This paper provides a critical analysis of the Chartered Institute for Management of Sport and Physical Activity's (CIMSPA) transformative aspirations for UK Higher Education (HE) based Sport and Physical Activity Vocational Education and Training (SPAVET) provision. In doing so, we apply selected elements of Giddens' Structuration theory to offer an analysis of CIMSPA's structural properties (as rules and resources) and dimensions (signification, domination and legitimation). Illustration of CIMSPA's structural properties and dimensions was developed through adopting an institutional analysis strategy alongside a document analysis method. Publicly available strategic documents and web-based resources (N = 18) were selected and analysed using open-coding techniques. Our analysis shows how continued state-sanctioned decentralisation of the UK HE sector has led to CIMSPA becoming an increasingly influential transformative structure; attempting to use its structural dimensions and resources as a medium to gain and exercise power, to promote (signification) – market-orientated discourses, practices and pedagogies in HEI provision, (domination) – in an attempt to significantly influence regulatory control over UK SPAVET provision and (Legitimation) – create partnerships which normalise its position and message within selected UK HEIs. We conclude that whilst the transformative aspirations of CIMSPA offer a number of possibilities for the HE SPAVET sector, they also raise critical questions. The central issue requiring debate is how these aspirations shift dialectical control towards vocationally focused, market-orientated principles and away from the liberal educational, research-led principles of some current UK HE SPAVET provision. The foundation degree is an intermediate-level qualification designed to widen participation in higher education while providing work-related skills and knowledge. It has a vocational focus and a pedagogic approach that integrates academic and work-based learning. Research into opportunities and the process of learning within foundation degrees is limited, particularly that which looks in depth at personal development occurring within these courses. This study contributes to the literature in this area, reporting on the results of a case study of the foundation degree in medicines management (University of Portsmouth). A key finding of this work was that knowledge was gained within a community of practice, and led to students’ development as individuals. Motivation, learning to learn and guidance and feedback, alongside peer learning within the course, were central aspects of this process. The learning and personal development taking place resulted in increased awareness, confidence and empowerment in students, which was reported to influence their work and personal lives. However, the development of specialist work-related skills and knowledge was found to be limited, questioning the fitness for purpose of the course researched. One of the most persistent white responses to the ‘problem’ of slavery and race relations in the nineteenth-century United States was to suggest the removal and relocation of African Americans. The transatlantic abolitionist movement, however, decisively rejected colonisation in the early 1830s, and conventional wisdom suggests that they maintained their firm opposition from that point onwards. It is curious, then, that abolitionists in Britain and the United States enthusiastically received a text in 1857 – The Impending Crisis of the South – calling for colonisation as well as abolition. This article explains the reasons why they did so. It demonstrates that the Garrisonians recognised the book's potential for influencing the critical presidential election of 1860 and consequently sought to aid efforts to ensure wide circulation, belying their reputation for avoiding electoral politics. As mid Victorian newspapers spoke of their ever more important role as educators and representatives of the ‘people’, the rise of a free and independent press seemed central to notions of an age of ‘improvement’. However, for many politicians, the press remained simply a tool to be exploited in order to advance their political agendas. By examining the relationship between politicians and metropolitan journalism in the mid nineteenth century, this article contrasts the claims of a press growing in confidence with those of an increasingly media-literate political class and argues that the press was in practice far more the instrument of politicians than the rhetoric suggests. Research on the British Paleocene Igneous Province (BPIP) has historically focused on the emplacement, chemistry and chronology of its elaborate central intrusive complexes and lava fields. However, the BPIP has also been dramatically shaped by numerous erosion, sedimentation and volcano-tectonic events, the significance of which becomes ever clearer as localities in the BPIP are reinvestigated and our understanding of volcano-sedimentary