Die vorliegende Dissertation untersuchte im Rahmen der nationalen, multizentrischen PRO – BRA Studie an der Frauenklinik der Technischen Universität München 206 Patientinnen, die sich nach erfolgter Mastektomie einer Brustkonstruktion mittels des titanisierten Polypropylennetzes TiLoop Bra unterzogen haben. Das primäre Ziel bestand in der Erfassung der Lebensqualität vor und 6 Monate nach erfolgter Operation. Sekundäre Ziele waren die Ermittlung der Komplikationsraten, die Beurteilung des kosmetischen Ergebnisses sowie die Erhebung der ästhetischen Selbsteinschätzung im Vergleich zur Zweitbegutachtung durch unabhängige Fachgutachter. Die Daten bzgl. der Lebensqualität wurden anhand des standardisierten Fragebogens BREAST – Q erhoben. Die Einteilung des Fragebogens basierte auf Aspekten der Patientenzufriedenheit (Zufriedenheit mit der Brust, der Mamma, dem Behandlungsoutcome und dem Pflege- und Wundheilungsprozess) sowie auf Aspekten der gesundheitsassoziierten Lebensqualität (psychosoziales, sexuelles und körperliches Wohlbefinden). Das Durchschnittsalter der behandelten Patientinnen belief sich auf 51,2 Jahre (19-77 Jahre), 78% der Patientinnen waren normalgewichtig (BMI 22 ± 3,1 kg/m²), 22,7% konsumierten regelmäßig Tabak und 2,9% litten an Diabetes mellitus. 67,9% (140 Patientinnen) erhielten unilaterale, 32,1% (66 Patientinnen) eine bilaterale Rekonstruktion. Die am häufigsten durchgeführten Operationstechniken beinhalteten eine „inframammäre“ (68,9%) bzw. eine „t-förmige“ (31,1%) Schnittführung. Insgesamt wurden 133 AE´s (adverse events) protokolliert, 61,6% davon wurden als „ernst“ (SAE, severe adverse event) eingestuft, 27,1% als „moderate“ und 11,3% als „minor“. Die häufigsten Komplikationen waren Hämatome (12,7%), nekrotische Veränderungen (11,2%), Serome (9,7%), Infektionen (7,5%), Wundheilungsstörungen bzw. Wunddehiszenzen (6,7%), Implantationsdislokationen (1,5%) und Kapselfibrosen (1,5%). Hinsichtlich der Lebensqualität wurde im Zuge des 6-Monats-Follow-ups eine Gesamtverschlechterung des durchschnittlichen BREAST-Q-Wertes von 67 (± 16, präoperativ) auf 65 (± 15, sechs Monate postoperativ) festgestellt. Diese Veränderung erwies sich jedoch als statistisch nicht signifikant (df = 59; t = -0,87; p = 0,81). Bezüglich der ästhetischen Beurteilung des Behandlungsergebnisses zeigten sich durchgängig schlechtere Beurteilungen durch die Patientinnen im Vergleich zu den Urteilen, die durch einen unabhängigen begutachtenden Arzt gefällt wurden. Die Unterschiede erwiesen sich mit Bezug auf die Größe und das Aussehen als statistisch signifikant (jeweils p < 0,05).
