Factors associated with the well-being of lesbian, gay, bisexual, transgender, queer, and questioning (LGBTQ) youth were qualitatively examined to better understand how these factors are experienced from the youths’ perspectives. Largely recruited from LGBTQ youth groups, 68 youth participated in focus groups (n = 63) or individual interviews (n = 5). The sample included 50% male, 47% female, and 3% transgender participants. Researchers used a consensual methods approach to identify negative and positive factors across 8 domains. Negative factors were associated with families, schools, religious institutions, and community or neighborhood; positive factors were associated with the youth’s own identity development, peer networks, and involvement in the LGBTQ community. These findings suggest a pervasiveness of negative experiences in multiple contexts, and the importance of fostering a positive LGBTQ identity and supportive peer/community networks. Efforts should work towards reducing and eliminating the prejudicial sentiments often present in the institutions and situations that LGBTQ youth encounter. [ABSTRACT FROM AUTHOR]

The article looks at issues related to replicating HIV interventions across cultures. Illustrations are presented which use a randomized controlled design to test the efficacy of an HIV prevention intervention designed for youth. The author feels that a scientific approach to replication needs to be developed. The limited generalizability of a single test of an intervention used to determine its efficacy has been a major difficulty in tailoring interventions to participants' cultures. The importance of adaptation to replicate interventions across cultures is discussed.

The article reports on public health efforts to control sexually transmitted diseases and HIV/AIDS within teenage and young adult populations. Prevention intervention efforts are described and their effectiveness is evaluated. Condom and contraceptive use is reported. A prevention intervention course titled Focus on Kids is described and the article reports on its transferability to other locations. African Americans, East Africans, Southeast Asians and rural West Virginians took part in the prevention intervention and the article evaluates its effectiveness.

This article compares two methods of collecting daily data: self-administered diaries and telephone interviews. Study participants included 44 men and 56 women between the ages of 16 and 35 who participated in a larger study of drinking, drug use, and sexual activity. Participants were randomly assigned to either the written diary or the telephone interview conditions; question wording and format were identical in both conditions. Daily data were collected for a period of 8 weeks. Results indicate that although telephone interviews resulted in slightly more missed days of data collection, they generally yielded less item-level missing data, produced cleaner data and therefore were less costly to process, and were as palatable to participants as self-administered diaries. Except for reports of drinking and vegetable consumption, telephone and diary conditions did not differ in the amount of behavior reported; more drinking and vegetable consumption were reported with telephone interviews, however. Parallel bodies of research have described the diverse and complex ways that men understand and construct their masculine identities (often termed "masculinities") and, separately, how adherence to traditional notions of masculinity places men at risk for negative sexual and health outcomes. The goal of this analysis was to bring together these two streams of inquiry. Using data from a national, online sample of 555 heterosexually active young men, we employed latent class analysis (LCA) to detect patterns of masculine identities based on men's endorsement of behavioral and attitudinal indicators of "dominant" masculinity, including sexual attitudes and behaviors. LCA identified four conceptually distinct masculine identity profiles. Two groups, termed the Normative and Normative/Male Activities groups, respectively, constituted 88 % of the sample and were characterized by low levels of adherence to attitudes, sexual scripts, and behaviors consistent with "dominant" masculinity, but differed in their levels of engagement in male-oriented activities (e.g., sports teams). Only eight percent of the sample comprised a masculinity profile consistent with "traditional" ideas about masculinity; this group was labeled Misogynistic because of high levels of sexual assault and violence toward female partners. The remaining four percent constituted a Sex-Focused group, characterized by high numbers of sexual partners, but relatively low endorsement of other indicators of traditional masculinity. Follow-up analyses showed a small number of differences across groups on sexual and substance use health indicators. Abstract: As part of an internet-based study to investigate experiences of sexual minority youth in the U.S., 544 youth, ages 14–19, were surveyed about their need for services, where they preferred to receive these services, and their preferred method of service delivery. The survey was anonymous and youth were recruited from LGBTQ-specific listservs and venues as well as through social networking sites as a way of reaching youth often not represented in LGBTQ surveys. Youth highly endorsed many types of services, settings, and modes of delivery. Some subgroup differences emerged. Transgender youth and those uncertain about their gender identity expressed stronger interest than others in services to address stress, family issues, and self-defense, and in receiving support and guidance from LGBTQ adults. Few differences in service preferences along demographic lines such as race/ethnicity emerged; however, African American youth were more likely than others to prefer services offered in a place of worship. Contrary to predictions, geographic and community variables were not related to service type, delivery format, or location preferences. [Copyright &y& Elsevier]

