Introduction: There is a high prevalence of cognitive difficulties in MS, but despite this, there are few programmes targeting cognition that focus on the ability to function well in everyday life. The Cognitive Occupation-Based programme for people with Multiple Sclerosis (COB-MS), an occupation-focused cognitive intervention, was developed to address this. It addresses both the functional difficulties and the wide-ranging symptoms that present in MS. Objective: Here we report on the results of a cluster-randomised controlled feasibility trial (ISRCTN11462710; registered 4th September 2019) evaluating the COB-MS in terms of feasibility and initial efficacy as a cognitive intervention for people with MS. Method: The eight-session COB-MS intervention was delivered remotely by occupational therapists to participants with MS in the intervention group. Following the end of the trial the COB-MS was delivered to the wait-list control group. Data was collected from people with MS experiencing cognitive difficulties at baseline, post-intervention, 12-weeks, and 6-month follow-up. The primary outcome measure was the Goal Attainment Scaling at 12 weeks. Data was also collected in the domains of cognition, quality of life, and mood. Results: One hundred and eighteen people with MS and cognitive difficulties were randomised to either usual care (n = 60) or COB-MS intervention (n = 58). Ninety-four participants were retained at 6-month follow-up. The COB-MS was found to be feasible, including trial procedures and protocol. Data indicates that the COB-MS is accepted by participants and had positive impacts on daily life. Those allocated to the COB-MS group had a significant improvement in the primary outcome compared to the control condition. Progression criteria set for the feasibility trial have been met therefore further testing of the COB-MS at a definitive trial is supported by the results. Conclusion: The results provide a strong basis for a pathway to a future definitive trial of COB-MS, with respect to both feasibility and preliminary, clinical efficacy. Traditional methods for quantifying the internal temperature of marine mammals require handling live animals, which is not practical for free-swimming baleen whales. Developing a less invasive, more repeatable method would significantly improve our understanding of whale health and thermal physiology. Infrared thermography (IRT) devices compatible with remotely piloted aircraft systems (RPAS) have facilitated qualitative assessments of heat signatures from marine mammals at sea, but absolute temperatures derived using this approach are rare. The goal of this study was to develop a precise empirical method for estimating intranasal temperatures of baleen whales using RPAS-based IRT. We conducted controlled field experiments and flights over North Atlantic right whales (Eubalaena glacialis, NARWs) to develop and test the methodology. Two approaches were evaluated to estimate intranasal temperatures from IRT sensor intensities: a three-point empirical line regression calibrated per flight using known-temperature objects and a generalized linear model incorporating environmental variables. Controlled field experiments demonstrated that the former approach had a median bias of −0.6°C (interquartile range: 1.5ºC), while the latter approach had unexplained negative proportional bias with increasing true temperature of the target object. After accounting for bias, the former approach yielded an average intranasal temperature of 26.9 ± 1.7°C for 21 unique NARWs. The anatomy of the mysticete upper respiratory tract and physiological heat conservation strategies may explain why estimates were low compared to internal temperatures measured from baleen whales using other techniques (30–39ºC). Variability within whales was less than ± 2°C, supporting the use of these methods to monitor the health of individuals over time. However, variability among whales was greater (up to 7ºC). Improvements in our understanding of whale physiology and respiratory mechanics and advancements in RPAS-based IRT calibrations could make this technology more reliable for assessing individual body temperatures and monitoring populations in the future. Background: Although cervical cancer (CC) is highly preventable through appropriate screening methods like the Papanicolaou (Pap) test, which enables early detection of malignant and precancerous lesions, access to such screening has not been equitable across social groups. Sex workers and people with records of incarceration are among the most under-screened populations in Ontario. Little is known about the acceptability and feasibility of HPV self-sampling (HPV-SS) as an alternative cervical cancer screening method for these groups. This online, community-based mixed-methods pilot study aimed to address this knowledge gap. Methods: Eighty-four under- and never-screened sex workers and ex-prisoners aged 25–69 years and residing in the Greater Toronto Area, were recruited by community peer associates. Participants completed an online survey and viewed short videos about CC and screening with Pap and HPV-SS. Those who opted for HPV-SS conducted the test at one of two collaborating organizations. Results: The median age of participants was 36.5 years. Most had limited knowledge about CC and screening. Approximately 13% identified as non-binary, and 5% as two-spirit or trans men, with the majority having completed secondary education. Of the participants, 88% chose HPV-SS, and one-third tested positive for high-risk HPV types. The ability to self-sample without judgment from healthcare providers was noted as a key advantage. However, there was a need for training on proper HPV-SS techniques. Conclusions: To improve cervical cancer screening among sex workers, increasing awareness through participatory community co-creation of sexual health education is essential. Additionally, offering HPV-SS as a screening option is crucial, given its demonstrated acceptability and feasibility within this population, many of whom lack a primary care provider and face discriminatory attitudes in healthcare settings. Plant growth promotion by Pseudomonas sp. 31-12 incorporated into a lettuce seed pelleting matrix was studied. We examined (1) the effect of five rhizosphere derived bacterial strains on green oak lettuce (Lactuca sativa L.) seed germination, root and shoot growth, as a strain selection step for seed coating and seed pelletizing studies, (2) population stability of