty ratification. Second, this Article uses this comparative lens to argue that, although the United States should ratify CEDAW, its potential impact rests in its ability to advocate for gender equality reforms on which society generally agrees while avoiding controversial reforms (e.g., legalizing abortion). In so doing, the research takes the scholarly debate beyond advocating for or against the United States' ratification of CEDAW based on the assumption that it will or will not make a difference in women's lives. Instead, this Article argues scholars cannot address that question until they can show a state has successfully implemented a CEDAW-inspired reform. Finally, this Article aims to provide peace of mind to CEDAW's ratification opponents in the United States by suggesting that CEDAW, if eventually ratified, will only result in reforms on which there is bi-partisan support (e.g., more support for mothers and pregnant women in the military) rather than reforms on divisive issues because of entrenched patriarchal beliefs in American society. Heart failure with preserved ejection fraction (HFpEF) has recently emerged as an insidiously and increasingly prevalent heart failure phenotype. HFpEF often occurs in the context of hypertension and obesity and presents with diastolic dysfunction, ventricular hypertrophy, and myocardial fibrosis. Despite growing study of HFpEF, the causal links between early metabolic changes, bioenergetic perturbations, and cardiac structural remodeling remain unclear. This study sought to elucidate the contribution of the respective pathophysiological drivers of the HFpEF symptom suite using a recently developed two‐hit mouse model. By studying the independent and concomitant consequences of hypertension and obesity‐driven metabolic dysfunction on cardiac structure and function, we revealed the causative drivers of cardiac functional, structural, and metabolic remodeling in male HFpEF mice. We found that hypertensive male mice developed diastolic dysfunction and cardiac hypertrophy regardless of obesity status and that obese mice exhibited altered systemic glucose metabolism and increased cardiac mitochondrial fatty‐acid metabolism independent of hypertension status. Taken together, our results suggest that the cardiac structural and metabolic HFpEF symptoms in this two‐hit model occur as direct results of each of the two "hits." The results of this study help to clarify the pathogenic HFpEF cascade, providing causal insights that may aid in the development of more precisely targeted therapeutics. [ABSTRACT FROM AUTHOR]

We propose a multivariate GARCH model for non‐stationary health time series by modifying the observation‐level variance of the standard state space model. The proposed model provides an intuitive and novel way of dealing with heteroskedastic data using the conditional nature of state‐space models. We follow the Bayesian paradigm to perform the inference procedure. In particular, we use Markov chain Monte Carlo methods to obtain samples from the resultant posterior distribution. We use the forward filtering backward sampling algorithm to efficiently obtain samples from the posterior distribution of the latent state. The proposed model also handles missing data in a fully Bayesian fashion. We validate our model on synthetic data and analyze a data set obtained from an intensive care unit in a Montreal hospital and the MIMIC dataset. We further show that our proposed models offer better performance, in terms of WAIC than standard state space models. The proposed model provides a new way to model multivariate heteroskedastic non‐stationary time series data. Model comparison can then be easily performed using the WAIC. [ABSTRACT FROM AUTHOR]

Abstract\nPUBLIC SIGNIFICANCE STATEMENTDespite recent increases in institutional efforts to improve sexual health safety, few studies have explored Latino college men’s sexual risk behaviors. Considering prior research examining links between sexual risk behaviors and factors such as condom use self-efficacy, sexual health knowledge and education, and sexual empowerment, it is critical to examine their influence on Latino college men’s sexual health in the context of cultural values and sexual scripts. Quantitative cross-sectional design and linear regression models were utilized to analyze responses from 177 Latino college men attending a Hispanic Serving Institution (HSI) in the Southeastern region of the United States (U.S.). Our findings revealed that higher satisfaction with sexual health knowledge and education when obtained from informal sources such as family members and peers significantly increased condom use self-efficacy. Contrastingly, greater levels of sexual empowerment significantly decreased Latino college men’s condom use self-efficacy. Findings underline the importance of considering the dynamic interaction of multilevel factors and the sexual decision-making process from a developmentally appropriate, evidence-based, and culturally syntonic lens to address sexual risk behaviors among Latino college men students.