A significant amount of researcher and practitioner effort has focused on developing new chemical controls for the parasitic Varroa destructor mite in beekeeping. One outcome of that has been the development and testing of "glycerol–oxalic acid" mixtures to place in colonies for extended periods of time, an off-label use of the otherwise legal miticide oxalic acid. The majority of circulated work on this approach was led by practitioners and published in nonacademic journals, highlighting a lack of effective partnership between practitioners and scientists and a possible failure of the extension mandate in beekeeping in the United States. Here, we summarize the practitioner-led studies we could locate and partner with a commercial beekeeper in the Southeast of the United States to test the "shop towel–oxalic acid–glycerol" delivery system developed by those practitioners. Our study, using 129 commercial colonies between honey flows in 2017 split into 4 treatment groups, showed no effectiveness in reducing Varroa parasitism in colonies exposed to oxalic acid–glycerol shop towels. We highlight the discrepancy between our results and those circulated by practitioners, at least for the Southeast, and the failure of extension to support practitioners engaged in research. While we have the tools to achieve this goal, the persistent barriers to healthcare services experienced by too many individuals will need to be addressed to make significant progress and improve the health and quality of life of all people with human immunodeficiency virus (HIV). The necessary structural changes require actions by federal, state, and local policymakers and range from ensuring universal access to healthcare services to optimizing care delivery to ensuring a robust and diverse infectious diseases and HIV workforce. In this article, we outlines 10 key principles for policy reforms that, if advanced, would make ending the HIV epidemic in the United States possible and could have much more far-reaching effects in improving the health of our nation. The Centers for Medicare & Medicaid Services' Severe Sepsis and Septic Shock Early Management Bundle (SEP-1) measure has appropriately established sepsis as a national priority. However, the Infectious Diseases Society of America (IDSA and five additional endorsing societies) is concerned about SEP-1's potential to drive antibiotic overuse because it does not account for the high rate of sepsis overdiagnosis and encourages aggressive antibiotics for all patients with possible sepsis, regardless of the certainty of diagnosis or severity of illness. IDSA is also concerned that SEP-1's complex "time zero" definition is not evidence-based and is prone to inter-observer variation. In this position paper, IDSA outlines several recommendations aimed at reducing the risk of unintended consequences of SEP-1 while maintaining focus on its evidence-based elements. IDSA's core recommendation is to limit SEP-1 to septic shock, for which the evidence supporting the benefit of immediate antibiotics is greatest. Prompt empiric antibiotics are often appropriate for suspected sepsis without shock, but IDSA believes there is too much heterogeneity and difficulty defining this population, uncertainty about the presence of infection, and insufficient data on the necessity of immediate antibiotics to support a mandatory treatment standard for all patients in this category. IDSA believes guidance on managing possible sepsis without shock is more appropriate for guidelines that can delineate the strengths and limitations of supporting evidence and allow clinicians discretion in applying specific recommendations to individual patients. Removing sepsis without shock from SEP-1 will mitigate the risk of unnecessary antibiotic prescribing for noninfectious syndromes, simplify data abstraction, increase measure reliability, and focus attention on the population most likely to benefit from immediate empiric broad-spectrum antibiotics. The Territorial Papers of the United States are a valuable and underused resource containing almost 10,000 documents written between 1789 and 1848 about the formation of new sovereign states from US territory. These communications between the federal government and frontier settlers comprise the actual discourse of the nation's expansion over six decades. Digitizing the Territorial Papers permits the possibility of analyzing the entire corpus globally. Text mining and topic modeling methods give us a lens on the language patterns through which new state governments and the expanding nation were formed. An initial statistical analysis of the textual information provides a visualization of content, helps discern how ideals about governance emerged, and lays the foundation for developing more sophisticated hypotheses and theoretical constructs. Over the past several decades, citizenship status has become more important in immigrant lives and communities in the United States. Undocumented adults who arrived as children, the 1.5 generation, comprise a growing percentage of the immigrant population. Although they are similar to children of immigrants born in the United States (the second generation) they face a variety of barriers to integration due to their lack of legal status. Based on over five years of ethnographic fieldwork with mainly 1.5 and second generation Mexican-American men during a period of major healthcare reform, this paper addresses how citizenship status and embeddedness within multi-status communities impacts immigrant experiences in the healthcare domain. In particular, I argue that the Affordable Care Act (ACA) has contributed to an institutionalization of the status differences and a further differentiation in the social integration of these groups of children of immigrants. The novel methodological approach and the data which emerges through fieldwork reveals important insights into the process whereby healthcare reforms have consequences for immigrant communities which I show through highlighting the status-signaling event that is generated through various forms of direct and indirect interaction with the ACA. The implications of this extend beyond healthcare, and I discuss its impact on issues including ethnic identity and psychological well-being. Studies are largely optimistic about the ability of standardized procedures to constrain decision-makers' biases and produce more equitable results across fields. However, work that embraces standardization as an equalizing force stands in contrast to research on standardization and street-level bureaucrats, which asserts that standardized procedures are not self-actuating and cannot be understood apart from the environments in which they are used. I examine how frontline workers vary in their approach to an actuarial-based tool intended to standardize judgments. In a highly controlled decision-making environment, child welfare workers whose