Objective: Retrospective reports of the association between drinking and high-risk sexual behavior can be biased by implicit theories of the effects of drinking or may represent post hoc justifications instead of accurate reports of behavior. Using data from a daily diary study, we compared daily reports of condom use when drinking and not drinking with the same participants' reports of these behaviors from a retrospective questionnaire administered after diary collection was complete. Method: Participants included adolescents (n = 145), adult sexually transmitted disease clinic clients (n = 167), college students (n = 145), and men who have sex with men (n = 147). All participants reported their alcohol consumption and sexual activity daily for 8 weeks and then completed a retrospective questionnaire about their behavior over the diary period. Results: Participants' retrospective judgments about whether they used condoms more or less when drinking were not significantly related to their behavior as reported in the diary. Fewer than two thirds of the participants were accurate in their recollection of the association of condom use and drinking. Teenagers and men who have sex with men were more likely to retrospectively overestimate the negative effect of alcohol on condom use. Conclusions: Retrospective questions about the association between drinking and condom use were consistent with actual behavior only among people who consistently either never or always used condoms. These individuals correctly reported that drinking had no effect on their condom use. For people whose condom use varies, questions about associations between drinking and sex may be difficult to answer, owing to their conditional nature, and may lead to error. [ABSTRACT FROM AUTHOR]

Objective: Six items were administered to a multiethnic sample of 435 middle school-age participants in a group-delivered safer sex intervention to determine their reliability and validity. Method: Exploratory analyses were followed by confirmatory factor analyses, and then correlations of scale scores with theoretically related variables were computed. Results: Findings favored a unidimensional scale with high internal consistency (with as few as two and as many as five items). Scale scores were generally related to other constructs as expected. Conclusions: The items show evidence of reliability and validity. In addition to the items 'use in peer group research, the findings have implications for practice in terms of group leader training and facilitation, and the items' use as assessment tools. Data from an online community sample of young men were analyzed to test predictors of sexual assault perpetration. We used structural equation modeling to test the relative contributions of specific sub-types of childhood adversity to subsequent sexual aggression. Mediators included hostile masculinity, impersonal sexual behavior and attitudes, and substance use variables. Findings suggested that childhood sexual abuse had direct and mediated effects on sexual assault perpetration, but hostile masculinity was the only proximal factor significantly related to aggression. Childhood polytrauma was also associated with increased perpetration risk, suggesting that prevention efforts may be aided by increased attention to childhood maltreatment. [ABSTRACT FROM AUTHOR]

