Digit ratio (2D:4D) – a proxy for prenatal sex steroids – shows sex, nationality and ethnic differences and is linked to pubertal onset. It is unclear whether right-left 2D:4D (Dr-l) also correlates with prenatal sex steroids, as evidence of these differences has been less conclusive. The present study examined the effects of sex, nation, ethnicity, age and self-reported pubertal development (i.e. the rate of physical development and age at menarche [females] or first shave [males]) on Dr-l in a large online study (the BBC internet study). Digit lengths were self-measured in 201,865 adults (110,955 males) and the sample of nations included 41 countries. Participants reported the self-perceived rate of physical pubertal development on a five-point scale (from very slow to very fast) and provided information on the age at menarche or first shave. Adult (>17 years) males had lower Dr-l than females with weak effect size across 41 nations (males-females; d = −.065, p <.0001). There were sex and ethnicity effects on Dr-l across seven ethnic groups with males < females and lower Dr-l in Whites and Middle/Near Eastern participants compared to Asian, Black and Chinese respondents. Considering age effects, the authors focused on participants >12 years; there were stable sex differences and a weak positive effect of age on Dr-l. Dr-l showed a positive relationship with the rate of physical development and a negative relationship with age at menarche or first shave. Relationships were present in males and females with stronger effects in the latter. Digit ratio – a putative measure of prenatal sex steroids – may be related to body mass index (BMI). However, reports of correlations between 2D:4D and BMI have yielded mixed results with some studies showing no relationship while others have reported positive associations in men or women only. This study considers associations between self-reported 2D:4D and BMI in a large online survey (i.e. the BBC Internet Study). At the individual level, there was a weak positive association between 2D:4D and BMI in both sexes with greater effect sizes in women. Body mass index was positively related to age and negatively related to parental income; however, the relationship between 2D:4D and BMI was independent of both variables. At the national level, mean 2D:4D per country showed positive associations with mean national BMI but those correlations were restricted to females. It is concluded that BMI is positively related to low prenatal testosterone and high prenatal oestrogen. Parental income inequality may influence both prenatal sex steroids (through a 'Trivers–Willard' effect) and BMI such that increases in inequality result in reductions in prenatal testosterone and increases in BMI at the individual and national level. Income inequality is associated positively with disease prevalence and mortality. Digit ratio (2D:4D) – a negative proxy for prenatal testosterone and a positive correlate of prenatal oestrogen – is related to several diseases. This study examined the association of income inequality (operationalized as relative parental income) and children's 2D:4D. Participants self-measured finger lengths (2D=index finger, and 4D=ring finger) in a large online survey conducted in July 2005 (the BBC Internet Study) and reported their parents' income. Children of parents of above-average income had low 2D:4D (high prenatal testosterone, low prenatal oestrogen) while the children of parents of below-average income had high 2D:4D (low prenatal testosterone, high prenatal oestrogen). The effects were significant in the total sample, present among Whites (the largest group in the sample), in the two largest national samples (UK and USA) and were greater for males than females. The findings suggest a Trivers-Willard effect, such that high-income women may prenatally masculinize their sons at the expense of the fitness of their daughters. Women with low income may prenatally feminize their daughters at the fitness expense of their sons. The challenges of terrestrial existence may mean that the early development of tetrapod limb traits is linked to the development of the urogenital system and sex steroids. One such limb trait is the sex-dependent ratio of the lengths of the 2nd and 4th digits (2D:4D). Direct evidence for the association between early sex steroids and offspring 2D:4D can be obtained by manipulating foetal sex hormones. However, this is not ethically permissible in humans. It is widely accepted that 2D:4D is a biomarker for early foetal sex hormones in tetrapods but the link in humans remains controversial. Here we review the evidence that (i) manipulation of sex steroids in early development leads to sex-dependent changes in 2D:4D throughout the tetrapods, and (ii) maternal sex steroids cross the placenta and thus are associated with offspring 2D:4D in both non-human and human animals. We suggest a research focus on associations between human maternal sex steroids and offspring 2D:4D to clarify the link between 2D:4D and early sex steroids. A protocol is proposed to examine the correlation between 1st-trimester maternal sex steroids and offspring 2D:4D. Abstract: Objective: Research documents that even subtle changes in visible skin condition affect perceptions of age, health and attractiveness. There is evidence that hair quality also affects the assessment of