The transport of particles through channels is of paramount importance in physics, chemistry, and surface science due to its broad real world applications. Much insight can be gained by observing the transition paths of a particle through a channel and collecting statistics on the lifetimes in the channel or the escape probabilities from the channel. In this paper, we consider the diffusive transport through a narrow conical channel of a Brownian particle subject to intermittent dynamics, namely, stochastic resetting. As such, resetting brings the particle back to a desired location from where it resumes its diffusive phase. To this end, we extend the Fick–Jacobs theory of channel-facilitated diffusive transport to resetting-induced transport. Exact expressions for the conditional mean first passage times, escape probabilities, and the total average lifetime in the channel are obtained, and their behavior as a function of the resetting rate is highlighted. It is shown that resetting can expedite the transport through the channel—rigorous constraints for such conditions are then illustrated. Furthermore, we observe that a carefully chosen resetting rate can render the average lifetime of the particle inside the channel minimal. Interestingly, the optimal rate undergoes continuous and discontinuous transitions as some relevant system parameters are varied. The validity of our one-dimensional analysis and the corresponding theoretical predictions is supported by three-dimensional Brownian dynamics simulations. We thus believe that resetting can be useful to facilitate particle transport across biological membranes—a phenomenon that can spearhead further theoretical and experimental studies. To evaluate the long-term effectiveness and consequences of augmentation cystoplasty in adult patients with small capacity bladder due to genitourinary tuberculosis (GUTB). After approval of the institutional ethical committee, we retrospectively analyzed the database of adult patients with small capacity bladder due to GUTB treated by augmentation cystoplasty from January 2010 to December 2022 at our center. The patients were followed up at 6 weeks postoperatively, then every 3 months for first year and 6 months in second year, and then annually. Patients were assessed for symptoms, renal function, blood biochemistry levels, and ultrasound KUB at each follow-up visit. A total of 40 patients underwent augmentation cystoplasty were included. All patients underwent augmentation cystoplasty either with ileum (n = 35) or sigmoid colon (n = 5). On long-term follow-up, there was statistically significant improvement in bladder capacity, decrease in voiding frequency (P =.00), decrease in eGFR (P =.02) and increase in serum creatinine (P =.02). Significant complications were wound infection 4 (10%), intestinal obstruction 1(2.5%), urinary tract infection 6 (15%), bladder stone 2 (5%), mucus retention 3 (7.5%), hypocalcemia with metabolic acidosis 2 (5%) and need of intermittent catheterization in 11 (27.5%) patients. Multidrug therapy with judicious reconstructive surgery is the optimal treatment modality for GUTB with cicatrization sequelae. Background: We define a "flagship hospital" as the largest academic hospital within a hospital referral region and a "flagship system" as a system that contains a flagship hospital and its affiliates. It is not known if patients admitted to an affiliate hospital, and not to its main flagship hospital, have better outcomes than those admitted to a hospital outside the flagship system but within the same hospital referral region. Objective: To compare mortality at flagship hospitals and their affiliates to matched control patients not in the flagship system but within the same hospital referral region. Design: A matched cohort study Participants: The study used hospitalizations for common medical conditions between 2018-2019 among older patients age ≥ 66 years. We analyzed 118,321 matched pairs of Medicare patients admitted with pneumonia (N=57,775), heart failure (N=42,531), or acute myocardial infarction (N=18,015) in 35 flagship hospitals, 124 affiliates, and 793 control hospitals. Main Measures: 30-day (primary) and 90-day (secondary) all-cause mortality. Key Results: 30-day mortality was lower among patients in flagship systems versus control hospitals that are not part of the flagship system but within the same hospital referral region (difference= -0.62%, 95% CI [-0.88%, -0.37%], P<0.001). This difference was smaller in affiliates versus controls (-0.43%, [-0.75%, -0.11%], P=0.008) than in flagship hospitals versus controls (-1.02%, [-1.46%, -0.58%], P<0.001; difference-in-difference -0.59%, [-1.13%, -0.05%], P=0.033). Similar results were found for 90-day mortality. Limitations: The study used claims-based data. Conclusions: In aggregate, within a hospital referral region, patients treated at the flagship hospital, at affiliates of the flagship hospital, and in the flagship system as a whole, all had lower mortality rates than matched controls outside the flagship system. This experimental study explains the phenomena of secondary flow development due to change in rib perforation and the effect on improvement in THPP values due to variation in β and W/w values of a perforated multi-V rib roughness applied in double-pass parallel flow solar air heater (DPPFSAH). The fixed parameters are e/D = 0.043, p/e = 10, α = 60° and W/H = 12, and the range of variable parameters includes β = 0.0–0.31, W/w = 2–10 and Re = 2000–18,000. The optimum values of Nu/Nus, f/fs, THPP and TEIF are observed as 9.66, 12.31, 3.96 and 1.33, respectively, at β = 0.27 and W/w = 6. The maximum improvement in THPP values was 3.96 times and 1.90 times in DPPFSAH with perforation and continuous rib with a smooth plate, respectively. Double-pass SAH outperforms the single-pass SAH. Horseshoe kidney is a rare congenital anomaly resulting from the fusion of the lower pole leading to medial and caudal deviation of kidneys that pose challenges to successful stone clearance. We evaluated the various patient- and treatment-related factors that can impact successful stone clearance. A total of 34 patients with 36 renal moieties with urolithiasis in horseshoe kidneys from 2015 to 2021 were included with a median age of 38 years (11–56 years). The mean calculus size was 39.27 mm (21–70 mm). Twenty-two patients had left, 10 had right, and 2 had bilateral disease. Twelve patients (33%) had previous interventions done. As per Guy stone score (GSS), 14 patients had grade IV, 16 patients had grade III, and 4 patients had grade II disease. Of these patients, 22 underwent unilateral percutaneous nephrolithotomy (PCNL), 2 underwent bilateral PCNL, 6 had pyelolithotomy, 2 had right nephrectomy, and 2 patients underwent shockwave lithotripsy (SWL). Six patients undergoing pyelolithotomy had complete clearance. In PCNL, infra-costal puncture was done uniformly. Complete clearance was seen in 12 out of 16 with superior calyceal access (75% stone-free rates, SFR), 2 out of 8 with middle calyceal puncture (25% SFR), and both the cases with inferior calyceal puncture (100% SFR). Factors affecting stone clearance are as follows: GSS, i.e., grade < IV vs ≥ IV (71.4 vs 50%, P = 0.42), STONE score < 12 vs ≥ 12 (83.3 vs 28.5%, P = 0.048), stone size < 40 vs ≥ 40 mm (50 vs 71.4%, P = 0.42). Depression, anxiety, sleep disturbances, and fatigue are inadequately addressed comorbidities in granulomatosis with polyangiitis (GPA). We determined the prevalence, severity, determinants, and the impact of these comorbidities on quality-of-life (QoL) in GPA. This observational study included adult GPA patients; patients with RA and lupus were included as comparators. Patient Health Questionnaire-9 for depression, Generalized Anxiety Disorder 7-item scale for anxiety, Epworth Sleepiness Scale for sleep disturbances, and Fatigue Severity Scale for fatigue were administered prospectively to estimate prevalence and severity. QoL and disability were estimated using PROMIS-HAQ, HAQ-health and HAQ-pain. Correlations among these parameters were assessed. Stepwise regression analyses were performed to identify determinants of depression, anxiety, excessive sleepiness, and fatigue. One hundred eighty-one patients—62 GPA [mean age 43 (13) years], 57 RA and 62 SLE- were included. The prevalence of depression (47%), excessive sleepiness (21%), and fatigue (39%) in GPA were comparable to RA and lupus; anxiety was less prevalent (29% versus 46% and 53%, p = 0.02). Severity was mostly mild-moderate. Younger age [OR = 0.93 (0.89–0.98)], higher BMI [OR = 1.2 (1.0–1.4)], and greater disease damage [OR = 2.0 (1.3–3.3)] independently predicted presence of depression. Higher BMI [OR = 1.3 (1.1–1.5)] and concomitant FMS [OR = 80.9 (5.1–1289.2)] were independently associated with excessive sleepiness. No association with disease activity, duration, or gender was seen. GPA patients with depression, anxiety, excessive sleepiness, and fatigue had worse PROMIS-HAQ, HAQ-pain, and HAQ-health. In conclusion, depression, anxiety, sleep disturbances, and fatigue are common in GPA. Although their severity is mostly mild-moderate, they impair QoL significantly. The effect of variation in open area ratio (β) in a multi-V rib roughened Single-Pass Solar Air Heater (SPSAH) is analyzed in this work. Fixing values of other parameters, such as open area ratio (β) and relative roughness width (W/w), were studied with a certain range of Reynolds number (Re) ranging from 2000 to 18000. The improvement in Nusselt number ratio (Nu/Nus) and thermo-hydraulic performance (THPP) and the decline in duct friction ratio (f/fs) support the application of perforation. Background: The term "multimorbidity" identifies high-risk, complex patients and is conventionally defined as ≥2 comorbidities. However, this labels almost all older patients as multimorbid, making this definition less useful for physicians, hospitals, and policymakers. Objective: Develop new medical condition-specific multimorbidity definitions for patients admitted with acute myocardial infarction (AMI), heart failure (HF), and pneumonia patients. We developed three medical condition-specific multimorbidity definitions as the presence of single, double, or triple combinations of comorbidities — called Qualifying Comorbidity Sets (QCSs) — associated with at least doubling the risk of 30-day mortality for AMI and pneumonia, or one-and-a-half times for HF patients, compared to typical patients with these conditions. Design: Cohort-based matching study Participants: One hundred percent Medicare Fee-for-Service beneficiaries with inpatient admissions between 2016 and 2019 for AMI, HF, and pneumonia. Main Measures: Thirty-day all-location mortality Key Results: We defined multimorbidity as the presence of ≥1 QCS. The new definitions labeled fewer patients as multimorbid with a much higher risk of death compared to the conventional definition (≥2 comorbidities). The proportions of patients labeled as multimorbid using the new definition versus the conventional definition were: for AMI 47% versus 87% (p value<0.0001), HF 53% versus 98% (p value<0.0001), and pneumonia 57% versus 91% (p value<0.0001). Thirty-day mortality was higher among patients with ≥1 QCS compared to ≥2 comorbidities: for AMI 15.0% versus 9.5% (p<0.0001), HF 9.9% versus 7.0% (p <0.0001), and pneumonia 18.4% versus 13.2% (p <0.0001). Conclusion: The presence of ≥2 comorbidities identified almost all patients as multimorbid. In contrast, our new QCS-based definitions selected more specific combinations of comorbidities associated with substantial excess risk in older patients admitted for AMI, HF, and pneumonia. Thus, our new definitions offer a better approach to identifying multimorbid patients, allowing physicians, hospitals, and policymakers to more effectively use such information to consider focused interventions for these vulnerable patients. Introduction: Pediatric sudden sensorineural hearing loss (SSNHL) is a rare otological emergency. With the emergence of the Coronavirus 19 pandemic, alcohol-based hand sanitizers are among the essential household items. Many hand sanitizers are frequently coupled with scents that young children may find pleasant. Case Report: A 5-year-old girl presented to our clinic with hearing loss after the consumption of alcohol-based hand sanitizer. A pure tone audiogram showed bilateral SSNHL. The child was prescribed systemic corticosteroids resulted in a slight improvement in hearing thresholds. The child was followed up at 6 and 18 months showing no further improvement in hearing thresholds. Conclusion: Although various infective, vascular, and immune responses have been proposed, alcohol-based hand sanitizer consumption has not been reported to present with SSNHL to the best of our knowledge. In the current scenario of the Coronavirus