Aim: We systematically reviewed the literature on the influence of low and moderate amounts of prenatal alcohol and nicotine exposure on early child development. This paper also suggests possible directions for future research in order to tackle the controversial findings identified. Methods: The PubMed and Web of Science electronic databases were searched together with the reference lists of the selected papers. Empirical studies were included if they focused on the effects of low or moderate exposure, reported outcomes on child development within the first 2 years of life and were published in English between January 2009 and December 2019. The eligibility of the included studies was based on three authors reading the full text. Results: The final sample comprised 17 papers. Of these, 13 focused on the effects of prenatal alcohol exposure and they reported decreased sensory sensibility, smaller body sizes and increased cognitive capacities. The other four looked at prenatal nicotine exposure, and they primarily found impairments in children's orienting, communication and motor skills. Conclusion: Any amount of prenatal alcohol and nicotine exposure appeared to risk healthy child development. Children from families with low income are at an increased risk of developmental delays. While early interventions are effective in supporting the development of these children, families with low income often face barriers to accessing suitable interventions. The aim of this paper was to apply a participatory codevelopment process to produce a tool adapted for families with low income that informs parents about early child development and strengthens their capacity to create a stimulating learning environment at home. The objectives were to identify 1) the prioritized needs for parental information (tool content) and 2) the most appropriate format to reach our target population (tool format). Parents, pediatric healthcare professionals, community workers, managers and scientific advisers participated in a two-phase consensus-seeking process (Technique of Research of Information by Animation of a Group of Experts (TRIAGE)) to reach an agreement regarding the tool content and format. The participants unanimously decided to present information about red flags, referral sources, support resources and autonomy development strategies in an original, appealing, and accessible format of a life-size growth chart. The developed tool, named Roots to Bloom, can be used in primary healthcare and community settings to support interventions aimed at preventing developmental delays. This study reinforced the effectiveness of the TRIAGE method in enabling a group of stakeholders to reach a consensus. Highlights: Optimally, tools to support child development should be developed to be accessible to all families. The TRIAGE method is effective in enabling a heterogeneous group of stakeholders to reach a consensus. Parents may benefit from strategies to foster the development of their children's autonomy as part of their daily routines. Background: Food insecurity, poverty and exposure to infectious disease are well‐established drivers of malnutrition in children in Sub‐Saharan Africa. Early development of cognitive and motor skills – the foundations for learning – may also be compromised by the same or additional factors that restrict physical growth. However, little is known about factors associated with early child development in this region, which limits the scope to intervene effectively. To address this knowledge gap, we compared studies that have examined factors associated with early cognitive and/or motor development within this population. Methods: Predetermined criteria were used to examine four publication databases (PsycInfo, Embase, Web of Science and Medline) and identify studies considering the determinants of cognitive and motor development in children aged 0–8 years in Sub‐Saharan Africa. Results: In total, 51 quantitative studies met the inclusion criteria, reporting on 30% of countries across the region. Within these papers, factors associated with early child development were grouped into five themes: Nutrition, Growth and Anthropometry, Maternal Health, Malaria and HIV, and Household. Food security and dietary diversity were associated with positive developmental outcomes, whereas exposure to HIV, malaria, poor maternal mental health, poor sanitation, maternal alcohol abuse and stunting were indicators of poor cognitive and motor development. Discussion: In this synthesis of research findings obtained across Sub‐Saharan Africa, factors that restrict physical growth are also shown to hinder the development of early cognitive and motor skills, although additional factors also influence early developmental outcomes. In 2017, an estimated 1 in every 4 (23%) children aged < 5 years were stunted worldwide. With slow progress in stunting reduction in many regions and the realization that a large proportion of stunting is not due to insufficient diet or diarrhea alone, it remains that other factors must explain continued growth faltering. Environmental enteric dysfunction (EED), a subclinical state of intestinal inflammation, can occur in infants across the developing world and is proposed as an immediate causal factor connecting poor sanitation