Objective To retrospectively investigate the related risk factors of critical illness in COVID-19 inpatients and predict the critical illness. Methods The clinical data of 2 745 inpatients of CO VID -19 treated in a tertiary hospital from November 2022 to January 2023 were collected and divided into two groups according to the outcome of hospitalization. The risk factors were identified by univariate analysis and multivariate Logistic regression analysis, and the predictive value of combined factors on the prognosis of COVID-19 inpatients was evaluated by the area under the receiver operator characteristic (ROC) curve (AUG), the calibration curve of training set and verification set, and the decision curve analysis (DCA) of training model and verification modeL Results The results of univariate and multivariate Logistic regression analysis showed that age (OR=l,05, 95%CI: 1.03-1.06): operation (OR=8.81,95%CI: 5.64-13.76), prognostic nutrition index (PNI) (OR=0.92, 95%CI: 0.89-0.96),absolute value of lymphocytes (OR二0.47,95%CI: 026-0.83),and pleural effusion (OR二0.49,95%CI: 0.28-0,86) were associated with critical illness of COVID-19 in hospital. The area under the ROC curve showed that the AUG value predicted by several indexes was 0.924. Conclusion Advanced age, high absolute value of neutrophils, low hematocrit, high lactate dehydrogenase, operation, poor nutritional status, low hemoglobin, low platelet count, low absolute value of lymphocytes, and pleural effusion may be the risk factors of critical illness in COVID-19 inpatients, and the combination of several indexes has good predictive value. Early identification of risk factors and taking positive and effective measures are helpful to prevent the occurrence of COVID-19 critical illness. [ABSTRACT FROM AUTHOR]