pandemic, otorhinolaryngologists must keep in mind that SSNHL may occur as a result of hazardous alcohol-based hand disinfectant consumption. A detailed study on Solar Air Heaters (SAHs) with single-pass roughening with plastic net perforated multi-V and continuous multi-V ribs was investigated and compared for its effectiveness. Improvements in the Nusselt number ratio (Nu/Nus) and the friction factor ratio (f/fs) studied with the Reynolds number (Re) ranged from 2000–18000 for a relative roughness height (e/Dh) of 0.043, a relative roughness pitch (P/e) of 10, an aspect ratio (W/H) of 12, and an angle of attack (α) of 60°, maintaining a similar flow scenario. For a syntactically roughened channel with a perforated multi-V rib, the significant enhancement was recorded in thermo-hydraulic performance with a slight decrease in pumping power. The present work deals with the optimization of the process parameters of in situ transesterification of dry spirogyra Algae–Jatropha powder along with engine efficiency and combustion analysis of the prepared biofuel. Three operational parameters, namely catalyst concentration (0–5 wt.%), methanol to dry algae–Jatropha curcas powder (v/v) (20–60%), and reaction time (60–180 min) at a constant reaction temperature of 50 °C, were selected. Response surface methodology (RSM) was used to design the experiments. The maximum biodiesel yield of 88.5% was obtained under the optimized conditions of a catalyst concentration of 3.396% (w/w), methanol/oil ratio of 19.86, and reaction time of 180 min. At varying loads, the performance and emissions of a diesel engine linked to a power source and fueled with various biodiesel mixes (Diesel, B5, B10, and B20) were tested. It was found that BSFC decreased as the applied load increased for all of the evaluated fuels. All of the biodiesel blends had greater BSFC than the diesel fuel. However, a substantial decrease in the emissions, including hydrocarbon (HC) and carbon monoxide (CO), was observed with the increase in NOx emissions. This method of preparing biodiesel will be beneficial in order to cater to the needs of the transportation sector because it has a lower energy consumption and less engine emissions. The present study describes the single-step transesterification method of biodiesel production from high free fatty acid (FFA) waste cooking oil blended with algae oil using a homogeneous base catalyst. Due to high FFA contents, two step transesterification is needed to convert oil into biodiesel and therefore the high FFA content of waste cooking oil is decreased by blending it with low FFA content algae oil, which would further lead only to single step transesterification of low FFA oil. The design and optimization studies were conducted using Response Surface Methodology (RSM). The box-Behnken design technique is applied to optimize the three process parameters, i.e., catalyst concentration (0–2 wt%), methanol concentration (v/v) (20–60%) and reaction time (60–180 min) at a uniform reaction temperature of 50 °C. The result of the current study indicates that an effective biodiesel yield of 92% can be obtained at the optimized condition of catalyst concentration of 1.5% (w/w), methanol/oil ratio of 21:1 and reaction time of 110 min at a constant reaction temperature of 50 °C. This analysis clearly shows that this study can resolve the storage problem of high FFA oils from different feedstock and RSM can be successfully used to model the reaction to maximize the biodiesel yield. After analyzing the effect of perforation in a single pass solar air heater in part A, an experimental comparative study was performed on a Double Pass Parallel Flow Solar Air Heater (DPPFSAH) roughened with continuous and perforated multi-V rib roughness. In DPPFSAH, for perforated rib roughness, the gain in Nu/Nus, f/fs, and Thermo-hydraulic performance parameter (THPP) for W/w = 6 are 8.33, 9.09, and 2.97, respectively. While for continuous rib for W/w = 6 the values of Nu/Nus, f/fs, and THPP are 5.93, 10.83, and 1.82, respectively, as compared to the smooth plate. Fibromyalgia syndrome (FMS) is common in patients of psoriatic arthritis (PsA), but the magnitude of its impact is uncertain. This cross-sectional study evaluated the impact of FMS on health-related quality of life (HRQoL) and disease activity in PsA. Adults classified with PsA (CASPAR criteria) at the rheumatology and dermatology outpatient clinics of PGIMER, Chandigarh, India between January 2014 and June 2015 were recruited. All patients were assessed for FMS using the 2010 ACR criteria. Health-related quality of life was assessed using PROMIS-HAQ, HAQ-pain, HAQ-health and revised fibromyalgia impact questionnaire (FIQR). Disease activity measures (SJC, TJC, BASDAI, enthesitis, dactylitis, PASI) and PROMIS-HAQ were correlated with measures of FMS [FIQR, symptom severity scale (SSS) score and widespread pain index (WPI)]. Multivariate regression analyses were used to identify predictors of PROMIS-HAQ and FMS. Out of 106 PsA patients screened, 102 [50 (49%) females; mean age 43.8 (12.4) years] were included. 19 (18.3%) had FMS. Patients of PsA with FMS had significantly (p < 0.05) higher TJC (14 vs 7), SJC (10 vs 5), BASDAI (6.1 vs 4.1) and enthesitis (53 vs 33%), but no difference in dactylitis, severity of skin disease and disease duration. A significant positive correlation of measures of FMS (FIQR, SSS and WPI) with SJC, TJC and BASDAI was noted. PROMIS-HAQ, HAQ-pain and HAQ-health were significantly worse (p < 0.001) in patients of PsA with coexisting FMS. Presence of FMS was found to be an independent predictor of worse PROMIS-HAQ. Female gender and higher TJC independently predicted presence of FMS. The effect of variation in open area ratio (β) in multi-V rib roughened single pass solar air heater (SPSAH) is computed in this study. Different perforated plastic ribs are used to develop variations in perforation. Four values of the open area ratio (β) ranged from 0 to 0.31, six values of relative roughness width (W/w) ranged from 2 to 10, and nine values of the Reynolds number (Re) ranged from 2000 to 18,000. These values were under consideration, along with fixed values of other parameters. Optimum values of performance parameters achieved at an open area ratio β = 0.27 with W/w = 6. The optimum range of improvement in Nusselt number ratio (Nu/Nus) and thermo-hydraulic performance parameter (THPP) was 6.33–8.19 and 3.78–5.41, while duct