BACKGROUND: Merkel cell carcinoma (MCC) is a rare primary cutaneous neuroendocrine carcinoma. Owing to its low incidence, epidemiological data are scarce and have never been analysed in France to identify the main epidemiological trends. METHODS: Data from MCC patients diagnosed between 1998 and 2010 were obtained from 11 French cancer registries in the FRANCIM network. The main epidemiological characteristics of MCC were investigated between 2006 and 2010 because comprehensive data were only available for this period. The main focus was tumour incidence and mortality over time. RESULTS: Between 1998 and 2010, 562 cases of MCC were reported in the registries. From 2006 to 2010 (290 cases), European- and world-standardized incidence rates were 0.26 and 0.43 per 100,000 person-years in men and 0.24 and 0.38 per 100,000 person-years in women. MCC is more frequent in females in France (56.9%) with male/female ratio 1.1. Relative survival rates were 84%, 56% and 42% at one, three and 5 years, respectively. CONCLUSIONS: The incidence of MCC clearly increased over time in all areas under focus. The standardized incidence in France was comparable to the incidence observed in other countries for the same period, but French data are too recent to conclude on an increase in MCC incidence. As part the Iseult/Inumac project, a French-German initiative focused on very high magnetic-field molecular imaging, the Whole Body 11.7 T MRI Magnet currently under development is the world's largest to-date. It is an actively shielded magnet system, manufactured from NbTi superconductor, with a homogeneous field level of 11.75 T within a 90 cm warm bore. It will operate at a current of 1483 A, in nonpersistent mode, in a bath of superfluid LHe at 1.8 K. The stored energy is 338 MJ and the inductance 308 H. The cryostat has external dimensions of 5 m in diameter and 5.2 m in length, the total weight of the magnet is 132 tons. The magnet is serviced by a separate cryogenic and electrical facility forming an integral part of the installation. It is currently being manufactured at Alstom Belfort under the supervision of CEA Saclay. Several reduced scale prototypes, each addressing a specific set of design and manufacturing risks, have been tested. Full-scale serial production of the 170 double pancakes that form the main coil has been finished by Alstom. The project plan includes finishing the cold mass and cryostat assembly in May 2014. Full tests and commissioning of the magnet at 1.8 K will be performed at the Neurospin center upon completion of assembly. The Whole Body 11.7 T MRI Magnet is an actively shielded magnet system, with a stored energy of 338 MJ and an inductance of 308 H. Operating at a homogeneous field level of 11.75 T within a 90 cm warm bore, the cryostat has external dimensions of 4.8 m in diameter and 5.0 m in length. It is part of the Iseult/Inumac project, a French-German initiative focused on very-high-magnetic-field molecular imaging to improve sensitivity, spatial, temporal, and spectral resolution for preclinical and/or clinical MR systems. After the qualification of two first unit lengths of 820 m, the NbTi conductor with a current of 1483 A is now being produced at Luvata Waterbury. Winding of the main coil, made of 170 double pancakes, is starting at Alstom Belfort. Several pieces of equipment have already been delivered to the Neurospin site, CEA Saclay,; including the main refrigerator produced by Air Liquide. Several prototypes have been tested and confirmed the soundness of the magnet design. This paper describes the 11.7 T magnet and the latest progress in its design and fabrication. A Whole Body 11.7 T MRI Magnet is presently being developed at the CEA Saclay for the Iseult/Inumac project, a French-German initiative focused on very-high-magnetic-field molecular imaging to improve sensitivity, spatial, temporal, and spectral resolution for preclinical and/or clinical MR systems. The magnet will be installed at the Neurospin center, Saclay, in 2012. This actively shielded magnet system, with a stored energy of 338 MJ and an inductance of 308 H, has external dimensions of 5 m in diameter and 5.2 m in length. The magnet will operate at a homogeneous field level of 11.75 T within a 90 cm warm bore and at a current of 1483 A. The technological choice for the cryostable winding is a double pancake structure, using NbTi conductors cooled with a pressurized bath of Helium II at 1.8 K. In April 2009, the project passed an important milestone with the publication of the Technical Design Report, which defines the