BCG instillations are considered to be the standard of care therapy for superficial urothelial bladder carcinoma. Although serious adverse events are uncommon, the presence of high fever for at least two days in conjunction with systemic and/or local organ manifestations (except for urogenital symptoms), with the exclusion of other causes, suffice for the diagnosis of a disseminated BCG infection. Microbiologic detection of the pathogen is not necessary for diagnosis, as the detection of granuloma is more often successful and sufficient. There has been a widespread call for the development of transformative adaptation knowledge and strategies in the Himalayan region because of the intensifying onset of climate change impacts. But such transformative thinking is absent in much of Himalayan climate knowledge production, which builds on environmental deterministic and techno-managerial renditions of exceptional precarity; advocates for an increase in the scientific and expert driven projects on the ground; and remains rooted in the scalar realities of the nation-state. This paper contributes to the rich scholarship that counterbalances depoliticized renditions of climate change adaptation, by presenting "everyday stories of adaptation" that have emerged from the authors' work alongside Himalayan communities. In this work we ask, who is the subject in Himalayan climate adaptation discourse and policies? And how can their stories help us envision an adaptation praxis, which challenges regional narratives of crisis and provides alternatives to climate reductionist thinking/planning, by foregrounding the intersectionality and plurality of communities and ecologies? The stories come from three parts of the Himalaya: Uttarakhand, Khumbu, and Assam, and highlight the daily labor for adaptation and its mercurial relationship with the labor for survival. We find that intertwined with changing climate-society relationships are, historical caste privileges and changing generational relationships to land; the complicated engagements between indigeneity, communal sovereignty, and exclusionary institutional mandates; and life with ethnoreligious othering in an aqueous and geopolitically fluid borderland. Together these stories witness the relational social-ecological worlds of regional inhabitants, challenging their powerless and pejorative depictions through climate reductive framings. Superconducting electronics are among the most promising alternatives to conventional CMOS technology, thanks to the ultra-fast speed and ultra-high energy efficiency of the superconducting devices. Having a cryogenic control processor is also a crucial requirement for scaling the existing quantum computers up to thousands of qubits. Despite showing outstanding speed and energy efficiency, Josephson junction-based circuits suffer from several challenges such as flux trapping leading to limited scalability, difficulty in driving high impedances, and so on. Three-terminal cryotron devices have been proposed to solve these issues, which can drive high impedances (> 100 k Ω) and are free from any flux trapping issue. In this work, we develop a reconfigurable logic circuit using a heater cryotron (hTron). In conventional approaches, the number of devices to perform a logic operation typically increases with the number of inputs. However, here, we demonstrate a single hTron device-based logic circuit that can be reconfigured to perform 1-input copy and NOT, 2-input AND and OR, and 3-input majority logic operations by choosing suitable biasing conditions. Consequently, we can perform any processing task with a much smaller number of devices. Also, since we can perform different logic operations with the same circuit (same layout), we can develop a camouflaged system where all the logic gates will have the same layout. The structural performance and durability of reinforced concrete (RC) dapped‐end beams may be significantly threated by the effects of deterioration and environmental actions, compromising the structural robustness of important infrastructures such as bridges. The study of the critical role of dapped‐ends has grown in recent years, focusing in particular on the implementation of innovative retrofit solutions capable of extending the service life of these structural elements. In this paper, an experimental investigation on the contribution of Fabric‐Reinforced Cementitious Matrix (FRCM) retrofitting system to the load bearing capacity of RC dapped‐end beams is presented. A set of eight full‐scale dapped‐ends was subjected to monotonic loading tests. The presence of cracks prior to the application of the retrofitting composite, which may represent the actual state of conservation of existing RC half‐joints subjected to heavy traffic loads, is reproduced by testing the specimens up to the reaching of the ultimate design load. Different orientations of the alkali‐resistant glass fabric composing the FRCM system were tested, and the results allow to discuss the effectiveness of the retrofitting solution. Three species belonging to the genus Dolichopoda (Orthoptera; Rhaphidopohoridae) are known so far from the Peloponnese, all endemic to the area. In particular, D. matsakisi is known from two mountains in the North, while D. dalensi is present in the east region. The third species, D. unicolor, is distributed in the southern part of the Peloponnese, inhabiting caves on Mt Taygetos and Mani Peninsula. Recently, extensive sampling work in most of the Peloponnese has led to the discovery of new taxa, morphologically differentiated by the above three known species. To investigate the delimitation of the Peloponnesian species of Dolichopoda, we performed both morphological and molecular analyses. Morphological analysis was carried out by considering diagnostic characters generally used to distinguish different taxa, as the shape of epiphallus in males and the subgenital plate in females. Molecular analysis was performed by sequencing three mitochondrial genes, 12S rRNA, 16S rRNA, and COI, and one nuclear gene, 28S rRNA. Results from both morphological and molecular analyses were used to revise the taxonomic arrangement of the Peloponnesian species. On the whole, we were able to distinguish seven lineages of Peloponnesian Dolichopoda species, of which D. kofinasi n.sp., D.epidavrii n.sp., D. poseidonica n.sp., and D. propanti n.sp. are described as new species. We introduce a novel learning-based method to recover shapes from their Laplacian spectra, based on establishing and exploring connections in a learned latent space. The core of our approach consists in a cycle-consistent module that maps between a learned latent space and sequences of eigenvalues. This module provides an efficient and effective link between