friction ratio (f/fs) was recorded within the range of 3.68–4.78 when compared to the flat channel, respectively. Arterial involvement, although rare, accounts for significant mortality and morbidity in patients of Behçet's disease (BD). There is paucity of data on arterial BD. The objective of this 5-year retrospective cohort study was to examine the clinical presentation, pattern of arterial involvement, and treatment outcome in Indian arterial BD patients. Data on demography, clinical presentation, radiology, instituted therapy, vascular interventions and treatment outcomes were recorded and analyzed. Ten (16.9%) out of 59 patients with BD had arterial involvement in 13 vascular territories [mean age 30 (8) years, 9 (90%) males]. Pulmonary artery was most commonly involved (46%), followed by abdominal aorta (15%), femoral artery (15%), descending thoracic aorta (8%), common iliac (8%), and dorsalis pedis artery (8%). Two patients had multi-territory involvement. The median interval between disease onset and development of arterial aneurysms was 3 years (3 months–12 years). Concomitant deep vein thrombosis was seen in 60% cases. Prednisolone and cyclophosphamide were the most common immunosuppressive therapy used; one patient who relapsed on cyclophosphamide responded to infliximab. Five surgical or endovascular interventions were performed. Four patients (40%) died due to aneurysm rupture—all had a delayed diagnosis, and three had pulmonary artery involvement, with death due to massive hemoptysis. Based on the present study, we concluded that arterial involvement in BD is seen predominantly in males and has a high mortality. In contrast to the conventional approach of directly comparing genomic sequences using sequence alignment tools, we propose a computational approach that performs comparisons between sequence generators. These sequence generators are learned via a data-driven approach that empirically computes the state machine generating the genomic sequence of interest. As the state machine based generator of the sequence is independent of the sequence length, it provides us with an efficient method to compute the statistical distance between large sets of genomic sequences. Moreover, our technique provides a fast and efficient method to cluster large datasets of genomic sequences, characterize their temporal and spatial evolution in a continuous manner, get insights into the locality sensitive information about the sequences without any need for alignment. Furthermore, we show that the technique can be used to detect local regions with mutation activity, which can then be applied to aid alignment techniques for the fast discovery of mutations. To demonstrate the efficacy of our technique on real genomic data, we cluster different strains of SARS-CoV-2 viral sequences, characterize their evolution and identify regions of the viral sequence with mutations. Current approach for the detection of cancer is based on identifying genetic mutations typical to tumor cells. This approach is effective only when cancer has already emerged, however, it might be in a stage too advanced for effective treatment. Cancer is caused by the continuous accumulation of mutations; is it possible to measure the time-dependent information of mutation accumulation and predict the emergence of cancer? We hypothesize that the mutation history derived from the tandem repeat regions in blood-derived DNA carries information about the accumulation of the cancer driver mutations in other tissues. To validate our hypothesis, we computed the mutation histories from the tandem repeat regions in blood-derived exomic DNA of 3874 TCGA patients with different cancer types and found a statistically significant signal with specificity ranging from 66% to 93% differentiating Glioblastoma patients from other cancer patients. Hydroxychloroquine (HCQ)/Chloroquine (CQ) has been evaluated for treatment and prophylaxis against SARS-CoV-2 infection in various studies with conflicting results. We performed a systematic review to synthesize the currently available evidence over the efficacy and safety of HCQ/CQ therapy alone against SARS-CoV-2 infection. We searched Embase, PubMed, Web of Science, and Cochrane central for randomized controlled trials (RCTs) and prospective cohort studies published until October 15, 2020 and assessing the efficacy of HCQ alone against SARS-CoV-2 infection. We included studies evaluating HCQ/CQ alone as intervention and placebo/standard care as a control group. Retrospective studies and studies using other drugs (namely azithromycin, corticosteroids, immunomodulators, etc.) we excluded. Thirteen RCTs and three prospective cohort studies were included in this review. We pooled data using a random-effect model. Pooled data from 12 studies (9917 participants) showed that HCQs increase mortality as compared to placebo/standard of care (RR 1.10; 95% CI:1.00–1.20). Hydroxychloroquine did not reduce the need for hospitalization in out-patients (RR 0.57; 95% CI 0.31–1.02). HCQ group has a significantly higher rate of any adverse event (RR 2.68; 95% CI 1.55–4.64), as compared to the control group. Also, using HCQ for prophylaxis against SARS-CoV-2 infection did not reduce the risk of acquiring SARS-CoV-2 infection (RR 1.04; 95% CI 0.58–1.88). HCQ therapy for COVID-19 is associated with an increase in mortality and other adverse events. The negative effects are more pronounced in hospitalized patients. In this work, the experiments of the transesterification process were carried out on jatropha-algae oil blend and the prediction of the synthesized biodiesel was investigated. The study was divided into two parts. In the first part, a series of experiments were employed practically and in the second part, the prediction is made with the artificial neural network (ANN). The ANN with Levenberg-Marquardt (LM) algorithm was trained with topology 4-10-1. The estimated results were compared with the experimental results. An ANN model was developed based on a back-propagation learning algorithm. An R-square value of the model from ANN was 0.9976. Background: Coaching institutes attract students aspiring for admission to professional courses and jobs. Physical stress during coaching includes poor study posture and sitting on chairs improperly in overcrowded classes for prolonged periods. Many students attending the coaching institutes report to outpatient clinics of multiple specialties