“Sexual health knowledge and education obtained from informal sources play a significant role in increasing Latino college men student’s condom use self-efficacy, in contrast to the effect shown by factors like sexual empowerment. Following a resource pulse, animals may finance reproduction by consuming concurrently available resources (income breeding) or by storing resources for future reproduction (capital breeding). Understanding how these reproductive strategies are used is important for determining the ecological mechanisms that structure timing of reproduction and that drive interannual population fluctuations in animals.We gathered a reproductive dataset for five small mammal species over a 12‐year period in Northeastern USA during which six masting events of American beech (Fagus grandifolia) and eastern hemlock (Tsuga canadensis) occurred. Masting created alternate years where seeds were either available late (masting year) or early (cached from the previous year) in the breeding season. The small mammal species differed in reliance on seeds and overwintering strategies. We quantified the diet using stable isotopes and recorded reproduction timing, proportion breeding and litter size in females and testes size in males.Timing of seed availability minimally affected litter size but strongly affected proportion breeding and timing of reproduction. During masting years (late seed availability), a higher proportion of females reproduced, with breeding taking place later in the season (lactation timed with peak seed availability), although the delay was restricted in Napaeozapus insignis, an obligate hibernator. After a fall mast, cached seeds were used as capital in the following spring (early seed availability) to support a litter that, depending on the species, occurred 24–79 days sooner than a mast year. No late‐season reproduction occurred in years with early seed availability except for Myodes gapperi which produced a second litter, likely financed by fungal consumption. Males also showed strong responses to seed availability, mirroring female reproduction with testes size staying constant in years with late seed availability and sharply decreasing over the breeding season in years with early seed availability.Our results highlight that although photoperiod and temperature broadly set bounds of the breeding season in temperate environments, resource availability influences the reproductive strategies that species use, which in turn alters reproductive timing and can drive large inter‐annual population fluctuations. There are several seismic assessment standards and guidelines available around the world that can be used to identify vulnerable buildings. The assessment procedures and criteria in these documents are different, and thus, the assessment outcomes for a particular building, if assessed using different standards, can also be different. In this study, provisions of the linear static and non-linear static analysis procedures of three prominent seismic assessment documents, the American Society of Civil Engineers /Structural Engineers Institute standard ASCE 41 (2017) [1], the New Zealand Seismic Assessment Guidelines (2017) [2], and the European Standard EN 1998-3 (2005) [3] (also known as Eurocode 8 Part-3 or EC8-3) are discussed and compared, highlighting some of their similarities and differences. A reinforced concrete (RC) wall building used in FEMA P-2006 (2018) [4] for demonstration of ASCE 41 provisions is taken as the case study building for comparison of the assessment provisions. The linear and non-linear static analysis procedures specified in the three documents are applied to the case study building and the assessment outcomes are compared. The assessment results are found to vary across the analysis methods and guidelines. However, the critical governing vulnerability for the building is found to be the same. It is observed that with the simplifying modelling assumptions, coupled with the inherent conservatism in the assessment using linear static analysis, a more conservative outcome is obtained using the linear static methods as compared to the non-linear static methods. Overall, EC8-3 provisions are found to be the most conservative of all three guidelines considered for the assessment of the example building. [ABSTRACT FROM AUTHOR]

Recent interest in scaling commercial kelp industries in Western cultures is juxtaposed by the apparent challenges in achieving more consistent and predictable yields. As such, site-level factors are a dominant and recurring conversation amongst stakeholders. The availability of seawater nitrogen (nitrate, ammonium, total N) can be highly variable across space and time and is often one of the top concerns for site selection and permitting. This study questions the relative importance of nitrogen availability on the yield of Saccharina latissima (sugar kelp) across five commercial farms on the U.S. East and West Coasts over two seasons, highlighting the relative influence of other interacting factors (i.e., farm design). We hypothesized that nitrate would strongly correlate with the harvested yield. Our results show significant spatial and annual variability in the kelp yield and ambient nutrients across and within farms, but with weak covariance. Standard linear regression suggests that seawater nitrogen is a poor explanatory factor for kelp yield, explaining 11.0% of the variation around the mean compared to the line spacing (explaining 26.1%) and the interaction between the total N and the line spacing (explaining 50.0%). Quartile regression, however, suggests that total N alone, is the strongest predictor of a lower threshold in terms of the