Pseudomonas sp. 31-12 incorporated into a pelleting matrix on lettuce seed stored three months at 4 °C, and (3) lettuce growth promotion in the laboratory and greenhouse by Pseudomonas sp. 31-12 coated and pelletized seed. A spontaneous streptomycin mutant of Pseudomonas sp. 31-12 (str) was used to determine population size on seed and roots of 15- and 30-day-old lettuce. The population of Pseudomonas sp. 31-12str on coated and pelleted seed decreased from 104 cfu/seed to 103 cfu/seed after 3 months storage at 4 °C. However, the population exceeded 104 cfu/g root dry mass and 105/g root dry mass after 15 days and 30 days in the greenhouse. Leaf fresh mass was significantly increased (P ≤ 0.05) with Pseudomonas sp. 31-12 seed treatment as compared to noninoculated seed. An increasing body of work has found higher education to be a challenging and stressful environment. Meanwhile, research on workaholism, characterised by an uncontrollable urge to work excessively, has been gaining momentum. However, few studies have explored its impact within higher education. This study addressed this gap by conducting 27 semi-structured interviews with academics in the greater Dublin region, exploring the micro, meso, and macro-level factors influencing workaholism. Using a constructivist grounded theory approach, the study examined workaholism in higher education, unpacking Clark, Smith, and Haynes ([2020]. "The Multidimensional Workaholism Scale: Linking the Conceptualization and Measurement of Workaholism." Journal of Applied Psychology 105 (11): 1281–1307.) recent definition of workaholism as a multidimensional construct comprising motivation, cognition, emotion, and behaviour. Twelve participants displayed tendencies of excessive working and constant thoughts about work, driven by negative feelings when not working. These tendencies were rooted in motivations to gain control and build a personal brand. All elements of workaholism have micro, meso, and macro influences, understood through a transdisciplinary lens. As a result, this study calls for responsible, sustainable management within higher education to avoid the negative impacts of workaholism on academics, students, and institutions. Protected areas provide significant recreational benefits worldwide, yet increasingly the behaviour of recreationalists can come into conflict with conservation goals. Zoning and limitations to access are options to reduce harm done by recreationalists in ecologically sensitive areas, but little is known about the economic impact of these management options. In this paper the contingent behaviour model (CBM) is used to estimate the change in recreational value of a protected coastal site after the implementation of a new zoning plan, which limits access to some areas. Unusually for a CBM, the alternative scenario presented to the recreationalists is an actual, rather than a hypothetical, change. Recreational value is predicted to decrease by €4.96 per annum per user, which is a small fraction of the total value. Results indicate that limiting access to sensitive ecosystems through zoning in protected areas can be done without economically significant reductions in site value. [ABSTRACT FROM AUTHOR]

An international medical graduate (IMG) is a doctor who has received their basic medical qualification from a medical school located in a different country from that in which they practice or intend to practice. IMGs are known to face difficulties in their working lives, including differential attainment in assessment. The objective of this review is to map key concepts and types of evidence in academic and gray literature relating to international medical graduates' experiences of clinical competency assessment and to identify knowledge gaps on this topic by systematically searching, selecting, and synthesizing existing knowledge. All studies will relate to IMGs. The concept of interest will be IMGs' experiences of assessment. The context will be postgraduate, licensing or credentialing medical assessments of clinical competence. This review will be conducted in accordance with the Joanna Briggs Institute (JBI) methodology for scoping reviews. Seven electronic databases will be searched for literature published between 2009 and 2024: the Australian Education Index, British Education Index, ERIC, PubMed, PsycINFO, Scopus, and SocINDEX. Gray literature will be searched using Google, Google Scholar, and published reports from postgraduate training bodies and medical licensing organizations. The Hawaii Ocean Time‐series (HOT) at Station ALOHA (22.75°N, 158°W) in the North Pacific Subtropical Gyre (NPSG) serves as a critical vantage point for observing plankton biomass production and its ecological implications. However, the HOT program's near‐monthly sampling frequency does not capture shorter time scale variability in phytoplankton populations. To address this gap, we deployed the SeaFlow flow cytometer for continuous monitoring during HOT cruises from 2014 to 2021. This approach allowed us to examine variations in the surface abundance and cell carbon content of specific phytoplankton groups: the cyanobacteria Prochlorococcus, Synechococcus, and Crocosphaera as well as a range of small eukaryotic phytoplankton (≤ 5 μm). Our data showed that daily to monthly variability in Prochlorococcus and Synechococcus abundance matches seasonal and interannual variability, while small eukaryotic phytoplankton and Crocosphaera showed the highest seasonal and interannual fluctuations. The study also found that eukaryotic phytoplankton and Crocosphaera had higher median cellular growth rates (0.076 and 0.090h−1, respectively) compared to Prochlorococcus and Synechococcus (0.037 and 0.045h−1, respectively). These variances in abundance and growth rates indicate that shifts in the community structure significantly impact primary productivity in the NPSG. Lead (Pb) is a heavy metal with no clear biological benefit that is toxic to wildlife and humans. Although Pb has been banned from several consumer products, it is not regulated in ammunition for big‐game hunting (e.g., cervids) in New York, USA. Wildlife species that scavenge remains of game harvested with Pb ammunition can be exposed to bullet fragments through ingestion and potentially develop toxicosis. To make evidence‐based decisions for reducing risk from bullet‐derived Pb, we first empirically identified avian and mammalian scavengers of white‐tailed deer (Odocoileus virginianus) remains in New York through