with musculoskeletal disorders (MSD). Materials and Methods: We carried out a cross-sectional study of 500 coaching students. We ascertained the 12-month MSD (period prevalence) and last 7-day MSD (point prevalence) using the Nordic Musculoskeletal Questionnaire. The duration of attending classes, hours of daily study, and duration of sitting continuously at a stretch were also enquired. Results: A total of 488 responses were retrieved. Males and females accounted for 63.9% and 36.1%, respectively. The respondents' mean age was 18.6 ± 1.06 years; mean body mass index was 21.4; mean duration of attending classes was 15.6 ± 7.66 months; mean hours of daily study were 4.78 ± 1.71 hours; mean duration of sitting continuously at a stretch was 2.2 hours. The overall prevalence of MSD was 87.1%. The mean frequency of MSD per participant was 2.6. Most participants reported pain in the neck region and lower back (43%), followed by ankle/foot (36%), followed by upper back (32%), followed by shoulder (28%); knee, elbow, and wrist/hand were lesser than 20%, while hip/thigh pain was the least common symptom (8%). Conclusion: This study serves to sensitize the medical community to this largely under-reported problem in young individuals who are in the phase of life preparing for their future career while inadvertently risking their long-term health in the process. Biodiesel production from different feedstocks is an effective method of resolving problems related to the fuel crisis and environmental issues. In this study, an adaptive neuro-fuzzy inference system (ANFIS) and the response surface methodology based Box-Behnken experimental design were used to model the parameters of biodiesel production for a jatropha-algae oil blend, including the molar ratio, temperature, reaction time, and catalyst concentration. A significant regression model with an R2 value of 0.9867 was obtained under a molar ratio of 6-12, KOH of 0-2% w/w, time of 60-180 min, and temperature of 35-55°C using response surface methodology (RSM). The ANFIS model was used to individually correlate the output variable (biodiesel yield) with four input variables. An R2 value of 0.9998 was obtained in the training. The amount of non-paraffinic components is directly associated with fuel sensitivity (S), which is an important property for using low-octane and low-cetane fuels in direct injection engines. In this paper, we examine the effects of S and RON (research octane number) on the ignition and combustion behavior of naphtha fuel surrogates in homogeneous mixtures and diesel sprays. Two binary blends (PRF70 and PRF80) and four ternary blends (TPRF70-a, TPRF70-b, TPRF80-a, TPRF80-b), with varying amounts of iso-octane, n-heptane and toluene, are considered. Simulations are performed using a reaction mechanism with 109 species and 543 reactions. The mechanism is validated against the shock tube and rapid compression machine ignition data, and non-reacting spray data. Ignition simulations in homogeneous mixtures are performed using CHEMKIN-Pro for temperature range of 625–1250 K, equivalence range of ɸ = 0.5–2.0, and pressure of 55 bar. The study is then extended to examine the transient ignition and flame structure in liquid fuel sprays in a constant-volume combustion vessel using the CFD software CONVERGE. Results indicate that the temperature dependence of ignition characteristics in both homogeneous mixtures and sprays is strongly influenced by fuel sensitivity. In particular, it affects the NTC behavior and temperature dependence of the 1st and 2nd stage ignition processes. The ignition kernel structure in sprays is also strongly modified by fuel sensitivity, as the ignition kernel in ternary sprays involves richer mixtures, while that in binary sprays contains near stoichiometric mixtures. Consequently, the spray flame structure is also modified by fuel sensitivity. While the spray flame is characterized by partially premixed combustion involving a lean premixed zone (LPZ), a rich premixed zone (RPZ), and a nonpremixed zone (NPZ), the effect of sensitivity is to enhance the relative contribution of RPZ compared to those of NPZ and LPZ. In addition, due to enhanced ignitability, the flame in ternary sprays is located closer to the injector compared to that in binary sprays. The effect of higher RON is to increase the ignition delay time. Consequently, the ignition kernel involves relatively leaner mixtures, and the flame structure is characterized by increased contributions from LPZ compared to RPZ, and the flame liftoff length is increased. Increasing the initial reactor temperature has the opposite effect. In prospective study from November 2011- November 2013, we performed 44 laparoscopic nephrectomies for benign non-functioning kidney diseases. Twenty eight patients underwent laparoscopic transperitoneal nephrectomies (63.6%), ten were laparoscopic assisted (22.7%) and six (13.6%) were converted to open. Patient's age, gender, laterality and etiology of renal failure were noted. Outcomes were measured as operative time, intraoperative and post operative complications, blood loss, pain score and hospital stay. Patients were followed up at one, three and 12 weeks and 6 monthly thereafter. Of the 44, ten (22.7%) were 15-24 years old, 32 (72%) between 25-50 years and two were more than 50 years old. Females were 54.6%. 22 patients had either right or left nephrectomy. Pelviureteric-junction (PUJ) obstruction was the commonest cause, 26 cases (59.0%). Operative time: less than two hours in 30 (68.2%) patients, more than two hours in 14 cases. Blood loss: less than 100 ml in 12 (27.3%), 100-200 ml in 20 (45.4%) and more than 200 ml in 12 (27.3%) patients. All four major complications were converted to open, two had injury to mesocolic veins and two had vascular stapler malfunction. Post-operative complications: surgical site infection (SSI), paralytic ileus and mild grade fever in six cases each and non infected benign intra abdominal collection in two cases. Maximum pain score on POD-1: four in 20 cases (45.7%), two in 24 (54%). Two had pain score between 3-4 three weeks after surgery. Oral intake started by POD-2 in 30 (68.2%) and by POD-4 in 100% cases. 