the shape geometry, encoded in a latent vector, and its Laplacian spectrum. Our proposed data-driven approach replaces the need for ad-hoc regularizers required by prior methods, while providing more accurate results at a fraction of the computational cost. Moreover, these latent space connections enable novel applications for both analyzing and controlling the spectral properties of deformable shapes, especially in the context of a shape collection. Our learning model and the associated analysis apply without modifications across different dimensions (2D and 3D shapes alike), representations (meshes, contours and point clouds), nature of the latent space (generated by an auto-encoder or a parametric model), as well as across different shape classes, and admits arbitrary resolution of the input spectrum without affecting complexity. The increased flexibility allows us to address notoriously difficult tasks in 3D vision and geometry processing within a unified framework, including shape generation from spectrum, latent space exploration and analysis, mesh super-resolution, shape exploration, style transfer, spectrum estimation for point clouds, segmentation transfer and non-rigid shape matching. Globally, illegal sport hunting can threaten prey populations when unregulated. Due to its covert nature, illegal sport hunting poses challenges for data collection, hindering efforts to understand the full extent of its impacts. We gathered social media data to analyze patterns of illegal sport hunting and wildlife depletion across Brazil. We collected data for 2 years (2018–2020) across 5 Facebook groups containing posts depicting pictures of illegal sport hunting events of native fauna. We described and mapped these hunting events by detailing the number of hunters involved, the number of species, the mean body mass of individuals, and the number and biomass of individuals hunted per unit area, stratified by Brazilian biome. We also examined the effects of defaunation on hunting yield and composition via regression models, rank–abundance curves, and spatial interpolation. We detected 2046 illegal sport hunting posts portraying the hunting of 4658 animals (∼29 t of undressed meat) across all 27 states and 6 natural biomes of Brazil. Of 157 native species targeted by hunters, 19 are currently threatened with extinction. We estimated that 1414 hunters extracted 3251 kg/million km2. Some areas exhibited more pronounced wildlife depletion, in particular the Atlantic Forest and Caatinga biomes. In these areas, there was a shift from large mammals and reptiles to small birds as the main targeted taxa, and biomass extracted per hunting event and mean body mass across all taxonomic groups were lower than in other areas. Our results highlight that illegal sport hunting adds to the pressures of subsistence hunting and the wild meat trade on Brazil's wildlife populations. Enhanced surveillance efforts are needed to reduce illegal sport hunting levels and to develop well‐managed sustainable sport hunting programs. Magnetically actuated lab-on-a-chip (LOC) technologies have enabled rapid, highly efficient separation of specific biomarkers and cells from complex biological samples. Nonlinear magnetophoresis (NLM) is a technique that uses a microfabricated magnet array (MMA) and a time varying external magnetic field to precisely control the transport of superparamagnetic (SPM) beads on the surface of a chip based on their size and magnetization. We analyze the transport and separation behavior of SPM monomers and dimers on four MMA geometries, i.e., circular, triangular, square and rectangular shaped micromagnets, across a range of external magnetic field rotation frequencies. The measured critical frequency of the SPM beads on an MMA, i.e., the velocity for which the hydrodynamic drag on a bead exceeds the magnetic force, is closely related to the local magnetic flux density landscape on a micromagnet in the presence of an external magnetic field. A set of design criteria has been established for the optimization of MMAs for NLM separation, with particular focus on the shape of the micromagnets forming the array. The square MMA was used to detect a model protein biomarker and gene fragment based on a magnetic bead assembly (MBA) assay. This assay uses ligand functionalized SPM beads to capture and directly detect an analyte through the formation of SPM bead aggregates. Human placenta is recognized as a valuable vascular microneurosurgery training model because of its abundant availability, ethical acceptance, and analogous vasculature with other vessels of the human body; however, human placenta laboratory preparation techniques are not well described in the literature. This study outlines a detailed and standardized laboratory protocol for preparation of a color-perfused human placenta model. Survey-based validation of the model is also reported herein. The protocol involved cleaning and cannulation of the umbilical vein and arteries, irrigation with heparin, and storage at 3°C or freezing at −18°C. Before use, arteries were perfused with carmine/cochineal, and veins were perfused with methylthioninium chloride. A questionnaire with 5 questions was administered to 40 participants among attending or resident neurosurgeons, otolaryngologists, and maxillofacial surgeons on 4 consecutive microsurgical courses to assess the reliability of the placenta model. Trainees were divided into 3 groups based on their experience. A χ2 test was used to identify differences between groups. Forty-two placentas were considered appropriate for training and were successfully perfused with dyes. Thirty-three participants completed the questionnaire, of which most, especially advanced and intermediate participants, indicated the placenta as a valuable, accurate, and reproducible model. No differences were observed among the groups. The human placenta is an excellent tool for vascular microneurosurgery laboratory training. Background: Tuberculosis is a worldwide health concern and isoniazid is the most used and considered one of the most effective drugs for its treatment. The “quality” concept must be incorporated into the final pharmaceutical product, according to the quality by design (QbD) definition. Therefore, the determination of analytical test conditions is extremely important and the design of experiments (DoE) becomes a very useful tool. Objective: This paper used the concept of QbD to assist the development of analytical conditions for isoniazid and its respective prodrug, applying HPLC. Method: HPLC analytical methodologies were developed for isoniazid and its succinylated derivative. The experimental design was carried out using three analytical parameters at three levels. Four chromatographic responses were