This study investigates the level and predictors of life satisfaction in people living in slums in Kolkata, India. Participants of six slum settlements (n = 164; 91% female) were interviewed and data on age, gender, poverty indicators and life satisfaction were collected. The results showed that the level of global life satisfaction in this sample of slum residents did not significantly differ from that of a representative sample of another large Indian city. In terms of life-domain satisfaction, the slum residents were most satisfied with their social relationships and least satisfied with their financial situation. Global life satisfaction was predicted by age, income and non-monetary poverty indicators (deprivation in terms of health, education and living standards) (R2 15.4%). The current study supports previous findings showing that people living in slums tend to report higher levels of life satisfaction than one might expect given the deprivation of objective circumstances of their lives. Furthermore, the results suggest that factors other than objective poverty make life more, or less, satisfying. The present study explores the nature of poverty by considering both consumption and multidimensional approaches and assesses the change in it due to variation in population subgroups and household characteristics in India between 2004–2005 and 2011–2012. We have developed a Multidimensional Poverty Index for India which has the advantage of being comparable with consumption poverty using National Sample Survey Organization data relating to the 'Level and Pattern of Consumer Expenditure' of the latest two rounds. Our analysis shows a significant reduction in consumption poverty as well as multidimensional poverty. Multidimensional poverty has reduced significantly for both consumption poor and non-poor but the reduction has not been uniform across subgroups and household characteristics. Satisfactorily, the reduction was faster among the poorer subgroups. Notwithstanding this trend, urban location, upper strata of social caste, literate head, self- or regular employment and higher level of benefits from Public Distribution System (PDS) have a favorable impact on the nature of poverty. Objectives mHealth interventions for MNCH have been shown to improve uptake of antenatal and neonatal services in low- and middle-income countries (LMICs). However, little systematic analysis is available about their impact on infant health outcomes, such as reducing low birth weight or malnutrition among children under the age of five. The objective of this study is to determine if an age- and stage-based mobile phone voice messaging initiative for women, during pregnancy and up to 1 year after delivery, can reduce low birth weight and child malnutrition and improve women's infant care knowledge and practices. Methods We conducted a pseudo-randomized controlled trial among pregnant women from urban slums and low-income areas in Mumbai, India. Pregnant women, 18 years and older, speaking Hindi or Marathi were enrolled and assigned to receive mMitra messages (intervention group N = 1516) or not (Control group N = 500). Women in the intervention group received mMitra voice messages two times per week throughout their pregnancy and until their infant turned 1 year of age. Infant's birth weight, anthropometric data at 1 year of age, and status of immunization were obtained from Maternal Child Health (MCH) cards to assess impact on primary infant health outcomes. Women's infant health care practices and knowledge were assessed through interviews administered immediately after women enrolled in the study (Time 1), after they delivered their babies (Time 2), and after their babies turned 1 year old (Time 3). 15 infant care practices self-reported by women (Time 3) and knowledge on ten infant care topics (Time 2) were also compared between intervention and control arms. Results We observed a trend for increased odds of a baby being born at or above the ideal birth weight of 2.5 kg in the intervention group compared to controls (odds ratio (OR) 1.334, 95% confidence interval (CI) 0.983–1.839, p = 0.064). The intervention group performed significantly better on two infant care practice indicators: giving the infant supplementary feeding at 6 months of age (OR 1.4, 95% CI 1.08–1.82, p = 0.009) and fully immunizing the infant as prescribed under the Government of India's child immunization program (OR 1.531, 95% CI 1.141–2.055, p = 0.005). Women in the intervention group had increased odds of knowing that the baby should be given solid food by 6 months (OR 1.89, 95% CI 1.371–2.605, p < 0.01), that the baby needs to be given vaccines (OR 1.567, 95% CI 1.047–2.345, p = 0.028), and that the ideal birth weight is > 2.5 kg (OR 2.279, 95% CI 1.617–3.213, p < 0.01). Conclusions for Practice This study provides robust evidence that tailored mobile voice messages can significantly improve infant care practices and maternal knowledge that can positively impact infant child health. Furthermore, this is the first prospective study of a voice-based mHealth intervention to demonstrate a positive impact on infant birth weight, a health outcome of public health importance in many LMICs. There has been significant progress in educational attainment in India since its independence. Yet the achievements are low in comparison to countries in similar stages of development. The situation is worse among groups who have a history of exclusion. Children from these groups have lower enrollment and higher dropout rates than national averages. Yet some of them are able to overcome these limitations and show academic resilience. Using data from the Indian Human Development Survey (2005), we study the role of the different child, household, and school-level factors associated with academic resilience among these children. Further, to understand the mechanism through which group membership affects resilience we also study the indirect effect of group membership mediated through those factors. We find that the protective factors vary across the different disadvantaged groups. The main effect of group membership is through structural factors like poverty and maternal education. Yet, the child level and household level factors play an important role in most cases. Schools fail to play the expected role of being the source of resilience among those who face the strongest burden of structural factors. For some, the schools are the main site where they experience most of the adversities of group memberships. The results highlight the need for educational policy, which addresses the importance of schools as drivers and determinants of academic resilience. [ABSTRACT FROM AUTHOR]