racial and sex characteristics afford them higher status report subverting the tool; conversely, workers in the same position whose ascriptive characteristics yield them lower status in terms of race and sex describe following the rules. In an environment where the same tool is adopted only ceremonially, all workers experience decision-making as unconstrained, regardless of their ascriptive characteristics. This work fills gaps in knowledge about how social status and organizational context intersect to affect rule abidance. Examining these dynamic relationships advances understanding of how organizations reproduce inequality and the limits and potential for standardization to transform social hierarchies. Persons living with human immunodeficiency virus (HIV) and others receiving antiretrovirals are at risk for medication errors during hospitalization and at transitions of care. These errors may result in adverse effects or viral resistance, limiting future treatment options. A range of interventions is described in the literature to decrease the occurrence or duration of medication errors, including review of electronic health records, clinical checklists at care transitions, and daily review of medication lists. To reduce the risk of medication-related errors, antiretroviral stewardship programs (ARVSPs) are needed to enhance patient safety. This call to action, endorsed by the Infectious Diseases Society of America, the HIV Medicine Association, and the American Academy of HIV Medicine, is modeled upon the success of antimicrobial stewardship programs now mandated by the Joint Commission. Herein, we propose definitions of ARVSPs, suggest resources for ARVSP leadership, and provide a summary of published, successful strategies for ARVSP that healthcare facilities may use to develop locally appropriate programs. The article examines the classified study of the Vietnam War conducted by the U.S. agency the Defense Department commissioned by U.S. defense secretary Robert McNamara, the role of U.S. military analyst Daniel Ellsberg in releasing the Pentagon Papers, and the actions of the administration of U.S. president Richard M. How do states' positions across multiple and interconnected economic networks affect their power? The Weaponized Interdependence (WI) scholarship emphasizes that states centrally located in global economic networks have access to new sources of coercion. In this paper, we look at how their positions across multiple networks interact with each other to create new opportunities and vulnerabilities. We use network analysis to map the semiconductor supply chain and show that it can be viewed as four interrelated networks: (1) design, (2) raw material, (3) manufacturing equipment, and (4) assembled chips. We then highlight how states' centrality varies across these networks and how it shapes their respective opportunities for coercion. Looking specifically at the United States, we emphasize how its centrality in the design network enables it to weaponize chokepoints in the trade network of assembled chips. In so doing the paper makes three contributions. First, it highlights how interactions among multiple economic networks provide new opportunities for states to weaponize interdependence. Second, it contributes to recent attempts using network analysis to analyze structural power on the global stage. An essay is presented on the subject of cigarette smoking among U.S. laborers in factories, corporate spying on workers, and the social culture of shopfloors in the U.S. during the 20th century. It examines the records of Erie, Pennsylvania, paper manufacturer Hammermill Paper Company concerning employee cigarette consumption, how employees attempted to evade corporate surveillance of their smoking, and labor relations in the U.S.

Scholars have documented strong anti-immigrant bias in mainstream media portrayals and public sentiment, especially associated with notions of "illegality." Yet certain groups of undocumented immigrants have been able to defend their continued residency. How are undocumented immigrants, as marginalized subjects, able to make claims for legal and social recognition? Through an analysis of 125 anti-deportation campaigns led by undocumented youth organizations in the United States, I show how organizations developed a nationally coordinated model using citizenship frames to challenge deportations and build support for pro-immigrant legislation. Citizenship frames are based in legal and normative ideologies of citizenship that underscore acculturation, civic engagement, and humanitarian concerns. The campaigns highlight undocumented immigrants' social integration, deservingness, and practice of citizenship, therefore contesting the boundaries between citizen and noncitizen, and between lawful and unlawful. Though citizenship frames are used across cases, campaign tactics vary: students are portrayed as high-achievers who would suffer greatly if deported, while campaigns for non-students emphasize the detrimental effects of deportation on others. Importantly, citizenship frames are bounded by the government's priorities for immigration enforcement, with potentially negative consequences for those immigrants considered less than model citizens. This article presents a case study of commercial cannabis in the United States. Drawing on 56 interviews with cannabis stakeholders collected between 2018–2020, I examine how different governmentalities of surveillance became distorted by the contradiction between state and federal cannabis laws. As in other regulated markets, these governmentalities informed state-sponsored surveillance initiatives to stop, contain, or support certain forms of deviance by commercial cannabis businesses. Due to fragmented governance, the efficacy of these initiatives depended in part upon the actions of the regulated cannabis industry. Commercial cannabis businesses looked to how surveillance was configured to develop strategies that could help them overcome challenges stemming from their semi-legality. These strategies included incorporating practices that were not required by law, partnering with the state in surveillance efforts, and engaging in activities to combat the black market. I argue that the embedded relationship between governmentalities, surveillance initiatives, and commercial cannabis activities transformed these strategies into mechanisms through which structure emerged in this nascent market. This paper introduces a set of surveillance categories, proposes new directions for research on social control and markets, and offers a novel study of commercial cannabis that can help to explain the trajectory of this market. Objectives Aging populations and an increasing number of immigrants in recent years have led to numerous changes in intergenerational relationships. Although many studies have investigated the impact of providing care to a parent