The toughness model proposed in this book incorporates psychological research and neuroscience to explain how a variety of toughening activities - ranging from confronting mental and physical challenges to meditation - sustain our brains and bodies, and ultimately build our mental and psychological capacities degenerated by stress and by aging.

From time to time, professional journals and edited volumes devote some of their pages to considerations of pain and aging as they occur among the aged in different cultures and populations. One starts from several reasonable assumptions, among them that aging per se is not a disease process, yet the risk and frequency of disease processes increase with ongoing years. The physical body's functioning and ability to restore all forms of damage and insult slow down, the immune system becomes compromised, and the slow-growing pathologies reach their critical mass in the later years. The psychological body also becomes weaker, with unfulfilled promises and expectations, and with tragedies that visit individuals and families, and the prospect that whatever worlds remain to be conquered will most certainly not be met with success in the rapidly passing days and years that can only culminate in death. Despair and depression coupled with infirmity and sensory and! or motor inefficiency aggravate both the threshold and the tolerance for discomfort and synergistically collaborate to perpetuate a vicious cycle in which the one may mask the other. Although the clinician is armed with the latest advances in medicine and phar­ macology, significant improvement continues to elude her or him. The geriatric specialist, all too familiar with such realities, usually can offer little else than a hortative to'learn to live with it,'but the powers and effectiveness of learning itself have declined.

This multiauthored introductory textbook could not have been conceived or brought to fruition 10 or 15 years ago. Indeed, at that time relatively little attention was accorded to the psychological needs of older adults. The general tenor of the field then was that older adults would not benefit from psychological intervention. As we now know, this was a faulty assumption that has been discredited with empirical data. Indeed, clinical research data adduced, primar­ ily in the last decade, clearly documents that older adults do benefit from specific psychotherapies that are tailored to their unique presentation of symptoms. Given the explosion of interest in this area (as evinced by increased investi­ gatory activity, national funding, and media attention) and the increased number of masters-and doctoral-level courses devoted to this topic, we felt that the time was right for a textbook in the area. Such flurry of activity also has been fueled by statistics showing how our population by the year 2030 will consist of 30% who are senior citizens. Moreover, since 12% of older adults in the community are estimated to have diagnosable psychiatric disorders and 40% to 50% of older adult medical inpatients have a concomitant psychiatric disorder, the task for clinicians in the next century will be overwhelming.

HEALTH BEHAVIOR AS BASIC RESEARCH Health behavior is not a traditional discipline, but a newly emerging interdisciplinary field. It is still in the process of establishing its identity. Few institutional or organizational structures, i. e., departments and programs, reflect it, and few books and journals are directed at it. The primary objective of this book is thus to identify and establish health behavior as an important area of basic research, worthy of being studied in its own right. As a basic research area, health behavior transcends commitment to a particular behavior, a specific illness or health problem, or a single set of determinants. One way of achieving this objective is to look at health behavior as an outcome of a range of personal and social determinants, rather than as a set of risk factors or as targets for intervention strategies directed at behavioral change. The book is thus organized pri­ marily in terms of the size of the determinants of concern, rather than in terms of specific health behaviors, or specific health problems or conditions. With the first part of the book establishing working defmitions of health behavior and health behavior research as basic frameworks, the second part moves from smaller to larger systems, informing the reader about basic research that demonstrates how health behavior is determined by personal, family, social, institutional, and cultural factors. These distinctions reflect some arbitrar­ iness: the family, organizations, and institutions, for example, are social units.