processes advances. The resultant deposits provide important palaeo-environmental, palaeo-geographical and stratigraphical information, and highlight the wide range of processes and events that occur in ancient volcanic settings such as the BPIP. In this paper we review the sedimentary and volcano-tectonic processes that can be distinguished in the BPIP, and conceptualize them within a generalized framework model.We identify, and describe, the sedimentary responses to four broadly chronological stages in the history of the BPIP volcanoes: (1) the development of the lava fields, (2) early intrusion-induced uplift, (3) caldera collapse and (4) post-volcano denudation and exhumation of central complexes. We highlight and illustrate the range of sedimentary processes that were active in the BPIP. These operated on and helped shape a dynamic landscape of uplands and lowlands, of alluvial fans, braided rivers, lakes and swamps, and of volcanoes torn apart by catastrophic mass wasting events and/or caldera collapse. Conventionally, it was Palmerston's political nemesis, Lord Aberdeen (foreign secretary 1841–46), who brokered an entente cordiale with François Guizot and France in the early 1840s, which the belligerent and unequivocally “English” Palmerston then systematically destroyed when he returned to office in 1846. However, not only is this a distortion of the relationship between Britain and France prior to 1846, it also fails to appreciate the nuances of Palmerston's approach to Anglo–French politics. Naval scares and rhetorical constructions of English and British identities certainly drew Palmerston to adopt aggressive positions towards France on occasion, but against this must be set his close personal ties with leading French statesmen, not least Napoleon III himself, and his desire to maintain peaceful harmonious relations with France in order to free Britain's diplomatic hand elsewhere in the world. Although superficially swinging between extremes of amity and enmity, therefore, Anglo–French relations under Palmerston's guidance were in fact far more cordial and close than has sometimes been allowed. Purpose — The purpose of this article is to introduce the theme of this special issue which contains a selection of papers written by the staff of UCL SLAIS. Design/methodology/approach — The history of the School is briefly described and its philosophy of taking a broad and inclusive approach to professional education is reviewed. Findings — There is considerable benefit to professional education in an institution which incorporates all branches of information work, and retains a sense of the value of traditional skills in tandem with an exploitation of new technologies. The contributors to this issue reflect this both by the range of disciplines which they represent and by the range of topics, from the most traditional to the most modern, and demonstrate the synergy between them. Originality/value — The article provides an introduction to the special issue and promotes the value of an inclusive approach to professional education in the information disciplines. Three hundred nine mumps virus (MuV) strains detected in the United Kingdom during 1995-2002 were characterized by partial sequencing of the small hydrophobic gene and were shown to belong to at least 6 different genotypes. A strain representing a new genotype was isolated from a seminal fluid specimen. Identical strains belonging to genotypes G and C were found to circulate for up to 3 years. One genotype H strain reappeared after an absence of 4 years. Distinct lineages (G1-G7) within genotype G were recognizable in the present study, and this level of characterization proved to be very useful for tracking MuV importations and subsequent transmission. We propose, here, a preliminary standardization of international nomenclature for genetic characterization of MuV strains, to facilitate future molecular epidemiological studies of mumps. Oral fluid (OF) specimens (n = 1441) were used to detect both anti-MuV IgM and MuV genome, and the results indicate that OF specimens can be used successfully for diagnosis and have the potential to play a valuable role in diagnosis and surveillance of mumps. This systematic economic review examined the cost–benefit and cost-effectiveness of park, trail, and greenway infrastructure interventions to increase physical activity or infrastructure use. The search period covered the date of inception of publications databases through February 2022. Inclusion was limited to studies that reported cost–benefit or cost-effectiveness outcomes and were based in the U.S. and other high-income countries. Analyses were conducted from March 2022 through December 2022. All monetary values reported are in 2021 U.S. dollars. The search yielded 1 study based in the U.S. and 7 based in other high-income countries, with 1 reporting cost-effectiveness and 7 reporting cost–benefit outcomes. The cost-effectiveness study based in the United Kingdom reported $23,254 