engineering parameters, design of the magnet, and establishes its engineering feasibility. Neurospin, a neuroscience research centre with very high field MRI equipments, just opened in November 2006 at Saclay by the CEA life science division. One of the imaging systems will require an 11.7 T magnet with a 900 mm warm bore. This magnet is currently under development at CEA Saclay, in collaboration with Siemens Medical Solutions and Alstom Magnets and Superconductors, within the framework of the French-German consortium Iseult/INUMAC (Imaging of Neuro disease Using high field. Magnetic resonance and Contrastophores). The main aim of the consortium is to promote magnetic resonance and molecular imaging within high magnetic fields The proposed magnet design is based on conservative options, but definitely unusual construction methods, for an MRI magnet (pancake winding, liquefier, stabilized power supply). These key design points therefore need to be assessed with several prototypes, integrated within a 5 years projected development plan, ending in 2011. The upgrade of JT-60U to JT-60 Super Advanced (JT-60SA), a fully superconducting tokamak, will be performed in the framework of the Broader Approach (BA) agreement between Europe (EU) and Japan. In particular, the Toroidal Field (TF) system, which includes 18 coils, is foreseen to be procured by France, Italy and Germany. This work covers activities from design and manufacturing to shipping to Japan. The present paper is mainly devoted to the analyses that lead to the conductor design and to the technical specifications of the joints for the JT-60SA TF coils. The conductor geometry is described, which is derived from Cable-In-Conduit concept and adapted to the actual JT-60SA tokamak operating conditions, principally the ITER-like scenario. The reported simulations and calculations are particularly dealing with the stability analysis and the power deposition during normal and off-normal conditions (AC losses, nuclear heating). The final conductor solution was selected through a trade-off between scientific approach and industrial technical orientation. Besides, the TF system connections layout is shown, derived from the industrially assessed twin-box concept, together with the associated thermo-hydraulic calculations ensuring a proper temperature margin. We present the main results of the first population-based survival study gathering all the French cancer registries (Francim network). Survival data on 205,562 cancer cases registered between 1989 and 1997 were analyzed. For breast and prostate cancer, the age-standardized five-year relative survivals (RS) were 84 and 77 % respectively. For colon cancer, RS was 55 % in men and 57 % in women. The corresponding results for lung cancer were 12 and 16 %. For most cancer sites, survival was better in women. For breast, prostate, and thyroid cancers, the more recently diagnosed tumours were of better prognosis. For most cancer sites, the excess mortality rate increased with age at diagnosis, but this effect was often limited to the first year after diagnosis. [ABSTRACT FROM AUTHOR]

However, the factors explaining this increase have not been clearly established. Some studies mention the effects of radioactive fallout, particularly after the accident in Chernobyl. Another probable cause is related to progress in medical practice, and particularly in diagnosis. In this article, we describe time trends in the incidence of papillary and follicular cancers, taking into account the size of the tumour at the time of diagnosis. The analysis was carried out on cases from six French cancer registries for the period 1983-2000. Anatomopathological reports concerning 3381 cancer cases were systematically recoded and centralised, following ICDO-3 rules. Over the whole period, the annual percent change of the incidence of papillary cancers was +8.13% and +8.98%, respectively in men and in women. For micropapillary carcinomas (< or = 10 mm), this increase was respectively +12.05% and +12.85%. There is no significant effect of period apart from micropapillary carcinomas in women. However, a birth cohort effect exists for some groups. This effect corresponds to an acceleration in the risk for people born after the 1930s. For the most recent period (1998-2000), half the cases of papillary cancer were micropapillary carcinomas, and for one third of these, the tumour was < or = 5 mm. Our description of a time trend of incidence as a function of tumour size supports the hypothesis of the role of medical practice in a context of high prevalence.
