L'objectif de notre étude est d'étudier l'intérêt de la scintigraphie osseuse Veriton (SO-Ver) corps entier dans l'évaluation et le diagnostic de sacroiliite inflammatoire, en comparaison à l'IRM. Au total, 78 patients consécutifs suspects de rhumatisme inflammatoire chronique ayant bénéficié d'une SO-Ver ont été inclus entre novembre 2021 et février 2023. Seuls les patients ayant bénéficié d'une IRM des sacro-iliaques dans le mois ont été inclus et classés en 2 groupes : spondyloarthrite axiale (SpA) et non-spondyloarthrite (non-SpA). La présence/l'absence de sacroiliite, en IRM (définie selon les critères ASAS) puis en SO-Ver aux temps précoce (TP) et tardif (TT), a été établie. Des analyses quantitatives des articulations sacro-iliaques ont également été réalisées, en SO-Ver en utilisant les SUV maximums et normalisés (TP et TT), en IRM par la réalisation de scores SPARCC, par deux paires de lecteurs indépendants et en aveugle des données patients. Au total, 47 patients ont été répartis dans les 2 groupes (25 dans le groupe SpA et 22 dans le groupe non-SpA). En SO-Ver, une sacroiliite a été retrouvée chez 7/25 (28 %) et 8/25 (32 %) patients dans le groupe SpA, comparé à 0/22 et 6/22 (27 %) patients dans le groupe Non-SpA, en TP et TT respectivement. Une sacro-iliite IRM a été observée chez 16/25 (64 %) et 11/22 (50 %) patients dans les groupes SpA et non-SpA respectivement. Pour le diagnostic de sacroiliite inflammatoire la SO-Ver présente une sensibilité de 28 % (14–47) et 32 % (17–51) et une spécificité de 100 % (82–100) et 72 % (51–87) en TP et TT respectivement. En IRM la sensibilité et la spécificité sont de 64 % (44–79) et 50 % (30–69) pour le diagnostic de sacroiliite. En analyse binaire, l'IRM était associée à la SO-Ver uniquement en TT (OR [95 %CI] = 5,5 [1,9–16,3]). Les scores SPARCC étaient corrélés aux SUV maximums en TP (valeur 0,41 ; p < 0,0001) et en TT (valeur 0,53 ; p < 0,0001). Les SUV maximums (TT) et les scores SPARCC étaient plus élevés chez les patients souffrant de douleurs sacro-iliaques (p = 0,002 et p = 0,026, respectivement). Seul le SUV normalisé en TP était associé à une CRP élevée (p = 0,012). Il s'agit de la 1re étude visant à évaluer les performances diagnostiques de la SO-Ver dans l'évaluation de la sacro-iliite inflammatoire. L'IRM reste l'examen de référence dans cette indication mais présente des limites que la SO-Ver peut espérer surpasser. L'IRM des sacro-iliaques a notamment conduit à de nombreux faux-positifs, en particulier en post-partum ou chez les patients sportifs. Les lésions inflammatoires évocatrices de spondyloarthrite à l'IRM augmentent avec l'âge, suggérant le rôle de facteurs dégénératifs (arthrosiques). Dans notre étude, aucun patient du groupe non-SpA n'a présenté de sacro-iliite en SO-Ver en TP, témoignant de son excellente spécificité. La SO-Ver présente une bonne corrélation avec l'IRM pour l'évaluation de la sacroiliite inflammatoire dans la spondylarthrite axiale. Une sacroiliite en So-Ver présente un niveau de spécificité supérieur à celle de l'IRM alors que la sensibilité est plus faible. En outre, au temps tardif, les SUV maximums étaient associés à la présence d'une douleur sacro-iliaque. Ces résultats prometteurs devront être confirmés dans des études futures. Objectively derived resolution-dependent criteria are defined for the detection of tropical cyclones in model simulations and observationally based analyses. These criteria are derived from the wind profiles of observed tropical cyclones, averaged at various resolutions. Both an analytical wind profile model and two-dimensional observed wind analyses are used. The results show that the threshold wind speed of an observed tropical cyclone varies roughly linearly with resolution. The criteria derived here are compared to the numerous different criteria previously employed in climate model simulations. Introduction: Depressive symptoms are associated with poor outcomes, increased risk of relapse and high suicide rates in patients with schizophrenia and schizoaffective disorder (1, 2). Objective: Assess the efficacy of quetiapine XR (QTP-XR) versus risperidone on depressive symptoms in schizophrenia and schizoaffective disorder. Methods: A randomised, open-label, parallel-group, flexible-dose study (NCT00640562). Primary endpoint: LSM change from baseline to Week 12 in Calgary Depression Scale for Schizophrenia (CDSS) (3) score. Secondary endpoints: change in HAM-D and PANSS scores, and adverse events (AEs). No scorrection for multiplicity was done for the secondary endpoints. Results: 216 patients received QTP-XR (n=109; 400–800mg/day) or risperidone (n=107; 4–6mg/day). From baseline, QTP-XR significantly reduced CDSS, HAM-D and PANSS negative total scores compared with risperidone (QTP-XR vs risperidone: -7.31 versus -5.53, p=0.0107; -14.68 versus -11.53, p=0.0005; -8.23 versus -5.45, p=0.0008, respectively). No major differences in AEs were observed between QTP-XR and risperidone. Four serious AEs were experienced with QTP-XR and 5 with risperidone. Two patients receiving QTP-XR died (unrelated to study drug). Prolactin levels were significantly reduced from baseline with QTP-XR versus risperidone (-9.15ng/mL and +22.18ng/mL respectively; p<0.0001). No important differences were seen in other laboratory parameters. Conclusions: In this study, QTP-XR was superior to risperidone at reducing depressive symptoms in schizophrenia or schizoaffective disorder according to CDSS, HAM-D and PANSS negative scores. Computed tomography is not often performed in the postoperative staging of colon cancer, however, it is mandatory if laparoscopy has to be performed.

