Academisation of schooling in England is a significant development with consequences for the disavowal of the role of community and democracy in education at the local level and wider resonance for geo-policy jurisdictions where neoliberal education reforms play out. This study analyses the operation of power and control of Multi Academy Trust (MAT) boards and the apparent diminution of local oversight and accountability, the bedrock of an inclusive, democratic polity. The explanatory and conceptual contribution made to understandings of the phenomenon of MATs is seen in the problematising of academisation and MAT formation as a technology for the operation of power, foreclosing democratic community engagement with their local schools. This has implications for the vitality of a democratic polity. Yet, more optimistically, generative possibilities of a renaissance of schools’ relations with their communities and reinvigoration of the public realm are envisioned as restorative of local democratic engagement in school governance. This article analyses, mingles and blends divergent and complementary strands from the thinking of Hannah Arendt (1906–1975) and Sir Alec Clegg (1909–1986), two contemporaneous but different influential public figures and thinkers in the post-World War Two period. The paper uses these strands to construct a critique of the current colonisation of education by neo-liberal economic logics and performative architecture. Their ideas emerge as bulwarks against populist discourse, reductivist, authoritarian framings of education policy and the restrictive and prescriptive direction of education practice. Conceptions of pluralism, imagination, the location of children and young people's perspectives in the adult world, citizenry and engagement with the world are illuminated for their refreshing power to replenish and sustain the conditions for an education that supports human flourishing. There is increasing evidence that musculoskeletal tissues are differentially regulated by sex hormones inmales and females. The influence of sex hormones, in addition to other sex-based differences such as in anatomical alignment and immune-system function, impact the prevalence and severity of disease as well as the types of injuries that affect the musculoskeletal system and the outcomes of prevention measures and treatment. Literature specifically addressing sex differences related to themusculoskeletal system is limited, underscoring the imperative for both basic and clinical research on this topic. This review highlights areas of research that have implications for bone and cartilage health, including growth and development, sports injuries, osteoarthritis, osteoporosis, and bone frailty. It is clear that important aspects of the musculoskeletal system have been understudied. Consideration of how sex hormone therapy will affect musculoskeletal tissues in prepuberty, during puberty, and in adults is vital, yet little is known. This paper reports on a study exploring the impact of the Covid-19 pandemic on secondary school children's mental health and well-being in the North of England. It explores three research questions: to what extent has the Covid-19 pandemic affected secondary school children’s mental health and wellbeing in England? What did students value most for their mental health and wellbeing in a secondary school context during the Covid-19 pandemic? What are the implications for the post-pandemic future? A qualitative multi-method research design was used consisting of an online questionnaire survey (n = 605) and follow-up focus group interviews (n = 16). The findings of the study show that the pandemic and associated restrictions had a detrimental effect on the lives of a very large proportion of the young people in our study, with greater impact on girls than boys. From the analysis the resilience and ability of the participants to “bounce back” from the upheavals caused by the restrictions was apparent. However, for a significant minority the adverse impacts on their mental health and wellbeing continue to affect their lives. The idea of returning to “normal” following a period of crisis and turmoil such as the Covid-19 pandemic, emerges as an unhelpful concept because of the implied associated expectation of a smooth return to the routines prior to the pandemic crisis. This assumption is problematic as this study’s findings demonstrate. The “new” normal is experienced differently by students, especially girls, to what was “normal” experience before the pandemic and this has significant implications for schools. This paper examines and evaluates some aspects of the legacy of the West Riding of Yorkshire County Council and its long serving Chief Education Officer, Sir Alec Clegg, who held the post between 1945 and 1974. Against a subsequent political discourse of markets, choice and autonomy which portrays local authorities as the cause of poor educational outcomes and school failure, political developments have led to a diminution of the role of local authorities, destruction of local democratic accountability and greater centralisation of power and control of education. Drawing on interviews and email correspondence with former pupils, teachers, officers and others of the West Riding, its successor authorities and beyond, we explore the context, tensions, successes and lasting impact of Clegg's ideas and practice, particularly in relation to support