with dementia, little is known about the impact of caregiving activities provided from a distance, such as in the case of immigration, and over a long period of time to a person with dementia. Our understanding of how transnational caregiving for a person with dementia affects relationships is also limited. Using the Intergenerational Solidarity Theory as a theoretical framework, this paper examines the experiences of adult children and immigrant caregivers of a parent with dementia living in Poland. Methods A qualitative, semistructured interview was conducted with 37 caregivers living in the United States while providing transnational care to a parent with Alzheimer's disease or other forms of dementia. The data analysis was based on the thematic analysis strategy. Results Four themes were identified: (1) filial obligations and solidarity, (2) caregivers' contradictory emotions regarding transnational care, (3) financial and emotional exhaustion, and (4) challenges of nursing home dilemmas. Discussion Transnational caregivers represent a unique group who face distinctive challenges related to competing demands and limited resources. This study contributes to a better understanding of their experiences whereas the findings highlight the importance of addressing the mental and physical well-being of immigrant caregivers of individuals with dementia and have important implications for health care professionals and immigration policies. Objective The primary aim of this paper is to accelerate the number of randomized experimental studies of the reliability and validity in-home tele-neuropsychological testing (tele-np-t). Method We conducted a critical review of the tele-neuropsychology literature. We discuss this research in the context of the United States' public and private healthcare payer systems, including the Centers for Medicare & Medicaid Services (CMS) and Current Procedural Terminology (CPT) coding system's telehealth lists, and existing disparities in healthcare access. Results The number of tele-np publications has been stagnant since the onset of the COVID-19 pandemic. There are less published experimental studies of tele-neuropsychology (tele-np), and particularly in-home tele-np-t, than other tele-np publications. There is strong foundational evidence of the acceptability, feasibility, and reliability of tele-np-t, but relatively few studies of the reliability and validity of in-home tele-np-t using randomization methodology. Conclusions More studies of the reliability and validity of in-home tele-np-t using randomization methodology are necessary to support inclusion of tele-np-t codes on the CMS and CPT telehealth lists, and subsequently, the integration and delivery of in-home tele-np-t services across providers and institutions. Topics include SEP-1 the Centers for Medicare and Medicaid Services has signaled that early treatment of severe sepsis and septic shock is a national priority, and the measure was launched in 2015 because hospitalization rates for sepsis.

Frailty represents an integrative prognostic marker of risk that associates with a myriad of age-related adverse outcomes in older adults. As a concept, frailty can help to target scarce resources and identify subgroups of vulnerable older adults that may benefit from interventions or changes in medical management, such as pursing less aggressive glycaemic targets for frail older adults with diabetes. In practice, however, there are several operational challenges to implementing frailty screening outside the confines of geriatric medicine. Electronic frailty indices (eFIs) based on the theory of deficit accumulation, derived from routine data housed in the electronic health record, have emerged as a rapid, feasible and valid approach to screen for frailty at scale. The goal of this paper is to describe the early experience of three diverse groups in developing, implementing and adopting eFIs (The English National Health Service, US Department of Veterans Affairs and Atrium Health—Wake Forest Baptist). These groups span different countries and organisational complexity, using eFIs for both research and clinical care, and represent different levels of progress with clinical implementation. Using an implementation science framework, we describe common elements of successful implementation in these settings and set an agenda for future research and expansion of eFI-informed initiatives. Background and Objectives Evidence-based practices to manage distress behaviors in dementia (DBD) are not consistently implemented despite demonstrated effectiveness. The Veterans Health Administration (VA) trained teams to implement Staff Training in Assisted Living Residences (STAR)-VA, an intervention to manage DBD in VA nursing home settings, or Community Living Centers (CLCs). This paper summarizes multiyear formative evaluation results including challenges, adaptations, and lessons learned to support sustained integration into usual care across CLCs nationwide. Research Design and Methods STAR was selected as an evidence-based practice for DBD, adapted for and piloted in VA (STAR-VA), and implemented through a train-the-trainer program from 2013 to 2018. Training and consultation were provided to 92 CLC teams. Evaluation before and after training and consultation included descriptive statistics of measures of clinical impact and survey feedback from site teams regarding self-confidence, engagement, resource quality, and content analysis of implementation facilitators and challenges. Results STAR-VA training and consultation increased staff confidence and resulted in significant decreases in DBD, depression, anxiety, and agitation for Veterans engaged in the intervention. Implementation outcomes demonstrated feasibility and identified facilitators and barriers. Key findings were interpreted using implementation frameworks and informed subsequent modifications to sustain implementation. Discussion and Implications STAR-VA successfully prepared teams to manage DBD and resulted in improved outcomes. Lessons learned include importance of behavioral health–nursing partnerships, continuous engagement, iterative feedback and adaptations, and sustainment planning. Evaluation of sustainment factors has informed selection of implementation strategies to address sustainment barriers. Repeated claims that a dwindling supply of potential caregivers is creating a crisis in care for the U.S. aging population have not been well-grounded in empirical research. Concerns about the supply of family care do not adequately recognize factors that may modify the availability and willingness of family and friends to provide care to older persons in need of assistance or the increasing heterogeneity of the older population. In this paper, we set forth a framework that places family caregiving in the context of older adults' care needs, the alternatives available to them, and the outcomes of