Previous research has identified links between changes in sea surface temperature (SST) and hurricane intensity. We use climate models to study the possible causes of SST changes in Atlantic and Pacific tropical cyclogenesis regions. The observed SST increases in these regions range from 0.32°C to 0.67°C over the 20th century. The 22 climate models examined here suggest that century-time-scale SST changes of this magnitude cannot be explained solely by unforced variability of the climate system. We employ model simulations of natural internal variability to make probabilistic estimates of the contribution of external forcing to observed SST changes. For the period 1906–2005, we find an 84% chance that external forcing explains at least 67% of observed SST increases in the two tropical cyclogenesis regions. Model ‘20th-century’ simulations, with external forcing by combined anthropogenic and natural factors, are generally capable of replicating observed SST increases. In experiments in which forcing factors are varied individually rather than jointly, human-caused changes in greenhouse gases are the main driver of the 20th-century SST increases in both tropical cyclogenesis regions. Background and aims: Transfer of CD4+CD45RBHi T cells into semi syngeneic immunodeficient mice represents a model of inflammatory bowel disease (IBD). As patients with IBD often suffer from osteopenia, we studied if this T cell transfer in mice results in osteopenia in addition to colitis, and if treatment with osteoprotegerin (OPG) has effects on the bone mineral density of T cell transferred mice. We also investigated whether osteopenia was due to malabsorption as a result of a dysregulated digestive tract or as a consequence of the !nflammatotv process. Methods: CD4+CD45RBHi or CD4+CD45RBLo T cells (4 × 105) were sorted from CB6F1 and transferred into C.B.17 scid/scid mice. Recipient mice were treated with human IgG1 Fc (control) or Fc-OPG three times per week in a prophylactic regimen as well as a therapeutic regimen (after 10% body weight loss) and were evaluated for osteopenia and colitis. Results: Mice that received CD4+CD45RBHi T cells developed osteopenia (as indicated by decreased bone density accompanied by decreased osteoblasts and increased osteodasts) and colitis (as indicated by histological changes in the large intestine). Mice that received CD4+D45RBHi T cells developed neither osteopenia nor colitis. All animals consumed, on average, the same amount of food and water over the course of the study. Prophylactic treatment with Fc-OPG increased bone density in mice that received either CD4+CD45RBHi or CD4+CD45RBLo T cells but had no effects on the gastrointestinal tract. Fc-OPG treatment of osteopenic mice with established IBD caused the normalisation of bone density. Osteopenia in CD4+CD45RBHi T cell recipients was accompanied by hypoparathyroidism that was partially normalised by treatment with Fc-OPG. CD4+CD45RBHi T cell recipients also had a bone marrow inflammatory cell infiltrate expressing tumour necrosis factor α which was unaffected by treatment with Fc-OPG. Conclusions: CD4+CD45RBHi T cell transfer results in osteopenia in addition to colitis. Evidence suggests that this osteopenia was induced by inflammatory cell infiltration and not by malabsorption of calcium. Recombinant human osteoprotegerin effectively treated the osteopenia. The NCEP–DOE Atmospheric Model Intercomparison Project (AMIP-II) reanalysis is a follow-on project to the “50-year” (1948-present) NCEP-NCAR Reanalysis Project. NCEP–DOE AMIP-II reanalysis covers the “20-year” satellite period of 1979 to the present and uses an updated forecast model, updated data assimilation system, improved diagnostic outputs, and fixes for the known processing problems of the NCEP-NCAR reanalysis. Only minor differences are found in the primary analysis variables such as free atmospheric geopotential height and winds in the Northern Hemisphere extratropics, while significant improvements upon NCEP-NCAR reanalysis are made in land surface parameters and land-ocean fluxes. This analysis can be used as a supplement to the NCEP-NCAR reanalysis especially where the original analysis has problems.