for community development and the arts and creativity. The paper also looks at Clegg's influence and legacy with regard to selection for secondary education and the role of middle schools, the development of primary education and the impact on successor authorities to the West Riding and beyond. This article recalls a time when local government infrastructure was strong and a Chief Education Officer's (CEO) vision could be realised across a region, in Clegg's case the West Riding of Yorkshire, one of the largest of the pre-1974 counties in England. It is timely to revisit Clegg's educational leadership and practice from 1945 to 1974, as a valuable source of learning, and challenge to the current ideologically-informed policy rhetoric which has undermined the framework of democratic educational decision-making. The authors identify four possible areas to learn from Clegg's work. Firstly, the need for CEOs or their modern-day equivalents or successors, to consider their potential as political and community leaders; secondly, Clegg's leadership style prefigures some of the recent discussion about the leadership of public services in uncertain times; thirdly a re-evaluation of administration and bureaucracy; and lastly the need for supportive partnerships to enable schools to thrive. Background In many countries, those with lower socioeconomic status are disproportionately affected by poor oral health. This can be attributed, at least in part, to differences in preventive dental visiting. While several theories have been applied to the area, they generally fail to capture the recursive nature of dental visiting behaviour, and fall short of informing the design of complex interventions to tackle inequalities. Objective To undertake a systematic review and synthesis of theory in order to provide an overview of the pathways which bring about socioeconomic inequalities in early dental visiting, and identify possible intervention points. Methods Electronic searching identified 8947 titles and abstracts. Paper screening and citation snowballing left 77 included papers. Drawing on the tenets of Critical Interpretive Synthesis, data extraction involved capturing concepts and relationships and translating these sometimes into synthetic constructs. Results We theorize that at the individual (micro-level), dental visiting behaviour is influenced by: the ' Importance of obtaining care', ' Emotional response' and ' Perceived control', which feed into a balancing of ' Competing Demands' against ' Internal resources' (coping, self-identity), although attendance is tempered by the effective ' Affordability and Availability of services'. Positive Care experiences are theorized to lower the demands and increase internal resources associated with dental visiting. We also outline meso-level factors ' Social norms and sanctions', 'Obligations, expectations and trust', 'Information channels', 'Social structures' and theorize how these can exert an overwhelming influence in deprived areas. Conclusions Socioeconomic inequalities in early dental visiting emerge from several stages in the care-seeking process. Dental visiting behaviour should be viewed not just as a one-off event, but extending over time and social space. Since there is recursivity in peoples' most recent dental experience any future visits we identify that interventions which make care a positive experience for low socioeconomic patients may be particularly beneficial in reducing inequalities. This paper investigates factors influencing women's engagement with diabetes preventative care after a pregnancy with gestational diabetes (GDM) from the perspectives of GPs and women and explores the role of the GP in that care. Qualitative research using semi-structured interviews with women who had experienced GDM (n = 16) and GPs (n = 18) were conducted and a thematic content analysis conducted. Women's interviews explored their experience of GDM, factors influencing, and engagement with, follow-up care for diabetes prevention and role of the GP in that care. GP interviews explored postnatal care provided to women with GDM, the role of the GP in that care and perceived factors influencing a mother's engagement in her self-care. Three themes were identified: (1) advice and testing; (2) role of the GP; and (3) barriers and enablers to care. Significant consensus about the role of the GP and barriers and enablers to care existed. Both groups believed postGDMfollow-up is best done by GPs and suggested recall and reminders would improve care. GPs gave consistent exercise advice, but lacked consensus on follow-up testing, dietary and weight-loss advice. Women's health literacy influenced how they viewed their GPs role. Consensus guidelines on follow-up testing and diabetes prevention advice, tailored advice according to health literacy and addressing barriers to care would likely improve the capacity of GPs to prevent unnecessary conversion to type 2 diabetes in these at-risk women. This paper presents the results from a reliability analysis of the resistance of composite beams in sagging bending designed according to Eurocode 4. Using the EN 1990 methodology, the partial factors γ M for the structural steel, concrete and shear connection were evaluated. The present study extends earlier work by considering geometrical tolerances given within the published European product and execution standards, which were unavailable during the original