an online survey during July‐August 2023, where community members submitted images of scavengers from game cameras. We conducted species vulnerability and utility assessments using published evidence and pre‐existing frameworks. Despite inherent limitations, we documented an extensive array of avian (n = 17) and mammalian (n = 14) scavengers. We estimated bald eagles (Haliaeetus leucocephalus) and golden eagles (Aquila chrysaetos) as most vulnerable to Pb toxicosis, followed collectively by American goshawks (Accipiter atricapillus), American crows (Corvus brachyrhynchos), and common ravens (C. corax). We also determined bald eagles to be best‐suited as bioindicators of bullet‐derived Pb, followed by American goshawks. Red‐tailed hawks (Buteo jamaicensis) and great horned owls (Bubo virginianus) equally ranked third. Integrating vulnerability and utility assessments point to bald eagles as the most vulnerable scavengers to Pb toxicosis and best‐suited bioindicators of bullet‐derived Pb. Because regulation of Pb ammunition for big‐game hunting is an ongoing socio‐political issue, wildlife managers and conservationists can address health risks through monitoring species vulnerable to toxicosis and instituting surveillance programs using the most effective bioindicators. Omaveloxolone is a nuclear factor (erythroid‐derived 2)‐like 2 activator approved in the United States and the European Union for the treatment of patients with Friedreich ataxia aged ≥16 years, with a recommended dosage of 150 mg orally once daily on an empty stomach. The effect of the US Food and Drug Administration (FDA) high‐fat breakfast on the pharmacokinetic profile of omaveloxolone observed in study 408‐C‐1703 (NCT03664453) deviated from the usual linear correlation between fed/fasted maximum plasma concentration (Cmax) and area under the concentration–time curve (AUC) ratios reported for various oral drugs across 323 food effect studies. Here, physiologically based biopharmaceutics modeling (PBBM) was implemented to predict and explain the effect of the FDA high‐fat breakfast on a 150‐mg dose of omaveloxolone. The model was developed and validated based on dissolution and pharmacokinetic data available across dose‐ranging, food effect, and drug–drug interaction clinical studies. PBBM predictions support clinical observations of the unique effect of a high‐fat meal on omaveloxolone pharmacokinetic profile, in which the Cmax increased by 350% with only a 15% increase in the AUC. Key parameters influencing omaveloxolone pharmacokinetics in the fasted state based on a parameter sensitivity analysis included bile salt solubilization, CYP3A4 activity, drug substance particle size distribution, and permeability. Mechanistically, in vivo omaveloxolone absorption was solubility and dissolution rate limited. However, in the fed state, higher bile salt solubilization led to more rapid dissolution with predominant absorption in the upper gastrointestinal tract, resulting in increased susceptibility to first‐pass gut extraction; this accounts for the lack of correlation between Cmax and AUC for omaveloxolone. Omaveloxolone (SKYCLARYS®) is approved for the treatment of Friedreich ataxia (FA) in patients aged ≥16 years in the United States and European Union (EU). The recommended dosage is 150 mg administered orally once daily as three 50‐mg capsules. However, some patients with FA may have oropharyngeal dysphagia or difficulty swallowing whole capsules; therefore, alternate method(s) of administration are needed. A Phase 1 clinical study in 32 healthy volunteers evaluated the relative bioavailability, safety, and tolerability of a single dose of omaveloxolone when capsule contents were sprinkled on and mixed in applesauce compared to when taken as intact capsules. Palatability when sprinkled on and mixed in applesauce was assessed with a questionnaire. After a single 150‐mg dose, the peak and overall exposures of omaveloxolone were similar irrespective of administration method, with the 90% CIs of the geometric least squares mean ratio (%) for maximum plasma concentration (Cmax), AUC0‐t, and AUC0‐∞ within the 80% to 125% reference intervals. Omaveloxolone was absorbed more slowly as intact capsules (median tmax, 10 h) compared with sprinkled capsule contents over applesauce (median tmax, 6 h). With chronic daily administration of omaveloxolone to treat FA, the 4‐h difference in tmax is not considered clinically relevant. Sprinkled omaveloxolone capsule contents on applesauce were well tolerated, with acceptable palatability and no serious adverse events. Given the similar systemic exposure when capsules were swallowed whole, sprinkling omaveloxolone capsule contents on and mixing in applesauce is a feasible alternative method of administering omaveloxolone and has been included in both the United States and EU prescribing information. As is now well-known, COVID-19 is a highly contagious disease, which for some can cause symptoms that last long after initial infection. In 2021, a clinical set of symptoms referred to as long-COVID was identified. For many patients, long-COVID is a confusing and frightening multisystem disease, with the potential for myriad negative psychosocial effects, including significant impacts on employment and mental health, and requiring ongoing care. Research and treatment of long-COVID will be facilitated by a sound measure that addresses aspects of well-being, symptom experiences, and psychosocial impacts among long-COVID patients. The present work addresses this need by presenting the results of the development and preliminary psychometrics for the Long-COVID Well-Being Scale (LCOVID-WBS). In an exploratory factor analysis with a panel of 236 participants, researchers identified four factors: Emotional Strain, Physical Ability Strain, Control of Life, and Overall Evaluation of Health. We observed the low-mass X-ray binary Cyg X-2 for a total of 18 nights over two observing runs in July and September of 2006, using the Otto Struve Telescope at McDonald Observatory and the Rossi X-ray Timing Explorer. Using discrete cross-correlations, we found peaks occurring at near-zero lags in the flaring branch of the colour–colour diagram, which could signify reprocessing, in addition to an anticorrelation within the normal branch. When comparing optical flux to the system's placement on the Z track, two distinct behaviours were seen: (1) a state with no correlation, and (2) a multivalued (horizontal and normal branches)/correlated (flaring