Context: There is little published information on how long it takes for the performance of phosphorus (P)-deficient mature cows to improve once they start receiving P supplements, or whether their long-term performance is impaired by P deficiency early in life. Aims: The performance of mature P-deficient and P-adequate Brahman cows was compared over 3 years to investigate the time it takes for the performance of P-deficient mature cows to improve once they are supplemented with P, and whether their long-term performance is compromised by P deficiency early in life. Methods: Cows that had been a part of a P-supplementation trial at Victoria River Research Station from weaning until they were 5.5 years old were used for this study. One treatment group had received continuous P supplement (P+) since weaning and the other treatment had never received any P supplement (P−). At the conclusion of the original trial, 42 cows were randomly selected from each treatment and then grazed together in the same paddock for 3 years, during which time they were given P supplement continuously. Performance attributes were measured and compared. Key results: This study found that P supplementation rapidly improved the liveweight, body condition, reproductive performance and survival rate of the P-deficient (P−) mature cows. Within a year of receiving P supplement, their performance improved to be similar to that of P+ cows. However, P deficiency early in life did have some residual stunting effects, resulting in lower mean hip height and liveweights of P− cows. Conclusions: The performance of P-deficient mature cows improved rapidly once they were given P supplements and matched that of cows that had always been supplemented. However, P deficiency early in life caused stunting, resulting in mature cows being shorter and lighter. Implications: Cattle producers in P-deficient country should see a rapid improvement in performance and return on investment once they start supplementing P-deficient cows. However, it is better to supplement cattle throughout their lives in P-deficient areas to avoid stunting. This aim of this paper is to set the scene for the need for impact assessment and return on investment in funded cardiovascular research in Australia, starting with the historical perspective on waste in health and medical research. Recently there has been a substantial move from discussion and policy about the need for research translation, into practice and application via the evolution of funding streams like the Australian Medical Research Future Fund (MRFF). Health and medical research funders play a critical role in both setting the expectations for research translation and impact and helping researchers to meet these expectations. As a leading cause of death, cardiovascular disease is a national health priority, recognised as such with a AUD$220 million MRFF allocation to the Cardiovascular Health Mission. Focussing on cardiovascular research, we address some of the barriers researchers face in prospectively planning for research translation and impact assessment, and call for an ecosystem that supports a return on investment for all stakeholders, especially the community and patient end-users. Northern Australian pastoral land prices have become higher and more volatile over the last twenty years, raising concerns about the potential implications of the financialization of the industry. These prices are not inevitable results of market forces, but mediated and co-constructed by a range of actors using two valuation devices: the 'Beast Area Value', a 'rule of thumb' which emerged during the early development of the industry, and the 'Return on Investment', a tool widely used to compare financial ventures. The Beast Area Value treats land as a commodity whose value is derived from its physical characteristics, while the Return on Investment treats land as an asset whose value is based on its future income generation potential. This article describes how some pastoral companies are strategically combining these devices to earn capital gains through 'speculative development' of properties in ways that do not necessarily increase their productivity. It argues that pastoral land is often developed in ways more reflective of the valuation devices used in the region than of the realities of station management, representing a shift from competing in the sphere of production to competing in the sphere of valuation and implicating these devices in the financialization of Northern Australian land. Residential battery energy storage system (BESS) adoption is hindered with its expensive price in current market. Optimally sized BESS can excel the fiscal benefits and thus can be economically sensible. An optimization problem, which targets to minimize the total annual cost including both energy and battery degradation-based costs, is formulated to investigate the added benefits of operational optimization of a BESS over the baseline operation of BESS based on self-consumption maximization (SCM). A case study is conducted considering a Victorian electricity customer in Australia. Results