This book presents the latest research pertaining to the diagnosis, therapy and management of diffuse low-grade gliomas (DLGG) in adults, with a particular focus on the path towards individualised therapy for this kind of tumour. Recent research on the natural history of DLGGs and their interaction with the brain has led to new diagnostic and therapeutic strategies which increase survival and quality of life of the patient, and these methods are described in this book.

In June 1998 the Fourth International Workshop on Digital Mammography was held in Nijmegen, The Netherlands, where it was hosted by the department of Radiology of the University Hospital Nijmegen. This series of meetings was initiated at the 1993 SPIE Biomedical Image Processing Conference in San Jose, USA, where a number of sessions were entirely devoted to mammographic image analysis. At very successful subsequent workshops held in York, UK (1994) and Chicago, USA (1996), the scope of the conference was broadened, establishing a platform for presentation and discussion of new developments in digital mammog­ raphy. Topics that are addressed at these meetings are computer-aided diagnosis, image processing, detector development, system design, observer performance and clinical evaluation. The goal is to bring researchers from universities, breast cancer experts, and engineers together, to exchange information and present new scientific developments in this rapidly evolving field. This book contains all the scientific papers and posters presented at the work­ shop in Nijmegen. Contributions came from as many as 20 different countries and 190 participants attended the meeting. At a technical exhibit companies demon­ strated new products and work in progress. Abstracts of all papers were reviewed by members of the scientific committee. Many of the accepted papers had excellent quality, but due to limited space not all of them could be included as full papers in these proceedings. Papers that were rated high by the reviewers are included as long or short papers, others appear as extended abstracts in the last chapter.

More than ten years have elapsed since the publication of the first edition of Tumors and Tumorlike Lesions of Bone and Joints in 1981. During this period significant progress has been made in diagnosis, and particularly in staging and treatment of bone tumors. In most centers, limb-sparing surgery has replaced the more radical ablative surgery, with or without preoperative chemotherapy. This became possible due to the introduction and increased application of newer high-quality imaging techniques (CT and MRI) and in particular the use of new cytomorphologic methods of diagnosis, especially immunohisto­ chemistry. These advances in diagnosis and treatment have been reflected by the publication of new editions of several important text­ books, since their authors - pathologists, orthopedic surgeons, and radiologists - felt the necessity of updating their works in recent years. However, in these publications different criteria of classification were adopted. The number of cases compiled at the Latin American Registry of Bone Pathology increased from 4913 primary bone tumors and more than 1100 tumorlike lesions at the time of publication of the first edition of this book, to 5274 bone tumors and 1665 tumorlike lesions by the end of December 1985, when I finished my work as head of the Registry and occupied the positions of Professor of Orthopedic Pathology at St. Louis University Medical Center and Distinguished Visiting Professor at Rush-Presbyterian-St. Luke's Medical Center in Chicago.

We welcome the publication of this volume, which discusses the diagnosis of bone tumours with particular reference to children and adolescents. As founder members of the International Skeletal Society we are delighted to learn that the book had its inception at one of the Society's meetings. There are several reasons for this. As primary bone tumors are rare in childhood, practitioners in a number of pediatric subspecialties are not familiar with them. The clinical symptoms and signs are often elusive, the biochemical investigations usually normal and the radio­ graphic features often uncharacteristic. Even the pathologist, who is the final step in arriving at the proper diagnosis and who has all the available clinical, biochemical and radiographic data, may encounter difficulties. A good tissue sample is the basis for microscopic investigation. However, bone tumors often show an extreme variety of structures which confuse even experienced bone pathologists. Therefore, histo­ pathologic analysis must take into account all available clinical, biochemical and radiographic data. The close cooperation of the pathologist with clinicians and, especially, radiologists is of the utmost importance.

Ascites is the most frequent and hepatorenal syndrome the most lethal complication in liver cirrhosis. In recent years, major progress has been made regarding effective treatment of these complications, thus reducing mortality in patients. This publication highlights and critically appraises recent achievements and novel advances, and at the same time provides the background needed to grasp novel concepts. Topics treated include complications of paracentesis, the right choice of plasma expanders, and selection of patients who will experience survival benefit from transjugular intrahepatic portosystemic shunt. Hepatorenal syndrome, on the other hand, is responsible for a broad spectrum of manifestations caused by acute kidney injury, which until recently was considered a lethal condition.