that care. We focus on care networks, rather than individuals, and discuss the demographic and social changes that may alter the formation of care networks in the future. As pressure increases to support social justice, coalitions try to understand their role in oppression while implementing practices to create equitable environments. Twenty-six education-focused community coalitions in the United States participated in the study. Coalition interviews and membership rosters are used to evaluate the degree to which these coalitions engage in community engagement practices conducive to empowerment. Involvement practices emphasize communication strategies and participatory design structures. Systems change practices emphasize agenda-setting processes and a level of awareness of structural sources that create social marginalization. This work investigates how these two practices create empowerment. This study uses fuzzy-set qualitative comparative analysis to examine the impact of community poverty, coalition age, government representation, coalition size, school district size, and coalition governance on involvement and systems change practices. The results demonstrate combinations of these conditions in four mutually exclusive solutions for involvement practices and two mutually exclusive solutions for systems change practices. The paper ends by identifying cases that coincide with these solutions and how communities thoughtfully configure their collaborations to coincide with empowerment theory. We explore the racial and ethnic politics associated with mask wearing early in the COVID-19 pandemic. Using panel and cross-sectional data from the National Panel Study of COVID-19 (n = 1,996) fielded in March and April 2020, we asked Americans about their self-reported race/ethnicity, their political party affiliation, and if they have worn a mask or scarf in public to stop the spread of the coronavirus. We also asked respondents who reported wearing a mask in public if they worry they might be mistaken for a criminal. Our logistic regression models found that racial and ethnic minorities are more likely to wear a mask or scarf over their faces to prevent the spread of COVID-19 despite being more likely to worry about being criminalized by police or security while wearing a mask. We argue that proximal contact with the virus, because racial and ethnic minorities are more likely to be affected by COVID-19, leads to increased willingness by people in these groups to protect themselves and others. Democrats were also more likely to wear masks compared with Republican respondents. and those living in Republican-led states were less likely to wear a mask in public and more likely to be criminalized for wearing a mask. Was mask use during the early COVID-19 pandemic political? In our paper, we use panel and cross-sectional data from the National Panel Study of COVID-19 fielded in March and April 2020 when we asked Americans whether they had worn masks. We also asked those who wore a mask if they felt criminalized while wearing a mask. In this paper, we analyze trends in Americans' immigration attitudes and policy preferences nationally and across partisan and racial/ethnic groups. In the 1990s and early 2000s, Democrats and Republicans shared similarly negative attitudes toward immigrants and high levels of support for restrictionist immigration policies. Beginning in the 2010s and continuing through the early 2020s, however, Democrats' aggregate immigration opinions liberalized considerably. We observed increasingly liberal immigration preferences among Democrats of all racial and ethnic backgrounds after 2016, but this trend was especially pronounced among white Democrats. Among Republicans, opinion on immigration remained mostly stable over this period, although in some cases it became more conservative (e.g. border security) and more liberal on others (e.g. amnesty). The marked liberalization in immigration opinion among Democrats has left partisans more divided on immigration than at any point since national surveys began consistently measuring immigration opinion in the late twentieth century. Opposition to LGBT rights remains a contemporary fixture within the United States in spite of increasingly liberalizing attitudes toward LGBT individuals. In this paper, I argue that a potentially overlooked factor driving this opposition is rural identity—or an individual's psychological attachment to a rural area. Using data from the 2020 ANES, I find that rural identity predicts less favorable estimations of LGBT individuals. Rural identifiers are also less likely to support pro-LGBT policy measures than nonrural identifiers. Nevertheless, I find the magnitude of the effects of rural identity on anti-LGBT views to be surprisingly small. It is also the case that, on average, rural identifiers exhibit net-positive estimations of LGBT individuals and are broadly supportive of LGBT rights, suggesting that elected officials enacting anti-LGBT legislation in rural areas of the United States are potentially out of step with the preferences of their electorate. These findings also have implications for what it means to hold a rural identity beyond a generalized animosity toward urban areas, and for understanding urban-rural divergences in US public opinion on issues such as LGBT rights. Objectives This paper examines the family ties of older adults in the United States and how they are associated with mental health and social activity. We compare older adults with 4 types of family ties: adults "close" to family in proximity and social network, "kinless" older adults without a partner or children, "distanced" adults who live far from close kin, and "disconnected" older adults who do not report kin in their social network or do not report a location for some kin. Methods Using pooled data from the National Health and Aging Trends Study 2015–2019 for older adults aged 70 and older (N = 24,818 person-waves), we examine how family ties are associated with mental health and social activity, and whether lacking family is tied to poor well-being because older adults' needs are not being met. Results Kinless older adults and disconnected older adults have poorer outcomes (lower mental health scores and less social activity), compared to those close to their family. These findings suggest that both the presence and quality of the connection, as measured here via both location and social network, are critical for understanding which older adults are "at risk." Older adults who were not geographically proximate to their close kin (i.e. distanced) were not disadvantaged relative to those close to their families. Unmet needs do not help explain these patterns. In 2019, the National Health Interview survey found that nearly 59% of adults reported pain some, most, or every day in the past 3 months, with 39% reporting back pain, making back pain the most prevalent