22 (50%) patients were ambulating by POD-2, 16 (36.7%) by POD-4. Our study and randomized and non-randomized published literature report acceptables complication and conversion rates. Background: Neurological outcomes in neonatal hypoxic-ischemic encephalopathy (HIE) continue to be sub-optimal despite therapeutic hypothermia (TH). Cerebral near-infrared spectroscopy provides real-time regional oxygen saturation (CrSO2) that may be a marker of adverse MRI findings and neurodevelopmental outcomes.Aim: The aim of this study was to examine the value of CrSO2 monitoring in infants with HIE undergoing TH.Study Design and Subjects: In this prospective study, CrSO2 was continuously recorded in 21 infants with HIE admitted for TH.Outcome Measures: Brain MRI signal abnormalities at 2weeks were scored in individual brain region and classified as none/mild, moderate and severe. 13 infants completed Bayley Scales of Infant Development (BSID) testing at 18-24months.Results: Between 24 and 36h of life, there was a significant increase in odds of having moderate-severe brain MRI abnormalities with higher absolute CrSO2 values. Per 10% increase in absolute CrSO2, the odds ratio for moderate-severe brain MRI abnormalities was greatest at 30h (OR 3.78; confidence intervals (CI): 1.23-11.6, p=0.011). CrSO2 increased more rapidly in infants with greater injury seen on MRI (0.20/h for MRI scores 0/1, by 0.48/h for MRI score 2, and by 0.68/h for MRI score 3, p=0.05). At 30h, absolute CrSO2 correlated significantly with abnormal MRI findings in basal ganglia (92% vs. 78%, p=0.001), white matter (88% vs. 76%, p=0.01), posterior limb of internal capsule (92% vs. 78%, p=0.001), and brain stem (94% vs. 80%, p=0.03) but not with cortical injury (86% vs. 80%, p=0.17). Higher CrSO2 beyond 24h correlated with greater odds of worse BSID scores.Conclusions: Increasing CrSO2 is associated with moderate-severe brain injury as assessed by MRI. Higher absolute CrSO2 values during TH correlates with subcortical injury on MRI and poor neurodevelopmental outcomes in infants with HIE undergoing TH. The majority of the human genome consists of repeated sequences. An important type of repeated sequences common in the human genome are tandem repeats, where identical copies appear next to each other. For example, in the sequence AGTC\underline TGTGC , $TGTG$ is a tandem repeat, that may be generated from $AGTCTGC$ by a tandem duplication of length 2. In this paper, we investigate the possibility of generating a large number of sequences from a seed, i.e. a small initial string, by tandem duplications of bounded length. We study the capacity of such a system, a notion that quantifies the system’s generating power. Our results include exact capacity values for certain tandem duplication string systems. In addition, motivated by the role of DNA sequences in expressing proteins via RNA and the genetic code, we define the notion of the expressiveness of a tandem duplication system as the capability of expressing arbitrary substrings. We then completely characterize the expressiveness of tandem duplication systems for general alphabet sizes and duplication lengths. In particular, based on a celebrated result by Axel Thue from 1906, presenting a construction for ternary squarefree sequences, we show that for alphabets of size 4 or larger, bounded tandem duplication systems, regardless of the seed and the bound on duplication length, are not fully expressive, i.e. they cannot generate all strings even as substrings of other strings. Note that the alphabet of size 4 is of particular interest as it pertains to the genomic alphabet. Building on this result, we also show that these systems do not have full capacity. The ability to store data in the DNA of a living organism has applications in a variety of areas including synthetic biology and watermarking of patented genetically modified organisms. Data stored in this medium are subject to errors arising from various mutations, such as point mutations, indels, and tandem duplication, which need to be corrected to maintain data integrity. In this paper, we provide error-correcting codes for errors caused by tandem duplications, which create a copy of a block of the sequence and insert it in a tandem manner, i.e., next to the original. In particular, we present two families of codes for correcting errors due to tandem duplications of a fixed length: the first family can correct any number of errors, while the second corrects a bounded number of errors. We also study codes for correcting tandem duplications of length up to a given constant $k$ , where we are primarily focused on the cases of $k=2,3$ . Methods. This prospective observational study included confirmed adult dengue patients hospitalized between August and November 2015 in a tertiary care centre in New Delhi, India. Appropriate statistical tests were used to compare clinicolaboratory characteristics, derive predictors of severe disease and mortality, and compute a predictive score for mortality. Serotyping was done. Results. Data of 369 patients were analyzed (mean age, 30.9 years; 67% males). Of these, 198 (54%) patients had dengue fever, 125 (34%) had dengue hemorrhagic fever (grade 1 or 2), and 46 (12%) developed dengue shock syndrome (DSS). Twenty-two (6%) patients died. Late presentation to the hospital (≥5 days after onset) and dyspnea at rest were identified as independent predictors of severe disease. Age ≥24 years, dyspnea at rest and altered sensorium were identified as independent predictors of mortality. A clinical risk score was developed (12*age + 14*sensorium + 10*dyspnea), which, if ≥22, predicted mortality with a high sensitivity (81.8%) and specificity (79.2%). The predominant serotypes in Delhi (2015) were dengue virus DENV2 and DENV4. Conclusion. Age ≥24 years, dyspnea at rest, and altered sensorium were identified as independent predictors of mortality. Platelet counts did not determine outcome in dengue patients. Timely referral/access to healthcare is important. Histoplasmosis is caused by a dimorphic fungus Histoplasma capsulatum in endemic areas, mainly America, Africa, and Asia. In India, it is being reported from most states; however, it is endemic along the Ganges belt. We report a case of an apparently immunocompetent male who presented with 3-month history of fever, cough, and weight loss with recent onset odynophagia and had hepatosplenomegaly and mucocutaneous lesions over the face. The differential diagnosis of leishmaniasis, tuberculosis, leprosy, fungal infection, lymphoproliferative malignancy, and other granulomatous disorders was considered, but he succumbed to his illness. Antemortem skin biopsy and