branch) state. The correlation was the result of direct steps and more gradual falls to and from the flaring branch, respectively. Finally, we modelled timed spectra with 64 s bins with an extended accretion disc corona model. We found that correlations occurred between the optical and the various fitted parameters, particularly the blackbody normalization (and blackbody radius by extension) in higher intensity regions. Despite this, the Z track location was found to be a far better predictor of physical parameters than the optical flux, with clean correlations seen in every branch of the Z track. The current study investigated adolescents' (N = 213) decision finding processes and affective reactions to interactions on social media via 29 focus groups. As part of a larger study, adolescents participated in focus groups at two time-points across an academic year while participating in a school-based intervention promoting healthy romantic, interpersonal, and family relationships, job readiness, and financial literacy. Qualitative analyses indicated adolescents' experiences and decisions on social media platforms were informed by their awareness of audiences, namely who they thought would view their posts and anticipated responses from "friends," "family members," "fans," "creeps," and "potential employers." The rapid increase in the use of remote platforms in court hearings has fundamentally changed the nature of hearings and produced a productive point of reflection for digital and legal geographies. This paper uses geographical theorisations of space to make sense of digital developments in court space. It proposes that the introduction of remote platforms generates three spatial ruptures in court hearings: the space of remote hearings is dispersed across multiple sites, stretched into non‐traditional justice spaces, and sometimes snapped back into the physical hearing centre. Analysing these spatial ruptures through ethnographic research conducted in immigration bail hearings in the UK offers insights into the relationship between physical and virtual hearing spaces. Specifically, it reveals three things about this relationship: that physical and virtual hearing spaces are relational; that remoteness does not annihilate hearing space; and that remoteness can be best understood as existing on a continuum. Sociological deployments of Radical Italian theory have offered insight into the changing role of art in light of the generalised valorisation of creativity in post-Fordist societies. Less has been said about the increasingly useful character of art, as exemplified by artistic engagement with interdisciplinary forms of problem solving. This article focuses on the field of practice commonly known as ArtScience. I argue that Lazzarato's theory gains new relevance in this novel field of cultural practice, where material production is an increasingly minor element of artistic labour. I draw on two case studies from artists practising ArtScience in Berlin as speculative provocations for thinking about the ways in which social life today becomes implicated in the production of value, via processes of subjectivation, collaboration and communication. The emergence of compulsive drug‐seeking habits, a hallmark feature of substance use disorder, has been shown to be predicated on the engagement of dorsolateral striatal control over behaviour. This process involves the dopamine‐dependent functional coupling of the anterior dorsolateral striatum (aDLS) with the nucleus accumbens core, but the mechanisms by which this coupling occurs have not been fully elucidated. The striatum is tiled by a syncytium of astrocytes that express the dopamine transporter (DAT), the level of which is altered in individuals with heroin use disorder. Astrocytes are therefore uniquely placed functionally to bridge dopamine‐dependent mechanisms across the striatum. Here we tested the hypothesis that exposure to heroin influences the expression of DAT in striatal astrocytes across the striatum before the development of DLS‐dependent incentive heroin seeking habits. Using Western‐blot, qPCR, and RNAscope™, we measured DAT protein and mRNA levels in whole tissue, culture and in situ astrocytes from striatal territories of rats with a well‐established cue‐controlled heroin seeking habit and rats trained to respond for heroin or food under continuous reinforcement. Incentive heroin seeking habits were associated with a reduction in DAT protein levels in the anterior aDLS that was preceded by a heroin‐induced reduction in DAT mRNA and protein in astrocytes across the striatum. Striatal astrocytes were also shown to be susceptible to direct dopamine‐ and opioid‐induced downregulation of DAT expression. These results suggest that astrocytes may critically regulate the striatal dopaminergic adaptations that lead to the development of incentive heroin seeking habits. [ABSTRACT FROM AUTHOR]

Purpose: An ageing population and a global pandemic has placed greater demands on palliative care services. Numerous studies describe the patient experience in palliative care, however, few explore the healthcare professional's experience of caring in this setting. This study explored the emotional challenges faced by palliative care professionals working in adult hospice services in Ireland. Design/methodology/approach: A narrative phenomenological approach was adopted, using interpretative phenomenological analysis to analyse results from five participants' semi-structured interviews. Findings: The type of challenge experienced reflected the impact it had on the participant's emotional wellbeing. Challenges perceived as achievable contributed to high levels of emotional wellbeing. These challenges often offered the opportunity for skill development and elicited positive feelings. Challenges perceived as uncontrollable negatively impacted the professional's emotional wellbeing and increased their risk of burnout. Examples of this included the shift in service provision and professional expectations. The challenges palliative care professionals experience on a daily basis can negatively or positively impact their emotional wellbeing. Conclusion: Overall, this study highlighted challenges and their impacts experienced by palliative care professionals, illustrating key areas for improvement to prioritise staff wellbeing. Soil quality monitoring schemes are a useful tool for assessing the potential of soils to perform desired services such as agricultural productivity. When