and stunting. A result of chronic pathogen exposure, EED presents multiple causal pathways, and as such the scope and sensitivity of traditional water, sanitation, and hygiene (WASH) interventions have possibly been unsubstantial. Although the definite pathogenesis of EED and the mechanism by which stunting occurs are yet to be defined, this paper reviews the existing literature surrounding the proposed pathology and transmission of EED in infants and considerations for nutrition and WASH interventions to improve linear growth worldwide. • Group-based caregiver training in rural Tanzania led to 0.29 SD improvement in early child development. • The intervention caused changes in home environment, parent-child interactions, and the use of positive parenting strategies. • Changes were measured on the practices of both mothers and fathers. Group-based parent training programmes present an affordable means to influence the early experiences of children at scale. This paper reports evidence on the effectiveness of a practice-led intervention piloted in rural Tanzania evaluated through a matched control study design. The core of the programme is an 8–10 week caregiver training course led by local facilitators, built around early stimulation and nurturing care. After two years of implementation, the intervention led to improvements in the development of 3-year olds of 0.29 standard deviations. Detailed data on caregivers indicates that these improvements are due to changes in the type and frequency of caregiver-child interactions for both mothers and fathers, as well as the quality of play materials in the home. • The Finnish Baby Box provides parents of newborns with a wide range of infant clothing and other basics. • The cardboard box in which the infant can sleep was emphasized by the media and commercial companies outside Finland. • The box preventing SIDS was advanced by private companies and the media. • This was less the case in Wales and Scotland, a result of their being governed by authorities with a "left" orientation. For over 80 years the Finnish state has provided expectant mothers with a cardboard box containing an extensive collection of clothing, bathing products for the baby, nappies, bedding and a small mattress. Numerous jurisdictions have implemented it in some form. We investigated through a critical case study methodology whether its translation into the liberal welfare states of Canada, UK, and USA experienced message distortion (the box as a means of preventing SIDS), commercialization, watering down of content; and separation from the complex of Finnish welfare state policies that support families with children. We find that only Scotland and Wales recognized the decommodification and equity roles played by the baby box. This study identified numerous barriers to building progressive public policy in Canada, UK, and USA: the structures and processes of the liberal welfare state, commercial interests that skew public policymaking and media logic that limits news reporting to the concrete and simple, eschewing complex analysis. This paper describes a scoping review of 42 studies of neighborhood effects on developmental health for children ages 0-6, published between 2009 and 2014. It focuses on three themes: (1) theoretical mechanisms that drive early childhood development, i.e. how neighborhoods matter for early childhood development; (2) dependence of such mechanisms on place-based characteristics i.e. where neighborhood effects occur; (3) dependence of such mechanisms on child characteristics, i.e. for whom is development most affected. Given that ecological systems theories postulate diverse mechanisms via which neighborhood characteristics affect early child development, we specifically examine evidence on mediation and/or moderation effects. We conclude by discussing future challenges, and proposing recommendations for analyses that utilize ecological longitudinal population-based databases. [ABSTRACT FROM AUTHOR]

Background: Low-cost, cross-culturally comparable measures of the motor, cognitive, and socioemotional skills of children under 3 years remain scarce. In the present paper, we aim to develop a new caregiver-reported early childhood development (ECD) scale designed to be implemented as part of household surveys in low-resourced settings. Methods: We evaluate the acceptability, test-retest reliability, internal consistency, and discriminant validity of the new ECD items, subscales, and full scale in a sample of 2481 18- to 36-month-old children from peri-urban and rural Tanzania. We also compare total and subscale scores with performance on the Bayley Scales of Infant Development (BSID-III) in a subsample of 1036 children. Qualitative interviews from 10 mothers and 10 field workers are used to inform quantitative data. Results: Adequate levels of acceptability and internal consistency were found for the new scale and its motor, cognitive, and socioemotional subscales. Correlations between the new scale and the BSID-III were high (r > .50) for the motor and cognitive subscales, but low (r < .20) for the socioemotional subscale. The new scale discriminated between children's skills based on age, stunting status, caregiver-reported disability, and adult stimulation. Test-retest reliability scores were variable among a subset of items tested. Conclusions: Results of this study provide empirical support from a low-income country setting for the acceptability, reliability, and validity of a new caregiver-reported ECD scale. Additional research is needed to test these and other caregiver reported items in children in the full 0 to 3 year range across multiple cultural and linguistic settings. [ABSTRACT FROM AUTHOR]

