Aims: The end-tidal carbon dioxide (ETCO2) is frequently measured in cardiac arrest (CA) patients, for management and for predicting survival. Our goal was to study the PaCO2 and ETCO2 in hypothermic cardiac arrest patients.Methods: We included patients with refractory CA assessed for extracorporeal cardiopulmonary resuscitation. Hypothermic patients were identified from previously prospectively collected data from Poland, France and Switzerland. The non-hypothermic CA patients were identified from two French cohort studies. The primary parameters of interest were ETCO2 and PaCO2 at hospital admission. We analysed the data according to both alpha-stat and pH-stat strategies.Results: We included 131 CA patients (39 hypothermic and 92 non-hypothermic). Both ETCO2 (p < 0.001) and pH-stat PaCO2 (p < 0.001) were significantly lower in hypothermic compared to non-hypothermic patients, which was not the case for alpha-stat PaCO2 (p = 0.15). The median PaCO2-ETCO2 gradient was greater for hypothermic compared to non-hypothermic patients when using the alpha-stat method (46 mmHg vs 30 mmHg, p = 0.007), but not when using the pH-stat method (p = 0.10). Temperature was positively correlated with ETCO2 (p < 0.01) and pH-stat PaCO2 (p < 0.01) but not with alpha-stat PaCO2 (p = 0.5). The ETCO2 decreased by 0.5 mmHg and the pH-stat PaCO2 by 1.1 mmHg for every decrease of 1° C of the temperature. The proportion of survivors with an ETCO2 ≤ 10 mmHg at hospital admission was 45% (9/25) for hypothermic and 12% (2/17) for non-hypothermic CA patients.Conclusions: Hypothermic CA is associated with a decrease of the ETCO2 and pH-stat PaCO2 compared with non-hypothermic CA. Abstract: Survival after severe trauma may depend on a structured chain of care from the management at the scene of trauma to hospital care and rehabilitation. In the USA, the trauma system is organized according to a pre-hospital triage by paramedics to facilitate the admission of patients to tertiary trauma centres. In France, trauma patients are transported to the most suitable facility, according to the on-scene triage by an emergency physician. Because French hospital's resources become scarce and expensive, the access to all techniques of resuscitation after severe trauma is restricted to tertiary trauma centres, at the expense of prolonged duration of transfer to these centres with a possible impact on mortality. The Northern French Alps Emergency Network created a regional trauma network system in 2008. This organization was based upon the interplay between the resources of each hospital participating to the network and the categorization of trauma severity at the scene. A regional registry allows the assessment of trauma system, which has included 3,690 severe trauma patients within the past 3years. Bystanders, medical call dispatch centres, and interdisciplinary trauma team should form a structured and continuous chain of care to allocate each severe trauma patient to the best place of treatment. Résumé Contexte Les recommandations internationales proposent des indications pour guider le choix de la technique de reperfusion (fibrinolyse ou angioplastie coronaire) des patients victimes d’infarctus du myocarde avec sus-décalage du segment ST (STEMI) de moins de 12 heures. Ces recommandations mentionnent la nécessité d’adapter celles-ci aux circonstances régionales, au sein de réseaux de soins d’urgence. Objectif Évaluer la conformité du type de reperfusion (thrombolyse ou angioplastie primaire) aux recommandations régionales et identifier les facteurs associés au respect de ces recommandations. Méthode Analyse d’un registre permanent des patients avec un STEMI datant de moins de 12 heures, au sein d’un réseau de prise en charge des urgences, sur trois départements des Alpes du Nord, de janvier 2009 à décembre 2012. Résultats Au total, 2620 patients ont été inclus. Une stratégie de revascularisation a été mise en œuvre pour 2425/2620 (92,6 %) patients. Le type de reperfusion était conforme aux recommandations du réseau pour 1567/2620 (60 %) patients. La conformité aux recommandations de la thrombolyse et de l’angioplastie primaire était respectivement de 47 % (656/1385) et 79 % (911/1149). En analyse multivariée, les facteurs indépendamment associés à la conformité des recommandations du type de reperfusion incluait l’âge > 65 ans (odds ratio [OR] = 1,60 ; IC 95 % : 1,33–1,90), la prise en charge en Haute-Savoie par rapport à l’Isère ou la Savoie (OR = 1,38 ; IC 95 % : 1,12–1,71), une tension artérielle systolique initiale inférieure à 100 mmHg (OR = 1,73 ; IC 95 % : 1,27–2,35), la présence d’un choc cardiogénique (OR = 0,5 ; IC 95 % : 0,3–0,84), d’un stimulateur cardiaque ou bloc de branche gauche à l’ECG (OR = 0,49 ; IC 95 % : 0,28–0,88), et une prise en charge initiale hors réseau (OR = 0,62 ; IC 95 % : 0,40–0,94 ). Une interaction qualitative significative de premier ordre était observée entre la filière initiale de prise en charge (prise en charge hospitalière versus préhospitalière) et le délai de prise en charge. L’OR de conformité aux recommandations