researchers or other stakeholders wish to compare results between different schemes or studies, failure to consider differences in soil sample storage conditions presents a significant potential for error. Here, we compared levels of nitrogen and potassium, as well as pH, in agricultural soil samples stored under three different conditions (refrigerated, frozen, and oven‐dried). All tests were performed after 7 and 24 weeks of storage. Nitrate decreased significantly in dried (p < 0.001) samples. When refrigerated, nitrate first increased (p < 0.01) and then decreased (p < 0.001). Nitrate levels where unchanged at Week 7 in the freezer but decreased significantly at Week 24 (p < 0.001). Nitrite and ammonium increased after drying (p < 0.001) and when frozen (p < 0.001 and p < 0.05) but remained stable when refrigerated. There was no significant difference in potassium levels between the fresh control and Week 7 in the freezer, but potassium had increased at Week 24 (p < 0.05). Potassium concentration increased in refrigerated samples (p < 0.001) and fluctuated up and down in dried samples (p < 0.01). pH measurements fluctuated significantly in refrigerated and frozen samples (p < 0.001 and p < 0.01, respectively) but were unchanged in dried samples. We suggest that soil monitoring schemes standardize their sample storage, and we encourage researchers to clearly report soil sample storage conditions in publications, to improve transparency and reproducibility. Estuarine water quality is declining worldwide due to increased tourism, coastal development, and a changing climate. Although well-established methods are in place to monitor water quality, municipalities struggle to use the data to prioritize infrastructure for monitoring and repair and to determine sources of contamination when they occur. The objective of this study was to assess water quality and prioritize sources of contamination within Town Creek Estuary (TCE), Beaufort, North Carolina, by combining culture, molecular, and geographic information systems (GIS) data into a novel contamination source ranking system. Water samples were collected from TCE at ten locations on eight sampling dates in Fall 2021 (n = 80). Microbiological water quality was assessed using US Environmental Protection Agency (U.S. EPA) approved culture-based methods for fecal indicator bacteria (FIB), including analysis of total coliforms (TC), Escherichia coli (EC), and Enterococcus spp. (ENT). The quantitative microbial source tracking (qMST) human-associated fecal marker, HF183, was quantified using droplet digital PCR (ddPCR). This information was combined with environmental data and GIS information detailing proximal sewer, septic, and stormwater infrastructure to determine potential sources of fecal contamination in the estuary. Results indicated FIB concentrations were significantly and positively correlated with precipitation and increased throughout the estuary following rainfall events (p < 0.01). Sampling sites with FIB concentrations above the U.S. EPA threshold also had the highest percentages of aged, less durable piping materials. Using a novel ranking system combining concentrations of FIB, HF183, and sewer infrastructure data at each site, we found that the two sites nearest the most aged sewage infrastructure and stormwater outflows were found to have the highest levels of measurable fecal contamination. This case study supports the inclusion of both traditional water quality measurements and local infrastructure data to support the current need for municipalities to identify, prioritize, and remediate failing infrastructure. Background: Levetiracetam is a readily available, safe anticonvulsive medication. It is frequently administered as IV piggyback with a pump, carrier fluid, and tubing. The Established Status Epilepticus Treatment Trial demonstrated levetiracetam being similarly effective to previously used treatments in doses up to 4500 mg administered over 10 minutes. Objective: We sought to compare usage, cost, and waste of IV piggyback with IV push administration of levetiractam following implementation of an IV push protocol in an academic emergency department. Methods: A three-month review of levetiracetam administration was done following protocol implementation using IV push for initial treatment of benzodiazepine-refractory status epilepticus. The review quantified the number of IV push vs IV piggyback doses for all indications and evaluated cost of supplies necessary for administration. Results: During the study period, 137 patients received 142 doses of IV levetiracetam. Fifty-one doses (36%) were given as IV push rather than IV piggyback. The majority of doses 116 (82%) were 1000-2000 mg and 11 doses (8%) 3500-4500 mg. Estimated three-month savings with complete transition of IV piggyback to IV push would exceed $6000 just in our ED. The amount of sterile solution carrier fluid was also reduced and IV pump time freed. Conclusion: Implementation of an emergency department IV push levetiracetam protocol resulted in cost savings. Opportunities remain to improve clinical implementation practices. Rationale: Win-paired stimuli can promote risk taking in experimental gambling paradigms in both rats and humans. We previously demonstrated that atomoxetine, a noradrenaline reuptake inhibitor, and guanfacine, a selective α2A adrenergic receptor agonist, reduced risk taking on the cued rat gambling task (crGT), a rodent assay of risky choice in which wins are accompanied by salient cues. Both compounds also decreased impulsive premature responding. Objective: The key neural loci mediating these effects were unknown. The lateral orbitofrontal cortex (lOFC) and the medial prefrontal cortex (mPFC), which are highly implicated in risk assessment, action selection, and impulse control, receive dense noradrenergic innervation. We therefore infused atomoxetine and guanfacine directly into either the lOFC or prelimbic (PrL) mPFC prior to task performance. Results: When infused into the lOFC, atomoxetine improved decision making score and adaptive lose-shift behaviour in males, but not in females, without altering motor impulsivity. Conversely, intra-PrL atomoxetine improved impulse control in risk preferring animals of both sexes, but did not alter decision making. Guanfacine administered into the PrL, but not lOFC, also altered motor impulsivity in all subjects, though in the opposite direction to atomoxetine. Conclusions: These data highlight a double dissociation