source of pain, and a significant issue among adults. Often, identifying a direct, treatable cause for back pain is challenging, especially as it is often attributed to complex, multifaceted issues involving biological, psychological, and social components. Due to the difficulty in treating the true cause of chronic low back pain (cLBP), an over-reliance on opioid pain medications among cLBP patients has developed, which is associated with increased prevalence of opioid use disorder and increased risk of death. To combat the rise of opioid-related deaths, the National Institutes of Health (NIH) initiated the Helping to End Addiction Long-TermSM (HEAL) initiative, whose goal is to address the causes and treatment of opioid use disorder while also seeking to better understand, diagnose, and treat chronic pain. The NIH Back Pain Consortium (BACPAC) Research Program, a network of 14 funded entities, was launched as a part of the HEAL initiative to help address limitations surrounding the diagnosis and treatment of cLBP. This paper provides an overview of the BACPAC research program's goals and overall structure, and describes the harmonization efforts across the consortium, define its research agenda, and develop a collaborative project which utilizes the strengths of the network. The fumes created from welding activities present a unique occupational hazard. Due to the complex processes which govern fume formation, the characterization of welding fumes is difficult. Emission factors (EFs) are one method to characterize fume formation from different processes and scenarios. This paper reviews the development of EFs and similar metrics both historic research which contributed to the US EPAs AP-42 summary of welding emission factors released in 1995, and more recent research initiatives. Through a critical analysis of what research has been done in this area and the strength of the emission factors developed, this paper proposes a set of recommendations for future research. Research on emission factors for gas metal arc welding (GMAW) is the most complete amongst the different types of electric arc welding. Despite it being generally known that flux core arc welding (FCAW) creates significant fume emissions compared to some of the other processes few studies have looked at FCAW since the AP-42. Shielded metal arc welding is also under-researched particularly in terms of metal-specific emission factors. The influence of different welding activity parameters such as welding location, speed or current is well defined for GMAW but requires more attention for other welding processes. Further effort towards compiling and comparing available emission factor data of quality, evaluating the available data statistically and organizing this data in a practically useful way is required. Objective The goal of this scoping review was to determine the types and rates of pain and dysfunction outcomes reported after gender-affirming surgical procedures. In addition, a summary of the involvement of conservative care reported in the literature was produced. Methods A research librarian conducted searches through multiple databases from inception to 2021. Abstracts and full texts were reviewed by a team of at least 2 reviewers. Data were extracted from a custom survey and exported for summary. Results Thirty-one papers discussed masculinizing interventions, and 87 papers discussed feminizing procedures. Most of the studies emphasized surgical outcomes. Of the studies reporting pain or dysfunction, few standardized outcomes were used to collect information from patients. The pain was experienced across body regions after surgery for both feminizing and masculinizing procedures. Vaginal stenosis and incontinence were the most common complications reported. Patients were most often managed by physical therapists for vaginal stenosis or dyspareunia. Conclusion Many published studies do not systematically collect specific or standardized information about pain and dysfunction after gender-affirming surgery. Of those studies that do report these outcomes, few detail the involvement of physical therapists in the recovery after surgery. Impact Pain and urogenital dysfunction, often managed by physical therapists, occur after gender-affirming surgery, suggesting that physical therapists could have a larger role in the recovery of this patient population after surgery. The extent to which interventions used to manage these conditions in cisgender people will be equally effective in transgender people is unknown at this time. This paper examines how straightforward application of two relatively simple statistical concepts, the measurement of relative variation or disparity in per-pupil expenditures by school districts and interpretation of the term 'percentile' as referring to school districts rather than students, triggered multimillion dollar litigation. As explained in Section 2, different interpretations regarding how to calculate relative disparities based on such percentages resulted in opposite conclusions as to whether funding equalization had been met and, in turn, as to which entity should receive federal funding-the Zuni school district or New Mexico state education officials. Section 3 covers Professor Gastwirth's two papers previously published in this journal, which effectively juxtaposed the lack of statistical expertise in the legislative and administrative processes related to the Federal Impact Aid Act, on one hand, and his own repeated surveys within the academic statistical community regarding the statistical meaning of this statute, on the other hand. Sections 4 and 5 discuss, the Supreme Court's 2007 decision affirming the Department of Education's funding formula, how this case reflects the ongoing tension between various Justices concerning the appropriate level of deference that courts should accord to federal agency regulations, the role of expertise (here statistical science) in the public policy arena that includes judicial review of disputes between aid recipients and disbursing agencies and the explicit mechanism through which external expertise on these issues becomes incorporated into the judicial process. Section 6 concludes that Zuni could have been decided differently if the consensus of statistical opinion concerning the term 'percentile' was part of the case record and taken into account by the Justices. Despite evidence of its effectiveness, tobacco cessation is not systematically addressed in routine healthcare settings. Its measurement is part of the problem. A pilot study was designed to develop and implement two different tobacco tracking systems in two independent primary care offices that participated in an IPA Model health maintenance organization in Portland, Oregon. The first clinic, which utilized a paper-based charting system, implemented CPT-like tracking codes to measure and report tobacco-cessation activities, which