bone marrow aspiration along with postmortem liver, lung, and spleen biopsy showed disseminated histoplasmosis. This case highlights the need for an early suspicion of progressive disseminated histoplasmosis in the presence of classical mucocutaneous lesions even in an immunocompetent patient suffering from a febrile illness. Economic competitiveness is one of the key factors in making decisions towards the development of waste conversion facilities and devising a sustainable waste management strategy. The goal of this study is to develop a framework, as well as to develop and demonstrate a comprehensive techno-economic model to help county and municipal decision makers in establishing waste conversion facilities. The user-friendly data-intensive model, called the FUN damental E N gineering Principl E s-based Mode L for Estimation of Cost of Energy and Fuels from MSW (FUNNEL-Cost-MSW), compares nine different waste management scenarios, including landfilling and composting, in terms of economic parameters such as gate fees and return on investment. In addition, a geographic information system (GIS) model was developed to determine suitable locations for waste conversion facilities and landfill sites based on integration of environmental, social, and economic factors. Finally, a case study on Parkland County and its surrounding counties in the province of Alberta, Canada, was conducted and a sensitivity analysis was performed to assess the influence of the key technical and economic parameters on the calculated results. Municipal solid waste (MSW) management is becoming a serious issue in all over the world. Anaerobic digestion (AD) is one of the technologies to convert that waste into useful form of energy. But megacities like Singapore having limited resources, cannot fulfill the cow dung or other animal manure requirements in AD. Therefore there is a need to study critically the operating parameters and also the pretreatment technologies available for treating the substrate so that one can get the maximum output with limited input. To fulfill the need, the present paper deals with the review of various operating parameters and their effects on AD. Introduction: Asthma and Chronic Obstructive Pulmonary Disease (COPD) patients constitute a major portion of the inflow of a medical Out Patient Department (OPD). Incorrect use of Metered Dose Inhalers (MDI) is a significant barrier in improving the quality of medical care given to most patients with chronic respiratory diseases. Aim: This study was carried out to analyse the issues pertinent to incorrect MDI use and to assess the impact of a quality improvement initiative involving practical demonstration of correct technique using a standardized checklist. Materials and Methods: This was an interventional pre and post-test study conducted in medical OPD among COPD or asthma patients already on MDI. A baseline assessment of correctness of MDI use technique was done based on ADMIT-GINA guidelines, following which a practical demonstration of correct technique was done. These patients were followed up after one month and their technique was reassessed. The mean score was calculated for each patient pre and post intervention based on the correctness of steps and was compared using paired t-test. Results: Total 56 out of 102 patients completed the study. The mean score of MDI technique knowledge was 3.5±1.5, which increased to 6.7±1.2 after intervention (p<0.001). Only 27 (48.2%) patients reported having received prior educational training regarding MDI use and only 22 (39.3%) patients were using spacers. Conclusion: The current level of knowledge of correct steps of MDI use with spacer is low among patients. The sliding window Lempel–Ziv (SWLZ) algorithm that makes use of recurrence times and match lengths has been studied from various perspectives in information theory literature. In this paper, we undertake a finer study of these quantities under two different scenarios: 1) zero entropy sources that are characterized by strong long-term memory and 2) the processes with weak memory as described through various mixing conditions. For zero entropy sources, a general statement on match length is obtained. It is used in the proof of almost sure optimality of fixed shift variant of Lempel–Ziv (FSLZ) and SWLZ algorithms given in literature. Through an example of stationary and ergodic processes generated by an irrational rotation, we establish that for a window of size nw , a compression ratio given by O(\log nw/{{nw}^{a}}) , where $a$ depends on n_{w} \rightarrow \infty , is obtained under the application of FSLZ and SWLZ algorithms. In addition, we give a general expression for the compression ratio for a class of stationary and ergodic processes with zero entropy. Next, we extend the study of Ornstein and Weiss on the asymptotic behavior of the normalized version of recurrence times and establish the large deviation property for a class of mixing processes. Abstract: The aim of the present study is to investigate engine performance and the exhaust emissions of a diesel engine using oxidatively stabilized biodiesel. Biodiesel fuel used in the study was prepared from Jatropha curcas oil. Different samples with various metal contaminants, generally available in the metallurgy of the containers are oxidatively stabilized with antioxidant (PY) according to EN 14112. The brake specific fuel consumption (BSFC) of biodiesel fuel with and without antioxidant is more than that of petro-diesel fuel. The BSFC of biodiesel fuel with antioxidants is lesser than that of biodiesel fuel without antioxidants, but no specific trends were detected according to the type of metal content and amount of antioxidants. The combustion characteristics and exhaust emissions in diesel engine were not influenced very much by the addition of antioxidants in biodiesel fuel. Melatonin is widely used for sleep disorders in patients with a range of developmental disorders and neurodisabilities, who also frequently have epilepsy. The aim of our review was to examine published data to assess the evidence for melatonin affecting seizure control. The literature search revealed 26 papers apparently reporting an association between melatonin and epilepsy or seizures but seven of these did not provide relevant information. Of the three double-blind, randomised, controlled trials, two showed no overall worsening or improvement in seizures, and one recent