Background: Cardiogenic stroke has increasing morbidity in China and brought economic burden to patient families. In cardiogenic stroke diagnosis, echocardiograph examination is one of the most important examinations. Sonographers will investigate patients' heart via echocardiograph, and describe them in the echocardiograph reports. In this study, we developed a machine learning model to automatically identify diagnosis evidences of cardiogenic stroke providing to neurologist for clinical decision making.Methods: We collected 4188 Chinese echocardiograph reports of 4018 patients, with average length 177 Chinese characters in free-text style. Collaborating with neurologists and sonographers, we summarized 149 phrases on diagnosis evidence of cardiogenic stroke such as "" (severe mitral stenosis), "" (aortic valve degeneration) and so on. Furthermore, we developed an annotated corpus via mapping 149 phrases to the 4188 reports. We selected 11 most frequent diagnosis evidence types such as "" (mitral stenosis) for further identifying. The generated corpus is divided into training set and testing set in the ratio of 8:2, which is used to train and validate a machine learning model to identify the evidence of cardiogenic stroke using BiLSTM-CRF algorithm.Results: Our machine learning method achieved the average performance on the diagnosis evidence identification is 98.03, 90.17 and 93.94% respectively. In addition, our method is capable to identify the novel diagnosis evidence of cardiogenic stroke description such as "-" (mitral stenosis), "" (aortic valve calcification) et al. CONCLUSIONS: In this study, we analyze the structure of the echocardiograph reports and summarized 149 phrases on diagnosis evidence of cardiogenic stroke. We use the phrases to generate an annotated corpus automatically, which greatly reduces the cost of manual annotation. The model trained based on the corpus also has a good performance on the testing set. The method of automatically identifying diagnosis evidence of cardiogenic stroke proposed in this study will be further refined in the practice. [ABSTRACT FROM AUTHOR]

For time immemorial, Chinese herbal medicines (CHMs) have been widely used in China for disease treatment and promotion of general well-being. However, in recent years, many studies have shown that mycotoxins produced by fungi could contaminate CHMs due to unfavourable pre- or post-harvest conditions, raising major concern for consumer safety. At present, there is a significant focus on developing novel mycotoxin detection methods for analysing CHMs, and numerous studies have aimed to determine which kinds of raw herbal materials are most susceptible to mycotoxin contamination. In this review, we focus on recent advances in understanding and detection of mycotoxins in domestic raw herbal materials and related products from 2000 to 2018. Aspects of mycotoxin contamination of CHMs covered in this review include common mycotoxin contaminants in CHMs, maximum mycotoxin residue limits, analytical methods for mycotoxin detection and their applications and limitations, as well as a brief discussion of the trends in ongoing research. Peanut (Arachis hypogaea L.) is one of the most important crops produced worldwide. Peanut is the dominant crop in the typical upland red soil areas of China; however, phosphorus bioavailability in red soil is very low, which severely affects peanut production. To improve the phosphorus bioavailability, which substantially promotes the green development of peanut production, a peanut–green manure rotation field experiment was conducted with six treatments (milkvetch; radish; brassica rape; mustard rape; winter fallow and no-tillage), commencing in September 2017 in the red soil area of Jiangxi province, China. The results show that compared with no-tillage (NT) treatments, different green manure returning treatments had significant effects on soil pH, soil phosphorus components and available potassium content. The particulate phosphorus and soil available phosphorus contents in the green manure treatments were significantly higher than those in the winter fallow (WF) treatment. Compared with the WF treatment, the content of particulate phosphorous in brassica rape (BR), radish (R) and milkvetch (MV) treatments was significantly increased by 6.55%, 3.66% and 2.50%, respectively; the available phosphorus content in mustard rape (MR), BR, R and MV was significantly increased by 20.93%, 25.60%, 23.76% and 18.10%, respectively. In addition, the total phosphorus content of peanut shell in the MV and R treatment was significantly higher than that in the WF treatment, increasing by 33.47% and 60.66%, respectively. Compared with the WF treatment, the peanut biomass of MR, BR and R treatments increased significantly by 19.51%, 29.83% and 19.77%, respectively. The total phosphorus accumulation in all green manure treatments was higher than that in the WF treatment, and the MV treatment reached a significant level at 18.83%. Based on these results, the particulate phosphorus (PP) and available phosphorus were significantly affected by different green manure treatments; green manure amendment improves peanut phosphorus uptake. The use of green manure (especially milkvetch and brassica rape) can be recommended to improve phosphorus bioavailability and yield of peanut in red soil areas. [ABSTRACT FROM AUTHOR]