were eventually included in the managed-care organization's (MCO) claims database. The second clinic implemented an electronic tracking system based on its computerized electronic medical record (EMR) charting system. This paper describes the pilot study, including the processes involved in building provider acceptance for the new tracking systems in these two clinics, the barriers and successes encountered during implementation, and the resources expended by the clinics and by the MCO during the pilot. The findings from the 3-month implementation period were that documentation of tobacco-use status remained stable at 42-45% in the paper-based clinic and increased from 79% to 88% in the EMR clinic. This pilot study demonstrated that Tracking Codes are a feasible preventive-care tracking system in paper-based medical offices. However, high levels of effort and support are needed, and a critical mass of insurers and health plans would need to adopt Tracking Codes before widespread use could be expected. In a national survey, 82% of U.S. neuropsychologists who offered services to Hispanics self-reported inadequate preparation to work with this population (Echemendia, Harris, Congett, Diaz, & Puente, 1997). The purpose of this paper is to improve the quality and accessibility of neuropsychological services for Hispanic people living in the United States by giving guidance for service delivery, training, and organizational policy. General guidance towards this end comes from professional ethics for psychologists and interpreters/translators, federal civil rights law, the International Test Commission, and the Office of Minority Health of the U.S. Department of Health and Human Services, among others. This guidance is specifically applied here to cover professional cultural and linguistic competence of neuropsychologists, psychometrists, interpreters, translators, and consultants; languages of evaluation; use of interpreters; evaluation of acculturation; test translation, adaptation, and interpretation; application of test norms; intervention issues; reimbursement; and organizational issues. During this centennial year of the American Society of Animal Science (ASAS), it is of interest to look back over the history of our Society and, in particular, to the many contributions made by researchers in the area of swine nutrition. A great number of basic and applied research studies involving the nutrition of weanling, growing, and finishing pigs, and gestating and lactating sows have been conducted by swine nutritionists during the past 100 yr. Most of these studies were conducted at universities by animal scientists or by the graduate students under their leadership. Others were conducted by nutritionists in the feed and pharmaceutical industries and government scientists at ARS/USDA research centers. Contributions were also made by animal scientists beyond our borders. Much of the research was published in the Journal of Animal Science during its 66 yr of existence. Before the first issue of the journal was published in 1942, some of the earlier studies were reported in the Proceedings of the Annual Meeting of the Society of Animal Production, the forerunner of ASAS. These research studies have progressively led to a better understanding of the role and utilization of dietary energy, protein, AA, carbohydrates, fats, minerals, and vitamins by pigs and have helped to quantify the nutrient requirements of pigs for various stages of growth, for sows during gestation and lactation, and to a limited extent, for boars. Determining the nutritional value of a wide array of feedstuffs, evaluating feeding strategies, and assessing the value of growth-promoting and carcass-enhancing agents have been important research contributions as well. To identify the particular studies that were among the most instrumental in contributing to our present knowledge of swine nutrition is, to say the least, a daunting assignment. To aid in this task, a survey of swine nutritionists was conducted in which they were asked to identify and rank the 10 most significant findings in swine nutrition during the past 100 yr. Forage-animal production agriculture is implementing infrastructure changes and management strategies to adjust to increased energy-related costs of fuel, feed grains, fertilizers, and seeds. The primary objectives of this position paper are to assess future research and extension scientific needs in forage utilization, financial support for the discipline, and changing status and number of scientists. A survey questionnaire returned from 25 land-grant universities in the eastern half of the United States rated the top 4 research needs as 1) pasture systems and efficiency of production; 2) interfacing with energy concerns; 3) forage cultivar evaluations and persistence; and 4) environment impacts. Plant-animal future research needs at 11 USDA-ARS regional locations are targeted at sustainable management and improved livestock performance, ecophysiology and ecology of grasslands, environment impacts, and improved technologies for nutritive value assessments. Extension scientists from 17 southern and northeastern states listed the top 3 needs as forage persistence, soil fertility and nutrient management, and pasture systems and efficiency of production. Grant funds currently provide more than 40% of land-grant university research and extension efforts in forage utilization, and scientists estimate that this support base will increase to 55 to 60% of the funding total by 2013. Reduced allocation of state and federal funding has contributed to a reduction in the number of full-time equivalent (FTE) scientists engaged in forage utilization research and extension activities. The current 25 state FTE conducting research number about 2.8 per state. This includes 10 states with >3, 11 states with <2, and 3 states with <1 FTE. Increased interest in cellulosic energy, climate change, and environmental impact may offer new opportunities for these FTE to participate in integrated cross-discipline research Extension programming, and technology transfer methods will change to accommodate reduced funding but with increasing numbers of novice, recreation-oriented landowners. Fisheries extension programmes frequently fail to secure mandatory or voluntary adoption of bycatch reduction devices and techniques. Approaches for improving the outcomes of extension programmes are often based on ad hoc assessments and do not consider human behaviour or change theories. This paper offers an in-depth analysis of extension activities that led to various adoption outcomes in two prominent bycatch case studies in the United States: turtle excluder devices in shrimp trawl fisheries and dolphin bycatch in the tuna purse seine fishery. Using a grounded theory approach to text analysis of interviews and documents, I examine five periods of voluntary or mandatory adoption