per disability-adjusted life year averted. The median benefit-to-cost ratio was 3.1 (interquartile interval=2.9–3.9) on the basis of 7 studies. The evidence shows that economic benefits exceed the intervention cost of park, trail, and greenway infrastructure. Given large differences in the size of infrastructure, intervention costs and economic benefits varied substantially across studies. Background: Seizures can occur unpredictably in patients with acute encephalitis syndrome (AES), and many suffer from poor long-term neurological sequelae. Establishing factors associated with acute seizures risk and poor outcomes could support clinical care. We aimed to conduct regional and volumetric analysis of cerebral oedema on magnetic resonance imaging (MRI) in patients with AES. We assessed the relationship of brain oedema with acute seizure activity and long-term neurological outcome. Methods: In a multi-centre cohort study, adults and children presenting with an AES were recruited in the UK. The clinical and brain MRI data were retrospectively reviewed. The outcomes variables were inpatient acute seizure activity and neurological disability at six-months post-discharge. A poor outcome was defined as a Glasgow outcome score (GOS) of 1–3. We quantified regional brain oedema on MRI through stereological examination of T2-weighted images using established methodology by independent and blinded assessors. Clinical and neuroimaging variables were analysed by multivariate logistic regression to assess for correlation with acute seizure activity and outcome. Results: The study cohort comprised 69 patients (mean age 31.8 years; 53.6% female), of whom 41 (59.4%) had acute seizures as inpatients. A higher Glasgow coma scale (GCS) score on admission was a negative predictor of seizures (OR 0.61 [0.46–0.83], p = 0.001). Even correcting for GCS on admission, the presence of cortical oedema was a significant risk factor for acute seizure activity (OR 5.48 [1.62–18.51], p = 0.006) and greater volume of cerebral oedema in these cortical structures increased the risk of acute seizures (OR 1.90 [1.12–3.21], p = 0.017). At six-month post-discharge, 21 (30.4%) had a poor neurological outcome. Herpes simplex virus encephalitis was associated with higher risk of poor outcomes in univariate analysis (OR 3.92 [1.08–14.20], p = 0.038). When controlling for aetiology, increased volume of cerebral oedema was an independent risk factor for adverse neurological outcome at 6 months (OR 1.73 [1.06–2.83], p = 0.027). Conclusions: Both the presence and degree of cerebral oedema on MRIs of patients with AES may help identify patients at risk of acute seizure activity and subsequent long-term morbidity. This paper offers insights into the increasing dichotomy that exists between official forms of opportunity and access and the actual ‘lived experience’ of young peoples' trajectories towards careers in the UK's market-orientated Sport-Fitness and Physical Education employment sectors. It does so by drawing on data generated by an 18-month ethnographic study to provide a case study of two students who chose to make the transition from a Foundation Degree in Sport Coaching (FdSc) onto a ‘Top Up’ Bachelor of Science (BSc) Sports Science and Coaching qualification. The paper illustrates their experiences of this transition in relation to the following phases of their trajectory (1)facilitators of transition(2)managing expectations(3)transformationand (4)isolation. The findings highlight how, for some individuals, current transitions facilitate a critical distance between individual dispositions towards education and the positions and practices of higher education. We suggest that education and employers must further listen to the voices and experiences of students that transcend the rhetorics of official policy discourse to facilitate a process in which the conditions of transition may begin to be reimagined. This paper explores the layered transitional experiences of a semi-professional athlete named Jack (a pseudonym) between the fields of professional sport and further and higher education. Our analysis is framed by the quadripartite framework of structuration and focuses on Jack’s ‘in-situ’ practices at his college and university in order to illustrate how these can operate to reproduce, transform, and challenge the habitual discourses and rituals that circulate within these institutions by generating forms of corporeal empowerment for young athletes who have valued conjunctural knowledge. The findings highlight the fragility of the transition process and raise questions regarding how the experiences of young people are shaped by the relationships between employment and post-16 education. Background: The oral infant rotavirus vaccine, Rotarix, was introduced in England and Wales in July 2013. We estimated the impact on laboratory-confirmed rotavirus infections and hospitalizations for all-cause acute gastroenteritis (AGE) during the first year after introduction.Methods: We