Introduction: Benign prostatic hyperplasia (BPH) is a common urological condition, particularly among middle-aged and elderly men. Trans urethral resection of prostate (TURP) has some drawbacks, particularly concerning ejaculatory function. Ejaculation preserving TURP (Ep-TURP) is one such technique that aims at preserving the tissues that are primarily responsible for antegrade ejaculation. Methods and material: In this prospective study, patients with bothersome LUTS were randomized into Ep-TURP and standard TURP groups. Supramontal tissue was preserved in Ep-TURP while standard TURP group had resection of the entire prostatic tissue up to the verumontanum. The outcomes with regard to IPSS, Qmax, IIEF(Q9), Ejaculation and PVR were studied. Ejaculation projection score was separately calculated and assessed. Results: About 60 patients were studied, with 30 in each group. At 6 months' follow-up, complication rates were the same in both groups. The IPSS, Qmax, IIEF were similar and comparable. There was no change in erection in all patients (P = 0.559). The ejaculation was well preserved in Ep-TURP (p<0.001). Ejaculation projection score was maintained at 3.77 in the Ep-TURP group while in the standard group it was 0 (P<0.001). Conclusions: Ep-TURP is a safe and a cost-effective method of preserving ejaculation in patients undergoing TURP. This procedure adds a better quality of life that greatly aids in enhancing the improving his psychological outlook towards life. Ep-TURP has now emerged as the standard procedure of choice in sexually active young males needing TURP, as the degree of ejaculation is largely well preserved in patients undergoing this procedure. Objectives: The objectives of this study were to detect whether the use of alpha-blockers in posterior urethral valve (PUV) pediatric patients after valve ablation will improve the persistent obstructive symptoms despite the absence of obstruction and if there are associated side effects of its use. Patients and Methods: A prospective, single-blinded randomized study was conducted at the urology department of two hospitals on 50 male children between September 2019 and June 2021 with PUV. All children were treated by endoscopic ablation of PUV using the cold knife and were followed clinically for voiding symptoms and with ultrasonography and laboratory tests. All patients underwent second-look cystoscopy 1 month after primary valve ablation to see residual valves as a routine procedure confirming no remnant of the valve and still complaining of obstructive symptoms. They were divided into two equal groups 25 patients each. Group A was given alpha-blockers and Group B placebo for 1 month. Results: Marked improvement of obstructive symptoms in Group A reaching about 90% (21 patients), whereas no mentioned improvement in Group B was noticed with no side effects of both medication the alpha-blocker and the placebo during its use. Conclusion: The use of alpha-blockers improves the obstructive symptoms in pediatric patients with PUV after valve ablation and in the absence of any further urethral obstruction with no side effects noticed during the period of its use. Introduction and hypothesis: Postpartum urinary retention (PUR) may cause long-term urogenital tract morbidity. The incidence ranges from 0.18 to 14.6%, but the importance of prompt diagnosis and appropriate management is often underappreciated. The paucity of data on long-term outcome after PUR contributes to these drawbacks. The aim of this study was to assess long-term persistence of elevated PVR (post-void residual urine) volume after PUR. Pathophysiology, risk factors and management of PUR are reviewed. Methods: In our tertiary referral urogynecology unit in the University Women's Hospital of Bern, Switzerland, all patients who were referred for PUR were asked to participate in this study. PVR was measured sonographically every 2 days until day 15, then after 6, 12, 24 and 36 months and, if increased, the patients were instructed to perform clean intermittent self-catheterization. If retention persisted longer than the lactation period, multichannel urodynamics was performed. Results: Sixty-two patients were included. The median PVR normalized at day 7. Long-term voiding disorders were found in 8.2%, 6.7%, and 4.9% after 1, 2, and 3 years respectively. Multichannel urodynamics confirmed in all patients with persisting retention an acontractile detrusor and de novo stress urinary incontinence in 4 cases. Quantile regression did