revealed that installation of residential BESSs are fiscally beneficial at current market prices only if adequate rebates are available. A fiscally beneficial upper limit of BESS capacity that can be installed with the prevailing government incentives depending on the PV system capacity is also found. For a given installed cost of BESS (AU$/kWh), optimal BESS capacity is larger when operational optimization of BESS is considered. However, with a time-of-use (TOU) tariff scheme, BESSs based on operational optimization outperforms BESSs based on SCM in terms of the minimum total annual cost, return on investment and PV curtailment avoidance which excels the fiscal benefits of a BESS and results in indirect greenhouse gas emission reduction. • Operational optimization of energy storage increases the return on investment. • It also reduces PV curtailment and greenhouse gas emission. • Results are compared with an energy storage based on self-consumption maximization. • A Victorian electricity customer in Australia is taken as the case study. • Battery energy storages are economically sensible with prevailing government rebates. Study Question: What are the success rates for women returning to ART treatment in the hope of having a second ART-conceived child.Summary Answer: The cumulative live birth rate (LBR) for women returning to ART treatment was between 50.5% and 88.1% after six cycles depending on whether women commenced with a previously frozen embryo or a new ovarian stimulation cycle and the assumptions made regarding the success rates for women who dropped-out of treatment.What Is Known Already: Previous studies have reported the cumulative LBR for the first ART-conceived child to inform patients about their chances of success. However, most couples plan to have more than one child to complete their family and, for that reason, patients commonly return to ART treatment after the birth of their first ART-conceived child. To our knowledge, there are no published data to facilitate patient counseling and clinical decision-making regarding the success rates for these patients.Study Design, Size, Duration: A population-based cohort study with 35 290 women who commenced autologous (using their own oocytes) ART treatment between January 2009 and December 2013 and achieved their first treatment-dependent live birth from treatment performed during this period. These women were then followed up for a further 2 years of treatment to December 2015, providing a minimum of 2 years and a maximum of 7 years of treatment follow-up.Participants/materials, Setting, Methods: Cycle-specific LBR and cumulative LBR were calculated for up to six complete ART cycles (one ovarian stimulation and all associated transfers). Three cumulative LBR were calculated based on the likelihood of success in women who dropped-out of treatment (conservative, optimal and inverse probability-weighted (IPW)). A multivariable logistic regression model was used to predict the chance of returning to ART treatment for a second ART-conceived child, and a discrete time logistic regression model was used to predict the chance of achieving a second ART-conceived child up to a maximum of six complete cycles. The models were adjusted for patient characteristics and previous and current treatment characteristics.Main Results and the Role Of Chance: Among the women who had their first ART-conceived live birth, 15 325 (43%) returned to treatment by December 2015. LBRs were consistently better in women who recommenced treatment with a previously frozen embryo, compared to women who underwent a new ovarian stimulation cycle. After six complete cycles, plus any surplus frozen embryos, the cumulative LBR was between 60.9% (95% CI: 60.0-61.8%) (conservative) and 88.1% (95% CI: 86.7-89.5%) (optimal) [IPW 87.2% (95% CI: 86.2-88.2%)] for women who recommenced treatment with a frozen embryo, compared to between 50.5% (95% CI: 49.0-52.0%) and 69.8% (95% CI: 67.5-72.2%) [IPW 68.1% (95% CI: 67.3-68.9%)] for those who underwent a new ovarian stimulation cycle. The adjusted odds of a second ART-conceived live birth decreased for women ≥35 years, who waited at least 3 years before returning to treatment, or who required a higher number of ovarian stimulation cycles or double embryo transfer to achieve their first child.Limitations, Reasons For Caution: Our estimates do not fully account for a number of individual prognostic factors, including duration of infertility, BMI and ovarian reserve.Wider Implications Of the Findings: This is the first study to report success rates for women returning to ART treatment to have second ART-conceived child. Reducing uncertainty surrounding the biological responses of degraded habitat is key to providing confidence and efficiency in its restoration. Many coastal habitats are so extensively degraded that organismal responses to restoration interventions are entirely unknown. Among the most degraded coastal ecosystems are oyster reefs, whose restoration typically occurs where oysters are degraded to the point of functional extinction. This loss creates uncertainty on the