yield (0.10 quartile, r2 = 0.431) relative to the median (0.50 quartile, r2 0.081). As such, seawater nitrogen may be a more useful metric in predicting baseline kelp yields rather than realized yields, and production above that baseline is likely more dependent on other factors that may or may not interact with seawater nitrogen. The current study examines medical students reported attitudes about their current medical training experiences and desired knowledge about lesbian, gay, bisexual, transgender, and queer (LGBTQ) health issues within India. Members of a medical college in the state of Karnataka, India, were recruited to complete a questionnaire examining perspectives about medical students in India and their knowledge, education, and training, specifically on LGBTQ individuals. This study included open-ended questions focused on the perspectives of medical students in India and their knowledge, education, and training, specifically on LGBTQ individuals. A total of 198 participants in different medical-related classes ages 19-35 completed the open-ended questions about their perceptions of current medical training focused on LGBTQ patients. Most participants reported the need for education and training related to LGBTQ sexual minorities. It was also suggested by the participants that education has to start with the training of professors in the field and the necessity to stop stigmas related to this community. [ABSTRACT FROM AUTHOR]

Previous population-based studies have reported that the majority of melanoma mortality is related to patients with thin (≤1 mm Breslow thickness) melanomas. The aim of the present study was to evaluate the relative proportion of melanoma-specific deaths across all stages of melanoma at diagnosis over the past 20 y in the United States. A review of all cutaneous melanoma cases in the US Surveillance, Epidemiology, and End Results registry from 2004 to 2020 was performed. Breslow thickness was categorized as thin (≤1.0 mm), intermediate (>1-4 mm), or thick (>4 mm). All-cause deaths and melanoma-specific deaths were compared across tumor thickness and stage groups at diagnosis. Survival analysis was performed with nonmelanoma deaths considered as a competing risk to estimate the cumulative incidence of melanoma-specific death. Most melanoma deaths occurred in patients who initially presented with local disease (53%) compared to regional (36%) or distant (11%) disease (P < 0.001). However, most (66%) of the melanoma-specific deaths in patients who presented with localized disease were in those with intermediate or thick (i.e., Breslow thickness >1.0 mm) primary tumors compared to those with thin melanomas (34%). The cumulative incidence of melanoma-specific death at 10 y in patients with localized thin melanomas at the time of diagnosis was 2.6% (95% confidence intervals 2.5%-2.7%). The public health burden in terms of melanoma-specific mortality is related to patients with tumors >1 mm Breslow thickness, many of whom have regional and distant metastatic disease at the time of diagnosis, not patients with thin melanomas. • Melanoma-specific deaths from 2004 to 2020 are predominately due to T2-T4, localized disease at diagnosis. • Melanoma-specific deaths are due to regional and distant metastatic disease at diagnosis in 47% of cases. • Cumulative incidence of melanoma-specific death of T1, localized melanoma is 2.6%. This article examines the images of working bodies seen in the photographic motion studies of work undertaken by the management consultants Frank and Lillian Gilbreth in the 1910s and 1920s. It contextualises their studies, called chronocyclegraphs, as the product of two key cultural developments: first, new practices of measuring and assessing productivity in the context of workplace management and second, the use of new technologies for visualising the body, which brought with them new aesthetics and visual conventions for representing bodies in motion. The Gilbreths' chronocyclegraphs provide a striking new vision of the working body in industrial capitalism, not as a thing of flesh and blood, but as a luminous field of energy or line of force. Taking these images as representative of new ideas about efficiency and productivity emergent at this time, this article examines their popularisation through the work of Lillian Gilbreth, who promised that the reward for increased productivity was a greater quantity of 'happiness minutes'. State sanctioned removal of Australian Aboriginal children from family, culture, and country has had harmful and traumatic effects on the Survivors of this process and their children. Known as the Stolen Generations, children were detained in government settlements and church missions and stripped of Aboriginality. This article explores ways that a virtual reality reconstruction of mission environments may be implemented to memorialise and commemorate Survivor experience that avoid narratives of victimhood and destructive post-colonial overlays on their stories of survival. Our project is to develop a virtual reality model of Carrolup-Marribank Mission in Western Australia as a living digital memorial for use by Survivors and their families to help address their displacement and loss of culture and identity. This article shows how virtual reality may be a potent didactic tool to convey difficult histories as well as a purveyor of powerful