Intimate partner violence (IPV) victimization is linked to sexual risk exposure among women. However, less is known about the intersection of IPV perpetration and sexual risk behavior among men. This study used data from a diverse, community sample of 334 heterosexually active young men, aged 18 to 25, across the United States to examine whether and how men with distinct IPV-related behavior patterns differed in sexual risk–related behavior and attitudes. Participants were recruited and surveyed online, and grouped conceptually based on the types of IPV perpetration behavior(s) used in a current or recent romantic relationship. Groups were then compared on relevant sexual risk variables. Men reporting both physical abuse and sexual coercion against intimate partners reported significantly higher numbers of lifetime partners, higher rates of nonmonogamy, greater endorsement of nonmonogamy, and less frequent condom use relative to nonabusive men or those reporting controlling behavior only. This group also had higher sexually transmitted infection (STI) exposure compared to men who used controlling behavior only and men who used sexual coercion only. Findings suggest that interventions with men who use physical and sexual violence need to account for not only the physical and psychological harm of this behavior but also the sexual risk to which men may expose their partners. Heterosexual men's sexual safety behavior is important to controlling the U.S. epidemic of sexually transmitted infections (STIs), including human immunodeficiency virus (HIV). While sexual safety is often treated as a single behavior, such as condom use, it can also be conceptualized as resulting from multiple factors. Doing so can help us achieve more nuanced understandings of sexual risk and safety within partner-related contexts. We used latent class analysis with data collected online from 18- to 25-year-old heterosexually active U.S. men (n = 432) to empirically derive a typology of the patterns of sexual safety strategies they employed. Indicators were sexual risk-reduction strategies used in the past year with the most recent female sex partner: condom use, discussing sexual histories, STI testing, agreeing to be monogamous, and discussing birth control. We identified four subgroups: Risk Takers (12%), Condom Reliers (25%), Multistrategists (28%), and Relationship Reliers (35%). Partner-related context factors—number of past-year sex partners, relationship commitment, and sexual concurrency—predicted subgroup membership. Findings support tailoring STI prevention to men's sexual risk-safety subgroups. Interventions should certainly continue to encourage condom use but should also include information on how partner-related context factors and alternate sexual safety strategies can help men reduce risk for themselves and their partners. Research on heterosexual men's sexual expectations has focused on self-described personal traits and culturally dominant models of masculinity. In a pair of studies, we used a sexual scripts perspective to explore the range and diversity of young men's thoughts about sex and relationships with women and to develop measures for assessing these scripts. In the first study, we conducted semi-structured interviews to elicit young men's accounts of their sexual relationships. We used these narratives to produce brief sexual script scenarios describing typical sexual situations, as well as conventional survey items assessing sexual behavior themes. In the second study, we administered the scenarios and theme items to an ethnically diverse, national sample of 648 heterosexually active young men in an online survey. Abstract: Young adults have high rates of sexually transmitted infections (STIs). Sexual minority youths' risk for STIs, including HIV, is as high as or higher than sexual majority peers'. Sexual safety, while often treated as a single behavior such as condom use, can be best conceptualized as the result of multiple factors. We used latent class analysis to identify profiles based on ever-used sexual safety strategies and lifetime number of partners among 425 self-identified LGBTQ youth aged 14–19. Data collection took place anonymously online. We identified four specific subgroup profiles for males and three for females, with each subgroup representing a different level and type of sexual safety. Profiles differed from each other in terms of age and outness for males, and in outness, personal homonegativity, and amount of education received about sexual/romantic relationships for females. Youths' sexual safety profiles have practice implications for sexuality educators, health care professionals, and parents. [Copyright &y& Elsevier]

CONTEXT: Little research has explored how teenagers think about abstinence and how it functions in their lives. These questions are particularly salient in light of widespread funding of abstinence-only programs in the United States. METHODS: Data on attitudes and intentions related to abstinence and sex were collected from 365 adolescents aged 12–15 who participated in an HIV risk reduction program in Seattle in 2001–2003. Logistic regression analyses assessed associations between these cognitions, as measured six months after the program, and teenagers’ likelihood of having vaginal or anal sex in the subsequent six months. RESULTS: Adolescents who had positive attitudes and intentions about abstinence had a reduced likelihood of subsequently engaging in sex (odds ratio, 0.6 for each), whereas those with positive attitudes and intentions about having sex had an elevated likelihood of engaging in sex (2.2 and 3.5, respectively). A regression model including only sex cognitions accounted for substantially more variation in sexual activity than did one including only abstinence cognitions (15–26% vs. 6–8%). Significant interaction effects were also seen: Among teenagers with low levels of sex intention, greater abstinence intention had little relationship to the predicted probability of having sex, but among teenagers with high levels of sex intention, greater abstinence intention was associated with increases in the predicted probability of having sex. CONCLUSIONS: Youth do not consider abstinence and sexual activity opposing constructs, and solely instilling positive abstinence attitudes and intentions in youth may not have robust effects in preventing sexual activity. This study tested the theory of planned behavior's (TPB) expectation that attitudes, social norms, and self-efficacy would mediate other variables' effects on intention for having sex and probability of having sex. Longitudinal data were collected from 790 high-school-aged adolescents. In an intrapersonal variable model, sensation seeking, alcohol use, hard work, smoking, and moral beliefs had mediated effects, but the last 2 also had direct effects (on intention and behavior, respectively). In an interpersonal model, school attachment had mediated effects, pressure to have sex had a direct effect on behavior, and parent communication and number of sexually active friends had both. Intrapersonal variable effects were better described by the TPB than those of interpersonal variables. Targeting distal psychosocial factors may enhance intervention efficacy. [ABSTRACT FROM AUTHOR]

Sexual health research often relies on single risk indicators. However, multi-variable composites may better capture the underlying construct of risk-taking. Latent Profile Analysis identified subgroups based on condom use consistency, partner numbers, and sex frequency among 605 adolescents. Three profiles were identified for each of grades 8 to 10 (Condom Users, Few Partners, and Risk-Takers) and 4 in grades 11 and 12 (Condom Users, One Partner, Two Partners, and Risk-Takers). Inconsistent condom use groups reported more non-condom (and often less effective) birth control use and STD and pregnancy histories. Females had greater representation in the Few Partners, One Partners, and Two Partners groups, which also contained increasing proportions of participants in each subsequent year Males had greater representation in the Risk-Takers group. A profile approach to measurement has methodological advantages, can add to substantive knowledge, and can inform content, timing, and targets of sexual health interventions. [ABSTRACT FROM AUTHOR]