studied. The impact of analytical parameters on chromatographic responses was assessed using a Pareto chart. Regression models were obtained using multiple regression analysis. DoE analysis was conducted using the Minitab® programand the experiments were performed sequentially, with varying factors. Results: We identify three main risk parameters: mobile phase (high), flow rate (moderate), and pH of buffer (moderate). The ratio of mobile phase buffer (X2) and mobile phase pH (X3) had a major influence on the peak resolutions (Y3). The capacity factors for iso-suc (Y1) and isoniazid (Y2) peaks should be within 3–9 and 4–10, respectively. The peak resolutions between iso-suc and isoniazid (Y3) should be above two. Conclusions: We designed 27 experiments, obtaining 1.0 mL/min flow rate, 95% buffer in the mobile phase, and pH 7.0 as the optimal analytical conditions. The two major earthquakes occurred in Northern Italy in 2012 (magnitude 6.1 and 5.8) shown that a serious debate over the structural safety of existing industrial buildings is utterly necessary, also in regions traditionally considered less exposed to the seismic hazard. A promising retrofitting strategy may be represented by the substitution of inefficient secondary roof components with innovative lightweight panels, capable of simultaneously ensuring a global reduction of the inertial forces and an increase of the building energy performances. In this paper, the structural behavior of a multilayered solution, consisting of an advanced cement-based case enclosing a thick insulating core, is presented and investigated on real-scale specimens. The mechanical characterization of the employed materials (High-Performance Fiber Reinforced Cementitious Composite and Textile Reinforced Concrete) at the laboratory level is followed by the discussion of the major challenges arisen in transferring such sensitive construction technologies to the prototyping plant. The relentless pursuit of miniaturization and performance enhancement in electronic devices has led to a fundamental challenge in the field of circuit design and simulation-how to accurately account for the inherent stochastic nature of certain devices. While conventional deterministic models have served as indispensable tools for circuit designers, they fall short when it comes to capturing the subtle yet critical variability exhibited by many electronic components. In this paper, we present an innovative approach that transcends the limitations of traditional modeling techniques by harnessing the power of machine learning, specifically Mixture Density Networks (MDNs), to faithfully represent and simulate the stochastic behavior of electronic devices. We demonstrate our approach to model heater cryotrons, where the model is able to capture the stochastic switching dynamics observed in the experiment. Our model shows 0.82% mean absolute error for switching probability. Inguinal lymph node enlargement is the seepnd-leading regional lymph node enlargement after cervical lymph node swelling. Differential diagnosis is broad and includes harmless local lesions, infections like sexually transmitted diseases, as well as malignant disorders. Crucial investigations should include a comprehensive anamnesis and a clinical examination followed by specific laboratory tests. If there is reason to suspect malignant causes or uncertainty remains, fine-needle puncture or lymph-node excisional biopsy should be performed. In this article, we review lab on a chip (LOC) devices that have been developed for processing magnetically labelled biological analytes, e.g., proteins, nucleic acids, viruses and cells, based on micromagnetic structures and a time-varying magnetic field. We describe the methods that have been developed for fabricating micromagnetic arrays and the bioprocessing operations that have been demonstrated using superparamagnetic (SPM) beads, i.e., programmed transport, switching, separation of specific analytes, and pumping and mixing of fluids in microchannels. The primary advantage of micromagnet devices is that they make it possible to develop systems that control individual SPM beads, enabling high-efficiency separation and analysis. These devices do not require hydrodynamic control and lend themselves to parallel processing of large arrays of SPM beads with modest levels of power consumption. Micromagnet devices are well suited for bioanalytical applications that require high-resolution separation, e.g., detection of rare cell types such as circulating tumour cells, or biosensor applications that require multiple magnetic bioprocessing operations on a single chip. Background: Broad-range fungal inter spacer region (ITS) polymerase chain reaction (PCR) has been evaluated for the detection and identification of fungi in clinical specimens from severely immunocompromised patients, but not in non-selected patients. Thus, the aim of this study was to compare the diagnostic performance of ITS PCR with that of fungal culture and to investigate its clinical impact on the diagnosis of fungal infections in non-immunocompromised patients. The corresponding patients' data were retrieved by detailed medical chart reviews. Results: Results from 251 specimens showed a high concordance of 89.6 % for ITS PCR and fungal culture. The analytical sensitivity and specificity of ITS PCR considering culture as gold standard were 87.7 and 90.3 %, respectively, the positive and negative predictive value (PPV and NPV) were 76 and 95.5 %, respectively. Assessing the clinical probability of a fungal infection based on detailed chart reviews, PCR had a clinical sensitivity of 88.9 %, a specificity of 86.3 %, a PPV of 64.0 % and a NPV of 96.6 %. The overall performance of fungal broad-range PCR was similar to that of culture. Conclusions: Our data show that, in non-selected and non-immunocompromised patients, the performance of ITS PCR is similar to that of culture for detecting fungal infections, not the least because sensitivity of culture in patients under antifungal treatment is surprisingly high. Compared to culture, PCR has the advantage of a rapid time-to-result (approximately two working days), proper identification of rare pathogens, prompt initiation of a species-targeted antifungal treatment, and prospects for automation. Summary Background Besides the six classical exanthems, other exanthems may occur, differing in morphology and causative agent (atypical exanthems). Their aetiological diagnosis is difficult but crucial for both the patient and community concerning issues such as time off school, immunizations, and risks for pregnant women and immunocompromised patients. Objectives To investigate whether morphology, associated symptoms and laboratory results can help to determine the aetiology