The first edition of Comprehensive Handbook of Psychopathology was published in 1984, al­ most a decade ago. In the interim there has been an explosion of information in psychopathology. Proliferation of knowledge has included a widening base of research data and changing or new concepts and theories regarding classification, measurement methods, and etiology of abnormal behaviors and mental disorders. It has been an active and productive period for biological and behavioral scientists and clinicians, particularly in terms of changing notions of the complex interaction of environmental and biological factors in many disorders. For example, with the classic disorders-such as anxiety and dissociative disorders-our understanding, while far from perfect, has been greatly enhanced in recent years. Whereas there was almost a vacuum of empirical knowledge ten years ago about the personality disorders, concentrated efforts have been undertaken to investigate classification, comorbidities, and expression of the personality disorders, and variants in normal personality traits. In addition, scientific advances in the fields of behavioral medicine, health psychology, and neuropsychology have greatly contributed to our knowledge of psychopathology and the interplay of psychobiological factors. It is now commonly acknowledged that psychopathology is not limited to the traditional mental illness categories; it also plays a significant role in many physical illnesses, such as cancer and AIDS. With these developments, it became clear that the first edition of this handbook was outdated and that a revision was needed.

This Handbook is the culmination of an interest in psychotherapy integration that led to our first professional collaboration in 1978. At that time we undertook (in research conducted for a doctoral dissertation by the second editor and supervised by the senior editor) to understand, from and within a psychodynamic perspective, the experiences of patients who had completed behavioral therapies. At that time, psychotherapy integration was a topic considered viable and interesting by only a few clinicians and scholars, with little communication among them and less awareness, concern, and appreciation on the part of psychotherapists in general. The situation today has changed. The appearance of this Handbook may be taken as a significant sign of maturation and legitimacy of work in psychotherapy integration. It is our hope and expectation that this volume will serve as an up-to-date and exhaustive overview of the status of ongoing scholarly and clinical work in the integration of the major schools of psychotherapy. This book could not have been conceptualized or published 20 years ago. Indeed, it is doubtful that we could have organized the material for this handbook 10 years ago. Over the last 20 years, however, the painstaking efforts of many clinical researchers working with a variety of resistive psychopathologies have resulted in specific psychotherapies and pharmacotherapies that are effective with a significant propor­ tion of patients, at least for some of the disorders. Much clinical research remains to be carried out in the forthcoming decades. But now that we are nearing the 21st century, at least some statement about efficacy can be made. In 1967, Gordon Paul succinctly stated that the ultimate goal of treatment outcome research is to determine'What treatment, by whom, is most effective for this individual with that specific problem, and under which set of circumstances'(p. 111). At that time, empirical evaluations of psychosocial and pharmacologic treatments were few and far between. Methodological strategies for determining treatment effectiveness were also in the formative stage, as exemplified by introduc­ tion of control groups that received inactive interventions (i. e., placebo) and the relatively recent practice of comparing two or more treatments in addition to placebo. In the almost three decades since Paul's oft-quoted dictum, both the quantity and the quality of treatment outcome research with adults have increased dramati­ cally.

In 1988, the Deparunent of Psychology at San Diego State University initiated the first in a planned conference series on Contemporary Issues in Clinical Psychology. It was decided that the focus of this first conference would be depression. Consequently, a number of distinguished scholars were invited to San Diego to discuss contemporary theoretical, empirical, and treatment issues in depressive disorders. This volume contains the results of this conference. Each chapter remains true to the original presentation, although each has been extensively reworked by the authors for inclusion in a book format, and in some cases co-authors have aided in revisions for the volume. Given the sheer quantity and impressive quality of contemporary research, it may not be possible to overstate the impact of psychological approaches on our understanding of depressive disorders. Accordingly, the aim of this conference was, within the limited amount of time available for such an endeavor, to chronicle the current status of the psychology of depression. In inviting participants to this forum, no attempt was made to reflect only certain theoretical views. Contemporary psychological theory and research in depression, however, are dominated by cognitive viewpoints, and the influence of cognitive perspectives is thus unmistakable throughout the present volume.

Even as psychology becomes increasingly splintered and specialized, as evi­ denced by the growing number of special interest divisions of the American Psy­ chological Association, many psychologists are devoting their energies to finding commonalities between traditionally distinct fields and building bridges between them. Developmental psychopathology, for example, has emerged as a synthesis of child development theory and clinical child psychology. Health psychology has resulted from the cooperation and collaboration of many psychologists from a number of fields, including clinical, counseling, social, developmental, and physiological. Within clinical psychology is a growing movement toward'rap­ prochement'that is dedicated to finding common themes among seemingly dis­ parate approaches to psychotherapy. Thus, integration among different fields has increased even as diversity in psychology has flourished. One such integration or interfacing effort that is related in several ways to the integrative efforts just noted involves social, clinical, and counseling psychology. Although this effort is not a new one (see chapter 1), it was given a new lease on life by the publication of the first issue ofthe Journal of Social and Clinical Psy­ chology in 1983. Since that time, several volumes and numerous journal article and book chapters have been devoted to the general notion that social psychologi­ cal theory and research has much to offer clinical and counseling psychology, such as greater understanding of psychological and everyday problems in living and insight into clinical and counseling activities such as psychotherapy.

Although senior undergraduate psychology students and first year master's- and doctoral-level students frequently take courses in advanced abnormal psychology, it has been almost two decades since a book by this title has appeared. Professors teaching this course have had a wide variety of texts to select from that touch on various aspects of psychopathology, but none has been as comprehensive for the student as the present volume. Not only are basic concepts and models included, but there are specific sections dealing with childhood and adolescent disorders, adult and geriatric disorders, child treatment, and adult treatment. We believe the professor and advanced student alike will benefit from having all the requisite material under one cover. Our book contains 26 chapters presented in five parts, each part preceded by an editors'introduction. The chapters reflect updates in the classification of disorders (i. e., DSM-IV). In Part I (Basic Concepts and Models), the chapters include diagnosis and classification, assessment strategies, research methods, the psychoanalytic model, the behavioral model, and the biological model.