efforts. I explain the outcomes through the lens of diffusion of innovation theory. The most effective extension programme involved informative and persuasive efforts, enforced regulations, and commercially practical bycatch reduction devices. Voluntary adoption occurred under exceptional circumstances of public and political pressure and a device that offered substantial benefits to the adopter. The two periods of successful adoption applied the most core principles of diffusion theory. This paper concludes with recommendations for how change agents can apply diffusion theory to future fisheries extension programmes to improve the adoption of bycatch reduction devices. We often look to the examples set by other countries when thinking about how to deal with problems at home. But what explains why some countries are more likely to serve as role models than others? Are people influenced mainly by the examples set by countries that are considered to be particularly successful in a given policy area? Or are they influenced to a greater extent by countries that are more familiar to them, or that appear more similar to their own? In this paper, we test these hypotheses using two large (n |$\ > {\rm{\ }}$| 2,000) survey-based experiments conducted in the United States. The first focuses on environmental issues, while the second focuses on human rights issues. In both studies, the respondents were presented with a randomly drawn list of countries and asked to indicate how relevant the examples that these countries set should be to policy discussions in the United States. Our results suggest that the US public's perception of the relevance of other countries' experiences is driven primarily by the extent to which the respondent perceives the other countries as familiar to them, or similar to the United States, rather than by more objective measures of policy success. The perverseness of racial and ethnic inequities in the United States continues to implore the investigation of their causes. While there have been improvements in the health of the US population, these improvements have not been equally distributed. To commemorate the 100th anniversary of the American Journal of Epidemiology , in this commentary, we aim to highlight the Journal 's contributions to: 1) the definition and use of race and ethnicity in research, and 2) understanding racial and ethnic inequities, both empirically and methodologically, over the past decade. We commend the Journal for its contributions and for spearheading many of the challenges related to measuring and interpreting racial and ethnic data for the past 20 years. We identify 3 additional areas in which the Journal could make further impact to address racial and ethnic inequities: 1) devote a section in every issue of the Journal to scientific papers that make substantive epidemiologic or methodological contributions to racial and ethnic inequities in health; 2) update the Journal 's guidelines for authors to include justifying the use of race and ethnicity; and 3) diversify the field of epidemiology by bringing a new cadre of scholars from minoritized racial and ethnic groups who represent the most affected communities into the research process. Claims about the deterrent role of forward military deployments often depend on the argument that attacks on troops stationed abroad will activate a "tripwire effect." It is assumed that this effect would generate strong domestic pressure for further military intervention by the country whose troops have come under attack. In theory, the anticipation of a strong tripwire effect prospectively ties the hands of leaders, thereby bolstering the reliability of extended deterrent threats and promises. In this paper, we define the tripwire effect and use both conjoint and vignette survey experiments to evaluate its operation and magnitude among Americans. Results suggest that the tripwire effect is, at best, far weaker than many analysts and policymakers commonly assume. This finding raises serious questions about a core logic underpinning the United States' forward military posture and highlights the need for more research on the deterrent functions of forward deployment. Pronounced disparities exist in detecting and treating breast cancer in women with disabilities, leading to cancer detection at advanced stages. This paper provides an overview of disparities for women with disabilities related to breast cancer screening and care, primarily focusing on clinically significant mobility disabilities. Current care gaps include screening barriers related to accessibility and inequitable treatment options, with race and ethnicity, socioeconomic status, geographic location, and disability severity factors mediating the disparities for this population. The reasons for these disparities are myriad and stem from both system-level deficiencies and individual-level clinician bias. Although structural changes are warranted, individual healthcare professionals must also be incorporated into the requisite change. Intersectionality is critical to disparities and inequities and should be central to any discussion of strategies for improving care for people with disabilities, many of whom have intersectional identities. Efforts to reduce screening rate disparities for breast cancer in women with mobility-related disabilities should start with improving accessibility through removing structural barriers, establishing comprehensive accessibility standards, and addressing healthcare professional bias. Future interventional studies are needed to implement and assess the value of programs to improve breast cancer screening rates in women with disabilities. Increasing the representation of women with disabilities in clinical trials may provide another avenue for reducing treatment disparities because these trials often provide breakthrough treatment to women with cancer diagnosed at later stages. Ultimately, attention to the specific needs of patients with disabilities should be improved across the United States to promote inclusive and effective cancer screening and treatment. Mathematics Learning Support (MLS) has been widely available in Higher Education Institutions in the United States (US) for decades. However, until recently, there has been little research that considered the extent of provision. In this paper, we present the results of a survey of MLS with responses from 268 institutions across the US, which indicate that such support is well established across responding institutions. To place these results in context, we compare them to other recent international surveys of provision. The micro-randomized trial is a sequential randomized experimental design to empirically evaluate the effectiveness of mobile health intervention components that may be delivered at hundreds or thousands of decision points. Micro-randomized trials have motivated a new class of causal estimands, termed