fundamental processes for reef recovery; the timing of oyster recruitment and their preferred substratum for settlement. Such knowledge can inform restoration strategies to accelerate habitat recovery. Near the site of Australia's largest restoration of native oyster reef, we quantified temporal variability in recruitment of the native flat oyster (Ostrea angasi) and assessed their preference between the settlement substrata deployed for the reef restoration. Combining half a decade of environmental data with oyster recruitment data, we provide a model that identified distinct peaks in oyster recruitment which correlate with food availability and seawater temperature. In addition, oysters preferentially settled on oyster shell relative to other materials used in local restoration. In combination, these results suggest that there are opportunities to augment recruitment through addition of shell substratum synced with recruitment peaks. Our recruitment model likely represents a minor investment with large returns, providing opportunities to capture peak recruitment and greater confidence in utilizing natural recruitment as a restoration resource. This paper analyzes the interplay of transmission and storage investments in a multistage game that we translate into a bilevel market model. In particular, on the first level we assume that a transmission system operator chooses optimal line investments and a corresponding optimal network fee. On the second level we model competitive firms that trade energy on a zonal market with limited transmission capacities and decide on their optimal storage facility investments. To the best of our knowledge, we are the first to analyze interdependent transmission and storage facility investments in a zonal market environment that accounts for the described hierarchical decision structure. As a first best benchmark, we also present an integrated, single-level problem that may be interpreted as a long-run nodal pricing model. Our numerical results show that adequate storage facility investments of firms may in general have the potential to reduce the amount of line investments of the transmission system operator. However, our bilevel zonal pricing model may yield inefficient investments in storages, which may be accompanied by suboptimal network facility extensions as compared to the nodal pricing benchmark. In this context, the chosen zonal configuration of the network will highly influence the equilibrium investment outcomes including the size and location of the newly invested facilities. As zonal pricing is used for instance in Australia or Europe, our models may be seen as valuable tools for evaluating different regulatory policy options in the context of long-run investments in storage and network facilities. Since 2006 there have been significant changes in the labour market for university graduates, most notably the Global Financial Crisis of 2007 and the subsequent downturn in gross domestic product growth, and a huge expansion in university places. This article presents estimates of the monetary returns in Australia associated with the completion of bachelor degrees in 2016 for a range of fields of study and compares them to the estimates for 2006. The results show that for the median person, there are still good monetary incentives to complete these degrees and the private rate of return compares favourably with the long-term real bond rate. However, the article also concludes that the return to an individual investing in bachelor education had mostly declined in the period 2006–2016. There is, again, considerable variation between disciplines and for some, completion of a bachelor degree does not yield positive returns. The key factors determining the monetary rate of return are the discipline of the degree and the graduates' subsequent labour force status. Graduates in full-time employment in most disciplines avoided being among those with low (or negative) rates of return. Background: The ways children spend their time is one of the most valuable inputs for healthy child development. It is unknown which time investment yields the greatest return for children's language and behavioural outcomes at school entry.Methods: We used data from the first three waves (2004, 2006, 2008) of the Longitudinal Study of Australian Children (n = 4253). At every wave, parents completed 24-h time-use diaries on one randomly selected week and one weekend day. The amount of time children spent on 11 activities at ages 0-1, 2-3 and 4-5 years was analysed. Receptive vocabulary was assessed using the Peabody Picture Vocabulary Test, and externalizing behaviours were measured by the Strengths and Difficulties Questionnaire, completed by parents and teachers at 4-5 years. To identify which time investment in the first 5 years of life mattered most for children's outcomes, a new production function was developed. This production function was estimated using a log-log linear regression model.Results: Relative to other time investments, time spent on educational activities at 2-3 years of age was the most important time investment for