physical appearance, as variations in hair diameter, hair density and hairstyle have systematic effects on perception. Here, we consider combined effects of hair colour and skin colour on the perception of female physical appearance. Methods: In two experiments, we digitally manipulated facial skin colour of lightly pigmented, young women, both between‐subjects (Experiment 1) and within‐subjects (Experiment 2), and investigated possible interactions with hair colour in regard to age, health, and attractiveness perception. Results: In both experiments, we detected hair colour and skin colour interaction effects on men's and women's assessments. For between‐subjects comparisons, participants with lighter hair colour were judged to be younger than those with darker shades; this effect was more pronounced in women with light skin colour. No such effect was observed for within‐subjects variation in skin colour. Both experiments showed that smaller perceived contrast between hair colour and skin colour resulted in more positive responses. Conclusion: We conclude that hair colour and facial skin colour together have an effect on perceptions of female age, health, and attractiveness in young women, and we discuss these findings with reference to the literature on the role of hair and skin in the assessment of female physical appearance. Abstract: Objective: Previous studies investigating the effects of skin surface topography and colouration cues on the perception of female faces reported a differential weighting for the perception of skin topography and colour evenness, where topography was a stronger visual cue for the perception of age, whereas skin colour evenness was a stronger visual cue for the perception of health. We extend these findings in a study of the effect of skin surface topography and colour evenness cues on the perceptions of facial age, health and attractiveness in males. Methods: Facial images of six men (aged 40 to 70 years), selected for co‐expression of lines/wrinkles and discolouration, were manipulated digitally to create eight stimuli, namely, separate removal of these two features (a) on the forehead, (b) in the periorbital area, (c) on the cheeks and (d) across the entire face. Omnibus (within‐face) pairwise combinations, including the original (unmodified) face, were presented to a total of 240 male and female judges, who selected the face they considered younger, healthier and more attractive. Results: Significant effects were detected for facial image choice, in response to skin feature manipulation. The combined removal of skin surface topography resulted in younger age perception compared with that seen with the removal of skin colouration cues, whereas the opposite pattern was found for health preference. No difference was detected for the perception of attractiveness. These perceptual effects were seen particularly on the forehead and cheeks. Removing skin topography cues (but not discolouration) in the periorbital area resulted in higher preferences for all three attributes. Conclusion: Skin surface topography and colouration cues affect the perception of age, health and attractiveness in men's faces. Background Visible facial skin condition in females is known to affect perception of age, health and attractiveness. Skin colour distribution in shape- and topography-standardized female faces, driven by localized melanin and haemoglobin, can account for up to twenty years of apparent age perception. Although this is corroborated by an ability to discern female age even in isolated, non-contextual skin images, a similar effect in the perception of male skin is yet to be demonstrated. Objectives To investigate the effect of skin colour homogeneity and chromophore distribution on the visual perception of age, health and attractiveness of male facial skin. Methods Cropped images from the cheeks of facial images of 160 Caucasian British men aged 10-70 years were blind-rated for age, health and attractiveness by a total of 308 participants. In addition, the homogeneity of skin images and corresponding eumelanin/oxyhaemoglobin concentration maps were analysed objectively using Haralick's image segmentation algorithm. Results Isolated skin images taken from the cheeks of younger males were judged as healthier and more attractive. Perception of age, health and attractiveness was strongly related to melanin and haemoglobin distribution, whereby more even distributions led to perception of younger age and greater health and attractiveness. The evenness of melanized features was a stronger cue for age perception, whereas haemoglobin distribution was associated more strongly with health and attractiveness perception. Different operative techniques of stem revision for the treatment of periprosthetic fractures type Vancouver B2 and B3 are described. Our prospective study of 32 periprosthetic fractures (22 Vancouver type B2, 10 Vancouver type B3) with a follow-up period of at least 24 months showed that stem revision employing a transfemoral surgical approach and a modular, cementless, tapered and fluted revision stem led to reproducibly good results with respect to fracture healing, stability of the prosthesis stem, dislocation, intraoperative fracture, and clinical outcome. All fractures healed with a mean time of 14.5 ± 5.2 weeks. No cases of subsidence of the stem were observed and, according