causal excursion effects , for which semiparametric inference can be conducted via a weighted, centred least-squares criterion (Boruvka et al. 2018). Causal excursion effects allow health scientists to answer important scientific questions about how intervention effectiveness may change over time or may be moderated by individual characteristics, time-varying context or past responses. Existing definitions and associated methods assume between-subject independence and noninterference. Deviations from these assumptions often occur. In this paper, causal excursion effects are revisited under potential cluster-level treatment effect heterogeneity and interference, where the treatment effect of interest may depend on cluster-level moderators. Background. In April 2008, experts reviewed updates on sexually transmitted disease (STD) prevention and treatment in preparation for the revision of the Centers for Disease Control and Prevention (CDC) STD Treatment Guidelines. This included a review of cervical cancer screening in the STD clinical setting. Methods. Key questions were identified with assistance from an expert panel. Reviews of the literature were conducted using the PubMed computerized database and shared with the panel. Updated information was incorporated in the 2010 CDC STD Treatment Guidelines. Results. We recommend that STD clinics offering cervical screening services screen and treat women according to guidelines by the American College of Obstetrics and Gynecology, the American Cancer Society, the US Preventive Services Task Force, and the American Society for Colposcopists and Cervical Pathologists. New to the 2010 guidelines are higher age for initiating cervical screening (age ≥21 years) and less frequent intervals of screening (at least every 3 years). New recommendations include new technologies, such as liquid-based cytology and high-risk human papillomavirus (HPV) DNA tests. Liquid-based technologies are not recommended over conventional testing. HPV DNA tests are recommended as adjunct tests and with new indications for use in cervical screening and management. Stronger recommendations were issued for STD clinics offering cervical screening services to have protocols in place for follow-up of test results and referral (eg, colposcopy). Conclusions. Important additions to the 2010 STD Treatment Guidelines include information on updated algorithms for screening and management of women and recommendations for use of liquid-based cytology and high-risk HPV testing. Christian nonprofits have an important place in the religious landscape of the United States. They are providing a wide range of goods and services to individuals, congregations, and even denominations. Despite their substantial and growing influence, we know little about how these organizations are using the billions of dollars they collectively receive. I examine one issue that is frequently of interest to both scholars and the public. Using new data on over 1,900 Christian nonprofits, the analysis looks at leadership compensation in these organizations. I focus on how organizational resources, internal and external monitoring, and leadership power influence whether and how much Christian nonprofits compensate their leaders. Resources and leadership power are found to be positively associated with the amount of compensation given to leaders in Christian nonprofits, while monitoring does not have a significant association. His political career in the Democratic Party is also discussed, including his time as governor of Indiana and his positions in the federal government.

Compared with the more traditional sciences of nutrition, physiology, and reproduction, the acceptance of animal welfare science in its own right is still relatively new. Seven colleagues, who had an average of 10 yr experience with beef (n = 5), swine (n = 5), dairy (n = 2), poultry (n 1), and sheep (n = 1) were asked several questions on the opportunities and challenges facing the field. The information collected was pooled for anonymity. General challenges identified by the group were (1) are we making progress and how can this be defined, (2) demand for information has outpaced the science, and (3) pressures from stakeholders. Solutions were (1) to continue providing sound science that has been validated, measured objectively, and is reliable; and (2) to continue to have animal science and veterinary medicine departments employ faculty trained in farm animal welfare. Highlights for the future were willingness for animal welfare scientists to work across disciplines and across departments, within the same institution, and enthusiastically across state lines; and expansion of new teaching models, in conclusion, new and innovative tools, personalities, and dedication to the field of animal welfare will continue to provide scientific information and direction for farm animal welfare science. Discoveries, understanding, and innovations in meat science during the last century have led to revolutionary changes in meat and poultry production, processing, marketing, and consumption. American Society of Animal Science members have made key contributions in most, if not all, categories of advancement. The first US university meat science program was begun in Minnesota in 1905. Use of mechanical refrigeration in the meatpacking industry, improved transportation and packaging, and home refrigeration provided more flexibility, variety, and consistency in meat and meat products in the early 1900s. Cooperative meat research was begun by 27 universities in 1925, with a focus on the observational characterization of carcass traits and composition, meat quality attributes, and causes of the wide variation in these variables. Scientific study of genetic, nutritional, and environmental influences on the growth, physiology, and postmortem biochemistry of muscle often used muscle-comparative investigations. Rigor mortis, cold shortening and thaw rigor, postmortem muscle metabolism, postmortem tenderization and tenderness variation, and postmortem myoglobin and lipid oxidation were studied vigorously in the 1960s and beyond, defining the biochemical bases for associated outcomes in fresh and processed products. Value-added benefits resulted from implementation of electrical stimulation, boxed beef and modified-atmosphere packaging, restructuring technologies, collagen recovery, and muscle profiling work. Isolation, purification, and definition of the primary structure and biophysical properties of the myofribillar and cytoskeletal proteins in muscle aided the understanding of contraction and postmortem changes. The role of Ca-dependent proteases in meat tenderness and muscle growth is being clarified. The chemistry of meat curing, meat emulsion formation, fermentation, and other processing methods led to new technologies, new meat products, and new benchmarks in product shelf life and quality.