not reveal any factor contributing to earlier recovery. Eighty-nine percent of the patients with PUR had operative vaginal deliveries, emphasizing the importance of this risk factor for PUR. Conclusions: In most cases PUR resolves early, but voiding difficulties persist more often than previously thought, and for these patients the consequences are devastating. Introduction Prolonged recovery time (RT) affects patient's quality-of-life adversely. Shorter dialysis RT in home haemodialysis (HHD) noted in observational studies has been attributed to intensive dialysis regimens. Home patients adopt different haemodialysis (HD) schedules. There is insufficient literature on RT in HHD patients receiving different HD schedules. Methods Data was prospectively collected for BASIC-HHD study, a multicentre study of home and hospital HD across five tertiary centres in the UK. Two hundred and eighty-eight patients answered the question, 'how long does it take for you to recover from a haemodialysis session?' Ninety-one patients (31.8%) of study population performed HHD. Participants completed neuropsychometric tests and depression and anxiety screening inventories. Results Recovery time is longest amongst 'in-centre' HD recipients (Mean 193 min; SD 295.37) and significantly higher than the mean RT of home HD recipients (Mean 67.3 min; SD 86.8). Within the home setting, RT was not significantly different between the intensive HD and conventional HD (67.8 vs 66.5 min; P > 0.05) groups but higher residual urine volumes in the standard home group had significantly shorter RT. Location of HD, not intensity, remained significant ( P = 0.001) in the unadjusted and adjusted multivariable analysis. Longer RT was associated with female gender, unemployed or retired 'work' status, 'non-white' ethnicity, lower predialysis systolic blood pressure and greater depression screening score. Conclusions Home-based HD and higher residual urine volumes are significantly associated with shorter RT. This home advantage for RT may be sustained by preserving residual renal function (in early stages) and increasing HD intensity (in later stages) of dialysis therapy. [ABSTRACT FROM AUTHOR]

Introduction. Complex urodynamic investigation adds objective functional data to the data obtained by clinical examination and imagistic. This investigation is started with the filling of the bladder, cystomanometry and continued with the data from the micturition phase, the pressure/flow study. The study brings valuable data to diagnose overactive bladder, bladder outlet obstruction and detrusor sensation and contractility. Method. This paper is reviewing the symptoms of the lower urinary tract and the techniques to perform the different urodynamic tests according to the standards of the International Continence Society (ICS). A literature search on MEDLINE using the terms: urodynamics, female urinary incontinence, prolaps, voiding dysfunction, was performed. Three recent papers were selected and commented with referrals to clinical practice. Conclusions. Complex urodynamic tests are providing valuable data to help understand urinary symptoms especially when these are combined, from both the filling and the emptying phase, when prolapse is present and when residual urine is high on ultrasound. Objective: Bladder outlet obstruction may occur after any incontinence surgery and may present as OAB, hesitancy and or the feeling of incomplete emptying. Aim of this study was to analyze the clinical and urodynamical outcome after urethrolysis in patients presenting with various clinical symptoms after Burch colposuspension for stress urinary incontinence.Study Design: Between January 2005 and December 2014, all patients who presented with symptoms and with bladder outlet obstruction were included. All patients had undergone Burch or Cowan colposuspension for stress urinary incontinence previously. Primary endpoint was the visual analogue scale (VAS) as measurement of patient perceived disease impact. Secondary endpoints were the various domains of the King's Health Questionnaire, urodynamic parameters as detrusor pressure at maximum flow, residual urine and sonographic bladder wall thickness before and six months after intervention.Results: Seventy-two female patients were included in this study whereof 42 suffered from urgency and urge incontinence, 20 from hesitancy and/or slow stream, seven from residual urine of more than 100ml and three from a combination of urgency and residual urine. VAS improved