Acute respiratory viral infections pose a significant healthcare burden on the pediatric population globally, but data on the dissemination pattern in the community due to the coronavirus disease 2019 (COVID-19) pandemic are scarce. We conducted a 2-year prospective multicenter study in Catalonia (Spain) that examined the prevalence and coinfection dynamics of respiratory viruses among 1276 pediatric patients from different age groups attending primary care. Coinfection analysis demonstrated complex patterns and revealed a coinfection rate of 23.8% for severe acute respiratory syndrome coronavirus 2, often in association with rhinovirus or influenza A. Respiratory syncytial virus (RSV) causes most of the cases of bronchiolitis and thousands of deaths annually, particularly in infants less than 6 months old. In Catalonia (Spain), infants born between April 2023 and March 2024 aged 0–6 months during their first RSV season have been candidates to receive nirsevimab, the novel monoclonal antibody against RSV, since October 2023. We aimed to analyse the dynamics of all-causes bronchiolitis diagnoses and RSV community infections in the current season and compare them to pre-nirsevimab epidemics. We collected epidemiological data from the Information System for Surveillance of Infections in Catalonia (SIVIC) on daily all-causes bronchiolitis clinical diagnoses and RSV-confirmed cases provided by rapid antigen tests in primary care practices. We calculated the rate ratio (RR) for the incidence of all-causes bronchiolitis for children aged 0-11 m-old with respect to 12-35 m-old between September 2014 and January 2024. We analysed the RR of the incidence of RSV-confirmed infection for 0-11 m-old and 12-35 m-old with respect to the > 35 m-old, from January 2021 to January 2024. We then computed the relative difference of the RR, designated as percentage of reduction of risk, between season 2023/2024 and former epidemics. With a global coverage recorded rate for nirsevimab of 82.2% in January 2024, the age-specific 0-11 m-old RR (95% CI) of RSV infection incidence for > 35 m-old was 1.7 (1.5–2.0) in season 2023/2024. The RR (95% CI) had been 7.4 (5.6–9.9), 8.8 (6.9–11.3), and 7.1 (5.7–8.9) in 2020/2021, 2021/2022, and 2022/2023, respectively. Regarding the incidence of all-causes bronchiolitis for the 0-11 m-old group compared to the 12-35 m-old, the pre-pandemic (2014/2015–2019/2020) and 2022/2023 RR (95% CI) were 9.4 (9.2–9.6) and 6.0 (5.7–6.2), respectively, significantly higher than the RR of 3.6 (3.4–3.8) for the most recent season, 2023/2024. Conclusion: Concurring with the introduction of nirsevimab, the risk of RSV infection for infants aged 0-11 m-old compared to > 35 m-old has been reduced by 75.6% (73.4–77.5) in last season, and the risk for all-causes bronchiolitis for 12-35 m-old by 61.9% (60.9–62.9) from the pre-pandemic period and by 39.8% (39.3–40.2) from the 2022/2023 epidemic, despite high RSV community transmission, especially in older infants What is Known: • RSV is responsible for approximately 70% of bronchiolitis cases and causes severe disease, particularly in infants < 6 months of age. • Nirsevimab effectiveness against RSV-associated disease, particularly hospitalisations, was expected to be around 80%; other Spanish regions, such as Galicia and Valencia, and European countries including Luxembourg and Germany, have already reported good results in implementing nirsevimab to prevent RSV-associated hospitalisations and PICU stays. The implementation of the circular economy in companies undoubtedly requires a transition of their business systems towards more environmentally oriented ones. However, there is a lack of empirical research on the topic in the micro-level. Based on a sample of 404 managers with positions of responsibility in SMEs that are in transition towards a circular economy, this paper analyses at the micro level, through the use of structural equation modelling, how internal and external drivers contribute to this transition process and, at the same time, how the process itself impacts the resilience of the company and, consequently, its transformation process. The obtained results contribute to the literature by observing that internal factors carry more weight than external factors when implementing the CE transition. They illustrate how resilience derived from the CE transition can foster transformative change and demonstrate the CE's impact beyond environmental sustainability, extending its influence into strategic management and organizational fields. The article presents comments on household severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmission and the role of the children, which has been a matter of debate since the beginning of the coronavirus disease 2019 pandemic. Topics include child at risk as a part of the household contact tracing even if asymptomatic, as described in the protocol of the Catalonian Health Department; and pediatricians working in primary care and district hospitals.

