The article presents a case of 83-year-old woman with a history of antiglomerular basement membrane (anti-GBM) disease and congestive heart failure with hemoptysis. Inspiratory crackles and egophony were present in the left lung base suggestive of pneumonia; superimposed pulmonary edema in the left lower lobe of chest. Development of hemoptysis and hypoxemic respiratory failure. Levofloxacin was given and the pulmonary symptoms resolved.