We examined the utility of the theory of reasoned action for predicting sexual intercourse among teenagers and determined whether it holds for both genders and for those with and without prior sexual experience. The data include 749 students who were in 9th-llth grades when the predictors were measured and in 10th-12th grades when sexual intercourse was assessed. About half (53%) were girls, about half (48%) were non-Hispanic European Americans. Results showed that prior sexual experience was related to a higher rate of sexual intercourse, but boys and girls did not differ. Tests of the causal model for subgroups (boy and girl virgins, boy and girl nonvirgins) yielded similar results. As predicted, paths from intentions to behavior and from norms and attitudes to intentions were significant, as were paths from outcome and normative beliefs to attitude and norm, respectively. This research examined the applicability of the theory of reasoned action to school-age children. Using structural equation modeling, we longitudinally modeled children's attitudes, norms, intentions, and behavior with regard to drinking alcohol. Respondents were 1,061 children attending public schools in the northwestern United States. Attitude, norms, and intentions (measured when the children were in 6th, 7th, 8th, and 9th grades) were used to predict alcohol use 1 year later. Two modifications of the model improved the overall fit at each grade level: separating attitude into 2 dimensions (positive and negative), and adding a path from social norm to behavior. Children's beliefs about smoking were examined in a multi-ethnic urban sample of 4th through 7th grade children. Results showed that, relative to those in earlier grades, children in higher grades held more positive beliefs about the positive outcomes of smoking and the long-term negative consequences of smoking, but there was no association between grade level and beliefs about the immediate negative consequences of smoking. Children in higher grades also perceived more favorable norms toward smoking. There were few gender or race/ethnicity differences at these ages in children's beliefs about smoking. These outcome and normative beliefs were related to smoking behavior. Of the nine beliefs about the effects of smoking, all but two were significantly related to smoking behavior. ABSTRACT: Researchers examined individual characteristics and peer influences related to adolescents' sexual behavior, taking gender and sexual experience into account. As part of a larger, longitudinal study investigating youth health awareness, 8th, 9th, and 10th graders reported their intentions to engage in sexual activity and use condoms in the next year, the amount of pressure they felt to engage in sexual activity, and their perceptions about the number of their peers engaging in sexual activity. Findings suggest intentions to engage in sexual behavior and use condoms, feelings of pressure to have sex, and perceptions about the number of friends engaging in sexual intercourse differ by gender and sexual experience status. Implications of these findings for health and sexuality education, as well as HIV prevention programs targeted at adolescents, are discussed. Is daily data collection of sexual and other health-related behaviors reactive? This question was examined by exploring trends in daily reports of sexual intercourse, condom use, drinking, smoking, and vegetable and fruit consumption over a 2-month period. The trends were examined for evidence of fatigue effects and socially desirable responding. The effects of data collection method and gender were also examined. Results from a hierarchical linear modeling analysis showed a significant trend toward decreased reporting of drinking, smoking, and sexual intercourse, but no significant trends in fruit and vegetable consumption or condom use. These patterns are not consistent with fatigue effects or socially desirable responding. The results are discussed in relation to choosing data collection methods that minimize error. This research examines the applicability of Fishbein and Ajzen's (1975) theory of reasoned action (TRA) to intentions to use cigarettes and alcohol among 5th- and 6th-grade students. It also addresses the question of unidimensionality of outcome beliefs and referent norms. Results suggest that the TRA model describes children's decision making well, although the ability of income and race to add to prediction, net of the effects of attitude and perceived norm, are contrary to the theory's prediction. This paper reports the results of a study in which age (grade level), racial/ethnic, and gender differences in beliefs and perceived norms about drinking were examined in a multi-ethnic urban sample of 4th through 7th grade children. Results showed that older children held beliefs and perceived norms that were more favorable toward drinking than younger children. The major difference between older and younger children lay in their differential estimates of the likelihood of certain consequences occurring and not in their evaluation of these consequences of drinking. Further, older children not only displayed less motivation to comply with their parents and greater motivation to comply with their peers, but they also perceived their parents, as well as their peers, as less disapproving of drinking than did younger children.