Trickle-down has been addressed, so far, using income-based measures of inequality and poverty. However concerns over the inequality in access to other dimensions important for quality of life remains. I revisit trickle-down using the Alkire and Foster (J Public Econ 95(7-8), 2011) class of measures to estimate multidimensional poverty in India. Using NSS data estimates are presented for the 16 major states and are compared to income-based measures. Adding dimensions in poverty measurement results in the reversal of several income-based conclusions about poverty across regions. The paper also finds that contrary to income-based findings, Muslims are less poor than Hindus under the multidimensional criteria. [ABSTRACT FROM AUTHOR]

Objectives In low-income settings, neonatal mortality rates (NMR) are higher among socioeconomically disadvantaged groups. Institutional deliveries have been shown to be protective against neonatal mortality. In Gujarat, India, the access of disadvantaged women to institutional deliveries has increased. However, the impact of increased institutional delivery on NMR has not been studied here. This paper examined if institutional childbirth is associated with lower NMR among disadvantaged women in Gujarat, India. Methods A community-based prospective cohort of pregnant women was followed in three districts in Gujarat, India (July 2013-November 2014). Two thousand nine hundred and nineteen live births to disadvantaged women (tribal or below poverty line) were included in the study. Data was analyzed using multivariable logistic regression. Results The overall NMR was 25 deaths per 1000 live births. Multivariable analysis showed that institutional childbirth was protective against neonatal mortality only among disadvantaged women with obstetric complications during delivery. Among mothers with obstetric complications during delivery, those who gave birth in a private or public facility had significantly lower odds of having a neonatal death than women delivering at home (AOR 0.07 95% CI 0.01-0.45 and AOR 0.03, 95% CI 0.00-0.33 respectively). Conclusions for Practice Our findings highlight the crucial role of institutional delivery to prevent neonatal deaths among those born to disadvantaged women with complications during delivery in this setting. Objectives Few Indian studies have examined the relationship between birth size and stunting in children. Studies on recovery from stunting in India are even fewer. This study, thus, investigates the relationship between birth size and stunting in Andhra Pradesh, India. This study further examines the factors associated with recovery from stunting using a longitudinal data. Methods We used data from the three waves of Young Lives Study (YLS) conducted in Andhra Pradesh in the years 2002, 2006-2007, and 2009 respectively. We used data from 1965 children in wave 1 to examine the association between birth size and stunting. For examining the factors associated with recovery from stunting between 1 and 5 years of age, and between 5 and 8 years, we use data from 582 and 670 children who were stunted at age 1 and age 5 respectively. We use multivariable logistic regression models to fulfil the objectives of the paper. Results The children who were of average- or large- size at birth were significantly less likely to be stunted than children who were of small size at birth (OR 0.61 and 0.47 respectively). Children of average/tall mothers were 0.41 times less likely to be stunted than children of shorter mothers. Severely stunted children were less likely than other stunted children to recover from stunting between 1 and 5 years of age, and between 5 and 8 years. Mother's height was statistically associated with recovery. Change in wealth status of the household was statistically associated with recovery between 1 and 5 years of age. In comparison, child immunization was associated with recovery between 5 and 8 years. Conclusions for Practice This study contributes to the understanding of the impact of birth size on childhood stunting, and to the extent of recovery from stunting in India. Purpose: VISHRAM is a community-based mental health program to address psycho-social distress and risk factors for suicide in a predominantly rural population in Central India, through targeted interventions for the prevention and management of Depression and Alcohol Use Disorders (AUD). The evaluation was designed to assess the impact of program on the contact coverage of evidence-based treatments for depression and AUD through a repeated survey design. This paper describes the baseline prevalence of depression among adults in rural community, association of various demographic and socio-economic factors with depression and estimates contact coverage and costs of care for depression.Methods: Population-based cross-sectional survey of adults in 30 villages of Amravati district in Vidarbha region of Central India. The outcome of interest was a probable diagnosis of depression which was measured using the Patient Health Questionnaire (PHQ-9). Data were analyzed using simple and multiple logistic regression.Results: The outcome of current depression (PHQ-9 ≥ 10) was observed in 14.6 % of the sample (95 % CI 12.8-16.4 %). The contact coverage for current depression was only 4.3 % (95 % CI 1.5-7.1 %). Prevalence of depression varied greatly between the two sites of the study; higher age, female gender, lower education, economic status below poverty line and indebtedness were associated with depression; and while a contact coverage with formal health care was very low, a large proportion of affected persons had consulted family members.Conclusions: Our findings clearly indicate that