Developmental disturbances in the permanent dentition are commonly seen due to traumatic injuries to the primary teeth, especially intrusive and avulsion injuries of the overlaying primary incisors. These disturbances may range from enamel discoloration or hypoplasia, crown/root dilaceration, odontoma-like malformation and eruption disturbances. Concrescence is extremely rare in anterior teeth. This case report describes a rare case that showed a concrescence of the unerupted maxillary right lateral incisor and maxillary central incisor with crown dilaceration. Radicular cysts arising from primary teeth are uncommon and, when present as large lesions, may adversely affect the underlying permanent teeth. The purpose of this paper is to describe the management of a very large atypical radicular cyst involving the primary dentition of a nine-year-old boy and causing extensive tooth displacement with transposition of the permanent maxillary left canine between the bicuspids. The root apex of the canine was close to the median level of the lateral border of the nasal cavity and showed incomplete root formation and signs of dilacerations in its apical third. Marsupialization was successfully performed, followed by long-term orthodontic treatment. A traumatic injury to primary maxillary anterior tooth is one of the common causes for problems with the succedaneous tooth leading to it noneruption. A missing anterior tooth can be psychologically and socially damaging to the patient. Despite a wide range of treatment options available, sometimes, it is inevitable to save the natural tooth. This paper describes the immediate replacement of a right central incisor using a fiber-composite resin splint with the natural tooth crown as a pontic following surgical extraction of the dilacerated impacted permanent maxillary central incisor. The abutment teeth can be conserved with minimal or no preparation, thus keeping the technique reversible and can be completed at chair side thereby avoiding laboratory costs. A number of developmental anomalies of morphology are there. However, as compared to the more common oral diseases like caries or periodontal problems, they account for a relatively lower number. When present, they may pose various problems of esthetic, function, malocclusion, or possible disposition to other oral problems. Hence, though rare, their timely diagnosis is very vital in proper treatment planning to avoid unseen complications during extractions, endodontic or orthodontic treatment. The present case is of a patient reporting with two very rare developmental anomalies, that is, fusion and root dilaceration, in contralateral sides of the same patient. A case with a dilacerated maxillary permanent central incisor, treated with forced eruption technique is illustrated and the results of the 4-year follow up are presented. After the successful eruption of the tooth, the root development was completed and the root canal was obliterated. The 4-year follow-up results revealed the tooth to be still functional and the radiographic evaluation showed that the periodontal and periapical tissues were intact and healthy. In conclusion, the impacted dilacerated incisor diagnosed in the early mixed dentition should be treated with the aid of orthodontic traction. The long-term follow up showed that once the tooth is placed in the occlusion properly, it may function well esthetically and preserve its periodontal and periapical health. [ABSTRACT FROM AUTHOR]

Many cultures present several intriguing ethnic dental practices. Therefore, in a multiethnic society, health care professionals have to be attuned to the patients' cultural values and traditions because they impact their attitude in seeking care and the implementation of the professionals' recommendations. Before seeking medical or dental care in the formal health system, families may rely first on an elder or healer for advice and interventions. In East Africa, the unerupted primary canines may be extracted in the first days or months of life because it is believed that they cause persistent fever, vomiting, diarrhea, weight loss, dehydration, infectious diseases and death. The practice is done using nonsterilized instruments leading to harmful localized and systemic sequelae, including fatalities. Alterations in the morphology of permanent teeth following trauma to their predecessors is a challenging task for diagnosis and it may require advanced imaging techniques. Newer imaging modalities have replaced the conventional radiological procedures, as they allow 3D reconstruction images and views which are free of superimposition. Cone Beam CT (CBCT) offers a three-dimensional view, which the conventional two-dimensional dental radiography fails to provide. The current article highlights the advantages of CBCT which are seen in evaluation of the morphological variations and eruptive disturbances in the permanent teeth, as sequelae to trauma in their predecessors. Abstract Hamasha AA, Al-Khateeb T, Darwazeh A. Prevalence of dilaceration in Jordanian adults. International Endodontic Journal , 35 , 910–912, 2002. Aim The purpose of this study was to determine the prevalence of dilaceration in a sample of Jordanian dental patients. Methodology The data were collected from examination of radiographs from a random sample of periapical films. In all, 4655 teeth from 814 dental records at the Faculty of Dentistry, Irbid, Jordan were evaluated. The films were viewed using a magnifying lens and an X-ray viewer. A tooth was recorded as having a dilaceration towards the mesial or distal direction if there was a 90° angle or greater along its long axis using a small ruler aligned along the pulp of the tooth. Dilacerated roots towards the buccal or lingual directions were determined by evaluating the appearance of a round opaque area with a dark shadow in its central region, cast by the apical foramen in which the root canal gave a ‘bull’s-eye' appearance. Results Dilacerations were recorded in 3.78% of the teeth examined. Mandibular third molar teeth were the most commonly affected (19.2%) followed by mandibular first molars (5.6%). The maxillary anterior teeth and mandibular incisors were least affected exhibiting dilaceration in approximately 1% of teeth. Two-thirds of the dilacerations were in the mandible and approximately 5% were in anterior teeth. Conclusion Dilaceration is not common, but it is an important dental anomaly that affects all teeth.