Although it has been assumed since early recorded history that psycho­ logical factors influence health and illness, it has only been within the past few years that a group of investigators and clinicians with a shared interest in the application of psychological principles and techniques to health and illness has existed. Over this same period of time, a number of multi-author books on the topic of health psychology and an associ­ ated field, behavioral medicine, have been published. Although these books are major resources for the investigator and the clinician in the field, it is often difficult for students, both undergraduate and graduate, to learn the basics of health psychology from such books. Thus, Health Psychology: A Psychobiological Perspective was written to provide such basics. The need for such a textbook in health psychology became appar­ ent to the first author when he was searching for reading material for an undergraduate course in health psychology at McGill University. This book grew out of the course in health psychology, and its structure represents the course content. The purpose of the book is to present the theoretical, empirical, and clinical aspects of the rapidly developing field of health psychology. Data from a number of subdisciplines within psychology and the behav­ ioral and health-related sciences are integrated throughout each chapter in an effort to provide a balanced perspective. Health Psychology explores the development of the field and its research methodologies, theoretical models, and intervention possibilities.

During the past several decades, the field of mental health care has expanded greatly. This expansion has been based on greater recognition of the prevalence and treatability of mental disorders, as well as the availability of a variety of forms of effective treatment. Indeed, throughout this period, our field has witnessed the introduction and the wide­ spread application of specific pharmacological treatments, as well as the development, refinement, and more broadly based availability of behavioral, psychodynamic, and marital and family interventions. The community mental health center system has come into being, and increasing numbers of mental health practitioners from the fields of psychiatry, psychology, social work, nursing, and related professional disciplines have entered clinical practice. In concert with these developments, powerful sociopolitical and socioeconomic forces-including the deinstitutionalization movement of the late 1960s and early 1970s and the cost-containment responses of the 1980s, necessitated by the spiraling cost of health care-have shaped the greatest area of growth in the direction of outpatient services. This is particularly true of the initial assessment and treatment of nonpsychotic mental disorders, which now can often be managed in ambulatory-care settings. Thus, we decided that a handbook focusing on the outpatient treatment of mental disorders would be both timely and useful. When we first began outlining the contents of this book, the third edition of the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disor­ ders (DSM-III) was in its fourth year of use.

Volume 4 considers the importance of health behavior research in practical settings. Particularly notable are treatments of the'narrative approach', the taxonomy of health behavior, and the organization of health behavior knowledge. Each volume features extensive supplementary and integrative material prepared by the editor, the detailed index to the entire four-volume set, and a glossary of health behavior terminology.

Perhaps the most difficult milestone in a young clinician's career is the completion of the first interview. For the typical trainee, the endeavor is fraught with apprehen­ sion and with some degree of dread. If the interview goes weIl, there is consider­ able rejoicing; if it goes badly, much consternation results. Irrespective of the amount of preparation that has taken place before the interview, the neophyte will justifiably remain nervous about this endeavor. Thus, the first edition of Diagnostic Interviewing was devoted to providing a clear outline for the student in tackling a large variety of patients in the interview setting. In consideration of the positive response to the first edition of Diagnostic Interviewing, published in 1985, we and our editor at Plenum Press, Eliot Werner, decided that it was time to update the material. However, the basic premise that a book of this nature needs to encompass theoretical rationale, clinical description, and the pragmatics of'how to'once again has been followed. And, as in the case of the first edition, this second edition does not represent the cat's being skinned in yet another way. Quite to the contrary, we still believe that our students truly need to read the material covered herein with considerable care, and once again the book is dedicated to them. We are particularly concerned that in the clinieal education of our graduate students, interviewing has been given short shrift.

An old woman walks slowly up the hill from the store to her house. The hill is quite steep and the packages she carries, heavy. The two ten-year-olds watching her feel sorry for her and, moving toward her, ask if they might help carry the packages. They easily lift them and with almost no effort bring the shopping bags to the top of the hill. After receiving all A's in his first term in college, F. finds that this term is much harder, especially his physics courses, in which he is failing. He has talked to his professor twice, but finds he cannot understand what she is teaching.'Somehow,'he thinks,'if she could only present the material in a different way, I could understand it better!'A month ago, as B. lay playing quietly in his crib, a toy key slipped out of his hand onto the floor. Almost immediately he turned his attention to another toy, close by, which he took up and put into his mouth. Yesterday, very nearly the same thing happened, except this time as soon as the toy key fell, he began to cry loudly, forcing me to stop what I was doing and retrieve it for him. It seemed in the first case that he forgot it, while yester­ day, even though it was gone, out of his sight, he still remembered it and wished it back.