Even though it is widely recognized that early childhood development (ECD) is one of the most important predictors of future social capital and national productivity, the recently published ECD Lancet Series reports that about 250 million children under 5 years are at risk of not reaching their developmental potential, mainly as a result of poverty and social injustice. So why is this and what will it take to reverse this situation? The purpose of this special issue is to highlight important contributions from previously published articles in Maternal & Child Nutrition to the field of nutrition and ECD. The collection of papers presented in this special issue collectively indicates that although nutrition‐specific interventions are essential for child development, they are not sufficient by themselves for children to reach their full developmental potential. This is because ECD is influenced by many other factors besides nutrition, including hand washing/sanitation, parenting skills, psychosocial stimulation, and social protection. Future research should focus on mixed‐methods implementation science seeking to understand how best to translate evidence‐based integrated ECD packages into effective intersectoral policies and programs on a large scale. In addition to health and nutrition, these programs need to consider and include responsive parenting (including responsive feeding), learning stimulation, education, and social protection. Early child development (ECD) – the development of physical, social–emotional, and language–cognitive capacities in the early years – is a foundation of health, well-being, learning, and behaviour across the life course. Consequently, the capacity to monitor ECD is an important facet of a modern society. This capacity is achieved by having in place an ongoing flow of high-quality information on the state of early child development, its determinants, and long-term developmental outcomes. Accordingly, there remains a considerable need for research that merges community-centred, longitudinal, and linked-data approaches to monitoring child development. The current paper addresses this need by introducing one method of summarising and quantifying the developmental trajectories of British Columbian children at the neighbourhood- or district-level: computing the Community Index of Child Development (CICD) for each geographic area. A simple index that describes change in children's developmental trajectories at the aggregate level, the CICD is computable because of our capacity to conduct individual-level linkage of two population data sets: the Early Development Instrument (EDI), a holistic measure of children's readiness for school which is administered at Kindergarten, and the British Columbia Ministry of Education's Foundation Skills Assessment (FSA), a Grade 4 measure of academic skills. In this paper, we demonstrate: (a) wide variation in the CICDs according to the children's district of residence in Kindergarten; (b) an association of the CICDs with an indicator of the socioeconomic character of the neighbourhoods; and (c) contrasting patterns of neighbourhood convergence and divergence in two different school districts – such that, in some areas, children from high vulnerability neighbourhoods tend to catch up between Kindergarten and Grade 4 whereas, in other areas, they tend to fall further behind. Short-term effectiveness of the International Child Development Programme (ICDP) for parents in the general population has been studied. The aim of this paper was to investigate the longer term impact of the ICDP programme on parents looking for sustained changes 6–12 months after the programme. For this, a non-clinical caregiver group attending the ICDP programme (N = 79) and a non-attending comparison group (N = 62) completed questionnaires on parenting, psychosocial functioning, and child difficulties before, on completion and 6–12 months after the ICDP programme. Analyses compare changes in scores over time. The results revealed that the ICDP group showed significantly improved scores on parenting measures, less loneliness, and trends towards improved self-efficacy compared to the comparison group 6–12 months after programme completion. The ICDP group also reported that their children spent significantly less time on television and computer games and a trend towards fewer child difficulties. Key positive effects sustained over time but at a somewhat lower level, supporting community-wide implementation of ICDP as a general parenting programme. In recent years, a growing number of studies have documented the prevalence of violence against children as well as its consequences. Across every country and cultural context in which research have been conducted, studies have consistently shown that exposure to violence negatively impacts the health and well-being of children, and hampers