stories that contest stereotyped perceptions of Aboriginal people. Pre‐implantation embryonic development occurs in the oviduct during the first few days of pregnancy. The presence of oviductal extracellular vesicles (oEVs, also called oviductosomes) is crucial for pre‐implantation embryonic development in vivo as oEVs often contain molecular transmitters such as proteins. Therefore, evaluating oEV cargo during early pregnancy could provide insights into factors required for proper early embryonic development that are missing in the current in vitro embryo culture setting. In this study, we isolated oEVs from the oviductal fluid at estrus and different stages of early embryonic development. The 2306–3066 proteins in oEVs identified at the different time points revealed 58–60 common EV markers identified in exosome databases. Oviductal extracellular vesicle proteins from pregnant samples significantly differed from those in non‐pregnant samples. In addition, superovulation changes the protein contents in oEVs compared to natural ovulation at estrus. Importantly, we have identified that embryo‐protectant proteins such as high‐mobility protein group B1 and serine (or cysteine) peptidase inhibitor were only enriched in the presence of embryos. We also visualized the physical interaction of EVs and the zona pellucida of 4‐ to 8‐cell stage embryos using transmission electron microscopy as well as in vivo live imaging of epithelial cell‐derived GFP‐tagged CD9 mouse model. Introduction: Driver anger and aggression have been linked to crash involvement and injury outcomes. Improved road safety outcomes may be achieved through understanding the causes of driver anger, and interventions designed to reduce this anger or prevent it from becoming aggression. Scales to measure anger propensities will be an important tool in this work. The measure for angry drivers (MAD; Stephens et al., 2019) is a contemporary scale designed to measure tendencies for anger across three types of driving scenarios: perceived danger from others, travel delays, and hostility or aggression from other drivers. Method: This study aimed to validate MAD using a representative sample of Australian drivers, stratified across age, gender, and location. Participants completed a 10-minute online survey that included MAD, sought demographic information (age, gender, driving purpose, crash history), as well as the frequency of aggressive driving. Multigroup confirmatory factor analyses (MGCFA) assessed how stable the structure of the MAD was across drivers of different ages, gender, purposes for driving and those who do or do not display anger aggressively. MAD was invariant across all groups, showing that all drivers interpreted and responded to MAD in the same way. Results: A comparison of latent means showed anger tendencies were higher for men compared to women, for younger drivers compared to older drivers, and for those who drive mainly for work compared to those who mainly drive for other reasons. When controlling for driver factors, driving anger was associated with increased odds of being aggressive while driving. Practical Applications : Overall, this study demonstrated that MAD is an appropriate scale to measure anger tendencies and can be used to support interventions, and evaluation of interventions, to reduce anger and aggressive driving. Schizophrenia is a neurodevelopmental disorder associated with deficits in cognitive development and childhood psychopathology. Previous studies have focused on older children and the few studies of early childhood have yielded inconsistent findings. We studied cognitive development and psychopathology in children at familial high risk (FHR) of schizophrenia and matched controls from 1 to 6 years and hypothesized that FHR children would show consistent deficits across cognitive and behavioral measures in early childhood. Cognitive development in children at high familial risk for schizophrenia or schizoaffective disorder (n = 33) and matched healthy controls (n = 66) was assessed at 1 and 2 years with the Mullen Scales of Early Learning, and at 4 and 6 years with the Stanford Binet Intelligence Scales, BRIEF-P/BRIEF and CANTAB. Psychopathology was assessed at 4 and 6 years with the BASC-2. General linear models were used to examine differences on outcome scores, and chi-square analyses were used to explore differences in the proportion of "at risk" or "below average" score profiles. FHR children scored significantly lower than controls on Mullen Composite at age 2, and demonstrated broad deficits in IQ, executive function and working memory and 4 and 6 years. FHR children were also rated as significantly worse on most items of the BASC-2 at ages 4 and 6. Children at FHR for schizophrenia demonstrate abnormal cognitive development and psychopathology at younger ages than previously detected, suggesting that early detection and intervention needs to be targeted to very early childhood. Most secondary and postsecondary institutions take a behavioral approach in dealing with student cheating—punishing those caught with grade reductions and/or suspensions. While some form of punishment may be necessary, it is not sufficient. As an instantiation of negative morality, academic misconduct offers an opportunity for moral education. The present investigation builds on the literature related to developmental approaches