receptive vocabulary and behavioural outcomes at school entry. After adjusting for confounding, every 1 h invested in educational activities at 2-3 years was associated with a 0.95% [95% CI (confidence interval): 0.62, 1.28] increase in receptive vocabulary, and a -5.72% (95% CI: -7.71, -3.73) and -9.23% (95% CI: -12.26, -6.20) reduction in parent- and teacher-reported externalizing problem behaviours. Time invested in play was also important to both receptive vocabulary and behaviour. One hour invested in play at 2-3 and 4-5 years was associated with a 0.68% (95% CI: 0.38, 0.98) and 0.71% (95% CI: 0.39, 1.03) increase in children's receptive vocabulary at school entry. In addition, time invested in play at 2-3 and 4-5 years was associated with reduced problem behaviours at school entry. In contrast, screen time at all ages was associated with poorer parent- and teacher-reported externalizing problem behaviours.Conclusions: These results suggest that time invested in educational activities at 2-3 years of age yield the greatest return for children's receptive vocabulary and behaviour at school entry. Off-grid groundwater cotton irrigators using traditional technologies have had few options to reduce water extraction costs. New options of blending of Alternating Current and Direct Current through an all-in-one soft start converter/controller offer opportunities to lower costs, diversify fuel sources and continuous off-grid power in remote locations. Diesel energy can be replaced by renewable fuel sources through installing an electric submersible pump powered by solar photovoltaic in combination with a diesel generator. An investment analysis using a discounted cashflow over 25 years showed a 23% internal rate of return and payback over 5 years. Benefits of installing a renewable fuel source included lower greenhouse gas emissions per bale of cotton produced and less labour to monitor and service traditional diesel irrigation plant. Emissions calculations reveal a reduction of 26%, or 35 kg CO 2 e per cotton lint bale, and an abatement of 2665 t/CO 2 e for the life of the project. The cost of emissions abatement under the Renewable Energy Target to displace almost one million litres of diesel was found to be $31 t CO 2 e. • A microgrid solution reducing energy costs and emissions is analysed for an irrigation pump in NSW, Australia. • The microgrid includes photovoltaic panels and a diesel generator used to power a submersible pump 24 h a day. • The analysis indicates a 5-year payback and 23 per cent internal rate of return for the investment. • Emissions are reduced by 26 per cent (per 227 kg cotton lint bale) equating to 2665 t/CO 2 e abatement over the project life. This research reports upon the scale of building information modeling (BIM) implementation maturity, from nonadoption to full-scale deployment, within small-to-medium enterprises (SMEs) operating within the Australian construction industry. The research utilizes a competitive dynamics perspective (CDP) as the theoretical lens and analyses data collected from 135 SMEs using Bayesian belief networks (BBNs) to provide a richer insight into levels of BIM implementation maturity. Findings reveal that there is no meaningful association between BIM implementation maturity within SMEs and their organizational attributes, such as size and level of experience. A paucity of solid evidence to support a reasonable return on investment (ROI) was also identified as a key barrier to using BIM in higher levels of maturity. In practical terms, the study focuses on pertinent issues associated with mandated BIM in Australia from SMEs' perspectives, pointing out potential consequences and challenging the pressure for mandating. The research concludes by outlining industrial implications and providing pragmatic recommendations designed to accelerate the pace that Australian SMEs move across a BIM trajectory from nonadoption to higher levels of maturity. Energy audits can provide businesses with valuable information and advice about their current energy use and costs and where changes can be made to improve both the environmental performance of the business and their triple bottom line. There is literature on the use of energy audits outside Australia, but little provision of empirical data that would help a business decide whether engagement is economically viable. This paper reports on 49 SME energy audits in Australia, and the results verify that economic returns can vary significantly and alone are not a reason for most SMEs to act. This article examines domestic marketing in the Australian higher education sector, specifically, the marketing investment patterns of universities and their levels of student growth as a return on marketing investment. Marketing expenditure by universities has risen 23 per cent in the five years to 2013, with several institutions allocating in excess of AU$10 million per annum to advertising and promotions. However, few universities convert strong marketing investment into strong revenue streams or student growth returns. With government funding cuts in prospect for Australian