extracted data on laboratory-confirmed rotavirus infections (July 2000 through June 2015) and all-cause AGE-associated hospitalizations (July 2007 through June 2014) for all age groups using national databases (LabBase2 and HES). We determined the ratio of the rate during the 2013-2014 rotavirus season to the rate during the prevaccination era.Results: In infants, there was a 77% decline (rate ratio [RR], 0.23; 95% confidence interval [CI], .16-.32) in laboratory-confirmed rotavirus infections and a 26% decline (RR, 0.74; 95% CI, .65-.84) in all-cause AGE-associated hospitalizations in 2013-2014, compared with the prevaccination era. Large reductions were also observed in older children, adults, and older adults. We estimated that 10 884 laboratory-confirmed infections and 50 427 all-cause AGE-associated hospital admissions were averted in 2013-2014. Similar reductions have been observed for laboratory-confirmed rotavirus infections during the 2014-2015 season.Conclusions: The rapid declines in rotavirus infection and AGE in vaccinated and unvaccinated age groups within 1 year of introducing an infant rotavirus vaccination program are far greater than expected and than previously reported by other countries. BACKGROUND: A low-cost virtual reality system that translates movements of the hand, fingers, and thumb into game play was designed to provide a flexible and motivating approach to increasing adherence to home-based rehabilitation. OBJECTIVE: Effectiveness depends on adherence, so did patients use the intervention to the recommended level? If not, what reasons did they give? The purpose of this study was to investigate these and related questions. DESIGN: A prospective cohort study, plus qualitative analysis of interviews, was conducted. METHODS: Seventeen patients recovering from stroke recruited to the intervention arm of a feasibility trial had the equipment left in their homes for 8 weeks and were advised to use it 3 times a day for periods of no more than 20 minutes. Frequency and duration of use were automatically recorded. At the end of the intervention, participants were interviewed to determine barriers to using it in the recommended way. RESULTS: Duration of use and how many days they used the equipment are presented for the 13 participants who successfully started the intervention. These figures were highly variable and could fall far short of our recommendations. There was a weak positive correlation between duration and baseline reported activities of daily living. Participants reported familiarity with technology and competing commitments as barriers to use, although they appreciated the flexibility of the intervention and found it motivating. LIMITATIONS: The small sample size limits the conclusions that can be drawn. CONCLUSIONS: Level of use is variable and can fall far short of recommendations. Competing commitments were a barrier to use of the equipment, but participants reported that the intervention was flexible and motivating. The United Kingdom and United States have recently experienced large outbreaks of mumps, which raises concerns about vaccine effectiveness. The effectiveness of the mumps component of the measles, mumps, rubella (MMR) vaccine was estimated using the screening method. In England from January 2004 through March 2005, 312 cases of mumps were reported in children eligible to have received 2 doses of MMR vaccine. Of these children, 52 (16.7%) had received 1 dose of MMR vaccine, and 97 (31.1%) had received 2 doses. Vaccine effectiveness was 88% (95% confidence interval [CI] 83%-91%) for 1 dose and 95% (95% CI 93%-96%) for 2 doses. The effectiveness of 1 dose declined from 96% (95% CI 81%-99%) in 2-year-olds to 66% (95% CI 30%-83%) in 11- to 12-year-olds, and the effectiveness of 2 doses declined from 99% (95% CI 97%-99.5%) in 5- to 6-year-olds to 86% (95% CI 74%-93%) in 11- to 12-year-olds (p<0.001 for 1 or 2 doses). Background: Noroviruses are the most common cause of gastroenteritis outbreaks in industrialised countries. Gastroenteritis caused by Norovirus infection has been described as a highly seasonal syndrome, often referred to as "winter vomiting disease". Methods: The Public Health Laboratory Service Communicable Disease Surveillance Centre has systematically collected reports of laboratory confirmed cases of Norovirus-gastroenteritis since 1995. We analysed these data for annual and seasonal trends and age distribution. Results: A mid-summer peak in reported cases of Norovirus was observed in 2002, unlike all six previous years when there was a marked summer decline. Total reports from 2002 have also been higher than all previous years. From the first 10 months of 2002, a total of 3029 Norovirus diagnoses were reported compared the previous peak in 1996 of 2437 diagnoses for the whole 12-month period. The increase in 2002 was most marked in the 65 and older age group. Conclusion: This surveillance data challenges the view that Noroviruses infections exclusively have wintertime seasonality.