in responding to academic misconduct. It does so by describing theoretical underpinnings and instructional design of a developmental approach (Intervention), as well as results from a quasi-experimental study of its effects on moral sensitivity. Participants (N = 798) included university students who had been found responsible for academic misconduct and completed the Intervention. As hypothesized, participants not only reported greater attentiveness to moral issues after completing the Intervention, they also demonstrated greater awareness of the moral values related to academic misconduct. The study aimed to establish an assessment model for mathematics teachers' knowledge of students' misconceptions in the Space and Shape domain, develop the testing tool, investigate and analyse the overall and differences in performance, and propose suggestions for improvement. The assessment model included content knowledge and performance standards. The content knowledge standard included cognition and content dimensions. The cognition dimension was subdivided into predicting the misconceptions, interpreting the misconceptions, analysing the reasons and teaching strategies to correct misconceptions. According to the division of Space and Shape in official curriculum standard, the content dimension was subdivided into Recognition of graphics, Measurement, Motion, and Location. 701 Chinese mathematics teachers from 8 provinces were involved to verify the assessment model. Item response theory was used to estimate teachers' performance, and combinations of response categories was used to divide the teachers' performance into three response levels: Low, Medium and High. 81% of teachers were performing at Medium and High response levels. Teachers performed the worst in Recognition of graphics and the best in Measurement. The worst performance occurred in analysing the reasons, the best in interpreting the misconceptions. Around 1.7 billion children lack access to surgical care worldwide. To reinvigorate the efforts to address these disparities and support work to address global challenges in surgery, anesthesia, emergency, and critical care, the World Health Assembly passed World Health Organization Resolution World Health Assembly 76.2: Integrated emergency, critical and operative care for universal health coverage and protection from health emergencies (ECO) in 2023. This resolution highlights the integral role of surgery, anesthesia, and perioperative care in health systems. However, understanding how best to operationalize this resolution is challenging. We review the ECO resolution and highlight points that the pediatric surgical and anesthesia community can leverage to advocate for its recommendations for operative care. [ABSTRACT FROM AUTHOR]

Purpose: To provide a synthesis of the published evidence pertaining to the intergenerational health effects of parental preconceptional exposure to ionizing radiation in humans. Methods: The study populations are the descendants of those who were exposed to ionizing radiation prior to conception. A Boolean search identified publications for review in accordance with Office of Health Assessment and Translation guidelines. Initially, a risk of bias assessment was conducted for each published study and relevant data extracted. Information was organized into adverse health outcome groups and exposure situations. To make an assessment from the body of evidence within each group, an initial confidence rating was assigned, before factors including inconsistencies between studies, magnitude of effect, dose response and confounders were considered. From this, 'an effect', 'no effect' or whether the evidence remained 'inadequate' to determine either effect or no effect, was ascertained. This assessment was based primarily upon the author's conclusions within that evidence-base and, by binomial probability testing of the direction of effect reported. Results: 2441 publications were identified for review which after screening was reduced to 127. For the majority of the adverse health groups, we find there to be inadequate evidence from which to determine whether the health effect was, or was not, associated with parental preconceptional radiation exposure. This was largely due to heterogeneity between individual study's findings and conclusions within each group and, the limited number of studies within each group. We did observe one health grouping (congenital abnormalities) in occupationally exposed populations, where an increase in effect relative to their controls or large magnitude of effects, were reported, although it is noted that the authors of these studies interpreted their findings as most likely not to be associated with parental radiation exposure. Conclusions: We find there to be a lack of evidence to enable the formal assessment of radiation-related adverse effects in offspring of exposed humans. This is not the same as there being no clear evidence that effects may occur but does infer that if adverse health effects do arise in children of exposed parents, then these effects are small and difficult to reproducibly measure. Inconsistencies in designing studies are unavoidable, however we highlight the need for an element of standardization and, more sharing of primary datasets as part of open access initiatives, in order for future reviews to make reasonable conclusions.