between the behavioural effects of noradrenergic signaling across frontal regions with respect to risky choice and impulsive action. Given that the influence of noradrenergic manipulations on motor impulsivity could depend on baseline risk preference, these data also suggest that the noradrenaline system may function differently in subjects that are susceptible to the risk-promoting lure of win-associated cues. Despite ample research demonstrating the developmental and clinical importance of psychosocial competencies (e.g., prosociality, attention regulation, compliance to caregivers) in early childhood, few measures exist that are clinically relevant, developmentally appropriate, psychometrically validated, and pragmatic to administer, score, and interpret. One promising option is the Psychosocial Strengths Inventory for Children and Adolescents (PSICA), a parent-report measure of affective, attentional, and social competencies in youth. Although the PSICA has growing psychometric support, it remains a relatively lengthy measure (i.e., 36 items). Thus, we sampled 865 community caregivers (75% White, 59% mothers) of children ages 2–10 (72% White, 55% boys) to empirically develop and validate the PSICA–Short Form (PSICA-SF). Items were winnowed based on best-practice internal and external criteria (i.e., structural validity, convergent validity, internal consistency, and item-response discrimination indices), then further assessed for judgmental validity via quantitative survey completed by a snowball sample of 18 early childhood experts. These methods identified a 9-item version of the PSICA-SF, which retained the original PSICA's trichotomous structure and related subscales (i.e., Prosociality, Compliance, Attention), good internal consistency, face validity, and convergent validity. Collectively, these findings support the PSICA-SF as a more pragmatic, while equally psychometrically sound, multidimensional measure of child psychosocial competence that might be used for relevant child screening, case conceptualization, progress monitoring, and program evaluation. Obstructive sleep apnoea is the most common form of sleep-disordered breathing. It is characterised by recurrent occlusion of the airway during sleep. Ensuing apnoeas terminate in arousal from sleep and lead to non-restorative sleep, excessive daytime sleepiness and adverse cardiovascular and neurocognitive effects. A sleep study should be offered to patients reporting witnessed apnoeas or symptoms related to non-restorative sleep. It should also be considered in the presence of predisposing factors for obstructive sleep apnoea (e.g. obesity, tonsillar hypertrophy, retrognathia, refractory hypertension). Treatment should aim to improve symptoms and reduce cardiovascular and neurocognitive risk. The treatment approach should consider the symptom burden, severity, anatomical factors, and patient preference. Background: Negative mental health-related effects of SARS-COV-2 infection are increasingly evident. However, the impact on suicide-related outcomes is poorly understood, especially among populations at elevated risk. Objective: To determine risk of suicide attempts and other self-directed violence (SDV) after SARS-COV-2 infection in a high-risk population. Design: We employed an observational design supported by comprehensive electronic health records from the Veterans Health Administration (VHA) to examine the association of SARS-COV-2 infection with suicide attempts and other SDV within one year of infection. Veterans with SARS-COV-2 infections were matched 1:5 with non-infected comparators each month. Three periods after index were evaluated: days 1–30, days 31–365, and days 1–365. Participants: VHA patients infected with SARS-COV-2 between March 1, 2020 and March 31, 2021 and matched non-infected Veteran comparators. Main Measures: Suicide attempt and other SDV events for the COVID-19 and non-infected comparator groups were analyzed using incidence rates per 100,000 person years and hazard ratios from Cox regressions modeling time from matched index date to first event. Subgroups were also examined. Key Results: 198,938 veterans with SARS-COV-2 (COVID-19 group) and 992,036 comparators were included. Unadjusted one-year incidence per 100,000 for suicide attempt and other SDV was higher among the COVID-19 group: 355 vs 250 and 327 vs 235, respectively. The COVID-19 group had higher risk than comparators for suicide attempts: days 1–30 hazard ratio (HR) = 2.54 (CI:2.05, 3.15), days 31–365 HR = 1.30 (CI:1.19, 1.43) and days 1–365 HR = 1.41 (CI:1.30, 1.54), and for other SDV: days 1–30 HR = 1.94 (CI:1.51, 2.49), days 31–365 HR = 1.32 (CI:1.20, 1.45) and days 1–365 HR = 1.38 (CI:1.26, 1.51). Conclusions: COVID-19 patients had higher risks of both suicide attempts and other forms of SDV compared to uninfected comparators, which persisted for at least one year after infection. Court ethnographies have commonly relied on the physical presence of the ethnographers. This paper explores the opportunities and the challenges of conducting court ethnographies without this physical presence. Specifically, it examines what it means to conduct remote ethnographies of legal processes where neither the ethnographer nor the other hearing participants are physically co‐present. The sudden shift towards remote hearings in fieldwork conducted during the COVID‐19 pandemic presented an opportunity to compare in‐person and remote ethnographic methods. Both psychostimulant use and engagement with probabilistic schedules of reward sensitize the mesocorticolimbic dopamine (DA) system. Such behaviors may act synergistically to explain the high comorbidity between stimulant use and gambling disorder. The salient audiovisual stimuli of modern electronic gambling may exacerbate the situation. To probe these interactions, we sensitized ventral tegmental area DA neurons via chronic chemogenetic stimulation while rats (n = 134) learned a rat gambling task in the presence or absence of casino-like cues. The same rats then learned to self-administer cocaine. In a separate cohort (n = 25), we confirmed that our chemogenetic methods sensitized the locomotor response to cocaine and potentiated phasic excitability of ventral tegmental area DA neurons through in vivo electrophysiological recordings. In the absence of cues, sensitization promoted risk taking in both sexes. When rewards were cued, sensitization expedited the development of a risk-preferring phenotype in males while attenuating