to the classification of Engh et al. concerning the biological fixation of the stem, bony ingrowth fixation was observed in 28 cases and stable fibrous fixation was observed in 4 cases. One dislocation and one case of deep vein thrombosis occurred, and there were no cases of intraoperative fracture. The Harris Hip Score rose continually (3 months postoperation: 59.2 ± 14.6 points; 24 months postoperation: 81.6 ± 16.5 points). Synopsis Although there is evidence that perception of facial age, health and attractiveness is informed by shape characteristics as well as by visible skin condition, studies on the latter have focused almost exclusively on female skin. Recent research, however, suggests that a decrease in skin colour homogeneity leads to older, less healthy and less attractive ratings of facial skin in both women and men. Here, we elaborate on the significance of the homogeneity of visible skin colouration in men by testing the hypothesis that perception of age, health and attractiveness of (non-contextual) digitally isolated fields of cheek skin only can predict that of whole facial images. Facial digital images of 160 British men (all Caucasian) aged between 10 and 70 were blind-rated for age, health and attractiveness by a total of 147 men and 154 women (mean age = 22.95, SD = 4.26), and these ratings were related to those of corresponding images of cheek skin reported by Fink et al. ( J. Eur. Acad. Dermatol. Venereol. in press). Linear regression analysis showed that age, health and attractiveness perception of men's faces could be predicted by the ratings of cheek skin only, such that older men were viewed as older, less healthy and less attractive. This result underlines once again the potent signalling role of skin in its own right, independent of shape or other factors and suggests strongly that visible skin condition, and skin colour homogeneity in particular, plays a significant role in the perception of men's faces. Perception of age and health is critical in the judgement of attractiveness. The few studies conducted on the significance of apparent skin condition on human physical appearance have studied faces alone or isolated fields of images facial skin. Little is known about whether perception of the face matches that of other body parts or if body skin affects overall age and attractiveness perception when presented in combination with facial skin. We hypothesized that independent presentation of female faces, chests and arms (including hands) - cropped from a full face and upper body image - would result in significant differences in perception of age and attractiveness compared to the corresponding composite. Furthermore, we sought to investigate whether relatively young and attractive looking skin on selected, individual parts of the body affects overall perception. Digital photographs of 52 women aged 45-65 years were collected and processed to yield four derivative sets of images: One set showed the composite of all features, i.e. the face, the chest and the arms, whereas the other three were cropped carefully to show each part of the upper body described above independently. A total of 240 participants judged these faces for perceived age and attractiveness. Our results showed significant differences in perception with the chest and the arms being judged significantly younger than the face or composite image of the same women. Moreover, arms and chest images were perceived as more attractive than face and composite images. Finally, regression analysis indicated that differences between the perceived and chronological values of overall age perception could be predicted by age perception of the face and arms. These results continue to support the significance of facial age perception in assessment of a woman's age, but highlight that body skin also plays a role in overall age impression. Background: Articulating spacers used in two-stage revision surgery of infected prostheses have the potential to abrade and subsequently induce third-body wear of the new prosthesis.Questions/purposes: We asked whether particulate material abraded from spacers could be detected in the synovial membrane 6 weeks after implantation when the spacers were removed for the second stage of the revision.Patients and Methods: Sixteen hip spacers (cemented prosthesis stem articulating with a cement cup) and four knee spacers (customized mobile cement spacers) were explanted 6 weeks after implantation and the synovial membranes were removed at the same time. The membranes were examined by xray fluorescence spectroscopy, xray diffraction for the presence of abraded particles originating from the spacer material, and analyzed in a semiquantitative manner by inductively coupled plasma mass spectrometry. Histologic analyses also were performed.Results: We found zirconium dioxide in substantial amounts in all samples, and in the specimens of the hip synovial lining, we detected particles that originated from the metal heads of the spacers. Histologically, zirconium oxide particles were seen in the synovial membrane of every spacer and bone cement particles in one knee and two hip spacers.Conclusions: The observations suggest cement spacers do abrade within 6 weeks. Given the presence of abrasion debris, we recommend total synovectomy and extensive lavage during the second-stage reimplantation surgery to