of atypical exanthems. Methods We studied 112 consecutive out-patients attending two university dermatology departments. Peripheral blood mononuclear cells (PBMC) and throat, rectal and vaginal swabs were studied to identify viral and bacterial growth. Nested polymerase chain reaction was performed on PBMC and plasma using specific primers for herpesviruses. Serology for common viruses was investigated. Results We classified the exanthems into seven morphological patterns: macular erythema (32 patients), papular erythema (eight), maculopapular erythema (42), maculopapular erythema with petechiae (seven), erythema with vesiculation (11), erythema with pustules (five) and urticaria (seven). On the basis of morphology, in concert with the associated symptoms and laboratory results, we found a causal relationship in 76 patients (68%): 25 cases due to drugs, 32 to viruses, 16 to bacteria and three to parasites. A good correspondence between morphology and aetiology was found. The erythematous–vesicular pattern was exclusive to viral infections and was often accompanied by enanthema. The erythemato-pustular and papular patterns were found only in drug-related cases and in some undiagnosed cases. In contrast, the macular and maculopapular patterns were almost evenly distributed among the various aetiologies, although their colour was duskier in the drug-related exanthems. Nonlinear magnetophoresis (NLM) is a novel approach for on-chip transport and separation of superparamagnetic (SPM) beads, based on a travelling magnetic field wave generated by the combination of a micromagnet array (MMA) and an applied rotating magnetic field. Here, we present two novel MMA designs that allow SPM beads to be focused, sorted, and separated on-chip. Converging MMAs were used to rapidly collect the SPM beads from a large region of the chip and focus them into synchronised lines. We characterise the collection efficiency of the devices and demonstrate that they can facilitate on-chip analysis of populations of SPM beads using a single-point optical detector. The diverging MMAs were used to control the transport of the beads and to separate them based on their size. The separation efficiency of these devices was determined by the orientation of the magnetisation of the micromagnets relative to the external magnetic field and the size of the beads and relative to that of micromagnets. By controlling these parameters and the rotation of the external magnetic field we demonstrated the controlled transport of SPM bead-labelled single MDA-MB-231 cells. The use of these novel MMAs promises to allow magnetically-labelled cells to be efficiently isolated and then manipulated on-chip for analysis with high-resolution chemical and physical techniques. Summary Interferon alpha 2h for the treatment of complex haemangiomas of infancy (HI) and childhood. Background Recombinant interferon α has been used with success in the treatment of complex haemangiomas of infancy. Materials and methods Eleven infants and children (10 females and 1 male) with complex haemangiomas (age 3-14 months) were treated with interferon &alpha 2b by subcutaneous infection of up to 1 million UI/m[SUP2]/day three times a week for 2 weeks; then the dosage was increased to 3 million UI/M[SUP2]/day three times a week. The duration of treatment ranged from 6 to 14 months. A decrease of 50% or more in the dimension of haemangiomas that was maintained for at least 6 months during the treatment or after the withdrawal of the medication was considered a consequence of therapy. Results In 1 case regression of H1 was of 20%; in the other 10 cases, regression ranged from 60% to 90%. The side effects of interferon therapy was mild and transient. Our treatment schedule shows that the administration of interferon α 2b three times a week is as effective as the daily administration of the drug. In our opinion interferon &alpha 2b is a safe agent in the treatment of complex haemangiomas of infancy and therefore should be considered a first line therapeutic option. [ABSTRACT FROM AUTHOR]

The genus Dolichopoda (Orthoptera; Rhaphidopohoridae) is present in Italy with 9 species distributed from northwestern Italy (Piedmont and Liguria) to the southernmost Apennines (Calabria), occurring also in the Tyrrhenian coastal areas and in Sardinia. Three morphologically very close taxa have been described in Piedmont and Liguria, i.e., D. ligustica ligustica, D. ligustica septentrionalis and D. azami azami. To investigate the delimitation of the northwestern species of Dolichopoda, we performed both morphological and molecular analyses. Morphological analysis was carried out by considering diagnostic characters generally used to distinguish different taxa, as the shape of epiphallus in males and the subgenital plate in females. Molecular analysis was performed by sequencing three mitochondrial genes, 12S rRNA, 16S rRNA, partially sequenced and the entire gene of COI. Results from both morphological and molecular analyses highlighted a very homogeneous group of populations, although genetically structured. Three haplogroups geographically distributed could be distinguished and based on these results we suggest a new taxonomic arrangement. All populations, due to the priority of description, should be assigned to D. azami azami Saulcy, 1893 and to preserve the names ligustica and septentrionalis, corresponding to different genetic haplogroups, we assign them to D. azami ligustica Baccetti & Capra, 1959, stat. n. and to D. azami septentrionalis Baccetti & Capra, 1959, stat. n. Background: Clostridium difficile infection (CDI) remains a major health problem worldwide. Antibiotic use, in general, and clindamycin and ciprofloxacin, in particular, have been implicated in the pathogenesis of CDI. Here, we hypothesized that antibiotics that are highly active in vitro against C. difficile are less frequently associated with CDI than others. The primary goals of our study were to determine if antibiotic susceptibility and CDI are associated and whether the antimicrobial susceptibility of C. difficile changed over the years. Methods and results: We examined a large panel of C. difficile strains collected in 2006–2008 at the University Hospital of Zurich. We found that the antimicrobial susceptibilities to amoxicillin/clavulanate, piperacillin/tazobactam, meropenem, clindamycin, ciprofloxacin, ceftriaxone, metronidazole and vancomycin were similar to those reported in the literature and that they are similar to those reported in other populations over the last two decades. Antibiotic activity did not prevent CDI. For example, thre use of meropenem, which is highly active against all strains tested, was a clear risk factor for CDI. Most of the antibiotics tested also showed a higher minimum inhibitory concentration distribution than that of EUCAST. All strains were susceptible to metronidazole. One strain was resistant to vancomycin. Conclusions: Antibiotic susceptibilities of the collection of C. difficile from the University Hospital of Zurich are similar to those reported by others since the 1980. Patients treated with carbapenems and cephalosporins had the highest risk of developing CDI irrespective of the antimicrobial activity of carbapenems. [ABSTRACT FROM AUTHOR]