cue-induced risk taking in females. While these results provide further confirmation that ventral tegmental area DA neurons critically modulate risky decision making, they also reveal stark sex differences in the decisional impact that dopaminergic signals exert when winning outcomes are cued. As previously observed, risky decision making on the cued rat gambling task increased as both males and females learned to self-administer cocaine. The combination of DA sensitization and win-paired cues while gambling led to significantly greater cocaine taking, but these rats did not show any increase in risky choice as a result. Therefore, cocaine and heavily cued gambles may partially substitute for each other once the DA system has been rendered labile through sensitization, thereby compounding addiction risk across modalities. Unilateral porokeratosis associated with neoadjuvant radiotherapy of the pelvis is a rare condition characterized by hyperkeratotic raised borders and an atrophic center. This case report presents a 72-year-old male with persistent pruritic plaques on the abdomen following radiotherapy for rectal adenocarcinoma. The patient's porokeratosis was successfully treated with cryotherapy and topical medications. The association between porokeratosis and radiotherapy is uncommon, and further research is needed to understand the underlying causes and develop effective treatments for this condition. The brain economy is primarily based on Brain Capital, an economic asset integrating brain health and brain skills, the social, emotional, and the diversity of cognitive brain resources of individuals and communities. People with healthy brains are essential to navigate increasingly complex systems. Objective: To describe and quantify independent prescribing of oncology pharmacists working in adult, ambulatory cancer centers in Alberta, Canada. Methods: A retrospective chart review of oncology pharmacists prescribing in the electronic health record, ARIA® was conducted. Prescriptions from January 1, 2018 to June 30, 2018 were analyzed. Descriptive statistics were used to quantify prescription volume and class of medications prescribed. A cross-sectional analysis was then performed on a random sample to determine the type of prescription intervention and evaluate pharmacist documentation. Results: Over 6 months, 3474 prescriptions were ordered by 33 clinically deployed pharmacists. The median number of medications prescribed was 7 per month (interquartile range: 1.50–27.00; Range: 0.17–79.5). When prescribing was standardized by pharmacist's time clinically deployed, the median was 21.67 (interquartile range: 5.00–79.67; range: 0.67–216.67) prescriptions per month per full-time equivalent. The most prescribed class of medication was antiemetic (24.1%). From a sample of 346 prescriptions, 172 (50%) were new medications initiated, 160 (46%) were the continuation of existing prescriptions and 14 (4%) were prescription dosage adjustments. Adherence to the specified documentation standards was 47%. Conclusions: Oncology pharmacists utilize their independent prescribing to initiate and continue supportive care medications for cancer patients. The novel SARS-CoV-2, or coronavirus, has greatly altered the landscape of college life for students across the United States. The ever-present health concerns and quarantine have been linked to increased anxiety, depression, stress, and post-traumatic stress disorder. To this end, we examined the influence of the COVID-19 pandemic on the psychosocial health of college students. We explored the impact of COVID-19 on levels of stress and distress in college students using a qualitative expressive writing methodology. Results of this study suggested that the college students included in the sample were moderately distressed - their scores on inventories of depression, anxiety, and COVID-19-related stressors suggested moderate distress. We observed the low-mass X-ray binary Sco X-1 for 12 nights simultaneously using the Rossi X-Ray Timing Explorer and the Otto Struve Telescope at McDonald Observatory at 1-s time-resolution. This is among the most comprehensive simultaneous X-Ray/optical data sets of Sco X-1. Evidence of reprocessing was observed in the form of nine positive, near-zero lag peaks in the cross-correlation function, eight of which were relatively small, and took the shape of piecewise exponential functions. These peaks were initially identified by eye, after which a computational identification scheme was developed to confirm their significance. Based on their short lags (less than 4 s), as well as their occurrence on the flaring branch (FB) and soft apex, the small cross-correlation features are likely to be caused by reprocessing off the outer disc, although the companion could still make a contribution to their tails. The Z track was parameterized using a rank number scheme so that the system's location on the track could be numerically defined. Plotting the results against the optical reveals an increasing step function when moving from the horizontal branch to the normal branch (NB) to the FB, with differential optical levels at ∼0.47, ∼0.57, and ∼1.1, respectively. An additional correlation between Z track location and the optical was found on the upper FB. A 60-year-old woman presented with a cutaneous eruption in the right submammary area. She had a history of psoriasis, lichen sclerosus, and previous breast cancer. The clinical differential diagnosis included various conditions such as cutaneous lymphangioma, varicella zoster reactivation, extragenital lichen sclerosus, and angiosarcoma. Bacterial swabs cultured coagulase-negative staphylococcus, while viral swabs were negative. Histopathological findings revealed an atypical vascular lesion (AVL) of the breast. AVLs are distinct lesions that occur after radiation therapy and breast conserving surgery, typically appearing as erythematous papules, plaques, or vesicles. They can have lymphatic or vascular histology and may be a precursor to angiosarcoma, although true cytological atypia and mitotic activity are absent in AVLs. The growing body of research on telehealth services provides evidence of the efficacy of services and the possibility for greater accessibility of counseling services for hard-to-reach clients. However, less is known how clients think about the process of telehealth for mental health services. This study focused on two dimensions of client perception: telehealth difficulties and telehealth