The use of cattle manure ( CM) for fertilization presents challenges for optimizing nitrogen (N) use. Our work aimed to assess N efficiencies, in a 6-year experiment with three biennial rotations of four crops: oat-sorghum (first year) and ryegrass-maize (second year) in a rainfed humid Mediterranean area of Spain. Fertilization treatments included the following: control (no N), 250 kg mineral N ha−1 year−1 (250 MN), three CM rates (supplying 170, 250 and 500 kg N ha−1 year−1) and four treatments where the two lowest CM rates were complemented with either 80 or 160 kg mineral N ha−1 year−1. Treatments were distributed randomly in each of three blocks. Maximum dry-matter yield (~44-49 t ha−1 rotation−1) was achieved in the third rotation, and only the control and the 170 CM yielded significantly less. Within the limitations of the EU Nitrate Directive, the N steady state supply of 170 CM always requires a complement of mineral N (80 kg N ha−1) to maximize N agronomic efficiency. The maximum N-fertilizer replacement value (250 CM vs. 250 MN) was 0·67, without significant differences between the two treatments in other N-related efficiency indexes, which indicates that plants took advantage of residual-N effects. Nitrogen losses by leaching in the 250 CM treatment were around 5-7% of the N applied. This reinforces the sustainability of manure recycling in long cropping seasons. [ABSTRACT FROM AUTHOR]

The efficient use by crops of nitrogen from manures is an agronomic and environmental issue, mainly in double-annual forage cropping systems linked to livestock production. A six-year trial was conducted for a biennial rotation of four forage crops: oat-sorghum (first year) and ryegrass-maize (second year) in a humid Mediterranean area. Ten fertilization treatments were introduced: a control (without N); two minerals equivalent to 250 kg N ha −1 year −1 applied at sowing or as sidedressing; dairy cattle manure at a rate of 170, 250 and 500 kg N ha −1 year −1 and four treatments where the two lowest manure rates were supplemented with 80 or 160 kg mineral N ha −1 year −1 . They were distributed according to a randomized block design with three blocks. The highest N mineral soil content was found in the summer of the third rotation, in plots where no manure was applied. The yearly incorporation of manure reduced, in successive cropping seasons, the amount of additional mineral N needed as sidedressing to achieve the highest yields. Besides, in the last two years, there was no need for mineral N application for the manure rate of 250 kg N ha −1 year −1 . This amount always covered the oat-sorghum N uptake. In the ryegrass-maize sequence uptakes were as high as 336 kg N ha −1 year −1 . In the medium term, the intermediate manure rate (250 kg N ha −1 year −1 ) optimizes nutrient recycling within the farming system, and it should be considered in the analysis of thresholds for N of organic origin to be applied to systems with high N demand. Synopsis Companies' perceptions of Corporate Social Responsibility (CSR) have been partially analysed based on top managers and mostly offer comparisons between men and women. According to our best knowledge, no study has focused on women managers and identified how their personal characteristics and values influence their perceptions of CSR. This paper analyses the differences in attitudes towards the different dimensions of CSR focusing on the viewpoint of women managers. Moreover, it measures how personal and professional backgrounds influence the perception of CSR. A total of 206 women in management positions were surveyed. The findings suggest that women in top management positions in Spain value the stakeholder orientation dimension more than they value other CSR dimensions. Furthermore, the results show that personal and professional backgrounds have a limited impact on women's perceptions of CSR. Finally, our results suggest that entrepreneurial women tend to have a better perception of CSR. A three-stage study is being performed in order to assess the adaptation of the traineeships to the EHEA Degrees at Universitat Pompeu Fabra (UPF). First, the situation previous to the EHEA deployment was considered. Second, the necessary changes due to the EHEA as forecasted were considered. Third, the current research, which is still in progress, aims to check student satisfaction in traineeships. This working paper describes thoroughly the methodology, and identifies room for improvement in traineeship performance. As a first conclusion we have found that the incorporation of the traineeships as a curricular subject in the degree studies has been a success, because the students' satisfaction level has increased. Their satisfaction is acceptably high with technical aspects (assignment process, preliminary information, monitoring, etc.) and it is very high with their motivation. Four out of five students believe it has been very important for them do the internships well. Furthermore, with regard