Consequences of a long stay in dog shelter have particular significance, because, since 1991, the Italian law (14/08/1991, n.281) prohibits euthanasia of dogs unless "they are seriously ill, incurable or proven dangerous". Caught dogs are recovered for a quarantine period in the sanitary kennel, if they are not returned to the owner, they are moved to shelters until adoption or death. The aim of this work was to identify the relationship between dogs characteristics and their destiny in order to define useful predictors to better manage the stay of dogs in shelter. We analysed the records of all dogs recovered in a sanitary kennel from 2005 to 2010 and subsequently moved to shelters (n=771). Descriptive and inferential statistics were performed in order to investigate possible factors that might affect adoptability of sheltered dogs. The characteristics of dogs that spent more time in PVCS, before being transfer to the CR, were: large size, male gender and age between 11 months and 2 years (p < 0,05). Male dogs spent more days in CR, as opposed to female (p < 0,05). In our sample 76% dogs were adopted, 18% were still in the shelter, 4% died and 2% were euthanized. Female dogs were adopted more than males; young dogs more than elderly (over seven years); sizes medium and small more than large. It would be interesting use the data from this research and complete them with information regarding dogs behaviour, to better manage dogs during the stay in shelter and to improve their relocation. The genus Dolichopoda (Orthoptera; Rhaphidopohoridae) is present in Italy with 9 species distributed from northwestern Italy (Piedmont and Liguria) to the southernmost Apennines (Calabria), occurring also in the Tyrrhenian coastal areas and in Sardinia. Three morphologically very close taxa have been described in Piedmont and Liguria, i.e., D. ligustica ligustica, D. ligustica septentrionalis and D. azami azami. To investigate the delimitation of the northwestern species of Dolichopoda, we performed both morphological and molecular analyses. Morphological analysis was carried out by considering diagnostic characters generally used to distinguish different taxa, as the shape of epiphallus in males and the subgenital plate in females. Molecular analysis was performed by sequencing three mitochondrial genes, 12S rRNA, 16S rRNA, partially sequenced and the entire gene of COI. Results from both morphological and molecular analyses highlighted a very homogeneous group of populations, although genetically structured. Three haplogroups geographically distributed could be distinguished and based on these results we suggest a new taxonomic arrangement. All populations, due to the priority of description, should be assigned to D. azami azami Saulcy, 1893 and to preserve the names ligustica and septentrionalis, corresponding to different genetic haplogroups, we assign them to D. azami ligustica Baccetti & Capra, 1959, stat. n. and to D. azami septentrionalis Baccetti & Capra, 1959, stat. n. Diarrhea is a frequent and complex entity which is caused by a large number of infectious and non-infectious reasons. A detailed clinical and epidemiological history will define whether an additional laboratory work-up and treatment is indicated. Acute diarrhea is predominantly caused by viral or bacterial pathogens and is usually of short duration without any therapeutic intervention. A parasitic or noninfectious cause must be primarily considered if the diarrhea persists over two weeks. Work-up of stool specimens must target the most likely pathogens or other causes considered. The key element of treatment of acute diarrhea is rehydration. A description of four new species of Dolichopoda Bolivar, 1880 (Orthoptera, Rhaphidophoridae) from Eastern Aegean region (Southern Sporades), including Western Turkey, is reported. This brings to a total of 11 the number of Dolichopoda species recorded for caves of the Aegean area. Overall, these species show a high degree of morphological homogeneity and they are very close to D. paraskevi Boudou-Saltet, 1973 from Crete and D. naxia Boudou-Saltet, 1972 from Cyclades (Naxos Island). The Western Turkish species are morphologically not closely related to the other Anatolian species; this suggests an independent origin for the taxa occurring in the Southern Taurus and Black Sea regions. These new data help to better define the already high level of diversity of the Hellenic Dolichopoda and strengthen the hypothesis that the central area of dispersal for the genus would correspond to the ancient Aegean plate. Background. Broad-range 16S ribosomal RNA (rRNA) gene polymerase chain reaction (PCR) is used for detection and identification of bacterial pathogens in clinical specimens from patients with a high suspicion for infection. However, prospective studies addressing the impact and clinical value of broad-range bacterial 16S rRNA gene amplification for diagnosis of acute infectious diseases in nonselected patient populations are lacking. Methods. We first assessed the diagnostic performance of 16S rRNA gene PCR compared with routine bacterial culture. Second, we addressed prospectively the impact and clinical value of broad-range PCR for the diagnosis of acute infections using samples that tested negative by routine bacterial culture; the corresponding patients' data were evaluated by detailed medical record reviews. Results. Results from 394 specimens showed a high concordance of >90% for 16S rRNA gene PCR and routine bacterial culture, indicating that the diagnostic performance of PCR for acute bacterial infections is comparable to that of bacterial culture, which is currently considered the gold standard. In thisprospective study, 231 specimens with a negative result on routine bacterial culture were analyzed with PCR, and patients' clinical data were reviewed. We found that broad-range 16S rRNA gene PCR showed a sensitivity, specificity, positive predictive value, and negative predictive value of 42.9%, 100%, 100%, and 80.2% for culture-negative bacterial infections. Conclusions. This study defines the role of 16S rRNA gene PCR for diagnosis of culture-negative bacterial infections. Background: Unnecessary or inappropriate use of antimicrobials is associated with the emergence of antimicrobial resistance, drug toxicity, increased morbidity and health care costs. Antimicrobial use has been reported to be incorrect or not indicated in 9-64% of inpatients. We studied the quality of antimicrobial therapy and prophylaxis in hospitalized patients at a tertiary care hospital to plan interventions to improve the quality of antimicrobial prescription. Methodology/Principal Findings: Experienced infectious diseases (ID) fellows performed audits of antimicrobial use at regular intervals among all patients—with or without antimicrobials—hospitalized in predefined surgical, medical, haemato-oncological, or intensive care units. Data were collected from medical and nursing patient charts with a standardized questionnaire. Appropriateness of antimicrobial use was evaluated using a modified algorithm developed by Gyssens et al.; the assessment was double-checked by a senior ID specialist. We evaluated 1577 patients of whom 700 (44.4%) had antimicrobials, receiving a total of 1270 prescriptions. 