universities and proposals to increase their exposure to market forces, it is inevitable that marketing budgets – and the outcomes of expenditure on marketing – will come under greater scrutiny in the future. This article presents estimates of the private monetary benefits in Australia associated with the completion of Bachelor degrees for a range of fields of study under a range of different assumptions. For the average person, results show strong monetary incentives to complete these degrees and the private rate of return compares favourably with the real long-term bond rate. However, differences can be observed in rates of return according to gender and discipline of study with, generally, lower returns for women and for those holding degrees in the humanities. The results are calculated on varying assumptions which provide evidence of the robustness of the conclusions. Aims: METHODS: RESULTS: We identified nine articles (conducted in Australia (n = 5), Europe (n = 3) and China (n = 1)); three reported healthcare costs associated with excessive sedentary time, whilst six were economic evaluations of interventions targeting sedentary behaviour. Healthcare costs associated with excessive sedentary time as reported in cost of illness studies were substantial; however, none explored non-health sector costs. In contrast, all full economics evaluations adopted a societal perspective; however, costs included differed depending on the intervention context. One sedentary behaviour intervention in children was cost-saving. The five interventions targeting occupational sitting time of adults in office workplaces were cost-effective. Physical environmental changes such as sit-stand desks, active workstations etc., were the key cost driver.Conclusions: Sedentary behaviour is likely associated with excess healthcare costs, although future research should also explore costs across other sectors. Cost-effectiveness evidence of sedentary behaviour reduction interventions in workplaces is limited but consistent. This paper describes a customised, six-month, leadership development program (LDP) that was designed for emerging leaders in the Australian water industry who were promoting sustainable urban water management (SUWM). It also presents results from an evaluation of the program's benefits, costs and overall 'return on investment' (ROI). The program was designed to help build emergent leadership capacity in the water industry, given strong evidence that this form of leadership plays an important role in advancing SUWM. It involved '360-degree feedback' processes, training, individual leadership development plans, and coaching sessions. Its design was informed by a review of the literature, and its content was informed by local empirical research involving effective SUWM leaders. The evaluation used a seven-tier assessment framework that examined different dimensions of the program's performance using source and methodological triangulation. The results indicate that such LDPs can produce a range of positive outcomes, such as promoting desired leadership behaviours and generating a positive ROI estimate. Specifically, the program's estimated ROI was approximately 190% after only one year. The primary conclusion is that evidence-based LDPs which are highly customised for specific types of leaders in the water industry represent a promising type of intervention to build forms of leadership capacity which are needed to successfully promote SUWM. The article focuses on the feasibility of teaching senior school students in Australia the methods of calculating the rate of return on investments. Since students know the concept of a compound interest rate, the computations for the internal rate of return (IRR) and the continuously rate of return may be taught with efficiency. The computations for equations reducible to quadratics and integration involving exponential functions are explained. A shortage of suitable franchisee applicants has been identified by franchisors as a major hindrance to franchise sector growth in Australia. However, there has been little investigation into this issue within organizational choice research. This represents an important gap in the literature. To explore the issue of franchisee shortage it was first necessary to determine what prospective franchisees are seeking from franchisors. Prospective franchisees considered a return on investment, provision of training, and being their own boss as being the most important attributes that franchisors could provide. These findings also indicate that attendees saw trade shows as an important means of gathering franchising information. Further, respondent's attributes generally aligned with what franchisors believed they supplied, and though possessing entrepreneurial tendencies, they desired ongoing support from franchisors. The Grain & Graze program attempted to improve the 'triple bottom line' of mixed-farming systems in Australia through a major program of research, development and extension (RD&E) that operated across nine regions, with a total budget of A$31 million provided by four