Imagery--the miraculous quality that human beings use to re-evoke and reorganize perceptions--is no longer considered idio­ syncratic. It is an absolutely integral part of human development and motivation which gives substance to subjective meaning and realistic aostract thought. A necessary ingredient of the trans­ mission and development of human life, imagery must be understood and carefully studied to enhance our knowledge and our lives. The imaginations people have of one another and the imagina­ tion one has of oneself are composed of the stuff that we call imagery. To my way of thinking, there is waking imagery (consist­ ing of our stream of images while we are awake) and dream, or sleep imagery (consisting of all that goes on in our minds while asleep). Daydreaming, reverie, fantasy, hallucinations and unbidden images are forms of waking imagery. Dreams, nightmares, hypnogogic and hypnopompic images are all part of sleep imagery. To be aware of and to study the manifestations and complexity of waking imagery--which appears to function in an effortless, instantaneous and ubiquitous manner--is now considered a fit sub­ ject for study after a half century of denial. The interest in and study of imagery has been far more empha­ sized in Europe than in America. In Sweden, for example, all clinical training for psychologists includes major emphasis on the works of Hanscarl Leuner and my own work in imagery.

In reviewing the Contents of this Handbook edited by Freeman, Simon, Beutler, and Arkowitz, I am both impressed and gratified with the enormous strides made by cognitive­ behavior therapy since the late 1960s. A perusal of the Contents reveals that it is used with adults, children, couples, and families; it is clinically appropriate for such problems as anxiety, depression, sexual dysfunctions, and addictions; and it is employed in conjunction with psy­ chopharmacological and other psychotherapeutic interventions. It was in the mid-1960s when Breger and McGaugh published an article in the Psychological Bulletin, taking behavior therapists to task for using only classical and operant principles in devising their therapeutic interventions. Breger and McGaugh argued that the field of learning was undergoing a major revolution, paying considerably more attention to cognitive processes than had previously been the case. In short, they criticized the growing behavioral orientation for being limited in its exclusively peripheralistic orientation. At the time, behavior therapists were initially somewhat resistant to any allusion to cognitive metaphors. Indeed, my own initial reactions to the Breger and McGaugh article was quite negative. Yet, in rereading their critique, many of their suggestions now seem most appealing. No doubt, I and my behavior colleagues lacked the appropriate'cognitive set'for incorporating such contradictory information. Nonetheless, the clinical evidence for the rele­ vance of cognitive factors in the behavior change process was simply too compelling to ignore.

Social development over one's lifetime is a complex area that has received consider­ able attention in the psychological, social-psychological, and sociological literature over the years. Surprisingl~ however, since 1969, when Rand McNally published Goslin's Handbook of Socialization, no comprehensive statement of the field has appeared in book form. Given the impressive data in this area that have been adduced over the last two decades, we trust that our handbook will serve to fill that gap. In this volume we have followed a lifespan perspective, starting with the social interactions that transpire in the earliest development stages and progressing through childhood, adolescence, adulthood, and, finall~ one's senior years. In so doing we cover a variety of issues in depth. The book contains 21 chapters and is divided into five parts: I, Theoretical Perspectives; II, Infants and Toddlers; ill, Children and Adolescents; Iv, Adults; and V, The Elderly. Each of the parts begins with introductory material that reviews the overall issues to be considered. Many individuals have contributed to the final production of this handbook. Foremost are our eminent contributors, who graciously agreed to share with us their expertise. We also thank our administrative and technical staff for their assistance in carrying out the day-to-day tasks necessary to complete such a project. Finall~ we thank Eliot Werner, Executive Editor at Plenum, for his willingness to publish and for his tolerance for the delays inevitable in the development of a large handbook.