958 (75.4%) prescriptions were for therapy and 312 (24.6%) for prophylaxis. 37.0% of therapeutic and 16.6% of prophylactic prescriptions were found to be inappropriate. Most frequent characteristics of inappropriate treatments included: No indication (17.5%); incorrect choice of antimicrobials (7.6%); incorrect application of drugs (9.3%); and divergence from institutional guidelines (8%). Characteristics of inappropriate prophylaxes were: No indication (9%); incorrect choice of antimicrobials (1%); duration too long or other inappropriate use (6.7%). Patterns of inappropriate antimicrobial varied widely in the different hospital units; empirical prescriptions were more frequently incorrect than prescriptions based on available microbiological results. Conclusions/Significance: Audits of individual patient care provide important data to identify local problems in antimicrobial prescription practice. In our study, antimicrobial prescriptions without indication, and divergence from institutional guidelines were frequent errors. This work presents a simple, cost-effective, and operational approach to monitor crop water requirements at the regional scale for water management and monitoring purposes. The recommended Food and Agricultural Organization of the United Nations methodology (FAO-56) calculates crop evapotranspiration using crop-specific coefficients (Kc), which vary according to the crop type, health, and phenological stage. This approach, though widely applied for irrigation planning, cannot always match the appropriate crop coefficient with the actual crop phenological stage and health condition, especially in anomalous situations. Previous research demonstrated that crop coefficients and spectral vegetation indexes are correlated. Recent studies have used this relationship with high-resolution satellite data from different sensors to provide information to irrigation advisory services. However, high-resolution data are not feasible for an operational and routine monitoring of water consumption and needs. This paper tests the usefulness of time series of coarse resolution satellite data such as those collected by the moderate-resolution imaging spectroradiometer (MODIS) sensor, to monitor crop coefficients temporal and spatial variability and therefore crop water needs at the regional scale taking advantage of the peculiar characteristics offered by MODIS in terms of high temporal resolution and preprocessed products availability. The outlined methodology takes into account the actual growing stage of the crops and nearly real-time vegetation variations, overcoming some limitations of the traditional FAO approach while preserving the maximum operability. The analysis was carried out in the South Milan agricultural area on data referring to 2003 and 2004. The results agreed with those of other studies and proved to be able to account for the anomalous conditions of the summer in 2003. These results were then compared with those obtained using the traditional FAO crop coefficient curves built with data collected during field campaigns in the same years in rice fields. As science makes progress, proposing new discoveries every day, it is more and more interesting for the astronomical world to perform sub-millimeter observations from very suitable terrestrial sites, due to the purity of the air as well as to other environmental factors. A site with these characteristics has been identified in the area of the DOME C in the South Pole. The problems, both technical and logistic, that the installation of a radio antenna in the South Pole requires, are easily understandable. Proceeding at the same rate as the progress of science, also the technological and industrial capacities improve, so that the problems that the realization of a sub-millimiter Observatory in the South Pole implies, can be overcome. Aim To investigate the molecular phylogenetic divergence and historical biogeography of cave crickets belonging to the genus Dolichopoda (Orthoptera, Rhaphidophoridae). Location Caves in continental and insular Greece. Methods We sequenced 1967 bp of mitochondrial DNA, corresponding to three fragments of the small and large subunit of the ribosomal RNA (16S and 12S rRNA, respectively) and to the subunit I of cytochrome oxidase (COI), to reconstruct phylogenetic relationships among all 30 known Greek species of Dolichopoda. Alternative hypotheses about the colonization of the Hellenic Peninsula by Dolichopoda species were tested by comparing the degree of discordance between species trees and gene trees under four plausible biogeographical scenarios. Results The present study revealed a rather well resolved phylogeny at species level, identifying a number of clades that represent long-separated lineages and diverse evolutionary histories within the genus Dolichopoda. Two main clades were revealed within Hellenic–Aegean species, identifying a north-western and a south-eastern species group. Based on Bayesian analysis, we applied a relaxed molecular clock to estimate the divergence times between the lineages. The results revealed that the origins of eastern Mediterranean lineages are much older than those of previously studied western Mediterranean Dolichopoda. Tests of alternative biogeographical hypotheses showed that a double colonization of the Hellenic Peninsula, following separate continental and trans-Aegean routes during the Messinian stage, best accounts for the present distribution of Greek Dolichopoda species. Main conclusions Reconstruction and biogeographical hypothesis testing indicated that the colonization of Greece by Dolichopoda species comprised two episodes and two different routes. The southern lineage probably arose from a trans-Aegean colonization during the Messinian salinity