to the role of the internships in the labour integration of university graduates, we found that one third of the trainees received an offer to continue the practicums or a labour contract. Abstract: This study explores the joint impact on firm performance of the decision to adopt quality and environmental practices in small service companies. In the literature existing to date insufficient attention has been given to small service companies in general, and to the restaurant sector in particular. Most of the studies conducted into environmental practices have thus focused on the industrial sector and on larger companies. This work is based on 374 surveys involving the managers of restaurants, using structural equation modeling to study the links between the dimensions studied. The findings of this study state that Quality Management Practices have a significant direct impact on market success factors but not on companies'' profit and loss statements. On the other hand, the study shows a significant relationship between Environment Management Practices and financial performance. The article suggests that being proactive on quality and environmental issues can bring small service companies a great number of benefits that improve their competitiveness. Given the current economic environment, high-growth companies are particularly relevant for their contribution to employment generation and wealth. This paper discusses the results of a survey that was conducted in order to gain a deeper understanding of high-growth cooperatives through analyzing their financial profiles and then identifying key contributing factors to their growth. To do this, we compared this particular sample with other cooperatives and other high-growth mercantile companies. The results show the main drivers related to high-growth companies success. They are the competitive advantages based on the surveyed group, modern management techniques, quality and productivity, innovation and internationalization. This article presents a current project of the Pompeu Fabra University which aims at assuring the quality of the external practices that undergraduate students have to do in the context of the European Higher Education Area. A fundamental aspect to guarantee the quality stems from an adequate monitoring of the practices development in the companies and organizations. For this reason, one of the objectives of our project is to define a Practicum monitoring protocol based on the competences to be developed during the practices. This project, also, includes the implementation of this monitoring protocol using ICT, specifically through an integrated block in the Moodle platform that permits to manage the Practium together with others subjects. Abstract: Data encryption is one of the key information security technologies used for safeguarding multimedia content from unauthorised access and manipulation in end-to-end delivery and access chains. This technology, combined with the appropriate cryptographic methods, effectively prevents the content against malicious attacks, so as to protect its authenticity as well as integrity. While encryption-based security is ensuring the authorised consumption of the multimedia content, content adaptation technologies have the primary goal of providing means for wider dissemination of the content across diverse networks, devices and users, and thus enriching user satisfaction and experience of the delivered content within a given set of usage environment constraints. Traditionally, protected contents can only be adapted at trusted adaptation engines residing between the source and end-users, since they have to be fully decrypted before performing the necessary adaptation operations. The drawback of such a process is that it significantly limits the availability and flexibility of adaptation engines applicable for adapting protected contents on the fly. Thus, this paper proposes a novel scalable H.264/advanced video coding (AVC)-compatible video encryption technique, which is also transparent to adaptation engines in an end-to-end video delivery scenario. The proposed technology relies on keeping syntax elements required for performing the adaptation operations clear (i.e., not encrypted). The effectiveness of the proposed technique has been successfully verified in scenarios, where both conventional joint scalable video model (JSVM) bit stream extracting and random packet dropping mechanisms are used. Background: Testing for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection is neither always accessible nor easy to perform in children. We aimed to propose a machine learning model to assess the need for a SARS-CoV-2 test in children (<16 years old), depending on their clinical symptoms. Methods: Epidemiological and clinical data were obtained from the REDCap® registry. Overall, 4434 SARS-CoV-2 tests were performed in symptomatic children between 1 November 2020 and 31 March 2021, 784 were positive (17.68%). We