significantly (p<0.0001). Quality of life as determined by the King's Health Questionnaire improved for the domains general health, role limitations, emotions, physical limitations, personal limitations and incontinence impact significantly. Micturition pressure dropped significantly from 43cmH2O (95% CI 19-59cmH2O) to 18cmH2O (95% CI 16-23.5 H2O). Residual urine changed from 110ml (range 20-380ml) to 32ml (20-115ml). Bladder wall thickness decreased from 7mm (95% CI 6.235-7.152) to 5mm (95% CI 5.037-5.607; p<0.01).Conclusion: Urethrolysis may resolve patients' symptoms and lower micturition pressure but irritative symptoms may persist. In 2000, the World Health Organization (WHO) published an ultrasound field protocol for assessing morbidity due to schistosomiasis. The present study aims to review the acceptance of the WHO protocol for Schistosoma haematobium. A PubMed literature research using the keywords 'ultrasound OR ultrasonography (US) AND schistosomiasis,' 'US AND S. haematobium,' 'US AND urinary schistosomiasis' from 2001 through 2014 was performed. Thirty-eight eligible publications reporting on 17,861 patients from 13 endemic and 2 non-endemic countries were analysed. Of these, 33 referred to field studies on 17,317 patients. The Niamey protocol was applied to 15,367/17,317 (88.74 %) patients in 23/33 (69.70 %) of field studies (all studies: 15,649/17,861 [87.61 %] patients (25/38 [68.42 %] studies). The acceptance of the protocol by single country in field studies varied from 0 to 100 %. It varied over time between 55.56 % (5/9) in the period from 2001 to 2004, to 87.50 % (7/8) from 2005 to 2008, to 62.50 % (5/8) from 2009 to 2011 and 75.00 % (6/8) from 2012 through 2014 (all studies: 50 % [5/10], 88.89 % [8/9], 62.50 % [5/8], 63.64 % [7/11], respectively). The Niamey protocol was applied also in 2/5 hospital studies in 282/544 (51.84 %) patients. The usefulness of the WHO protocol for S. haematobium infections is confirmed by its worldwide acceptance. Some simplifications might facilitate its use also for focused ultrasound examinations performed by less skilled examiners. Introduction and hypothesis: The relationship between free flow (FFS) and pressure flow (PFS) voiding studies remains uncertain and the effect of a urethral catheter on flow rates has not been determined. The relationship between residuals obtained at FF and PFS has yet to be established. Methods: This was a prospective cohort study based on 474 consecutive women undergoing cystometry using different sized urethral catheters at different centres. FFS and PFS data were compared for different conditions and the relationship of residuals analysed for FFS and PFS. The null hypothesis was that urethral catheters do not produce an alteration in maximum flow rates for PFS and FF studies. Results: Urethral catheterisation results in lower flow rates ( p < 0.01) and this finding is confirmed when flows are corrected for voided volume ( p < 0.01). FFS and PFS maximum flow rates are lower in women with DO than USI ( p < 0.01). A 6-F urethral catheter does not have a significantly greater effect than a 4.5-F urethral catheter. A mathematical model can be applied to transform FFS to PFS flow rates and vice versa. There was no significant difference between the mean residuals of the two groups (FFS vs PFS-two-tailed t = 0.54, p = 0.59). Positive residuals in FFS showed a good association with positive residuals in the PFS ( r = 0.53, p < 0.01) Conclusions: Urethral catheterisation results in lower maximum flow rates. Objective. A study of the prevalence of urinary voiding dysfunction was carried out preoperatively in 209 patients undergoing lumbar spine operations, using two non-invasive screening tests: free uroflowmetry and ultrasound measurement of residual urine. The value of using the two urodynamic screening tests in the evaluation of a larger group of patients with sciatica from disc lesions or spinal stenosis has not been published as an article before. Material and methods. In 209 consecutive patients, 159 with lumbar disc lesions and 50 with lumbar spinal stenosis, the main indications for operative treatment were pain and/or somatic paresis. In preoperative interviews, information about voiding, anal and sexual functions were obtained. Then uroflowmetry was performed and the volume of residual urine was measured. In this study, the definition of normal bladder function is based on normal flow rates, the shape of the curves, normal voided volumes and the absence of