minimize the number of abraded particles and any retained bacteria. Synopsis Evolutionary psychology suggests that certain human beauty standards have evolved to provide reliable cues of fertility and health. Hence, preferences for some physical characteristics of the face and body are thought to reflect adaptations for the promotion of mate choice. Studies that have investigated facial attractiveness have concentrated mainly on features such as symmetry, averageness and sex-typical traits, which are developed under the influence of sex steroids. Few studies, however, have addressed the effect of human skin condition on perception of facial appearance in this context, and possible implications for sexual selection. There is now accumulating evidence that skin pigmentation and skin surface topography cues, particularly in women, have a significant influence on attractiveness judgements, as they seem primarily to signal aspects of age and health. This article (i) reviews briefly some of the main determinants of visible skin condition, (ii) presents recent evidence on its signalling value in face perception and (iii) suggests areas for future research with reference to an evolutionary psychology framework. Unlabelled: Cementless two-stage revision of infected total hip prostheses lacks the possibility of local antibiotic protection of the implant at the time of reimplantation, which leads to the concern that this protocol may not sufficiently eradicate periprosthetic infection. Moreover, early implant loosening as much as 18% and stem subsidence as much as 30% have been reported. To determine whether a cementless revision could eradicate infection and achieve sufficient implant stability, we prospectively followed 36 patients with two-stage revisions for septic hip prostheses. We used a uniform protocol of a 6-week spacer interval, specific local and systemic antibiotic therapies, and cementless modular revision stems. The minimum followup was 24 months (mean, 35 months; range, 24-60 months). In one patient, the spacer was changed when the C-reactive protein value failed to normalize after 6 weeks, and the reimplantation was performed after an additional 6 weeks. No infections recurred. There was no implant loosening and a 94% bone-ingrowth fixation of stems. Subsidence occurred in two patients. The Harris hip score increased from a preoperative mean of 41 to 90 at 12 months after reimplantation and later. Using cementless prostheses in two-stage revisions of periprosthetic infections of the hip in combination with a specific local and systemic antibiotic therapy seems to eradicate infection and provide implant stability.Level Of Evidence: Level IV, therapeutic study. Background: Rheumatoid arthritis often leads to severe destruction of the glenohumeral joint, including synovitis and inflammation-induced alterations of the rotator cuff. Cup arthroplasty, or surface replacement of the shoulder, was introduced in the 1980s. The aim of this study was to evaluate the midterm results of the DUROM cup surface replacement for patients with rheumatoid arthritis affecting the glenohumeral joint.Methods: From 1997 to 2000, forty-two DUROM cup hemiprostheses were implanted in a cohort of thirty-five patients (forty-two shoulders), who were evaluated preoperatively and again at three, twelve, and more than sixty months postoperatively. Six patients (seven shoulders) were lost to follow-up. Thirty-five shoulders in twenty-nine patients (twenty-one women and eight men with an average age of 61.4 years) could be evaluated prospectively after an average follow-up period of 73.1 months. Patients were evaluated clinically with the use of the Constant score, and a detailed radiographic analysis was performed to determine the presence of endoprosthetic loosening, glenohumeral subluxation, and glenoid bone loss.Results: The mean Constant score for the thirty-five shoulders increased from 20.8 points preoperatively to 64.3 points at a mean of 73.1 months postoperatively. There were three revisions: one to replace an implant that was too large, another to treat glenoid erosion, and a third due to loosening of the implant. No additional cases of loosening of the prosthesis or changes in cup position were observed radiographically. Over the five-year follow-up period, proximal migration of the cup increased in 63% of the shoulders, and glenoid depth increased in 31%. With the numbers studied, no differences in clinical outcome were identified between patients with a massive rotator cuff tear and those with a smaller or no tear.Conclusions: The midterm results of the cemented DUROM cup surface replacement for patients with advanced rheumatoid arthritis of the shoulder are very encouraging, even for patients with a massive tear of the rotator cuff. According to evolutionary psychology, the preference for some facial characteristics reflects adaptations for mate choice because they signal aspects of mate quality. Although morphological features such as facial symmetry and sexually dimorphic traits have been studied extensively in recent years, little is known about skin condition in this context. The preferences for young and healthy looking skin could offer an explanation as to why women place such an importance on the condition of their skin and its refinement through e.g., cosmetic products. Recent research showed that facial skin