pre-processed the data to be suitable for a machine learning (ML) algorithm, balancing the positive-negative rate and preparing subsets of data by age. We trained several models and chose those with the best performance for each subset. Results: The use of ML demonstrated an AUROC of 0.65 to predict a COVID-19 diagnosis in children. The absence of high-grade fever was the major predictor of COVID-19 in younger children, whereas loss of taste or smell was the most determinant symptom in older children. Conclusions: Although the accuracy of the models was lower than expected, they can be used to provide a diagnosis when epidemiological data on the risk of exposure to COVID-19 is unknown. Measuring soil quality and the use of indicators for its evaluation is a worldwide challenge. In Garrotxa Volcanic Zone Natural Park (northeastern Spain), different parameters related to oribatid mites as indicators of soil quality were evaluated under different land uses: forest, pasture, and a biennial double-crop rotation of forage crops. In forage crops, previous fertilization management (one based on mineral fertilizers, three on cattle manure, and one using both types) was also evaluated. Three samplings (April, June, and September) were performed over one season. Fifty-four oribatid species belonging to 28 families were identified. Abundance was the lowest in June for all land uses (average of 1184 individuals m−2). In the study period, abundance, diversity (Shannon index, H'), and dominance (Berger–Parker index, d) varied with different land uses, with the highest values of abundance and H' in forests (9287 individuals m−2 and 2.19, respectively) and the lowest dominance in forests (d = 0.29) without differences between the other uses. Additionally, in the studied parameters, no differences were associated with previous fertilization management in forage crops. Hypochthoniella minutissima, Xenillus (X.) tegeocranus characterized the forest system, Epilohmannia cylindrica minima the forage crops, and Tectocepheus sarekensis the pasture. Hepatic fibrosis is the consequence of an unresolved wound healing process in response to chronic liver injury and involves multiple cell types and molecular mechanisms. The hepatic endocannabinoid and apelin systems are two signalling pathways with a substantial role in the liver fibrosis pathophysiology—both are upregulated in patients with advanced liver disease. Endogenous cannabinoids are lipid-signalling molecules derived from arachidonic acid involved in the pathogenesis of cardiovascular dysfunction, portal hypertension, liver fibrosis, and other processes associated with hepatic disease through their interactions with the CB1 and CB2 receptors. Apelin is a peptide that participates in cardiovascular and renal functions, inflammation, angiogenesis, and hepatic fibrosis through its interaction with the APJ receptor. The endocannabinoid and apelin systems are two of the multiple cell-signalling pathways involved in the transformation of quiescent hepatic stellate cells into myofibroblast like cells, the main matrix-producing cells in liver fibrosis. The mechanisms underlying the control of hepatic stellate cell activity are coincident despite the marked dissimilarities between the endocannabinoid and apelin signalling pathways. This review discusses the current understanding of the molecular and cellular mechanisms by which the hepatic endocannabinoid and apelin systems play a significant role in the pathophysiology of liver fibrosis. Accurate short-term predictions of COVID-19 cases with empirical models allow Health Officials to prepare for hospital contingencies in a two–three week window given the delay between case reporting and the admission of patients in a hospital. We investigate the ability of Gompertz-type empiric models to provide accurate prediction up to two and three weeks to give a large window of preparation in case of a surge in virus transmission. We investigate the stability of the prediction and its accuracy using bi-weekly predictions during the last trimester of 2020 and 2021. Using data from 2020, we show that understanding and correcting for the daily reporting structure of cases in the different countries is key to accomplish accurate predictions. Furthermore, we found that filtering out predictions that are highly unstable to changes in the parameters of the model, which are roughly 20%, reduces strongly the number of predictions that are way-off. The method is then tested for robustness with data from 2021. We found that, for this data, only 1–2% of the one-week predictions were off by more than 50%. Phosphatase and TENsin homolog (Pten) and p53 are two of the most frequently mutated tumor suppressor genes in endometrial cancer. However, the functional consequences and histopathological manifestation of concomitant p53 and Pten loss of function alterations in the development of endometrial cancer is still controversial. Here, it is demonstrated that simultaneous Pten and