PURPOSE The coronavirus disease 2019 (COVID-19) pandemic facilitated the rapid development of digital detection surveillance (DDS) for outbreaks. This qualitative study examined how DDS for infectious diseases (ID) was perceived and experienced by primary care physicians and patients in order to highlight ethical considerations for promoting patients' autonomy and health care rights. METHODS In-depth interviews were conducted with a purposefully selected group of 16 primary care physicians and 24 of their patients. The group was reflective of a range of ages, educational attainment, and clinical experiences from urban areas in northern and southern China. Interviews were audio recorded, transcribed, and translated. Two researchers coded data and organized it into themes. A third researcher reviewed 15% of the data and discussed findings with the other researchers to assure accuracy. RESULTS Five themes were identified: ambiguity around the need for informed consent with usage of DDS; importance of autonomous decision making; potential for discrimination against vulnerable users of DDS for ID; risk of social inequity and disparate care outcomes; and authoritarian institutions' responsibility for maintaining health data security. The adoption of DDS meant some patients would be reluctant to go to the hospital for fear of either being discriminated against or forced into quarantine. Certain groups (older people and children) were thought to be vulnerable to DDS misappropriation. CONCLUSIONS These findings indicate the paramount importance of establishing national and international ethical frameworks for DDS implementation. Frameworks should guide all aspects of ID surveillance, addressing privacy protection and health security, and underscored by principles of social equity and accountability. [ABSTRACT FROM AUTHOR]

One of the primary limiting factors for biological activities in desert ecosystems is nitrogen (N). This study therefore examined the effects of N and investigated the responses of an arid ecosystem to global change. We selected the typical desert plant Populus euphratica in a desert ecosystem in the Ebinur Lake area to evaluate the effects of N deposition on desert soil respiration. Three levels of N deposition (0, 37.5 and 112.5 kg·N·ha-1·yr-1) were randomly artificially provided to simulate natural N deposition. Changes in the soil respiration rates were measured from July to September in both 2010 and 2013, after N deposition in April 2010. The different levels of N deposition affected the total soil N, soil organic matter, soil C/N ratio, microorganism number, and microbial community structure and function. However, variable effects were observed over time in relation to changes in the magnitude of N deposition. Simulated high N deposition significantly reduced the soil respiration rate by approximately 23.6±2.5% (P<0.05), whereas low N deposition significantly increased the soil respiration rate by approximately 66.7±2.7% (P<0.05). These differences were clearer in the final growth stage (September). The different levels of N deposition had little effect on soil moisture, whereas N deposition significantly increased the soil temperature in the 0–5 cm layer (P<0.05). These results suggest that in the desert ecosystem of the Ebinur Lake area, N deposition indirectly changes the soil respiration rate by altering soil properties. [ABSTRACT FROM AUTHOR]

Primary lung cancer is one of the most common malignant tumors in China. Approximately 60% of lung cancer patients have distant metastasis at the initial diagnosis, so it is necessary to find new tumor markers for early diagnosis and individualized treatment. Tumor markers contribute to the early diagnosis of lung cancer and play important roles in early detection and treatment, as well as in precision medicine, efficacy monitoring, and prognosis prediction. The pathological diagnosis of lung cancer in small biopsy specimens determines whether there are tumor cells in the biopsy and tumor type. Because biopsy is traumatic and the compliance of patients with multiple biopsies is poor, liquid biopsy has become a hot research direction. Liquid biopsies are advantageous because they are nontraumatic, easy to obtain, reflect the overall state of the tumor, and allow for real-time monitoring. At present, liquid biopsies mainly include circulating tumor cells, circulating tumor DNA, exosomes, microRNA, circulating RNA, tumor platelets, and tumor endothelial cells. Background: The concept of health-related quality of life (HRQoL) was brought up decades ago and has been well utilized in many different areas. Regarding immune thrombocytopenia (ITP) management, much work has been done to emphasize the necessity of taking HRQoL into consideration. However, data on HRQoL of children with chronic ITP remain rare.Methods: This is a cross-sectional study. Children with chronic ITP aged from 2 to 18 and their parents were recruited. Participants completed the Pediatric Quality of Life Inventory™ (PedsQL™) and Kids' ITP Tools (KIT) questionnaires at only one time. The Pearson's correlation was examined between these measures for the pooled samples.Results: A total of 42 families participated. Mean child self-report scores of KIT and PedsQL™ were 78.60 (SD = 12.40) and 85.13 (SD = 14.12), respectively, corresponding to parent proxy report scores, which were 73.40 (SD = 19.96) and 85.10 (SD = 13.56), respectively. Mean score of KIT parent impact report was only 40.39 (SD = 19.96). Significantly higher KIT scores (self-report and parent proxy) were observed in children with PLT more than 30 × 10*9/L compared to others, and this difference was even more noticeable in the PedsQL™ parent proxy report group (p < 0.001). As with intravenous immunoglobulin, the statistics difference appeared only in KIT child self-report group (p = 0.03), while for bone marrow examination, the difference appeared only in PedsQL™ parent proxy report group (p = 0.01). A negative relationship was apparent between duration of disease and scores. Gender and use of corticosteroids had no impact on the KIT and PedsQL™ scores here. Internal consistency reliability was demonstrated with Cronbach's alpha for all scales above the acceptable level of 0.89 (range from 0.88 to 0.97). There was a substantial concordance (p < 0.001) between the child and parent proxy scores (ICC for KIT is 0.59, ICC for PedsQL™ is 0.85). Meanwhile, KIT scores are correlated with PedsQL™ (r = 0.75 for child self report, r = 0.61 for parent proxy report).Conclusions: ITP affects HRQoL of children and parents. Parents are much more concerned with the disease than their children, which seriously influence their HRQoL as a result. The cross-culture translated KIT is reliable and valid with acceptable correlation to the PedsQL™. KIT provides valuable information of childhood ITP and will be a reliable outcome measure for further clinical research on HRQoL. [ABSTRACT FROM AUTHOR]