colour distribution significantly influences the perception of age and attractiveness of female faces, independent of skin surface topography cues. However, the relative effect of skin colour distribution and topography cues on age and health perception remains to be investigated. We present data showing that both skin colour distribution and skin surface topography cues not only significantly influence the perception of female facial age and health but also convey differential information with regard to the strength of these effects. Our data indicate that skin surface topography cues account for a large proportion of variation in facial age perception, whereas skin colour distribution seems to be a stronger health cue. Background:The ratio of the lengths of the 2nd and 4th digit (2D:4D) is negatively related to prenatal and adult concentrations of testosterone (T). Testosterone appears to be a protective against myocardial infarction (MI) in men as men with low 2D:4D are older at first MI than men with high 2D:4D, and men with coronary artery disease have lower T levels than men with normal angiograms. Neck circumference (NC), a simple and time-saving screening measure to identify obesity is reported to be positively correlated with the factors of the metabolic syndrome, a complex breakdown of normal physiology characterized by obesity, insulin resistance, hyperlipidemia, and hypertension, and is therefore likely to increase the risk of coronary heart disease (CHD).Objective:To investigate possible associations between 2D:4D ratios and NC in men and women.Research methods and procedures:2D:4D ratios, NC, along with measures of waist and hip circumferences, body mass index (BMI), and waist-to-hip ratio was recorded from 127 men and 117 women.Results:A significant positive correlation between 2D:4D and NC and was found for men but not for women after controlling for body mass index (BMI); the higher the ratio the higher the NC.Discussion:This finding supports the suggestion of NC to serve as a predictor for increased risk for CHD as previously suggested. Synopsis It was once widely believed that standards of beauty were arbitrarily variable. Recent research suggests, however, that people's views of facial attractiveness are remarkably consistent, regardless of race, nationality or age. Facial characteristics are known to influence human attractiveness judgements and evolutionary psychologists suggest that these characteristics all pertain to health, leading to the conclusion that humans have evolved to view certain bodily features as attractive because the features were displayed by healthy others. Background : Prenatal sex steroids have been broadly discussed in terms of their possible effect on brain differentiation, whereas pubertal/adult sex hormones are thought to be the main regulators of sexually dimorphic physical features in males and females. Assessing prenatal steroid exposure has previously been difficult but evidence now suggests that finger length ratio may provide a 'window' into prenatal hormone exposure. The length of the second digit (the index finger) relative to the length of the fourth digit (the ring finger) is sexually dimorphic as males have a lower second to fourth digit ratio (2D:4D). The sexual dimorphism is determined as early as the 14th week of fetal life, and remains unchanged at puberty. There is evidence that sex differences in 2D:4D arise from in utero concentrations of sex steroids, with a low 2D:4D (male typical ratio) being positively related to prenatal testosterone, while a high 2D:4D (female typical ratio) is positively associated with prenatal oestrogen. Aim : The studied aimed to determine whether, and to what extent, adult sexually dimorphic physical traits, which are largely determined at puberty, relate to traits that are largely determined in utero . This work examined the relationship between three sexually dimorphic traits--body mass index (BMI), waist-to-hip ratio (WHR) and waist-to-chest ratio (WCR)--and digit ratio. Participants and methods : BMI, WHR and WCR were assessed in 30 heterosexual male and 50 heterosexual female participants by recording their body height, weight, and measuring their waist, hip and chest circumference. Digit lengths of the second and fourth fingers were measured from photocopies of the ventral surface of the hand and by actual finger measurements. Results : Digit ratio was found to be significantly lower in men than in women. Significant negative correlations were found between female's left and right hand 2D:4D, waist and hip circumference, and WCR. In males, BMI was found to be positively related to digit ratio but remained significant only for left hand 2D:4D. Generally, the relationships were stronger for females than for males. Although not all relationships were found to be significant, they were in accord with our predictions. Conclusion : In addition to an activational effect of sex hormones at puberty, the present data suggest an early organizational effect of sex hormones through the association between indices of female body shape, male BMI, and human finger length patterns. The human face communicates an impressive number of visual signals. Although adults' ratings of facial attractiveness are consistent across studies, even cross-culturally, there has been considerable controversy surrounding attempts to identify