their development. Many actors have worked to implement programs aimed at addressing this problem, and some, in particular parenting programs, have shown promise as a means of effectively reducing child maltreatment. It is essential, however, to take an integrated approach in settings where fundamental concerns exist over the provisioning of basic health and nutritional needs as well as adequate stimulation. The childs experiences during these early years are critical for their future developmental trajectory and life course. Additional program components, generally implemented as part of early child development programs, are indispensable for ensuring the healthy development of the child. This position paper takes a global perspective in summarizing the key literature and approaches from both violence prevention and early child development, outlines common objectives shared by the two fields, and demonstrates the urgent necessity for holistic cooperation across the two fields. • Parental belief is associated with Child social-emotional development. • Parental investments mediate parental belief and child development. • The mediation effects vary across investments and developmental outcomes. • The varieties of play materials and activities are the strongest mediators. Parental investments are associated with early child outcomes, and some evidences outside China suggest that parental belief might affect parental investments. However, the interrelationships of parental belief, parental investments, and early child development has not been well documented in China. This paper aims to study the interrelationships between the caregiver's parental belief, the caregiver's parental investments, and the child's early developmental outcomes in rural China. A total of 1787 sample households in an undeveloped rural area of western China are enrolled in the cross-sectional study. A parental belief questionnaire, the Family Care Indicators (FCI), the Bayley Scales of Infant Development version III (BSID-III), and a socioeconomic questionnaire were used to measure the caregiver's parental belief on parenting practices, the caregiver's parental investments, the child's early developmental outcomes, and the socioeconomic characteristics of sample households, respectively. The mediation model was then applied to estimate the interrelationships. The results find that the caregiver's parental investments significantly mediate in the relationships between the caregiver's parental belief and the child's early developmental outcomes. Through parental investments, one standard deviation (SD) increase in the caregiver's parental belief is corresponding to 3% of one SD increase in the child's four developmental outcomes (cognition, language, motor, and social–emotion) respectively. For future studies aimed at designing targeted interventions on early child development in rural China, the key findings of this paper might be informative. Early interventions aimed at strengthening the caregiver's subjective belief on parenting practices and increasing the parental investments in the household might be effective to improve the development of rural children. Sustainable development, a foundation of the post-2015 global agenda, depends on healthy and productive citizens. The origins of adult health begin early in life, stemming from genetic-environmental interactions that include adequate nutrition and opportunities for responsive learning. Inequities associated with inadequate nutrition and early learning opportunities can undermine children's health and development, thereby compromising their productivity and societal contributions. Transactional theory serves as a useful framework for examining the associations that link early child development and nutrition because it emphasizes the interplay that occurs between children and the environment, mediated through caregiver interactions. Although single interventions targeting early child development or nutrition can be effective, there is limited evidence on the development, implementation, evaluation, and scaling up of integrated interventions. This manuscript introduces a special edition of papers on six topics central to integrated child development/nutrition interventions: (1) review of integrated interventions; (2) methods and topics in designing integrated interventions; (3) economic considerations related to integrated interventions; (4) capacity-building considerations; (5) examples of integrated interventions; and (6) policy implications of integrated interventions. Ensuring the health and development of infants and young children through integrated child development/nutrition interventions promotes equity, a critical component of sustainable development. Stunting and poor child development are major public health concerns in Malawi. Integrated nutrition and early child development (ECD) interventions have shown potential to reduce stunting, but it is not known how these integrated approaches can be implemented in Malawi. In this paper, we aimed to evaluate the current jobs status of community health workers and their potential to implement integrated approaches. This was accomplished by a desk review of nutrition and ECD policy documents, as well as interviews with key informants, community health workers, and community members. We found that Malawi has comprehensive policies and well-outlined coordination structures for nutrition and ECD that advocate for integrated approaches. Strong multidisciplinary interaction exists at central levels but not at the community level. Integration of community health workers from different sectors is limited by workload, logistics, and a lack of synchronized work schedules. Favorable, sound policies and well-outlined coordination structures alone are not enough for the establishment of integrated nutrition and ECD activities. Balanced bureaucratic structures, improved task allocation, and synchronization of work schedules across all relevant sectors are needed for integrated intervention in Malawi. [ABSTRACT FROM AUTHOR]

