The book provides up-to-date information on all aspects of colonic diverticular disease, particularly common in Western countries. Most people suffering from it remain asymptomatic, while about 20% develop symptoms without complications, experiencing the so-called'Symptomatic Non-Complicated Diverticular Disease'(SUDD). 10-25% of this last group eventually develop diverticulitis, with or without complications. The incidence of diverticulitis and diverticular colon disease is increasing worldwide, and is becoming a significant burden for national health systems in terms of direct and indirect costs, also because its prevalence increases with age. Unfortunately, the disease pathogenesis is relatively unknown and several aetiological factors may play a role in its onset. The first parts of the book will deal with the disease epidemiology and etiopathogenesis, focusing on the roles of inflammation and dysbiosis as part of the pathophysiology of diverticular disease. Auf fahrende Züge aufzuspringen, zählt nicht zu den charaktervollsten Formen der Fortbewegung. Und dennoch gibt es Angebote, die man nicht ausschlagen kann, weil sie einfach zu verlockend sind. Die Nachfolge von Felix Harder bei diesem Standardwerk anzutreten, war gleichermaßen ehrenvoll und verpflichtend. Standards, die Felix Harder zusammen mit den beiden anderen Herausgebern gesetzt hatte, galt es zu halten, Gutes sollte bewahrt, Bewährtes gegebenenfalls verbessert werden. Ergänzungen waren gewünscht (so die komplette Hernienchirurgie), aber der Umfang des Bandes durfte nicht - nehmen, wohl aber die Qualität eines ohnehin schon sehr guten Werkes. Diese Aufgabe war verlockend, aber eigentlich kaum lösbar, und nun liegt der fertige Band vor uns. Dass dies möglich war, verdanke ich vor allem den bisherigen Autoren, die ohne Murren ihre Beiträge großzügig auf das Essenzielle gekürzt und auf den aktuellen Kenntnisstand ausgerichtet haben. Aber auch allen neuen Autoren ist zu danken, dass sie sich in dieses Konzept eingebunden und uns mit ihren Ausführungen bereichert haben. Insbesondere danke ich meinen Mitarbeitern und hier vor allem in erster Linie Herrn Kollegen Priv. -Doz. Dr. Marc Jansen für die straffe Hand bei der Realisation in de- bar kurzer Zeit. Ohne das jetzige und frühere Aachener Team wäre diese Aufgabe nicht zu lösen ge- sen, hierfür danke ich allen Beteiligten.