the facial features that cause faces to be judged attractive or unattractive. Studies of physical attractiveness have attempted to identify the features that contribute to attractiveness by studying the relationships between attractiveness and (a) symmetry, (b) averageness, and (c) nonaverage sexually dimorphic features (hormone markers). Evolutionary psychology proposes that these characteristics all pertain to health, suggesting that humans have evolved to view certain features as attractive because they were displayed by healthy individuals. However, the question remains how single features that are considered attractive relate to each other, and if they form a single ornament that signals mate quality. Moreover, some researchers have recently explained attractiveness preferences in terms of individual differences that are predictable. The article presents a study concerning soy intake to reduce the risk of breast cancer among Asian women. According to the study, by Wu-Williams and company shows that high intake of 10-20 milligrams per day of isoflavones that are found among Asian populations, reduces at least 10-30% risk of breast cancer. However, some studies seems to contradict with their findings, additional research is needed in order to shade light to some questions surrounding the breast cancer reductions.

The amount of new information in all subspecialties of orthopaedic surgery continues to increase at an exponential rate. It is no longer possible for one textbook to contain all of the information necessary to practice state-of-the-art orthopaedic surgery. To keep abreast of this tide of information, most major textbooks are being revised more frequently. Similarly, a single specialty journal can no longer be expected to cover all of the important advances in all of the subspecialties. Like other subspecialties, the field of foot and ankle surgery is perpetually evolving. Many procedures that were considered on the cutting edge ten years ago have become either routine or obsolete. The purpose of this report is to review the information and ideas presented at recent meetings of the American Orthopaedic Foot and Ankle Society (AOFAS), the American Academy of Orthopaedic Surgeons (AAOS), and other specialty and subspecialty associations. Pertinent points from recent articles in Foot and Ankle International will also be highlighted in an effort to provide an overview of new concepts and major advances in foot and ankle surgery. Abstract: Vulnerability of the fetus upon maternal obesity can potentially occur during all developmental phases. We aimed at elaborating longer-term health outcomes of fetal overnutrition during the earliest stages of development. We utilized Naval Medical Research Institute (NMRI) mice to induce pre-conceptional and gestational obesity and followed offspring outcomes in the absence of any postnatal obesogenic influences. Male adult offspring developed overweight, insulin resistance, hyperleptinemia, hyperuricemia and hepatic steatosis; all these features were not observed in females. Instead, they showed impaired fasting glucose and a reduced fat mass and adipocyte size. Influences of the interaction of maternal diet∗sex concerned offspring genes involved in fatty liver disease, lipid droplet size regulation and fat mass expansion. These data suggest that a peri-conceptional obesogenic exposure is sufficient to shape offspring gene expression patterns and health outcomes in a sex- and organ-specific manner, indicating varying developmental vulnerabilities between sexes towards metabolic disease in response to maternal overnutrition. Background: The rate of infection control for one-stage revision of infected knee arthroplasties is unclear as are the factors influencing infection control. Such factors include duration of infection and the type of infected prosthesis.Questions/purposes: We therefore determined: (1) the rate of infection control with one-stage revision of septic knee prostheses, (2) the clinical knee scores that can be achieved, (3) whether the duration of infection or the type of prosthesis influence the level of infection control, and (4) whether different types of prostheses influence the knee scores.Methods: We retrospectively reviewed prospectively collected data from 63 patients who underwent one-stage revisions of septic knee endoprostheses (six unicondylar, 37 primary total knee replacement prostheses, and 20 hinged knee endoprostheses) between 2004 and 2006. All were treated locally and systemically with microorganism-specific antibiotics. For this study we excluded patients with Methicillin-resistant Staphylococcus aureus and Methicillin-resistant Staphylococcus epidermidis or unknown microorganisms. The patients were examined for infection every 3 months and Oxford and Knee Society scores were assessed at the same time. The minimum followup was 24 months (mean followup, 36 months; range, 24-70 months).Results: None of the patients with replacement unicondylar and primary total knee replacement prostheses had recurrence of infection. Three of the 20 patients with the hinged infected knee prostheses had recurrences; these three patients had chronic infections and had undergone two to three revision operations during at least a 5-year period. The likelihood of infection control was influenced by the duration of infection. The mean Knee Society