significant residual urine. Bladder dysfunction differs from normal function by one, two, three or all four of these parameters. Results. Normal bladder function was found in 66 patients (31.6%). The other 143 (68.4%) had signs that could indicate sensory paresis and/or neurogenic detrusor underactivity. Seventy-nine of the 143 were also tested 3 months postoperatively. In 58% the bladder function had improved. Conclusions. The very high prevalence of abnormal uroflowmetry and/or postvoiding residual urine suggestive of possible bladder dysfunction (68.4%) indicates that the two tests should be used routinely in cases of lumbar disc lesions and spinal stenosis. Abstract: Urinary retention is complex and may present in various ways as a result of a myriad of pathologic processes. Retention is >10 times more common in men than in women, and acute urinary retention (AUR) is rare in younger men; men in their 70s are at five times more risk of AUR than men in their 40s. Most of the epidemiologic data referred to in the literature are for AUR; data for chronic urinary retention (CUR) are sparse. Management of urinary retention must begin with modifying risk factors for developing AUR by using 5α-reductase inhibitors, follow-up, and early surgical intervention for those who may benefit. Once retention occurs, delay of surgery when possible must be the aim to reduce the risk of perioperative morbidity and mortality as well as to allow the bladder to recover its contractility. Finally, perhaps it is time to use suprapubic catheterisation for retention patients as a first-line approach. [Copyright &y& Elsevier]

Objective. To evaluate the ability of a new 7-cm long female catheter (SpeediCath Compact®; Coloplast A/S, Humlebaek, Denmark) to drain the urinary bladder compared to standard-length female catheters. Material and methods. This was a prospective, single-blind, randomized, crossover study. Twenty-four female intermittent catheter users with neurogenic bladder dysfunction took part. Each test person catheterized three times with the test catheter on one day and three times with a standard-length female catheter on another day. The residual urine (RU) volume in the bladder after catheterization was measured by means of ultrasound. Furthermore, participants were asked to evaluate the length and handling of the test catheter during insertion, and to rate their overall satisfaction with the test catheter. Results. There was no difference between the catheters in terms of volume of RU. One test person could not use the SpeediCath Compact catheter. Twenty-three participants found handling the SpeediCath Compact very easy or easy and rated their overall satisfaction with it as either very satisfying or satisfying. Conclusion. In most females, the SpeediCath Compact catheter is at least as efficient at emptying the bladder as more conventional female catheters. [ABSTRACT FROM AUTHOR]

We evaluated the parameters of preoperative pressure-flow for predicting the postoperative voiding difficulties in women with stress urinary incontinence. The preoperative urodynamic study records of 14 women treated using the tension-free vaginal tape (TVT) procedure were retrospectively analyzed. Of the patients treated with the TVT procedure, urinary retention occurred in one patient, and three had a residual urine volume of more than 30 ml. All patients became completely free of stress urinary incontinence postoperatively. The lowest Pdet max (5 cmH2O) in the preoperative pressure-flow study was found in a patient with a remarkable postoperative residual urine volume of more than 50 ml. The second lowest Pdet max value (8 cmH2O) was seen in a patient with postoperative urinary retention, whose residual urine volume, however, decreased to almost zero 1 year after the operation. The preoperative Pdet max×Qave values were remarkably low for these three patients, including the one with the lowest Pdet max, with a post-void residual urine volume of more than 30 ml. The plots of Pdet max×Qave versus the age of patients show that the Pdet max×Qave values tend to decrease with aging. The preoperative Pdet max×Qave value can be an important parameter for predicting increased residual urine after TVT sling surgery. [ABSTRACT FROM AUTHOR]