Background: The expression of µ-opioid receptor has important role in cognitive dysfunction in Schizophrenia (SZ). The results of studies about the association of polymorphisms of µ-opioid receptor gene (OPRM1) with SZ were inconsistent. Methods: We conducted a case-control study to investigate the genetic association between OPRM1 polymorphisms and SZ among the Han chinese population. 264 SZ patients and 264 age-matched control subjects were recruited. Four SNPs of OPRM1 were successfully genotyped by using PCR-RFLP. Results: Of four polymorphisms, rs1799971 and rs2075572 were shown to associate with SZ. Compared with the A allele of rs1799971 and C allele of rs2075572, the G allele of rs1799971 and rs2075572 was associated with an almost 0.46-fold risk (OR = 0.46, 95% CI: 0.357-0.59, P < 0.01) and 0.7-fold risk (OR = 0.707, 95% CI: 0.534-0.937, P = 0.015) of the occurrence of SZ,. When subjects were divided by gender, rs1799971 remained significant difference only in males (OR = 0.309, 95% CI: 0.218-0.439 for G allele, P < 0.01), and rs2075572 only in females (OR = 0.399, 95% CI: 0.246-0.648 for G allele, P < 0.01). In secondary analysis with subsets of patients, the G allele of rs1799971 (compared to the A allele) was associated with a decreased risk of all patients and male patients with apathy symptoms (OR = 0.086, 95% CI: 0.048-0.151, P = 0.01; OR = 0.083, 95% CI: 0.045-0.153, P < 0.01), and the G allele of rs2075572 (compared to the C allele) was associated with a decreased risk of all patients and female patients with positive family history (OR = 0.468, 95% CI: 0.309-0.71, P < 0.01; OR = 0.34, 95% CI: 0.195-0.593, P < 0.01). In addition, haplotype analysis revealed that two SNP haplotypes (A-C-C-G and G-C-C-A) were associated with decreased risks of SZ (P < 0.01). The other two (G-C-C-G and G-G-C-G) with increased risks of SZ (P < 0.01). Conclusions: The present study demonstrated for the first time that the OPRM1 polymorphism may be a risk factor for schizophrenia in the Han Chinese. Further studies are needed to give a global view of this polymorphism in pathogenesis of schizophrenia in a large-scale sample, family-based association design or well-defined subgroups of schizophrenia. Vitex rotundifolia L. is an important plant species used in traditional Chinese medicine. For its efficient use and conservation, genetic diversity and clonal variation of V. rotundifolia populations in China were investigated using inter-simple sequence repeat markers. Fourteen natural populations were included to estimate genetic diversity, and a large population with 135 individuals was used to analyze clonal variation and fine-scale spatial genetic structure. The overall genetic diversity ( GD) of V. rotundifolia populations in China was moderate ( GD = 0.190), with about 40% within-population variation. Across all populations surveyed, the average within-population diversity was moderate ( P = 22.6%; GD = 0.086). A relatively high genetic differentiation ( Gst = 0.587) among populations was detected based on the analysis of molecular variance data. Such characteristics of V. rotundifolia are likely attributed to its sexual/asexual reproduction and limited gene flow. The genotypic diversity ( D = 0.992) was greater than the average values of a clonal plant, indicating its significant reproduction through seedlings. Spatial autocorrelation analysis showed a clear within-population structure with gene clusters of approximately 20 m. Genetic diversity patterns of V. rotundifolia in China provide a useful guide for its efficient use and conservation by selecting particular populations displaying greater variation that may contain required medicinal compounds, and by sampling individuals in a population at >20 m spatial intervals to avoid collecting individuals with identical or similar genotypes. Background: There has been little literature about leukemia epidemiology in Nanjing in recent years. We aimed to explore the incidence rate, gender and age distribution of leukemia in Nanjing using the leukemia database of the city. Results: The average yearly incidence rate of leukemia was 3.68/105 during 2003 - 2007 in Nanjing. There were no significant difference in gender (x2 = 3.266, p > 0.05) or seasons (x2 = 11.36, p > 0.05). The incidence rate was the highest in group aged 80~ years (18.64/105). AML accounted for approximately 36.8% of all leukemias. Conclusions: The incidence rate of leukemia, especially in the aged population, was relatively high in Nanjing. Leukemia is the major malignant tumor in children. Therefore, more attention should be paid to leukemia in children and the aged people. [ABSTRACT FROM AUTHOR]