knee score 24 months after surgery was 72 points (range, 20-98 points), the Knee Society function score was 71 points (range, 10-100 points), and the Oxford-12 knee score was 27 points (range, 13-44 points).Conclusions: One-stage revision of septic knee prostheses achieved an infection control rate of 95% and higher knee scores than reported for two-stage revisions. In three experiments, match-to-sample procedures were used with undergraduates to establish arbitrary relational functions for three abstract visual stimuli. In the presence of samples A, B, and C, participants were trained to select the smallest, middle, and largest member, respectively, of a series of three-comparison arrays. In Experiment 1, the B (choose middle) stimulus was then used to train a steady rate of keyboard pressing before the A (choose smallest) and the C (choose largest) stimuli were presented. Participants pressed slower to A and faster to C than to B. Then B was paired with mild shock in a Pavlovian procedure with skin conductance change as the dependent variable. When presented with A and C, 6 of 8 experimental participants showed smaller skin conductance changes to A and larger skin conductance changes to C than to B. In Experiment 2, A was then used as a sample in a match-to-sample procedure to establish an arbitrary size ranking among four same-sized colored circle comparisons. One of the middle circles was then used to establish a steady rate of pressing before the other circles were presented. Five of 6 participants responded slower to the "smaller" circle and faster to the "larger" circle than they did to the "middle" circle. In Experiment 3, A, B, and C were then presented on a series of test trials requiring participants to pick the comparison that was less than, greater than, or equal to the sample. Novel stimuli were included on some trials. Results indicated that the relational training procedures produced derived relations among the stimuli used in training and that these allowed correct inferences of relative size ranking among novel stimuli. BACKGROUND: Double-row arthroscopic rotator cuff repair has become more popular, and some studies have shown better footprint coverage and improved biomechanics of the repair. HYPOTHESIS: Double-row rotator cuff repair leads to superior cuff integrity and early clinical results compared with single-row repair. STUDY DESIGN: Randomized controlled trial; Level of evidence, 1. METHODS: Forty patients were randomized to either single-row or double-row rotator cuff repair at the time of surgical intervention. Patients were followed with clinical measures (UCLA, Constant, WORC, SANE, ASES, as well as range of motion, internal rotation strength, and external rotation strength). Magnetic resonance imaging (MRI) studies were performed on each shoulder preoperatively, 6 weeks, 3 months, and 1 year after repair. RESULTS: Mean anteroposterior tear size by MRI was 1.8 cm. A mean of 2.25 anchors for single row (SR) and 3.2 for double row (DR) were used. There were 2 retears at 1 year in each group. There were 2 additional cases that had severe thinning in the DR repair group at 1 year. The MRI measurements of footprint coverage, tendon thickness, and tendon signal showed no significant differences between the 2 repair groups. At 1 year, there were no differences in any of the postoperative measures of motion or strength. At 1 year, mean WORC (SR, 84.8; DR, 87.9), Constant (SR, 77.8; DR, 74.4), ASES (SR, 85.9; DR, 85.5), UCLA (SR, 28.6; DR, 29.5), and SANE (SR, 90.9; DR, 89.9) scores showed no significant differences between groups. Background/aims: To determine if disease severity is associated with a family history of keratoconus. Methods: Markers of disease severity in the CLEK Study cohort were assessed to determine if they could discriminate individuals with and without family history. Logistic regression was used to examine association between corneal scarring, average corneal power, flat and steep keratometry readings, and higher-order root mean square (RMS) wavefront error with family history. Results: In univariate analyses, none of the severity indices had any significant associations with family history; however, contact lens use, gender, and Caucasian race were found to be significant predictors. After controlling for these confounders, there were no significant associations between any severity indices and family history. Conclusions: Presence or absence of family history is not associated with more severe clinical disease, at least when each marker for severity is considered independently. The results of this analysis are important for genetic studies of keratoconus in that it will allow recruitment of keratoconus patients across all stages of disease severity because it does not influence familial aggregation. OBJECTIVE. To characterize the gene expression profile and determine potential diagnostic markers and therapeutic targets in pigmented villonodular synovitis (PVNS). METHODS: Gene expression patterns in 11 patients with PVNS, 18 patients with rheumatoid arthritis (RA), and 19 patients with osteoarthritis (OA) were investigated using genome-wide complementary DNA microarrays. Validation of differentially expressed genes was performed by real-time quantitative polymerase chain reaction and immunohistochemical analysis on tissue arrays (80 patients with PVNS, 51 patients