psycho-social distress in rural communities in Maharashtra is strongly associated with social determinants such as gender, poverty and indebtedness and affects the entire population and not just farmers. Using three quinquennial rounds of consumption expenditure data over two decades (1993-2012), this paper estimates the extent of money metric poverty and inequality in regions of India. Regions are made comparable, and the poverty head count ratio and the poverty gap ratio for 81 regions are derived using the state specific poverty lines as recommended by the Planning Commission of India. The gini index, rich-poor ratio and regression analyses are used to understand the extent of economic inequality in regions of India. Results indicate that though the extent of poverty has declined, economic inequality has increased in regions of India. During 1993-2012, the poverty head count ratio had decreased in 70 regions, increased in seven regions and remained similar in four regions of India. The southern regions of Odisha and southern regions of Chhattisgarh are reeling under high persistent poverty. The spread in poverty head count ratio among regions has increased from 0.38 in 1993-1994 to 0.64 in 2011-2012 confirming divergence in regional poverty in India. The pattern is similar with respect to poverty gap ratio. Regions of Tripura and Sikkim had highest improvements in poverty level. On contrast to poverty estimates, the gini index has decreased in 20 regions and increased in 61 regions. Likewise, 57 regions have recorded increase in rich-poor ratio. The rich-poor ratio was higher in developed regions and lower in less developed regions. Based on these findings, we suggest that regions with persistently high poverty be accorded priority in poverty alleviation program and explore the factors leading to increasing economic inequality. The paper examines the household asset dynamics in India as well as Indian rural States. The paper contributes to the empirical analysis of poverty trap by investigating the presence of one potential poverty trap to simultaneous poverty trap. The paper uses the India Human Development Survey for the year 1993 and 2005. We use the local polynomial regression with Epanechnikov kernel weights to test the existence of multiple or single equilibrium in asset poverty dynamics. Moreover, we use the partial linear mixed model to test the impact of illiteracy trap and under-nutrition trap on asset dynamics process. Across all the States we find only single dynamic asset equilibrium for rural households. However the nature of the asset dynamics varies from one state to another. We find that, in most of the States, asset accumulation does not take place and welfare dynamics is very poor in rural areas. Further, we find under-nutrition trap uniformly affect the asset accumulation in most of the States. However an illiteracy trap affects the asset level heterogeneously over the income and regional distribution. We find the most deprived States (Bihar, Uttar Pradesh, Orissa and Madhya Pradesh) have the multiple poverty trap compared to richer States. Our result implies that asset dynamics of the household varies in the long term according to the types of traps. Using consumption expenditure data of the National Sample Survey 2004-2005, this paper estimates the size of elderly poor and tests the hypotheses that elderly households are not economically better-off compared to non-elderly households in India. Poverty estimates are derived under three scenarios-by applying the official cut-off point of the poverty line to household consumption expenditure (unadjusted), consumption expenditure adjusted to household size and consumption expenditure adjusted to household composition. Results show that an estimated 18 million elderly in India are living below the poverty line. On adjusting the consumption expenditure to household size and composition, there are no significant differences in the incidence of poverty among elderly and non-elderly households in India. This is in contrast to the notion that elderly households are better off than non-elderly households in India. Information and knowledge have re-emerged as essential resources in the fight against poverty, with new opportunities for their dissemination seen to empower the poor and the agencies that work with them. While optimism is high, little empirical evidence exists as to the actual effect of these changes within localized development contexts. This article contributes to overcoming this lacuna by exploring the role of local non-government organizations (NGOs) in Uttarakhand, North India. It examines how the promotion of information and knowledge as development goods have changed the way NGOs understand poverty and their own roles within the sector. I propose the metaphor of NGOs as 'peddlers of information' to draw attention to the current emphasis on awareness, sensitisation and assemblage of information about the grassroots as the primary-and in many cases only-development work NGOs undertake. The objective of this study is to identify individual level factors that determine the use of skilled birth attendants in India. Data from the cross-sectional, population-based NFHS 3 Survey 2005-06 was used. A sample of 31,797 women, aged 15-49 years, who gave births in the 3 months preceding the survey and for whom information was available on most variables, were included in the analysis. Both bivariate and multivariate techniques were used to determine factors associated with the use of skilled attendants at birth. In the study we found that wealth is one of the strongest determinants of skilled birth attendant use, with the poor being at a disadvantage. There are significant differences in the use of skilled delivery care among the urban and rural populations in India. Women in urban areas are more likely to use skilled attendants. Muslim women and women with lower levels of education are also less likely to avail skilled delivery services. The use of skilled care depended significantly on the place of delivery. Women who gave history of antenatal visits were more likely to have skilled attendants at birth. Our analysis demonstrates that there are several financial, social, regional and cultural barriers to skilled birth attendant use in India. Effective strategies need to be planned to generate demand for skilled birth attendants and reduce barriers to care seeking, especially among rural poor. [ABSTRACT FROM AUTHOR]