crisis (5.96–5.33 Ma). The northern lineage could be the result of dispersal from the north through the Balkan Peninsula. The opening of the Mid-Aegean Trench could have promoted an initial diversification within the uprising Anatolian Plateau, while the Messinian marine regression offered the conditions for a rapid dispersal through the whole Aegean–Hellenic region. In addition, climatic events during the Plio-Pleistocene may have been responsible for the speciation within each of the two different phylogeographical units, principally attributable to vicariance events. Abstract: The purpose of this study is to investigate the tension properties and dilatational viscoelastic modulus of various skim milk proteins (whole milk, EDTA-treated milk, β-casein, and β-lactoglobulin) at an oil/water interface at 20 °C. Measurements are performed using a dynamic drop tensiometer for 15,000 s. The aqueous bulk phase is a skim milk simulated ultrafiltrate containing milk protein. At pH 6.7, β-casein appears as the best to decrease the interfacial tension, whereas β-lactoglobulin leads to the highest interfacial viscoelastic modulus value. Whole milk was almost as surface-active as individual β-casein in terms of the final (steady-state) lowering of the interfacial tension, but the rate of tension lowering was smaller. EDTA treatment improved the rate of tension lowering of whole milk. The acidification of milk, from previous measurements, would lead to the enhancement of surface activity. At , the order of effectiveness is pH 4.3 > pH 5.3 = pH 5.6 > pH 6.7 whole milk, suggesting that pH 4.3 whole milk is the best surface active. As compared to pH 6.7 whole milk, the use of pH 5.3 and pH 5.6 milk as surface active would result in the use of milk containing more free β-casein born of pH-dissociated casein micelles. For a long time, Dolichopoda palpata was the only species of this genus thought to live in the cave habitats of the Calabrian region. The results of our study, based on morphological and molecular data, highlight the existence of three species in the Calabria region. In particular, our data demonstrate the occurrence of two well-separated species, one in the northern Tyrrhenian area and one in the Calabrian Ionian area, attesting to the validity of D. calabra Galvagni 1968, and the discovery of a new species, D. apollinea sp. nov. Both species appear to be geographically very limited. Dolichopoda calabra occurs in the mountain region between Scalone Pass in the north and the Savuto Valley in the south (a district called 'Catena Costiera'), whereas D. apollinea sp. nov. appears to be distributed on the eastern slope of Pollino Massif. On the other hand, D. palpata shows a vast distribution, occurring in caves from La Sila in the north to the Aspromonte Massif in the extreme south of Calabria. About 1/3 of the Italian Dolichopoda species are present in this relatively small geographical area, supporting the importance of Calabria as a geographical region characterised by a high level of biodiversity. It shows the maximum values of genetic diversity and species richness in comparison with the rest of the Italian peninsula. This fact could be related to the palaeogeological and palaeoclimatic history of the region, as it was affected by glaciation events and eustatic movements. The combination of these two conditions that occurred during the Pliocene and Pleistocene transformed the Calabria into a vast archipelago of little islands, favouring the isolation of populations and the consequent speciation processes. Background: The prevalence of acute cardiovascular diseases (CVDs) in cancer patients is steadily increasing and represents a significant reason for admission to the emergency department (ED). Methods: We conducted a prospective observational study, enrolling consecutive patients with cancer presenting to a tertiary oncological ED and consequently admitted to the oncology ward. Two groups of patients were identified based on main symptoms that lead to ED presentation: symptoms potentially related to CVD vs. symptoms potentially not related to CVD. The aims of the study were to describe the prevalence of symptoms potentially related to CVD in this specific setting and to evaluate the prevalence of definite CV diagnoses at discharge. Secondary endpoints were new intercurrent in-hospital CV events occurrence, length of stay in the oncology ward, and mid-term mortality for all-cause. Results: A total of 469 patients (51.8% female, median age 68.0 [59.1–76.3]) were enrolled. One hundred and eighty-six out of 469 (39.7%) presented to the ED with symptoms potentially related to CVD. Baseline characteristics were substantially similar between the two study groups. A discharge diagnosis of CVD was confirmed in 24/186 (12.9%) patients presenting with symptoms potentially related to CVD and in no patients presenting without symptoms potentially related to CVD (p < 0.01). During a median follow-up of 3.4 (1.2–6.5) months, 204 (43.5%) patients died (incidence rate of 10.1 per 100 person/months). No differences were found between study groups in terms of all-cause mortality (hazard ratio [HR]: 0.85, 95% confidence interval [CI] 0.64–1.12), new in-hospital CV events (HR: 1.03, 95% CI 0.77–1.37), and length of stay (p = 0.57). Conclusions: In a contemporary cohort of cancer patients presenting to a tertiary oncological ED and admitted to an oncology ward, symptoms potentially related to CVD were present in around 40% of patients, but only a minority were actually diagnosed with an acute CVD. Background: Rib fractures are one of the most common traumatic injuries and may result in significant morbidity and mortality. Despite growing evidence, technological advances and increasing acceptance, surgical stabilization of rib fractures (SSRF) remains not uniformly considered in trauma centers. Indications, contraindications, appropriate timing, surgical approaches and utilized implants are part of an ongoing debate. The present position paper, which is endorsed by the World Society of Emergency Surgery (WSES), and supported by the Chest Wall Injury Society, aims to provide a review of the literature investigating the use of SSRF in rib fracture management to develop graded position statements, providing an updated guide and reference for SSRF. Methods: This position paper was developed according to the WSES methodology. A steering committee performed the literature review and drafted the position paper. An international panel of experts then critically revised the manuscript and discussed it in detail, to develop a consensus on the position statements. Results: A total of 287 studies (systematic reviews, randomized clinical trial, prospective and retrospective comparative studies, case series, original