with RA, and 20 patients with OA). RESULTS: The gene expression profile in PVNS was clearly distinct from those in RA and OA. The present study aims to define cell cycle phases, polyploidy and the immunophenotype of proliferating synovial cells in both diseases.: Synovial tissues from patients with proliferative-active RA, localized and diffuse PVNS were analysed by DNA flow cytometry, immunohistochemistry and double immunofluorescence with confocal laser scan microscopy. Expression of macrophage markers (CD68/CD163), fibroblast markers (h4Ph/CD55) and Ki67 antigen was examined. Synovial cells positive for either macrophage or fibroblast markers as well as double-labelled cells were found in both RA and PVNS. In RA, CD68/CD163+ synoviocytes were preferentially located in the vicinity of the synovial lining layer, while they were more randomly distributed in PVNS. Of cases with diffuse PVNS, 20% showed an aneuploid cell pattern. All samples of localized PVNS and RA were diploid. Proliferative activity was significantly higher in aneuploid PVNS.: In spite of their histologically homogeneous appearance, proliferating synovial cells display a heterogeneous immunophenotype in both RA and PVNS, indicating functional properties of both macrophages and fibroblasts. Background. The purpose of this study was to verify a partial bursa-side rupture of the rotator cuff (RC) using different imaging techniques with special emphasis on the validity of a specific method of subacromial arthrography (SAA). Methods. Patients (n=92, mean age 53.8 years) with a subacromial impingement syndrome underwent sonography, magnetic resonance imaging (MRI), and SAA. All diagnostic results were checked by subsequent arthroscopic surgery. Results. Of 31 surgically verified ruptures, 17 showed a partial rupture located towards the bursa. These had been detected by MRI and ultrasound with a sensitivity of 64% and 41%, respectively, while SAA as a diagnostic tool yielded a sensitivity of 82%. In 14 cases of complete RC ruptures, all imaging techniques had a similar sensitivity of 86–93%. Conclusion. It appears that SAA is an adequate and valid diagnostic tool for the detection of partial bursa-sided RC ruptures. Ultrasound and MRI showed a comparably lower sensitivity. Adrenergic responsiveness to acute hypoglycemia is impaired after prior episodes of hypoglycemia. Although circulating epinephrine responses are blunted, associated alterations in adrenal sympathetic nerve activity (SNA) have not been reported. We examined adrenal nerve traffic in normal conscious rats exposed to acute insulin-induced hypoglycemia compared with insulin with (clamped) euglycemia. We also examined adrenal SNA and catecholamine responses to insulin-induced hypoglycemia in normal conscious rats after two antecedent episodes of hypoglycemia (days -2 and -1) compared with prior episodes of sham treatment. Acute insulin-induced hypoglycemia increased adrenal sympathetic nerve traffic compared with insulin administration with clamped euglycemia (165 +/- 12 vs. 118 +/- 21 spikes/s [P < 0.05]; or to 138 +/- 8 vs. 114 +/- 10% of baseline [P < 0.05]). In additional experiments, 2 days of antecedent hypoglycemia (days -2 and -1) compared with sham treatment significantly enhanced baseline adrenal SNA measured immediately before subsequent acute hypoglycemia on day 0 (180 +/- 11 vs. 130 +/- 12 spikes/s, respectively; P < 0.005) and during subsequent acute hypoglycemia (229 +/- 17 vs. 171 +/- 16 spikes/s; P < 0.05). However, antecedent hypoglycemia resulted in a nonsignificant reduction in hypoglycemic responsiveness of adrenal SNA when expressed as percent increase over baseline (127 +/- 5% vs. 140 +/- 14% of baseline). Antecedent hypoglycemia, compared with sham treatment, resulted in diminished epinephrine responsiveness to subsequent hypoglycemia. Norepinephrine responses to hypoglycemia were not significantly altered by antecedent hypoglycemia. In summary, prior hypoglycemia in normal rats increased adrenal sympathetic tone, but impaired epinephrine responsiveness to acute hypoglycemia. Hence, these data raise the intriguing possibility that increased sympathetic tone resulting from antecedent hypoglycemia downregulates subsequent epinephrine responsiveness to hypoglycemia. Alternatively, it is possible that the decrease in epinephrine responsiveness after antecedent hypoglycemia could be the result of reduced adrenal sympathetic nerve responsiveness. Before an open reduction and internal fixation (ORIF) is performed, arthroscopy may help to decide if and what kind of surgery should be chosen. We use SQUID NMR to observe the magnetization of 3He films with densities between 20 and 24 atoms/nm2 in the zero field limit. Ferromagnetism in these nearly 2D Heisenberg exchange systems is stabilized by weak anisotropies. In the ferromagnetic phase, the NMR line becomes very broad and shifts to lower frequencies, consistent with a large dipolar field opposing a perpendicular applied field. Grafoil can be modeled by a Gaussian distribution of platelet angles centered on the normal to the Grafoil plus a randomly distributed set of platelets. Using the spin dynamics of a 2D polarized sheet with reasonable assumptions for the distributions of platelet angles, we show that the magnetization, frequency shift and line shape form a consistent picture of a highly polarized sheet.