trial reported a statistically significant reduction in seizures. The open studies reported conflicting results. The few studies on the effect of seizures on melatonin levels have reported that baseline melatonin levels may be low in patients with uncontrolled epilepsy and that levels increase markedly following seizures. The striking finding of this review is the paucity of relevant data from the remarkably small number of studies. These results allow no firm conclusions to be drawn, although it would seem reasonable to observe that there was no marked overall effect on seizures, neither improvement nor worsening. There is a need for large, well designed, randomised, double-blind, placebo-controlled trials to establish the role of melatonin in either predisposing to or decreasing the likelihood of seizures. There are standards available discussing the minimum specification of oxidation stability of biodiesel while using in IC engine but at the same time there is no specification is available for thermal stability. Specification for thermal stability can be developed if one can develop the relationship between oxidation and thermal stability. Abstract: The vegetable oil, fats and their biodiesel suffer with the drawback of deterioration of its quality during long term storage unlike petroleum diesel because of large number of environmental and other factors making the fuel stability and quality questionable. There are various types of stabilities such as oxidation, storage and thermal, playing key roles in making the fuel unstable. In the present paper the thermal stability of Jatropha curcas biodiesel (JCB) as engine fuel was studied. The thermodynamic parameter of activation energy (E a) of the samples was determined by direct Arrhenius plot. The results show that the thermal degradation of all JCB samples can be treated as a first order reaction. It seems at this stage that the additives under study can offer a significant solution in inhibiting the degradation rate of biodiesel. The pyrogallol (PY) has been found to have more pronounced effect on the onset temperature (T on) as well as on the E a followed by propyl galate (PG)>tert-butyl hydroquinone (TBHQ)>butylated hydroxytoluene (BHT)>butylated hydroxyanisole (BHA). Abstract: Naphthalene degrading bacterial consortium (DV-AL) was developed by enrichment culture technique from sediment collected from the Alang-Sosiya ship breaking yard, Gujarat, India. The 16S rRNA gene based molecular analyzes revealed that the bacterial consortium (DV-AL) consisted of four strains namely, Achromobacter sp. BAB239, Pseudomonas sp. DV-AL2, Enterobacter sp. BAB240 and Pseudomonas sp. BAB241. Consortium DV-AL was able to degrade 1000ppm of naphthalene in Bushnell Haas medium (BHM) containing peptone (0.1%) as co-substrate with an initial pH of 8.0 at 37°C under shaking conditions (150rpm) within 24h. Maximum growth rate and naphthalene degradation rate were found to be 0.0389h−1 and 80mgh−1, respectively. Consortium DV-AL was able to utilize other aromatic and aliphatic hydrocarbons such as benzene, phenol, carbazole, petroleum oil, diesel fuel, and phenanthrene and 2-methyl naphthalene as sole carbon source. Consortium DV-AL was also efficient to degrade naphthalene in the presence of other pollutants such as petroleum hydrocarbons and heavy metals. [Copyright &y& Elsevier]

Abstract: Energy is a basic requirement for economic development. Every sector of Indian economy-agriculture, industry transport, commercial and domestic needs input of energy. The economic development plans implemented since independence have necessarily required increasing amount of energy. As a result consumption of energy in all forms has been steadily rising all over the country. This growing consumption of energy has also resulted in the country becoming increasingly dependent on fossil fuels such as coal, oil and gas. Rising prices of oil and gas and potential shortage in future lead to concern about the security of energy supply needed to sustain our economic growth. Increased use of fossil fuels also causes environmental problems both locally and globally. In view of the fast depletion of fossil fuel, the search for alternative fuels has become inevitable, looking at huge demand of diesel for transportation sector, captive power generation and agricultural sector, the biodiesel is being viewed a substitute of diesel. The vegetable oils, fats, grease are the source of feed stocks for the production of biodiesel. Biodiesel is an engine fuel that is created by chemically reacting fatty acids and alcohol. This usually means combining vegetable oil with methanol in the presence of a catalyst (usually sodium hydroxide). Biodiesel is much more suitable for use as an engine fuel than straight vegetable oil for a number of reasons, the most notable one being its lower viscosity. Abstract: Biodiesel, an ecofriendly and renewable fuel substitute for diesel has been receiving the attention of researchers around the world. Due to heavy import of edible oil, the production of biodiesel from edible oil resources in India is not advisable. Therefore it is necessary to explore non-edible seed oils, like Jatropha curcas (J. curcas) and Pongamia for biodiesel production. The oxidation stability of biodiesel from J. curcas oil (JCO) is very poor and therefore an idea is given to increase the oxidation stability of biodiesel by blending it with petro-diesel. J. curcas biodiesel (JCB), when blended with petro diesel leads to a composition having efficient and improved oxidation stability. The results have shown that blending of JCB with diesel with less than 20% (v/v) would not need any antioxidants but at the same time, need large storage space. Similarly, if the amount of diesel is decreased in the blend, it will require the addition of antioxidant but in lesser amount compared to pure JCB. For the purpose five antioxidants were used namely butylated hydroxytoluene (BHT), tert-butyl hydroquinone (TBHQ), butylated hydroxyanisole (BHA), propyl gallate (PG), and pyrogallol (PY). A B30 blend (30% JCB in the blend of JCB and petro-diesel) has been tested for the same purpose. PY is found to be the best antioxidant among all five antioxidants used. The optimum amount of antioxidant (PY) for pure biodiesel tested for the present experiment is around 100ppm while it is around 50ppm for B30 blend to maintain the international specification of oxidation stability.