calibration of Eurocode 4. Furthermore, recently reported European production data on the yield strength of structural steel is also included. The analyses consider test data from 164 beams with full shear connection, partial shear connection, ductile connectors, non-ductile connectors and beams with high strength steel, which ar?e supplemented with over 3 million simulations. It was found in the present work that the current recommended values for γ M were only justified for beams with full shear connection. For beams with partial shear connection, the calculated values of γ M were larger than recommended because the partial factor associated with the uncertainty of the resistance model varied considerably. To remedy this situation, conversion factors that are a function of the overall composite beam depth are proposed which, when applied to the design models, justifies lower partial factors than that currently recommended by Eurocode 4. Composite slabs using cold-formed profiled steel sheeting are widely used in multi-storey steel-framed buildings. This paper presents the results from a comprehensive test programme that considers the longitudinal shear resistance of composite slabs with two types of trapezoidal profiled sheeting, where the only parameter that was varied was the yield strength of the steel. Although the ductility of high-strength steel material used in two of four test series was low, the longitudinal shear behaviour for all of the composite slabs was deemed to be ductile according to Eurocode 4, which meant that the partial connection method was permitted. However, the longitudinal shear resistance was shown to be significantly lower in the specimens that used the higher-strength steel, which clearly demonstrates that the rules given in Eurocode 4, annex B.3.1 for reducing the number of tests lead to unconservative design values. The present article presents an exploratory qualitative process evaluation study of 'Ambassador' participation in CityNet, an innovative information- communication technology-based (ICT) project that aims to build aspects of social capital and improve access to information and services among disadvantaged groups in Nottingham, UK. A purposive sample of 40 'Ambassadors' interviewees was gathered in three waves of data collection. The two emergent analytic themes highlighted how improvements in confidence, self-esteem and social networks produced via participation were mitigated by structural problems in devolving power within the project. Background Context: Imaging modalities used to visualize spinal anatomy intraoperatively include X-ray studies, fluoroscopy, and computed tomography (CT). All of these emit ionizing radiation.Purpose: Radiation emitted to the patient and the surgical team when performing surgeries using intraoperative CT-based spine navigation was compared.Study Design/setting: This is a retrospective cohort case-control study.Patient Sample: Seventy-three patients underwent CT-navigated spinal instrumentation and 73 matched controls underwent spinal instrumentation with conventional fluoroscopy.Outcome Measures: Effective doses of radiation to the patient when the surgical team was inside and outside of the room were analyzed. The number of postoperative imaging investigations between navigated and non-navigated cases was compared.Methods: Intraoperative X-ray imaging, fluoroscopy, and CT dosages were recorded and standardized to effective doses. The number of postoperative imaging investigations was compared with the matched cohort of surgical cases. A literature review identified historical radiation exposure values for fluoroscopic-guided spinal instrumentation.Results: The 73 navigated operations involved an average of 5.44 levels of instrumentation. Thoracic and lumbar instrumentations had higher radiation emission from all modalities (CT, X-ray imaging, and fluoroscopy) compared with cervical cases (6.93 millisievert [mSv] vs. 2.34 mSv). Major deformity and degenerative cases involved more radiation emission than trauma or oncology cases (7.05 mSv vs. 4.20 mSv). On average, the total radiation dose to the patient was 8.7 times more than the radiation emitted when the surgical team was inside the operating room. Total radiation exposure to the patient was 2.77 times the values reported in the literature for thoracolumbar instrumentations performed without navigation. In comparison, the radiation emitted to the patient when the surgical team was inside the operating room was 2.50 lower than non-navigated thoracolumbar instrumentations. The average total radiation exposure to the patient was 5.69 mSv, a value less than a single routine lumbar CT scan (7.5 mSv). The average radiation exposure to the patient in the present study was approximately one quarter the recommended annual occupational radiation exposure. Navigation did not reduce the number of postoperative X-rays or CT scans obtained.Conclusions: Intraoperative CT navigation increases the radiation exposure to the patient and reduces the radiation exposure to the surgeon when compared with values reported in the literature. Intraoperative CT navigation improves the accuracy of spine instrumentation with acceptable patient radiation exposure and reduced surgical team exposure. Surgeons should be aware of the implications of radiation exposure to both the patient and the surgical team when using intraoperative CT navigation. [ABSTRACT FROM AUTHOR]