associé à un délai de prise en charge inférieur à 3 heures était de 2,05 (IC 95 % : 1,61–2,59) en cas de prise en charge préhospitalière contre 0,67 (IC 95 % : 0,46–0,97) en cas de prise en charge hospitalière. Les évènements indésirables cardiaques (9,1 % versus 8,5 %) et la mortalité (6,4 % versus 5,1 %) au cours du séjour hospitalier, n’étaient pas significativement différents en fonction de la conformité des pratiques de reperfusion aux recommandations. Conclusion Quarante pour cent des patients de notre réseau sont reperfusés par une technique (fibrinolyse ou angioplastie primaire) non conforme aux recommandations régionales. L’identification des facteurs associés à l’absence de conformité permet au comité scientifique du réseau de proposer des axes d’amélioration pour une meilleure prise en charge des patients. Background International guidelines have recommendations for selecting the type of reperfusion (fibrinolysis or angioplasty) in the setting of ST-segment elevation myocardial infarction (STEMI), and suggest that emergency-care networks adapt these recommendations according to the local environment. Aim To assess the proportions of STEMI patients treated with fibrinolysis or angioplasty in accordance with regional guidelines. Method Observational study based on a permanent registry of patients with STEMI of <12 h duration in an emergency network in the French North Alps (Isère, Savoie, Haute-Savoie) from January 2009 to December 2012. Results The registry included 2620 patients. Reperfusion was given in 2425/2620 (93%) of patients. Reperfusion type was in accordance with recommendations in 1567/2620 (60%) patients. Guideline-recommended fibrinolysis and angioplasty were performed in 47% (656/1385) and 79% (911/1149) respectively, of patients. In multivariable analysis, variables independently associated with guideline-recommended reperfusion were: an age < 65 years (OR 1.60; 95%CI 1.33–1.90), being managed in Haute-Savoie versus Isère or Savoie (OR 1.38; 95%CI 1.12–1.71), an arterial tension < 100 mmHg (OR 1.73; 95%CI 1.27–2.35), a cardiogenic shock (OR 0.50; 95%CI 0.30–0.84), a pacemaker or left bundle branch block (OR 0.49; 95%CI 0.28–0.88), and an initial management outside the network (followed by treatment in an interventional centre in the network) (OR 0.62; 95%CI 0.40–0.94). Patients initially treated by mobile intensive care units were more often reperfused in accordance with recommendations when admitted < 3 (versus ≥ 3) h following symptom onset (adjusted OR 2.05; 95% CI 1.61–2.59), while those initially treated by in-hospital emergency units were less often reperfused in accordance with recommendation when treated < 3 h following symptom onset (adjusted OR 0.67; 95% CI 0.46–0.97). In-hospital major adverse cardiac events (9.1% vs. 8.5%) and in-hospital mortality (6.4% vs. 5.1%) were not significantly different between patients reperfused in accordance with (versus not) recommendations. Conclusions Forty percent of patients with STEMI were not reperfused with fibrinolysis or angioplasty in accordance with regional guidelines. Introduction Cette étude a pour objectifs d’évaluer l’impact de la mise en place du ROTEM® et d’un algorithme transfusionnel sur la consommation de produits sanguins labiles et de facteurs de coagulation chez les traumatisés sévères dans un centre hospitalier français. Matériel et méthodes Étude observationnelle, de type avant après, monocentrique, incluant tous les patients traumatisés, ayant un ISS ≥ 13, admis au déchoquage du centre hospitalier Annecy genevois, entre octobre 2010 et juillet 2012. Les patients ont été séparés en 2 groupes, avant et après la mise en place du ROTEM® qui a eu lieu en juillet 2011. Un algorithme décisionnel a été mis en place pour guider la transfusion à cette date. Les données démographiques, de gravité initiale, transfusionnelles (type et quantité), de durée de séjour et de décès hospitalier ont été recueillies et comparées entre les 2 groupes. Résultats Deux cent quatorze patients ont été inclus, dont 105 (49 %) sur la période après ROTEM®, parmi lesquels 48 (46 %) ont eu un ROTEM®. L’âge moyen était de 44,1 (±18,6) ans, significativement plus élevé sur la période après ROTEM®. L’ISS moyen était de 28,4 (±12,3). Le taux de décès était de 15,4 %, significativement supérieur dans la période après ROTEM® (20,9 % vs 10,1 % période avant, p = 0,014). On ne mettait pas en évidence de différence statistiquement significative entre les deux périodes en termes de quantité de produits sanguins administrés ou de durée de séjour hospitalier. L’administration de produits sanguins était comparable entre les 2 périodes mais avec une répartition différente : moindre transfusion de CGR et de PFC et augmentation de la quantité de fibrinogène administrée dans le groupe après ROTEM® ( Fig. 1 , Tableau 1 ) Discussion La combinaison de l’analyse ROTEM®, de l’utilisation d’un algorithme diagnostique et thérapeutique et de fibrinogène, tend à réduire la transfusion de produits sanguins labiles au cours de la prise en charge du polytraumatisé.