Upon feeling the urge to urinate, the urinary bladder contracts, the urethral sphincters relax and urine flows through the urethra. These actions are mediated by the micturition reflex. We investigated the hypothesis that vesical contraction is maintained by positive feedback through continuous flow of urine through the urethra, and that the cessation of urine flow aborts detrusor contraction. Normal saline was infused into the urinary bladders of 17 healthy volunteers (age 35.2 years±4.2(SD); ten women and seven men) at a rate of 100 ml/min. On urge, which occurred at a mean volume of 408.6 ml±28.7of saline, the subject micturated while the vesical and urethral pressures during voiding were being recorded; residual urine was measured. The test was repeated after anesthetizing the urethra with xylocaine gel or, on another occasion, after applying a bland gel . On micturition, the urine was evacuated as a continuous stream without straining; no residual fluid was collected. After urethral anesthetization, the fluid came out of the urethra in multiple intermittent spurts and only with excessive straining. There was a large amount of residual fluid (184.6 ml±28.4). The results of bland gel application showed no significant difference (P>0.05) from those without gel. Detrusor contraction during micturition is suggested to be maintained by positive urethrovesical feedback elicited by the continued passage of urine through the urethra. This feedback seems to be effected through the urethrovesical reflex, which produces vesical contraction on stimulation of the urethral stretch receptors. Abortion of this reflex by urethral anesthetization resulted in failure of detrusor contraction and excessive straining was needed to achieve bladder evacuation in multiple spurts. The urethrovesical reflex is thus assumed to constitute a second micturition reflex responsible for the continuation of detrusor contraction and urination. The Unidad de Urodinamia y Disfunción Sexual del Hospital Español de México presents its experience with the noninvasive urodynamic uroflowmetry study and the quantification of post void residual urine for diagnosis of benign prostatic hyperplasia (BPH). It were analyzed 1,196 men, 440 were considered normal with ages between 40 and 93 years old with a median age of 61.8 years, on the uroflowmetry its Qmax were 22.5 mL/s. It were also clinically, evaluated 756 patients with ages between 40 and 99 years old with a median age 67.3. Its Qmax were 10.45 mL/s and it was undergone the diagnosis of obstructive lower urinary tract syndrome by BPH through uroflowmetry and post void residual urine. Our results confirm the maximum flow rate (Qmax) as the main measurement of the uroflowmetry. The post void residual urine quantification furnishes a good additional information when is related with Qmax and the flow pattern in urinary obstructive problems. It was also studied a randomized group of 84 patients with clinical manifestations of BPH in the last three years. They were evaluated with the IPSS (International Prostate Symptoms Score) in three groups. All of them had a very good correlation among the clinical manifestations (minor, moderate and severe). Qmax and post void residual urine. It was confirmed a statistical relations among the different types of obstructive flow patterns (from I to IV) and unobstructive flow pattern. Qmax is considered the most important value that established correlation with post void residual urine. Objective: To evaluate the relation between post-voiding residual urine (PVR) and urodynamic findings, voiding patterns, and the degree of prolapse in women with cystourethrocele. Patients and Methods: According to the average PVR value measured by means of catheterization after three separate and spontaneous micturitions, 73 women with minimal urethral hypermobility (n = 8) or colpocystocele (n = 65) were subdivided into 3 groups: group 1 = patients with a PVR of ≤50 ml; group 2 = patients with a PVR of >50 ml and ≤150 ml, and group 3 = patients with a PVR of >150 ml and ≤500 ml. The patients underwent a thorough physical examination, and prolapse was evaluated and graded. The women then underwent a complete urodynamic investigation. The 3 groups of patients were statistically correlated according to the urodynamic findings, voiding patterns and degree of prolapse. Results: A large PVR in women with cystourethrocele was often associated with poor maximum flow rate, intermittent flow and with the highest degree of prolapse. Conclusion: The present study suggests that uroflowmetry is a very simple, noninvasive, but also essential method of investigation in evaluating female patients with cystourethrocele, particularly when a large PVR is suspected.Copyright © 2002 S.