technology competency. This study examined the factor structure of two client-reported new measures, the Telehealth Difficulties Scale and the Telehealth Technology Competency Scale, through exploratory factor analyses. It is crucial to find an effective, environmentally acceptable solution, such as bioplastics or biodegradable plastics, to the world's rising plastics demand and the resulting ecological destruction. This study has focused on the environmentally friendly production of bioplastic samples derived from corn starch, rice starch, and tapioca starch, with various calcium carbonate filler concentrations as binders. Two different plasticizers, glycerol and sorbitol, were employed singly and in a rich blend. To test the differences in the physical and chemical properties (water content, absorption of moisture, water solubility, dissolution rate in alcohol, biodegradation in soil, tensile strength, elastic modulus, and FT-IR) of the produced samples, nine samples from each of the three types of bioplastics were produced using various ratios and blends of the fillers and plasticizers. The produced bioplastic samples have a multitude of features that make them appropriate for a variety of applications. Objectives: This study aimed to identify facilitators and barriers to participation in vision impaired golf. Design: Semistructured interviews were conducted with 12 vision impaired golfers, 10 guides, and 5 novices during the British VI Golf Open tournament in August 2021. Results: Qualitative content analysis identified the benefits, facilitators, and challenges of playing vision impaired golf. The role of guides and future developments of the sport were furthermore identified. The benefits extend beyond those related to health, including personal development and a sense of purpose for players and of reward for guides. Facilitatorswere largely organizational and having appropriate support and a guide. The challenges to playing and guiding were practical, financial, and logistical. The need to attract a more diverse range of players, along with a reconsideration of the eligibility criteria, was central to the perceived sustainability of vision impaired golf. Conclusions: These results highlight the practical, financial, and logistical challenges contributing to vision impaired golf activity limitations and participation restrictions. Because of the health, personal, and psychosocial benefits, ways of providing practical, financial, and support networks to facilitate engagement in vision impaired golf should be sought. Couple and family therapists (CFTs) may use professional social media to build a therapy brand and reach potential clients, disseminate knowledge, and/or express themselves. There currently is a lack of guidance on the ethics of professional social media use for CFTs. This qualitative study asked CFTs about their professional social media. The authors asked questions regarding intended audiences, how boundaries are maintained between therapy practice and social media, and how information is shared on participants' social media. Thirty CFTs (n = 30) accessed and completed the online survey. The authors conducted a thematic analysis where patterns were constructed based on participant responses (Braun and Clarke, in Qual Res Psychol 3(2):77–101, 2006. https://doi.org/10.1191/1478088706qp063oa). The analysis revealed five major themes. Ethics included participant descriptions that identified ethical principles and professional resources for making ethical decisions on social media. Research was a pattern constructed from participant descriptions about how and if research informs their social media posts. Influencing was a code identified when participants either rejected or accepted that their social media platform could affect their audience. Audience was coded when participants described their intended audience. Participants described their intended audience to include the general public, potential clients, and colleagues. Social Justice was coded when CFTs described dismantling systems of oppression in their goals for their professional social media. We investigated how engagement in activism among white women is predicted by their political efficacy, exposure to gendered microaggressions (i.e., verbal and/or nonverbal slights targeting a subordinate gender identity), and color-blind racial attitudes. We recruited 93 white undergraduate women for whom exposure to gendered microaggressions, color-blind racial attitudes, and political efficacy explained a significant proportion of variance in participants' ratings of political activism on social media, although these predictors did not significantly explain a proportion of the variance in their in-person political activism (e.g., rally attendance). Background Clinical trials initiated during emerging infectious disease outbreaks must quickly enroll participants to identify treatments to reduce morbidity and mortality. This may be at odds with enrolling a representative study population, especially when the population affected is undefined. Methods We evaluated the utility of the Centers for Disease Control and Prevention's COVID-19–Associated Hospitalization Surveillance Network (COVID-NET), the COVID-19 Case Surveillance System (CCSS), and 2020 United States (US) Census data to determine demographic representation in the 4 stages of the Adaptive COVID-19 Treatment Trial (ACTT). We compared the cumulative proportion of participants by sex, race, ethnicity, and age enrolled at US ACTT sites, with respective 95% confidence intervals, to the reference data in forest plots. Results US ACTT sites enrolled 3509 adults hospitalized with COVID-19. When compared with COVID-NET, ACTT enrolled a similar or higher proportion of Hispanic/Latino and White participants depending on the stage, and a similar proportion of African American participants in all stages. In contrast, ACTT enrolled a higher proportion of these groups when compared with US Census and CCSS. The proportion of participants aged ≥65 years was either similar or lower than COVID-NET and higher than CCSS and the US Census. The proportion of females enrolled in ACTT was lower than the proportion of females in the reference datasets. Conclusions Although surveillance data of hospitalized cases may not be available early in an outbreak, they are a better comparator than US Census data and surveillance of all cases, which may not reflect the population affected and at higher risk of severe disease.