p53 deletion is sufficient to cause epithelial to mesenchymal transition phenotype in endometrial organoids. By a novel intravaginal delivery method using HIV1 trans‐activator of transcription cell penetrating peptide fused with a Cre recombinase protein (TAT‐Cre), local ablation of both p53 and Pten is achieved specifically in the uterus. These mice developed high‐grade endometrial carcinomas and a high percentage of uterine carcinosarcomas resembling those found in humans. To further demonstrate that carcinosarcomas arise from epithelium, double Pten/p53 deficient epithelial cells are mixed with wild type stromal and myometrial cells and subcutaneously transplanted to Scid mice. All xenotransplants resulted in the development of uterine carcinosarcomas displaying high nuclear pleomorphism and metastatic potential. Accordingly, in vivo CRISPR/Cas9 disruption of Pten and p53 also triggered the development of metastatic carcinosarcomas. The results unfadingly demonstrate that simultaneous deletion of p53 and Pten in endometrial epithelial cells is enough to trigger epithelial to mesenchymal transition that is consistently translated to the formation of uterine carcinosarcomas in vivo. Multisystem inflammatory syndrome in children (MIS-C) is a rare but severe disease temporarily related to SARS-CoV-2. We aimed to describe the epidemiological, clinical, and laboratory findings of all MIS-C cases diagnosed in children < 18 years old in Catalonia (Spain) to study their trend throughout the pandemic. This was a multicenter ambispective observational cohort study (April 2020–April 2022). Data were obtained from the COVID-19 Catalan surveillance system and from all hospitals in Catalonia. We analyzed MIS-C cases regarding SARS-CoV-2 variants for demographics, symptoms, severity, monthly MIS-C incidence, ratio between MIS-C and accumulated COVID-19 cases, and associated rate ratios (RR). Among 555,848 SARS-CoV-2 infections, 152 children were diagnosed with MIS-C. The monthly MIS-C incidence was 4.1 (95% CI: 3.4–4.8) per 1,000,000 people, and 273 (95% CI: 230–316) per 1,000,000 SARS-CoV-2 infections (i.e., one case per 3,700 SARS-CoV-2 infections). During the Omicron period, the MIS-C RR was 8.2 (95% CI: 5.7–11.7) per 1,000,000 SARS-CoV-2 infections, which was significantly lower (p < 0.001) than that for previous variant periods in all age groups. The median [IQR] age of MIS-C was 8 [4–11] years, 62.5% male, and 80.2% without comorbidities. Common symptoms were gastrointestinal findings (88.2%) and fever > 39 °C (81.6%); nearly 40% had an abnormal echocardiography, and 7% had coronary aneurysm. Clinical manifestations and laboratory data were not different throughout the variant periods (p > 0.05). Conclusion: The RR between MIS-C cases and SARS-CoV-2 infections was significantly lower in the Omicron period for all age groups, including those not vaccinated, suggesting that the variant could be the main factor for this shift in the MISC trend. Regardless of variant type, the patients had similar phenotypes and severity throughout the pandemic. What is Known: • Before our study, only two publications investigated the incidence of MIS-C regarding SARS-CoV-2 variants in Europe, one from Southeast England and another from Denmark. What is New: • To our knowledge, this is the first study investigating MIS-C incidence in Southern Europe, with the ability to recruit all MIS-C cases in a determined area and analyze the rate ratio for MIS-C among SARS-CoV-2 infections throughout variant periods. • We found a lower rate ratio of MISC/infections with SARS-CoV-2 in the Omicron period for all age groups, including those not eligible for vaccination, suggesting that the variant could be the main factor for this shift in the MISC trend. Residential relocation leads to environmental changes, besides being likely to be influenced by sociodemographic characteristics. The relationship between them is, however, still not well described. We aimed to investigate changes in patterns of urban, natural and physical environmental exposures due to relocation and related sociodemographic determinants in a population-based cohort study of children and adolescents. We included individuals between 0 and 17 years of age, registered in primary healthcare records in Catalonia, Spain from 2006 to 2018 and categorized them as non-movers and movers (first residential change). Hierarchical clustering identified groups of individuals based on their exposure to air pollution, green spaces and built environment. Low, medium and high environmental hazard exposure clusters were identified, which were used to identify changes in environmental exposure patterns due to relocation, namely same, worse, and better environment. Multinomial logistic regression determined whether sex, age, nationality and area-level deprivation influenced these changes. From 1,316,138 individuals, 221,058 were movers. Compared to non-movers, movers were more frequently with foreign nationality (12.1% vs. 21.3%, respectively) and more likely to live in the most deprived areas at baseline (19.2% vs. 24.4%). Younger children had higher risk of moving to better or worse environments. Individuals living in areas with the highest levels of deprivation were least likely to move within the same environments (i.e., either moving to better (Odds Ratio [95% Confidence Interval] 1.39[1.34–1.44]) or worse environment (1.56[1.49–1.63])). Compared to people from Spain, those from Africa moved to similar environments (OR of moving to a better environment 0.90 (0.87–0.94) with respect to move to the same environment, and OR of moving to a worse environment 0.82 (0.78–0.85)), those from America tended to move to other environments (1.17 (1.12–1.23) better and 1.09 (1.03–1.15) worse), while those from Asia and Europe moved to better environments (1.22 (1.12–1.33) and 1.20 (1.14–1.27) respectively). No associations were found for sex. The extracellular matrix and the correct establishment of epithelial cell polarity plays a critical role in epithelial cell homeostasis and cell polarity. In addition, loss of tissue structure is a hallmark of carcinogenesis. In this study, we have addressed the role of extracellular matrix in the cellular responses to TGF-β. It is well known that TGF-β is a double-edged sword: it acts as a tumor suppressor in normal epithelial cells, but conversely has tumor-promoting effects in tumoral cells. However, the factors that determine cellular outcome in response to TGF-β remain controversial. Here, we have demonstrated that the lack of extracellular matrix and consequent loss of cell polarity inhibits TGF-β-induced apoptosis, observed when endometrial epithelial cells are polarized in presence of extracellular matrix. Rather, in absence of extracellular matrix, TGF-β-treated endometrial epithelial cells display features of epithelial-to-mesenchymal transition. We have also investigated the molecular mechanism of such a switch in cellular response. On the one hand, we found that the lack of Matrigel results in increased AKT signaling which is sufficient to inhibit TGF-β-induced apoptosis. On the other hand, we demonstrate that TGF-β-induced epithelial-to-mesenchymal transition requires ERK and SMAD2/3 activation. In summary, we demonstrate that loss of cell polarity changes the pro-apoptotic function of TGF-β to tumor-associated phenotype such as epithelial-to-mesenchymal transition. Metformin is a widespread antidiabetic agent that is commonly used as a treatment against type 2 diabetes mellitus patients. Regarding its therapeutic potential, multiple studies have concluded that Metformin exhibits antineoplastic activity on several types of cancer, including endometrial carcinoma. Although Metformin's antineoplastic activity is well documented, its cellular and molecular anticancer mechanisms are still a matter of controversy because a plethora of anticancer mechanisms have been proposed for different cancer cell types. In this study, we addressed the cellular and molecular mechanisms of Metformin's antineoplastic activity by using both in vitro and in vivo studies of Pten-loss driven carcinoma mouse models. In vivo, Metformin reduced endometrial neoplasia initiated by Pten -deficiency. Our in vitro studies using Pten -deficient endometrial organoids focused on both cellular and molecular levels in Metformin's tumor suppressive action. At cellular level, we showed that Metformin is involved in not only the proliferation of endometrial epithelial cells but also their regulation via a variety of mechanisms of epithelial-to-mesenchymal transition (EMT) as well as TGF-β-induced apoptosis. At the molecular level, Metformin was shown to affect the TGF-β signalling., a widely known signal that plays a pivotal role in endometrial carcinogenesis. In this respect, Metformin restored TGF-β-induced apoptosis of Pten -deficient endometrial organoids through a p38-dependent mechanism and inhibited TGF-β-induced EMT on no-polarized endometrial epithelial cells by inhibiting ERK/MAPK signalling. These results provide new insights into the link between the cellular and molecular mechanism for Metformin's antineoplastic activity in Pten -deficient endometrial cancers. Background: There are many different techniques for reducing acute anterior dislocations of the shoulder, and their use depends on surgeon preference. The objective of this study was to compare the pain experienced by a patient performing a self-reduction technique with the pain felt during a reduction performed by a trained physician.Methods: The study was carried out at the emergency department of a tertiary referral center. Patients between 18 and 60 years of age with an acute anterior shoulder dislocation were randomly allocated into 2 groups. In 1 group the emergency doctor actively guided the reduction process with the Spaso technique (Sp group), and in the other group the patient used the Boss-Holzach-Matter (also known as Davos or Aronen) self-reduction