• Climatic shocks suffered during early-life stages impact children's cognitive and anthropometric development. • Income, consumption, nutrition and diet composition are potential drivers of the effects. • Evidence of government programs that partially mitigate the impact of the weather shocks is provided. The fact that shocks in early life can have long-term consequences is well established in the literature. This paper examines the effects of extreme precipitations on cognitive and health outcomes and shows that impacts can be detected as early as 2 years of age. Our analyses indicate that negative conditions (i.e., extreme precipitations) experienced during the early stages of life affect children's physical, cognitive and behavioral development measured between 2 and 6 years of age. Affected children exhibit lower cognitive development (measured through language, working and long-term memory and visual-spatial thinking) in the magnitude of 0.15 to 0.19 SDs. Lower height and weight impacts are also identified. Changes in food consumption and diet composition appear to be key drivers behind these impacts. Partial evidence of mitigation from the delivery of government programs is found, suggesting that if not addressed promptly and with targeted policies, cognitive functioning delays may not be easily recovered. This paper reviews the literature on early child development among Koreans, with a focus on autism spectrum disorders (ASD). The literature review of 951 abstracts in English, 101 abstracts in Korean and 27 full articles published from 1994 to 2011 was performed to understand the presentation of and response to ASD in Korean culture. Based on research to date on the identification, description, and treatment of ASD in Korean populations, we argue that at both conceptual and practical levels, early child development and interventions must be understood within cultural context. Culturally informed research on ASD is vital for increasing awareness of the importance of early intervention and the need for educational and psychological services in countries in which autism is stigmatized, misdiagnosed or undiagnosed. Summary: Community based growth promotion is a widely advocated but less studied means to improve child nutrition in low income countries. Using a longitudinal comparison of treatment and control communities, this paper indicated a significant impact of one such program, in Uganda, on the nutritional status of children less than one year of age in the treatment group. Parenting practices are associated with early childhood development (ECD), and some evidences suggest that mental health might affect parenting practices. However, the interrelationships of mental health, parenting practices, and ECD outcomes have not yet been well documented in developing contexts like rural China. This paper aims to investigate the interrelationships between the caregiver's mental health, parenting practices, and the child's ECD outcomes in rural China. A total of 1787 sample households in an undeveloped rural area of western China are enrolled in the study. A socioeconomic questionnaire, the Depression, Anxiety and Stress Scale, the Family Care Indicators, the Parent and Family Adjustment Scales, and the Bayley Scales of Infant Development version III were used to measure the socioeconomic characteristics of sample households, the caregiver's mental health, parental investments and parenting skills, and the child's development outcomes, respectively. Mediation model was then applied to estimate the interrelationships. The results showed that parental practices significantly mediated between the caregiver's mental health and the child's cognition, language, motor, and social–emotion development. Through parental investments, one standard deviation increases in the caregiver's mental health test score was associated with the decline in the child's four development scores by 0.6% standard deviation, respectively. Through parenting skills, one standard deviation increases in the caregiver's mental health test score was associated with the decline in the child's language and social-emotional score by 2% and 5% standard deviation, respectively. Different dimensions of caregiver mental health, parental investments and skills played heterogeneous roles in the interrelationships. Early interventions aimed at improving the caregiver's mental health, parental investments and skills are important and might be effective to improve early childhood development in rural China.