articles) have been selected from an initial pool of 9928 studies. Thirty-nine graded position statements were put forward to address eight crucial aspects of SSRF: surgical indications, contraindications, optimal timing of surgery, preoperative imaging evaluation, rib fracture sites for surgical fixation, management of concurrent thoracic injuries, surgical approach, stabilization methods and material selection. Conclusion: This consensus document addresses the key focus questions on surgical treatment of rib fractures. The expert recommendations clarify current evidences on SSRF indications, timing, operative planning, approaches and techniques, with the aim to guide clinicians in optimizing the management of rib fractures, to improve patient outcomes and direct future research. A 67-year-old white man is described who had large sclerotic plaques with a violaceous border on his back and abdomen and a scleroderma-like induration on the legs, sparing the feet. Additionally, he had multiple skin-coloured, large, sometimes centrally depressed papules and nodules on the nape of the neck, back and upper limbs. These occurred in both the normal and sclerotic areas. Histologically, large amounts of focal mucin were seen in the upper dermis consistent with the diagnosis of papular mucinosis and associated with typical underlying features of scleroderma. No paraproteinaemia was found. We excluded scleromyxoedema on clinical, histopathological and laboratory grounds and diagnosed our patient as having generalized morphoea with papular mucinosis. Metabolism of estradiol by dorsal and plantar skin of Wistar rats was examined during three consecutive hair cycles. Conversion of estradiol to estrone increased and decreased in parallel with the growth and regression of the hair follicles in the dorsal skin, but was minimal and non-cyclic in the glabrous plantar skin. When the metabolic activity was related to DNA content of the skirt, the data suggested that the changes in activity were due to changes in enzyme activity within the cell as well as changes in the number of cells. Arachnoid cysts are well known to the neurosurgeon, and the evolution of their surgical treatment has followed technological advances in neurosurgical procedures and techniques. With the recent mastering of neuroendoscopy by the neurosurgeons, it is becoming the modality of choice for the treatment of arachnoid cysts. A neonate harboring a middle parasagittal arachnoid cyst benefited from stereotactically guided endoscopy. Outgassing from spacecraft materials is known to degrade performance of various components, of which particularly affected are cold and cryogenic surfaces. For those surfaces that suffer from performance losses due to molecular contamination, it is important to assess the contamination levels at an early stage of a project to apply necessary risk mitigation and/or risk avoidance. The focus of this work is on the reemission parameters of the molecular contamination model used in the simulations and improvement of the long-term predictions of in-flight contamination levels. This is realized by experimentally finding a number of sticking coefficients for a set of chemical species released from a contaminated surface during the thermogravimetric analysis. Chronic generalized pruritus is a common symptom. Dermatological causes must be distinguished from non-dermatological causes. Non-dermatological chronic pruritus has many causes, such as systemic, infectious, neurological, psychogenic disorders, and drug-related side effects, some of which may be associated with significant morbidity. The possibility of a systemic disease should be considered in patients with generalized pruritus and no signs of primary skin lesions. In addition to a careful history and physical examination, selected laboratory examinations can be helpful in making a diagnosis. Pruritus can be the first sign of a malignant hematological disease. Pruritus associated with solid tumors is not that rare. This article offers an approach to chronic generalized pruritus in adults without concomitant skin changes with a viable clarification strategy and consideration of the most important differential diagnoses. Micromagnetic arrays (MMAs) have proven to be powerful tools for controlling the transport and separation of bioanalytes, i.e., they allow bioanalyte-superparamagnetic (SPM) bead complexes of specific size and magnetization to be moved in a synchronized manner that is precisely controlled with the orientation of an external magnetic field. This article presents a laser-photodetector system for the simple detection of individual SPM beads moving on a specific region of an MMA. This system detects the SPM beads through the change in intensity of reflective light as they move from the highly reflective micromagnetics to the supporting substrate. We demonstrate that this opti-MMA system allowed the size, number, and magnetic and optical properties of the SPM beads to be rapidly determined for regions > 49 µm2 in size. The response of the opti-MMA system was characterized in several optical configurations to develop a theoretical description of its sensitivity and dynamic range. Surgical approaches to orbital apex lesions involve the medial and lateral corridors. The transorbital neuroendoscopic (TONE) approach has been recently proposed as an elegant, bone-sparing, and minimally invasive alternative for tumors of the lateral orbital compartment. However, its effectiveness compared with the standard lateral orbitotomy approach still requires confirmation. The aim of the present report was to describe the key technical aspects of the lateral trans-eyebrow TONE approach and to review the relevant reported data on the surgical management of ectopic orbital intraconal meningiomas. The TONE approach was used in a 63-year-old man with a diagnosis of a left ectopic intraconal meningioma of the orbital apex involving the lateral compartment. The TONE approach involves performing a lateral 3-cm eyebrow skin incision. A subperiosteal dissection of the periorbita permits intermittent medialization of the eyeball and extracapsular access to the orbital apex region. A lateral periorbital incision under full endoscopic view allows for early access to 2 different intermuscular corridors that do not require mechanical retraction of the oculomotor muscles. The reported data included studies of 23 cases of intraconal meningioma, which were usually benign, although the meningothelial variant has been associated with a greater recurrence rate. In our limited experience, the lateral TONE approach proved to be an effective, feasible, versatile, and minimally invasive promising alternative to resect orbital apex meningiomas involving the lateral compartment.