technique (BHM group). The pain experienced by the patient during the reduction was recorded by means of a visual analogue scale (VAS) ranging from 0 to 10. Other recorded data included demographic characteristics, reduction time, and success rate.Results: Of 378 patients assessed for eligibility from May 2015 until February 2017, 197 did not meet the inclusion criteria, 58 met exclusion criteria, 22 declined to participate, and 41 withdrew before randomization. Sixty acute anterior shoulder dislocations were randomized into the Sp group (n = 30) or the BHM group (n = 30). The BHM group experienced significantly less pain during reduction than the Sp group (p = 0.047), with mean pain scores of 3.57 (standard deviation [SD] = 2.1]) and 5.26 (SD = 2.9), respectively. No significant difference between groups was found with respect to reduction time (105 seconds [range, 10 to 660 seconds] in the Sp group and 90 seconds [range, 5 to 600 seconds] in the BHM group; p = 0.6) or success rate (67% and 77%, respectively; p = 0.39).Conclusions: The self-reduction technique results in less pain than, and is as efficient in achieving reduction of anterior shoulder dislocations as, the Spaso technique. These findings favor the use of the self-assisted method as an effective first-line treatment for shoulder dislocations seen in the emergency department as well as its use by patients with recurrent dislocation.Level Of Evidence: Therapeutic Level I. Background: The results of research into the outcomes of physical rehabilitation and its relationship with post-myocardial ischaemia survival and readmissions are inconclusive. Our primary aim was to evaluate the efficacy of a supervised exercise training programme in terms of decreasing hospital cardiac readmission in patients with myocardial ischaemia. Methods: We conducted a randomised controlled trial including patients with myocardial ischaemia. Eligible patients were assigned to a control group receiving standard care or to an intervention group that took part in a supervised exercise training programme. The follow-up period was 12 months after hospital discharge. Results: Of 478 patients assessed for eligibility, 86 were randomised to the control group (n = 44) or the intervention group (n = 42). Cardiac readmission rates were 14% versus 5% (p = 0.268) in the control and intervention groups, respectively, and all-cause readmission rates were 23% versus 15% (p = 0.34). There were no deaths in either group. More control patients were treated in the emergency services (50% vs. 24%; p = 0.015). In terms of health-related quality of life, patients in the intervention group presented with significant increases in functional capacity and mobility. More intervention patients returned to work (77.3% vs. 36.0%; p = 0.005). Conclusions: The supervised physical exercise programme was effective at reducing the number of emergency room visits and at increasing the percentage of patients who returned to work. It also improved patients’ exercise capacity and increased their health-related quality of life. Circular economy supposes a transformational and radical process of change from a linear to a circular economic model, where every production phase represents a systemic shift at all levels. Nevertheless, CE philosophy is easy to understand but very complex to put into practice. For that reason, using institutional entrepreneurship theory, institutional enablers to push the transition to a more CE in the European Union will be analysed. In particular, the impacts achieved by CE strategies are oriented to priority CE goals. Thus, this empirical study based on a public consultation survey uses structural equation modelling to analyse links between institutional entrepreneurship enablers and impacts on CE strategies oriented to main CE goals. The findings support the effectiveness of acting like an institutional entrepreneur to force transformational and radical changes, although differences are found between enablers and the impacts of CE. The paper concludes with some useful reflections for institutions and policymakers in order to maximise the efforts taken to effect changes at all levels. [ABSTRACT FROM AUTHOR]

Fluorizoline is a new synthetic molecule that induces apoptosis by selectively targeting prohibitins. In this study, we have assessed the pro-apoptotic effect of fluorizoline in 3 different multiple myeloma cell lines and 12 primary samples obtained from treatment-naïve multiple myeloma patients. Fluorizoline induced apoptosis in both multiple myeloma cell lines and primary samples at concentrations in the low micromolar range. All primary samples were sensitive to fluorizoline. Moreover, fluorizoline increased the mRNA and protein levels of the pro-apoptotic BCL-2 family member NOXA both in cell lines and primary samples analyzed. Finally, NOXA-depletion by CRISPR/Cas9 in cells that do not express BIM conferred resistance to fluorizoline-induced apoptosis in multiple myeloma cells.