R&D corporations and over 60 regional partners. Regional activities were complemented by a series of national projects and management and governance arrangements were organised at both regional and national levels with significant producer input. While the two-tiered management structure resulted in both tensions and opportunities the outputs of the program were substantial including 278 demonstration and trial sites, 180 training courses, over 200 publications, tools and manuals, over 100 research papers and a database of national and regional data. Over 8000 producers participated in program events, over 5000 actively trialled new activities and around 3200 adopted recommended practices despite severely unfavourable seasonal conditions over the 5 years of the program. A return on investment of 3.4 : 1 for the core funders was comparable to some other agricultural RD&E programs if lower than others. The program expanded the scope of farming systems RD&E in Australia through explicit recognition of the triple bottom line and development of formal and informal approaches to integration across these dimensions. It established regional and inter-regional networks of producers and scientists that can be expected to have ongoing significance. Abstract: Australian immigration policy, in common with the US and Canada, has increased the emphasis on skill-based selection criteria. A key premise of this policy is that skilled immigrants are more employable and can add to the productive capacity of the economy. However, this effect will be diminished if immigrants are working in occupations that fail to utilise their skills. We examine the extent of overeducation for recently arrived immigrants to Australia. We find that they are more likely to be overeducated than the native population, even if they enter on skill assessed visas. Overeducation is greater for immigrants from non-English speaking backgrounds (NESB) and generates lower returns to education. Tighter restrictions to welfare support on entry raised employment levels but increased overeducation. This will serve to reduce the potential productivity gains from skill biased immigration policies. [Copyright &y& Elsevier]

Purpose - To investigate the extent of over-education for recently arrived tertiary educated male immigrants in order to ascertain if higher educated immigrants face assimilation hurdles in the Australian labour market. Design/methodology/approach - Using immigrant longitudinal data (LSIA), this paper uses the job analysis/objective method of defining over-education. Also, bivariate probits are used to account for selectivity into employment when studying the determinants of graduate over-education. The over, required and under-education (ORU) earnings function is utilised to find the rates of return to education investment. Findings - It is found that English speaking background (ESB) immigrants to have similar rates of over-education compared to the native born, while only Asian non-English speaking background (NESB) immigrants see a rise in over-education after tighter immigration and welfare policies were introduced. Returns to required schooling are substantial, but the penalty for excess years of schooling is large, though consistent with the stylised facts of over-education. Research limitations/implications - Short time-frame (up to five years) only allows for an investigation of initial assimilation. Originality/value - Using panel data, this paper is the first to study the initial phase of highly educated immigrant assimilation into the Australian labour market from the viewpoint of job matching rather than just employment. Abstract: This study, based on 3 years of commercial data, presents the results of an economic analysis of a 20-tonne per annum (TPA) commercial recirculating aquaculture system (RAS) facility located in Warrnambool, Victoria, Australia. Based on the assumptions of the analysis, results highlight the non-viability of the facility, with a 10-year projected negative cumulative cash flow of −$648,038, and negative net present value (NPV) of −$707,546. Economies of scale were assessed by the development of economic models for hypothetical 50-TPA and 100-TPA facilities, based on the actual figures obtained from the 20-TPA case study. These analyses highlighted marginal viability for the 50-TPA facility (with a ten-year projected cumulative cash flow of $1,030,300; negative NPV of −$167,651 and internal rate of return (IRR) of 11.75%), and an economically viable 100-TPA facility (with a ten-year projected cumulative cash flow of $3,176,750; NPV of $522,200 and IRR of 21.03%). Sensitivity analysis highlighted that the greatest gains to be realised in improving profitability were those associated with increasing the productive capacity of the facility, increasing the sale price of the product, and decreasing the capital costs of RAS facilities. Contradictions between the results from the present study to similar studies clearly highlight a need for further economic analyses of commercial RAS facilities, using commercial data sets and standard economic analysis