A number of factors converged to prompt this volume at this particular time. For several years, supervisors in predoctoral and internship programs have noted the need for a compendium of selected articles to illustrate the range of research and practice of pediatric psychology. Although the field is still relatively young, the many new pediatric psychologists in recent years might benefit from a perspective on the history and development of the psychological concepts, the organizational home in the Society of Pediatric Psychology (Section V of the Division of Clinical Psychology of the Ameri­ can Psychological Association), and its publication archive, Journalof Pe­ diatric Psychology. Such'classics'help capture the richness and excitement that portrays the field. Noting the continuing need for resources to serve the specialty, the Ex­ ecutive Committee of the Society of Pediatric Psychologyauthorized the spon­ sorship of publication of the HandbookofPediatric Psychology (Routh, 1988) as a synthesis of the field by expert chapter authors. The Society then ap­ pointed a committee led by C. Eugene Walker and Annette M. La Greca to develop a series of biennial volumes entitled Advancesin Pediatric Psychology. A number of discussions among the editorial board of the Journal and with Plenum Publishing Corporation, the Journal's publisher, recognized the value of articles carefully selected to exemplify how pediatric psychology is done.

Although we speak of'the elderly'as if there were one body of people with common characteristics, older adults are more heterogeneous than any other popu­ lation. People over the age of 65 are also the fastest-growing segment of the population in the United States, currently numbering 25 million. The majority of older adults reside in their communities; a small fraction of them are cared for in institutions. Most may expect to experience some kind of physical impairment. Approximately a quarter of the population may expect to suffer amental health impairment. While traditional therapies have not been especially effective for older adults, behavior therapy has shown exceptional promise as a treatment modality. This book presents a comprehensive explication of the relatively new field of behavioral gerontology. It was written for the clinician interested in the interaction of medical, environmental, and psychological variables and their effects on treatment of elderly clients and for the researcher who will be looking to extend knowledge about interventions with this population. It will be useful for the graduate student in clinical psychology, as weIl as the experienced clinician, who will want to include the elderly in his or her therapeutic population.

Contemporary Issues in Behavior Therapy presents innovative approaches to various societal problems worldwide. Contributors explore issues from diverse areas such as behavioral medicine, education, developmental disability, poverty, problematic behavior, and developmental considerations (ie., early family experiences and aging process). The volume stimulates ideas for research, prevention, and treatment, as well as for managing other modern ills including homelessness, crime, and aggression.

It is about their daily coping in the workplace and at home; about economic problems, the loss of friendship and family support, and physical and emotional pain. But it is also about empowering them to deal with their disease, viewing them not as victims but as warriors, vital and active par­ ticipants in their battle against AIDS. This book is also about the social context in which HIV-infected persons and people with AIDS live. It is about how we must learn to deal with sickness in more compassionate and humanitarian ways and what we yet need to learn. It touches on the health care system that confronts those who are ill, on programs of prevention and education, and on the personal implications of broader national and local policies.

Since 1950, when Hans Selye first devoted an entire book to the study of stress,professional and public concern with stress has grown tremendous­ ly. These concerns have contributed to an understanding that has impli­ cations for both prevention and treatment. The present book is designed to combine these data with the clinical concerns of dealing with stressed populations. In order to bridge the gap between research and practice, contributions are included by major researchers who have been con­ cerned with the nature of stress and coping and by clinical researchers who have developed stress management and stress prevention programs. The book is divided into three sections. The goal of the first section is to survey the literature on stress and coping and to consider the implica­ tions for setting up stress prevention and management programs. Follow­ ing some introductory observations by the editors are the observations of three prominent investigators in the field of stress and coping. Irving JaniS, Seymour Epstein, and Howard Leventhal have conducted seminal studies on the topic of coping with stress. For this book they have each gone beyond their previous writings in proposing models and guidelines for stress prevention and management programs. While each author has tackled his task somewhat differently, a set of common suggestions has emerged.