To examine experiences of perinatal (in pregnancy or post-partum) abuse from in-laws and to assess associations between such experiences and perinatal intimate partner violence (IPV) from husbands, as reported by Indian women residing in low-income communities in Mumbai. The present study includes both qualitative and quantitative research conducted across two phases of study. The qualitative phase involved face-to-face, semi-structured in-depth interviews ( n = 32) with women seeking health care for their infants (6 months or younger) and self-reporting emotional or physical abuse from their husband. The quantitative arm involved survey data collection ( n = 1,038) from mothers seeking immunization for their infants 6 months or younger at three large Urban Health Centers in Mumbai. Results of the qualitative study documented the occurrence of both non-physical and physical abuse from in-laws during pregnancy and post-partum. Non-physical forms of abuse included forced heavy domestic labor, food denial and efforts toward prevention of medical care acquisition. Quantitative results demonstrated that 26.3% of the sample reported perinatal abuse (non-physical and physical) from in-laws and that women experiencing perinatal sexual or physical IPV from husbands were significantly more likely to report perinatal abuse from in-laws (AOR = 5.33, 95% CI = 3.93-7.23). Perinatal abuse from in-laws is not uncommon among women in India and may be compromising maternal and child health in this context; such abuse is also linked to perinatal violence from husbands. Programs and interventions that screen and address IPV in pregnant and post-partum populations in India should be developed to include consideration of in-laws. [ABSTRACT FROM AUTHOR]

This paper examines trends in three reproductive healthcare indicators- namely, antenatal care, medical assistance at delivery, and contraceptive use among the urban poor and non-poor in India using data from the National Family Health Surveys, 1992-1993 and 2005-2006. The urban poor and non-poor are derived from composite wealth indices based on a set of economic proxies. Results indicate that the estimates of poor and non-poor are reliable. During the last 14 years, the service coverage in all three indicators has increased in the country, among both the urban poor and non-poor. However, the utilization of reproductive health services is concentrated among the urban non-poor cutting across the states, with the exception of Kerala. While the non-poor/poor gap in antenatal care and medical assistance at delivery remained large over the years, the gap in contraceptive use has narrowed down cutting across states. After adjusting for other confounders, household poverty was found to be a significant barrier in the utilization of reproductive healthcare services across the states. How and to what extent do different citizens experience democratic governance on a day-to-day basis? What agencies do they utilize in order to have their voices heard and grievances addressed? How do they gain access to government agencies responsible for delivering social welfare services, such as education, security, health care, and poverty relief? Investigations conducted in two states of India inquired about the manner in which different social groups living in rural areas gain access to the welfare services of the Indian state. These results show that an intermediary is required for gaining access. Different types of intermediaries are consulted by separate segments of society. For a large majority of poorer individuals, a newly arisen type of mediator, the naya neta (literally, new leader), is the intermediary of choice. Neither usually low status nor high status, but younger and better educated than other types of village leaders, naya netas play important roles in shaping welfare consequences in these villages of Rajasthan and Andhra Pradesh, most importantly, by affecting equity of access. Using data on 697 individuals from 375 rural low income households in India, we test expectations on the effects of relative income and conspicuous consumption on subjective well-being. The results of the multi-level regression analyses show that individuals who spent more on conspicuous consumption report lower levels of subjective well-being. Surprisingly an individual's relative income position does not affect feelings of well-being. Motivated by positional concerns, people do not passively accept their relative rank but instead consume conspicuous goods to keep up with the Joneses. Conspicuous consumption always comes at the account of the consumption of basic needs. Our analyses point at a positional treadmill effect of the consumption of status goods. [ABSTRACT FROM AUTHOR]

Understanding local attitudes towards human–wildlife conflict (HWC) is key to developing successful conflict mitigation strategies. In this paper, in-depth interview and questionnaire data about resolution of HWC in Uttarakhand, India are examined from both qualitative and quantitative approaches ( n = 70). Responses are differentiated between and within three subgroups: gender, literacy status, and relative wealth. Overall, the plurality of respondents said that fencing is the best solution, that the Forest Department should take leadership, and that villagers would be willing to participate in a cooperative management institution. However, cooperative action was only actively supported by 27.4% of respondents, suggesting that comanagement of this protected area will require significant capacity building and trust building activities. Intragroup differences show that all three factors are significant, and underscore the importance of addressing gender differences in attitudes about HWC in particular. Women were less likely than men to support compensation, more likely to prefer that the village take leadership, and less willing to participate in a cooperative management institution.