procedures. At least 25% of lambs born in Australia die before marking. Research across southern Australia has demonstrated that reducing mob size at lambing by 100 ewes will, on average, increase the survival of single-born lambs by 0.8% and twin-born lambs by 2.2%. In this paper we assessed the economic value of lambing ewes in smaller mobs to improve lamb survival. The analysis considered scenarios for Merino and non-Merino flocks where producers subdivided lambing paddocks using temporary or permanent fencing, or where ewes were reallocated within existing paddocks based on ewe pregnancy status. Optimum mob sizes were most sensitive to the type of fencing used for subdivision, whether ewes were single- or twin-bearing, whether the effect of paddock size on potential stocking rate was included and the target return on investment. Breed, ewe stocking rate, scanning percentage and lamb price also impacted optimum mob size but had a smaller effect. The optimum mob size for twin-bearing Merino and non-Merino ewes was between 34% and 60% that of single-bearing ewes and this was similar for scenarios where paddocks were subdivided or ewes were reallocated within existing paddocks. Permanent subdivision of paddocks also presents an opportunity to increase stocking rate by capitalising on the benefits of improved pasture utilisation in smaller paddocks. The relationship between paddock size and pasture utilisation is not well quantified but it has an important effect on optimum mob and paddock size. The value of reallocating ewes within existing paddocks was greatest at a scanning percentage of 150% as this presents the greatest opportunity to alter mob sizes for single- and twin-bearing ewes. The economic return from adjusting the relative mob size of twin-bearing ewes from 100% to 50% that of single-bearing ewes was up to $0.27/Merino ewe/yr and $0.44/non-Merino ewe/yr with lamb at $6/kg carcass weight. Overall, these analyses demonstrate that reducing mob size at lambing can be a profitable strategy for improving lamb survival depending on the current size of lambing mobs. However, it is difficult to provide generic recommendations to producers for the optimum mob size of ewes and value of paddock subdivision because these are dependent on enterprise-specific factors. • The optimum mob size for ewes and value of paddock subdivision are enterprise-specific. • Optimum mob size was smaller for twin-bearing ewes compared with single-bearing ewes. • Optimum mob size was smaller where the costs of paddock subdivision were reduced. • The return on investment was greater when splitting up larger mobs. Introduction: Achieving the virtual elimination of HIV requires equitable access to HIV prevention tools for all priority populations. Restricted access to healthcare means migrants face particular barriers to HIV prevention services. In February 2016, a peer‐led rapid HIV testing service for gay, bisexual and other men who have sex with men (gay and bisexual men, GBM) in Melbourne, Australia, introduced free sexually transmissible infection (STI) testing funded through Medicare (Australia's universal healthcare system). Medicare ineligible migrant clients were required to pay up to $158AUD for STI tests. We determined the uptake of STI testing and assessed the impact on repeat HIV testing among Medicare eligible and ineligible clients. Methods: All HIV tests conducted between August 2014 and March 2018 were included. We describe client characteristics, STI testing uptake and HIV/STI positivity among Medicare eligible and ineligible clients. Repeat HIV testing, assessed as the percentage of HIV tests with a return test within six months, was compared pre‐integration (August 2014‐June 2016) and post‐integration(July 2016‐March 2018) of STI testing using segmented linear regression of monthly aggregate data for Medicare eligible and ineligible clients. Results: Analyses included 9134 HIV tests among 4753 individuals. Medicare ineligible clients were younger (p < 0.01), and fewer reported previously testing for HIV (p < 0.01) and high HIV risk sexual behaviours. There was no difference in HIV positivity between the two groups (p = 0.09). STI testing uptake was significantly lower among Medicare ineligible clients (7.6%, 85.3%; p < 0.01). Following STI testing introduction there was an immediate increase in six‐month return HIV testing (6.4%; p = 0.02) and a significantly increasing rate of return HIV testing between July 2016 and March 2018 (0.5% per month; p < 0.01) among Medicare eligible clients but no immediate change in return testing (−0.9%; p = 0.7) or the rate of change in return testing between July 2016 and March 2018 (0.1% per month; p = 0.3) among Medicare ineligible clients. In March 2018, six‐month return HIV testing was 52.3% and 13.2% among Medicare eligible and ineligible clients respectively. Discussion: Improvements in return HIV testing observed among Medicare eligible clients did not extend to Medicare ineligible clients highlighting the impact of inequitable access to comprehensive sexual healthcare on test‐and‐treat approaches to HIV prevention.
